4. Coarctation of the aorta : management in a non-surgical centre and evaluation of aortic stiffness & cardiac function

Abstract

Background: Adult Congenital Heart Disease (ACHD) is growing rapidly as patients with corrected congenital heart disease are living longer. Coarctation of the aorta is a common congenital heart disease characterised by stenosis of the proximal descending aorta. Despite advances in management, long-term complications remain common. Methods: The first part analyses data from a prospective clinical register of ACHD patients conducted in the only non-surgical specialist centre with a focus on coarctation patients using cross sectional and longitudinal observational methods. Second part consists of a prospective observational study using cardiac magnetic resonance (CMR) imaging to evaluate the associations of pulse wave velocity (PWV, a measure of arterial stiffness), systemic vascular resistance (SVR) and cardiac function in patients with corrected coarctation. Results: The NORPAP database was set up in 1993 and in January 2015, had 2322 ACHD patients (mean age 42 years, 52% female, 62%). Coarctation (n=223, 9.6%), 90% had a corrective procedure and a third required additional procedures during follow-up. Older patients at registration and those diagnosed after age 1 year had a greater likelihood of hypertension, death and recoarctation. Forty-five repaired patients were included in the CMR study: mean age 33 years left ventricular ejection fraction 64%, baseline PWV 5.8m/s and SVR 1345 dynes.cm-5. There were significant univariate associations between PWV and SVR with advancing age, female gender and hypertension, although only age was retained in the multivariate model. Conclusions: Coarctation of the aorta comprised approximately 10% of the ACHD population, with a low risk of long-term adverse outcomes. Patients who were not diagnosed in the 1st year of life had higher risk of hypertension, death and recoarctation. Abnormalities in vascular function were found in CMR in patients with corrected coarctation, and arteriolar resistance may adversely influence myocardial function. These observations could help coarctation patients at risk of future complications.

Abstract

Improved survival rates for patients with a Fontan circulation has allowed more women with this complex cardiac physiology to contemplate pregnancy. However, pregnancy in women with a Fontan circulation is associated with a high risk of adverse maternal and foetal outcomes, high rates of miscarriage, and preterm delivery. Factors associated with a successful pregnancy outcome are younger age, normal body weight, absence of significant functional limitation, no Fontan-related complications, and well-functioning single ventricle physiology. Appropriate care with timely preconception counselling and regular, frequent clinical reviews by a multidisciplinary team based at a tertiary centre improves the chance of a successful pregnancy. Empowerment of patients with education on their specific congenital cardiac condition and its projected trajectory helps them make informed choices regarding their health, reproductive choices, and assists them to achieve their life goals.

Abstract

Background: To date, clinical trials have been underpowered to demonstrate a benefit from ACE inhibitors (ACEis) or angiotensin II receptor blockers (ARBs) in preventing systemic right ventricle (sRV) failure and disease progression in patients with transposition of the great arteries (TGA). This observational study aimed to estimate the effect of ACEi and ARB on heart failure (HF) incidence and mortality in a large population of patients with an sRV.Methods: Data on all patients with an sRV under active follow-up at two tertiary centres between January 2007 and September 2018 were studied. The effect of ACEi and ARB on the incidence of HF and mortality was estimated using a propensity score weighting approach to control confounding.Results: Among the 359 patients with an sRV (32.2 (IQR 26.4-38.3) years, 59.3% male, 66% complete TGA with atrial switch repair and 34% congenitally corrected TGA), 79 (22%) had a moderate to severe sRV dysfunction and 138 (38%) were treated with ACEi or ARB. Fourteen (3.6%) patients died, 8 (2.1%) underwent heart transplantation and 46 (11.8%) had a new HF event over a median follow-up of 7.1 (IQR 4.0-9.4) years. On multivariate Cox analysis with adjustment using propensity score weighting approaches, ACEi or ARBs treatment was not significantly associated with a lower HF incidence or mortality in patients with an sRV.Conclusions: Despite significant neurohormonal activation described in patients with an sRV, there is still no evidence of a beneficial effect of ACEi or ARB on morbidity and mortality in this population. Abstract

INTRODUCTION: Patients with Tetralogy of Fallot (TOF) and pulmonary insufficiency and a dilated right ventricle (RV) may suffer from a reduction in left ventricular (LV) performance. It is not clear whether timely pulmonary valve replacement (PVR) preserves LV mechanics. METHODS: Ten TOF patients who underwent PVR were identified from hospital records, and pre- and postoperative cardiac magnetic resonance images were post-processed with a semi-automatic tissue tracking software. LV circumferential strain, time to peak strain, and torsion were compared before and after PVR. A control group of 10 age-matched normal volunteers was assessed as a comparison. RESULTS: LV circumferential strain did not change before vs. after PVR (basal -18.3 ± 3.7 vs. -20.5 ± 3%, = 0.082; mid-ventricular -18.4 ± 3.6 vs. -19.1 ± 2%, = 0.571; apical -22.7 ± 5.2 vs. -22.1 ± 4%; = 0.703). There was also no difference seen between the baseline strain and normal controls (control basal -18.2 ± 3.3%, = 0.937; mid -18 ± 3.2%, = 0.798; apex -24.1 ± 5%, = 0.552). LV torsion remained unchanged from baseline to post PVR [systolic 2.75 (1.23-9.51) °/cm vs. 2.3 ± 1.2°/cm, = 0.285; maximum 5.5 ± 3.5°/cm vs. 2.34 (1.37-8.07) °/cm, = 0.083]. There was no difference in time to measured peak LV circumferential strain before vs. after PVR (basal 0.44 ± 0.1 vs. 0.43 ± 0.05, = 0.912; mid-ventricular 0.42 ± 0.08 vs. 0.38 ± 0.06, = 0.186; apical 0.40 ± 0.08 vs. 0.40 ± 0.06, = 0.995). At the same time, pulmonary regurgitation and RV end-diastolic and end-systolic volume indices decreased and LV end-diastolic volume increased after PVR. RV and LV ejection fractions remained constant. CONCLUSION: PVR allows for favorable remodeling of both ventricular volumes for TOF patients with significant pulmonary regurgitation. In this cohort, LV myocardial functional parameters such as circumferential strain, time to peak strain, and LV torsion were normal at baseline and remain unchanged after PVR.

Abstract

Non-invasive assessment of haemodynamic data, such as pressure and flow profiles, is helpful in detecting cardiac disease at an early stage. However, current methods lack spatial accuracy and do not take local variations into account. This paper presents a software tool that extracts the arterial geometry and blood inflow profiles from MR images, which are subsequently used to run a 1D haemodynamic simulation model, and displays its output. The workflow is highly automated but allows user-interaction to correct inaccuracies. The tool was evaluated for inter-observer agreement on one healthy volunteer, and results are shown for one patient with an aortic coarctation. The resulting haemodynamic parameters show high agreement between different users and reveal local changes within a coarctation patient.
