Abstract

IntroductionSurgical-related injuries are frequent, in fact the reported percentage of musculoskeletal disorders in surgeons is between 47% and 87%. These conditions are caused by long periods of standing, incorrect postures, repeated movements, little rest between operations, the lack of integrated operator rooms, the correct number and arrangement of monitors and the use of non-ergonomic instruments. This survey aims to assess the Italian overview both highlighting how prevalent surgical-related injury is in our surgeons and whether there is an operating room ergonomics education program in Italian surgical specialty schools.MethodsAn anonymous questionnaire was designed through SurveyMonkey© web application. This survey was composed of 3 different sections concerning the general characteristics of the participants, their surgical background and any training performed, and any injuries or ailments related to the surgical activity. The survey was carried out in the period 1th of December 2022 and the 6th of February 2023.ResultsAt the close of our survey, 300 responses were collected. Among the participants, the two most represented specialties were Gynecology and Obstetrics (42.3%) and General Surgery (39.7%) and surgeons were mainly employed in the Northern regions of Italy (54.8%). Analyzing the participants’ background, 61.7% of the respondents had laparoscopic training during their training and only 53.1% had a pelvic trainer during their residency. In accordance with 98.7% of the respondents, during surgery we have the feeling of being in an uncomfortable position that causes discomfort or muscle pain, and regarding the frequency of these discomforts, the majority of our study population experiences these problems monthly (46.2%), while in 29.6% it is experienced weekly, 12.1% annually and finally 12.1% daily. The surgical approach that is most correlated with these disorders is laparoscopy (62.7%) while the one that causes the least discomfort is robotic surgery (1.4%). These discomforts cause 43.9% of our population to take a break or do short exercises to reduce pain during surgery, and the body areas most affected are the back (61.6%), neck (40.6%) and shoulders (37.8%).ConclusionDespite this, our survey allows us to highlight some now-known gaps present in the surgical training program of our schools and the lack of protection toward our surgeons during their long career.

Abstract

Abstract Background Malignant epithelioid neoplasm with ACTB-GLI1 fusion are considered different from the more common pericytic lesions, such myopericytoma, because they have a spectrum of different genetic abnormalities. They appear to pursue a benign clinical course in young adults, although in sporadic cases lymph node metastasis were described. The categorization of this new type of tumor may also lead to new therapeutic strategies, because they might be sensitive to SHH pathway inhibitors. Case presentation The case involves a 72-years-old multiparous woman who accessed our department after an incidental finding of a right adnexal mass of 43 mm with contrast-enhancement on a control computed tomography scan made for suspected diverticulitis. Our intervention was a detailed ultrasound description of the suspected neoplasm; a diagnostic laparoscopy and the contextual laparotomic removal of abdominal mass; its histological and immunohistochemical analysis. Our main outcome measure is the definition and future recognition of new pathologic entity called malignant epithelioid neoplasm with ACTB-GLI1 fusion. Conclusions We described for the first time the ultrasound characteristic of this type of lesion using standardized terminology and we believe that it may be the first step to improve the acknowledgement of this novel pathologic entity defined as malignant epithelioid neoplasm with GLI-1 fusions.

5. How to measure observational implicit learning of complex sequences: a novel paradigm involving rapid visual presentation and serial reaction time task

Abstract

Observational learning has been studied using the serial reaction time task (SRTT) reporting inconsistent findings on its nature. When present, observational learning appears to be due to explicit learning, even for complex second-order sequences (SOC). In contrast, statistical learning has been studied using the rapid serial visual presentation (RSVP) reporting implicit observational learning of simple sequences. We combined elements of the SRTT and RSVP to investigate whether observational learning of SOC can occur. Two groups were exposed to either a repeated or a random sequence in RSVP. A completion and a recognition tasks were performed as a measure of explicit learning, and an SRTT as a measure of implicit learning. Although results showed no difference between groups in the SRTT, the early learning index predicted the recovery from interference exclusively in the experimental group, which also showed a greater awareness of the repetitiveness of the sequence.

6. The effects of mindfulness meditation on peripersonal space

Abstract

Peripersonal space (PPS) is the multisensory representation of the near-body space. Several factors modulate PPS size and the sharpness of the boundary separating PPS and the far extrapersonal space, suggesting that PPS may be involved in the subjective experience and in the self-other representation. Such representations seem to be shaped by mindfulness meditation (MM); however, evidence on the effects of MM on PPS is limited. To test the hypothesis that MM modulates both PPS size and the sharpness of PPS boundary, we enrolled 26 non-meditators, who performed an audio-tactile task before and after a 15-minute guided focused attention meditation (FAM). Despite no changes of PPS size, after FAM we found a significantly reduced sharpness of PPS boundary, as if it dissolved. We suggest that the reduced separation between the self and the environment, reported by meditators in some phenomenological studies, may relate to the altered PPS sharpness.

Consciousness will be introduced axiomatically, inspired by Buddhist insight meditation and psychology, logic in computer science, and cognitive neuroscience, as consisting of a stream of $configurations$ that is $compound$, $discrete$, and (non-deterministically) $computable$. Within this context the notions of self, concentration, mindfulness, and various forms of suffering can be defined. Abstract

After the Phase-2 high-luminosity upgrade to the Large Hadron Collider (LHC), the collision rate and therefore the background rate will significantly increase, particularly in the high $\eta$ region. To improve both the tracking and triggering of muons, the Compact Muon Solenoid (CMS) Collaboration plans to install triple-layer Gas Electron Multiplier (GEM) detectors in the CMS muon endcaps. Demonstrator GEM detectors were installed in CMS during 2017 to gain operational experience and perform a preliminary investigation of detector performance. We present the results of triple-GEM detector performance studies performed in situ during normal CMS and LHC operations in 2018. The distribution of cluster size and the efficiency to reconstruct high $p_T$ muons in proton--proton collisions are presented as well as the measurement of the environmental background rate to produce hits in the GEM detector., Comment: 10 pages, 5 figures

Abstract

An estimate of environmental background hit rate on triple-GEM chambers is performed using Monte Carlo (MC) simulation and compared to data taken by test chambers installed in the CMS experiment (GE1/1) during Run-2 at the Large Hadron Collider (LHC). The hit rate is measured using data collected with proton-proton collisions at 13 TeV and a luminosity of 1.5$\times10^{34}$ cm$^{-2}$ s$^{-1}$. The simulation framework uses a combination of the FLUKA and Geant4 packages to obtain the hit rate. FLUKA provides the radiation environment around the GE1/1 chambers, which is comprised of the particle flux with momentum direction and energy spectra ranging from $10^{-11}$ to $10^{4}$ MeV for neutrons, $10^{-3}$ to $10^{4}$ MeV for $\gamma$'s, $10^{-2}$ to $10^{4}$ MeV for $e^{\pm}$, and $10^{-1}$ to $10^{4}$ MeV for charged hadrons. Geant4 provides an estimate of detector response (sensitivity) based on an accurate description of detector geometry, material composition and interaction of particles with the various detector layers. The MC simulated hit rate is estimated as a function of the perpendicular distance from the beam line and agrees with data within the assigned uncertainties of 10-14.5%. Abstract

21st Century education is rapidly changing due to the emerging of new media technologies that provide easy access to a great amount of information. Accordingly, as today's students are immersed in a technological society, they need to be equipped with the skills necessary to interpret, synthesize and produce new information. Thus, today's educators and researchers face the challenge of making students become skillful users of new technologies so to increase their motivation and engagement by also improving their learning outcomes. In particular, in the context of Second Language Acquisition (SLA), using technology appropriately could be a valuable approach to create a meaningful learning context where the acquisition of language and literacy skills is accompanied by that of digital skills. To that end, Digital Storytelling (DST) seems to perfectly embody and meet the various educational needs of today's students because due to its dynamic feature of combining traditional storytelling with a variety of digital multimedia it provides opportunities for the improvement of learning skills including cooperative learning, motivation, and engagement. In the light of CALL (Computer-Assisted Language Learning), Collaborative Learning (CL), and the Constructivist Paradigm (CP), this paper aims at addressing the potential of Interactive Digital Storytelling in enhancing ESL (English as a Second Language) students' academic engagement and 21st Century Skills. Therefore, this paper describes the implementation of DST at "L'Orientale" University of Naples with 24 ESL students involving qualitative and quantitative methods of collecting data. Results showed that DST enhanced students' engagement with their studies and the reinforcement of their digital skills. [For the complete proceedings, see ED608557.]

Abstract

We present analytical calculations, Finite Element Analysis modeling, and physical measurements of the interstrip capacitances for different potential strip geometries and dimensions of the readout boards for the GE2/1 triple-Gas Electron Multiplier detector in the CMS muon system upgrade. The main goal of the study is to find configurations that minimize the interstrip capacitances and consequently maximize the signal-to-noise ratio for the detector. We find agreement at the 1.5--4.8% level between the two methods of calculations and on the average at the 17% level between calculations and measurements. A configuration with halved strip lengths and doubled strip widths results in a measured 27--29% reduction over the original configuration while leaving the total number of strips unchanged. We have now adopted this design modification for all eight module types of the GE2/1 detector and will produce the final detector with this new strip design., Comment: 22 pages, 14 figures

218 p., During the Spanish Civil War and the posterior dictatorship, a huge number of people disappeared during the war or in episodes of repression. A large number of victims remain unidentified. One of the main limitations of these analyses is the difficulty of obtaining informative STRs profiles from the remains, limiting the identification of missing persons. With this aim, this work focuses on multidisciplinary research to investigate the relationship between the factors (physical-chemical and biological) that influence the state of preservation (anthropological and genetic) in order to understand the influence of these factors and increase the number of identifications of victims in the context of the Spanish Civil War. For this purpose, we have collected and analyzed the physicochemical composition of the graves and performed the anthropological analysis. Subsequently, the autosomal and Y-chromosome STRs of the skeletal remains were analysed. Finally, we developed a predictive statistical model to determine the most influential variable in obtaining an informative profile of autosomal STRs (>12STRs).With this, it has been possible to demonstrate that, in addition to DNA concentration and inhibition, also organic matter influence in successfully obtaining an informative autosomal STRs profile from skeletal remains buried in mass graves of Spanish Civil War.

41. Cariprazine augmentation in patients with treatment resistant unipolar depression who failed to respond to previous atypical antipsychotic add-on. A case-series

Abstract

Among individuals receiving an adequate pharmacological treatment for Major Depressive Disorder (MDD), only 30% reach a full symptom recovery; the remaining 70% will experience either a pharmacological response without remission or no response at all thus configuring treatment resistant depression (TRD). After an inadequate response to an antidepressant, possible next step options include optimizing the dose of the current antidepressant, switching to a different antidepressant, combining antidepressants, or augmenting with a non-antidepressant medication. Augmentation strategies with the most evidence-based support include atypical antipsychotics (AAs). Few data are available in literature about switching to another antipsychotic when a first augmentation trial has failed. We present a case-series of patients with unipolar treatment resistant depression who were treated with a combination of antidepressant and low dose of cariprazine after failing to respond to a first augmentation with another AA. We report data about ten patients affected by unipolar depression, visited at the outpatients unit of Mental Health Department of ASL CN2 of Bra and NA1 of Napoli (Italy). All patients failed to respond to conventional antidepressant therapy. A low dose of AA (aripiprazole, risperidone or brexpiprazole) was added for one month to the ongoing antidepressant treatment without clinical improvement. A second augmentation trial was then made with cariprazine. Seven out of ten patients were responders at the end of period, of them 1 patient reached responder status by week 2. HAM-D mean scores decreased from 23.9 ± 3.9 (baseline) to 14.8 ± 5.3 (4 weeks). Cariprazine was well tolerated, no severe side effect was observed during the trial. Our sample of treatment resistant unipolar patients showed good response to augmentation with cariprazine. Failure to a first AA-augmentation trial does not preclude response to a second one. This preliminary result requires confirmation through more rigorous studies conducted over greater samples.
