Abstract

Background: The benefit of pharmacogenetic testing before starting drug therapy has been well documented for several single gene–drug combinations. However, the clinical utility of a pre-emptive genotyping strategy using a pharmacogenetic panel has not been rigorously assessed. Methods: We conducted an open-label, multicentre, controlled, cluster-randomised, crossover implementation study of a 12-gene pharmacogenetic panel in 18 hospitals, nine community health centres, and 28 community pharmacies in seven European countries (Austria, Greece, Italy, the Netherlands, Slovenia, Spain, and the UK). Patients aged 18 years or older receiving a first prescription for a drug clinically recommended in the guidelines of the Dutch Pharmacogenetics Working Group (ie, the index drug) as part of routine care were eligible for inclusion. Exclusion criteria included previous genetic testing for a gene relevant to the index drug, a planned duration of treatment of less than 7 consecutive days, and severe renal or liver insufficiency. All patients gave written informed consent before taking part in the study. Participants were genotyped for 50 germline variants in 12 genes, and those with an actionable variant (ie, a drug–gene interaction test result for which the Dutch Pharmacogenetics Working Group [DPWG] recommended a change to standard-of-care drug treatment) were treated according to DPWG recommendations. Patients in the control group received standard treatment. To prepare clinicians for pre-emptive pharmacogenetic testing, local teams were educated during a site-initiation visit and online educational material was made available. The primary outcome was the occurrence of clinically relevant adverse drug reactions within the 12-week follow-up period. Analyses were irrespective of patient adherence to the DPWG guidelines. Abstract

The aim of the present study was to investigate the distribution of CYP2C8 variants *3 and *5, as well as their effect on carbamazepine pharmacokinetic properties, in 40 epileptic pediatric patients on carbamazepine treatment. Genotyping was conducted using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP), and allele-specific (AS)-PCR methods, and steady-state carbamazepine plasma concentrations were determined by high performance liquid chromatography (HPLC). The CYP2C8 *3 and *5 polymorphisms were found at frequencies of 17.5 and 0.0%, respectively. After dose adjustment, there was a difference in daily dose in CYP2C8*3 carriers compared to non carriers [mean ± standard deviation (SD): 14.19 ± 5.39 vs. 15.46 ± 4.35 mg/kg; p = 0.5]. Dose-normalized serum concentration of carbamazepine was higher in CYP2C8*3 (mean ± SD: 0.54 ± 0.18 vs. 0.43 ± 0.11 mg/mL, p = 0.04), and the observed correlation between weight-adjusted carbamazepine dose and carbamazepine concentration after dose adjustment was significant only in CYP2C8*3 non carriers (r = 0.52, p = 0.002). However, the population pharmacokinetic analysis failed to demonstrate any significant effect of CYP2C8 *3 polymorphism on carbamazepine clearance [CL L/h = 0.215 + 0.0696*SEX+ 0.000183*DD]. The results indicated that the CYP2C8*3 polymorphism might not be of clinical importance for epilepsy treatment in pediatric populations.

This article discusses the use of prenatal exome sequencing in diagnosing and understanding the genetic origins of recurrent isolated fetal anomalies. The authors present three cases where exome sequencing identified gene mutations associated with specific anomalies detected through ultrasound. The first case involved microcephaly, the second case involved hydrocephalus, and the third case involved omphalocele. Abstract

The local intrinsic dimensionality (LID) model enables assessment of the complexity of the local neighbourhood around a specific query object of interest. In this paper, we study variations in the LID of a query, with respect to different subspaces and local neighbourhoods. We illustrate the surprising phenomenon of how the LID of a query can substantially decrease as further features are included in a dataset. We identify the role of two key feature properties in influencing the LID for feature combinations: correlation and dominance. Our investigation provides new insights into the impact of different feature combinations on local regions of the data.

Abstract

Planning based on reliable information about the Earth's surface is an important approach to minimize economic expenses conditioned by natural factors. Data collected by Earth remote sensing (ERS), as well as the analysis of such data using automated classification methods, are becoming more and more important for research and practice activities related to assessing the spatio-temporal structure and sustainability of the Earth's surface. The analysis of the authenticity of the surrounding areas enables a more objective classification of land plots on the basis of spatial patterns. Combined use of various environmental descriptors enables high-quality handling of neighborhood properties, as each descriptor provides its own specific information about a geospatial system. Experiments have shown that the diagnostics of the emergent properties of such internal structure by analyzing the diversity of dynamic characteristics allows reducing exposure to noise, obtaining a generalized result, and improving the classification accuracy.

Abstract

The solar energy is widely used around the world for electricity generation and heating systems in municipal services. But its use is complicated in the number of territories with uneven receipts of solar radiation on the earth’s surface and large number of cloudy days during a year. A hypothesis on the possibility of application of individual solar collectors for heating of houses in the number of cities of Russia has been tested. The existing designs of solar collectors and checking the possibility of their application in northern territories of Russia are investigated. The analysis was carried out taking into account features of relief and other climatic conditions of the Perm and Sverdlovsk regions. As the result of research, the basic recommended conditions for application of solar batteries in houses of the northern Russian cities have been resumed. © 2017 IJCRSEE. All Right Reserved.

Abstract

In this paper we analyze the impact of the quality of human capital on the main economic indicators of South-Eastern Europe countries [SEE] at the NUTS 2 level. The subjects of this research are the human capital indicators of regional competitiveness. The quality of human capital depends largely on the age structure of the population and the quality of education. Those regions, which have the highest percentage of the working-age population and highly educated people, are able to achieve higher productivity and gain a competitive advantage over other regions. As main indicators of the quality of human capital we identified: population; persons aged 25-64 with tertiary education attainment; students in tertiary education and participation of adults aged 25-64 in education and training and human resources in science and technology. As main economic indicators, we identified: regional gross domestic product; employment and income of households. The aim of this paper is to determine whether there is a correlation between the indicators of the quality of human capital and economic indicators. As a main methodology we have used the correlation coefficient which shows interdependence of the analyzed indicators. As part of our analysis, we consider only EU member states that belong to the SEE countries: Slovenia, Croatia, Romania, Bulgaria and Greece. We conclude that in all countries there is a high multiple correlation coefficient between the indicators human resources in science and technology, number of students and employment. Abstract

The chromatic number of a triangle-free graph can be arbitrarily large. In this article, we show that if all subdivisions of K2, 3 are also excluded as induced subgraphs, then the chromatic number becomes bounded by 3. We give a structural characterization of this class of graphs, from which we derive an coloring algorithm, where n denotes the number of vertices and m the number of edges of the input graph.

Abstract

A simple ball milling/thermal treatment procedure was applied to obtain fine thermistor powders. Three different powder compositions were analyzed-Cu0.2Ni0.5Zn1.0Mn1.3O4, Cu0.25Ni0.5Zn1.0Mn1.25O4 and Cu0.4Ni0.5Mn2.1O4. XRD analysis showed that all three powder compositions had a cubic spinel structure. Correlation between the sintering temperature, structure and resulting electrical properties was analyzed on bulk samples. Thick film pastes were composed and segmented thick film thermistors were screen printed on alumina, dried and fired. SEM analysis revealed a typical dendrite structure with small grains and a developed surface area. Thick film sheet resistance was measured on a test matrix and the resistance decreased with increasing Cu content. The temperature dependence of sample resistance was measured in a climatic chamber enabling calculation of the material constant and activation energy. Aging of the obtained segmented thermistors was analyzed and the resistivity drift was 0.23% for the Cu0.2Ni0.5Zn1.0Mn1.3O4 NTC thick film thermistor confirming greater stability of thermistors containing Zn and Cu that in combination with the determined good thermistor characteristics make them good candidates for temperature and heat loss sensor applications.
