Abstract

Science is not tool only, nowdays, but the creator of the world, and the world becomes object of its research and creative work. However, such a position of the science is not easy to afford in concrete social reality. It presumes developed democracy as a social form which affords the highest degree of freedom and equality. The task of the science and theory is to help society to find rational solutions of the concrete social goals, on the causality to social reality basis, without using any force. The freedom of scientific creativity, however, initiates the scientist's obligation of using his knowledge and achievements in accordance to interests of the society and the whole mankind. For such a position of scientific achievements some material and other presumtions are necessary, being the obligation of the whole society, as a way to avoid poverty in the global, technology developed reality, although there is no equality between rich society and developed democracy, necessarily. The developed democracy presumes developed democratic institutions and, among them, as one of the most important, like fundamental rights and freedoms are, legal state institution. In XIX century in german legal theory, the leading ones were theories of state sovereignty as a way to gain stability of legal state in its task to afford the realisation of the most important social values. Such theories had various degradations in history, as a result of their tries to change national sovereinty theory to state sovereignty theory as a leading one, for the reason of national sovereignty instability, as to their opinion. Due to possibility of degradation, theories of state sovereignty face the necessity of their revision, by putting them in the context of humanity and mankind principals, in order to avoid negative consequences in history, like nazyism surely was. In European community the leading concept of governing, which includes all institutions and organs, state ones and civil, as well, in decision making process, refering to the interests of all, is two level governance and multilevel governance.

Abstract

Introduction: Medical and legal problems of organ transplantation are numerous. The donor must be fully and properly informed about all elements regarding the transplantation, and especially about the possible consequences resulting from the removal of tissues and organs, which is an essential ethical problem. Aim: The aim of this research was to determine the attitude of health professionals and the general population in Montenegro towards some of the legal and ethical aspects of organ donation. Material and methods: The research was conducted with the consent of the Ethics Committee of the Clinical Center of Montenegro, in Podgorica number 03 / 01-1504. The sample was formed by random selection and consisted of 400 adult citizens of the city of Podgorica. The research included 200 health workers employed at the Clinical Center of Montenegro in Podgorica and 200 adult citizens of the city of Podgorica. Results: The majority of health workers (84.6%) state that the donor should be the one who gives consent for organ donation for life. Similarly, the vast majority (76%) of the general population believe the same. The largest percentage of participants, more than a third, in both groups, believe that using organs for the wrong purposes is sometimes possible, while almost a quarter of respondents said they did not believe it could happen. The difference in the opinion of health workers and the general population on this issue was not statistically significant (p = 0.522). Conclusion: From our research it can be concluded that both groups of respondents generally believe that the guarantee that organs will be used for the right purposes is the most important factor in organ donation and that the donor is the one who will give consent for living organ donation. The proposed measures need to create a strategy to increase confidence that organ donation will be done only for the right purposes, both in the general population and among health professionals.

Abstract

Background/Aim. Depression is the most prevalent mental disorder which affects approximately 7% of the world’s older population. This study aimed at examining the prevalence of depression among older adults and its relations to chronic illnesses. Methods. Study was con-ducted within the National Health Survey of the Serbian population in 2013. The questionnaires used as instruments in this study were created in accordance with the questionnaires of the European Health Interview Survey – Second Wave. The Patient Health Questionnaire-8 (PHQ-8) was used to evaluate the presence of depressive symptoms. The relations between depression symptoms (a de-pendent variable) and a set of independent variables were examined with univariate and multivariate logistic regression analyses. Results. The study showed that there was a 10.0% prevalence of depression within this population with statistically significant differences between the genders – 12.6% of women and 6.5% of men. The multivariate analysis revealed that multimorbidity [odds ratio (OR) = 1.89], chronic pain (OR = 2.35) and self-evaluations of poor health (OR = 8.37) were strongly associated to depression. In terms of individual chronic illnesses, the study showed that strokes double the odds of developing depression (OR = 1.82) while the deformities of lower spine increased this odds by 27%. Conclusion. Depression is very frequent in older persons who suffer from chronic diseases and medical conditions. It is crucial to enable ad-equate screening in primary healthcare institutions in order to diagnose depression in its early stages and start its treatment as soon as possible.

Abstract

Knowledge is the value connected with each social development and development of economy, as well. It is generally recognised value and, therefore, it is in reference to ius naturale, native rights, generally recognised also. Such values are, nowdays, the part of concept of fundamental freedoms and rights and their protection. However, their protection must be accompanied by the consciousness of wellbeing of everybody. The values like universalisation, globalisation, education, help restauration of society and social and economic progress. In such a progress, there must be place for free economy, market, economic growth, and such a development belongs to people. Without knowledge and education these values could turn to their opposition, under the exuse of achievement of "welfare state". It is why they are protected by the highest domestic and international regulations.

5. The prevalence and factors associated with cervical cancer screening among women in the general population: Evidence from National Health Survey

Abstract

Introduction/Objective. Serbia has been burdened with one of the highest cervical cancer incidence and mortality rates in Europe. The objective of the study was to estimate the prevalence and factors associated with compliance to cervical cancer screening among women in the general population. Methods. The study used the data from 2013 National Health Survey of the population of Serbia. Logistic regression analysis was further used to examine demographic and socio-economic factors which affect the disparities in cervical cancer screening practices among the female population. Results. Every third woman (35.4%) has never done a Pap test in her lifetime. The highest percentage of respondents did their Pap tests after they were recommended by doctors (52.3%); 45% of women did it on their own initiative, and only 2.7% did it after they had been summoned to participate in an organized screening by their doctor. The multivariate logistic regression analysis revealed that the most important factors in women who had never undergone Pap tests were the following: age (being within the youngest or the oldest age group), rural residence and low level of education, poor socio-economic status, and marital status (have never married). Conclusion. Further strategies and interventions for improving cervical cancer incidence and mortality rates should be focused on socially and economically endangered population groups in order to reduce disparities in cervical cancer screening more effectively.

6. Legal significance of the preamble: With special reference to the constitutional and legal significance

Abstract

Preamble is a contemporary way for some elements of different written acts to be defined. Therefore, preamble is commonly introductory clause or clauses, usually located at the begining of the act which it is prefixed. Preamble may be very often a part of some legal acts. It could also be found in other written acts which are not necessarily legal by its nature. Therefore, it is wildly spread. Central problem in relation to preamble is the question of its legal binding and the scope of it. The specific is the significance of the preamble in constitutional law: it may consists of declaration on motive for delivery of constitution, declaration on politically crucial moments in history and it could define objectives of new constitutional system, or the solution of some urgent political question. In the last two cases legal obligation of preamble is beyond any despute. In theory, other approaches could also be found concerning the question of legal binding of preamble clauses.

7. Free citizens, democracy: Assumptions of the application of Roman law

Abstract

Majority of the nowdays societies consider that spreding of democracy, rather than limitation of state power, preserves freedom. This idea is in the contrary to antic idea of freedom as the essence of human being and its highest value. By such an idea, the society will be preserved from negative tendencies like democratic despotism (glorifiing power and authority as well as concetration of power in the hands of the leader), and decline of the highest values in global society. In antic polis ius civile used to be applied to free citizens only, rather than strangers, being slaves. As slaves, strangers had no any right recognised, and were acquisited like any other property. Slavery in Roman empire was considered as normal phenomena and was also approved mostly by social and biologic reasons. Ius gentium was, however, by spreading Empire to provncies, apllied to both citizens and strangers. Antic polis produced direct democracy, being ruling of the people, which was duty as well as the priviledge of the free citizens, thus, the priority were public obligations. Nowdays, however, democracy is considered individually, as a protection of individual, its property and private life.

The definition of human and minority rights is grounded on ideas of ius naturale. These ideas are developed in later progress of human society and legal theory and are adjusted to new social conditions. Thus, modern ius naturale parcially differs from original, primarly developed idea od ius nature (from the era od natural law in works of Platon, Aristotel, sophists. In materia of human and minority rights, ideas of ius naturale developed by sophists are in the function of explanation the universality and general acceptance of legal protection of human and minority rights and day to day development.

10. The efficacy of moxifloxacin in patients with bacterial keratitis

Abstract

Background/Aim. Keratitis is a sociomedical problem of moderately developed countries. including Serbia, too. The incidence of bacterial keratitis in the world is about 20% in relation to all keratitis, but its frequency in Serbia is still not known. Bearing in mind the complications in the front segment of the eye after bacterial keratitis (ulcer, neovascularization, fibrosis) and decline in visual acuity, it was necessary to assess the efficacy of local therapy by moxifloxacin which will shorten the healing time and reduce complications. The aim of the study was to analyze the efficiency of shortening the treatment of bacterial keratitis with moxifloxacin. Methods. The study was designed as prospective, randomized, double-blind study. The study included 30 patients with diagnosed keratitis and locally applied antibiotic moxifloxacin, and 60 patients in a control group, with locally applied artificial tears. All participants were subjected to complete clinical ophthalmologic analysis (2015/16), for a period of 1–15 days after the application of therapies (healing time of corneal pathology). The following was determined in all patients: degree of hyperemia, degree of epithelial defect, level of corneal sensitivity, level of tear secretion and tear quality, degree of conjunctival secretion, degree of neovascularisation and corneal scarring, degree of visual acuity, score of subjective symptomatology and correlation of ophthalmological findings and subjective symptoms. Results. There was a statistically significant difference in times of therapeutic efficacy/clinical response between the study group patients who received moxifloxacin and the control group patients who received arteficial tears. Conclusion. Local application of moxifloxacin had therapeutic effect (total benefit) both in terms of the effective shortening of the healing time and also the reduction of the complications of bacterial keratitis, without unwanted effects.

11. Socioeconomic inequalities and non-communicable diseases in Serbia: National health survey

Abstract

Background/Aim. Non-communicable diseases (NCDs) are a major public health challenge worldwide. Although they are preventable, NCDs are the major global causes of morbidity and mortality, absenteeism, disability and premature death. The aim of this study was to examine socioeconomic inequalities in the prevalence of non-communicable diseases in Serbia. Methods. Data from the 2013 National Health Survey of the population of Serbia was used in this study. There were 13,765 adults interviewed, aged ≥ 20 years. We used multivariate logistic regression analyses with demographic and socioeconomic determinants of health as independent variables and prevalence of non-communicable diseases as a dependent variable. The minimum level of significance was p < 0.05. Results. Hypertension was the most prevalent NCDs (36.1%). The prevalence of multimorbidity was 47.1%. Multivariate logistic regression analysis showed that gender, age, place of residence, employment status and education were associated with the presence of NCDs. The odds ratio (OR) for age was 1.074 [95% confidence interval (CI) : 1.070–1.077). Women were at a higher risk of NCDs by 58.9% when compared to men (OR = 1.589; 95% 95% CI : 1.467–1.726). Respondents who lived in the rural areas were at a higher risk for NCDs by 14.1% compared to those who lived in urban areas (OR = 1.141; 95% CI : 1.047– 1.244). Odds ratio for unemployment was 1.227 (95% CI: 1.118–1.346). Respondents with primary education were at a higher risk for chronic diseases by 47.1% (OR = 1.471; 95% CI : 1.281–1.687) while those with secondary school were at a higher risk by 27.7% (OR = 1.277; 95% CI : 1.142–1.428) compared to respondents who had higher education. When it comes to Wealth Index, univariate logistic regression analysis showed that respondents who belonged to the poor and middle classes were at a higher risk for NCDs (OR = 2.031; 95% CI : 1.819–2.267; OR = 1.473; 95% CI : 1.343– 1.615) compared to respondents who belonged to the rich class. Multivariate logistic regression analysis did not show statistically significant correlations between the Wealth index and NCDs. Conclusion. Socioeconomic inequalities in health status are the major challenge and should be a target of national health policy in Serbia, not only because they represent social injustice but also because solving the health problems of underprivileged groups of the population can influence improvement of health status of the population as a whole.

Abstract

Contemporary social process, globalization, free market, and technological development impose a permanent need for further training and education. Technocrats become a generally accepted model, and the engagement of experts and expert teams as a solution could be found not only in private corporations, but also in public nomenclatures. However, professionalism as a criterion, may be connected with certain complexity, especially on the field of responsibility, and requires to be supplemented with other criteria, the subjective ones, primarily loyalty. This subjective critera amortize negative aspects of the engagement of technocrats, in the sense of their bureaucratization, as they often start to follow their own interests, sometimes different, even opposite, then corporation's ones. The settlement of such a problem like the problem of bureaucratisation, being a complex issue, is the requirement of the developed civil societies and a way to achieve the citizens' wellbeing.

14. The influence of sociodemographic factors on the use of antihypertensive drugs among adults in Serbia: Cross-sectional study

Abstract

Background: Despite the knowledge that they have high blood pressure, which can be controlled with medicines, one portion of the patients still ignores doctor's advice. The aim of the research was to examine the impact of socio-demographic factors for treatment of hypertension among adults in Serbia. Subjects and methods: In the study, we used data from the National Health Survey of the Republic of Serbia in 2013. The sample consisted of respondents who had diagnosed hypertension, 5073 of them. The dependent variable in the study was binary (treatment, or no treatment of hypertension) and independent was socio-demographic characteristics of respondents. Logistic regression was used to determine sociodemographic predictors associated with blood pressure control. Results: From a total of 5073 diagnosed hypertensive patients, 91.3% of them cures high blood pressure. Women, older, medium rich patients, those who follow health topics on television and those who do not follow them on the Internet, who, in the past year, have been hospitalized or used the services of daily care significantly more often treat hypertension. The results of binary logistic regression in which the dependent variable is the treatment of high blood pressure, showed that a statistically significantly predictors are: age, gender, the wealth index and monitoring topics related to health through television and the Internet. In the treatment of hypertension, the strongest predictor was the index of well-being. Thus, people who are medium rich are almost three times more likely to treat high blood pressure compared to the richest (OR=2.94, 95%CI=1.54-5.59). A statistically significant difference in treatment is not recorded between the richest and the poorest (p> 0.05). Men are 1.5 times more likely to treat their tension as compared to women (OR=1.54, 95%CI=1.02-2.32). People following itopics related to the health via the internet are1.7 times more likely to treat hypertension in comparison to subjects who do not follow these topics via the Internet (OR=1.72, 95%CI=1.07-2.76). Conclusion: Analysis of the impact of sociodemographic characteristics on the awareness of patients about the importance of treatment of hypertension is an important step in defining the strategies for treatment of patients.

Abstract

Social factors such as social support, integration, and belonging to the community are positive resources against adverse events and living conditions. This study at examining the influence of social support on depression among elderly people in Serbia. The research was done according to the type of cross-sectional study, a mass survey of a random, representative stratified two-stage sample of the population of Serbia in 2013, which was conducted by the Ministry of Health of the Republic of Serbia. The target population consisted of 3540 respondents aged 65 and over. The Patient Health Questionnaire-8 (PHQ-8) was used to evaluate the presence of depressive symptoms and the social support score from the questionnaire Oslo-3 scale of social support. The relations between depression symptoms and social support were examined with univariate logistic regression analyses. In the univariate regression model, social support stood out as a strong predictor of depression. People with weak social support are three and a half times more likely (OR = 3.45) to have depression compared to those with strong social support. Men with small social support were more likely to have depression (OR = 5.08) than women (OR = 3.41). These results indicate the urgency of addressing depression as a public health priority to reduce the burden and disability and improve the overall health of the elderly population. Abstract

Uvod: Nezadovoljna potreba za zaštitom zdravlja može se posmatrati kao spektar zdravstvenih potreba koje nisu optimalno zadovoljene. Cilj: Identifikacija faktora povezanih sa neostvarenim zdravstvenim potrebama u odrasloj populaciji Srbije. Metod: Istraživanje je sastavni deo Istraživanja zdravstvenog stanja stanovništva Srbije sprovedenog 2013. godine. U svrhu dobijanja rezultata za naše istraživanje upotrebljen je reprezentativni uzorak od 13 765 odraslog stanovništva starosne dobi od 20 i više godina. Upitnici koji su usklađeni sa upitnikom Evropskog istraživanja zdravlja, predstavljali su instrument istraživanja. Rezultati: Najveću prepreku za ostvarivanje zdravstvenih potreba kod odraslog stanovništva Srbije, činili su finansijski razlozi (19,9%), dugo čekanje na zakazanu posetu ili pregled (12,8%) i udaljenost odnosno problemi sa prevozom do mesta pružanja zdravstvene usluge (4,7%). Najteže se dolazilo do stomatološke (13,3%) i medicinske zdravstvene zaštite (12.9%), a zatim slede propisani lekovi (10,7%) i zaštita mentalnog zdravlja (2,4%). Zaključak: Istraživanje daje uvid u prediktore nejednakosti u korišćenju, pristupu i ostvarivanju zdravstvene zaštite sa ciljem skretanja pažnje donosiocima politike zdravstvene zaštite koje političke akcija treba preduzeti., Background: An unsatisfied need for health care can be viewed as a spectrum of health needs that are not optimally met. Aim: Identification of factors related to unmet health needs in the population of Serbia. Methods: The research is an integral part of the Health Survey of the Population of Serbia conducted in 2013 under the auspices of the Ministry of Health of the Republic of Serbia. For the purposes of this study, a representative sample of 13,765 adults aged 20 and over was used. Questionnaires, which were harmonized with the European Health Research Questionnaire, were a research instrument. Results: The biggest obstacle to meeting the health needs of the adult population of Serbia was financial reasons (19,9%), long waits for a scheduled visit or examination (12,8%) and distance or problems with transportation to the place of health care (4,7%). The most difficult were dental (13,3%) and medical health care (12,9%), followed by prescribed medication (10,7%) and mental health care (2,4%). Conclusion: The research provides insight into the predictors of inequality in the use, access and realization of health care in order to draw attention to health policy makers what policy action should be taken.

Abstract

Zdravstvena pismenost žena može značajno uticati na različite aspekte ženskog zdravlja. Cilj ove studije bio je procena odnosa zdravstvene pismenosti, socio-demografskih karakteristika i zdravstvenog ponašanja žena u oblasti reproduktivnog zdravlja u Srbiji. Istraživanje je sprovedeno kao studija preseka, na uzorku žena starijih od 18 godina, sa teritorije Republike Srbije, stratifikovanom prema regionu, vrsti naselja i starosnim kategorijama. Pri proceni zdravstvene pismenosti žena korišćena je standardizovana verzija evropskog upitnika o zdravstvenoj pismenosti (HLS-EU-Q47). The aim of this study was the assessment of relationship of health literacy, socio-demographic characteristics and reproductive health behaviors of women in Serbia. This was a crosssectional study on a random sample of women aged 18 and over from the territories of three Serbian regions, stratified according to age groups, region and type of settlement. A standardized version of the European Health Literacy Survey Questionnaire (HLS-EU-Q47) was used in assessing health literacy of women. Limited levels of health literacy were significantly higher among middle-aged women, among those living in rural areas, among women who rated their health as poor or very poor, and who exhibited more negative patterns of reproductive health behavior, indicating the need for implementation of prevention programs and strategies with the aim of increasing the level of health literacy. Abstract

Uvod: Karcinom dojke čini oko 25% svih malignih tumora žena, sa incidencom u porastu. Novije dijagnostičke procedure i terapijski modaliteti lečenja rezultiraju uspešnijim lečenjem i većom stopom preživljavanja žena obolelih od karcinoma dojke. Cilj savremenog lečenja podrazumeva i bolji kvalitet života (QoL) pacijentkinja obolelih od karcinoma dojke. Cilj ovog istraživanja je bio da se proceni psihosocijalna dimenzija kvaliteta života pacijentkinja obolelih od karcinoma dojke pre i posle hiruške intervencije. Materijal i metode: Analiziran je kvalitet života kod 170 pacijentkinja sa karcinomom dojke (43 pacijentkinje <50 i 127 pacijentkinje ≥50 godina) koje su dijagnostikovane u I i II kliničkom stadijumu bolesti, pre i nakon hiruškog lečenja. Pacijentkinje su popunjavale uputnike Evropske organizacije za istraživanje i lečenje karcinoma (EORTC-C30), specifičnu verziju za karcinom dojke (EORTC-BR23), Bekovu skalu depresivnosti (BDI) i sociodemografski upitnik. Rezultati: Analizom upitnika EORTC-C30 pokazano je da je hiruško lečenje visoko statistički značajno uticalo (p<0,001) na promenu kvaliteta života svih analiziranih pacijentkinja sa karcinomom dojke a najveći pad je u dimenzijama kognitivne i socijalne funkcije. Analiza uticaja hiruškog lečenja na kvalitet života primenom upitnika EORTC-BR23, pokazala je statistički značajno pogoršanje kvaliteta života nakon hiruškog lečenja u svim analiziranim kategorijama kod svih pacijentkinja. Analiza rezultata skora depresivnosti (BDI skor) pre i nakon hiruške intervencije, pokazala je da je prosečna vrednost skora BDI nakon hiruške intervencije gotovo dva puta povećana i da je ova razlika statistički značajna. Analizom potreba za socijalnom podrškom u vidu savetovališta i drugim vidovima socijalne podrške, pokazano je da postoji statistički značajna povezanost sa opštim zdravstvenim stanjem i socijalnim funkcionisanjem pacijentkinja pre hiruške intervencije. Newer diagnostic procedures and therapeutic modalities have resulted in more successful treatment and a higher survival rate for women with breast cancer. The goal of modern treatment is also to improve the quality of life (QoL) of breast cancer patients. The aim of this study was to evaluate the psychosocial dimension of quality of life (QoL) of breast cancer patients before and after surgery. Material and Methods: The quality of life of 170 breast cancer patients (43 female patients <50 and 127 female patients ≥50 years) who were diagnosed in the first and second clinical stages of the disease, before and after surgical treatment, was analyzed. Patients completed the guidelines of the European Organization for Research and Treatment of Cancer (EORTC QLQ-C30), breast cancer specific version (EORTC QLQ-BR23), Beck Depression Inventory (BDI), and sociodemographic questionnaire. Results: The analysis of the EORTC QLQ-C30 questionnaire showed that surgical treatment had a statistically significant (p <0.001) change in the quality of life of all breast cancer patients analyzed, with the largest decrease in the dimensions of cognitive and social function. The analysis of the impact of surgical treatment on quality of life using the EORTC QLQ-BR23 questionnaire showed a statistically significant deterioration in quality of life after surgical treatment in all analyzed categories in all patients. An analysis of the results of the depression score (BDI score) before and after surgery showed that the average value of the BDI score after surgery increased nearly twofold and that this difference was statistically significant. Uvod: Javnozdravstveni problemi sa kojima se susreću zemlje širom sveta su brojni. Među njima značajno mesto zauzima poremećaj uhranjenosti gde se posebno ističe gojaznost kao jedan od najvećih izazova u XXI veku čije je prisustvo povezano sa pojavom hroničnih nezaraznih bolesti. Smatra se da su socio-ekonomske karakteristike, ishrana, životni stilovi i higijenske navike ključne determinante uhranjenosti koje mogu da oblikuju zdravstvene ishode. Cilj: Cilj ove studije jeste ispitivanje povezanosti determinanti zdravlja sa gojaznošću i hroničnim bolestima kao njenim posledicama, kako bi se dobijeni rezultati iskoristili u kreiranju preventivnih mera i programa u borbi protiv ovog značajnog problema. Materijal i metod: Istraživanje predstavlja deo Istraživanja zdravlja stanovništva Srbije iz 2013. godine koje je sprovedeno od strane Ministarstva zdravlja Republike Srbije, kao studija preseka na reprezentativnom stratifikovanom dvoetapnom uzorku. Istraživanjem je obuhvaćeno 13922 ispitanika starosti od 19 godina i više. Instrument istraživanja su bili upitnici konstruisani u skladu sa upitnikom Evropskog istraživanja zdravlja. Rezultati : Rezultati istraživanja su pokazali da najveću šansu da budu prekomerno uhranjeni u celokupnoj populaciji imaju ispitanici muškog pola (OR=1,705), koji su u braku, starosti 55-64 godine (OR=3,260) sa završenom samo osnovnom školom (OR=1,486). Kada su u pitanju muškarci, najveću šansu za prekomernu uhranjenost imaju oženjeni muškarci, starosti od 55-64 godine, iz ruralne sredine koji su završili višu ili visoku školu, zaposleni su i spadaju u najbogatiji kvintil u pogledu materijalnog blagostanja. U slučaju žena najveću šansu za prekomernu uhranjenost imaju udate žene, starosti od 65-74 godine, iz ruralne sredine, koje su završile osnovnu školu ili manje, neaktivne u pogledu radnog statusa i koje pripadaju srednjem sloju u pogledu materijalnog blagostanja. One of the significant issues is definitely related to nutrition disorders, that is, obesity, as one of the greatest health challenges in the 21st century, correlating with the development of chronic non-communicable diseases. Socioeconomic characteristics, nutrition, lifestyles and hygiene habits are considered to be the key determinants of nutrition that may have its role in shaping possible health outcomes. Aim: The aim of this study is to examine the correlation between the health determinants and obesity and chronic diseases resulting from obesity, so that the results obtained could be used for the purpose of creating disease prevention measures and programs addressing this significant issue. Materials and method: The research is a part of the 2013 Serbian National Health Survey, conducted by the Ministry of Health of the Republic of Serbia as a cross-sectional study on a representative stratified two-tier sample. Abstract

UVOD: U radu zdravstvenih ustanova nastaje medicinski otpad, koji se sastoji iz dve komponente, neopasne (75% od ukupne količine) i opasne komponente (25%). U zdravstvenim ustanovava u posledljih deset godina unapređen je rad u postupanju sa medicinskim otpadom, u skladu sa Zakonom o otpadu i Pravilnikom o upravljanju medicinskim otpadom Republike Srbije. Promene u upravljanju sa medicinskim otpadom (UMO) se sastoje iz uvođenja procedura za bezbedno postupanje opasnim medicinskim otpadom od generisanja medicinskog otpada do odlaganja. METOD RADA: Studija predstavlja istraživanje prospektivnog i retrospektivnog tipa na reprezentativnom uzorku zdravstvenih ustanova državnog sektora zdravstva, na teritoriji Republike Srbije bez Kosova i Metohije, koje se međusobno razlikuju u odnosu na vrstu ustanove, nivo zdravstvene zaštite i teritorijalnu pripadnost. Instrumenti istraživanja koji su korišćeni u studiji, su standardizovani upitnici za ispitivanje UMO u sistemima zdravstvene zaštite. Ukupno 116 ustanova je obuhvaćeno istraživanjem i to sa srazmernom zastupljenošću svih vrsta zdravstvenih ustanova (domovi zdravlja, opšte i specijalne bolnice, kliničko-bolnički centri, klinički centri, zavodi i instituti za javno zdravlje, ostale ustanove) iz tri nivoa zdravstvene zaštite. REZULTATI: Izdvojeni su ključni činioci UMO koji utiču na pružanje i kvalitet zdravstvene zaštite u zdravstvenim ustanovama na različitim nivoima zdravstvene zaštite (primarni, sekundarni, tercijarni) i u različitim vrstama zdravstvenih ustanova (dom zdravlja, opšta bolnica, klinički centar, kliničko bolnički centar, institut ili zavod za javno zdravlje) pojedinačno. Ovom studijom utvrđene su razlike među zdravstvenim ustanovama u odnosu na UMO. Činioci za procenu UMO na primarnom nivou zdravstvene zaštite su broj i vrste zdravstvenih usluga, dok na nivou sekundarne i tercijarne zdravstvene zaštite, ključne činioce predstavlja broj kreveta i zauzetost postelja. ZAKLjUČAK: Istraživanjem je utvrđeno da, INTRODUCTION: Healthcare waste is generated within daily activity of healthcare institutions, and it’s composed of two components, non-hazardous healthcare waste (75% of the total quantity) and hazardous components (25%). In the healthcare institutions in the last ten years, the practices concerning healthcare waste management has been improved, in accordance with the Law on Waste Management and with the Rulebook on the management of healthcare waste in the Republic of Serbia. Changes in the management of healthcare waste (HCWM) are based on introduction of procedures for the safe handling of hazardous healthcare waste from the point of its generation to safe disposal. METHOD: The study is a prospective and retrospective type survey on a representative sample of health institutions in the state health sector, on the territory of the Republic of Serbia without Kosovo and Metohija, which differ from one another in relation to the type of institution, the level of health care and territorial affiliation. The research tools used in the study are standardized questionnaires for testing UMO in health care systems. A total of 116 institutions were included in the survey, with proportional representation of all types of health institutions (health centers, general and special hospitals, clinic and hospital centers, clinical centers, institutes and public health institutes, other institutions) from three levels of health care. RESULTS: Study was defined the key factors of HCWM that influence the quality of health care services in health care institutions at different levels of health care (primary, secondary, tertiary) as well as in the different types of health institutions (health center, general hospital, clinical center, clinical hospital center, institute or Public Health Institute). Abstract

Zloupotreba psihoaktivnih supstanci među mladima, predstavlja značajan problem pojedinca, porodice i društva. Predstavlja globalni problem i tiče se adolescenata širom sveta. Problem zloupotrebe i zavisnosti od psihoaktivnih supstanci najčešće se posmatra sa individualnog aspekta problema ličnosti, dok je socijalni kontekst često zanemaren, a kako bi se preduzele mere prevencije koje bi dale zadovoljavajuće rezultate neophodno je napraviti odgovarajuće strategije za borbu protiv zloupotrebe psihoaktivnih supstanci koje treba da budu u skladu sa demografskim i socioekonomskim karakteristikama zajednice. U tom kontekstu želeli smo da našim istraživanjem utvrdimo učestalost korišćenja psihoaktivnih supstanci, sa aspekta demografskih i socioekonomskih faktora koji doprinose ovakvom ponašanju mladih u Srbiji, kako bi dobijene rezultate iskoristili u kreiranju promotivnih intervencija i programa za suzbijanje ovog značajnog problema. Studija se bavi ispitivanjem uticaja demografskih i socioekonomskih faktora na učestalost korišćenja psihoaktivnih supstanci u populaciji mladih u Srbiji. Istraživanje predstavlja deo Istraživanja zdravlja stanovništva Srbije iz 2013. godine koje je sprovedeno od strane Ministarstva zdravlja Republike Srbije, kao studija preseka na reprezentativnom stratifikovanom dvoetapnom uzorku. Istraživanjem je obuhvaćeno 1722 ispitanika starosti od 15 do 24 godine. Instrument istraživanja su bili upitnici konstruisani u skladu sa upitnikom Evropskog istraživanja zdravlja. Abstract

Doktorska disertacija predstavlja studiju kojom je procenjen značaj problema zloupotrebe psihoaktivnih supstanci kod mladih uzrasta 12-19 godina u Srbiji, kojom su ispitani i utvrđeni faktori rizika za zlpupotrebu psihoaktivnih supstanci ove populacione grupe. U istraživanju je primenjen dizajn studije preseka, na reprezentativnom uzorku stanovništva Srbije kao celine i stanovništva velikih područja (Centralna Srbija, Vojvodina, Beograd), po polu, starosti i mestu življenja. Uzorak ovog istraživanja čini 1939 ispitanika starosti 12-19 godina ekstrahovan iz uzorka stanovništva Srbije formiranog za potrebe istraživanja u okviru Projekta ,,Zdravstveno stanje, zdravstvene potrebe i korišćenje zdravstvene zaštite stanovništva Srbije“ sprovedenog 2006. godine. Izvor podataka o odabranim obeležjima čini posebno konstruisan Upitnik za decu i omladinu uzrasta od 7 do 19 godina. Svi podaci iz anketnih upitnika su prikazani i analizirani adekvatnim statističkim metodama primerenim tipu i vrsti podataka. Najznačajniji rezultati istraživanja su da ispitanici koji imaju lošiji uspeh u školi 16 puta češće puše od onih koji imaju odličan uspeh. Mladi čiji drugovi puše, 13 puta češće puše od onih čiji drugovi ne puše, dok 9 puta češće puše ispitanici koji potiču iz porodica čiji su članovi pušači. Alkoholna pića 4 puta češće piju ispitanici koji puše, 3 puta češće ispitanici koji koriste opijate, dok ispitanici koji imaju ispravan (negativan) stav prema alkoholu 3 puta manje piju. Mladi u Srbiji su u najvećem procentu probali marihuanu, tablete (bensedin, trodon, amfetamin) i lepak. U slučajevima kada neko od društva koristi drogu ispitanici 7,5 puta češće koriste drogu što pokazuje da je vršnjački uticaj na konzumiranje psihoaktivnih supstanci veliki. Na osnovu navedenih zaključaka sledi da je neophodno intenzivirati preventivne aktivnosti kao i uključivanje mladih u timove za kreiranje preventivnih programa kao ključnih prenosioca poruka koje imaju za cilj sprečavanje započinjanja upotrebe psihoaktivnih supstanci. Doctoral dissertation presents a study that assessed the importance of the problem of psychoactive substances abuse among young people aged 12-19 years in Serbia, and which allowed risk factors for psychoactive substances abuse for this population to be tested and identified. We have applied a cross-sectional study design on a representative sample of the population of Serbia as a whole, and residents of large areas (Central Serbia, Vojvodina, Belgrade), by sex, age and place of living. The sample of this research, which included 1939 respondents aged 12-19 years, was extracted from a sample of the population of Serbia, formed for the purpose of research in the project „Health status, health needs and use of health services in Serbia“ conducted in year 2006th. Source of information on the selected features was the specially designed questionnaire for children and adolescents aged 7 to 19 years. All data from questionnaires are presented and analyzed using appropriate statistical methods in accordance with the data type. The most important results of the research were that subjects who have worse school grades are 16 times more likely to smoke than those who have a good academic record. Young, whose friends smoke, are 13 times more likely to smoke than those whose friends do not smoke, and 9 times more likely to smoke are respondents that come from families whose members are smokers. Respondents who smoke consume alcoholic drinks 4 times more frequent, three times more respondents who use opiates, while respondents with the correct (negative) attitude towards alcohol drink alcohol 3 times less. Young people in Serbia in the highest percentage tried marijuana, pills (bensedin, Trodon, amphetamine) and glue. In cases where one of members of the company use drugs, respondents use drugs 7.5 times more likely, which indicates that the peer influence on psychoactive substances abuse is significant. Based on these findings, it can be concluded that it is necessary to intensify preventive actions and the involvement of young people, as key messengers, in teams for creating prevention programs that aim to prevent initiation of psychoactive substances abuse.

Uzorak ovog istraživanja čini 1939 ispitanika starosti 12-19 godina ekstrahovan iz uzorka stanovništva Srbije formiranog za potrebe istraživanja u okviru Projekta ,,Zdravstveno stanje, zdravstvene potrebe i korišćenje zdravstvene zaštite stanovništva Srbije“ sprovedenog 2006. godine. Izvor podataka o odabranim obeležjima čini posebno konstruisan Upitnik za decu i omladinu uzrasta od 7 do 19 godina. Svi podaci iz anketnih upitnika su prikazani i analizirani adekvatnim statističkim metodama primerenim tipu i vrsti podataka. Najznačajniji rezultati istraživanja su da ispitanici koji imaju lošiji uspeh u školi 16 puta češće puše od onih koji imaju odličan uspeh. Mladi čiji drugovi puše, 13 puta češće puše od onih čiji drugovi ne puše, dok 9 puta češće puše ispitanici koji potiču iz porodica čiji su članovi pušači. Alkoholna pića 4 puta češće piju ispitanici koji puše, 3 puta češće ispitanici koji koriste opijate, dok ispitanici koji imaju ispravan (negativan) stav prema alkoholu 3 puta manje piju. Mladi u Srbiji su u najvećem procentu probali marihuanu, tablete (bensedin, trodon, amfetamin) i lepak. U slučajevima kada neko od društva koristi drogu ispitanici 7,5 puta češće koriste drogu što pokazuje da je vršnjački uticaj na konzumiranje psihoaktivnih supstanci veliki. Na osnovu navedenih zaključaka sledi da je neophodno intenzivirati preventivne aktivnosti kao i uključivanje mladih u timove za kreiranje preventivnih programa kao ključnih prenosioca poruka koje imaju za cilj sprečavanje započinjanja, Doctoral dissertation presents a study that assessed the importance of the problem of psychoactive substances abuse among young people aged 12-19 years in Serbia, and which allowed risk factors for psychoactive substances abuse for this population to be tested and identified. We have applied a cross-sectional study design on a representative sample of the population of Serbia as a whole, and residents of large areas (Central Serbia, Vojvodina, Belgrade), by sex, age and place of living. The sample of this research, which included 1939 respondents aged 12-19 years, was extracted from a sample of the population of Serbia, formed for the purpose of research in the project „Health status, health needs and use of health services in Serbia“ conducted in year 2006th. Source of information on the selected features was the specially designed questionnaire for children and adolescents aged 7 to 19 years. All data from questionnaires are presented and analyzed using appropriate statistical methods in accordance with the data type. The most important results of the research were that subjects who have worse school grades are 16 times more likely to smoke than those who have a good academic record. Young, whose friends smoke, are 13 times more likely to smoke than those whose friends do not smoke, and 9 times more likely to smoke are respondents that come from families whose members are smokers. Respondents who smoke consume alcoholic drinks 4 times more frequent, three times more respondents who use opiates, while respondents with the correct (negative) attitude towards alcohol drink alcohol 3 times less. Young people in Serbia in the highest percentage tried marijuana, pills (bensedin, Trodon, amphetamine) and glue.
