Abstract Purpose The aim of this study is to compare dental implant placement accuracy of three surgical guide fabrication methods: single (SCT) and double computed tomography (DCT), and a newly developed modified SCT (MSCT) scan method. Methods A total of 183 cases (183 surgical guides, and 485 implants) of static-guide-assisted implant placement surgery using the SCT, DCT, or MSCT methods in a dental clinic were included in the study. Three-dimensional (3D) deviations (mm) at the entry and tip of the implant body between preoperative simulation and actual placement were measured as surrogate endpoints of implant placement accuracy. The following survey details were collected from medical records and CT data: sex, age at implant placement surgery, surgical guide fabrication method, number of remaining teeth, implant length, implant location, alveolar bone quality, and bone surface inclination at implant placement site in preoperative simulation, etc. Risk factors for reducing implant placement accuracy were investigated using generalized estimating equations. Results The SCT and DCT methods (odds ratios [ORs] vs. MSCT method: 1.438, 1.178, respectively), posterior location (OR: 1.114), bone surface buccolingual inclination (OR: 0.997), and age at implant placement surgery (OR: 0.995) were significant risk factors for larger 3D deviation at the entry; the SCT (OR: 1.361) and DCT methods (OR: 1.418), posterior location (OR: 1.190), implant length (OR: 1.051), and age at implant placement surgery (OR: 0.995) were significant risk factors for larger 3D deviation at the tip of the implant body. Conclusions Implant placement accuracy was better using the MSCT method compared to the SCT and DCT methods.

Diagnosis and implant treatment planning in cases where hopeless teeth need to be extracted and immediately replaced with implants are more difficult and challenging than a standard multistage approach. Software and CAD/CAM technologies are becoming increasingly common to help clinicians in their practices. They use data from CT scans of the patient and of a radiographic guide for virtual implant treatment planning on a computer and the successive production of a surgical template, which will facilitate surgery and enable fabrication of a prosthesis to be delivered immediately. These procedures, however, are well-established only for patients who are already edentulous in the area to be treated; they are difficult to apply to a patient whose desire is to keep the hopeless dentition until the day of implant surgery to avoid having to wear a removable partial or complete denture. This article describes the advantages and the technique for fabrication of a novel 2-piece radiographic guide that, in conjunction with CAD/CAM technology, will help clinicians in the diagnosis and implant treatment planning of patients who want to retain their teeth until surgery. A method is presented describing the laboratory fabrication and clinical use of an innovative template. This template was designed to function as a radiographic implant positioning guide, accurate bone sounding guide, surgical guide, an aid in flap reflection and as a pick-up impression tray. ABSTRACT Purpose: The aim of this study was to develop and evaluate a new stereoscopic technique for conversion of radiographic guide into surgical guide for dental implant placement. Materials and Methods: Ten partially dentate patients requiring 18 implants for tooth replacement were recruited. Radiographic guides were modified with the addition of index rods for double computed tomography scanning. Implant positions were planned with implant planning software, and the stereoscopic angulations were measured. The radiographic guides were converted into surgical guides using either a generic bench drill (Group A, n = 9) or a milling machine (Group B, n = 9). Stereolithographic surgical guides were also made for three patients (Group S, n = 5). Differences between the planned and actual angulations were tested by pair-sample t-test. Difference of mean angle deviation among groups was tested by Brown-Forsythe test. Differences were considered significant if p < .05. Results: Eighteen implant sites were successfully treated with the converted surgical guides. The mean angle deviation of Group A (1.3 ± 0.6°) was significantly greater than Group S (0.4 ± 0.6°), while no differences were found between Group B (0.9 ± 0.3°) and Group S. The linear error was greatest in Group A with 1.5 mm at the head and 1.8 mm at the apex of the implant. Conclusions: The use of this new stereoscopic technique appears to be an acceptable alternative method for converting radiographic guide into surgical guide. [ABSTRACT FROM AUTHOR]

The traditional prosthetic steps in the fabrication of a fixed complete denture after implant osseointegration include final impression, verification of implant positioning in the working cast, mounting of the working cast, and mock denture wax trial insertion prior to the laboratory fabrication of the metal substructure; however, in patient scenarios of immediate loading of implants, the interim conversion prosthesis can be used to advance from the final impression to the milling of the underlying framework in one appointment. Consistency in the initial wax trial insertion, radiographic guide, and intraoral positioning of the conversion prosthesis can result in a well-designed definitive prosthesis in less time with the use of the existing duplicate complete denture. A novel technique using the patient's existing complete dentures as a radiographic guide for diagnosis and treatment planning in implant dentistry is presented. Tin foil is used to cover the denture teeth before the radiographic scan is performed. Advantages of the described technique include its cost-effectiveness, simplicity, efficiency, and lack of need to modify or duplicate the patient's existing dentures. This article focuses on radiographic imaging with regard to planning, treating, and maintaining partially and completely edentulous prosthodontic patients with dental implants. Cone-beam computed tomography (CBCT) is the preferred imaging method for pretreatment dental implant treatment planning. Radiographic guides containing radiopaque materials and/or fiducial markers transfer both the proposed prosthesis design and desired implant location for appropriate radiographic evaluation.
