The territories of AP Kosovo and Metohija represent the challenging and fast-growing wine market. However, many wine producers from Kosovo and Metohija are forced to acquire an innovative marketing approach which provides enhanced consumer experience and value. Therefore, they have implemented various marketing strategies in order to develop a positive brand reputation and create new opportunities for small and medium-sized wine producers. This paper aims to inspect the role of digital marketing in promotion of wine from Kosovo and Metohija, which enables producers to combine and integrate different marketing techniques in order to promote their strengths, that is, brand. Based on the primary data collected through the questionnaires, in-depth interviews and surveys, this study attempts to show how using digital marketing is statistically significant with reported increases in wine sale. The findings show that digital marketing facilitates the implementation of various marketing strategies which can improve the value of the consumption experience offered to the customer.

7. Warren shunt combined with partial splenectomy in children with extra-hepatic portal hypertension, massive splenomegaly and severe hypersplenism

Introduction. Extra-hepatic portal vein obstruction (EHPVO) is one of the most often causes of portal hypertension in children. Objective. Establishing the importance of shunt surgery in combination with partial spleen resection in selected pediatric patients with EHPVO, enormous splenomegaly and severe hypersplenism. Methods. Distal splenorenal shunt (DSRS) with partial spleen resection was performed in 22 children age from 2 to 17 years with EHPVO. Indications for surgery were pain and abdominal discomfort caused by spleen enlargement, as well as symptomatic hypersplenism with leucopenia, thrombocytopenia and anemia. The partial spleen resection was performed by ligation of blood vessels to caudal two thirds of the spleen. After ischemic parenchymal demarcation transection with electrocautery LigaSure was performed with preservation of 20-30% of spleen tissue, and then Warren DSRS was created. Platelet and leucocytes counts and liver function tests were obtained before, one month and one year after surgery. Growth was assessed with SD scores (Z scores) for height, weight and body mass index at the time of surgery and one year later. Results. In all patients postoperative period was without significant complications. Platelets and leucocytes counts were normalized. Patency rate of shunts was 100%. Two significant shunts stenosis were observed and successfully treated with percutaneous angioplasty. During the follow-up period (1 to 9 years) all patients were asymptomatic, with improved quality of life and growth. Conclusion. Results of our study indicate that shunt surgery with a partial spleen resection is an effective and safe procedure for patients with enormous splenomegaly and severe hypersplenism caused by EHPVO.

8. Early results of liver resection due to metastases of colorectal carcinoma

Background/Aim. Liver metastases are most frequently the result of colorectal carcinoma. The aim of this study was to analyse early results of operative treatment of the patients with the liver metastases of colorectal carcinoma. Methods. This retrospective, prospective study included 387 patients with colorectal carcinoma operated during the period from 2005-2007. All the patients were submitted to oncologic surgery protocol. The functional state of the liver was assessed, especially in the patients with hemotherapy. Diagnostic protocol further included color Doppler ultrasonography of the liver and port system, as well as spiral computed tomography includy angiography. Nine (5.7%) of the patients were submitted to explorative laparotomy. Results. Of all the patients 157 (40.6%) had metastases in the liver, synhronous 78 (20.15%) and metachronous 79 (20.45%), Forty two (26.7%) patients were indicated for the liver resection. In 33 (21%) of them it was performed successfully. There were 18 females and 15 males of the average age 60.09 (42-81) years. Up to 4 metastases had 90.9% of the patients and in 9.09% had up to 5 and more metastases. On average, metastases occupied 2.6 of the liver segments. There were 21.2% of solitary lesions (7 patients), 63.6% of multilple unilobular (21 patients) and 15.15% of multiple bilobular metastases (5 patients). Liver resection was done using ultraharmonic scalpel. At spacious liver resections we used a device for intraoperative saving of blood (cell saver) and applied a technique of selective hepatic vascular exclusion. An average disease-free interval was 16.7 months. There were 60.6% of anatomical liver resections and 39.4% of atypical resections. In 31.1% of the operated patients we made resection on up to two segments of the liver and in 30.3% we made resection of four segments. An average number of segments where resection had been performed was 2.65 and the duration of operation was 143 minutes. In 39.4% of the cases we used a technique of liver vascular isolation, an average duration was 38 minutes. In 30.3% of resections we used cell saver and 70% of operated patients were transfused. On average, 493 ml of blood was transfused. Of the total number of resections, 90.9% was of the type R0, 9.09% of the type R1. An average duration of postoperative hospitalization was 10.6 days. Operative morbidity rate was 15.15% and operative mortality 3.03%. During a six-month observing there were not any mortality or repeated metastases in liver. Conclusion. Anatomic liver resection including selective vascular hepatic excision by the use of an ultraharmonic scalpel and cell saver is considered to be efficient and secure method for the reduction of intraoperative and postoperative complications rendering good surgical results.

9. Distal splenorenal shunt with partial spleen resection

Introduction: Hypersplenism is a common complication of portal hypertension. Cytopenia in hypersplenism is predominantly caused by splenomegaly. Distal splenorenal shunt (Warren) with partial spleen resection is an original surgical technique that regulates cytopenia by reduction of the enlarged spleen. Objective. The aim of our study was to present the advantages of distal splenorenal shunt (Warren) with partial spleen resection comparing morbidity and mortality in a group of patients treated by distal splenorenal shunt with partial spleen resection with a group of patients treated only by a distal splenorenal shunt. Method. From 1995 to 2003, 41 patients with portal hypertension were surgically treated due to hypersplenism and oesophageal varices. The first group consisted of 20 patients (11 male, mean age 42.3 years) who were treated by distal splenorenal shunt with partial spleen resection. The second group consisted of 21 patients (13 male, mean age 49.4 years) that were treated by distal splenorenal shunt only. All patients underwent endoscopy and assessment of oesophageal varices. The size of the spleen was evaluated by ultrasound, CT or by scintigraphy. Angiography was performed in all patients. The platelet and white blood cell count and haemoglobin level were registered. Postoperatively, we noted blood transfusion, complications and total hospital stay. Follow-up period was 12 months, with first checkup after one month. Results In the first group, only one patient had splenomegaly postoperatively (5%), while in the second group there were 13 patients with splenomegaly (68%). Before surgery, the mean platelet count in the first group was 51.6±18.3x109/l, to 118.6±25.4x109/l postoperatively. 10. Isolated aneurysm of the internal iliac artery and disorders of the pelvic organs functions

Background. Isolated aneurysm of internal iliac artery is very rare and often asymptomatic. Aneurysm itself can produce malfunction of the pelvic organs. Aneurysm rupture is followed by high mortality. Case report. A 76-year old patient was admitted to the hospital with abdominal and left groin pain, disuric problems, obstipation and the signs of intestinal subocclusion. These problems persisted over the last few months. Digitorectal examination showed pulsatile tumor. Computed tomography and angiography revealed isolated aneurysm of the left internal iliac artery with a maximal diametar of 13.5 cm. The aneurysm was treated operatively using extraperitoneal approach in general anesthesia. During the operation a Cell Saver was used. The left internal iliac artery was resected and ligated with end-aneurysmatic suture of its branches. Conclusion. Isolated aneurysm of the internal iliac artery should be considered by differential diagnosis in any case of the occurrence of disorders of the pervic organs functions. Clinical findings, ultrasound examination, computed tomography and angiography are the diagnostic techniques of choice which can confirm the diagnosis. Surgical treatment with the use of retroperitoneal approach lead to complete recovery, so it could be considered the method of choice for patients with the condition permitting a radical surgical approach. .

Background. Abdominal compartment syndrome (ACS) is a rapid increase in intra-abdominal pressure associated with multi-organs dysfunction. It is caused mostly by abdominal bleeding und massive volume compensation. Case report. We reported a 76-year-old patient admitted to the hospital with aortic abdominal aneurysm, 13.7 cm in diameter, ruptured in vena cava, which caused intraabdominal hypertension, the liver and kidney dysfunction, as well as circulation, respiration and metabolic disorders. Intraabdominal pressure was measured by bladder manometry. Central venous pressure and systemic arterial pressure were monitored continuously. Clinical signs were thrill and typical abdominal bruit. Aorto-caval fistula was diagnosed by the use of contrast computerized tomography. Caval endoaneurysmatic suture and aortobiiliac bypass with 18 × 9 mm Dacron prothesis were performed. Haemodynamic changes were mostly corrected during the surgery. The complete correction of haemodynamics, liver, kidney, respiration and metabolic changes was established in the next few weeks. Conclusion. The ACS was caused by rupture of abdominal aortic aneurysm in vena cava followed by edema of the abdominal organs, retroperitoneum, abdominal wall and ascites. Caval endoaneurysmatic suture and aortobiiliac bypass with 18 × 9 mm Dacron prothesis solved aortocaval fistula as well as all the organs and metabolic dysfunctions caused by ACS.

Background. The coexistence of a well-differentiated liposarcoma (atypical lipomatous tumor) and benign smooth muscle component in a single soft tissue neoplasm is extremely rare. Case report. Histologic and immunohistochemical characteristics of tumor, which represented the reccurence of a retroperitoneal atypical lipoma in a 50-year-old female patient are presented in this paper. The tumor represented the recurrence of a retroperitoneal atypical lipoma. Lipomatous component consisted of a mixture of lipoma-like and sclerosing variant of well-differentiated liposarcoma. Characteristically, the heterologous smooth muscle differentiation was manifested as a macroscopically visible nodular growth in the form of a leiomyoma within the adipocytic component. It consisted of intersecting fascicles of spindle cells, which lacked mitoses and significant atypia, and were immunopositive for smooth muscle markers. Conclusion. This case of well-differentiated liposarcoma with benign smooth muscle is the first reported tumor of retroperitoneal localization. It is also the first one to exhibit the heterologous smooth muscle component as a distinct leiomyoma.

17. Monitoring and prevention of anemia relying on nutrition and environmental conditions in sports.

Conflict of Interest: none declared., Introduction: Anemia is a blood disorder characterized by abnormally low levels of healthy red blood cells or reduced hemoglobin, the iron-bearing protein in red blood cells that delivers oxygen to tissues throughout the body. The most common symptoms of this disorder are fatigue, weakness and, in extreme cases, shortness of breath or palpitations, or you may have no symptoms at all. Sports anemia is a term loosely applied to a least three different conditions: hemodilution, iron deficiency anemia and foot-strike anemia. Not exclusive to athletes, iron deficiency anemia occurs most often among women who may lose more iron each month when they menstruate than they take in., Material and Methods: Therefore, we examined its effect on the physical condition of female athletes. Several years (since 2010th until 2012th), we studied how anemia among girls (pioneers, juniors and seniors categories) that are involved in sports (women's soccer, volleyball and handball) in Rasina's district (Serbia), affecting their physical fitness. When their trainers approach to us, complaining that they have players who are great, so extraordinary talents, but by no means able to withstand more than twenty minutes in the game, we suggest them to perform laboratory tests. It was tested 134th female athletes., Results and Discussion: Anemia was observed in 43. (9. pioneers, 19. juniors and 15. seniors). So, laboratory results showed that in these girls anemia causes poor sport condition. After that, the girls enhanced nutrition. Their diet consisted of iron supplements and vitamins. Altitude training was organized for them, also. After all these treatments, condition significantly improved. It was first time that trainers in Rasina's district realizing significance of laboratory tests.

- Abstract
Background: Visceral artery aneurysms (VAA) represent a rare clinical entity with possible life-threatening complications. The presentation, diagnosis and management vary accordingly to the artery involved and the underlying pathology., Patients and Methods: During a 25-year period (1980-2005), 35 patients (25 males + 10 females, age range 36-73 years-median 59.2 years) with VAA were treated at two tertiary vascular surgery centers in Belgrade. All data were retrospectively collected from the patient's records., Results: On presentation, 19/35 patients were symptomatic, and 3/35 had ruptured VAA. Surgery was performed in 28 cases; most commonly involved arteries were splenic (11), hepatic (5), celiac trunk (5), superior mesenteric (3), inferior mesenteric (3) and gastroduodenal (1). Fatal rupture occurred in two patients. In 5 patients abdominal aortic aneurysm was associated with VAA, and in 4 patients multiple aneurysms of the involved artery were noted. Successful embolization was performed in 3 patients. Overall, four patients were treated medically. In the surgically treated patients, perioperative mortality and morbidity were 11% (3/28) and 40% (10/25) respectively. Of 25 patients included in the long-term follow up, six died., Conclusion: Since VAA have considerable tendency to rupture, an active approach is necessary. Based on our experience, surgical treatment could be recommended for any VAA patient with symptoms. In addition, we believe that the choice of the therapeutic procedure should be made on an individual basis.

- Abstract
We evaluated results after angioplasty of the distal abdominal aorta using "kissing-balloon" technique. MATERIALS AND METHODS. From 1981 through 1993, 14 patients (12 women and 2 men, average age 52.4 years), underwent balloon angioplasty of the distal segment of the abdominal aorta. In 8 patients stenosis involved one or both iliac arteries as well. In all cases double balloon technique was used. Follow-up was based on clinical, angiographical and duplex scanning examinations. RESULTS. The initial success rate was 85% (12/14). In one case there was residual stenosis of 20% (redilated successfully 1 month later), and in one case there was acute thrombosis of the right iliac artery (successfully relieved surgically). Long-term follow-up (6-143 months) showed good patency of the dilated segment in all patients (including patients that initially failed). At follow-up, all patients were asymptomatic. CONCLUSION. Dilatation of the distal abdominal aorta using "kissing-balloon" technique is a safe and efficient modality of treatment, with acceptable initial and excellent long-term results.

21. Effect of blood flow on the fate of venous grafts in the arterial circulation.

- Abstract
The paper deals experimentally with the influence of the blood flow on the long-term fate of venous grafts in the arterial circulation. Based on the results of experiments, and applying the laws of haemodynamics, the paper provides evidence that the blood flow has a decisive influence on the behavior of the implanted vein. Veins with a mean flow of 364.5 ml/min, dilate by 70% while the external diameter of veins with a mean flow of 65.4 ml/min. remain without major changes. This difference in the behaviour is statistically significant at the 1% level. From the results ensues that the vein is suited for reconstructions where a blood flow of 50--200 ml/min. may be expected.
