Abstract

Odezva hráček týmových sportů na tréninkovou a zápasovou zátěže je individuálně rozdílná. Ačkoliv při kolektivním tréninku a shodných tréninkových zátěžích lze očekávat obdobnou odezvu, korespondující se záměrem trenéra a charakterem zátěže, individuální rozdíly řady parametrů, charakterizujících reakci jednotlivých systémů na zátěž, se budou mezi jednotlivými hráčkami lišit. Rozdíly v odezvě na zátěž souvisejí nejenom s různou trénovaností hráček, různou délkou jejich hráčské kariéry, věkem, rozdílným somatotypem a dalších somatických parametrech, ale i na různém přístupu hráček k tréninkovým úkolům a na jejich rozdílných psychických a morálních vlastnostech. Pro trenéra je vítané, může-li se při hodnocení odezvy hráček na tréninkové či zápasové zatížení opřít o objektivně stanovené tzv. biomarkery, které mu pomohou při zpětnovazebním hodnocení účinnosti tréninkového procesu resp. poskytnou informaci o individuálních odlišnostech reakce jednotlivých hráček na zatížení. Cílem pilotního projektu sledování odezvy hráček na zátěž při házené bylo a) hodnocení odezvy na tréninkovou zátěž s různým herním zaměřením, b) hodnocení úrovně zotavení druhý den po tréninkové zátěži kondičního charakteru a c) hodnocení odezvy hráček na zápasovou zátěž.

Abstract

Although laboratory medicine practise varies across the European Union's (EU) member states, the extent of overlap in scope is such that a common syllabus describing the education and training associated with high-quality, specialist practise can be identified. In turn, such a syllabus can help define the common set of skills, knowledge and competence in a Common Training Framework (CTF) for non-medical Specialists in Laboratory Medicine under EU Directive 2013/55/EU (The recognition of Professional Qualifications). In meeting the requirements of the directive's CTF patient safety is particularly enhanced when specialists seek to capitalise on opportunities for free professional migration across EU borders. In updating the fourth syllabus, the fifth expands on individual discipline requirements, new analytical techniques and use of statistics. An outline structure for a training programme is proposed together with expected responsibilities of trainees and trainers; reference is provided to a trainee's log book. In updating the syllabus, it continues to support national programmes and the aims of EU Directive 2013/55/EU in providing safeguards to professional mobility across European borders at a time when the demand for highly qualified professionals is increasing in the face of a disparity in their distribution across Europe. In support of achieving a CTF, the syllabus represents EFLM's position statement for the education and training that underpins the framework.

25. Creatine Kinase and Myoglobin Levels as Indicators of Perioperative Muscle Damage during Open- and Mini-Invasive Stabilization of Thoracic and Lumbar Spine Fracture - a Prospective Randomized Study.

Abstract

PURPOSE OF THE STUDY In this randomized prospective study, we monitored and compared perioperative changes in skeletal muscle enzymes blood levels in open and mini-invasive stabilization of thoracolumbar spine fractures. The established hypothesis was to confirm higher blood levels of muscle enzymes in open stabilization. MATERIAL AND METHODS This study included 38 patients with the mean age of 46.4 years. 19 injuries were managed in an open procedure and 19 procedures were mini-invasive. Venous blood was taken intermittently at short intervals to determine the levels of skeletal muscle enzymes. The catalytic concentration of creatine kinase was determined via an enzymatic UV-test, and the concentration of myoglobin via electro-chemiluminescent immunoassay. Enzyme levels were processed statistically. The Wilcoxon test was used. RESULTS The median increase in the values of both enzymes is higher in the mini-invasive method than in the open method in both the surgery phase for the injury and in the extraction phase. The median increase in the values of both enzymes is higher in both methods for the primary procedure phase compared to the extraction phase. All results are statistically significant at p of <0.05. All tests were calculated using the MATLAB Statistics Toolbox. DISCUSSION A very surprising finding, when testing the hypothesis of the levels increasing mainly in open stabilization, was confirming the opposite. Both enzymes were higher in the mini-invasive approach to stabilising the spine after the injury, but also after the extraction. This contradicts the available literature. However, this can be explained by the methodology of enzyme levels determination in the previously published studies. We believe that this phenomenon can be partially caused by an iatrogenic mini-compartment of muscles in the postoperative period, absence of wound drainage, but also by higher muscle contusion when inserting bolts through the tubes via small incisions, when the tubes penetrate to the entry points relatively violently and the muscles in this area are affected more than in the classical skeletization. CONCLUSIONS Analysis of biochemical changes in open and mini-invasive surgery did not confirm the hypothesis that levels of creatine kinase and myoglobin enzymes increase especially in open stabilization. Abstract

Objectives: To identify factors involved in reno-pulmonary interactions during mechanical ventilation. Materials and methods: The study included a total of 25 domestic piglets. The animals were divided into three groups based on different strategies of ventilation. Group A; spontaneously breathing piglets; group B animals ventilated with tidal volume of 6 ml.kg-1 and group C with animals ventilated with tidal volume 10 ml.kg-1. Clini- cal monitoring and laboratory tests were performed for all groups at baseline and then at 1 hour and 12 hours for groups B and C. Ventilation indices, hemodynamics, urine output, creatinine clearance, glomerular fi ltration index, fractional excretion of sodium, free water clearance and tissue samples were recorded. The data ob- tained were statistically analysed. Results: Lower creatinine clearance and renal indices were seen in group B (p < 0.05) and in group C (p < 0.001) at 1 hour, and a difference in urine output for group C (p < 0.01) compared to group A was observed. At 12 hours, there was a further reduction in creatinine clearance and renal indices for group B (p < 0.05) and group C (p < 0.01). The lung mechanics and hemodynamics were not signifi cantly infl uenced. Conclusions: The study showed a causal relationship between renal dysfunction and positive pressure mechani- cal ventilation with respect to tidal volume and time (Tab. 4, Fig. 2, Ref. 17). Abstract

Objective: The aim of our study was the implementation of HPLC method with UV detection for betaine and dimethylglycine in human plasma samples. Then quantification of these analytes in patients with metabolic syndrome and prediabetes and compare results between them and the control group. Design: retrospective. Settings: Department of Clinical Biochemistry and Hematology, Faculty of Medicine, Charles University and Faculty Hospital, Alej Svobody 80, 304 60 Pilsen (Czech Republic). Material and methods: We developed the HPLC method on liquid chromatograph Thermo Separation Products Spectra SYSTEM (Thermo Fisher Scientific, Waltham, MA, USA) with these chromatographic conditions: mobile phase consisting of 90 % acetonitrile, 10 % water and 22 mmol/L choline; flow rate 1.5 ml/min; temperature 33 °C; UV detection in the wavelength 254 nm; column SUPELCOSIL™ LC-SCX with particle size 5 µm. For implementation of method we used plasma samples of blood donors from transfusion department Faculty Hospital in Pilsen. The group consisted of 15 males and 15 females in the range of age of 39 to 61 years. The group with metabolic syndrome and prediabetes contained 12 males and 8 females in the range of age 36 to 67 years. Results: Values of precision expressed as coefficient of variation were 10.6 % and 11.8 % for betaine and dimethylglycine. Intermediate precisions were 7.1 % and 12.3 %. Recovery of both analytes were 97.8 % for betaine and 97.7 % for dimethylglycine. Plasmatic concentrations of betaine and dimethylglycine in the control group were 135.8 ± 47.4 µmol/L and 4.7 ± 1.3 µmol/L. Plasmatic concentrations in patients with metabolic syndrome and prediabetes were 100.9 ± 40.9 µmol/L (significantly lower than in control group, p < 0.005) and 10.3 ± 6.2 µmol/L (significantly higher than in control group, p < 0.01). Plasma concentration of homocysteine in patients with metabolic syndrome was 14.4 ± 5.0 µmol/L (p < 0.001), in the control group 10.0 ± 3.5 µmol/L. Conclusion: Patients with metabolic syndrome and prediabetes had lower plasma concentration of betaine and higher plasma level of dimethylglycine and homocysteine. These changes could be in relation with the development of non-alcoholic fatty liver disease and increased cardiovascular risk in these patients. [ABSTRACT FROM AUTHOR]

Abstract

NiTi shape-memory alloys may release poisonous Ni ions at the alloys’ surface. In an attempt to prepare a well-performing surface layer on an NiTi sample, the thermally grown TiO2 layer, which formed during the heat treatment of NiTi, was removed and replaced with a new TiO2 layer prepared using the atomic layer deposition (ALD) method. Using x-ray photoelectron spectroscopy, it was found that the ALD layer prepared at as low a temperature as 100 °C contained Ti in oxidation states + 4 and + 3. As for static corrosion properties of the ALD-coated NiTi samples, they further improved compared to those covered by thermally grown oxide. The corrosion rate of samples with thermally grown oxide was 1.05 × 10−5 mm/year, whereas the corrosion rate of the ALD-coated samples turned out to be about five times lower. However, cracking of the ALD coating occurred at about 1.5% strain during the superelastic mechanical loading in tension taking place via the propagation of a localized martensite band. Abstract

Calcific aortic stenosis (AS) is an atherosclerosis-related process and the most common cause of valve disease requiring surgery.To assess the association of inflammatory markers with AS in advanced atherosclerosis.Consecutive patients with coronary artery disease (CAD) associated with AS were prospectively identified (mean transvalvular aortic gradient of 30 mmHg or greater). Subjects with aortic sclerosis (mean transvalvular aortic gradient of 10 mmHg or less) served as controls. All patients underwent clinical evaluation, echocardiography and coronary angiography.One hundred twenty-two patients with AS (85 men) and 101 with aortic sclerosis (76 men) of similar CAD severity were enrolled. The AS patients were older (mean [+/- SD] 71+/-7 years versus 66+/-7 years; P0.001), had higher soluble vascular adhesion molecule-1 (s-VCAM-1) levels (1533+/-650 mug/L versus 1157+/-507 mug/L; P0.001), but lower soluble intercellular adhesion molecule-1 (s-ICAM-1) (254+/-81 mug/L versus 293+/-84 mug/L; P0.01) and soluble E-selectin (53+/-28 mug/L versus 62+/-29 mug/L; P0.05) levels. The two groups did not differ with respect to C-reactive protein level (3+/-2.9 mg/L versus 3.4+/-2.6 mg/L; P not significant). Higher s-VCAM-1 (OR 1.09, 95% CI 1.04 to 1.14; P0.001) and lower s-ICAM-1 (OR 0.82, 95% CI 0.72 to 0.94; P0.001) levels were associated with AS after adjustment for age.Increased s-VCAM-1 levels were associated with calcific AS in patients with significant CAD.

Abstract

To date, peritoneal dialysis has been performed almost exclusively using dialysis solutions containing glucose as the osmotic agent. Use of these solutions is fraught with problems regarding adequate fluid removal from the body and is also associated with undesirable metabolic effects; hence the search for alternative osmotic agents. A dialysis solution with the glucose polymer icodextrin generates ultrafiltration on the principle of colloidal osmosis. The aim of the study was to establish the effect of icodextrin-base dialysis solution on the magnitude of ultrafiltration and evaluate selected metabolic parameters of patients treated by ambulatory peritoneal dialysis.A total of 9 patients whose glucose-based solution was replaced by an icodextrin-based solution during the night-time exchange were evaluated. A control group of 9 patients used glucose-solution during all exchanges. Night-time bag ultrafiltration, blood pressure, and the serum levels of lipids, insulin, leptin, maltose, and amylase were determined before icodextrin administration (time 0), at one-month intervals (time 1, 2, 3), and one month after study completion (time 4). In icodextrin-treated patients, ultrafiltration rose from 246.5 +/- 60.5 ml (mean +/- SEM) at time 0 to 593.1 +/- 87.4 ml; p0.01, at time 1, to 547 +/- 67 ml; p0.05, at time 2, and to 586.7 +/- 58.8 ml; p0.01, at time 3, the icodextrin administration led to a rise in maltose from 0.02 +/- 0.01 g/l at time 0 to 0.1 +/- 0.1 g/l; p0.01, at time 1, to 1.0 +/- 0.09 g/l; p0.01, at time 2, and to 1.1 +/- 0.09 g/l; p0.01, at time 3, with a fall to zero values at time 4 (NS). Icodextrin administration was followed by a decrease in leptinemia from 34.6 +/- 17.2 ng/ml at time 0 to 21.7 +/- 8.9 ng/ml; p0.05, at time 1, to 21.4 +/- 9.5 ng/ml; p0.05, at time 2, and to 15.9 +/- 24.1 ng/ml; p0.05 at time 4. Insulin and lipid levels were not affected. There was no change in the above parameters in the control group. Icodextrin-treated patients reduced their antihypertensive medication, but not statistically significantly.Icodextrin administration significantly increase ultrafiltration thus providing for effective control of hydration status without the need for high-level glucose-based dialysis solutions. The use of a glucose polymer-based dialysis solution is associated with a significant yet reversible rise in serum maltose. The decrease in leptin may signal a reduction in body weight after replacing glucose in dialysis solutions with icodextrin, or enhanced rates of leptin elimination as a result of ultrafiltration-induced convective transport.
