Abstract Understanding the long-term trends in temperature is crucial for assessing the impacts of climate change on agriculture, human and animal health, and natural habitats. An in-depth analysis of long-term spatio-temporal trends of monthly maximum and minimum temperatures along with Diurnal Temperature Range (DTR) from 1951 to 2022 in various Agro-Climatic Zones (ACZs) of India was performed using newly introduced Innovative Polygonal Trend Analysis (IPTA) and Innovative Trend Analysis (ITA) along with traditional Mann-Kendall (MK) or modified Mann-Kendall (m-MK) tests. The MK/m-MK and IPTA assessments detected significant trends in 44.6% and 86% of the time series data, respectively, while the ITA method detected significant trends in every time series data. The southern, western, and central parts of India are consistently experiencing rising monthly maximum and minimum temperatures. The western dry region, Central Plateau & Hills, East Coast Plains & Hills, Gujarat Plains & Hills, West Coast Plains & Hills, Southern Plateau & Hills, Western Plateau & Hills and Eastern Himalayan Region predominantly show increasing trends in both maximum and minimum temperatures in all months. The Indo-Gangetic Plains (Middle, Trans, Upper and Lower) exhibit decreasing trends in maximum temperatures during January, February, May, and June. The DTR showed increasing trends in the northern and western parts, while it has increasing trends in the central and southern parts of the country. The results of this study provide valuable information to assist in precise and informed decision-making on agricultural resource management and socio-economic policies.

In an increasingly complex and dynamic era, the capabilities of naval armaments become crucial aspects in ensuring a country's maritime sovereignty. State-owned Enterprises (SOEs) have significant potential to act as drivers in strengthening the capabilities of naval armaments. This paper aims to delve deeper into the role and contribution of SOEs in the development of the defense industry, particularly in the context of strengthening the capabilities of naval armaments. This approach includes conceptual analysis and case studies that allow researchers to explore various aspects of policies and practices related to SOE development in the defense industry. The experts were interviewed with a total of 12 expert panels. Several important findings resulting from this analysis reinforce awareness of the crucial role of SOEs in supporting maritime defense while highlighting the potential of specific strategies in achieving the goals of strengthening naval armaments, such as Variation of Strategies Adopted by SOEs in Developing the Defense Industry, consists of Product Diversification and Technological Innovation; Collaboration with Strategic Partners; Operational Efficiency Improvement. Besides that, the success of SOE development policies in the defense industry encompasses coordinated inter-agency cooperation, investment in research and development (R&D), and supportive regulations. Further improvement and development steps in the maritime defense industry include enhancing synergy with the government through the establishment of special teams and discussion forums, boosting R&D investment through fiscal incentive programs and the establishment of innovation centers, as well as improving responsive regulations through the formation of regulatory committees and independent audits.

Abstract In an era marked by growing global population and climate variability, ensuring food security has become a paramount concern. Rice, being a staple crop for billions of people, requires accurate and timely yield prediction to ensure global food security. This study was undertaken across two rice crop seasons in the Udham Singh Nagar district of Uttarakhand state to predict rice yield at 45, 60 and 90 days after transplanting (DAT) through machine learning (ML) models, utilizing a combination of optical and Synthetic Aperture Radar (SAR) data in conjunction with crop biophysical parameters. Results revealed that the ML models were able to provide relatively accurate early yield estimates. For summer rice, eXtreme gradient boosting (XGB) was the best-performing model at all three stages (45, 60, and 90 DAT), while for kharif rice, the best-performing models at 45, 60, and 90 DAT were XGB, Neural network (NNET), and Cubist, respectively. The combined ranking of ML models showed that prediction accuracy improved as the prediction date approaches harvest, and the best prediction of yield was observed at 90 DAT for both summer and kharif rice. Overall rankings indicate that for summer rice, the top three models were XGB, NNET, and Support vector regression, while for kharif rice, these were Cubist, NNET, and Random Forest, respectively. The findings of this study offer valuable insights into the potential of the combined use of remote sensing and biophysical parameters using ML models, which enhances food security planning and resource management by enabling more informed decision-making by stakeholders such as farmers, policy planners as well as researchers.

Objetivos: Dentre as manifestações clínicas da anemia falciforme (AF), a crise vaso-oclusiva (CVO) é a principal causa de internações hospitalares. Além disso, episódios de infarto e necrose tecidual, podem ocasionar a osteonecrose, uma complicação crônica da AF na superfície articular óssea, degradando-a. Na fisiopatologia da AF, a P-selectina, glicoproteína codificada pelo gene SELP, participa do processo vaso-oclusivo, atuando na adesão de leucócitos, plaquetas e eritrócitos falcizados ao endotélio. Essas interações celulares resultam na quebra e liberação da forma solúvel da P-selectina (sP-selectina). Alguns polimorfismos de nucleotídeo único (SNPs) no gene SELP estão associados com a modulação dos níveis plasmáticos da sP-selectina e, portanto, podem influenciar na susceptibilidade a doenças. Dessa forma, o objetivo deste trabalho foi investigar a associação dos níveis plasmáticos da sP-selectina com polimorfismos no gene SELP e complicações clínicas de pacientes com AF acompanhados no hospital de hematologia da Fundação HEMOPE. Métodos: Foram selecionados 75 indivíduos com AF acima de 18 anos de ambos os sexos, que não estivessem em regime de hipertransfusão e/ou em uso de hidroxiuréia no momento da coleta. Para avaliar a dosagem de sP-selectina, subdividiu os pacientes em três grupos: 17 pacientes com AF durante o episódio de CVO; 20 pacientes com AF com osteonecrose e 38 com AF sem a ocorrência de eventos agudos clínicos por pelo menos 3 meses anteriores à coleta, que constituíram o grupo controle. Foi feita a genotipagem dos SNPs G1057A Ser290Asn (rs6131), G1980T Leu599Val (rs6133) e -1969G/A (rs1800805) por meio da técnica de PCR em tempo real. Resultados: Foi observado que os indivíduos do grupo CVO possuíam maiores níveis de sP-selectina (mediana: 155,7 ng/mL, intervalo: 115,9 ng/mL – 271,0 ng/mL), quando comparado ao grupo controle (mediana: 119,8 ng/mL, intervalo: 54,13 ng/mL – 167,3 ng/mL) (p < 0,0001). Com relação a osteonecrose (mediana: 98,09 ng/mL, intervalo: 65,90 ng/mL – 199,1 ng/mL), não foi observada diferença dos níveis de sP-selectina quando comparado ao grupo controle (p = 0,159). Por fim, ao analisar os níveis de sP-selectina de acordo com genótipos dos polimorfismos de SELP no grupo total, dentro dos grupos de cada complicação clínica, não foi observada diferença estatística para nenhuma variante. Discussão: A P-selectina é uma importante molécula para desencadeamento da vaso-oclusão, pois medeia o rolamento e adesão dos leucócitos e hemácias falcizadas à superfície do vaso, além de favorecer a formação de agregados entre as plaquetas e os neutrófilos. A importância da P-selectina para o processo vaso-oclusivo foi também demostrada pela eficácia do crizanlizumabe, um anticorpo monoclonal anti-P-selectina, na redução das taxas de CVO em pacientes com AF. Além disso, apesar de haver uma relação com as variantes do SELP com a dosagem da sP-selectina com os diferentes polimorfismos não foi encontrada na população estudada. Conclusão: Portanto, nossos resultados confirmam que a dosagem da sP-selectina em pacientes falcêmicos é um fator relevante para o desenvolvimento da CVO, sendo um possível critério para avaliação prognóstica e melhor acompanhamento dos pacientes.

Objetivos: A Anemia Falciforme (AF) é caracterizada principalmente por episódios de dor devido às crises vaso-oclusivas (CVOs) que levam a complicações como o AVC, além da síndrome torácica aguda (STA) e osteonecrose. O gene SELP codifica a P-selectina, uma proteína responsável pela adesão celular, mediando a interação entre células endoteliais, plaquetas e leucócitos. Polimorfismos neste gene podem influenciar a expressão e função da P-selectina, afetando a severidade e a frequência das CVOs e as outras complicações da AF. Assim o objetivo deste trabalho foi verificar a existência de associação entre as frequências alélicas e genotípicas dos polimorfismos de SELP com as manifestações clínicas em pacientes adultos com anemia falciforme. Métodos: Este estudo foi realizado com 372 pacientes com AF acompanhados no HEMOPE, Recife-PE. Os dados clínicos dos pacientes foram coletados a partir de prontuários médicos. Foram selecionados três polimorfismos de nucleotídeos únicos (SNPs) do gene SELP : rs6131, rs6133 e rs1800805, que foram genotipados através da técnica de PCR em tempo real. Resultados: Para o polimorfismo rs6133 ao aplicar o modelo de associação codominante, observamos que indivíduos com genótipo homozigoto selvagem GG apresentavam menor frequência de síndrome torácica aguda, seguido dos pacientes com genótipo variante TT e genótipo heterozigoto GT com maior frequência da complicação (P = 0,025). No entanto, os outros dois SNPs, rs6131 e rs1800805, não mostraram associação significativa com as complicações clínicas investigadas, como CVO e osteonecrose. Discussão: Estudos anteriores associaram os polimorfismos no gene SELP com o aumento de sP-selectina, fator que contribui para o aumento na ocorrência de doenças cardiovasculares e trombóticas. Neste trabalho, demonstramos pela primeira vez a associação do SNP de SELP rs6133 com a ocorrência de STA em pacientes com AF. A P-selectina tem sido relacionada com a inflamação induzida pelo grupo heme livre, favorecendo o dano pulmonar agudo na AF. Além disso, já foi observado que a P-selectina estava constitutivamente mais expressa nos pulmões de camundongos HbSS quando comparado a animais sem Hb SS, sugerindo papel desta molécula em complicações pulmonares como a STA. Conclusão: Portanto, o polimorfismo rs6133 no gene SELP está associado a um risco maior de desenvolvimento de STA em pacientes com AF, podendo ser um importante marcador genético na modulação do quadro clínico da doença. Essa associação ilustra a necessidade de investigações futuras avaliando a associação dos polimorfismos de SELP em haplótipos e a relação do rs6133 com os níveis de sP- selectina, a fim de melhor compreender seu papel na fisiopatologia da STA.

Introduction: DLBCL commonly affects the older population. Elderly patients often present high-risk molecular profile, lower tolerability to chemotherapy, and poor outcomes. In this setting, attenuated therapeutic regimens, such as R-miniCHOP and R-miniCHOP of the elderly, have emerged for this fragile population. Albeit its relevance, response rates, clinical outcomes, and toxicities of these regimens remain poorly understood, because these individuals are usually excluded from controlled trials. Therefore, this study aims to assess the outcomes, determine survival predictors, and compare responses and toxicities between R-CHOP, R-miniCHOP, and R-miniCHOP of the elderly protocols in the management of elderly DLBCL patients. Methods: This retrospective, observational, and single-center study involved 185 DLBCL patients older than 70 years, treated at USP, from 2009 to 2020. Endpoints included ORR, OS, PFS. Survival curves were constructed using the KM method and the Log-Rank test was used to assess the relationship between variables and outcomes. The chi-square test and the Kruskal-Wallis test were applied to assess statistically significant differences in clinical characteristics, adverse event profile, and responses between different treatment modalities. Univariate analysis was performed using the Cox test and multivariate analysis by Cox regression method. The results were presented in HR and 95% CI, and a p-value ≤0.05 was considered significant. Results: The median age at diagnosis was 75 years, and 58.9% were female. Comorbidities were prevalent, including 19.5% with immobility, 28.1% with malnutrition and 24.8% with polypharmacy. Advanced clinical stage (III/IV) was observed in 72.4%, 48.6% had bulky ≥ 7 cm, 63.2% had B-symptoms, and 67.0% presented IPI ≥ 3. Among the 182 (98.4%) effectively treated cases, 57.1% received R-CHOP, 18.0% R-miniCHOP, 13.2% R-miniCHOP of the elderly, and 1.7% were palliated using R-CVP. The ORR for the whole cohort was 68.1%, with CR achieved in 65.9%. ORR was 72.1% for R-CHOP, 70.6% for R-miniCHOP, and 45.6% for R-miniCHOP of the elderly, p = 0.040. Although R-miniCHOP of the elderly regimen promoted lower ORR, patients in this group had higher rates of unfavorable clinical-laboratory findings, including hypoalbuminemia (p = 0.001), IPI ≥ 3 (p = 0.013), and NCCN-IPI ≥ 3 (p = 0.002). With a median follow-up of 6.3 years, the estimated 5-year OS and PFS were 50.2% and 44.6%, respectively. The estimated 2-year OS was 82.0% for R-CHOP, 67.5% for R-miniCHOP, and 48.1% for R-miniCHOP of the elderly, p = 0.003. The estimated 2-year PFS was 61.1% for R-CHOP, 56.4% for R-miniCHOP, and 20.5% for R-miniCHOP of the elderly, p = 0.005. Although correlated with increased OS and PFS in comparison to attenuated protocols, R-CHOP regimen was associated with higher rates of severe neutropenia (p = 0.003), but not translated into febrile neutropenia (p = 0.907), therapy interruption (p = 0.671), or higher early mortality rates (p = 0.681). In multivariate analysis, age≥75 years (HR: 2.08, p = 0.001), neutrophilia (HR: 2.18, p = 0.007), low lymphocyte/monocyte ratio (HR: 1.98, p = 0.010), and clinical stage III/IV (HR: 2.36, p = 0.003) were predictors of decreased OS. Conclusion: In this large and real-life LATAM cohort, we demonstrated that DLBCL patients older than 70 years still do not have satisfactory outcomes, with half of cases not reaching 5 years of life expectancy after diagnosis. Although a significant portion of older DLBCL patients is highly fragile and ineligible for enhanced regimens, attenuated anthracycle-based protocols promoted remarkably inferior outcomes compared to those achieved by the R-CHOP.

Introduction: Chronic myeloid leukemia (CML) is a myeloproliferative neoplasm characterized by the presence of a BCR-ABL1 rearrangement. Its detection and quantification by real-time quantitative PCR (RQ-PCR) play a central role in CML diagnosis, therapy monitoring, and sequencing to identify resistance mutations in case of therapy failure. As previously demonstrated (Pugliesi et al., Hematol Transfus Cell Ther. 2023;45 Suppl 4), combining magnetic RNA extraction with the use of only 2mL of peripheral blood (PB) provides a fast, cost-effective, and highly sensitive strategy for CML diagnosis and monitoring. Objectives: To optimize the existing method by integrating magnetic RNA extraction with multiplex real-time PCR amplification for both the BCR (internal control) and BCR::ABL1 p210 transcript within the same PCR reaction. This enhancement aims to increase scalability and traceability while reducing costs and turnaround time (TAT). Materials and methods: Total RNA was extracted from 100 PB samples. After erythrocyte lysis, leukocytes from 2mL of PB were automatically extracted using Extracta-MPTA (Loccus do Brasil). To validate a multiplex assay combining the BCR gene and the target BCR::ABL1, the reference standard curve ERM-AD263 was used to calibrate the assay, and a commercial reference RNA was used to set the International Scale (IS%). BCR and BCR::ABL1 were measured using previously validated singleplex assays and by multiplex (BCR and BCR::ABL1 in the same reaction). Multiplex assay accuracy was measured by comparing it to the previously validated singleplex real-time PCR for BCR and BCR::ABL1 copy number and BCR::ABL1 ratio (IS%) using 100 CML samples (paired t-test). Precision was determined using internal quality control (IQC) (BCR::ABL1: 1%) in duplicates in 20 independent assays. Results: The singleplex and multiplex five-point standard curves for BCR and BCR::ABL1 were completely superposed (Spearman correlation, p = 0.003). Comparisons in 100 samples revealed that BCR and BCR::ABL1 measurements were similar (paired t-test, p = 0.42 and 0.07, respectively) and that BCR::ABL1 IS% values were identical (p = 0.18). Pairing was significantly effective for all determinations (p < 0.0001). The total coefficient of variation for internal quality control (IQC) was 25%, with a standard deviation of 0.36. Utilizing the multiplex test reduced the turnaround time (TAT) by 20%, allowing results to be released within 24 hours of sample reception. Discussion: As demonstrated, the combination of automated magnetic RNA extraction from 2mL of PB with the quantification of the BCR::ABL1 gene for patients under CML monitoring is an effective, fast, safe, and low-cost method. Conclusion: The integration of automated magnetic RNA extraction from 2mL of peripheral blood with multiplex real-time PCR for BCR::ABL1 quantification provides a robust, efficient, and economical approach for monitoring CML patients. This optimized method significantly reduces turnaround time, enabling results to be available within 24 hours of sample reception. Such a strategy is particularly advantageous for public healthcare systems, such as the Brazilian Unified Health System (SUS), enhancing accessibility and ensuring timely and accurate monitoring of CML patients. The streamlined process supports scalable and traceable diagnostics, ultimately contributing to improved patient management and outcomes.

Objetivo: Descrever as tendências no diagnóstico de síndrome mielodisplásica no Brasil entre o período de 2014 e 2023 e sua associação com variáveis demográficas, sexo e faixa etária. Metodologia: Refere-se a um estudo documental quantitativo, retrospectivo e descritivo, realizado com base em informações referentes aos casos de síndrome mielodisplásica diagnosticados no período de 2019 a 2023, disponíveis no banco de dados do Sistema de Informações Ambulatoriais (SIA), no banco de informações em saúde TABNET (DATASUS). Nesta pesquisa, foram utilizadas as seguintes variáveis: sexo e faixa etária, fazendo uma comparação entre regiões do país e casos por ano. Resultados: Houve um aumento de 117,8% nos diagnósticos de Síndrome Mielodisplásica (SMD) no Brasil, de 450 casos em 2014 para 980 casos em 2023. Em se tratando da distribuição por sexo, em 2023, 54,1% dos casos diagnosticados eram homens (530 casos) e 45,9% eram mulheres (450 casos). Já na análise da faixa etária, nota-se que a maioria dos casos ocorreu em indivíduos acima de 60 anos, que representaram 65% dos diagnósticos em 2023. A faixa etária de 40 a 59 anos teve 25% dos casos, enquanto os indivíduos com menos de 40 anos representaram 10% dos diagnósticos. A região Sudeste apresentou o maior número de casos em relação ao total (40%), enquanto a menor taxa é da região Norte (5%). Discussão: Este estudo epidemiológico apresentou relevancia por fortalecer o direcionamento assertivo aos cuidados de saúde em pessoas com Síndrome Mielodisplásica (SMD), sobretudo a partir da identificação da maior incidência em pessoas do sexo masculino e acima de 60 anos. Aproximadamente 65% dos pacientes com SMD têm mais de 60 anos de idade no momento do diagnóstico e a taxa de incidência dobra a cada década acima dos 40 anos de idade. Apresentando um curso clínico extremamente variável, ela se enquadra em um espectro que varia desde um curso estável da doença ao longo de 10 ou mais anos, até evolução para morte, com evolução acelerada entre alguns meses subsequente à complicações de citopenia ou transformação leucêmica. Além disso, o fato de ter um aumento no número de casos entre 2014 e 2023, sendo a maior quantidade de pacientes no Sudeste, demonstra que a SMD é uma doença que necessita de estratégias de prevenção e diagnóstico precoce. Conclusão: Diferenças étnicas e influências regionais desempenham um papel significativo na patogênese e estabelecimento da Síndrome Mielodisplásica. Sendo assim, fatores genéticos, fatores étnicos, ocupacionais, de estilo de vida e ambientais contribuem para a história natural e desenvolvimento da síndrome em questão. A compreensão desses aspectos é crucial para o desenvolvimento de estratégias eficazes de prevenção, diagnóstico precoce e manejo da SMD, visando melhorar a qualidade de vida dos pacientes e otimizar os recursos de saúde.

A transfusão de hemocomponentes, enquanto vital para salvar vidas, apresenta riscos e custos que precisam ser cuidadosamente considerados. Embora existam diversos estudos que comprovam a segurança de estratégias transfusionais mais restritivas, a prática ainda varia significativamente entre médicos da mesma especialidade e instituição. Nesse cenário, a educação médica é essencial para assegurar que os profissionais de saúde estejam bem informados sobre quando e como realizar transfusões, os tipos de hemocomponentes a serem utilizados, e como evitar complicações. Além disso, é importante que eles compreendam as questões éticas e legais associadas ao uso de sangue, garantindo transfusões seguras e eficazes, reduzindo o risco de problemas legais e custos hospitalares. A educação médica enfrenta desafios, especialmente devido às longas jornadas de trabalho que limitam o tempo disponível para estudo convencional. Em contrapartida, o uso de smartphones é comum entre os médicos, com o WhatsApp® sendo uma ferramenta popular. Com isso em mente, foi implementada uma intervenção na unidade hospitalar utilizando guidelines transfusionais no formato Pocket, de fácil disseminação e acesso via WhatsApp®, utilizando triggers transfusionais. A metodologia envolveu a criação de um guideline transfusional baseado no Manual de Uso de Hemocomponentes da instituição, resumido em formato visual de página única, com um arquivo específico para cada hemocomponente, facilitando o acesso dos colaboradores pelo WhatsApp®. Como ferramenta de mensuração, a equipe de laboratório sinalizava ao responsável técnico quando surgia uma requisição de transfusão possivelmente fora do protocolo. Esse responsável então contatava o prescritor e registrava todas as intervenções feitas, anotando se houve redução na necessidade transfusional. Caso não fosse possível contatar o médico, a ocorrência era registrada, mas não contabilizada como intervenção. Após alguns dias, o prontuário do paciente era revisado para verificar se a transfusão havia sido realizada até 72 horas depois. Os resultados mostraram que, antes da intervenção, em abril de 2021, foram identificadas 46 requisições possivelmente fora do protocolo, com 43 intervenções resultando na solicitação de 58 bolsas, levando a uma redução de 60,5% nas transfusões (26 bolsas em 23 pacientes). Após 72 horas, 73,1% das bolsas não foram transfundidas. Em maio de 2021, durante o período educacional, houve uma redução para 32 intervenções solicitando 56 bolsas, com uma redução de 44,6% nas transfusões (25 bolsas em 20 pacientes), e 68% dessas bolsas permanecendo não transfundidas após 72 horas. Em junho de 2021, após o período educacional, houve 22 intervenções solicitando 45 bolsas, com uma redução de 22,2% nas transfusões (10 bolsas), e 4 dessas bolsas permanecendo não transfundidas após 72 horas. Nos meses seguintes, a adesão ao protocolo melhorou significativamente, com uma média mensal de apenas 4 intervenções em 2022. O uso do WhatsApp® mostrou ser um meio eficaz e conveniente para a educação médica, permitindo fácil acesso a ferramentas de consulta rápidas. A implementação dessa estratégia resultou em excelente adesão ao protocolo transfusional, promovendo uma estratégia mais restritiva, sem prejuízo aos pacientes, e contribuindo para a redução de custos no serviço público.

We examined the antibacterial efficacy of streptomycin, hibiscus acid, and their combination against multidrug-resistant Shiga-toxin-producing Escherichia coli (STEC) and Salmonella Typhimurium in mice. We determined the minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) for streptomycin, hibiscus acid, and their combination against STEC and Salmonella. Fifteen sets of six mice in each set were utilised: six groups were orally exposed to 4 log10 colony forming units (CFUs) of S. Typhimurium and another six to STEC, and three acted as the controls. Six hours post-inoculation, specific groups of mice received either oral solutions containing hibiscus acid at 5 and 7 mg/ml; streptomycin at 50 and 450 µg/ml; hibiscus acid/streptomycin (5 mg/ml hibiscus acid and 50 µg/ml streptomycin); or isotonic saline. The study determined the MIC and MBC of 7 mg/ml of hibiscus acid; 300 and 450 µg/ml of streptomycin; and two concentrations of hibiscus/streptomycin (3 mg/ml / 20 µg/ml and 5 mg/ml / 50 µg/ml). Interestingly, the mice that were infected and subsequently treated with hibiscus acid at 7 mg/ml alone or in conjunction with streptomycin did not have either STEC or Salmonella in their faecal samples, and none of the mice died. In contrast, the untreated mice and those exclusively treated with streptomycin had the pathogens present in their stool, leading to the mortality of all the subjects.

Abstract Background Observational studies and randomized controlled trials have found evidence that higher maternal circulating cortisol levels in pregnancy are associated with lower offspring birth weight. However, it is possible that the observational associations are due to residual confounding. Methods We performed two-sample Mendelian Randomisation (MR) using a single genetic variant (rs9989237) associated with morning plasma cortisol (GWAS; sample 1; N = 25,314). The association between this maternal genetic variant and offspring birth weight, adjusted for fetal genotype, was obtained from the published EGG Consortium and UK Biobank meta-analysis (GWAS; sample 2; N = up to 406,063) and a Wald ratio was used to estimate the causal effect. We also performed an alternative analysis using all GWAS reported cortisol variants that takes account of linkage disequilibrium. We also tested the genetic variant’s effect on pregnancy cortisol and performed PheWas to search for potential pleiotropic effects. Results The estimated effect of maternal circulating cortisol on birth weight was a 50 gram (95% CI, -109 to 10) lower birth weight per 1 SD higher log-transformed maternal circulating cortisol levels, using a single variant. The alternative analysis gave similar results (-33 grams (95% CI, -77 to 11)). The effect of the cortisol variant on pregnancy cortisol was 2-fold weaker than in the original GWAS, and evidence was found of pleiotropy. Conclusions Our findings provide some evidence that higher maternal morning plasma cortisol causes lower birth weight. Identification of more independent genetic instruments for morning plasma cortisol are necessary to explore the potential bias identified.

Abstract Evaluating crop health and forecasting yields in the early stages are crucial for effective crop and market management during periods of biotic stress for both farmers and policymakers. Field experiments were conducted during 2017–18 and 2018–19 with objective to evaluate the effect of yellow rust on various biophysical parameters of 24 wheat cultivars, with varying levels of resistance to yellow rust and to develop machine learning (ML) models with improved accuracy for predicting yield by integrating thermal and RGB indices with crucial plant biophysical parameters. Results revealed that as the level of rust increased, so did the canopy temperature and there was a significant decrease in crop photosynthesis, transpiration, stomatal conductance, leaf area index, membrane stability index, relative leaf water content, and normalized difference vegetation index due to rust, and the reductions were directly correlated with levels of rust severity. The yield reduction in moderate resistant, low resistant and susceptible cultivars as compared to resistant cultivars, varied from 15.9–16.9%, 28.6–34.4% and 59–61.1%, respectively. The ML models were able to provide relatively accurate early yield estimates, with the accuracy increasing as the harvest approached. The yield prediction performance of the different ML models varied with the stage of the crop growth. Based on the validation output of different ML models, Cubist, PLS, and SpikeSlab models were found to be effective in predicting the wheat yield at an early stage (55-60 days after sowing) of crop growth. The KNN, Cubist, SLR, RF, SpikeSlab, XGB, GPR and PLS models were proved to be more useful in predicting the crop yield at the middle stage (70 days after sowing) of the crop, while RF, SpikeSlab, KNN, Cubist, ELNET, GPR, SLR, XGB and MARS models were found good to predict the crop yield at late stage (80 days after sowing). The study quantified the impact of different levels of rust severity on crop biophysical parameters and demonstrated the usefulness of remote sensing and biophysical parameters data integration using machine-learning models for early yield prediction under biotically stressed conditions.

Acute Necrotising Encephalopathy of Childhood (ANEC) is a fatal disease diagnosed on basis of clinical and typical Magnetic Resonance Imaging (MRI) findings. The prognosis of this encephalopathy is generally poor, with fewer than 10% of cases recovering completely. Patients with a favourable outcome had reversible neuroimaging findings. Long-term sequelae commonly include neurologic deficits such as spasticity, epilepsy, and involuntary movements. In this case series, five cases of ANEC were reported. The children included in this series ranged in age from nine months to 16 years. All the children were female, and their presenting symptoms included seizures (n=5), altered sensorium (n=5), features of raised intracranial pressure (n=5), and posturing (n=1). Neuroimaging revealed characteristic lesions in the thalamus with varied involvement of other parts of the brain in all cases (n=5). Cerebrospinal Fluid (CSF) analysis was normal in the majority of children. Four out of five cases survived and responded to early steroids, intravenous immunoglobulin, physiotherapy, and rehabilitation. One child had complete recovery, with three children experiencing disabilities and one child succumbing to the disease.

Abstract Background Physician decision-making skills training is a priority to improve adoption of the cerebral palsy (CP) clinical guideline and, through this, lower the age of CP diagnosis. Clinical guideline implementation aims to improve physician practice, but evaluating meaningful change is complex. Limitations in the validity evidence of evaluation instruments impact the evidence base. Validity frameworks, such as Kane’s, enable a targeted process to gather evidence for instrument scores, congruent to context and purpose. Yet, application of argument-based methodology to implementation validation is rare. Key-features examination methodology has established validity evidence supporting its use to measure decision-making skills, with potential to predict performance. We aimed to apply Kane’s framework to evaluate a pilot key-features examination on physician decision-making in early CP diagnosis. Methods Following Kane’s framework, we evaluated evidence across inferences of scoring, generalisation, extrapolation and implications in a study design describing the development and pilot of a CP diagnosis key-features examination for practising physicians. If found to be valid, we proposed to use the key-feature scores as an outcome measure of decision-making post education intervention to expedite CP diagnosis and to correlate with real-world performance data to predict physician practice. Results Supporting evidence for acceptance of scoring inferences was achieved through examination development with an expert group (n = 10) and pilot results (n = 10): (1) high internal consistency (0.82); (2) acceptable mean item-discrimination (0.34); and (3) acceptable reliability of examination scorers (95.2% congruence). Decreased physician acceptance of examination time (70%) was identified as a threat and prioritised in case reduction processes. Partial acceptance of generalisation, extrapolation and implications inferences were defensible with: (1) accumulated development evidence following established key-features methodology; (2) high pilot acceptance for authenticity (90%); and (3) plausibility of assumptions of score correlation with population register data. Conclusions Kane’s approach is beneficial for prioritising sources of validity evidence alongside the iterative development of a key-features examination in the CP field. The validity argument supports scoring assumptions and use of scores as an outcome measure of physician decision-making for CP guideline education implementation interventions. Scoring evidence provides the foundation to direct future studies exploring association of key-feature scores with real-world performance.

Introduction The empty pelvis syndrome is a significant source of morbidity following pelvic exenteration surgery. It remains poorly defined with research in this field being heterogeneous and of low quality. Furthermore, there has been minimal engagement with patient representatives following pelvic exenteration with respect to the empty pelvic syndrome. ‘PelvEx—Beating the empty pelvis syndrome’ aims to engage both patient representatives and healthcare professionals to achieve an international consensus on a core outcome set, pathophysiology and mitigation of the empty pelvis syndrome.Methods and analysis A modified-Delphi approach will be followed with a three-stage study design. First, statements will be longlisted using a recent systematic review, healthcare professional event, patient and public engagement, and Delphi piloting. Second, statements will be shortlisted using up to three rounds of online modified Delphi. Third, statements will be confirmed and instruments for measurable statements selected using a virtual patient-representative consensus meeting, and finally a face-to-face healthcare professional consensus meeting.Ethics and dissemination The University of Southampton Faculty of Medicine ethics committee has approved this protocol, which is registered as a study with the Core Outcome Measures in Effectiveness Trials Initiative. Publication of this study will increase the potential for comparative research to further understanding and prevent the empty pelvis syndrome.Trial registration number NCT05683795.

Reliable and quick estimation of wheat yellow rust (WYR) severity in field is essential to manage the disease and minimize the losses. Field experiments were conducted during 2017–18 and 2018–19 to obtain visible and thermal images of 24 wheat cultivars having different levels of WYR resistance at critical growth stages. Machine learning (ML) models were constructed using the combinations of image indices (IN) and partial least square regression (PLS) scores of image indices with disease severity (DS) and Yeo-Johnson (YJ) transformed values of disease severity. The results revealed that 26 visible and 2 thermal indices considered in this study have significant correlations with WYR. The models performances were evaluated using four possible dataset combinations of (1) disease severity + indices, (2) disease severity + PLS scores of indices, (3) YJ transformed disease severity + indices, and (4) YJ transformed disease severity + PLS scores. Disease severity with image derived indices was found to be the best dataset for the prediction of WYR severity using machine learning models with an R2 and d-index above 0.95 during calibration, while up to 0.67 and 0.87, respectively during validation. Cubist model with disease severity + indices dataset was the best to predict WYR severity, while the Gaussian process regression with YJ transformed disease severity + PLS scores dataset was the poorest predictor. The results obtained in the present study showed the potential of ML models for non-destructive prediction of WYR in field using visible and thermal imaging.

Objetivo: Identificar novos tratamentos para pacientes com anemia falciforme. Material e métodos: Trata-se de uma revisão literária, no qual, para guiar a revisão formulou-se a questão condutora: “Quais as atualizações em ensaios clínicos de Anemia Falciforme?”. Coletaram-se os dados entre janeiro de 2018 a julho de 2023, na base de dados PUBMED. Os critérios utilizados foram: artigos publicados em inglês e português, disponíveis na íntegra. Elencaram-se os (DeCs) e MeSH Os descritores utilizados foram: Voxelotor, Crizanlizumabe, Inclacumabe, Terapia Gênica, Anemia Falciforme. Excluíram-se artigos de revisão sistemática ou integrativa, teses, dissertações e trabalhos completos que não possibilitavam acesso on-line. Resultados: Após Aplicação dos filtros, foram encontrados 52 artigos, dos quais 14 foram excluídos pelo título e 38 foram avaliados, em seguida a leitura aprofundada, obteve-se uma amostra final de 14 artigos. Discussões: A anemia falciforme é a doença monogênica mais comum no mundo, negligenciada por muitos anos. Caracterizada por um quadro hemolítico crônico. Uma mutação na cadeia beta globina, faz com que aconteça uma troca de um ácido nucléico e consequentemente vai ocorrer uma troca de um aminoácido, onde deveria ser um ácido glutâmico, terá uma Valina, isto vai dar origem a hemoglobina S que em situação de hipóxia, desoxigenação, forma polímeros e desta forma modifica o formato das hemácias, levando ao formato de foice. Uma importante molécula de adesão vem sendo bastante estudada, a P-selectina, ela é codificada pelo gene SELP. No processo inflamatório e hipóxia, esta molécula, é liberada para a superfície do endotélio vascular, contribuindo assim para o rolamento dos leucócitos e aderência das células ao endotélio. Nos últimos anos importantes moléculas vêm sendo estudadas, entre elas o Crizanlizumabe (já aprovado pela ANVISA), o Inclacumabe (alguns estudos em andamento), o Voxelotor (aprovado em 2019 pela FDA) e a terapia gênica (com possibilidade de cura e com estudos em andamento). Os estudos demonstram que o Voxelotor é um tratamento via oral com boa tolerância, seguro e eficaz como monoterapia e em combinação com hidroxiureia para crianças a partir de 12 anos de idade. A terapia gênica tem se mostrado uma alternativa ao tratamento em países em desenvolvimento, que possuem alta prevalência de Doença Falciforme, tais como, Brasil, Índia e países da África Sub-Saariana. O medicamento Crizanlizumabe acresce a perspectiva quando usado isoladamente ou em combinação com Hidroxiureia para a prevenção das crises vaso oclusivas. Inclacumabe, ainda não possui registro nas agências FDA e EMA, este promove a diminuição do desenvolvimento de agregados plaquetas-leucócitos mediada pela p-selectina, diminuindo a frequência e gravidade das crises vaso oclusiva. Conclusão: A Doença falciforme apresenta distintas complicações e comprometimento expressivo da qualidade de vida. As terapias modificadoras da doença concebem o início de uma nova era e são na maioria das vezes seguras e eficazes, aprovadas pelas agências FDA e/ou EMA, estão associadas à boa tolerabilidade apesar da incidência de eventos adversos. Seus benefícios clínicos relacionam-se à diminuição de crises de dor, CVOs e aumento dos níveis de hemoglobina.

Introdução: Profissionais de enfermagem que tem na sua rotina assistencial curativos, precisa ser capacitados para cuidar com destreza e segurança, precisando de conhecimentos sobre as diversas tecnologias disponíveis no mercado. O programa de tratamento das coagulopatias no Brasil é centrado no SUS, sob gerência da Coordenação-Geral de Sangue e Hemoderivados do Ministério da Saúde. Objetivo: Relatar o manejo sob a ótica das coberturas especiais, nos distúrbios de coagulação do Fator VII, VIII. Material e métodos: Relatos de caso, em hospital referência em Recife -PE, de junho a julho de 2023, com dois pacientes, apresentando lesões por trauma e infecções. As evoluções cicatriciais, foram por análise nos prontuários e imagens fotográficas. O estudo obedeceu aos aspectos éticos e legais da Resolução N°466/12 do Conselho Nacional de Saúde do Ministério da Saúde. Parecer nº6.194.531, Certificado de Apresentação de Apreciação Ética (CAAE): 71528323.5.0000.519. Resultados: Caso 1: Lesão em membro inferior esquerdo, no dia 04/06/2023. Avaliação, bolhas extensas, arroxeadas, exsudato serossanguinolento, volume moderado, sem odor. Na face externa, próximo a região maleolar, necrose de coagulação. Caso 2:admitida em 09/07/2023, acidente doméstico em membro superior direito com espinha de peixe, lesão, sangrante, associada a lesões bolhosas, equimoses extensa, e edema importante. Avaliada pelo ortopedista e vascular que descartou síndrome compartimental. Suspeita de infecção secundária. Após avaliação das fotografias, foi orientado por telefone iniciar o PHMB, porém a conduta foi descontinuada. Discussão: Logo a clínica do paciente interfere diretamente na lesão, na conduta e no processo de cicatrização. A cobertura polyhexametileno biguanida (PHMB), antimicrobiana, bactérias Gram positivas e negativas, ajuda na redução de carga microbiana, estimula o processo da cicatrização, atenua a dor, absorve exsudatos, reduz o odor. Urgoclean-Ag é um correlato desbridante, fibras absorventes, antimicrobiano com matriz cicatrizante TLC-Ag: Lipídio-Colóide, composta por substâncias lipofílicas e íons de prata (Ag +) revestido por uma matriz de cicatrização micro aderente, entra em contado com o exsudato gelifica e libera esses sais. Favorece o desbridamento eletrostático, absorção do exsudato, controle microbiano e biofilme. Aplicabilidade ferramenta e Acrômio TIME, utilizada para abordar e otimizar a cicatrização do leito de lesões. Para o método de desbridamento, constituiu avaliação do tipo de tecido, materiais biológicos e coleção de exsudato presentes. Imprescindível abordagem com as técnicas de Square e Cover. Conclusão: Desbridamento procedimento para limpeza e preparo do leito da lesão, desloca e remove tecidos inviáveis, diminui a colonização por microorganismos, favorecer a cicatrização e conservar o leito saudável até a reepitelização. O tratamento de lesão é realizado com foco na lesão crônica, não na patologia, pois não há tratamento específico. Escolha da cobertura para a lesão deve atender considerações, como custo/benefício, praticidade em sua troca, minimizar risco de infecção, realizar o desbridamento, se necessário. O uso das coberturas, cientificamente empregado, corrobora sua eficácia, na reparação tecidual, sinais de infecção local com ação antimicrobiana de amplo espectro e ação de limpeza consecutiva dos esfacelos e ação biofilme.

Natural Killer (NK) cells represent a cytotoxic group of cells that play important roles in immune surveillance and antitumor activities. They mediate the death of target cells through activating receptors in a Human Leukocyte Antigen (HLA)-unrestricted manner. NK cells can be obtained from a wide variety of allogeneic sources, opening roads for the development of a well-characterized and off-the-shelf product. As a result, these cells have become an attractive source for cancer immunotherapy. The most recent application of NK cells in immunotherapy is through the generation of CAR-NK cells, which are redirected to specific cancer epitopes. The co-stimulatory region of CARs used to generate CAR-NK cells are primarily derived from those developed to activate T-cells, therefore, it may not be well suitable to properly activate NK cells to make them exert their full cytotoxic potential. In this context, we developed new CAR constructs combining in their stimulatory regions molecules that naturally are involved in NK activation and compared with the commonly used 4-1BB-CD3ζ based CAR, aiming to improve effector functions of CAR-NK cells. It is described the capacity of Tyrosine Kinase Inhibitors (TKI) to improve NK cells cytotoxic function, therefore, in combination with the new CARs, we assessed the treatment of CAR-NK cells with the TKI Dasatinib to enhance CAR-NK effector function in the context of antitumor activity. NK-92 cells were lentivirally transduced with viral particles containing the coding sequences of 4th generation anti-CD19 CARs, named: A (4-1BB-CD3ζ), B, C and D. Then, the CAR+ cells were FACS-sorted based on the same fluorescence intensity in order to generate populations with equal CAR presence on the cell membrane. Next, cytotoxicity, degranulation and cytokine production were assessed in co-culture assays against CD19-expressing Nalm-6 or Raji (leukemia and lymphoma, respectively) cell lines. For Dasatinib treatment, the CAR-NK cells were incubated for 24 hours with 10nM of the inhibitor or DMSO as control and then the cells were washed and co-culture assays were performed against Nalm-6 and Raji cells. Using flow cytometry based cytotoxicity assay, CAR-NK cells demonstrated higher killing capacity against either Nalm-6 and Raji cells in all different effector:target ratios, compared with non-transduced NK-92 cells. This higher cytotoxicity was corroborated by the increase in CD107a detection on the cell surface of CAR-NK cells (degranulation). This robust cell activation resulted in higher production of the pro-inflammatory cytokines IFN-γ and TNF-α compared with non-transduced cells. The effector functions against Nalm-6 performed by the cells carrying the CARs C and D demonstrated to be superior compared with CARs A and B. However, when the target was Raji cells, there were no differences between the constructions. Strikingly, the treatment of the CAR-NK cells with Dasatinib enhanced almost 40% in cell cytotoxicity against Raji cells. Interestingly, considering assays against Nalm-6 cells, only the cells expressing CARs C and D demonstrated enhancement in effector functions, probably due the differences in the activation domains, with an augment of about 50% in cell cytotoxicity, and higher degranulation and cytokine production compared to cells expressing CARs A and B. Taken together, these findings indicate that the use of NK-specific molecules in the CAR stimulatory domain augments the effector function potency of NK-92 cells. Furthermore, the utilization of the Src kinase inhibitor Dasatinib appears to further amplify the antitumor activity of CAR-NK cells in a CAR-specific manner.

Autologous T-cells expressing Chimeric Antigen Receptor (CAR) have shown great efficacy against hematologic malignancies, however, severe side effects limit their clinical use. Natural Killer (NK) cells are an attractive alternative to T-cells as carriers for CARs, since they could be used as an allogeneic treatment, and have no, or minimal, risk for Cytokine Release Syndrome (CRS). CAR-NK cells have recently shown promising effect against leukemia without serious side effects after adoptive transfer. Possible allogeneic application of CAR-NK cells could dramatically reduce the costs of such adoptive cell therapy, nevertheless sufficient ex vivo expansion of NK cells remains challenging. Transgenic expression of cytokines in CAR-NK cells was shown to partially improve pro-survival or proliferative properties (e.g., IL-15), however what is the optimal cytokine for CAR NK cells is not clear to date. Here, we compared 2nd generation CD19-targeted CAR (CAR.19) with a fourth generation CAR.19 coexpressing Il-27 (CAR.19-IL27) in promoting NK-92 cell line proliferation, activation, and cytotoxic activity against B-cell cancers. CAR constructs were inserted into lentiviral vectors and subsequently transduced into the NK-92 cell line. Effects of CAR-NK cells were assessed against CD19-expressing B-cell lines NALM-6 or Raji in vitro and in vivo in a murine model of B-cell neoplasia with Raji cells. Tumor burden was measured by bioluminescence and by the curve of survival. We have demonstrated that CAR.19-IL27 exhibits a superior expansion capacity in comparison to both NK-92wt and CAR.19 cells. Moreover, CAR.19-IL27 cells showed higher cytotoxicity towards NALM-6 and Raji compared to CAR.19 in vitro (**p < 0.001). Degranulation assay, assed by CD107a, showed that CAR.19-IL27 have higher expression of CD107a compared to CAR.19 cells (**p < 0.001), which indicates increased secretion of perforin and granzymes. Our systematic transcriptome analysis of activated NK-92 CAR variants shows that IL-27 may be involved in the PI3K/AKT, and MAPK signaling pathways, as genes related to proliferation from these pathways were increased, e.g., PDK1, RPS6, MYC, and PGK1. In vivo studies with a xenograft mouse model of B-cell neoplasia showed that CAR.19-IL27 reduced tumor burden compared to CAR.19-treated. Altogether, CD19-CAR NK cells co-expressing Il-27 showed potent and selective antitumor activity with the potential to improve clinical use of CAR NK cells.

Objetivos: Atualmente, temos em média 5.624.450 registros de doadores cadastrados no Redome (REDOME ‒ Registro Nacional de Doadores Voluntários de Medula Óssea) com interesse em se tornarem doadores de medula óssea. Logo, realizou-se estudo observacional longitudinal retrospectivo de dados fornecidos pela plataforma de análise de NGS: TSV, de 2021 até a presente data para identificar a frequência alélica dos doadores cadastrados pela Fundação Hemocentro de Ribeirão Preto junto ao Laboratório de HLA. Materiais e métodos: Utilizou-se a base dados da plataforma do software de análise de NGS (Next Generation Sequencing) Type Stream Visual (TSV), com dados obtidos dos resultados da tipificação HLA de cadastro de doadores voluntários de medula óssea no Hemocentro de Ribeirão Preto-SP, de junho de 2021 a julho de 2023. Os grupos alélicos foram tipificados molecularmente em alta resolução para aos alelos HLA Classe I (locus A, B e C) e II (locus DRB1, DQA1, DQB1, DPA1, DPB1). Foram utilizados filtros do TSV e planilhas excel para identificação da frequência de todos os alelos presentes nos cadastros. Resultados: O estudo abrangeu doadores voluntários de medula óssea cadastrados na Fundação Hemocentro de Ribeirão Preto-SP, de Junho de 2021 a Julho de 2023. Com auxílio do Software TSV, foi possível identificar quais alelos são mais frequentes para classe I e classe II destes doadores, assim como perceber a presença de alelos não frequentes. Os alelos mais encontrados para classe I – foram: A*02:01, A*01:01, A*03:01, A*24:02. B*07:02, B*08:01, B*14:02, B*15:01. C*04:01, C*06:02, C*03:04, C*02:02. Para classe II – obtivemos: DPA1*01:03, DPA1*02:01, DPA1*02:02. DPB1*01:01, DPB1*03:01, DPB1*04:01. DQA1* 01:02, DQA1*02:01, DQA1*01:01. DQB1*03:03, DQB1*02:02, DQB1*03:02. DRB1*07:01, DRB1*03:01, DRB1*01:01. Foram encontradas tipificações não frequentes para Classe I: A*01:156, A*02:872, A*03:351. B*07:120, B*08:188, B*15:146, B*18:145, B*35:386. C*02:168, C*03:407, C*06:289, C*07:985. Como em Classe II: DQB1*03:296, DQB1*03:393, DQB1*05:234. DRB1*03:124, DRB1*03:181, DRB1*11:261. Discussão: As características apresentadas quanto as frequências alélicas em HLA vem se tornando similar entre as populações. Assim como as frequências alélicas, a presença ou ausência de alguns alelos específicos se associam aos cadastros de doadores voluntários de medula óssea (Pimtanothai et al., 2001; Luo et al., 2002; Piancatelli et al., 2004; Ayed et al., 2004). Ferreira (2007), identificou doadores voluntários mais frequentes para HLA-A*02:01 em estudo realizado no Maranhão, onde também apresentou maior frequência no locus DRB1*07:01. De acordo com Guimarães (2015) em outro estudo similar de frequência alélica: A*02, A*24, A*03, A*01, B*07, DRB1*07 e DRB1*01 fazem parte dos grupos mais frequentes cadasrados no REDOME. No estado de São Paulo (Campagnuolo, D. G., Cita, R. F., & Colombo, T. E. 2018) demonstram também a prevalência de alelos mais frequentes em doadores, sendo A*24, A*03, DQB1*03 e DRB1*01 em voluntários cadastrados para doação de medula óssea. Conclusão: A implantação da metodologia de tipificação de HLA por alta resolução (NGS) demonstrou uma melhoria na avaliação do perfil dos cadastros de doadores voluntários de medula óssea, tornando-se possível uma avaliação imunológica mais precisa dos doadores voluntários aos receptores. Os resultados obtidos neste estudo somam-se aos já existentes, demonstrando a miscelânea da população do Brasil e polimorfismo genético do Sistema HLA.

Introduction: Our previous study demonstrated that monocytes from breast cancer patients display alterations in their transcriptomic profile that include the down regulation of genes involved in their differentiation and in their ability to stimulate the immune system, in addition to producing reduced levels of inflammatory cytokines when compared to healthy donors'monocytes. We aim here to reveal common gene signatures present in monocytes from patients with distinct hematologic and solid cancer. Methods: An integrative bioinformatics analysis was performed to investigate monocyte signatures in ten public RNA-seq datasets from Hematologic Tumor (HT) (Chronic Myeloid Leukemia, acute myeloid leukemia and Diffuse large B-cell lymphoma) and Solid Tumors (ST) (esophageal, lung, head and neck, brain, colon). We generated a list of Differentially Expressed Genes (DEGs) between monocytes from healthy donors versus cancer patients or between healthy and tumoral tissues using the DESeq2 package in R-studio and performed the functional enrichment analysis using Gene Ontology (GO) and protein-protein interactions considering the common up-regulated genes. Results: In blood monocytes from the HT datasets, we found 13 common genes, including MSR1, CAMP, ELANE and CCL3. These genes are mainly involved in the signaling pathways for Phagocytosis and Calcium Ion Homeostasis. For monocytes in ST tissues, we found 161 commonly expressed genes in all datasets, including genes belonging to the immunoglobulin family (IGHG3, IGHM, IGHG4), genes involved in the organization of the extracellular matrix (MMP3, MMP8, MMP9, MMP12) and genes from the CXCL chemokine family (CXCL10, CXCL9, CXCL13). Conclusion: Our data suggest that monocytes are involved in extracellular matrix organization, cell migration and antigen recognition in HT and TS. We will next evaluate the expression of those molecules in monocytes from multiple myeloma patients and correlate with clinical data.

Background: T lymphocytes expressing chimeric antigen receptors (CAR) have been shown to be efficient for tretment of hematologic, but less efficient for solid tumors, due to the great complexity of the tumor microenvironment involving: heterogeneity of tumor antigens, difficulty in maintaining and persisting of immune responses, the immunosuppressive millieu, and the dense extracellular matrix. Thereby, macrophages emerge as an exceptional option for the insertion of CAR molecules, given their functional capacity to infiltrate the tumor matrix, to phagocyte, present antigens and secrete cytokines. Aim: To generate human macrophages expressing CAR from umbilical cord blood CD34+ progenitor cells to treat solid tumors. Methods: CD34+ umbilical cord cells were isolated, expanded in vitro for 07 days and differentiated into macrophages using in either StemSpan™ SFEM II (SP) or RPMI 1640 (RP) media with different combinations of the following cytokines: GM-CSF, M-CSF and IL-3, for additional 10 days. After differentiation, the cells were phenotypically characterized by multi-color flow cytometry and morphology (cytospin) and evaluated for their phagocytic capacity after co-culture with Nalm6 leukemic cell line (ratio 1:3). Subsequently, aiming to set-up a cell transformation protocol we used the piggyBac transposon system to insert the green fluorescent protein (GFP) via electroporation on CD34+ cells. Results: Our data revealed that 93.3% of the CD34+ differentiated cells were CD64+CD14+CD1clowCD163+CD16+CD86lowCD80lowHLA-DR+ and their morphological evaluation indicate a typical macrophage phenotype when compared to classical monocyte-derived macrophages. The phagocytosis assay showed a percentage of 20.1% and 16.2% when cells were differentiated in SP and RP, respectively. Furthermore, 26.8% of CD34+ cells expressed GFP within 24 hours after electroporation with a viability of 60.7%. Discussion and conclusions: Our data indicate that cells differentiated from human CD34+ umbilical cord stem cells showed phenotypic and functional features consistent with human macrophages. In addition, the piggyBac transposon system is an interesting protocol for the insertion of CAR molecules in CD34+ cells. Our next steps involve improving the electroporation efficiency of CD34+ cells and the transformation of CAR-expressing CD34+ cells into CAR-macrophages for their functional characterization against solid tumor cell lines.

The movements of 17 eastern sarus cranes Grus antigone sharpii in Cambodia and Vietnam were tracked during both wet and dry seasons in 1998-2002 and 2015-2017, revealing previously unknown but important sites. Crane breeding territories were located in Cambodia’s northern dry deciduous dipterocarp forests, with territories of cranes captured in the Tonle Sap basin located further west and more likely to fall within protected areas than those captured in the Mekong Delta. During the non-breeding (dry) season, cranes returned to sites in which they had originally been captured. Most cranes initially used a different part of the same floodplain in the early stages of the dry season, but cranes from the Mekong Delta that nested west of the Mekong River would initially stop in the eastern Tonle Sap floodplains before continuing to the Mekong Delta floodplains. Protected area coverage of key dry season habitat within the Tonle Sap basin was lower than in the Mekong Delta. Out of 5 juveniles tracked in 2015/2016, 1 disappeared, 1 died and 1 was injured; 1 adult also disappeared. All mortality and disappearances occurred during the wet season and at least 1 mo after capture. Persistence of the eastern sarus crane will require improvement of protected area coverage of both breeding areas and previously unknown but important sites used during the dry season. In the dry season, engagement of farmers in conservation efforts is also important, as crane home ranges included agricultural areas even in the direct surroundings of protected wetland habitat.

Introduction Middle-aged women experience several changes that can affect their biological and psychosocial health. A healthy lifestyle is a fundamental requirement for achieving optimal health. Objectives To examine the impact of psychosocial factors on health promotion behaviors among middle-aged women in Saudi Arabia. Methods A cross-sectional, descriptive, correlational study design was used. One hundred and eighteen middle-aged women completed the demographic questionnaire, Stunkard Figure Rating Scale, Perceived Stress Scale, and Health-Promoting Lifestyle Profile-II from September 2, 2020 to February 15, 2021. Multiple regression analysis was performed to estimate the impact of psychosocial factors on health promotion behaviors. Results Participants “sometimes” engaged in health promotion behaviors. Spiritual growth scored the highest, whereas physical exercise scored the lowest. Most of the participants were dissatisfied with their body image and reported moderate levels of perceived stress. The participants were more likely to report a preferred body image that was smaller than their perceived body image. Perceived stress and body image were significantly correlated with the total score for health promotion behaviors. Perceived stress level, age, and menopause status were significant predictors of health promotion behaviors. The model significantly predicted all the health promotion behavior subscales, excluding nutrition. Conclusion There were relationships between certain psychosocial predictors and health-promoting behaviors. This study can guide healthcare professionals in promoting middle adult women's adoption of health-promoting behaviors. More research can be conducted in Saudi Arabia to understand the health promotion behaviors of middle-aged women.

Abstract Background Cell phones are an integral part of modern day life and have become companions for individuals irrespective of age, gender and socio-economic status. In this study, we assessed the factors affecting risk of cell phone addiction among teachers attending Life Skills Training and Counselling Services (LSTCS) program in Karnataka. Methods This cross sectional secondary data analysis utilised data from baseline assessment of trainees attending a Life Skills Training and Counselling Services program (LSTCP). Various factors hypothesised to be affecting risk of cell phone addiction (outcome) was analysed using univariate and multivariable logistic regression analysis. All the analysis was done using STATA 12.0 software. Results Multivariable logistic regression analysis was conducted with risk of cell phone addiction as outcome. A conceptual framework of hypothesized exposure variables was developed based on expert consultation and literature review. Overall, data of 1981 participants was utilized. Gender (AOR=1.91; 95% CI=1.27-2.77), number of peers (AOR=1.01; 95 CI=1-1.008) and social quality of life (AOR=1.01; 95% CI=1.00-1.03) were associated with increased risk of cell phone addiction. Age (AOR=0.98; 95%CI=0.96-1.00), empathy (AOR=0.96;95%;CI=0.93-0.99), communication skills(AOR=0.92, 95%;CI=0.88-0.96) and physical quality of life (AOR=0.96; 95% CI=0.95-0.98) were associated with reduced risk of cell phone addiction. Conclusions This study on precursors of risk of cell phone addiction, conducted mostly among apparently healthy individuals, provide important insights into interventions to reduce risk of cell phone addiction. The complexity of associations between peers, gender, quality of life and risk of cell phone addiction needs further exploration.

Summary: Background: This evaluation of doula care emerges at a time when maternal mortality in the U.S. outranks every country in the developed world. Study objectives were to: 1) examine when over the maternity care continuum and with whom (i.e., clinical providers) doula care provides the greatest benefits to clinical health outcomes and health care utilization; and 2) evaluate whether women gain differential benefits from doulas depending upon race/ethnicity and health status. Methods: We conducted a retrospective cohort study using Medicaid medical claims from California, Florida, and a northeastern state (USA) to compare maternal health outcomes between women who did and did not receive doula care between January 1, 2014 and December 31, 2020. We used propensity score matching and logistic regression models to calculate associations between selected health outcomes and doula care. Our analysis included 298 pairs of women matched on age, race/ethnicity, state, socioeconomic status, and hospital type (teaching or non-teaching). Findings: Women who received doula care had 52.9% lower odds of cesarean delivery (OR: 0.471 95% CI: 0.29–0.79) and 57.5% lower odds of postpartum depression/postpartum anxiety (PPD/PPA) (OR: 0.425 95% CI: 0.22–0.82). Doulas who provided care with a clinical team that included a midwife most consistently showed a reduction in odds of cesarean delivery, regardless of the trimester when doula care was received. Women who received doula care during labor and birth, but not necessarily during pregnancy, showed a 64.7% reduction in odds of PPA/PPD (OR: 0.353 95% CI: 0.16–0.78) of PPA/PPD. Interpretation: The use of doulas appears an effective strategy for improving maternal health, especially among socioeconomically vulnerable and marginalized minority populations. Future studies could address research gaps through focusing on the relationship between doula care received in the postpartum period and postpartum health. Introduction: Pseudomonas aeruginosa (P. aeruginosa) is one of the most frequently co-infecting bacteria reported. Development of drug resistance, biofilm formation, cell associated factors make the P. aeruginosa more virulent. Type III secretion system controls expression of genes. P. aeruginosa chromosome harbours exoT, exoS, exoU, and exoY virulent genes. Gene exoT plays an important role in causation of infection. The identification of virulent markers of pathogens for identification of acute and chronic infections at early stage remains a critical area and still need large research. Aim: To study the prevalence of exoT gene encoding exotoxin T in P. aeruginosa from clinical samples. Materials and Methods: A descriptive cross-sectional research was conducted during January 2015 to March 2016 at the Department of Microbiology in Dr. D. Y. Patil Medical College, Pimpri, Pune, Maharashtra, India. Various clinical samples were processed using standard laboratory methods. The statistical analysis was done by using Chi-square test. Strains of P. aeruginosa isolated from various clinical samples were identified using standard laboratory methods, and exoT gene was detected by Polymerase Chain Reaction (PCR) and gel electrophoresis technique. Results: Out of 30 strains of P. aeruginosa, 20 (66.67%) were isolated from male and 10 (33.33%) from female patients. Most of them belonged to the age group 41-60 years (46.67%). The exoT gene occurred in 20 (66.67%) isolates of P. aeruginosa, while 10 (33.33%) showed negative amplification results. Out of 20 exoT genes in P. aeruginosa, 17/20 (85%) were detected from male and 3/10 (15%) from female patients. Conclusion: Gene exoT of P. aeruginosa plays the crucial role in causation of disease. It is concluded that exoT gene can be a notable virulent element expressed by 66.67% of P. aeruginosa clinical isolates. The proven role of exoT virulence gene in the pathogenicity of P. aeruginosa would help in understanding the prognosis of Pseudomonas infection and designing an effective treatment and vaccine against the Pseudomonas infections to prevent them.

Introduction: Thyroid hormones regulate growth, metabolism and cellular differentiation in our body. Thyroid disorders are among the commonly encountered endocrinopathies across the globe in the recent past. Hypothyroidism is prevalent to the extent of 9.45-11.73% among urban inland regions of India as reported in a recent survey. Thyroid hormones influence lipid metabolism. The dyslipidemia in hypothyroidism presents as high serum Total Cholesterol (TC) levels, elevated Low Density Lipoprotein-Cholesterol (LDL-C) levels, hypertriglyceridemia and Increased Triglyceride (TGL)- Rich Lipoproteins or Remnant Lipoproteins (RLPs). Non High Density Lipoprotein-Cholesterol (Non-HDL-C) reflects the cholesterol content of these RLPs. The potential of Non-HDL-C seems to be underutilised in patients with hypothyroidism. Aim: To know the levels of Non-HDL-C in hypothyroid patients as compared to normal subjects and to assess its reliability to predict the future cardiovascular risk in hypothyroid patients. Materials and Methods: The study was a case-control study, that was conducted in District Hospital, Chamarajanagar, Karnataka, India, between December 2019 to September 2020, involving two groups. The first group were the cases, consisting of 50 hypothyroid patients between 18 and 55 years of age, 50 healthy age and sex matched controls were selected and inducted into this study. The ‘Case group was further divided into “Known cases” and ‘New cases’, based on the disease course. The ‘New cases’ are further classified into group of ‘Overt Hypothyroids’ and ‘Subclinical hypothyroids’. In all the subjects of present study, serum TC and serum HDLCholesterol (HDL-C) was estimated. The value of HDL-C was subtracted from TC level and Non-HDL-C level was calculated for all the study subjects. The values of the lipid fractions in both groups and in the subgroup were tabulated and compared using suitable statistical tool. Results: In the present study it was noted that Non-HDL-C was significantly high in hypothyroid patients when compared to healthy controls (p=0.02). Also, the newly diagnosed hypothyroid patients had significantly high Non-HDL-C levels than controls (p=0.01). The subgroup of ‘Known cases’ who were on Levothyroxine replacement did not show significantly elevated NonHDL-C levels. Overt Hypothyroid (OH) patients had higher values of Non-HDL-C than those with Subclinical Hypothyroidism (SCH). Conclusion: At each level of the present study, (controls vs cases, known vs new cases and OH vs SCH) non-HDL-C has emerged in an expected pattern, which is consistent with pathophysiology, course and severity of the disease. Therefore, present study suggest the feasibilty of non-HDL-C replacing the traditional lipid profile assays for assessment of dyslipidemia in hypothyroid patients.

Introduction: The antiretroviral drugs have improved the quality and extent of life of Human Immunodeficiency Virus (HIV) infected patients, yet like any other long-term medication, these are known to cause several adverse effects. One such adverse effect is on the lipid metabolism in individuals on Antiretroviral Therapy (ART). Aim: To analyse the effect of ART on the circulating lipid levels in HIV patients. The secondary aim was to compare the lipid changes in patients treated with ZLN (Zidovudine+Lamivudine +Nevirapine) drug regimen against those, with TLE (Tenofovir+ Lamivudine+Efavirenz). Materials and Methods: This cross-sectional study was conducted from December 2019 to March 2021 at the District Hospital, Chamarajanagar Karnataka, India. A total of 200 HIV positive patients between 18-55 years of age with no associated co-morbidities and who have been on ART were recruited into this study. Of the total 91 patients were on TLE (Tenofovir+ Lamivudine+ Efavirenz) and 109 were on ZLN (Zidovudine+ Lamivudine+Nevirapine) regimen. Blood samples were collected from all the patients and lipid profile analysis was done. results: Statistically significant increase was observed in all lipid parameters in the ZLN group compared to TLE group. COVID-19 pandemic impacts on vocational education. Lectures that were originally conducted face-to-face learning are diverted to online learning to avoid the spread of the pandemic. Online learning is very difficult to apply for courses conducted in the laboratory. This study discusses vocational students’ responses to the practice of online learning during the COVID-19 pandemic. Data were collected through a questionnaire created on Google form consisting of 20 questions. The questionnaire used a Likert scale to find out the attitudes and students’ perceptions of the implementation of online learning. The number of research respondents was 107 people consisting of 45 respondents from the West Aceh State Community Academy and 62 respondents from Lhokseumawe State Polytechnic, Aceh, Indonesia. The results of this study found that 59.81% of students disagree with online learning. The results also showed a score of 76.95% of the students agree that internet access is the main obstacle in online learning. However, students’ satisfaction with the current online learning system for students shows a score of 67.50%. Opinions related to online learning from 107 respondents showed that 45.42% of them less agree if online learning is still applied when the COVID-19 pandemic ends.

Introduction: Age at menarche is said to be a risk factor for Gallbladder Cancer (GBC). The available research about the association of age at menarche with GBC, by taking a cut-off age of 13 years, is neither uniform nor consistent and at best inconclusive. Aim: To evaluate the association between age at menarche and the risk of GBC, based on cut-off age of 13 years and treating age at menarche as a continuous variable. Materials and Methods: A hospital based case-control study was conducted on 92 female patients and 92 female controls. The data was collected from the Department of Surgical Oncology, Sir Sunderlal (SS) Hospital, Banaras Hindu University, Varanasi, Uttar Pradesh, India during the period of October 2018 to June 2019. Logistic regression analysis was performed and Adjusted Odds Ratio (AOR) with its 95% CI (Confidence Interval) was obtained. Results: In the present study, the mean age of GBC cases and control were 56.84±8.74 years and 51.21±6.37 years, respectively. With the reference category of menarche age, the females with menarche age >13 years were insignificantly at lower risk of incidence of GBC (AOR=0.569; 95% CI: 0.263-1.233). But when age at menarche was considered as a continuous variable; the AOR was significantly lower among females found decreasing with increasing age at menarche (AOR=0.798; 95% CI: 0.666-0.955). Conclusion: This exercise indicates, using a reference category of continuous measurements obtained result may be inconclusive with an influence of subjective decision making so it is suggested to prefer the recorded discrete continuous value for the analysis procedure.

Introduction Current literature is inconclusive about the optimal treatment of elderly patients with displaced intra-articular distal radius fractures. Cast treatment is less invasive and less expensive than surgical treatment. Nevertheless, surgery is often the preferred treatment for this common type of distal radius fracture. Patients with a non-acceptable position after closed reduction are more likely to benefit from surgery than patients with an acceptable position after closed reduction. Therefore, this study aims to assess non-inferiority of functional outcomes after casting versus surgery in elderly patients with a non-acceptable position following a distal radius fracture.Methods and analysis This study is a multicentre randomised controlled trial (RCT) with a non-inferiority design and an economic evaluation alongside. The population consists of patients aged 65 years and older with a displaced intra-articular distal radius fracture with non-acceptable radiological characteristics following either inadequate reduction or redisplacement after adequate reduction. Patients will be randomised between surgical treatment (open reduction and internal fixation) and non-operative treatment (closed reduction followed by cast treatment). We will use two age strata (65–75 and >75 years of age) and a web-based mixed block randomisation. A total of 154 patients will be enrolled and evaluated with the patient-rated wrist evaluation as the primary outcome at 1-year follow-up. Secondary outcomes include the Disabilities of the Arm, Shoulder and Hand questionnaire, quality of life (measured by the EQ-5D), wrist range of motion, grip strength and adverse events. In addition, we will perform a cost-effectiveness and cost-utility analysis from a societal and healthcare perspective. Incremental cost-effectiveness ratios, cost-effectiveness planes and cost-effectiveness acceptability curves will be presented.Ethics and dissemination The Research and Ethics Committee approved this RCT (NL56858.100.16). The results of this study will be reported in a peer-reviewed journal. The research sought to understand the value of patriotism in the Hikayat Prang Sabi by using a qualitative approach. Hikayat was referred to narratives (saga) of war rooted in Acehnese cultural tradition, and Prang Sabi in Acehnese language meant ‘Holy War’. As a literary work, Hikayat Prang Sabi embodied the concept of jihad, which was usually sung during the time of war in the Acehnese history: Portuguese in 1511, Dutch in 1873, Japan in 1942, and the Republic of Indonesia in 1976. The source of the data was verses or stanzas of Hikayat Prang Sabi that contained patriotic values. These verses were then treated as the unit of analysis. A hermeneutic approach was employed to analyze the data. The findings show that Hikayat Prang Sabi contains two types of patriotic values: national and religious. National value refers to the sense of nationalism, ethnicity, and humanity. Religious value, on the other hand, displays the value of spirituality, prophecy, and peace. These values indicate the foundation of thinking and doing of the Acehnese people manifested in a literary work, which then can be transferred into the notion of character education.

Maternal anaphylaxis is rare albeit life-threatening critical incident dreaded by many due to negative effects on not only the mother but the foetus as well. Antibiotics and anaesthetic agents still contribute to majority of the episodes. Consequences of anaphylaxis such as placental insufficiency and subsequent foetal neurocognitive deficits are devastating outcomes. Acute respiratory distress syndrome following anaphylaxis is even rarer among the normal population. The management of maternal anaphylaxis does not differ from routine recommendations even though close monitoring and preparedness for early delivery should be embedded in the protocols. This is a rare case report of a primi mother who developed anaphylactic shock following intravenous penicillin in the background of negative allergic history, resultant foetal distress requiring emergency lower segment caesarian section and delayed onset acute respiratory distress syndrome which was later attributed to anaphylaxis. Pertinent identification and management which included a multidisciplinary team culminated in favourable outcomes.

Introduction: The high prevalence of Low Birth Weight (LBW) due to Preterm Birth (PTB) and Intrauterine Growth Restriction (IGUR) still persists a challenge in India. This happens due to poor and infrequent utilisation of Antenatal Care (ANC) service. Aim: To assess the difference of PTB and LBW newborns in mothers, who availed ANC at a tertiary care hospital against those who availed ANC at other healthcare facility. Materials and Methods: This cross-sectional study was conducted at Gynaecology Department in Sir Sundarlal Hospital, Institute of Medical Sciences, Banaras Hindu University, Varanasi, Uttar Pradesh, India and 1858 mothers and their newborn were analysed. The data was obtained from the delivery register maintained at the Labour Room of the Department. Introdução: A HPN é um distúrbio clonal causado por uma mutação adquirida no gene PIGA das células-tronco hematopoiéticas. O gene PIGA, localizado no cromossomo X, codifica uma proteína que é parte integral da formação da âncora de glicosilfosfatidilinositol (GPI) para proteínas de membrana. Mutações no PIGA resultam na perda de todas as proteínas ancoradas no GPI, como a CD59, uma importante proteína reguladora do complemento, na superfície dos eritrócitos. Como consequência, as células ficam suscetíveis à ativação pelo complemento, levando à hemólise intravascular contínua. Objetivo: Relatar um caso de Anemia Aplástica, com a associação à Hemoglobinúria Paroxística Noturna (HPN), enfatizando os aspectos fisiopatológicos, clínicos, diagnósticos e tratamento. Material e métodos: Trata-se de um estudo de caráter exploratório transversal por meio da análise dos prontuários e resultados de exames. Resultados: Paciente feminino de 54 anos, com diagnóstico (AA) severa sintomática (resposta da série granulocítica e plaquetária após o uso de imunossupressor (ciclosporina), apresentando associado à anemia, DHL elevado, aumento da bilirrubina indireta e teste de coombs negativo, com citometria de fluxo positiva HPN, demonstrando que 33% das hemácias apresentam perda total de expressão de CD59 (população HPN III). Da mesma forma, 86% dos neutrófilos apresentam perda de expressão CD24 e aerolisina, em 91% dos monócitos apresentam perda de expressão de CD14 e aerolisina. Na ocasião do tratamento a paciente teve necessidade transfusional (hemácias filtradas) em decorrer da AA. No decorrer do monitoramento apresentou diminuição das plaquetas associada à astenia, sangramento retal, leucopenia, tendo necessidade transfusional para manter a homeostasia. Foi tratado com ciclosporina pelo quadro da anemia aplástica severa e em seguida foi administrado Eculizumabe em tratamento continuo de periodicidade semanal dentro de quatro semanas (fase de indução) seguindo de período quinzenal após a quinta semana (fase de manutenção) no tratamento especifico para HPN. Discussão: A situação da paciente é grave por característica própria da doença base, porém em resposta ao tratamento apresentou melhoras no quadro clínico e na qualidade de vida, obtendo melhoras da anemia sintomática e amenizando o quadro de dor abdominal e necessidade transfusional, permitindo a paciente retomar as atividades doméstica e laborais. Conclusão: O conhecimento precoce da doença (AA) e HPN, beneficiara o paciente no tratamento e na melhoria da qualidade de vida minimizando os riscos de sequelas graves.

Introdução: Descrita pela primeira vez pelo médico francês Philippe Charles Ernest Gaucher, em 1882, a doença de Gaucher é uma moléstia genética decorrente da deficiência da enzima betaglicocerebrosidase, ou betaglicosidase, cuja função é digerir o glicocerebrosídeo, um tipo de gordura resultante dos resíduos das células que todos os dias morrem e são substituídas por outras novas. Seu tratamento e feito por Terapia de Reposição Enzimática sendo aplicado por meio de infusões semanais ou quinzenais para a reposição da enzima Beta-Glicoserebrosidase. Havendo relatos do desenvolvimento de anticorpos nos indivíduos tratados com o uso de infusões de Imiglucerase com um percentual aproximado de 10-15%. O diagnóstico precoce e tratamento com frequência correta das infusões dos medicamentos levam a uma redução dos sintomas da doença e melhora da qualidade de vida do paciente. Objetivo: Avaliar o quantitativo de infusões dos medicamentos de Reposição Enzimatica Imiglucerase e Taliglucerase utilizados para tratamento da Doença de Gaucher no período de junho a dezembro de 2019 atendidos em um ambulatório do nordeste brasileiro. Material e métodos: Trata-se de um estudo de caráter exploratório transversal por meio da análise dos relatórios disponíveis no setor do serviço ambulatorial. Resultados: O total de infusões de medicamentos para Doença de Gaucher no período de junho a dezembro de 2018 foi de 114, sendo o maior número de aplicações de Imiglucerase 56% (64) e o menor de Taliglucerase 44% (50) sendo que a meta de infusões para o período estudado foi de 180 doses para Taliglucerase e 115 para Imiglucerase o que demonstra que a meta prevista para o período estudado não foi atingida. Discussão: A ausência do paciente ao tratamento, pode implicar diretamente na piora da sintomatologia dos portadores da doença, levando a um aumento do número de doses do medicamento necessárias para realizar o controle. A baixa quantidade de doses administradas pode ser decorrente ao pequeno número de pacientes por se tratar de uma doença rara, pela falta de adesão dos mesmos ao tratamento e principalmente por ser uma rotina impactante ao cotidiano das pessoas que fazem o tratamento de várias formas, como em relação ao tempo reduzido na rotina de trabalho para apresentar-se nos centros de tratamento, além da distância dos centros de tratamento e custos com deslocamento, fatores estes que alteram os números de meta e conclusão, ocasionando valores fora dos padrões preconizados. Conclusão: Nesse sentido, esta pesquisa pode ser utilizada como importante ferramenta para que a instituição obtenha uma melhor compreensão acerca das manifestações clínicas dos pacientes portadores da doença de Gaucher.

Introdução: A doença de Erdheim-Chester é uma doença rara, clonal, de histiócitos. A apresentação clínica é heterogênea e o diagnóstico é pautado na apresentação clínica, radiológica e anatomopatológica. O tratamento é indicado para casos sintomáticos. A seguir, descrevemos e realizamos uma breve revisão de literatura de um caso de Erdheim-Chester atípico, atendido no Hospital das Clínicas de São Paulo em novembro de 2020. Relato de caso: Paciente MGP, 56 anos, masculino, com antecedente de síndrome metabólica e psoríase. Relatava desde abril de 2020 dificuldade de marcha com tendência à queda, disartria, diplopia esporádica e perda ponderal de 10 kg. Admitido no Hospital das Clínicas de São Paulo em novembro de 2020. Ao exame físico havia placas eritematosas e descamativas em joelhos, cotovelos e membro inferior esquerdo. Apresentava avaliação neurológica compatível com síndrome cerebelar global. Investigação complementar com análise liquórica, sorológica, hormonal e marcadores de autoimunidade negativas. Em Ressonância Magnética de encéfalo, evidenciada extensa área com alteração de sinal no tronco encefálico e pedúnculos cerebelares médios, com intenso realce pós-contraste na ponte e pequeno efeito expansivo local, associada a espessamento e realce pós-contraste da haste hipofisária. A angio-tomografia de abdome/ pelve evidenciou tecido mal definido e com atenuação de partes moles nos espaços perirrenais (hairy kidney) e espessamento da parede da aorta (coated aorta). Em cintilografia óssea havia reação osteogênica indeterminada nos úmeros, tíbias e fêmur, e o PET-CT não foi evidenciado acometimento ósseo. Realizada biópsia da gordura perirrenal com visualização de histiócitos, cuja imunohistoquímica evidenciou positividade para CD68, CD163 e Fator XIIIa e negatividade para S-100. Detectada mutação BRAF-V600E nestes fragmentos. Por fim, iniciado tratamento com Interferon-alfa, até o momento realizados 8 ciclos com melhora clínica. Discussão: A doença de Erdheim-Chester é uma neoplasia de células histiocitárias rara com maior prevalência no sexo masculino e na quinta década de vida e a mutação ativadora mais frequente é a BRAF-V600E, presente em 57-75% dos casos. O curso clínico é heterogêneo com acometimento multissistêmico. O acometimento ósseo é o mais frequente (95% dos casos) e a lesão de osteoesclerose na diáfise dos ossos longos de forma simétrica apresenta alta especificidade para a doença. Achados radiográficos típicos são: “coated aorta”e “hairy kidney”. Na atualidade, o tratamento envolve a inibição da via de sinalização, RAS-RAF-MEK-ERK, com inibidores do BRAF e inibidores da via MEK, nem sempre disponíveis. As principais alternativas à terapia alvo são interferon-alfa ou os análogos de purina. Conclusão: A doença Erdheim-Chester é rara, com acometimento sistêmico característico, principalmente ósseo. A lesão mais específica é a osteocondral simétrica em platô tibial. Nos 5% dos casos sem acometimento ósseo, torna-se um desafio diagnóstico.

Introdução: Mieloma Múltiplo (MM) é uma neoplasia maligna caracterizada pela proliferação clonal de plasmócitos na medula óssea, resultando em produção anômala de imunoglobulinas. Essa produção anômala apresenta-se com pico monoclonal de determinado anticorpo, com predominância de IgG na maioria dos casos, e implica em diversas repercussões clínicas. Tem prevalência maior a partir da quinta década de vida e seu diagnóstico é estabelecido por meio da história, de sinais clínicos e exames complementares. Com isso, este trabalho tem como objetivo relatar um caso de MM com dificuldade diagnóstica devido à atipia na apresentação pelo hemograma. Relato: Paciente feminina, 68 anos, com quadro de anemia (Hb 6,8 g/dL) normocítica em dezembro de 2020 associado a quadro de perda ponderal, fraqueza e neuropatia em mãos e pés. Apresentava hemograma evidenciando leucocitose às custas de segmentados e bastões há cerca de 5 anos, VHS aumentado e normocalcemia. Em USG de abdome total, apresentou discreta esplenomegalia. Inicialmente foi pensado em doença mieloproliferativa crônica. JAK- 2 e BCRL-ABL resultaram negativos. Enquanto aguardava resultado de biópsia de medula óssea, foram achadas lesões líticas em exames de imagem e assim iniciado investigação de possível mieloma múltiplo. A EFP sérica apresentou pico monoclonal de 1,96 gl/dL e B2M inicial era > 20.000; proteinúria 24h: 4,51 g. A biópsia de medula óssea evidenciou mieloma múltiplo/plasmocitoma com monoclonalidade para cadeia leve kappa; grande carga tumoral (70% do tecido hematopoiético). Atualmente encontra-se em tratamento com genuxal, dexametasona, talidomida e ácido zoledrônico; em resposta parcial muito boa, encaminhada para consolidação com transplante autólogo de medula óssea. Discussão: O quadro clínico heterogêneo do Mieloma Múltiplo é causado pelas imunoglobulinas anômalas produzidas pelo linfócito B clonal neoplásico, o que o diferencia das doenças mieloproliferativas crônicas. Este grupo é definido por neoplasias de células-tronco hematopoiéticas desencadeadas pela produção excessiva de leucócitos (exceto linfócitos), eritrócitos ou plaquetas e engloba cerca de sete patologias, sendo a leucemia mieloide crônica, policitemia vera, trombocitemia essencial e mielofibrose primária as principais. No presente relato houve inicialmente alterações no hemograma caracterizadas por leucocitose neutrofílica de longa data e anemia normocítica associadas à esplenomegalia discreta. Tais dados sugeriam um quadro de doença mieloproliferativa crônica até exames de imagem evidenciarem a presença de múltiplas lesões líticas, indicando acometimento ósseo sugestivo de Mieloma Múltiplo, o que foi confirmado com a bioquímica específica e biópsia de medula óssea, reforçando a necessidade de sempre investigar a doença em toda anemia em pacientes idosos.

Introdução: A Síndrome de POEMS revela-se através de um quadro multifacetado, apresentando-se primordialmente como polineuropatia periférica e distúrbio de células plasmáticas monoclonais. O acrônimo “POEMS”proposto por Bardwick e col. em 1980, busca descomplexificar o diagnóstico desta rara síndrome: Polyneuropathy, Organomegaly, Endocrinopathy, monoclonal gammopathy e Skin changes. O diagnóstico é clínico e estabelecido a partir dos critérios da Mayo Clinic, encaixando-se o paciente nos critérios maiores obrigatórios (neuropatia periférica e proliferação monoclonal) e pelo menos um dos outros critérios menores (doença de Castleman, lesão osteoesclerótica, organomegalia, endocrinopatia, alterações cutâneas, papiledema, sobrecarga de volume extravascular e trombocitose). Trata-se de uma síndrome rara, sendo sua prevalência inexata na literatura. A incidência é maior entre a quinta e sexta décadas de vida e maior número de casos em pacientes do sexo masculino. Ainda que sua patogênese não esteja completamente elucidada, têm-se encontrado nesses pacientes elevados níveis séricos de fator de crescimento vascular endotelial (VEGF), alterações da proteína M, interleucina (IL) 1, IL-6 e fator de necrose tumoral-alfa (TNF-α). O objetivo do presente estudo foi relatar um caso de síndrome de POEMS, ressaltando a importância do conhecimento desta entidade. Material e método: Trata-se de um estudo descritivo baseado na análise do prontuário do paciente e revisão da literatura. Relato de caso: Paciente de 60 anos, masculino, procura serviço em abril de 2019 com dor lombar há 3 anos irradiada para MMII com dificuldade para andar e perda de mobilidade, fazendo uso de cadeira de rodas. Fez uso de paracetamol + fosfato de codeína, sem melhora. Acompanhava com neurologista que investigava uma polineuropatia (a esclarecer). Além disso, apresentava baqueteamento digital, com atrofia bilateral das mãos, adenomegalias e lesões osteoescleróticas em coluna vertebral. Em junho de 2019 foi feito o diagnóstico de Síndrome de POEMS e iniciado tratamento com ciclos de Dexametasona 40 mg/dia durante 4 dias, mantido até maio de 2021. Em agosto de 2020 foi associada a Ciclofosfamida 50 mg/dia. Paciente teve melhora importante da mobilidade, do baqueteamento e da dor neuropática, interrompendo o uso de cadeira de rodas e da morfina. Foi encaminhado para transplante de medula óssea autólogo, realizado em julho de 2021. Discussão e conclusão: O quadro clínico inicial do paciente foi marcado pelo acrônimo da síndrome de POEMS (polineuropatia, organomegalia, endocrinopatia, proteína M e alterações da pele), o que permitiu o diagnóstico correto e um ponto de partida para o início do tratamento. Atualmente não há uma linha terapêutica definida, entretanto, alguns estudos sugerem que a associação entre corticoesteróides e quimioterápicos a longo prazo aumentam a sobrevida e diminuem os sintomas neuropáticos dos pacientes, como observado no relato acima. Distinguir a Síndrome de POEMS de seus diagnósticos diferenciais é importante para a escolha do tratamento, cuidados de suporte, e as toxicidades em relação aos medicamentos envolvidos. Um diagnóstico incorreto pode levar a piora do quadro e aumento da morbidade.

Introdução: Uma das principais manifestações clínicas das hemofilias, especialmente as graves, são os hematomas, principalmente as hemartroses. Elas são de hemorragias intra-articulares que ocorrem desde os primeiros anos de vida. Estima-se que mais de 80% de todos os casos de hemorragias em portadores de hemofilia grave sejam intra-articulares. Ao longo do tempo, a recorrência das hemartroses levam à ativação de enzimas líticas e fibrose, gerando dano articular permanente, o que causa vários níveis de incapacidade física. Os demais sangramentos geralmente acontecem em episódios isolados, principalmente após algum trauma ocorrido. OBJETIVO: O presente estudo teve como objetivo analisar as manifestações clínicas da hemofilia dos pacientes atendidos no ambulatório de uma região do nordeste brasileiro. Material e métodos: A pesquisa constitui-se de uma análise retrospectiva a partir de informações dos prontuários de pacientes portadores de hemofilia A que foram atendidos no setor ambulatorial em uma região do nordeste brasileiro, durante o período de janeiro de 2018 a março de 2020. Resultados: Foram avaliados 100 prontuários de pacientes portadores de hemofilia A atendidos no ambulatório destes, foram selecionados 17% (17), levando-se em consideração os pacientes que apresentam aloanticorpos inibidores do fator VIII. Dos pacientes diagnosticados, 18% (3) apresentam inibidores de alta titulação e, dentre eles, 67% (2) apresentam a forma grave da doença e 33% (1) apresentam a forma leve; dentre os 82% (14) dos pacientes que apresentam inibidores de baixa titulação, por sua vez, 64% (9) têm a forma grave da doença, 21% (3) têm a forma leve e 14% (2) apresentam a forma moderada (gráfico 1). Dos sintomas apresentados pelos pacientes que apresentaram inibidores, todos apresentaram hemartrose e hematomas, e entre aqueles que desenvolveram algum tipo de artropatia: 80% (8) eram da forma grave e 20% (2) tinham hemofilia leve; enquanto isso, entre os que sofreram algum sangramento gengival: 60% (6) tinham a forma grave e 40% (4) tinham a leve. Discussão: Uma das principais manifestações clínicas das hemofilias, especialmente as graves, são os hematomas, principalmente as hermatroses. Elas são de hemorragias intra-articulares que ocorrem desde os primeiros anos de vida. Introdução: Hemofilia é uma doença crônica do sistema hemostático resultante de deficiência quanti ou qualitativa de uma ou mais proteínas plasmáticas, e pode se manifestar através de quadros hemorrágicos ou trombóticos. Etiologicamente, podem ser classificadas de acordo com as deficiências de fator em: hemofilia A (Fator VIII) ou hemofilia B (Fator IX). O Programa de Dose Domiciliar (DD), implementado pelo Ministério da Saúde em 1999, tem como objetivo permitir que o paciente com hemofilia possua em sua residência pelo menos uma dose suficiente de pró-coagulante, para uso em caso de hemorragia, até que ele possa buscar atendimento médico em seu centro de tratamento, sem interromper, de imediato, suas atividades diárias. Objetivo: Descrever o perfil dos pacientes portadores de hemofilia em um ambulatório do nordeste brasileiro que participam do programa oficial do Ministério de Saúde de DD. Material e métodos: Trata-se de um estudo descritivo, transversal, de abordagem quantitativa, com utilização de dados de prontuários dos pacientes cadastrados no programa de DD no estado de Sergipe. A coleta de dados ocorreu de janeiro de 2013 a dezembro de 2019. Foram coletadas informações sobre as seguintes variáveis: idade, sexo, estado civil, município, estado de origem e diagnóstico. Após a seleção de dados válidos, os mesmos foram organizados e analisados em planilhas do programa Excel. Resultados: Havia 66 pacientes cadastrados na unidade sendo todos do sexo masculino. A idade dos pacientes variou entre 4 a 60 anos com a média de 26,3 (DP+14,1) e a maioria tinha entre 11 e 30 anos (54,6%). Sobre o local de procedência, a maior parte residia no estado onde a pesquisa foi realizada (92,4%) sendo 36,1% da capital e 63,9% do interior. Em relação ao estado civil, os solteiros corresponderam a 77,3% dos pacientes, seguidos dos casados (15,2%); os outros 7,6% não foi informado no prontuário. No que se refere ao tipo de hemofilia (A ou B), a hemofilia A foi o diagnóstico mais frequente e representou 92,4% dos pacientes cadastrados. Discussão: Hemofilia é uma doença genético-hereditária ligada ao cromossomo X e transmitida quase exclusivamente a indivíduos do sexo masculino, o que justifica o resultado desse estudo. Os dados encontrados em relação à idade dos pacientes se assemelham aos da literatura, tanto no que se referiu à média quanto à variação das idades. Quanto ao município de origem, verificou-se que a maior parte pertencia a municípios distantes da cidade sede, o que poderia dificultar um tratamento em caso de emergência. A DD permite o tratamento precoce do sangramento, reduzindo assim, suas complicações. Em relação ao estado civil, a quantidade de pacientes solteiros pode estar relacionada à média das idades encontradas. No que se refere ao tipo de hemofilia, foi encontrada uma quantidade maior de casos de hemofilia A e menor de B, quando comparado aos dados da Federação Mundial de Hemofilia, com relatos de 80 a 85% de hemofilia A e 10 a 20% hemofilia B. Conclusão: Este estudo atingiu seus objetivos, destacando a importância da atenção à saúde e do monitoramento constante dos dados, o que pode gerar subsídios para a elaboração de outros estudos relacionados às coagulopatias, com o intuito de contribuir para a qualidade de vida desses pacientes.

Objectives: In recent years, studies with natural killer cells expressing CAR (CAR-NK) for cancer treatment have shown exciting results. Nevertheless, more studies focusing on the development of specific CAR molecules for NK cells are needed to generate CAR-NK cells with greater capacity for proliferation and activation and consequently improving their therapeutic efficacy. The aim of this study is to develop new combinations of CAR transmembrane region and intracellular domains to enhance CAR-NK signaling and cytotoxicity functions. Methods: Eight lentiviral vectors containing IL-15 secreting CARs composed of an extracellular single-chain variable fragment (scFv) anti-CD19 followed by different combination of transmembrane region, activator receptors and co-receptors were designed. Four of them were constituted by CD8 transmembrane region in combination with CD3z, DAP12, 4-1BB and/or 2B4: CAR19-CD8-41BB-CD3z (Insert A), CAR19-CD8-41BB-DAP12 (Insert B), CAR19-CD8-2B4-CD3z (Insert C), CAR19-CD8-2B4-DAP12 (Insert D). The other 4 vectors were composed of NKG2D transmembrane region in combination with CD3z, DAP12, 4-1BB and/or 2B4: CAR19-NKG2D-41BB-CD3z (Insert E), CAR19-NKG2D-41BB-DAP12 (Insert F), CAR19-NKG2D-2B4-CD3z (Insert G), CAR19- NKG2D -2B4-DAP12 (Insert H). Then, NK92 cell line was transduced, CAR19+ cells were enriched using magnetic beads and CAR19+ expression on cell surface evaluated by flow cytometry. Lastly, we evaluated in vitro potential of CAR-NK92 cells to kill Raji CD19+ cancer cell lines stained with PKH67 cell linker at E:T ratio of 10:1 after 5 hours co-cultivation. Results and discussion: The expression of CARs containing CD8 transmembrane region (Insert A, B, C and D) on cell surface ranged from 2-4% CAR on day 7 post-transduction and remained stable until 21 days post-transduction and, after two selections, NK92 cells expressing 70-90% of CAR were obtained. The expression of CARs containing NKG2D transmembrane region (Insert E, F, G and H) ranged from 0.15-0.38% on day 7 post-transduction, and it was unfeasible to select these cells. Next, the cytotoxicity of NK92-CAR cells containing CD8 transmembrane region was evaluated in vitro. All NK92 cells containing CAR were more efficient in killing target cells than NK92WT cells (% Cytotoxicity rate mean±SD: -1.80±6.32). In addition, NK92-CAR Insert B (68.53±3.72), C (87±2.77) and D (65.18±2.12) cells were more cytotoxic than NK92-CAR Insert A cells (42.22±7.23; p < 0.05), being the NK92-CAR Insert C cells the most cytotoxic of all CARs at 10:1 ratio (p < 0.05). It is important to note that CAR-Insert A is the CAR commonly used in CAR-T cell therapy, suggesting that the vector containing receptors and co-receptors specific for NK cells was able to improve their function. Conclusion: Our results demonstrated that the transmembrane region affected CAR expression on cell surface, being the CD8 region more effective. Furthermore, NK92 cells expressing CAR specific for NK cells were more efficient in killing tumor target cells, and the NK92-CAR cell constituted by 2B4 and CD3z intracellular domain (Insert C) was especially effective. The therapeutic effectiveness of these cells will be evaluated in vivo. Introduction: Anti-CD19 CAR-T cell therapy has been associated with improved outcomes in R/r DLBCL. In Brazil, management of patients treated with this therapy can be cumbersome due to the lack of experience with this therapeutic product in most centers. Objective: Describe a case of successful anti-CD19 CAR-T cell treatment as salvage therapy in a patient previously exposed to 5 therapy lines, including autologous HSCT, as well as detail the patient's follow-up post CAR-T cell infusion. Methods: Case description, with patient's laboratory and imaging results during follow-up. Results: A 62-year-old male patient with a 7-year history of DLBCL, previously treated with 5 lines of therapy, was referred to Cleveland (Dr Marcos de Lima) to receive anti-CD19 CAR T-cells in March 2020 following disease relapse after autologous transplant and refractoriness to later therapeutic lines. Referral was due to the lack of available therapies with curative intent in Brazil, associated to the availability to enroll in an anti-CD19 CAR-T cell therapy clinical trial (CASE 2417) at the Seidman Cancer Center in Cleveland, Ohio, USA. PET-CT results before CAR-T cell treatment are shown in Figure 1. The infused cell product was manufactured with the automated CliniMACS®Prodigy platform using a commercial lentiviral vector. As pre-infusion lymphodepletion therapy, the patient received cyclophosphamide 60 mg/kg on D-6 and fludarabine 25 mg/m2 on days D-5 to D-3. The infusion of the CAR T cells occurred on 20/02/2020 – cellular dose: 2 x 106/kg. As early complications, the patient had grade II CRS and severe neutropenia related to the use of sulfamethoxazole-trimethoprim for pneumocystis prophylaxis. There was no neurotoxicity. Patient remained hospitalized for a month, and returned to follow-up at HC/FMUSP in May 2020. During patient follow-up, regular evaluation of immune reconstitution is being performed – table 1. Analysis of immune reconstitution by flow cytometry after CAR-T cell therapy showed reconstitution of CD3+, CD8+ and NK (CD3-CD16+CD56+) lymphocytes, reduced levels of CD4+ lymphocytes and absence of CD19+ B cells (Table 1). Given the old age of the patient, exposure to multiple lines of therapy and lack of consensus in the literature, we opted for monthly immunoglobulin replacement in the first year. Patient is receiving infectious prophylaxis with acyclovir and monthly pentamidine. There have been no infectious complications so far. Disease reassessment with PET-CT 6 and 12 months after therapy demonstrated a complete response (CR) (Figure 2). Although we do not have available data regarding CAR-T cell persistence, the serial evaluation of lymphocyte subpopulations confirms persistent B cell aplasia. Conclusions: Anti-CD19 CAR T-cell therapy is efficient therapy in R/r DLBCL, with manageable late toxicities. This patient's persistent B cell aplasia associated with CR from disease 16 months post-infusion suggests persistence of circulating CAR-T cells.

Aims: Natural Killer (NK) cells have gained much recent attention as a promising alternative platform for CAR engineering owing to their unique biological attributes, their specialized cytotoxicity against tumors, their safety profile, and their potential use as an off-the-shelf cellular therapy. However, NK transgene expression level is a major technical obstacle to the development of NK cell immunotherapy. Promoters, present in vectors, are responsible for maintaining the expression of CAR in host cells. Furthermore, promoters may differ in their ability to produce lentiviral particles and in their transduction efficiency. The present study aimed to evaluate whether different internal promoters result in a different percentage of transduced cells and transgene expression level in NK cells and thus to be able to choose an ideal promoter to drive the expression of CAR.CD19 increasing the antitumor potency of NK cells. Methods: Lentiviral particles with EF1a and SFFV promoters driving GFP gene expression or CAR.CD19 were produced, and the virus titer was measured by flow cytometry. Lentiviral particles were used for transduction of NK-92, NK from peripheral blood and NK from cord blood. CAR positive cells were detected by flow cytometry using anti F(ab)2 antibody. CAR-NK cell cytotoxic potential and specificity were assessed using established human Raji and NALM-6 lymphoblastoid cell lines (CD19+) as targets, which express varying antigen levels and MDA-MB-453 (CD19-) cells were used as negative control. Results and discussion: First, we explored the potential EF1a and SFFV promoters to drive GFP gene expression. SFFV promoter produced high lentiviral titers and achieved the best transduction efficacy in NK cells. The major difference between the promoters was the ability of SFFV to maintain longer and more robust GFP expression in NK cells. Therefore, we chose this promoter to drive CAR.CD19 expression and interleukin (IL)-15 co-expression in NK cells. There was a transduction efficiency of 13% (+/- 13.7) for NK-92 cells, assessed 48h after transduction. NK cells from Peripheral Blood (NK-PB) and Cord Blood (NK-CB) were also transduced with an efficiency of 27% and 49%, respectively. After 3 weeks in culture, the percentage of CAR-NK cells progressively decreased in primary cells. Cell growth of transduced cells from both sources was not statistically different; despite wild type NK-CB cells had a higher fold-change expansion than wild type NK-PB. CAR-NK-92 cells modified with SFFV were enriched with magnetic beads to 98% of NK cells positive for CAR expression and showed efficient and stable transgene expression during ex vivo expansion. The enriched CAR.CD19-NK-92 cells showed a higher killing potential against Raji CD19+ cells, compared with non-transduced NK-92 cells. Further, we evaluated the CAR-NK primary cells in vitro cytotoxic potential. CAR-NKs both from PB and CB had a higher killing potential against Raji and NALM-6 CD19+ cell lines than non-transduced NK cells. Conclusions: Taken together, these results indicate that the developed vector is functional and able to transduce NK cells from different sources, enhancing their anti-tumoral activity against tumoral CD19+ cells. Our results highlight that the promoter choice is essential to generate CAR-NK cells suitable for therapeutic applications. Introdução: A doença de Chagas humana (DC) constitui uma endemia predominantemente rural, com a estimativa de prevalência da infecção de 16 a 18 milhões de indivíduos nos diversos países do continente americano, estando relacionada a fatores ambientais, sociais e políticos. Na década de 1970, as transfusões de sangue se concentravam nas grandes cidades brasileiras onde a tecnologia era rudimentar, existia alta proporção de doadores remunerados, não havia controle do produto transfundido e a cobertura de testes sorológicos para doença de Chagas em doadores de sangue era menor que 20%. Objetivo: O presente trabalho teve como objetivo conhecer a prevalência de Trypanosoma cruzi entre os candidatos a doação de sangue, em um hemocentro de uma região do nordeste brasileiro. Material e métodos: Os dados foram obtidos pelo sistema informatizado do Hemocentro no período de janeiro a dezembro no ano de 2019. Os métodos utilizados para a triagem sorológica foi ELISA (ensaio imunoenzimático), e os testes confirmatório através do IFI (imunofluorescência indireta). Resultados: Do total de 26.831 pessoas que doaram em 2015, houve um percentual de 94,3% de bolsas liberadas pela sorologia; 5,6% foram reagentes para alguma doença infecciosa e, dessas, 5,5% tiveram bolsas bloqueadas por apresentarem sorologia reagente para doença de Chagas, desses, sendo que das bolsas bloqueadas, apenas 13 pacientes (65%) tiveram sorologia reagente confirmada para Chagas, destes; 76,9% eram do sexo masculino e 23,0% do sexo feminino, e a idade foi entre 28 e 58 anos e média de idade entre estes foi de 44 anos. Discussão: A pesquisa mostrou uma diminuição da doença de chagas em relação aos anos anteriores, no entanto se faz necessário maior ênfase nas entrevistas realizadas na triagem clínica e sorológicas para reduzir a contaminação pelo Trypanossoma cruzi em bancos de sangue. Conclusão: Conhecer o perfil do doador inapto é importante para a garantia da prática hemoterápica segura, evitando a transmissão de doenças infecciosas a partir da transfusão de sangue.

A contagem dos elementos figurado do sangue periférico proporciona avaliação dos três componentes principais do sangue (eritrócitos, leucócitos e plaquetas) e, portanto, é a base de qualquer avaliação hematológica, para verificação quantitativa e qualitativa, sendo o exame mais indicado para o conhecimento de diagnósticos, monitoramento clínico e acompanhamento da evolução das doenças humanas. As células sanguíneas vermelhas são as mais abundantes no corpo. A contagem normal das células está entre 4,5 e 6,5 milhões de células/mm3. Trata-se de um estudo de caráter exploratório transversal por meio da análise dos resultados dos hemogramas dos pacientes e candidatos a doação de sangue de um Centro de Hemoterapia no período de fevereiro a maio de 2019. Os resultados dos hemogramas analisados no equipamento de Hematologia Lumiratek H5 Automático do laboratório de apoio do banco de sangue. Dos 153 hemogramas realizados 68% (104) foram de indivíduos do sexo masculino e 32% (49) do sexo feminino. Em relação à média dos resultados glóbulos vermelhos (RBC) do gênero masculino foi de 4,7×106 μL com a máxima 6,9×106 μL e a mínima de 2,16×106 μL com valores de referência entre (4,50 – 5,80), quanto a média dos resultados (RBC) do gênero feminino foi de 4,02×106 μL com a máxima 6,14×106 μL e a mínima de 1,76×106 μL com valores de referência de (4,10 – 5,50). Quando analisados os linfócitos LYM a média para o gênero masculino foi de 2,31×103 μL, com a máxima 13,7×103 μL e a mínima 0,51×103 μL com valores de referência de (0,80 – 5,50), o feminino a média foi 1,89×103 μL, a máxima foi 4,26×103 μL e a mínima 0,48×103 μL com valores de referência de (0,74 – 5,50). Ao analisar a hemoglobina HGB a do gênero masculino obteve-se a média de 15,1 g/dL, com a máxima 19,1 g/dL e a mínima 5,6 g/dL com valores de referência entre (13 – 17), e o sexo feminino a média de 12,4 g/dL, com a máxima 19,3 g/dL e a mínima 5,4 g/dL com valores de referência de (12 – 16). Quanto ao volume ocupado pelas hemácias no volume total de sangue (HCT) a média para o do gênero masculino 45,1% com a máxima 61,3% e a mínima 21% com valores de referência entre (37 – 54), o feminino a média foi 38,1%, a máxima 58,6% e a mínima 15,7% com valores de referência de (36 – 48). O aumento significativo dos glóbulos brancos pode ajudar a confirmar a presença de uma infecção e sugere a necessidade de outros testes para identificar a sua causa. A diminuição do número de glóbulos vermelhos (anemia) pode ser avaliada através das variações no tamanho e forma dos glóbulos vermelhos para ajudar a determinar se a causa se deve a uma diminuição da produção, a um aumento da perda ou a um aumento da destruição de glóbulos vermelhos. Conclui-se que as médias dos valores estão dentro dos parâmetros, entretanto algumas máximas e mínimas sofreram discrepâncias devidas às condições clínicas dos pacientes e candidatos a doação de sangue atendidos no banco de sangue.

A hepatite B é uma enfermidade infecto-contagiosa, cujo agente etiológico é o vírus da hepatite B (VHB), que pode ser adquirido através relações sexuais, de forma perinatal e através de transfusão sanguínea. A Hepatite é encarada como um dos maiores problemas de saúde pública no mundo, caracterizando- se por 02 estágios bem definidos, que se manifestam como enfermidade aguda e crônica. Este estudo objetivou traçar o perfil epidemiológico dos portadores da Hepatite B em um centro de hemoterapia do estado de Sergipe no ano de 2019. Os dados foram coletados em banco de dados informatizado institucional, referentes ao período de janeiro a dezembro de 2019. Observou-se dos 31.316 potenciais doadores, 16,84% (5.274) foram considerados inaptos para a doação, após triagem clínico-laboratorial. Destes 3,05% (795) casos foram positivos para Hepatite B, sendo excluídos e as suas bolsas desprezadas. Prevalência de sorologia positiva foi entre os doadores do sexo masculino, 80,38% (639). Sendo que, 26,67% (212) se encontravam na faixa etária de 30 a 39 anos. Nas mulheres, a maior prevalência foi também na faixa etária entre 30 e 39 anos (55–6,91%). Dentre os indivíduos que apresentaram positividade para o HbsAg, tanto os homens quanto as mulheres, demonstraram uma maior incidência na faixa etária de 18 a 29 anos. Nas pessoas que apresentaram uma positividade para anti-HBc, observou-se uma maior incidência do sexo masculino, sendo uma faixa etária maior acometida dos 30 aos 39 anos. Quanto aos resultados sorológicos, o HBsAg prevaleceu no gênero masculino na faixa etária entre 30 a 49 anos (40%) e no feminino de 18 a 29 anos (8,42%) e o anti-HBc prevaleceu em ambos os gêneros na faixa etária entre 30 a 49 anos, correspondendo a 48,14% no masculino e a 11,28% no feminino. Em relação ao tipo de doação, 51% do total de doações foram de doadores de reposição. Estes dados fortalecem a importância da detecção de anti- HBc na melhoria da qualidade do sangue a ser transfundido, tornando-o mais seguro do ponto de vista. Esta pesquisa poderá servir aos gestores públicos da saúde, a fim de subsidiar o desenvolvimento de estratégias de prevenção, intensificando campanhas de caráter junto à população, visando estabelecer a profilaxia, tratamento e controle de cura de doenças que pode se adquirir por transmissão sanguínea, e dentre elas a Hepatite.
