For a person who finds himself in a situation where there is a threat to his life or health, or the life or health of his relatives, the time within which the competent authorities will come to his aid is critically important. In order to respond to emergencies in a timely manner, Ukraine has established a mechanism for responding to citizens' appeals. This mechanism includes the State Emergency Service of Ukraine, the National Police of Ukraine, emergency (ambulance) medical care, emergency gas service and other emergency services. However, it is not entirely convenient to call these services for help, as the call is made by different phone numbers and in situations where a variety of phone numbers will lead to a loss of time and serious consequences. Ukraine's integration into the European community requires our country to review the mechanisms for protecting a person in an emergency situation. It is established that the main purpose of implementing the system of emergency assistance to the single telephone number 112 in Ukraine is to provide timely assistance to European level with the ability to communicate in foreign languages, prompt involvement of necessary emergency services, which is a decisive factor in providing assistance, emergency response. The article also considers the issues of training employees who receive calls to the single emergency number «112», their interaction with each other during the performance of duties at emergency response centers, as well as their interaction with employees who directly provide assistance to scene. It is established that the training of operators who organize the rescue operation is carried out at the police academy. The training system for 112 Centre operators in Estonia is one of the best in the world and can be borrowed for implementation in Ukraine. At the same time, the «Emergency Assistance System for the Population via a Single Phone Number 112» is a rather complex mechanism for the implementation of which, in addition to analysing positive foreign experience, it is necessary to involve a fairly wide range of specialists from various fields, and therefore to create an appropriate regulatory framework to regulate the interaction between different response services. The research reveals the signs of sociocultural identity of rescue officers of the region on the basis of the analysis of professional culture level. The novelty of the research topic consists in the fact that social stability in the country in general and in the region particularly depends on the effectiveness of rescue services. A special role in the effectiveness of rescue services is assigned to executors involved in overcoming catastrophes and their consequences-employees of special search and rescue squads (SRS). The externality of the rescue service is widely covered in mass media, especially in emergency situations. Personal characteristics and features of interaction of participants in rescue operations, together with special knowledge and skills that form the basis of professional culture, often remain behind the scenes. The research methodology is based on institutional and structural and functional approaches to the analysis of rescuers' activities. Empirical sociological data obtained by a sociological survey of the general totality of the search and rescue squad of the General Directorate of the Ministry of Emergency Situations in the Republic of Adygeya. As a result of the research, an integrative model of a rescuer’s personality was obtained, as well as data indicating a high level of professional culture and an adequate socio-cultural identity of SRS officers. An indicator of professional culture is understanding of the value of professional training for rescue activities by workers. Most of the officers have developed a valuable attitude towards human life both in emergency situations and in the conditions of everyday life in the course of their service. Understanding the leading role of the state in ensuring effective operation of rescue services and high self-esteem of rescue services indicate the stability of the structure and effectiveness of the activities of the Emergencies Ministry.

The article discusses the role of rescue services on reducing the level of negative social tensionassociated with economic, political, medical and biological crisis situations. An analysis of the phenomenon and the concept of "social tension" has been carried out, and forms of the reaction of the population to various kinds of crises revealed. Prolonged social tension is considered to provoke the onset of socially dangerous phenomena that can destabilize social development and cause entropic processes. Modern methods of diagnosing public dissatisfaction with various processes in the economy, politics, social sphere have been briefly described, which can enable the competent authorities to properly respond to emerging crisis phenomena, or level their consequences. Rescue services are considered to be competent bodies in the structure of the state; their activities are aimed directly at the prevention and elimination of the consequences of emergency situations. The research method is a theoretical analysis of the problem based on theoretical and empirical data from previous sociological, psychological and pedagogical studies. The main conclusions are that rescue services are an important factor in maintaining an optimal level of social tension, an effective means of reducing the possibility of the onset of socially dangerous phenomena, the main tool for eliminating the consequences of catastrophes, a guarantee of people's confidence that they will be provided with assistance in emergency situations, which significantly reduces the level of their anxiety, anomic, propensity to unmotivated actions of a destructive plan.

7. Locating people in tunnels using Wi-Fi technology.

The aim of the project is to investigate the possibility of using people's mobile phones to locate people in a tunnel environment using the mobile phone's Wi-Fi connection. In total, 39 different trials were carried out under different conditions in a road tunnel in Stockholm, Sweden. In the trials, the Wi-Fi-based predicted location has been compared with the actual location of the recruited 16 participants in the tunnel. The conditions include the number of people in a group, the number of available access points in the tunnel, whether the mobile phone has an active or passive connection, whether a person is moving or standing still and whether the mobile phone is held in the hand or is stored in the person's pocket. The results indicate that the mean value for the distance between actual and predicted locations is in the order of 20 m or less, which is higher than reported in other studies. Despite this, there is a good potential to locate individuals in a tunnel emergency as the distance between emergency exits is often much longer than the uncertainties in the predicted locations of people. Improving the location algorithms will possibly reduce the uncertainty of the predicted location. • Wi-Fi technology can be used to locate people in a road and rail tunnel using people's smart phones. • Location precision is reasonable for the current purpose. • Experimental results are presented as evidence for evaluation of the technology. • Experiments compare real location using video recordings and Wi-Fi-predicted locations. • Better location algorithms have been suggested as one strategy to improve precision. 10. Societal Protection and Population Vulnerability: Key Factors in Explaining Community-Level Variation in Fatal Fires Involving Older Adults in Sweden.

Based on evidence from previous studies, fire mortality rates among older adults on a local level seem to be related two dimensions; population vulnerability and societal protection. In this paper, we investigate the magnitude of the impacts of variation along these two dimensions on the rate of fatal fires involving at least one older adult (65 + years) in Swedish municipalities. Our empirical strategy involves using principal components analysis to estimate the two dimensions based on data on the health of the older population, factors related to municipal rescue service efficiency, as well as other factors that may indirectly effect efficiency (such as urbanization rates and economic conditions). We then use Poisson regression models to study how mortality rates vary with changes along each dimension while keeping the other constant. The results support previous evidence suggesting that an efficient rescue service is important for low fire mortality rates. However, we also find evidence that variations in the vulnerability of the older population may play an equally important role. As such, the results indicate while improved societal protection is important in order to reduce fire-related mortality rates, merely focusing on these aspects will not eliminate the risk-increasing aspect of population vulnerability. In turn, this implies a need for a multifaceted approach to local fire mortality prevention that targets both rescue service efficiency and population vulnerability. [ABSTRACT FROM AUTHOR]

Training device prototype is primarily intended conditions research and the specifics of each part of the car and practical training cutting operations using a hydraulic recovery device. In practice, in delivering persons in traffic accidents, most often occurs to cut A, B, and C posts what they prove past experience and statistics. Since 2010 occurred several changes in the automotive industry from the point of view safety standards. Accelerate to incorporate high strength steel to vehicles is resulted in strengthening the important design elements of the car. The construction itself in this direction allows replacement of pillar samples the car with the handles suitable for the specific properties of the posts different types of vehicles. In the study program emergency services students will allow a training device various experiments during the final works, such as analysis of time frames of selected operations analysis of the time dependence of the individual posts, pressure data for cutting and other parameters. Next Generation Emergency Notification System after emergency call (eCall) equipped vehicle regulations with the legislation EU-2015/758 will be an obligation in EU in 2018 and eCall units are set up into the vehicles in Turkey after 2018. ‘Data Gathering by stage PSAP1 and resource Dispatching by stage PSAP2 in an integrated control room’ which is applied in Turkey will be modified via Turkey’s traffic management. Traffic and vehicle infrastructures will be updated after eCall equipped vehicle regulations. In this study, next generation emergency call system in vehicles is introduced and Optimal traffic infrastructure in Turkey is analysed via cost modeling.

The paper deals with problem of analyzing the changes between the extent of the territory, which is affected by the release of ammonia from the stationary source and the changing atmospheric conditions in this territory. To determine the extent of threatened area the ALOHA program environment as a spatial decision support tool was applied. As a scenario the release of ammonia from the tank, i.e. tank wagon staying at the freight railway station in Zvolen. The results of the analyses confirmed the fact the atmospheric conditions, mostly the wind speed has an important influence on the ammonia dispersion in the air its concentration and the extent of the threatened area. From the results is also evident that the cloud cover has no influence on gas dispersion in the air. Results of such analyses are further applicable to be included in the emergency plans and as a spatial decision support of crises managers at the local level, for creation of crises scenarios to be used in real time, etc. They are also applicable in the sphere of persons and property protection and also for the assignment of safety of the rescue services members acting at the site of intervention. Summary: Climate change poses a serious challenge to our health and wellbeing. The increasing frequency of extreme weather events such as floods, droughts, and heatwaves, and the direct impacts of changes in temperature have direct impacts on health. At the same time, broader environmental change affects infectious disease risk, air pollution, and other forms of exposure. The different ways in which climate change will affect health are complex, interactive, and different communities are disproportionately affected. International actions such as the Paris Agreement and the Sustainable Development Goals recognise the future risks to society and acknowledge that we are already committed to a certain level of climate change. Future adaptation measures therefore need careful assessment and implementation for us to be able to minimise the potential risks from climate change and, at the same time, maximise the potential health benefits of a cleaner, greener world. This Special Issue comprises original research articles and detailed reviews on the likely impacts of climate change on health in a range of geographical settings, and the potential for adaptation measures to reduce some of these risks. Ultimately, studies like these will motivate policy level action for mitigation and help in determining the most effective methods of adaptation to reduce negative impacts in future through embedding scientific evidence into practice.
21. Varför samverkar kommuner? : - Politikers incitament att samverka inom kommunal räddningstjänst

Will Sweden's municipal rescue services meet future challenges? How should small municipalities cope with increased requirements at the same time as the tax revenue is reduced? What are the incentives for municipalities to cooperate? The purpose of the thesis is to investigate what makes small municipalities choose to seek cooperation with other municipalities to solve their statutory task. In this essay, the municipal rescue service will be used as the case where the municipalities cooperate. The thesis has a theory-consuming method which means that the focus is on the different cases that are investigated. In the essay, the chairman of the municipal board in five smaller municipalities was interviewed to see what reasons, incentives and effects they see in their cooperation. The two incentives that clearly was higlighted by the municipalities are as follows:- To achieve sufficient quality of business- To improve the competence provision and recruitmentAll municipalities have chosen to highlight these areas as key incentives for why collaboration in municipal associations for the rescue service activities is important. In the interview part, most of the municipalities have highlighted these two areas that are difficult to manage on their own., Kommer Sveriges kommunala räddningstjänster klara framtida utmaningar? Hur ska små kommuner klara av ökade krav samtidigt som skatteunderlaget minskar? Vad finns det för incitament för kommuner att samarbeta? Syftet med uppsatsen är att undersöka vad som gör att kommuner väljer att söka samarbete med andra kommuner för att lösa sin lagstiftade uppgift. I detta fallet kommer kommunal räddningstjänst att användas som verksamhet där kommunerna samarbetar. Uppsatsen tittar på vilka incitament och vad för effekter som upplevs göra att samarbete mellan kommuner uppstår. Uppsatsen tittar på befintliga samarbeten/samverkan mellan kommuner som finns etablerad och vad som lyfts fram varför de väljer att samverka. Uppsatsen har en teorikonsumerande metod som innebär att fokus ligger på de olika fall som undersöks. Fokus i uppsatsen är att synliggöra effekter och incitament till att ingå i samverkan mellan kommuner och hur det uppstår. I uppsatsen intervjuades kommunstyrelsens ordförande i fem mindre kommunerna för att se vilka skäl, incitament och effekter som dom ser i sitt samarbete. Vid intervjuerna framkommer två områden som tydligt lyfts fram både i intervjudelen samt graderingen av incitament. Dessa två områden är följande: - Att uppnå tillräcklig kvalité på verksamheten - Att förbättra kompetensförsörjning och rekrytering Samtliga kommuner har valt att lyfta fram dessa områden som nyckelincitament för varför samverkan i kommunalförbund för räddningstjänstverksamheten är viktig. Responding to mass casualty incidents in a tunnel environment is problematic not least from a prehospital emergency medical services (EMS) perspective. The aim of this review was to 1) categorize preconditions for emergency response in tunnel environments based on Haddon’s matrix and 2) identify specific EMS knowledge of providing prehospital care. Twenty eight articles, reports and book chapters were selected for further analysis. Firstly, sorting the data from each included article was done according to Haddon’s matrix. The result covers human factors, technical factors, physical environmental factors and socioeconomic environmental factors all related to preconditions for emergency response. To describe the EMS’s knowledge the data was also sorted according to command and safety, communication, assessment, and triage treatment and transport, also known as CSCATT. Few studies, especially of high quality, actually provide detailed information regarding emergency response to tunnel incidents and those that do, often have a main focus on management by the rescue service. While many incidents studied were caused by fires in tunnels, thus requiring rescue service in action, the subsequent EMS response issues that have taken place appear to have been given limited attention. To optimize the survival rates and health of the injured, as well as to provide a safe and effective work environment for the emergency services, there is a need to explore the event phase.

Es ist bekannt, dass die Qualit��t medizinischer Ma��nahmen abh��ngig von der Tageszeit sein kann. Dies trifft m��glicherweise auch auf Ma��nahmen in der rettungsmedizinischen Versorgung zu. Im Rettungsdienst kommen verschiedene Berufsgruppen zum Einsatz, so gibt es beispielsweise in Deutschland ein fl��chendeckendes Notarztsystem, w��hrend in Finnland in weiten Teilen Sanit��ter prim��r eigenverantwortlich t��tig sind. Sowohl in not��rztlichen als auch paramedizinischen Systemen besteht bei den ausf��hrenden Berufsgruppen Unsicherheit bez��glich der p��diatrischen Versorgung. Diese Arbeit untersucht deshalb am Beispiel der Reanimation, ob es einen tagezeitlichen Unterschied im Outcome nach pr��klinisch rettungsdienstlich versorgten Reanimationen gibt und welche p��diatrischen Notf��lle in der rettungsdienstlichen Versorgung angetroffen werden. Anschlie��end werden Untersuchungsergebnisse aus zwei Rettungsdienstsystemen dargestellt und ein L��ndervergleich zwischen einem not��rztlich orientiertem Versorgungssystem und einem paramedizinisch orientiertem Versorgungssystem mit dem Schwerpunkt auf Reanimationen und die p��diatrische Versorgung vorgenommen. Es wurden 469 Reanimationseins��tze und 215 p��diatrische Notf��lle eines Berliner Notarztfahrzeuges retrospektiv analysiert. Insgesamt konnten 90 Fragebogen-Antworten von deutschen Not��rzten und 81 von finnischen Sanit��tern zur Selbsteinsch��tzung der rettungsmedizinischen Erfahrung und F��higkeiten ausgewertet werden. Wesentliche Ergebnisse sind, dass es keinen tageszeitlichen Unterschied im prim��ren ROSC (return of spontaneous circulation) gibt, jedoch zu Off-Hour-Zeiten (16-8 Uhr) im Vergleich zu On-Hour-Zeiten (8-16 Uhr) deutlich h��ufiger Patienten mit einem Herz-Kreislauf-Stillstand an nicht ��ffentlichen Orten angetroffen werden (44 % gegen��ber 21 %). Pr��klinisch ist das h��ufigste p��diatrische Krankheitsbild der Krampfanfall (28,37 %), davon 52,5 % als Fieberkrampf eingeordnet. In 3,26 % der p��diatrischen Eins��tze lag ein Herz-Kreislauf-Stillstand als Krankheitsbild vor. Im Vergleich zwischen Not��rzten und Sanit��tern geben die Not��rzte in der Befragung an, ��ber mehr Erfahrung u.a. in der Versorgung von p��diatrischen Reanimationen zu verf��gen und sch��tzen ihre F��higkeiten als besser ein als die paramedizinische Gruppe. 83,9 % der Not��rzte und 98,7 % der Sanit��ter w��nschen sich eine Fortbildung in der p��diatrischen Notfallversorgung. Auf den Ergebnissen basierend wird ein Vorteil f��r die not��rztliche Gruppe gesehen. Die p��diatrische Notfallversorgung sollte bessere Ber��cksichtigung in der Fortbildungsplanung finden. There are several occupational groups working in rescue services: for example, Germany has a nationwide emergency physician system, while in Finland large numbers of rescue assignments are covered by paramedics. Emergency physicians and paramedics likewise experience generally uncertainty in paediatric emergency care. This study investigates, in the context of OHCA (out-of-hospital cardiac arrest), whether there is a difference in the outcome of medical interventions by rescue services based on the time of day and which paediatric emergencies are encountered by rescue services. The results of two different rescue service systems are presented, along with a country comparison between an emergency physician-oriented and a paramedic-oriented system with a focus on OHCA and paediatric care. Two-hundred-and-fifteen paediatric emergency missions and four-hundred-and-sixty-nine resuscitation missions of ambulance-based emergency physicians in Berlin have been analysed. Ninety responses to a questionnaire by German emergency physicians and eighty-one by Finnish paramedics have also been evaluated. There is no difference in primary ROSC (return of spontaneous circulation) based on the time of day, but during out-of-hours times (16:00-08:00) distinctly more OHCA patients are encountered in non-public places (44% vs. 21%). The most frequent paediatric emergency is a seizure (28.37%), of which 52.5% are classified preclinically as a febrile seizure. In 3.26% of the calls a resuscitation was the cause of the intervention. The Finnish paramedical group was less experienced in most categories and rated their own emergency care skills lower than the German emergency physician group. 83.9% of the German emergency doctors and 98.7% of the Finnish paramedics have sought further training in the care of paediatric emergencies. Based on the results, there is a marked difference in favour of the emergency physician group. Paediatric emergency care should be considered better in training planning. In both study groups, fewer treatment problems and errors were reported than expected based on literature research, which may point to the failure to implement a culture of continuous improvement. In summary, there is no difference in primary ROSC in OHCA based on time of day. The emergency physician system appears to have advantages compared to a paramedic-oriented emergency medical system. Paediatric emergency care is a challenge to many rescue service professionals and the implementation of a learning culture in rescue services offers potential for improvement.

To facilitate resilience to a changing climate, it is necessary to go beyond quantitative studies and take an in-depth look at the functioning of health systems and the variety of drivers shaping its effectiveness. We clarify the factors determining the effectiveness of the Estonian health system in assessing and managing the health risks of climate change. Document analyses, expert interviews with key informants from health systems whose responsibilities are relevant to climate change, and analysis of a population-based survey conducted in 2015, indicate that the health effects of climate change have not been mainstreamed into policy. Therefore, many of the potential synergistic effects of combining information on health systems, environment, and vulnerable populations remain unexploited. The limited uptake of the issue of climate change-related health risks may be attributed to the lack of experience with managing extreme weather events; limited understanding of how to incorporate projections of longer-term health risks into policies and plans; unclear divisions of responsibility; and market liberal state approaches. Minority groups and urban dwellers are placing strong pressure on the health system to address climate change-related risks, likely due to their lower levels of perceived control over their physical wellbeing. The results have implications for national, community, and individual resilience in upper-middle income countries in Eastern Europe.

This article builds on the notion that, in order to meet the increasing management requirement for data, successful in-house accountancy professionals are internalising ever more capabilities and responsibilities, thereby reflecting a partnership role in professions related to basic operations. The study investigates the organisational interfaces of the accountancy profession vis-à-vis other professions during the change process. The findings identify some possible factors contributing to the traditional way of producing and utilising accounting information, and suggest some possible measures to be used as key facilitators in starting a change process aimed at securing an enlarged, more customer-oriented and highly interactive role for accountancy. The contribution of the study lies in increasing our understanding of surplus-value-producing inter-professional development processes. In addition, based on the case of rescue services the study provides a holistic view on accountancy knowledge management in public-sector services provided in inter-municipal cooperation.

27. Factors contributing to survival and evacuation in residential fires involving older adults in Sweden

It has been known for a long time that older adults suffer a significantly higher risk of dying in residential fires compared to younger people. The characteristics of these fatal fires and the fatalities are also well known. However, less is known about older adults who survive fires and how they differ from those who die. This distinction can be assumed to be of great importance when designing effective prevention efforts. In the current paper, factors that contribute to survival of older adults (65+) has been investigated together with factors that contribute to different modes of evacuation (e.g. evacuation assisted by neighbours) based, primarily, on incident reports. The results show that fewer than half of the victims (39%) evacuated independently and many rely on evacuation assisted by neighbours (18%), first-responders (27%) or homecare personnel (8%). Living in urban areas was found to increase the odds of survival. Based on the results from the analysis of evacuation, this is likely due to a combination of proximity to neighbours and a short response time. For successful evacuation by non-firefighters, the fires often had to be confined to the object of ignition while the rescue service could evacuate from larger fires.

28. Tactical Depressurization of Hydrogen and CNG Tanks Using Rifles and Other Projectiles

After a tank has been exposed to crash violence, or an external fire, it might, in some situations, be judged dangerous to move the vessel due to the risk of a sudden tank rupture. Therefore, Swedish rescue services have a long history of using rifles to penetrate and therefore depressurize the vessels. In this paper, some first steps on providing guidance on the selection of ammunition and required stand back distance are presented. The results indicate that a stand back distance on the order of 100 m is required and that the standard 7.62 Ball should only be used for composite CNG-tanks, while stronger ammunitions are needed for steel and composite hydrogen tanks. However, more research is required to provide a more solid scientific underpinning of the tactic guidance.

Hintergrund Zur Eindämmung der COVID-19-Pandemie wurde das öffentliche Leben in Deutschland durch Kontaktbeschränkungsmaßnahmen (im weiteren Verlauf zur Lesevereinfachung als „Lockdown“ bezeichnet) reduziert, u. a. um Ressourcen des Gesundheitssystems für die Behandlung von COVID-19-Patienten zur Verfügung zu stellen. Parallel dazu konnte man im öffentlichen Gesundheitswesen einen Rückgang von Notfallpatienten feststellen. Methode Für zwei 10-wöchige Zeitspannen vor und während des Lockdowns wurden die Einsatzzahlen des Rettungsdiensts in 6 Rettungsdienstbereichen in der Metropolregion Rhein-Main für 6 Tracerdiagnosen analysiert. Zufällige Effekte wurden durch einen Vergleich mit den entsprechenden Zeitspannen 2018 und 2019 und einem errechneten Erwartungswert minimiert. Ergebnisse Für Notfalleinsätze des Rettungsdiensts kam es während des Lockdowns zu einer Verminderung der Einsatzzahlen (−16 %). Eine Reduktion unter dem Stichwort kardiale und zerebrale Ischämien fand sich in 20 %. Im Stadtgebiet Frankfurt am Main war der Rückgang bei kardialen Ischämien mit 14 % geringer ausgeprägt als im Umland mit 23 %. Die Einsatzzahlen für Intoxikationen gingen um 27 %, für psychiatrische Notfälle um 16 % zurück. Schlussfolgerung Der öffentliche Rettungsdienst wurde durch die COVID-19-Pandemie zahlenmäßig nicht überbelastet, es kam während des Lockdowns zu einem Rückgang der Einsätze. Für die Reduktion bei den zerebralen oder kardialen Ischämien fehlen die Erklärungsmodelle für den Einfluss des Lockdowns. Weitere Studien zum Nutzungsverhalten des Rettungsdiensts während eines Lockdowns erscheinen notwendig, um für das Patientenoutcome möglicherweise fatale Mindernutzungen zu detektieren und durch Aufklärung gegensteuern zu können.

Background: To contain the coronavirus disease (COVID-19) pandemic, public life was reduced through contact restriction measures (referred to as "lockdown" in the further course for reading simplicity), among other things to make health system resources available for the treatment of COVID-19 patients. In parallel, a decrease in emergency patients was observed in the public health system., Methods: For two 10-week periods before and during the lockdown, ambulance service deployment rates were analysed in 6 ambulance service areas for 6 tracer diagnoses. Random effects were minimised by comparing the results with the corresponding 2018 and 2019 time periods and a calculated expected value., Results: For emergency ambulance service calls, there was a reduction in call numbers (-16%) during the lockdown. A 20% reduction for the categories cardiac and cerebral ischaemia was found. In the urban area, the reduction in cardiac ischaemia was less pronounced at 14% than in the surrounding area at 23%. The deployment figures for intoxications decreased by 27% and for psychiatric emergencies by 16%., Conclusion: The public ambulance service was not overwhelmed by the COVID-19 pandemic; there was a decrease in depolyments during the lockdown. For the reduction in cerebral or cardiac ischaemias, the explanatory models for the influence of the lockdown are missing. Further studies on the utilisation behaviour of the ambulance service during a lockdown appear necessary in order to detect potentially fatal reductions in utilisation for the patient outcome and to be able to counteract them through education., (© Springer Medizin Verlag GmbH, ein Teil von Springer Nature 2021.)

With current, sudden climate changes, an increase in the frequency of various natural disasters can be observed which affects populated, urban areas. This may cause the need to increase the number of rescuers so that they can respond appropriately to various disasters or the need to use new technologies that will help in efficient and safe disaster management, especially in difficult terrain. This article focuses on the second approach pointing to the most important advantages and possibilities offered by new technologies during disaster mitigation such as: • Measurement of air pollution using an unmanned aerial vehicle (UAV), • Measurement of life parameters of rescuers, • Checking the location of rescuer and visualization on the map for better action management and situational awareness. The above-mentioned aspects have an impact on the organization of the airspace and air traffic management. Attention should be given to the necessity of coordinate flights of unmanned aerial vehicles with manned aviation (extinguishing operations, flights with the SAR status).

Based on evidence from previous studies, fire mortality rates among older adults on a local level seem to be related two dimensions; population vulnerability and societal protection. In this paper, we investigate the magnitude of the impacts of variation along these two dimensions on the rate of fatal fires involving at least one older adult (65 + years) in Swedish municipalities. Our empirical strategy involves using principal components analysis to estimate the two dimensions based on data on the health of the older population, factors related to municipal rescue service efficiency, as well as other factors that may indirectly effect efficiency (such as urbanization rates and economic conditions). We then use Poisson regression models to study how mortality rates vary with changes along each dimension while keeping the other constant. The results support previous evidence suggesting that an efficient rescue service is important for low fire mortality rates. However, we also find evidence that variations in the vulnerability of the older population may play an equally important role. As such, the results indicate while improved societal protection is important in order to reduce fire-related mortality rates, merely focusing on these aspects will not eliminate the risk-increasing aspect of population vulnerability. In turn, this implies a need for a multifaceted approach to local fire mortality prevention that targets both rescue service efficiency and population vulnerability.

36. Limited medical perspective at a strategic level in relation to mass casualty incidents in Swedish tunnels

Mass casualty incidents (MCI) in tunnels can result in devastating consequences. Despite this, there is a knowledge gap in the perspectives and experiences of strategic stakeholders in relation to the emergency medical response to MCI in tunnels. This study aims to explore and describe this issue. The study includes 11 interview participants from the emergency services organisations and governmental and municipal organisations in Sweden. The results indicate that the medical perspective is inadequately represented at a strategic level due, among other, to limited input from emergency medical representatives during planning stages and a lack of social networks. The results implicate a need to create and improve networks among the stakeholders from the involved organisations. Emergency medical personnel should not only be acknowledged for their expertise but should also be included to a greater extent in the planning stages and exercises. Medical and rescue personnel need time to jointly discuss how they, in the best way, can save lives in tunnel MCIs.

Myndigheten för samhällsskydd och beredskap (MSB) investerar årligen cirka 120 miljoner kronor på forskning. I MSB:s forskningsstrategi ingår att forskningen ska nyttiggöras genom bland annat innovationer, främst i form av varor och tjänster. Men samtidigt som förväntningarna är höga på nyttiggörande, saknas myndighetsdirektiv för hur krishanteringsförmågan ska kunna förbättras genom utveckling av innovationer. MSB beslutade därför 2016 att utlysa medel för att satsa på att utveckla en så kallad "innovationsmekanism". Det var ett försök att som en central aktör i det svenska krishanteringssystemet stärka nyttiggörandet av forskningsresultat inom svensk räddningstjänst. FOI fick i uppdrag att stödja arbetet med aktionsforskningsmetodik, att dokumentera och analysera förloppet för att bidra till aktörernas lärande under och efter försöksverksamheten. Resultatet visar att utveckling av en innovationsmekanism inte är någon enkel process, inte minst eftersom de involverade aktörernas intressen om vad som anses värdefullt förändras över tid. Denna rapport bidrar med ny kunskap om hur ledning av innovationsmekanismer organiseras. Kunskapen kan vara tillämpbar på andra samhällsutmaningar, vilket exemplifieras i rapportens avslutande diskussion., Saving lives and mitigating the negative impact on public rights requires continuous societal crisis capability renewal. Therefore, the Swedish Civil Contingencies Agency (MSB) annually invests 120 MSEK in research. The agency's research strategy values the utilization of the results of the research it finances, for instance, new innovations such as products and services. Despite this expectation, governance in terms of instructions and rules for reinvigorating societal crisis capability is largely missing. In 2016, MSB therefore decided to develop an "innovation mechanism." In turn, the Swedish defense research agency FOI was assigned by MSB to support the work by using actions research methodology. FOI was to document the process and thereby add to the innovation mechanism in terms of stakeholder learnings during as well as after the pilot phase. The results show that the development of an innovation mechanism is not a simple process, not the least as interest on what is considered valuable changes continuously. This report contributes to new knowledge on the organization of innovation mechanisms and has implications for other areas of society that need innovation.

Настоящее исследование направлено на выявление признаков социокультурной идентичности работников службы спасения региона на основе анализа уровня профессиональной культуры. Актуальность темы обусловлена тем, что от эффективности служб спасения во многом зависит социальная стабильность в стране и отдельном регионе. Особая роль в эффективности спасательных служб отводится непосредственным исполнителям, участвующим в преодолении катастроф и их последствий служащим специальных поисково-спасательных отрядов (ПСО). Внешняя сторона деятельности службы спасения достаточно широко освещается в СМИ, особенно в периоды наступления чрезвычайных ситуаций. Личностные характеристики и особенности взаимодействия участников спасательных операций, в совокупности со специальными знаниями и навыками составляющие основу профессиональной культуры, часто остаются за кадром. Методология исследования основывается на институциональном и структурно-функциональном подходах к анализу деятельности спасателей. Эмпирический социологические данные получены методом социологического опроса генеральной совокупности Поисково-спасательного отряда Главного управления МЧС по Республике Адыгея. В результате проведенного исследования были получены интегративная модель личности спасателя, а также данные, свидетельствующие о высоком уровне профессиональной культуры и адекватной социокультурной идентичности работников ПСО. Показателем профессиональной культуры является понимание работниками ценности профессиональной подготовки для спасательных мероприятий. У большинства работников в процессе службы сложилось ценностное отношение к человеческой жизни как в чрезвычайных ситуациях, так и в условиях повседневности. Понимание ведущей роли государства в обеспечении эффективной деятельности служб спасения и высокая самооценка деятельности служб спасения свидетельствуют о стабильности структуры и эффективности деятельности МЧС., The research reveals the signs of sociocultural identity of rescue officers of the region on the basis of the analysis of professional culture level. The novelty of the research topic consists in the fact that social stability in the country in general and in the region particularly depends on the effectiveness of rescue services. A special role in the effectiveness of rescue services is assigned to executors involved in overcoming catastrophes and their consequences-employees of special search and rescue squads (SRS). The externality of the rescue service is widely covered in mass media, especially in emergency situations. Personal characteristics and features of interaction ofparticipants in rescue operations, together with special knowledge and skills that form the basis of professional culture, often remain behind the scenes. The research methodology is based on institutional and structural and functional approaches to the analysis of rescuers' activities. Empirical sociological data obtained by a sociological survey of the general totality of the search and rescue squad of the General Directorate of the Ministry of Emergency Situations in the Republic of Adygeya. As a result of the research, an integrative model of a rescuer’s personality was obtained, as well as data indicating a high level of professional culture and an adequate socio-cultural identity of SRS officers. An indicator of professional culture is understanding of the value of professional training for rescue activities by workers. Most of the officers have developed a valuable attitude towards human life both in emergency situations and in the conditions of everyday life in the course of their service. Understanding the leading role of the state in ensuring effective operation of rescue services and high self-esteem of rescue services indicate the stability of the structure and effectiveness of the activities of the Emergencies Ministry.

The manifestation of a new pathogen, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), constitutes a new problem for modern health care systems. Developing updated standards for all emergency services working at an accident site during the pandemic has been a continuous challenge. The principal method of preventing the transmission of the SARS-CoV-2 virus is the use of personal protective equipment, such as protective suits, masks and goggles, or face shields. The study aims to present the recommended on-site procedures during the coronavirus pandemic based on the description of an accident of an ambulance transporting a patient with confirmed SARS-CoV-2 infection, emphasizing the actions taken by the emergency services sent to the accident site.

43. Factors contributing to survival and evacuation in residential fires involving older adults in Sweden.

It has been known for a long time that older adults suffer a significantly higher risk of dying in residential fires compared to younger people. The characteristics of these fatal fires and the fatalities are also well known. However, less is known about older adults who survive fires and how they differ from those who die. This distinction can be assumed to be of great importance when designing effective prevention efforts. In the current paper, factors that contribute to survival of older adults (65+) has been investigated together with factors that contribute to different modes of evacuation (e.g. evacuation assisted by neighbours) based, primarily, on incident reports. The results show that fewer than half of the victims (39%) evacuated independently and many rely on evacuation assisted by neighbours (18%), first-responders (27%) or homecare personnel (8%). Living in urban areas was found to increase the odds of survival. Based on the results from the analysis of evacuation, this is likely due to a combination of proximity to neighbours and a short response time. For successful evacuation by non-firefighters, the fires often had to be confined to the object of ignition while the rescue service could evacuate from larger fires. • Proximity to neighbours and rescue services increases the probability of rescue. • At least 53% of the older adults depended on other actors for evacuation. • Most important actor was first-responders followed by neighbours. • The general public need to be alerted that they may need to evacuate others. • The increase in home-dwelling older adults need to be problematized. To facilitate resilience to a changing climate, it is necessary to go beyond quantitative studies and take an in-depth look at the functioning of health systems and the variety of drivers shaping its effectiveness. We clarify the factors determining the effectiveness of the Estonian health system in assessing and managing the health risks of climate change. Document analyses, expert interviews with key informants from health systems whose responsibilities are relevant to climate change, and analysis of a population-based survey conducted in 2015, indicate that the health effects of climate change have not been mainstreamed into policy. Therefore, many of the potential synergistic effects of combining information on health systems, environment, and vulnerable populations remain unexploited. The limited uptake of the issue of climate change-related health risks may be attributed to the lack of experience with managing extreme weather events; limited understanding of how to incorporate projections of longer-term health risks into policies and plans; unclear divisions of responsibility; and market liberal state approaches. Minority groups and urban dwellers are placing strong pressure on the health system to address climate change-related risks, likely due to their lower levels of perceived control over their physical wellbeing. The results have implications for national, community, and individual resilience in upper-middle income countries in Eastern Europe.

45. Review of On-Scene Management of Mass-Casualty Attacks

Background: The scene of a mass-casualty attack (MCA) entails a crime scene, a hazardous space, and a great number of people needing medical assistance. Public transportation has been the target of such attacks and involves a high probability of generating mass casualties. The review aimed to investigate challenges for on-scene responses to MCAs and suggestions made to counter these challenges, with special attention given to attacks on public transportation and associated terminals. Methods: Articles were found through PubMed and Scopus, "relevant articles" as defined by the databases, and a manual search of references. Inclusion criteria were that the article referred to attack(s) and/or a public transportation-related incident and issues concerning formal on-scene response. An appraisal of the articles' scientific quality was conducted based on an evidence hierarchy model developed for the study. Results: One hundred and five articles were reviewed. Challenges for command and coordination on scene included establishing leadership, inter-agency collaboration, multiple incident sites, and logistics. Safety issues entailed knowledge and use of personal protective equipment, risk awareness and expectations, cordons, dynamic risk assessment, defensive versus offensive approaches, and joining forces. Communication concerns were equipment shortfalls, dialoguing, and providing information. Assessment problems were scene layout and interpreting environmental indicators as well as understanding setting-driven needs for specialist skills and resources. Triage and treatment difficulties included differing triage systems, directing casualties, uncommon injuries, field hospitals, level of care, providing psychological and pediatric care. Transportation hardships included scene access, distance to hospitals, and distribution of casualties. Ex ante, võttes arvesse oodatud nõudlust, võib ressursside jaotus olla optimaalne, kuid mitte ex post, kui on teada reaalne nõudluse tase. Doktoritöö eesmärgiks oli välja töötada teoreetiline kontseptsioon ja rakendus, kuidas hinnata sooritust riigiasutuses, mis toimib ebakindla nõudluse tingimustes. Et hinnata mitme üksusega ja mitmel tasandil toimivat teenuseid osutavat riigiasutust ebakindla nõudluse tingimustes, loodi süsteem, milles hinnati (a) keskse riigiasutuse kulutõhusust, (b) allüksuste alavarustatust ja (c) allüksuste väljundtõhusust. Soorituse hindamiseks kasutati piirianalüüsi meetodeid (DEA, FDH, DFA) ning kontseptsiooni rakendati Eesti, Soome ja Rootsi päästevaldkonna näitel. Tulemused ja tähtsus. Päästeteenuseid pakutakse mitmete allüksuste poolt erinevates piirkondades. Teenuste osutaja otsustab esialgselt, kuidas ressursid (päästjad, masinad) erinevates piirkondades jaotada, teadmata seejuures, kui palju päästesündmusi seal tegelikult aset leiab. Allüksused hoiavad teatud valmisoleku taset, et vajaduse korral päästesündmusele reageerida. When planning the allocation of resources in public agencies, the demand for services is often unknown and prone to uncertainty. Without having the full information of the possible demand, the decision maker will insure oneself with additional standby capacity against upsurges in demand. Cost and efficiency studies predominantly assume known demand, which is unrealistic and hinders understanding the essence of service provision in public agencies. In many cases, it has probably resulted in underestimation of efficiency. The observed excess capacity can partly be explained by risk averse behaviour and should be incorporated to the analysis to avoid unjustly labelling such input usage as inefficiency. Ex ante, given expected demand, the resource allocation is optimal, but not ex post, given realised levels of demand. The challenge is to distinguish the necessary standby capacity from excessive mismanagement. This thesis develops the theoretical concept and application to measure the performance of public agencies in the case of demand uncertainty. To evaluate the efficiency of a multi-unit and multilevel service providing public agency in the presence of demand uncertainty, one is interested in: (a) the cost-efficiency of the central agency, (b) any under-resourcing of subunits, and (c) the output-oriented technical efficiency of each subunit in jurisdictions. The suggested models would be the basis for planning resource allocation improvement in public agencies. The models are implemented using the example of the Estonian, Finnish and Swedish fire and rescue services. For estimation, different frontier analysis methods (DEA, FDH, DFA) are proposed. Result and Benefit. Typically, the rescue authority decides beforehand to allocate resources (rescuers, vehicles, etc.) between different subunits in jurisdictions without knowing how many emergencies will occur. The subunits must maintain a certain level of standby capacity to be able to respond to emergencies. Sustaining the readiness, however, is the most expensive component in the running costs. The results show convincingly that the models taking the demand uncertainty in the form of minimum service level into account, estimate a higher cost-efficiency irrespective of the estimation method or country (with different level of centralisation for management) analysed.

Objective: to investigate the scientific productions on the working conditions of nursing staff in the emergency and urgent care sector. Method: integrative literature review including scientific articles indexed in the Virtual Health Library databases over the past five years (2010-2014). Results: Among the 87 articles identified, 13 were selected productions, whose approach has raised two categories: capacity and working conditions; Risks and accidents. Conclusion: the analyzed articles revealed inadequate working conditions to which nursing professionals are exposed daily in urgent and emergency services, which put them at risk of acquiring infectious diseases, exposure to physical and verbal violence, occupational stress, among others. In addition, it was found that the exhaustive hours and low pay are factors responsible for wear in the workplace.
