Abstract

Stereotactic radiosurgery under local anesthesia with the Leksell Gamma Knife can effectively treat some patients with recurrent tic douloureux after unsuccessful medical/surgical procedures. Seven of 12 patients have shown complete relief or improvement of their trigeminal neuralgia. No complications have been observed.

Abstract

The Leksell Gamma Knife is a useful and safe method to perform thalamotomy and pallidotomy in selected older patients with Parkinson's disease and related movement disorders. In this preliminary report, 2 of 3 patients with severe intention tremor were relieved of their symptoms by thalamotomy, as were 4 of 7 patients with Parkinson's tremor. Four of 8 patients had significant improvement of contralateral rigidity following pallidotomy.

Abstract

Fifteen patients were treated in the Gamma Knife Unit and followed for 18 months or longer. Four patients had Cushing's disease, 4 had acromegaly, 3 had Nelson's syndrome and 3 had prolactinomas. One patient had no endocrinopathy. One of the patients with acromegaly and 2 of those with prolactinomas had been operated prior to Gamma Knife treatment. Radiological tumor localization was not an insuperable problem in this series. The effect of Gamma Knife treatment on the anterior pituitary neoplasia, as such, was consistently successful. All the tumors which received 10 Gy or more to the edge showed either a reduction in volume or at least cessation of growth. On the other hand, the effect of the treatment was less consistent in respect to the endocrinopathies. These results are discussed in respect of dose and tumor size. It is suggested that the role of the Gamma Knife in the treatment of pituitary adenomas requires further clarification, based on prospective studies.

Abstract

✓ A multiinstitutional study was conducted to evaluate the technique, dose-selection parameters, and results of gamma knife stereotactic radiosurgery in the management of trigeminal neuralgia. Fifty patients at five centers underwent radiosurgery performed with a single 4-mm isocenter targeted at the nerve root entry zone. Thirty-two patients had undergone prior surgery, and the mean number of procedures that had been performed was 2.8 (range 1–7). The target dose of the radiosurgery used in the current study varied from 60 to 90 Gy. The median follow-up period after radiosurgery was 18 months (range 11–36 months). Twenty-nine patients (58%) responded with excellent control (pain free), 18 (36%) obtained good control (50%–90% relief), and three (6%) experienced treatment failure. The median time to pain relief was 1 month (range 1 day–6.7 months). Responses remained consistent for up to 3 years postradiosurgery in all cases except three (6%) in which the patients had pain recurrence at 5, 7, and 10 months. At 2 years, 54% of patients were pain free and 88% had 50% to 100% relief. A maximum radiosurgical dose of 70 Gy or greater was associated with a significantly greater chance of complete pain relief (72% vs. 9%, p = 0.0003). Three patients (6%) developed increased facial paresthesia after radiosurgery, which resolved totally in one case and improved in another. No patient developed other deficits or deafferentation pain. The proximal trigeminal nerve and root entry zone, which is well defined on magnetic resonance imaging, is an appropriate anatomical target for radiosurgery. Radiosurgery using the gamma unit is an additional effective surgical approach for the management of medically or surgically refractory trigeminal neuralgia. A longer-term follow-up review is warranted.

Abstract

Twenty patients who suffered persistent intractable pain from a variety of disorders underwent medial thalamotomy with the Leksell Gamma Knife. The lesions were directed at the intralaminar nuclei, the lateral portion of the medial dorsal nucleus, the centromedian and the parafascicular nuclei. Lesions were made with radiation doses from 140 to 180 Gy using a 4-mm beam collimator helmet and either a single isocenter (1 patient), two isocenters (17 patients) or three isocenters (2 patients). Two thirds of the patients experienced either excellent or good pain relief in a follow-up period between 1 and 22 months. One patient showed temporary complications which resolved, and 2 other patients also suffered complications which currently are improving. One patient died due to radiation necrosis following a bilateral thalamotomy. Gamma Knife thalamotomy may offer a potentially safe and effective alternative for the treatment of certain difficult persistent pain problems.

Abstract

A technique for the experimental study of tremor is described. Injection of oxotremorine into acutely decorticated cats consistently induced a generalized tremor. The electromyographic analysis showed that the induced tremor was regular with simultaneous contractions of flexor and extensor muscles, although extensors were more affected. The frequency spectrum was wide (8–28 c/s). Tremors started first in the head and neck muscles and were visible during one to two hours as either short or long bursts, with or without waxing and waning characteristics. Frequency spectrum, phase relations of flexor and extensor activity and mode of onset of oxotremorine tremors were similar to those observed in shivering. However, atropine sulfate blocked oxotremorine tremors as it does parkinsonian tremor but it does not affect shivering. Pentobarbital in subanesthetic and anesthetic doses was able to block oxotremorine tremor. Tolerance to oxotremorine appeared to develop with administration of successive doses.

Abstract

The activity of the gastrocnemius medialis (GM) alpha motoneuron pool was studied in acutely decorticated cats during Oxotremorine-induced tremor. The output of the motoneuron pool was monitored by recording efferent activity in a fine filament of GM nerve while taking an electromyogram and recording isometric tension of the muscle. The results show that the GM alpha motoneuron pool sends rhythmical and grouped discharges (responsible for the tremor) only through cells with axons of large diameter; however, if the excitatory drive is strong enough, the same motoneurons will fire steadily, with the corresponding disappearance of tremor. A given motoneuron does not always fire in successive contractions, i.e., there is rotation among the active cells of the pool regarding their contribution to tremor; thus, a mechanism for sustained oscillatory activity without loss of tension due to fatigue is provided. The alpha motoneurons with axons of small diameter do not participate in this oscillatory activity (tremor), but fire continuously. In cases in which gamma spikes are clearly identified, a similar lack of grouped, rhythmical discharges is observed.

We propose a new technique to accurately determine the volume of cerebellopontine angle (CPA) tumors. It has been determined that the measurement of the long axis in a slice plane of the CPA tumor does not adequately measure the total growth of the tumor. Volume measurements are more accurate indicators of the mass of the tumor.
