A Litopenaeus vannamei injected with the serum from the shrimps infected with WSSV-showed the typical symptoms of white spot syndrome at 48hpi. These include lethargy, lack of appetite, reduced and disoriented swimming activity. The negative control and serum from healthy shrimps did not show any band and the test samples contained a band corresponding to 310 bp which was like that of the positive control. Thus, the test samples were confirmed positive for the WSSV-infection. The mean serum protein concentration in healthy shrimp was 86.4 ± 6.3 mg/ml. on the other hand the serum protein concentration of WSSV-injected shrimps declined linearly and significantly to 58.8 ± 3.9, 46.2 ± 5.32, and 38.7 ± 6.7 mg/ml at 12, 24, and 48 hpi, respectively. Relative quantity of hemocyanin gene (I) expression in WSSV-injected shrimps increased gradually from 6 to 48 hpi. Analyses of relative quantification of hemocyanin gene (II) in the experimental groups showed that the relative quantity in WSSV- injected shrimps increased from 6 hpi up to 24 hpi, whereas at 48 hpi the WSSV-injected shrimps showed a decrease in relative quantity of hemocyanin gene (II). Analyses of relative quantification of hemocyanin gene (III) in the experimental groups showed that the relative quantity in WSSV-injected shrimps increased slowly from 6 to 12 hpi and the increase was found to be high at 24 and 48 hpi. The relative quantity increased in sham-injected shrimps. The melt curve analysis showed two peaks corresponding to 18S rRNA and hemocyanin gene I, II and III, respectively. The objective of the present study was directed towards the relative quantification of the hemocyanin genes (I, II and III) in L. vannamei upon WSSV-infection.

We evaluated survival to hospital discharge in children with heart disease (HD), where calcium is more frequently administered during CPR.Retrospective study of a multicenter registry database.Data reported to the American Heart Association's (AHA) Get With The Guidelines-Resuscitation registry.Children younger than 18 years with HD experiencing an index IHCA event requiring CPR between January 2000 and January 2019. Using propensity score matching (PSM), we selected matched cohorts of children receiving and not receiving IV calcium during CPR and compared the primary outcome of survival to hospital discharge.None.We included 4,556 children with HD experiencing IHCA. Calcium was administered in 1,986 (44%), more frequently in children younger than 1 year old (65% vs 35%; p0.001) and surgical cardiac (SC) compared with medical cardiac patients (51% vs 36%; p0.001). Calcium administration during CPR was associated with longer duration CPR (median 27 min [interquartile range (IQR): 10-50 min] vs 5 min [IQR, 2-16 min]; p0.001) and more frequent extracorporeal-CPR deployment (25% vs 8%; p0.001). In the PSM cohort, those receiving calcium had decreased survival to hospital discharge (39% vs 46%; p = 0.02) compared with those not receiving calcium. In a subgroup analysis, decreased discharge survival was only seen in SC cohorts.Calcium administration during CPR for children with HD experiencing IHCA is common and is associated with worse survival. Administration of calcium during CPR in children with HD should be restricted to specific indications as recommended by the AHA CPR guidelines.

Research evaluating hemostatic agents for the treatment of clinically significant bleeding has been hampered by inconsistency and lack of standardized primary clinical trial outcomes. Clinical trials of hemostatic agents in both cardiac surgery and mechanical circulatory support, such as extracorporeal membrane oxygenation and ventricular assist devices, are examples of studies that lack implementation of universally accepted outcomes.A subgroup of experts convened by the National Heart, Lung, and Blood Institute and the US Department of Defense developed consensus recommendations for primary outcomes in cardiac surgery and mechanical circulatory support.For cardiac surgery the primary efficacy endpoint of total allogeneic blood products (units vs mL/kg for pediatric patients) administered intraoperatively and postoperatively through day 5 or hospital discharge is recommended. For mechanical circulatory support outside the perioperative period the recommended primary outcome for extracorporeal membrane oxygenation is a 5-point ordinal score of thrombosis and bleeding severity adapted from the Common Terminology Criteria for Adverse Events version 5.0. The recommended primary endpoint for ventricular assist device is freedom from disabling stroke (Common Terminology Criteria for Adverse Events AE ≥ grade 3) through day 180.The proposed composite risk scores could impact the design of upcoming clinical trials and enable comparability of future investigations. Harmonizing and disseminating global consensus definitions and management guidelines can also reduce patient heterogeneity that would confound standardized primary outcomes in future research.

Severe forms of pulmonary embolism (PE) in children are rare but cause significant morbidity and mortality. In this article, we review the pathophysiology of severe (high- and intermediate-risk) PE and suggest novel pediatric-specific risk stratifications and an acute treatment algorithm to expedite emergent decision making. We define pediatric high-risk PE as causing cardiopulmonary arrest, sustained hypotension or normotension with signs or symptoms of shock. Rapid primary reperfusion should be pursued with either surgical embolectomy or systemic thrombolysis in conjunction with a heparin infusion and supportive care as appropriate. We define pediatric intermediate-risk PE as those without systemic hypotension nor compensated shock but with evidence of right ventricular strain by imaging and/or myocardial necrosis by elevated cardiac troponin levels. The decision to pursue primary reperfusion in this group is complex and should be reserved for patients with more severe disease; anticoagulation alone may also be appropriate in some cases. If primary reperfusion is pursued, catheter-based therapies may be beneficial. In summary, acute management of severe PE in children may include systemic thrombolysis, surgical embolectomy, catheter-based therapies or anticoagulation alone, and may depend on patient and institutional factors. Pediatric emergency and intensive care physicians should be familiar with the risks and benefits of each therapy in order to expedite care. PE Response Teams may also have added benefit in streamlining care during these critical events.

To provide optimal, equitable care to patients, hospital systems must have intentional efforts to advance health equity. We present both intentional and actionable steps that our senior executive leadership prioritized to improve the health equity of the children and families we serve and to create a more inclusive working and learning environment for our employees, staff, faculty and trainees in our academic pediatric medical center. The four key concepts or lessons learned that we found essential to successfully advancing and sustaining equity, diversity, and inclusion (EDI) in our academic pediatric medical center were to: 1) Prioritize the strategy for EDI at the levels of the Board of Trustees and senior executive hospital leadership, 2) Collaborate with multi-disciplinary departments, offices, programs, and subject matter experts, 3) Take an Academic Approach by creating educational initiatives and scholarship in EDI, and (4) Commit to intentionality and accountability in the work by developing and tracking metrics for EDI and health equity safety disparities. Our hospital’s approach to its EDI goals and initiatives can serve as a roadmap for other academic medical centers and healthcare organizations in their efforts to improve health equity for all patients, families, and communities.

Children presenting with acute myocarditis may experience rapid clinical deterioration requiring extracorporeal membrane oxygenation (ECMO); however, our understanding of best practices and timing of ECMO initiation are lacking. We explored the relationships between pre-cannulation factors and survival in this high-acuity patient population.Retrospective review of a large international registry. Primary outcome was survival to hospital discharge, stratified by incident cardiac arrest (CA) prior to ECMO and time to cannulation after intubation.The Extracorporeal Life Support Organization registry was queried for patients less than or equal to 18 years old receiving ECMO support for myocarditis between 2007 and 2018. Exclusion criteria included being nonindex runs, non-venoarterial ECMO or missing data points for main variables studied.None.Population characteristics and survival were compared usingThe results of this multicenter analysis of ECMO utilization and outcomes for pediatric myocarditis suggest that patients approaching ECMO cannulation who have not experienced CA may have better survival outcomes if cannulated onto ECMO early after intubation.

Background: Indigenous cattle are well adapted to the hot tropical climate. Conservation and genetic improvement in the indigenous cattle breeds have become a necessity due to the changing climatic condition. Genome-based research in indigenous cattle for in-depth characterization of genetic variants will help in the improvement of economically important traits in these cattle breeds. Methods: Blood samples were collected for five indigenous cattle breeds of Tamil Nadu from their breeding tracts. Pooled DNA samples were sequenced using the Illumina platform. Short sequences were aligned using BWA, variant calling and annotation were done using GATK and snpEff respectively. Result: This study identified 29,366,340 variants, which include 25,944,935 SNPs and 3,421,405 indels with a variant rate of 91 bp across the genome. Around 215,820 SNPs were present in the coding region of 5,572 genes. This study will provide the framework for further genetic analysis of phenotypic variations in economically important traits in native cattle of Tamil Nadu.

Objectives In the vast majority of Children's Hospitals, the critically ill patient can be found in one of three locations: the PICU, the neonatal ICU, and the cardiac ICU. Training, certification, and maintenance of certification for neonatology and critical care medicine are over seen by the Accreditation Council for Graduate Medical Education and American Board of Pediatrics. There is no standardization of training or oversight of certification and maintenance of certification for pediatric cardiac critical care. Data sources The curricula from the twenty 4th year pediatric cardiac critical care training programs were collated, along with the learning objectives from the Pediatric Cardiac Intensive Care Society published "Curriculum for Pediatric Cardiac Critical Care Medicine." Study selection This initiative is endorsed by the Pediatric Cardiac Intensive Care Society as a first step toward Accreditation Council for Graduate Medical Education oversight of training and American Board of Pediatrics oversight of maintenance of certification. Data extraction A taskforce was established of cardiac intensivists, including the directors of all 4th year pediatric cardiac critical care training programs. Data synthesis Using modified Delphi methodology, learning objectives, rotational requirements, and institutional requirements for providing training were developed. Conclusions In the current era of increasing specialized care in pediatric cardiac critical care, standardized training for pediatric cardiac critical care is paramount to optimizing outcomes.

Objectives To describe the use and outcomes of extracorporeal membrane oxygenation support among children with immune-mediated conditions. Design Retrospective cohort study. Setting The Extracorporeal Life Support Organization registry. Patients Patients 1 month to 18 years old with International Classification of Diseases, 9th Edition and International Classification of Diseases, 10th Edition codes for immune-mediated conditions from 1989 to 2018. Interventions None. Measurements and main results During the study period, 207 patients with an immune-mediated condition received extracorporeal membrane oxygenation, and 50% survived to discharge. Most patients (63%) received extracorporeal membrane oxygenation for respiratory support with 53% survival, 21% received cardiac support (55% survival), and 15% received extracorporeal cardiopulmonary resuscitation (34% survival). The most common diagnosis among nonsurvivors was hemophagocytic lymphohistiocytosis/macrophage activation syndrome with 37% survival. Patients with juvenile idiopathic arthritis (23%) and dermatomyositis (25%) had the lowest survival. Nonsurvivors had a higher frequency of infections, neurologic complications, and renal replacement therapy use. Use of preextracorporeal membrane oxygenation corticosteroids was associated with mortality. Conclusions Children with immune-mediated conditions can be successfully supported with extracorporeal membrane oxygenation. Extracorporeal membrane oxygenation use has increased over time, and survival varies considerably by diagnosis.

The history of cardiopulmonary resuscitation and the Society of Critical Care Medicine have much in common, as many of the founders of the Society of Critical Care Medicine focused on understanding and improving outcomes from cardiac arrest. We review the history, the current, and future state of cardiopulmonary resuscitation.

Although extracorporeal cardiopulmonary resuscitation (ECPR) is increasingly utilized in the pediatric critical care environment, our understanding regarding pediatric candidacy for ECPR remains unknown. Our objective is to explore current practice and indications for pediatric ECPR. Scenario-based, self-administered, online survey, evaluating clinical determinants that may impact pediatric ECPR initiation with respect to four scenarios: postoperative cardiac surgery, cardiac failure secondary to myocarditis, septic shock, and chronic respiratory failure in a former preterm child. Responders are pediatric critical care physicians from four societies. 249 physicians, mostly from North America, answered the survey. In cardiac scenarios, 40% of the responders would initiate ECPR, irrespective of CPR duration, compared with less than 20% in noncardiac scenarios. Nearly 33% of responders would consider ECPR if CPR duration was less than 60 minutes in noncardiac scenarios. Factors strongly decreasing the likelihood to initiate ECPR were out-of-hospital unwitnessed cardiac arrest and blood pH6.60. Additional factors reducing this likelihood were multiple organ failure, pre-existing neurologic delay,10 doses of adrenaline, poor CPR quality, and lactate18 mmol/l. Pediatric intensive care unit location for cardiac arrest, good CPR quality, 24/7 in-house extracorporeal membrane oxygenation (ECMO) team moderately increase the likelihood of initiating ECPR. This international survey of pediatric ECPR initiation practices reveals significant differences regarding ECPR candidacy based on patient category, location of arrest, duration of CPR, witness status, and last blood pH. Further research identifying prognostic factors measurable before ECMO initiation should help define the optimal ECPR initiation strategy.

An essential aspect of the transport system is public passenger transport and the Public Transport (PT) movement prediction is significant issues faced in the transport planning area because of its operational importance. In recent years, Intelligent Transportation Systems (ITS) have received a growing amount of interest. There are many advances and innovative applications that have been introduced for a safer, highly efficient, and even congenial environment from PT. A reliable and efficient system of traffic flow prediction is required for accomplishing these applications that build an event with the application of ITS implementations to resolve the potential road situation in advance. However, the PT network efficiency plays the main role for all urban authority areas in which the advancement of both communication and location devices are randomly increasing the data availability generated over the operational platform. In order to recognize trends useful for improving the Schedule Plan, adequate Machine Learning (ML) approaches need to be implemented. Therefore, this paper focused in heterogeneous data that affect the prediction value which is utilized for predicting the demand transport required in the particular route and arrival time of public transport using Density-Based Spatial Clustering of Applications with Noise (DBSCAN) with Seasonal Autoregressive Integrated Moving Average (SARIMA) algorithm to analyze the forecasting of the real-time passenger demand dynamically endorsed the growth of the dynamic bus management and scheduling. Moreover, the accuracy of proposed SARIMA Model is compared with traditional hybrid model such as Gaussian Mixture Model (GMM) with ARIMA model for providing an efficient and robust prediction of PT based on passenger demand.

Objectives Fluid boluses are commonly administered to improve the cardiac output and tissue oxygen delivery in pediatric septic shock. The objective of this study is to evaluate the effect of an early fluid bolus administered to children with septic shock on the cardiac index and mean arterial pressure, as well as on the hemodynamic response and its relationship with outcome. Design, setting, patients, and interventions We prospectively collected hemodynamic data from children with septic shock presenting to the emergency department or the PICU who received a fluid bolus (10 mL/kg of Ringers Lactate over 30 min). A clinically significant response in cardiac index-responder and mean arterial pressure-responder was both defined as an increase of greater than or equal to 10% 10 minutes after fluid bolus. Measurements and main results Forty-two children with septic shock, 1 month to 16 years old, median Pediatric Risk of Mortality-III of 13 (interquartile range, 9-19), of whom 66% were hypotensive and received fluid bolus within the first hour of shock recognition. Cardiac index- and mean arterial pressure-responsiveness rates were 31% and 38%, respectively. We failed to identify any association between cardiac index and mean arterial pressure changes (r = 0.203; p = 0.196). Cardiac function was similar in mean arterial pressure- and cardiac index-responders and nonresponders. Mean arterial pressure-responders increased systolic, diastolic, and perfusion pressures (mean arterial pressure - central venous pressure) after fluid bolus due to higher indexed systemic vascular resistance and arterial elastance index. Mean arterial pressure-nonresponders required greater vasoactive-inotrope support and had higher mortality. Conclusions The hemodynamic response to fluid bolus in pediatric septic shock was variable and unpredictable. We failed to find a relationship between mean arterial pressure and cardiac index changes. The adverse effects of fluid bolus extended beyond fluid overload and, in some cases, was associated with reduced mean arterial pressure, perfusion pressures and higher vasoactive support. Mean arterial pressure-nonresponders had increased mortality. The response to the initial fluid bolus may be helpful to understand each patient's individualized physiologic response and guide continued hemodynamic management.

The Underwater Wireless Sensor Network (UWSN) is capable of supporting a wide range of low-data-rate acoustic sensor networks, as well as scalability and energy-efficient routing algorithms. Furthermore, because of the lower bandwidth and longer propagation delays, energy consumption is a major concern with underwater networks. Because radio transmissions are unstable in deep water, a UWSN often communicates via acoustic channels. UWSN features create constraints on data packet transmission and energy-efficient routing. The hardest challenge is creating an effective routing protocol for UWSNs that uses sensor node localization. There have been several routing algorithms reported for identifying nodes via a localization process. This paper proposes a novel vector-based forwarding (NVBF) and efficient depth-based routing (DBR) protocol to provide robust, scalable, and energy-efficient routing. Efficient DBR is a stochastic model that can survive even in acoustic channels with substantial transmission loss. An adjacent node table is created to decrease the energy consumption and end-to-end delay by limiting the adjacent node request generated every single time. The redundant packet transmission is reduced by creating a packet queue which also minimizes the energy. The NVBF algorithm uses a random waiting time to overcome the collision that occurs while sending the acknowledgment packets. Based on the previous sender’s depth and the depth of the own node, the DBR makes packet transmission decisions. The experiments are conducted using the NS3 simulator and the efficiency of the proposed approach is evaluated in terms of propagation delay, network lifetime, energy consumption, and delivery ratio. When compared to the existing techniques such as DBR, VBF, and energy-efficient DBR (EDBR), the proposed methodology offers improvement up to 40% and 30% in terms of propagation delay and delivery ratio.

OBJECTIVE The goal of this work is to provide insight into survival and neurologic outcomes of pediatric patients supported with extracorporeal cardiopulmonary resuscitation. DATA SOURCES A systematic search of Embase, PubMed, Cochrane, Scopus, Google Scholar, and Web of Science was performed from January 1990 to May 2020. STUDY SELECTION A comprehensive list of nonregistry studies with pediatric patients managed with extracorporeal cardiopulmonary resuscitation was included. DATA EXTRACTION Study characteristics and outcome estimates were extracted from each article. DATA SYNTHESIS Estimates were pooled using random-effects meta-analysis. Differences were estimated using subgroup meta-analysis and meta-regression. The Meta-analyses Of Observational Studies in Epidemiology guideline was followed and the certainty of evidence was assessed using Grading of Recommendations Assessment, Development and Evaluation system. Twenty-eight studies (1,348 patients) were included. There was a steady increase in extracorporeal cardiopulmonary resuscitation occurrence rate from the 1990s until 2020. There were 32, 338, and 1,094 patients' articles published between 1990 and 2000, 2001 and 2010, and 2010 and 2020, respectively. More than 70% were cannulated for a primary cardiac arrest. Pediatric extracorporeal cardiopulmonary resuscitation patients had a 46% (CI 95% = 43-48%; p < 0.01) overall survival rate. The rate of survival with favorable neurologic outcome was 30% (CI 95% = 27-33%; p < 0.01). CONCLUSIONS The use of extracorporeal cardiopulmonary resuscitation is rapidly expanding, particularly for children with underlying cardiac disease. An overall survival of 46% and favorable neurologic outcomes add credence to this emerging therapy.

Once a mainstay in the treatment of neonates with d-transposition of the great arteries (d-TGA), the application of balloon atrial septostomy (BAS) in the d-TGA population has become more selective. Currently, there is no clear evidence for or against a selective BAS strategy. The aims of this single-center retrospective study were to determine the incidence of BAS in the neonatal d-TGA population in the current era, to measure the rate of procedural success, and to compare the outcomes and complication rates of patients who underwent BAS to those who underwent neonatal ASO alone. Between 2012 and 2018, 147 patients with d-TGA underwent initial medical management and ASO, 73 of which underwent BAS. The percentage of patients that underwent BAS decreased from 73 to 33% over the study time period. In patients with d-TGA with intact ventricular septum, 33% of patients remained off of PGE1 at the time of surgery regardless of BAS. In d-TGA with ventricular septal defect, 85.7% of those that underwent BAS and 54.1% of those who did not remained off of PGE1 at the time of surgery, however, this difference did not reach statistical significance. In this single institution retrospective cohort of patients with d-TGA, the performance of a technically successful balloon atrial septostomy did not eliminate the need for PGE1 therapy at the time of definitive ASO. This was true regardless of the presence or absence of a ventricular septal defect.

To keep the process of routing and data transmission operations, it’s needed for a proper layer of security to keep it safe from malicious attacks. So in this paper, we proposed a framework that helps in detecting the malicious nodes at each of the destinations. After the detection process, it is isolated and discarded in while path establishment through various techniques. The development of an algorithm that supports Multipath Reliable Routing helps in determining paths for a set of disjoint nodes. Based on the index of reliability the paths are reordered. Now the process of information in form of data are been dispersed from source to destination. The main primary path is set to transfer the information. The final destination in case of mismatch of information transferred from a source when received at destination then feedback is given in negative manner stating that there is a mismatch of data along with the information related to the transmission and its path. Hence due to this, the destination will be able to recover since the information is checked each time for reliability. The final results show that the proposed approach results and have a greater impact when compared to the existing methods. Further, it reduces delay in packet transmission and overhead parallel it increases the delivery of packets.

This 2020 International Consensus on Cardiopulmonary Resuscitation and Emergency Cardiovascular Care Science With Treatment Recommendations (CoSTR) for pediatric life support is based on the most extensive evidence evaluation ever performed by the Pediatric Life Support Task Force. Three types of evidence evaluation were used in this review: systematic reviews, scoping reviews, and evidence updates. Per agreement with the evidence evaluation recommendations of the International Liaison Committee on Resuscitation, only systematic reviews could result in a new or revised treatment recommendation. Systematic reviews performed for this 2020 CoSTR for pediatric life support included the topics of sequencing of airway-breaths-compressions versus compressions-airway-breaths in the delivery of pediatric basic life support, the initial timing and dose intervals for epinephrine administration during resuscitation, and the targets for oxygen and carbon dioxide levels in pediatric patients after return of spontaneous circulation. The most controversial topics included the initial timing and dose intervals of epinephrine administration (new treatment recommendations were made) and the administration of fluid for infants and children with septic shock (this latter topic was evaluated by evidence update). All evidence reviews identified the paucity of pediatric data and the need for more research involving resuscitation of infants and children.

Background Fluid boluses aiming to improve the cardiac output and oxygen delivery are commonly administered in children with shock. An increased mean arterial pressure in addition to resolution of tachycardia and improved peripheral perfusion are often monitored as clinical surrogates for improvement in cardiac output. The objective of our study is to describe changes in cardiac index, mean arterial pressure, and their relationship to other indices of cardiovascular performance. Objective The objective of our study is to describe changes in cardiac index, mean arterial pressure, and their relationship to other indices of cardiovascular performance. Design, setting, patients, and interventions We prospectively analyzed hemodynamic data from children in the cardiac ICU who received fluid bolus (10mL/kg of Ringers-Lactate over 30 min) for management of shock and/or hypoperfusion within 12h of cardiac surgery. Cardiac index responders and mean arterial pressure-responders were defined as CI ≥10% and mean arterial pressure ≥10%, respectively. We evaluated the gradient for venous-return (mean systemic filling pressure-central venous pressure), arterial load properties (systemic vascular resistance index and elastance index) and changes in vasopressor support after fluid bolus. Measurements and main results Fifty-seven children between 1 month and 16 years (median Risk adjustment after congenital heart surgery Model for Outcome Surveillance in Australia and New Zealand score of 3.8 (interquartile range 3.7-4.6) received fluid bolus. Cardiac index-responsiveness and mean arterial pressure-responsiveness rates were 33% and 56%, respectively. No significant correlation was observed between changes in mean arterial pressure and cardiac index (r = 0.035, p = 0.79). Although the mean systemic filling pressure - central venous pressure and the number of cardiac index-responders after fluid bolus were similar, the arterial load parameters did not change in mean arterial pressure-nonresponders. Forty-three patients (75%) had a change in Vasoactive-Inotrope Score after the fluid bolus, of whom 60% received higher level of vasoactive support. Conclusions The mean arterial pressure response to fluid bolus in cardiac ICU patients was unpredictable with a poor relationship between cardiac index-responsiveness and mean arterial pressure-responsiveness. Because arterial hypotension is frequently a trigger for administering fluids and changes in blood pressure are commonly used for tracking changes in cardiac output, we suggest a cautious and individualized approach to repeat fluid bolus based solely on lack of mean arterial pressure response to the initial fluid, since the implications include decreased arterial tone even if the cardiac index increases.

Objectives Extracorporeal membrane oxygenation is used to support refractory cardiorespiratory failure. Outcomes and complications when extracorporeal membrane oxygenation is used to support cardiorespiratory failure secondary to arrhythmia in pediatric patients remain poorly defined. Our purpose is to describe pediatric patients requiring extracorporeal membrane oxygenation support for supraventricular arrhythmias in the context of normal cardiac anatomy and congenital heart disease and identify patient/peri-extracorporeal membrane oxygenation variables associated with extracorporeal membrane oxygenation-related complications and survival. Design Retrospective multicenter review from 1993 to 2016. Setting Extracorporeal Life Support Organization registry. Subjects Patients younger than 21 years old requiring extracorporeal membrane oxygenation support for supraventricular arrhythmias. Interventions None. Measurements and main results A total of 342 patients were identified (weight, 3.8 kg [3.2-7.5 kg]; age at extracorporeal membrane oxygenation initiation, 24 d [6-222]; 61% male). Sixty-five percentage survived to hospital discharge. Complications were frequent (85%) and most commonly cardiac related (31%). In multivariable modeling, mortality was associated with congenital heart disease, time from intubation to extracorporeal membrane oxygenation initiation, use of bicarbonate prior to extracorporeal membrane oxygenation, extracorporeal membrane oxygenation duration, and the presence of a complication. The presence of any complication was associated with a three-fold increase in the odds of death. In subgroup analysis of isolated supraventricular arrhythmias patients, similar patient and extracorporeal membrane oxygenation characteristics were associated with outcome. A lower pre-extracorporeal membrane oxygenation pH and PCO2 and site of venous cannulation were associated with complications (p Conclusions Extracorporeal membrane oxygenation use for medically refractory supraventricular arrhythmias was associated with a 65% survival to hospital discharge. However, there was a high rate of complications, the presence of which was associated with decreased survival. Complications appeared to be related to pre-extracorporeal membrane oxygenation clinical status and whether earlier extracorporeal membrane oxygenation cannulation prior to patient deterioration would improve outcomes needs additional evaluation.

ObjectiveTo examine EEG features of major pathophysiology in children undergoing extracorporeal membrane oxygenation (ECMO).MethodsThis was a single-center, retrospective study of 201 pediatric patients on ECMO, using the first 24 hours of continuous EEG (cEEG) monitoring, collating background activity and electrographic seizures (ES) with imaging, ECMO type, and outcome.ResultsSeverely abnormal cEEG background occurred in 12% (25/201), and was associated with death (sensitivity 0.23, specificity 0.97). ES occurred in 16% (33/201) within 3.2 (0.6–20.3) hours (median [interquartile range]) of cEEG commencement, and higher ES burden was associated with death. ES was always associated with ipsilateral injury (p= 0.006), but occurred in only one-third of cases with abnormal imaging. In 28 patients with isolated hemisphere lesion, type of arterial ECMO cannulation was associated with side of injury: right carotid cannulation was associated with right hemisphere lesions, and ascending aorta cannulation with left hemisphere lesions (odds ratio, 0.29 [95% confidence interval, 0.08–0.98],p= 0.03).ConclusionsAfter starting ECMO, cEEG background activity has the potential to inform prognosis. Type of arterial (carotid vs aortic) ECMO correlates with side of focal cerebral injury, which in ≈33% is associated with presence of ES. We hypothesize that the differential distribution reflects abnormal flow dynamics or embolic injury.
