Chronic pain is among the most common reasons adults in the U.S. seek medical care. Despite chronic pain's substantial impact on individuals' physical, emotional, and financial wellness, the biologic underpinnings of chronic pain remain incompletely understood. Such deleterious impact on an individuals' wellness is also manifested in the substantial co-occurrence of chronic stress with chronic pain. However, whether chronic stress and adversity and related alcohol and substance misuse increases risk of developing chronic pain, and, if so, what the overlapping psychobiological processes are, is not well understood. Individuals suffering with chronic pain find alleviation through prescription opioids as well as non-prescribed cannabis, alcohol, and other drugs to control pain, and use of these substances have grown significantly. Substance misuse also increases experience of chronic stress. Thus, given the evidence showing a strong correlation between chronic stress and chronic pain, we aim to review and identify overlapping factors and processes. We first explore the predisposing factors and psychologic features common to both conditions. This is followed by examining the overlapping neural circuitry of pain and stress in order to trace a common pathophysiologic processes for the development of chronic pain and its link to substance use. Based on the previous literature and our own findings, we propose a critical role for ventromedial prefrontal cortex dysfunction, an overlapping brain area associated with the regulation of both pain and stress that is also affected by substance use, as key in the risk of developing chronic pain. Finally, we identify the need for future research in exploring the role of medial prefrontal circuits in chronic pain pathology. Critically, in order to alleviate the enormous burden of chronic pain without exacerbating the co-occurring substance misuse crisis, we emphasize the need to find better approaches to treat and prevent chronic pain.

We propose an innovative methodology to estimate the formative discharge of alluvial rivers from remote sensing images. This procedure involves automatic extraction of the width of a channel from Landsat Thematic Mapper, Landsat 8, and Sentinel-1 satellite images. We translate the channel width extracted from satellite images to discharge using a width–discharge regime curve established previously by us for the Himalayan rivers. This regime curve is based on the threshold theory, a simple physical force balance that explains the first-order geometry of alluvial channels. Using this procedure, we estimate the formative discharge of six major rivers of the Himalayan foreland: the Brahmaputra, Chenab, Ganga, Indus, Kosi, and Teesta rivers. Except highly regulated rivers (Indus and Chenab), our estimates of the discharge from satellite images can be compared with the mean annual discharge obtained from historical records of gauging stations. We have shown that this procedure applies both to braided and single-thread rivers over a large territory. Furthermore, our methodology to estimate discharge from remote sensing images does not rely on continuous ground calibration.

Abstract We study operator complexity on various time scales with emphasis on those much larger than the scrambling period. We use, for systems with a large but finite number of degrees of freedom, the notion of K-complexity employed in [1] for infinite systems. We present evidence that K-complexity of ETH operators has indeed the character associated with the bulk time evolution of extremal volumes and actions. Namely, after a period of exponential growth during the scrambling period the K-complexity increases only linearly with time for exponentially long times in terms of the entropy, and it eventually saturates at a constant value also exponential in terms of the entropy. This constant value depends on the Hamiltonian and the operator but not on any extrinsic tolerance parameter. Thus K-complexity deserves to be an entry in the AdS/CFT dictionary. Invoking a concept of K-entropy and some numerical examples we also discuss the extent to which the long period of linear complexity growth entails an efficient randomization of operators.

Chiral-icosahedral symmetry is exceptionally rare in molecular systems. Here, the authors predict the spontaneous formation of a stable chiral-icosahedral gold shell, in which all 60 atoms are symmetrically equivalent.

We identified genetic polymorphism in Mucin13 gene affecting E. coli adhesion patterns using (local isolate) diarrhoeagenic E. coli in Indian desi pigs. Five SNPs and one indel previously reported to be associated with enterotoxigenic E. coli (ETEC) F4ab/ac adhesion pattern were examined by designing PCR-RFLP protocol. The genotypic frequencies of only one SNP (g.22304A  >  G) differed significantly (at P ≤ 0.05) in adhesive, non-adhesive and weakly adhesive population. The AA (306 sbp, 231 bp), AG (306, 231, 108, 198 bp) and GG (231, 198 bp, 108 b) genotypes of g.22304A  >  G locus were observed with frequencies 50.0 %, 21.25 % and 28.75 %, respectively and AG genotype was significantly (P ≤ 0.05) associated with a non-adhesive pattern. The polymorphism information content of SNPs ranged from 17.67 (g.22124T  >  C) to 37.36 % (g.21471C  >  T) loci. Three loci (g.21471C  >  T, g.22124T  >  C and g.22304A  >  G) were significantly departed from Hardy–Weinberg equilibrium. The linkage disequilibrium analysis revealed locus g.22124T  >  C and g.22304A  >  G were significantly (P ≤ 0.05) associated with each other. Expression profiling of target gene in jejuna of animals having AA, AG and GG genotypes revealed differences in various genotypes with the highest in the AA, moderate in the GG and low levels in the AG genotype, although they were statistically non-significant (at P ≤ 0.05). The absence of significant effect of genotypes on MUC13 mRNA expression indicates no direct functional role, although the structural role can not be ignored as the putative receptor gene is located within targeted genomic region. Further, reports of same SNP association with an ETEC F4ab/ac adhesion pattern indicate the target gene's role in diarrhoea even caused by other strains of E. coli which is not ETEC.

High soil erosion and excessive sediment load are serious problems in several Himalayan river basins. To apply mitigation procedures, precise estimation of soil erosion and sediment yield with associated uncertainties are needed. Here, the revised universal soil loss equation (RUSLE) and the sediment delivery ratio (SDR) equations are used to estimate the spatial pattern of soil erosion (SE) and sediment yield (SY) in the Garra River basin, a small Himalayan tributary of the River Ganga. A methodology is proposed for quantifying and propagating uncertainties in SE, SDR and SY estimates. Expressions for uncertainty propagation are derived by first-order uncertainty analysis, making the method viable even for large river basins. The methodology is applied to investigate the relative importance of different RUSLE factors in estimating the magnitude and uncertainties in SE over two distinct morphoclimatic regimes of the Garra River basin, namely the upper mountainous region and the lower alluvial plains. Our results suggest that average SE in the basin is very high (23 ± 4.7 t ha−1 yr−1) with higher values in the upper mountainous region (92 ± 15.2 t ha−1 yr−1) compared to the lower alluvial plains (19.3 ± 4 t ha−1 yr−1). Furthermore, the topographic steepness (LS) and crop practice (CP) factors exhibit higher uncertainties than other RUSLE factors. The annual average SY is estimated at two locations in the basin – Nanak Sagar Dam (NSD) for the period 1962–2008 and Husepur gauging station (HGS) for 1987–2002. The SY at NSD and HGS are estimated to be 6.9 ± 1.2 × 105 t yr−1 and 6.7 ± 1.4 × 106 t yr−1, respectively, and the estimated 90 % interval contains the observed values of 6.4 × 105 t yr−1 and 7.2 × 106 t yr−1, respectively. The study demonstrated the usefulness of the proposed methodology for quantifying uncertainty in SE and SY estimates at ungauged basins.

Objective: To describe our technique of retrograde pericatheter urethrography (RPU) and its clinical use after urethroplasty. Subjects and methods: Between January 2008 and December 2013, 387 patients with urethral stricture underwent urethroplasty at our center. A total of 343 of these patients underwent RPU 3 weeks post-operatively. For this retrospective study their files were evaluated with regard to: demographics, duration of symptoms, site, mean length of stricture, type of surgery, RPU findings, contrast-medium related complications and need of re-intervention. The eventual surgical success was defined as asymptomatic voiding with no clinical evidence of residual stricture (good flow rate and no residual urine) until the last follow up. Results: Follow up ranged from 8 to 41 (mean 28) months. The mean duration of symptoms was 4.8 months. The mean stricture length, as seen on radiography, was 2.1 cm. 183 patients (53.3%) underwent anastomotic urethroplasty, while 160 (46.6%) underwent substitution urethroplasty. RPU showed urethral healing in 292 (85.2%) and contrast extravasation in 51 (14.8%) patients. No contrast-medium related complications were reported. Re-intervention was needed in 7.2% (21/292) of the patients who showed normal urethral healing and in 74.5% (38/51) of the patients who showed contrast extravasation on RPU. By the time of the last follow up the overall success rate was 82.7% (284/343 patients). Conclusion: RPU is the most useful radiological diagnostic method for evaluating the appropriate time for catheter removal after urethroplasty. It helps to assess urethral healing and patency after urethroplasty. Prolonged catheterization in patients showing contrast extravasation may be helpful.

For the development of sustainable and realistic water security, generating information on the behaviours, characteristics, and drivers of users, as well as on the resource itself, is essential. In this paper we present a methodology for collecting qualitative and quantitative data on water use practices through semi-structured interviews. This approach facilitates the collection of detailed information on actors' decisions in a convenient and cost-effective manner. Semi-structured interviews are organised around a topic guide, which helps lead the conversation in a standardised way while allowing sufficient opportunity for relevant issues to emerge. In addition, they can be used to obtain certain types of quantitative data. While not as accurate as direct measurements, they can provide useful information on local practices and users' insights. We present an application of the methodology on farmer water use in two districts in the state of Uttar Pradesh in northern India. By means of 100 farmer interviews, information was collected on various aspects of irrigation practices, including irrigation water volumes, irrigation cost, water source, and their spatial variability. Statistical analyses of the information, along with data visualisation, are also presented, indicating a significant variation in irrigation practices both within and between districts. Our application shows that semi-structured interviews are an effective and efficient method of collecting both qualitative and quantitative information for the assessment of drivers, behaviours, and their outcomes in a data-scarce region. The collection of this type of data could significantly improve insights on water resources, leading to more realistic management options and increased water security in the future.

Introduction: Colonic perforations are known complications of percutaneous nephrolithotomy (PCNL). However, to the best of our knowledge, small bowel perforation has rarely been reported.. Observation: We report the case of a 7-year-old girl who presented with a duodenal perforation in the early postoperative period after undergoing PCNL for a calculus sized 2.5 cm in the right renal pelvis. She was successfully managed conservatively. The diagnostic workup and management are discussed.

We report on a new set of measurements on the morphology of braided and meandering threads of the Kosi megafan, North Bihar, India. All threads develop on a uniform sandy sediment and under a similar climate. The data set is composed of the width, depth, water discharge and grain size of 51 threads. Downstream slopes and sinuosity are also available. Using this data set, we show that braided and meandering threads share common hydraulic geometries. We then use the threshold theory to explain why the aspect ratio of threads is almost naturally detrended, and rescale the data according to this theory. As expected, the rescaled dimensions of braided and meandering threads are weakly correlated to water discharge. We propose that the large dispersion observed, which is common to meandering and braided threads, is the signature of sediment transport, vegetation or cohesion effects.

Pseudomonas putida is an uncommon opportunistic pathogen, usually susceptible to antimicrobial agents. Data concerning resistance to antimicrobial agents in clinical P. putida isolates are limited. To the best of our knowledge we report for the first time the isolation of NDM-1-producing multidrug-resistant P. putida from a case of acute gastroenteritis. The isolate showed resistance to a wide range of antimicrobials, including fluoroquinolones, third-generation cephalosporins and carbapenems. The isolate also exhibited multiple mutations in the quinolone resistance determining region and showed the presence of qepA, blaTEM, blaOXA1 and blaOXA7 genes. The present study highlights the importance of looking for the relatively rare aetiological agents in clinical samples that do not yield common pathogens.

Burkitt’s lymphoma is the most frequent subtype of non-Hodgkin’s lymphoma in childhood. Radiographic findings are protean and can often overlap with other neoplastic and nonneoplastic processes. We present an unusual case of Burkitt’s lymphoma in a 12-year-old boy presenting with a one-week history of urinary retention, dysuria, and “tailbone pain,” as well as a 4-week history of jaw pain, initially treated as a dental abscess. On dental radiography, the patient was found to have resorption of alveolar bone adjacent to the lower first molars bilaterally, in keeping with “floating teeth,” classically associated with Langerhans cell histiocytosis. Additionally, a large, eccentric, prostatic mass was noted, prompting the inclusion of rhabdomyosarcoma on the differential diagnosis, with subsequent definitive diagnosis of Burkitt’s lymphoma on tissue and bone marrow biopsy. This case highlights the imaging overlap of these childhood neoplasms with an unusual lymphomatous prostate mass. It is important that the radiologists and pediatricians be aware of this potential overlap and the unusual presentation of Burkitt’s lymphoma.
