Every year Africa is hit by extreme floods which, combined with high levels of vulnerability and increasing population exposure, often result in humanitarian crises and population displacement. Impact-based forecasting and early warning for natural hazards is recognized as a step forward in disaster risk reduction, thanks to its focus on people, livelihoods, and assets at risk. Yet, the majority of the African population is not covered by any sort of early warning system. This article describes the setup and the methodological approach of Flood-PROOFS East Africa, an impact-based riverine flood forecasting and early warning system for the Greater Horn of Africa (GHA), with a forecast range of 5 d. The system is based on a modeling cascade relying on distributed hydrological simulations forced by ensemble weather forecasts, link to inundation maps for specific return period, and application of a risk assessment framework to estimate population and assets exposed to upcoming floods. The system is operational and supports the African Union Commission and the Disaster Operation Center of the Intergovernmental Authority on Development (IGAD) in the daily monitoring and early warning from hydro-meteorological disasters in eastern Africa. Results show a first evaluation of the hydrological reanalysis at 78 river gauging stations and a semi-quantitative assessment of the impact forecasts for the catastrophic floods in Sudan and in the Nile River basin in summer 2020. More extensive quantitative evaluation of the system performance is envisaged to provide its users with information on the model reliability in forecasting extreme events and their impacts.

The Canary Island Long-Baseline Observatory (CILBO) is a double-station meteor camera setup located on the Canary Islands operated by ESA's Meteor Research Group since 2010. Observations of meteors are obtained in the visual wavelength band by intensified video cameras from both stations, supplemented by an intensified video camera mounted with a spectral grating at one of the locations. The cameras observe during cloudless and precipitation-free nights, and data are transferred to a main computer located at ESA/ESTEC once a day. The image frames that contain spectral information are calibrated, corrected, and finally processed into line intensity profiles. An ablation simulation, based on Bayesian statistics using a Markov chain Monte Carlo method, allows determining a parameter space, including the ablation temperatures, chemical elements, and their corresponding line intensities, to fit against the line intensity profiles of the observed meteor spectra. The algorithm is presented in this paper and one example is discussed. Several hundred spectra have been processed and made available through the Guest Archive Facility of the Planetary Science Archive of ESA. The data format and metadata are explained.

BACKGROUND: Leptomeningeal metastasis (LM) currently lacks standardization with respect to response assessment. METHODS: Investigators with expertise in LM developed a consensus proposal for evaluating patients treated with LM. RESULTS: Three basic elements in assessing response in LM are proposed; a standardized neurological examination, CSF cytology or parametric flow cytometry and radiographic evaluation. The group recommends that all patients enrolling in clinical trials undergo CSF analysis (cytology in all cancers; flow cytometry in hematologic cancers), complete contrast-enhanced neuraxis MRI and in instances of planned intra-CSF therapy, radioisotope CSF flow studies. In conjunction with the Response Assessment in Neuro-Oncology (RANO) Neurological Assessment Working Group, a standardized instrument was created for assessing the neurological exam in patients with LM. Response based on CSF cytology is considered when CSF converts from positive to negative at all previously positive sites, and with a confirmatory determination after 4 weeks. CSF flow cytometry is a quantitative analysis that also utilizes a binary outcome, i.e. positive or negative. Either CSF cytology (assessed in all patients) or CSF flow cytometry (assessed in patients with hematologic cancers), if unresponsive, constitutes progressive (refractory) disease irrespective of neurological or radiological determination. Considering that most lesions in LM are non-measurable and that assessment of neuroimaging in LM is subjective, neuroimaging is graded as stable, progressive or improved using radiological LM response criteria. Radiographic disease progression in isolation i.e. negative CSF cytology/flow cytometry and stable neurological assessment would be defined as LM disease progression. CONCLUSIONS: RANO LM has proposed a method of response evaluation for patients with LM that will require further testing, validation and likely refinement with use.

Thirty-six patients diagnosed with diskitis from 1978 to 1988 and followed for an average of 29.2 months were reviewed. The study included 23 boys and 13 girls with an average age of 5.3 years. The initial symptoms varied, but distinct clinical patterns emerged and were identifiable in different age groups. Both the leukocyte count and sedimentation rate were elevated. Routine roentgenograms were positive for intervertebral disk space narrowing in 82% of cases, technetium bone scans positive in 72%, and magnetic resonance imaging positive in all recent cases. Treatment consisted of bed rest for all patients, plaster casts for 50%, antibiotics for 40%, and traction for 23%. Regardless of the treatment combination, the course of the disease in most children is benign. At the completion of the study, all patients were asymptomatic including three children who had recurrences. In spite of being asymptomatic, 74% had persistent roentgenographic changes. The administration of antibiotics appears to be appropriate when indicated, i.e., failure to respond to immobilization. Disk space aspiration or biopsy should be reserved for those cases that are refractory to immobilization and antibiotics.

During the autumn of 2011 two catastrophic, very intense rainfall events affected two different parts of the Liguria Region of Italy causing various flash floods. The first occurred in October and the second at the beginning of November. Both the events were characterized by very high rainfall intensities (> 100 mm h−1) that persisted on a small portion of territory causing local huge rainfall accumulations (> 400 mm 6 h−1). Two main considerations were made in order to set up this work. The first consideration is that various studies demonstrated that the two events had a similar genesis and similar triggering elements. The second very evident and coarse concern is that two main elements are needed to have a flash flood: a very intense and localized rainfall event and a catchment (or a group of catchments) to be affected. Starting from these assumptions we did the exercise of mixing the two flash flood ingredients by putting the rainfall field of the first event on the main catchment struck by the second event, which has its mouth in the biggest city of the Liguria Region: Genoa. A complete framework was set up to quantitatively carry out a “what if” experiment with the aim of evaluating the possible damages associated with this event. A probabilistic rainfall downscaling model was used to generate possible rainfall scenarios maintaining the main characteristics of the observed rainfall fields while a hydrological model transformed these rainfall scenarios in streamflow scenarios. A subset of streamflow scenarios is then used as input to a 2-D hydraulic model to estimate the hazard maps, and finally a proper methodology is applied for damage estimation. This leads to the estimation of the potential economic losses and of the risk level for the people that stay in the affected area. The results are interesting, surprising and in a way worrying: a rare but not impossible event (it occurred about 50 km away from Genoa) would have caused huge damages estimated between 120 and EUR 230 million for the affected part of the city of Genoa, Italy, and more than 17 000 potentially affected people.

During the last decade the opportunity and usefulness of using remote-sensing data in hydrology, hydrometeorology and geomorphology has become even more evident and clear. Satellite-based products often allow for the advantage of observing hydrologic variables in a distributed way, offering a different view with respect to traditional observations that can help with understanding and modeling the hydrological cycle. Moreover, remote-sensing data are fundamental in scarce data environments. The use of satellite-derived digital elevation models (DEMs), which are now globally available at 30 m resolution (e.g., from Shuttle Radar Topographic Mission, SRTM), have become standard practice in hydrologic model implementation, but other types of satellite-derived data are still underutilized. As a consequence there is the need for developing and testing techniques that allow the opportunities given by remote-sensing data to be exploited, parameterizing hydrological models and improving their calibration. In this work, Meteosat Second Generation land-surface temperature (LST) estimates and surface soil moisture (SSM), available from European Organisation for the Exploitation of Meteorological Satellites (EUMETSAT) H-SAF, are used together with streamflow observations (S. N.) to calibrate the Continuum hydrological model that computes such state variables in a prognostic mode. The first part of the work aims at proving that satellite observations can be exploited to reduce uncertainties in parameter calibration by reducing the parameter equifinality that can become an issue in forecast mode. In the second part, four parameter estimation strategies are implemented and tested in a comparative mode: (i) a multi-objective approach that includes both satellite and ground observations which is an attempt to use different sources of data to add constraints to the parameters; (ii and iii) two approaches solely based on remotely sensed data that reproduce the case of a scarce data environment where streamflow observation are not available; (iv) a standard calibration based on streamflow observations used as a benchmark for the others. Two Italian catchments are used as a test bed to verify the model capability in reproducing long-term (multi-year) simulations. The results of the analysis evidence that, as a result of the model structure and the nature itself of the catchment hydrologic processes, some model parameters are only weakly dependent on discharge observations, and prove the usefulness of using data from both ground stations and satellites to additionally constrain the parameters in the calibration process and reduce the number of equifinal solutions.

Full process description and distributed hydrological models are very useful tools in hydrology as they can be applied in different contexts and for a wide range of aims such as flood and drought forecasting, water management, and prediction of impact on the hydrologic cycle due to natural and human-induced changes. Since they must mimic a variety of physical processes, they can be very complex and with a high degree of parameterization. This complexity can be increased by necessity of augmenting the number of observable state variables in order to improve model validation or to allow data assimilation. In this work a model, aiming at balancing the need to reproduce the physical processes with the practical goal of avoiding over-parameterization, is presented. The model is designed to be implemented in different contexts with a special focus on data-scarce environments, e.g. with no streamflow data. All the main hydrological phenomena are modelled in a distributed way. Mass and energy balance are solved explicitly. Land surface temperature (LST), which is particularly suited to being extensively observed and assimilated, is an explicit state variable. A performance evaluation, based on both traditional and satellite derived data, is presented with a specific reference to the application in an Italian catchment. The model has been firstly calibrated and validated following a standard approach based on streamflow data. The capability of the model in reproducing both the streamflow measurements and the land surface temperature from satellites has been investigated. The model has been then calibrated using satellite data and geomorphologic characteristics of the basin in order to test its application on a basin where standard hydrologic observations (e.g. streamflow data) are not available. The results have been compared with those obtained by the standard calibration strategy based on streamflow data.

On the 4 November 2011 a flash flood event hit the area of Genoa with dramatic consequences. Such an event represents, from the meteorological and hydrological perspective, a paradigm of flash floods in the Mediterranean environment. The hydro-meteorological probabilistic forecasting system for small and medium size catchments in use at the Civil Protection Centre of Liguria region exhibited excellent performances for the event, by predicting, 24–48 h in advance, the potential level of risk associated with the forecast. It greatly helped the decision makers in issuing a timely and correct alert. In this work we present the operational outputs of the system provided during the Liguria events and the post event hydrological modelling analysis that has been carried out accounting also for the crowd sourcing information and data. We discuss the benefit of the implemented probabilistic systems for decision-making under uncertainty, highlighting how, in this case, the multi-catchment approach used for predicting floods in small basins has been crucial.

PREVIEW is an European Commission FP6 Integrated Project with the aim of developing, at an European level, innovative geo-information services for atmospheric, geophysical and man-made risks. Within this framework, the Landslides Platform Service 2 (forecasting of shallow rapid slope movements) has developed an integrated procedure for the forecasting and warning of distributed shallow landsliding to be used for civil protection purposes. The Service consists of an automated end-to-end forecasting chain which uses data from a probabilistic downscaled short-term rainfall forecast, soil saturation estimates and meteorological radar outputs. The above data are entered into a hydro-geological model that makes use of an infinite slope approach to calculate the distributed Factor of Safety over the entire basin. All outputs, and much of the input data, are shown on a WebGIS system so that end-users can interactively access and download data. A distinctive feature of the service is the use of an innovative soil depth model for predicting the distributed thickness of the regolith cover within the basin, which is one of the most important parameters controlling shallow landslide triggering. The service was developed in a pilot test site in NE Italy, the Armea basin. Validation makes use of two rainfall events: one that occurred in 2000 and a smaller, more recent event (2006) that caused fewer landslides. Rainfall data have been used to compute a distributed factor-of-safety map that has been overlaid onto the landslide inventory. Instead of a traditional validation approach based on the number count of correctly identified landslides, we carried out an alternative procedure based on the landslides area that gave outcomes which, for this preliminary stage of the research, can be considered promising.

Flood forecasting undergoes a constant evolution, becoming more and more demanding about the models used for hydrologic simulations. The advantages of developing distributed or semi-distributed models have currently been made clear. Now the importance of using continuous distributed modeling emerges. A proper schematization of the infiltration process is vital to these types of models. Many popular infiltration schemes, reliable and easy to implement, are too simplistic for the development of continuous hydrologic models. On the other hand, the unavailability of detailed and descriptive information on soil properties often limits the implementation of complete infiltration schemes. In this work, a combination between the Soil Conservation Service Curve Number method (SCS-CN) and a method derived from Horton equation is proposed in order to overcome the inherent limits of the two schemes. The SCS-CN method is easily applicable on large areas, but has structural limitations. The Horton-like methods present parameters that, though measurable to a point, are difficult to achieve a reliable estimate at catchment scale. The objective of this work is to overcome these limits by proposing a calibration procedure which maintains the large applicability of the SCS-CN method as well as the continuous description of the infiltration process given by the Horton's equation suitably modified. The estimation of the parameters of the modified Horton method is carried out using a formal analogy with the SCS-CN method under specific conditions. Some applications, at catchment scale within a distributed model, are presented.

Hydrologic rainfall-runoff models are usually calibrated with reference to a limited number of recorded flood events, for which rainfall and runoff measurements are available. In this framework, model's parameters consistency depends on the number of both events and hydrograph points used for calibration, and on measurements reliability. Recently, to make users aware of application limits, major attention has been devoted to the estimation of uncertainty in hydrologic modelling. Here a simple numerical experiment is proposed, that allows the analysis of uncertainty in hydrologic rainfall-runoff modelling associated to both quantity and quality of available data. A distributed rainfall-runoff model based on geomorphologic concepts has been used. The experiment involves the analysis of an ensemble of model runs, and its overall set up holds if the model is to be applied in different catchments and climates, or even if a different hydrologic model is used. With reference to a set of 100 synthetic rainfall events characterized by a given rainfall volume, the effect of uncertainty in parameters calibration is studied. An artificial truth – perfect observation – is created by using the model in a known configuration. An external source of uncertainty is introduced by assuming realistic, i.e. uncertain, discharge observations to calibrate the model. The range of parameters' values able to "reproduce" the observation is studied. Finally, the model uncertainty is evaluated and discussed. The experiment gives useful indications about the number of both events and data points needed for a careful and stable calibration of a specific model, applied in a given climate and catchment. Moreover, an insight on the expected and maximum error in flood peak discharge simulations is given: errors ranging up to 40% are to be expected if parameters are calibrated on insufficient data sets.

Floods result from the limited carrying capacity of stream channels when compared to the discharge peak value. The transit of flood waves - with the associated erosion and sedimentation processes - often modifies local stream geometry. In some cases this results in a reduction of the stream carrying capacity, and consequently in an enhancement of the flooding risk. A mathematical model for the prediction of potential altimetric stream network evolution due to erosion and sedimentation processes is here formalized. It works at the regional scale, identifying the tendency of river segments to sedimentation, stability, or erosion. The model builds on geomorphologic concepts, and derives its parameters from extensive surveys. As a case study, tendencies of rivers pertaining to the Valle d'Aosta region are analyzed. Some validation is provided both at regional and local scales of analysis. Local validation is performed both through a mathematical model able to simulate the temporal evolution of the stream profile, and through comparison of the prediction with ante and post-event river surveys, where available. Overall results are strongly encouraging. Possible use of the information derived from the model in the context of flood and landslide hazard mitigation is briefly discussed.

The operational meteo-hydrological forecasting chain of the Liguria Region (NW Italy) is applied to a different morphoclimatic environment, such as the Emilia Romagna Region (N Italy). Modification to the chain, both in models and in procedures, are introduced to overcome problems related to medium dimension catchments (A≈1000km2), characterized by complex altimetry profiles and antropical interventions along the river. The main feature of the original operational procedure, that is the probabilistic approach, is maintained. Hydraulic hazard reduction through artificial reservoirs management is exploited with reference to a specific event occurred on the Reno basin (Emilia Romagna Region).

This article analyzes the significance of environmental management schemes (EMAS and ISO 14001) as of the measures, assisting in achieving the aims of sustainable development, discloses the substance of the requirements, related to the implementation of such measures, the similarities and differences between these environmental management schemes. Similarly, the article examines the meaning of the selection criterion, used in public procurement, requiring suppliers to apply environmental management schemes or measures, and reveals the complexity of evaluation of equivalence of non formal environmental management measures to EMAS and ISO 14001, with the purpose to ensure that a supplier has a technical ability to carry out a public contract in an environmentally sound manner.

- Abstract
Immunotherapies against brain metastases have shown clinical benefits when applied to asymptomatic patients, but they are largely ineffective in symptomatic cases for unknown reasons. Here we dissect the heterogeneity in metastasis-associated astrocytes using scRNAseq and report a population that blocks the antitumoral activity of infiltrating T cells. This pro-tumoral activity is mediated by the secretion of TIMP1 from a cluster of pSTAT3+ astrocytes that acts on CD63+ CD8+ T cells to modulate their function. Using genetic and pharmacologic approaches in mouse and human brain metastasis models, we demonstrate that combining immune checkpoint blockade antibodies with the inhibition of astrocyte-mediated local immunosuppression may benefit patients with symptomatic brain metastases. We further reveal that the presence of TIMP1 in liquid biopsies provides a biomarker to select patients for this combined immunotherapy. Overall, our findings demonstrate an unexpected immunomodulatory role for astrocytes in brain metastases with clinical implications.

28. Deep learning-based algorithm for postoperative glioblastoma MRI segmentation: a promising new tool for tumor burden assessment.

Objective: Clinical and surgical decisions for glioblastoma patients depend on a tumor imaging-based evaluation. Artificial Intelligence (AI) can be applied to magnetic resonance imaging (MRI) assessment to support clinical practice, surgery planning and prognostic predictions. In a real-world context, the current obstacles for AI are low-quality imaging and postoperative reliability. The aim of this study is to train an automatic algorithm for glioblastoma segmentation on a clinical MRI dataset and to obtain reliable results both pre- and post-operatively., Methods: The dataset used for this study comprises 237 (71 preoperative and 166 postoperative) MRIs from 71 patients affected by a histologically confirmed Grade IV Glioma. The implemented U-Net architecture was trained by transfer learning to perform the segmentation task on postoperative MRIs. The training was carried out first on BraTS2021 dataset for preoperative segmentation. Performance is evaluated using DICE score (DS) and Hausdorff 95% (H95)., Results: In preoperative scenario, overall DS is 91.09 (± 0.60) and H95 is 8.35 (± 1.12), considering tumor core, enhancing tumor and whole tumor (ET and edema). In postoperative context, overall DS is 72.31 (± 2.88) and H95 is 23.43 (± 7.24), considering resection cavity (RC), gross tumor volume (GTV) and whole tumor (WT). Remarkably, the RC segmentation obtained a mean DS of 63.52 (± 8.90) in postoperative MRIs., Conclusions: The performances achieved by the algorithm are consistent with previous literature for both pre-operative and post-operative glioblastoma's MRI evaluation. - Abstract
Patients with low-grade glioma frequently have brain tumor-related epilepsy, which is more common than in patients with high-grade glioma. Treatment for tumor-associated epilepsy usually comprises a combination of surgery, anti-epileptic drugs (AEDs), chemotherapy, and radiotherapy. Response to tumor-directed treatment is measured primarily by overall survival and progression-free survival. However, seizure frequency has been observed to respond to tumor-directed treatment with chemotherapy or radiotherapy. A review of the current literature regarding seizure assessment for low-grade glioma patients reveals a heterogeneous manner in which seizure response has been reported. There is a need for a systematic approach to seizure assessment and its influence on health-related quality-of-life outcomes in patients enrolled in low-grade glioma therapeutic trials. In view of the need to have an adjunctive metric of tumor response in these patients, a method of seizure assessment as a metric in brain tumor treatment trials is proposed., (© The Author(s) 2016. - Abstract
Despite advances in multidisciplinary approaches, the prognosis for most patients with malignant gliomas is poor. Malignant gliomas are highly vascularized tumors with elevated expression of vascular endothelial growth factor (VEGF), an important mediator of angiogenesis. Recent studies of bevacizumab, an anti-VEGF monoclonal antibody, alone or associated with chemotherapy, have demonstrated high response rates and prolongation of median and 6-month progression-free survival. Clinical evaluation of several multitarget small molecule tyrosine kinase inhibitors is ongoing. Other promising antiangiogenic compounds are cilengitide and continuous temozolomide. Toxicity is acceptable. Open issues are represented by patterns of tumor progression, resistance mechanisms and biomarkers.

- Abstract
Ependymomas are rare primary central nervous system tumors in adults. They occur most commonly in the spinal cord, where histopathologic evaluation is critical to differentiate the grade I myxopapillary ependymoma from the grade II ependymoma or grade III anaplastic ependymoma. Brain ependymomas are either grade II or III. Treatment for all grades and types includes maximum surgical resection. For myxopapillary ependymoma, complete removal while maintaining capsule integrity may be curative. Some grade II ependymomas may be observed carefully after imaging confirms complete resection, but grade III tumors require adjuvant radiation treatment. Radiation commonly is given to the region of tumor, except in cases in which there is imaging or cerebrospinal fluid evidence of tumor dissemination. Chemotherapy has not been studied extensively, although most reports suggest only modest benefit. Ongoing laboratory studies have uncovered important signal transduction pathways that may be better therapeutic targets, leading to the development of clinical trials using targeted agents.

- Abstract
Thirty-six patients diagnosed with diskitis from 1978 to 1988 and followed for an average of 29.2 months were reviewed. The study included 23 boys and 13 girls with an average age of 5.3 years. The initial symptoms varied, but distinct clinical patterns emerged and were identifiable in different age groups. Both the leukocyte count and sedimentation rate were elevated. Routine roentgenograms were positive for intervertebral disk space narrowing in 82% of cases, technetium bone scans positive in 72%, and magnetic resonance imaging positive in all recent cases. Treatment consisted of bed rest for all patients, plaster casts for 50%, antibiotics for 40%, and traction for 23%. Regardless of the treatment combination, the course of the disease in most children is benign. At the completion of the study, all patients were asymptomatic including three children who had recurrences. In spite of being asymptomatic, 74% had persistent roentgenographic changes. The administration of antibiotics appears to be appropriate when indicated, i.e., failure to respond to immobilization. Disk space aspiration or biopsy should be reserved for those cases that are refractory to immobilization and antibiotics.

- Abstract
Soft tissue neoplasms of the hand are rare in general and even more uncommon in children. The most commonly considered diagnoses include ganglion, inclusion cyst, lipoma, subcutaneous foreign body granuloma, and tendon sheath xanthoma. Aside from pressure sensitivity, masses on the palmar aspect of the hand are often asymptomatic. The case reported here identifies a primary neural neoplasm (neurilemmoma) that involves a small peripheral nerve in the hand of a twelve-year-old child. The patient had no neurologic symptoms and illustrates the need to include neurilemmomas in the differential diagnosis of hand masses in children.
