ZusammenfassungBei einer Störung im Ablauf der Wundheilung kann es zu einem verzögerten oder fehlenden Wundverschluss kommen. Als ein möglicher Störfaktor gilt die Ernährung. Man muss zwischen quantitativer und qualitativer Mangelernährung unterscheiden. Gerade übergewichtige Patienten leiden oft an einer qualitativen Mangelernährung und Sarkopenie. Spezifische Erkrankungen machen eine entsprechende diätetische Nahrungsempfehlung notwendig. So findet man bei dialysepflichtigen Patienten, Dekubitus-Patienten und Ulcus cruris-Patienten oft einen Eiweißmangel. Auch niedrige Level an Vitamin C, Zink, Folsäure, Vitamin D oder Spurenelementen können die Wundheilung negativ beeinflussen. Größere prospektive Studien sollten klären, ob eine Substitution dieser Mangelzustände den gewünschten positiven Effekt auf die Wundheilung hat. Es erscheint aber sinnvoll, eine ausgewogene Ernährung und an die individuellen Ernährungsgewohnheiten angepasste Verbesserung der Aufnahme von Vitaminen, Spurenelementen und Eiweißen anzustreben.

6. Geochemistry of the igneous rocks of the Belingwe Greenstone Belt: Alteration, contamination and petrogenesis

The Belingwe Belt contains an andesitic suite, two suites of komatiites associated with voluminous tholeiites, and assorted intrusive igneous rocks. Some of the latter include remarkably fresh komatiite and basalt, containing little altered olivine and clinopyroxene phenocrysts and optically fresh glass. However most rocks are altered, probably both by early hydrothermal and later regional metamorphism, as well as by recent weathering. Within the very fresh flows in the Reliance Formation SiO2, TiO2, Al2O3, FeO, MgO, Na2O, NiO, Cr2O3, Zr, Y, REE and platinum group elements vary in a manner consistent with models which involve fractionation of olivine of the observed compositions and consistent with experimentally determined partition coefficients. These elements were probably nearly immobile during alteration. The Bend and Reliance komatiite suites are compositionally distinct in incompatible trace element ratios (e.g. Zr/Ti, Y/Ti). Crustal contamination of komatiite lavas is probably less than 1 % by mass. The lowermost volcanic unit, the Hokonui Formation is, in contrast, a typical calc-alkaline sequence.

10. A measure of majorization emerging from single-shot statistical mechanics

The use of the von Neumann entropy in formulating the laws of thermodynamics has recently been challenged. It is associated with the average work whereas the work guaranteed to be extracted in any single run of an experiment is the more interesting quantity in general. We show that an expression that quantifies majorization determines the optimal guaranteed work. We argue it should therefore be the central quantity of statistical mechanics, rather than the von Neumann entropy. In the limit of many identical and independent subsystems (asymptotic i.i.d) the von Neumann entropy expressions are recovered but in the non-equilbrium regime the optimal guaranteed work can be radically different to the optimal average. Moreover our measure of majorization governs which evolutions can be realized via thermal interactions, whereas the non-decrease of the von Neumann entropy is not sufficiently restrictive. Our results are inspired by single-shot information theory.

We address the question of whether there is a way of characterizing the quantum information transport properties of a medium or material. For this analysis, the special features of quantum information have to be taken into account. We find that quantum communication over an isotropic medium, as opposed to classical information transfer, requires the transmitter to direct the signal toward the receiver. Furthermore, for large classes of media there is a threshold, in the sense that ‘sufficiently much’ of the signal has to be collected. Therefore, the medium's capacity for quantum communication can be characterized in terms of how the sizes of the transmitter and receiver have to scale with the transmission distance to maintain quantum information transmission. To demonstrate the applicability of this concept, an n -dimensional spin lattice is considered, yielding a sufficient scaling of δ ^n ^/3 with the distance δ .

13. Importance and Reliability of Small Basis Set CCSD(T) Corrections to MP2 Binding and Relative Energies of Water Clusters

MP2 describes hydrogen-bonded systems well, yet a higher-order electron correlation correction in the form of a CCSD(T) calculation is usually necessary to achieve benchmark quality energies. We evaluated the importance and reliability of small basis set CCSD(T) corrections to MP2 (δ(MP2)(CCSD(T))) both on the binding (ΔE) and relative (ΔΔE) energies for a large number of systems including four water dimer stationary points and 57 other clusters up to undecamers, (H2O)11. By comparing the MP2 energies with CCSD(T) and the explicitly correlated MP2-F12 energies with variants of CCSD(T)-F12 using different basis sets, we were able to establish that the correction to the binding energy (ΔE) is sensitive to basis set size, especially for small double-ζ basis sets. On the other hand, the basis set sensitivity of the correction to the relative energy (ΔΔE) within each cluster size is very small. While the δ(MP2)(CCSD(T)) correction to the binding energy might vary in magnitude with basis set size, its effect on relative energy (and hence the stability of isomers) is remarkably consistent. Therefore, we recommend the inclusion of this correction to obtain the relative stability of closely spaced isomers using a double-ζ basis set with polarization and diffuse functions such as aug-cc-pVDZ.

The first observation of (anti)deuterons in deep inelastic scattering at HERA has been made with the ZEUS detector at a centre-of-mass energy of 300--318 GeV using an integrated luminosity of 120 pb-1. The measurement was performed in the central rapidity region for transverse momentum per unit of mass in the range 0.3, Comment: 26 pages, 9 figures, 5 tables, submitted to Nucl. Phys. B

Einer chronischen Wunde konnen eine Vielzahl von Erkrankungen ursachlich zugrunde liegen. Es sollte daher stets eine Differenzialdiagnostik erfolgen, um zielgerichtet therapeutisch agieren zu konnen. Zudem sollte bei therapieresistenten Ulzera regelmasig die gestellte Diagnose uberpruft und gegebenenfalls uberdacht werden. Neben der Auswahl geeigneter lokaler Therapiemasnahmen unter Berucksichtigung des Wundzustandes spielen allgemeine Faktoren wie Patientencompliance, Kosteneffizienz und Dauer der Behandlung zunehmend eine Rolle. In dieser Ubersicht wollen wir 10 Empfehlungen fur die Behandlung chronischer Wunden geben, die in Ihrem Arbeitsalltag als Unterstutzung oder Diagnose-Algorithmus dienen konnen.

A new method is employed to measure the neutral current cross section up to Bjorken-x values of one with the ZEUS detector at HERA using an integrated luminosity of 65.1 pb-1 for e+p collisions and 16.7 pb-1 for e-p collisions at sqrt{s}=318 GeV and 38.6 pb-1 for e+p collisions at sqrt{s}=300 GeV. Cross sections have been extracted for Q2 >= 648 GeV2 and are compared to predictions using different parton density functions. For the highest x bins, the data have a tendency to lie above the expectations using recent parton density function parametrizations., 13 pages, 14 figures, 6 tables, DESY-06-116, submitted to Eur. A search for stop production in R-parity-violating supersymmetry has been performed in $e^{+}p$ interactions with the ZEUS detector at HERA, using an integrated luminosity of 65 pb$^{-1}$. At HERA, the R-parity-violating coupling $\lambda'$ allows resonant squark production, $e^+d\to\tilde{q}$. Since the lowest-mass squark state in most supersymmetry models is the light stop, $\tilde{t}$, this search concentrated on production of $\tilde{t}$, followed either by a direct R-parity-violating decay, or by the gauge decay to $b\tilde{\chi}^+_{1}$. No evidence for stop production was found and limits were set on $\lambda'_{131}$ as a function of the stop mass in the framework of the Minimal Supersymmetric Standard Model. As institutional practices become increasingly digitized, the role of faculty, scholars, and professionals are constantly reshaped and re-negotiated. The rise and use of emerging technologies in the field of research, has the potential to significantly impact the individual researcher, their institutions and ultimately the State. The project Critical Conversations Research Network is a part of a broader initiative undertaken by the Tennessee Board of Regents Office of Academic Affairs.

ZusammenfassungEs existiert eine Vielzahl von Dermatosen, die im Rahmen einer Adipositas entweder direkt oder indirekt assoziiert auftreten können. Eine begleitende Fettstoffwechselstörung kann zu Xanthomen oder Xanthelasmen führen, eine begleitende Hyperinsulinämie und Diabetes mellitus zu einer Acanthosis nigricans oder Hyperandrogenismus mit Virilisierung. Indirekt assoziierte Folgen der Adipositas an der Haut sind u. a. eine Candidose, Keratosis pilaris, Follikulitiden oder plantare Hyperkeratosen. Wichtig ist in jedem Fall der kausale Therapieversuch durch Gewichtsreduktion sowie gegebenenfalls eine zusätzliche spezifische Therapie der Dermatose. Manche der Hautveränderungen sind nicht nur kosmetischer Natur, sondern können auch auf ein erhöhtes kardiovaskuläres Risiko mit erhöhter Mortalität hindeuten, wie etwa Xanthome oder eine Psoriasis vulgaris.

The PROSIT (Proteinuria Screening and Intervention) Project started in 1993 in order to obtain data on the prevalence of micro- and macroalbuminuria in diabetic patients treated in primary care, to establish an easy screening programme for microalbuminuria, in which also diabetic patients can participate in self-responsibility, and to implement a specific intervention programme for incipient nephropathy. In 58 representative doctor's offices 647 diabetic patients were included, who performed at home self-tests for microalbuminuria on three days within one week using the early morning urine and a newly developed qualitative immunologic test-strip for microalbuminuria. After storage they returned the same urine samples to their doctors' offices for semiquantitative retesting with the immunologic test-strip Micral-Test II. In case of positive results the proteinuria dipstick Combur-9-Test was applied in order to exclude other causes of positive microalbuminuria (e.g. urinary tract infection). Data of 569 patients (6% Type 1, 88% Type 2 and 6% secondary diabetes) could be analysed. Both qualitative self-testing for microalbuminuria at home and semiquantitative retesting in doctors' offices were found to be feasible. Based on semiquantitative retesting the prevalences of microalbuminuria (macroalbuminuria) were 19.6% (0%) in Type 1 diabetes, 17.2% (10.8%) in Type 2 diabetes and 11.7% (7.8%) in secondary diabetes. Type 2 diabetic patients showed a clear correlation between albuminuria and diabetes duration, HbA1c, serum creatinine, triglycerides as well as micro- and macrovascular complications. 227 patients with micro- or macroalbuminuria were included into the ongoing PROSIT intervention programme.

Das Programmsegment der Ratgeber- und Lebenshilfeformate hat in den letzten Jahren zugenommen. Das zeigt sich insbesondere, wenn man aktuelle Zahlen mit den Ergebnissen einer Programmbeobachtung der Ratgebersendungen im Jahr 1979 vergleicht. Dabei ist klar zu erkennen, dass fur die Zunahme des Sendevolumens die Entwicklung von neuen Formaten entscheidend war, die nicht mehr einmal die Woche, sondern taglich ausgestrahlt werden. Die neuen Formate entstanden nicht durch eine thematische Ausdifferenzierung wie im Zeitschriftensektor, sondern durch unterschiedliche Ausdifferenzierungen der kommunikativen Handlung „einen Rat geben“, die das Grundschema aller Ratgeber- und Lebenshilfeformate bildet. Weitere Faktoren sind die Einbeziehung von Unterhaltungselementen und die fernsehspezifische Umsetzung der einzelnen Sendekonzepte als Studio-, Film- oder Magazinformate. Eine Schlusselrolle kommt den Online-Auftritten zu, die alle Ratgeberformate eingerichtet haben. Mithilfe des Internets wurden neue Formen der medialen Beratung moglich, was die Entwicklung neuer Ratgeberformate erleichterte. English Karl N. Renner: Expansion of advising and counselling TV programs in Germany. A quantitative und qualitative analysis The segment of advising and counselling TV programs got a remarkable increase during the last years. This becomes obvious, if one compares the actual volume of these programs with the volume of 1979, the last period before the release of private TV programs in Germany. It’s also obvious that this increase was only made possi- ble with the creation of new TV formats which could be broadcasted every day and not only once a week. These new formats didn’t emerge by topical differentiation like newspaper journals, but by differentia- tion of the communicative act „to advise“, which is the fundamental procedure of all advising and counselling formats. Further factors are the influence of entertaining elements and the particular concepts TV-specific transformation as a studio-, film- or magazine-format. The advising formats’ online presence which they created in the course of media convergence got a key function for this expansion. As the TV formats’ new internet modules enabled new kinds of medial advising and thus fostered the evolution of new advising formats.

Deep inelastic scattering and its diffractive component, ep {yields} e{prime} {gamma}* p {yields} e{prime} XN, have been studied at HERA with the ZEUS detector using an integrated luminosity of 52.4 pb{sup -1}. The M{sub X} method has been used to extract the diffractive contribution. A wide range in the center-of-mass energy W(37-245 GeV), photon virtuality Q{sup 2} (20-450 GeV{sup 2}) and mass M{sub X} (0.28-35 GeV) is covered. The diffractive cross section for 2 < M{sub X} < 15 GeV rises strongly with W, the rise becoming steeper as Q{sup 2} increases. The data are also presented in terms of the diffractive structure function, F{sub 2}{sup D(3)}, of the proton. For fixed Q{sup 2} and fixed M{sub X}, xpF{sub 2}{sup D(3)} shows a strong rise as xP {yields} 0, where xp is the fraction of the proton momentum carried by the pomeron. For Bjorken-x < 1 x 10{sup -3}, xpF{sub 2}{sup D(3)} shows positive logQ{sup 2} scaling violations, while for X {ge} 5 x 10{sup -3} negative scaling violations are observed. The diffractive structure function is compatible with being leading twist. The data show that Regge factorization is broken.

We present a patient with prolonged and therapeutically resistant pemphigus foliaceus despite appropriate immunosuppressive treatment. As a severe and life-threatening complication, the patient acquired a herpes simplex superinfection with bilateral keratitis herpetica as well as bacteriemia following streptococcus pyogenes superinfection. After disease control, extensive thrombosis and pulmonary embolism occurred which may be partially caused by the prednisolone therapy. This case demonstrates the high risk potential of patients with pemphigus foliaceus.

In five women (mean age 58 [31-74] years) with clinically diagnosed varicose phlebitis of the long saphenous vein appositional thrombus growth was demonstrated by serial ultrasonography (B-mode compression and colour-coded duplex sonography). In one patient the appositional thrombus was found to be free-floating as far as the common femoral vein, but this was not seen by phlebography. During therapeutic heparinization there was ultrasonographic evidence of softening and partial liquefaction of thrombus material in a cranial direction. There were no clinical signs of pulmonary embolism, but pulmonary perfusion scintigraphy demonstrated perfusion deficit characteristic of emboli. Four of the five patients had a surgical crossectomy and partial saphenous vein resection. The congruence between ultrasonographic and macro- as well as micropathological findings demonstrates the great value of B-mode and duplex ultrasound examination in the area of the epifascial veins. Ultrasonography should be performed in every case of varicose phlebitis of the large veins to exclude the presence of apposition thrombi.

Porcine and semisynthetic human insulin preparations (Nordisk) having the same content of regular insulin, same galenic characteristics and same degree of purity, were tested in type I, type IIa and type IIb diabetics. There was no difference as to the duration of action. This is contrary to some earlier reports stating that protamine-bound biosynthetic human insulin has a shorter duration of action than porcine preparations. The onset of the hypoglycaemic action in type IIa diabetics is significantly more rapid with isophane human insulin than with isophane pork insulin. The use of an insulin pump prior to testing the duration of action of intermediate insulin preparations appears to be superior to other methods.

In eight type I diabetics-three of them under intensified insulin therapy and five on continuous subcutaneous insulin infusion (CSII)-blood-glucose concentrations fell significantly during 20-minute isometric and dynamic muscle work at about 50% of individual maximal work capacity and subsequent intensive exercise of five minutes until exhaustion. However, 6 1/2 hours later blood-glucose levels (221 +/- 39 mg/dl after isometric exercise and 219 +/- 24 mg/dl after dynamic exercise) were clearly above the mean level measured at the same time of day on the three preceding days of 138 +/- 12 mg/dl (P less than 0.05). No hypoglycaemia occurred. This blood-sugar profile can be explained by a significant rise in cortisol and growth hormone and their long-lasting blood-sugar raising effect.

In 15 type I diabetics the mean postprandial rise in blood glucose levels, 90 min after breakfast at about 8 a.m. (the customary time for this meal in hospitals), was 62.2 +/- 34.3 mg/dl, but 90 min after a breakfast which had been put forward by about 1 1/2 hours it was only 2.6 +/- 18.6 mg/dl (P less than 0.001). Seven patients developed hypoglycaemia after the early breakfast, none after the late one. This finding suggests that, to avoid hypoglycaemia after an early breakfast, an additional small meal should be given at about 8:30 a.m. or the insulin content of the morning injection should be changed. The clear connection between the time of the morning insulin injection and the insulin action during the morning explains why, in uninstructed patients who--under their usual daily conditions of starting school or work--are forced to inject insulin early or to eat early, a state of hypoglycaemia may occur.

The production of D-*+/-: (2010) mesons in e(+/-) p scattering in the range of exchanged photon virtuality 0.05 D-0 pi(+) with D-0 -> K-pi(+) and corresponding antiparticle decay were used to identify D* mesons and the ZEUS beampipe calorimeter was used to identify the scattered electron. Differential D* cross sections as functions of Q(2), inelasticity, y, transverse momentum of the D* meson, p(T)(D*), and pseudorapidity of the D* meson, eta(D*), have been measured in the kinematic region 0.02 < y < 0.85, 1.5 < p(T)(D*) < 9.0 GeV and \eta(D*)\< 1.5. The measured differential cross sections are in agreement with two different NLO QCD calculations. Diode-pumped lasers based on mixed Nd:Gd0.7Y0.3VO4 vanadate crystals are studied. Continuous-wave lasing with the slope efficiency of 71% and the average output power up to 8.2 W is obtained. Active mode locking with an acousto-optic modulator, passive mode locking by the Kerr nonlinearity, and hybrid mode locking are investigated. Picosecond laser pulses of duration 1.7 ps are obtained at an average output power of 340 mW and a pulse repetition rate of 140 MHz.

Active fibre devices are conventionally manufactured using MCVD technique. Recently it has been shown that nearly equivalent results can also be obtained with sol–gel technology. Now we present a novel technique allowing simplification of the manufacturing process even more. The required constituents are mixed in the form of dry micro- and nano-sized particles. A silica glass tube forming the future core region of a fibre preform is filled with a powder mix of SiO2, 1% Nd (as Nd2O3) and 10% Al (as Al2O3). This tube is mounted in the centre of a larger tube forming the future cladding. The empty space between the two tubes is filled with SiO2 powder. After preheating, the evacuated preform is drawn to a fibre. A length of 45 cm, cladding-pumped with a diode laser at 808 nm as well as a core-pumped fibre of 5.1 cm length showed laser action between 1.05 and 1.1 μm.

The cross sections for charged and neutral current deep inelastic scattering in e + p collisions with a longitudinally polarised positron beam have been measured using the ZEUS detector at HERA. The results, based on data corresponding to an integrated luminosity of 23.8 pb−1 at s = 318 GeV , are given for both e + p charged current and neutral current deep inelastic scattering for both positive and negative values of the longitudinal polarisation of the positron beam. Single differential cross sections are presented for the kinematic region Q 2 > 200 GeV 2 . The measured cross sections are compared to the predictions of the Standard Model. A fit to the data yields σ CC ( P e = − 1 ) = 7.4 ± 3.9 ( stat. ) ± 1.2 ( syst. ) pb , which is consistent within two standard deviations with the absence of right-handed charged currents in the Standard Model.

Objectives – To assess the efficacy and safety of high-dose (up to 20 mg/day) cabergoline in Parkinson's disease (PD) patients with motor fluctuations and/or dyskinesias. Materials and methods – Thirty-four PD patients had cabergoline up-titrated and their levodopa (l-dopa) reduced over a maximum of 20 weeks, followed by at least 6 weeks steady cabergoline dosing. Primary endpoint was change in mean hyperkinesia intensity at the final visit (week 26). Results – Mean (± SD) cabergoline was increased from 6.43 ± 2.66 to 12.78 ± 5.67 mg/day and mean l-dopa reduced from 606.6 ± 263.9 to 370.6 ± 192.5 mg/day. A significant reduction (P

The diffractive photoproduction of dijets has been studied using 77.6 pb−1 of data taken by the ZEUS detector at HERA. The measurements have been made in the kinematic range 0.2 x IP 0.035 , where y is the inelasticity and x IP is the fraction of the proton momentum taken by the diffractive exchange. The jets are reconstructed using the k T algorithm. The two highest transverse energy jets are required to satisfy E T > 7.5 and 6.5 GeV, respectively, and to lie in the pseudorapidity range −1.5

The dramatic expansion into the Wildland–Urban Interface (WUI) places property, natural assets, and human life at risk from wildfire destruction. The U.S. National Fire Plan encourages communities to implement laws and outreach programs for pre-fire planning to mitigate the risk to area residents. Starting in 2003, we surveyed the administrators of regulatory and voluntary wildfire risk reduction programs in 25 U.S. states. These state and local programs are listed on the United States Department of Agriculture (USDA) Forest Service's National Wildfire Programs Database website, www.wildfireprograms.usda.gov , and are concerned with vegetation management on private lands. Analyses of the administrators' responses suggest several new insights about these risk mitigation efforts, including 1) how they are organized, 2) what they are trying to accomplish, 3) what are the obstacles, and 4) how well they may be working. In our study we describe the goals and objectives of these programs as well as the obstacles confronting managers. Further, we explore trends in these programs including participation in collaborative planning and use of program evaluation to measure progress toward goals. Additionally, we explore the program managers' perceptions of what are their most effective programs for creating defensible space.

The beauty production cross section for deep inelastic scattering events with at least one hard jet in the Breit frame together with a muon has been measured, for photon virtualities Q^2 > 2 GeV^2, with the ZEUS detector at HERA using integrated luminosity of 72 pb^-1. The total visible cross section is sigma_b-bbar (ep -> e jet mu X) = 40.9 +- 5.7 (stat.) +6.0 -4.4 (syst.) pb. The next-to-leading order QCD prediction lies about 2.5 standard deviations below the data. The differential cross sections are in general consistent with the NLO QCD predictions; however at low values of Q^2, Bjorken x, and muon transverse momentum, and high values of jet transverse energy and muon pseudorapidity, the prediction is about two standard deviations below the data., 18 pages, 4 figures

A resonance search has been made in the K0s p and K0s pbar invariant-mass spectrum measured with the ZEUS detector at HERA using an integrated luminosity of 121 pb-1. The search was performed in the central rapidity region of inclusive deep inelastic scattering at an ep centre-of-mass energy of 300-318 GeV for exchanged photon virtuality, Q2, above 1 GeV2. Recent results from fixed-target experiments give evidence for a narrow baryon resonance decaying to K+ n and K0s p, interpreted as a pentaquark. The results presented here support the existence of such state, with a mass of 1521.5+/-1.5(stat.)^{+2.8}_{-1.7}(syst.) MeV and a Gaussian width consistent with the experimental resolution of 2 MeV. The signal is visible at high Q2 and, for Q2>20 GeV2, contains 221+/-48 events. The probability of a similar signal anywhere in the range 1500-1560 MeV arising from fluctuations of the background is below 6x10^{-5}., 4 eps figures, 15 pages, 1 table, accepted by Physics Letters B

AIM: To evaluate the technical performance of the non-covered CHOO enteral stent in the stomach, duodenum and jejunum. METHODS: In 8 patients (two men, 6 women) with malignant tumors of the stomach (n = 4), duodenum (n = 3) and jejunum (n = 1) stent implantation was performed under fluoroscopic control. In two patients endoscopy was needed additionally. The patients were followed for 1 to 168 days. RESULTS: Stent implantation was technically successful in 7 of 8 patients. In 6 patients symptoms improved. In two patients a second stent implantation was necessary 35 and 161 days after primary implantation, respectively. There was no stent migration or tumor ingrowth. CONCLUSION: The implantation of the non-covered CHOO enteral stent into the upper gastrointestinal tract is safe and effective.

Due to the increasing problem of antibiotic resistance in gram-negative pathogens, the Commission for Hospital Hygiene and Infection Prevention (KRINKO) decided to establish a new clinically oriented definition of multi-resistance. Gram-negative pathogens with a multidrug-resistance (MRGN) are divided into those with resistance to three (3MRGN) or four (4MRGN) antibiotic groups.In this multicenter study which was done in ten dermatological wound clinics, the bacteriological swabs from up to 100 patients with chronic leg ulcers per center were analyzed according to the current classification KRINKO and evaluated.Overall, the results of 970 patients (553 women, 417 men) could be evaluated. We found 681 gram-positive and 1155 gram-negative bacteria. Pseudomonas aeruginosa was with a detection-rate of 31.1% the most frequent gram-negative pathogen, followed by Proteus mirabilis with 13.7% and various enterobacteria with 28.6%. According to the current KRINKO classification,eight patients with 4MRGN and 34 patients with 3MRGN could be identified.Our results demonstrate the current spectrum of bacteria in patients with chronic leg ulcers with a variety of gram-negative pathogens, some of which are classified as multi-drug resistant. As a clinical consequence some of the patients require individualized preventive measures and therapy.

Background and objective: Even though there are simple and cost-effective means for the early diagnosis of diabetic nephropathy, only a small proportion of diabetics in Germany is regularly tests for microalbuminuria. On the basis of evidence-based knowledge and of international guidelines the PROSIT project (proteinuria screening and intervention) aims to make good this deficiency in the German Federal Republic by introducing nephropathy screening and a structured intervention to improve blood sugar and blood pressure regulation, optimizing lipid metabolism and nutritional intake. It was the aim of this study to assess with a computer-aided diabetes model the clinical value and cost-effectiveness of such an intervention. Patients and methods: From data collected for 589 diabetics who participated in the PROSIT project, the short-time effects after one year on HbA1(c), systolic blood pressure and lipid levels were obtained and cost-effectiveness compared with that of standard care. Life expectancy, life-time costs to be met by health insurance and event frequency of the diabetic nephropathy stages were calculated with a Markov model for type 2 diabetics. Results: PROSIT improved individual life expectancy by 0.23 years with reduction of life-time costs by DM 9,772 (ca. $ 4,900). The cumulative incidence of microalbuminuria was lowered by 30.5 %, that of terminal renal failure by 55.9%. Even after discounting the results (i.e. the inclusion of time preference for cost and benefit) and stepwise changes of all variables by ± 10%, PROSIT remained the more cost-effective variant. Conclusion: From a health economy viewpoint PROSIT is superior to standard management. Early recognition of albuminuria and the introduction of a multifactorial treatment strategy make it possible to delay progression to terminal renal failure. In addition to its clinical benefits, prevention of dialysis and transplantation would reduce the annual savings of the health care system by several billion DM.

Forward jet cross sections have been measured in neutral current deep inelastic scattering at low Bjorken-x with the ZEUS detector at HERA using an integrated luminosity of ${81.8 \rm pb}^{-1}$. Measurements are presented for inclusive forward jets as well as for forward jets accompanied by a dijet system. The explored phase space, with jet pseudorapidity up to 4.3 is expected to be particularly sensitive to the dynamics of QCD parton evolution at low x. The distribution of the azimuthal angle of charged and neutral hadrons relative to the lepton plane has been studied for neutral current deep inelastic $ep$ scattering using an integrated luminosity of 45 pb-1 taken with the ZEUS detector at HERA. The measurements were made in the hadronic centre-of-mass system. The analysis exploits the energy-flow method, which allows the measurement to be made over a larger range of pseudorapidity compared to previous results. The dependence of the moments of the azimuthal distributions on the pseudorapidity and minimum transverse energy of the final-state hadrons are presented. Although the predictions from next-to-leading-order QCD describe the data better than do the Monte Carlo models incorporating leading-logarithm parton showers, they still fail to describe the magnitude of the asymmetries. The photoproduction of prompt photons, together with an accompanying jet, has been studied in ep collisions at a centre-of-mass energy of 318 GeV with the ZEUS detector at HERA using an integrated luminosity of 77 pb-1. Cross sections were measured for the transverse energy of the photon and the jet larger than 5 and 6 GeV, respectively. The differential gamma+jet cross sections were reconstructed as functions of the transverse energy, pseudorapidity and x_gamma^obs, the fraction of the incoming photon momentum taken by the photon-jet system. Predictions based on leading-logarithm parton-shower Monte Carlo models and next-to-leading-order (NLO) QCD generally underestimate the cross sections for the transverse energies of prompt photons below 7 GeV, while the kT-factorisation QCD calculation agrees with the data better. When the minimum transverse energy of prompt photons is increased to 7 GeV, both NLO QCD and the kT-factorisation calculations are in good agreement with the data., Comment: 22 pages, 7 eps figues, 2 tables

The photoproduction of dijet events, where the two jets with the highest transverse energy are separated by a large gap in pseudorapidity, have been studied with the ZEUS detector using an integrated luminosity of 39 pb$^{-1}$. Rapidity-gap events are defined in terms of the energy flow between the jets, such that the total summed transverse energy in this region is less than some value $E_{\rm T}^{\rm CUT}$. The data show a clear excess over the predictions of standard photoproduction models. This is interpreted as evidence for a strongly interacting exchange of a color-singlet object. Monte Carlo models which include such a color-singlet exchange are able to describe the data., Comment: 28 pages, 13 figures, accepted by EPJ C

The production of dijets in diffractive deep inelastic scattering has been measured with the ZEUS detector at HERA using an integrated luminosity of $61 \pbi$. The dijet cross section has been measured for virtualities of the exchanged virtual photon, $5 < Q^2 < 100 \gev^2$, and $\gamma^{*} p$ centre-of-mass energies, 100 < W < 250 GeV. The jets, identified using the inclusive k_{T} algorithm in the $\gamma^* p$ frame, were required to have a transverse energy $E^*_{T, \rm jet} > 4 \gev$ and the jet with the highest transverse energy was required to have $E^*_{T,\rm jet} > 5 \gev$. All jets were required to be in the pseudorapidity range $-3.5 < \eta^*_{\rm jet} < 0$. The differential cross sections are compared to leading-order predictions and next-to-leading-order QCD calculations based on recent diffractive parton densities extracted from inclusive diffractive deep inelastic scattering data., Proceedings of DIS 2007, p.671-674, ISBN 978-3-935702-23-2, ISSN 1435-8077
