Abstract

INTRODUCTION: In this world of internet and artificial intelligence, Natural Language Processing has emerged as mostdemanding research area. Under Natural Language Processing, sentence simplification is one of the research area thatdeals with simplification or conversion of complex sentences in to simple sentences. OBJECTIVES: In this researcharticle, author has proposed a novel approach for conversion (simplification) of complex sentences (participial type) ofPunjabi language into easily understandable simple sentences. METHODS: Author performed lexical and morphologicalsimplification by using morphological features of the language. Morphological features are used to identify the participialtype complex sentences. RESULTS: on testing the proposed algorithm, a precision of 96.39%, recall of 91.37% and Fmeasure as 93.79% was reported. CONCLUSION: the developed system can be helpful for Aphasic and Dyslexia readersand can be used as subpart for machine translation system, summarization system and other Natural Language Processingapplications.

Abstract

A novel interpenetrating polymer network (IPN) has been synthesized through enzymatic initiation using lipase as initiator, glutaraldehyde as cross-linker, acrylic acid as primary monomer and acrylamide as secondary monomer. Biodegradability of synthesized interpenetrating polymer network was studied through soil burial and composting methods. Synthesized hydrogel was completely degraded within 70 days using composting method, while it was 86.03% degraded within 77 days using soil burial method. This was confirmed by Fourier transform Infrared spectroscopy (FTIR) and Scanning electron microscopy (SEM) techniques. Synthesized interpenetrating polymer network hydrogel was used as a device for controlled release of urea and also act as water releasing device. Their impact on soil fertility and plant growth was also studied. The initial diffusion coefficient has a greater value than the later diffusion coefficient indicating a higher fertilizer release rate during the early stage. Fertilizer release kinetic was also studied which showed Non-Fickian diffusion behavior, as the rate of fertilizer release was comparable to the relaxation time of the synthesized matrix. Synthesized IPN enhance the water uptake capacity up to 6.2% and 7.2% in sandy loam and clay soil, respectively.

4. Efficacy and safety of hydroxychloroquine for managing glycemia in type-2 diabetes: A systematic review and meta-analysis

Abstract

No Cochrane meta-analysis with grading of evidence is available on use of hydroxychloroquine (HCQ) in type-2 diabetes (T2DM). This meta-analysis evaluated the efficacy and safety of HCQ in T2DM.Electronic databases were searched using a Boolean search strategy: ((hydroxychloroquine) OR (chloroquine*)) AND ((diabetes) OR ("diabetes mellitus") OR (glycemia) OR (glucose) OR (insulin)) for studies evaluating hydroxychloroquine for glycemic control in T2DM. The primary outcome was a change in glycated haemoglobin (HbA1c). The secondary outcomes were changes in other glycemic/lipid parameters and adverse effects.Data from 11 randomized controlled trials (RCTs) (3 having placebo as controls [passive controls] and 8 having anti-diabetes medications as controls [active controls]) involving 2,723 patients having a median follow-up of 24 weeks were analyzed. About 54.54% of the RCTs were of poor quality as evaluated by the Jadad scale. The performance bias and detection bias were at high risk in 63.64% of the RCTs. The HbA1c reduction with HCQ was marginally better compared to the active (mean differences [MD]-0.17% [95%, CI:-0.30--0.04;P=0.009;I2=89%; very low certainty of evidence, VLCE]), and passive (MD-1.35% [95%CI:-2.10--0.59;P=0.005;I2=74%]) controls. A reduction in fasting glucose (MD-16.63mg/dL[95%, CI: -25.99 - -7.28mg/dL;P0.001;I2=97%;VLCE]) and post-prandial glucose [MD -8.41mg/dL (95%CI: -14.71 - -2.12mg/dL;P=0.009;I2=87%;VLCE]), appeared better with HCQ compared to active controls. The total adverse events (risk ratio [RR]0.93 [95% CI:0.68-1.28]; P=0.65;I2=66%) were not different with HCQ compared to the controls.The routine use of HCQ in T2DM cannot be recommended based on the current evidence.

5. Dermoscopy of skin parasitoses, bites and stings: a systematic review of the literature

Abstract

Besides classic applications, dermoscopy has gained significant appreciation also for non-tumoral dermatoses (general dermatology). In this field, skin parasitoses (infestations), bites and stings have been reported among the conditions that may benefit most from dermoscopic examination. However, published data on this topic are sparse and often lack a standardized analytical approach. In this systematic review of the literature, we sought to summarize available data on dermoscopy of cutaneous parasitoses, bites and stings, analysing dermoscopic features and used setting (polarization or not/magnification), pathological correlation and level of evidence of published studies as well as trying to align dermoscopic terminology of reported findings following a standardized methodology. A total of 150 publications addressing 29 different dermatoses and reporting 96 different dermoscopic findings were included in the analysis. Most of them displayed a level of evidence of V (113 single case reports and 36 case series), with only one study featuring a level of evidence of IV (case-control study). Additionally, our analysis also displayed poor uniformity in the terminology used in published articles (even for the same dermatosis). Therefore, albeit promising, dermoscopy of cutaneous parasitoses, bites and stings needs future systematized studies for a better characterization.

Abstract

Introduction: Cataract is the major cause of treatable blindness in India (62.6 %). The state of Gujarat is performing more than 100% of the target given by NPCB. Ophthalmic services in private setup are very costly. Government tertiary eye care centres are now well equipped. So assessing perception of patients is very important is- sues and that will help to improve the services. Objective of the study was to assess the perceptions of cataract patients about the health services. Method: This cross sectional study was carried out between August 2012 and October 2012 with purposive sampling of 300 cases. Results: 52.3% patients came here due to good quality service. Im- proved visual acuity is not necessarily the most important factor for patient satisfaction after cataract surgery as many patients are influ- enced by the care that is provided by medical (77.7%) and paramed- ical staff (57.6%). The out-patient department significantly affected the level of patient satisfaction. 80%were satisfied with overall eye care services provide at hospital and 88.3% cases would recommend others to take eye care services at this centre. Conclusions: Highly competent and professional healthcare person- nel are required for providing highest quality and satisfaction to the patients.

Abstract

Background: PARADIGM-HF demonstrated superiority of sacubitril/valsartan (sac/val) over enalapril in patients with heart failure with reduced ejection fraction (HFrEF). However, patients in clinical practice may differ in their characteristics and overall risk compared with patients in clinical trials, and additional outcomes can be observed in real world (RW). Hence, a systematic review was conducted to identify and describe RW data on sac/val. Methods: RW studies evaluating the effects of sac/val in adult patients with HFrEF with a sample size ≥100 were identified via MEDLINE® and Embase® from 2015 to January 2020. Citations were screened, critically appraised and relevant data were extracted. Results: A total of 68 unique studies were identified. Nearly half of the studies were conducted in Europe (n = 34), followed by the US (n = 15) and Asia (n = 11). Median follow-up period varied from 1 to 19 months. Mean age ranged between 48.7 and 79.0 years; patients were mostly male and in New York Heart Association (NYHA) functional class II/III, and mean left ventricular ejection fraction varied between 23%and 38%. Of studies performing comparisons, most reported superior efficacy of sac/val in reducing the risk of HF hospitalisations, all-cause hospitalisations, and all-cause mortality as compared to standard-of-care. Many studies reported significant improvements in NYHA functional class and reduction in biomarker levels post sac/val. Hypotension and hyperkalaemia were the most frequently reported adverse events. Conclusions: This comprehensive overview of currently available RW evidence on sac/val complements the evidence from randomised controlled trials, substantiating its effectiveness in heterogeneous real-world HF populations.

13. Modelling a spade rudder as a hollow two-way tapered Kirchhoff’s plate : free dry and wet vibration study with numerical verification

A semi-analytical approach to free dry and wet vibration of a trapezoidal, 2-way tapered, pivoted hollow spade rudder is presented. The rudder is modeled as a hollow Kirchhoff’s plate, with a NACA0018 profile chord section. The rudder pivot is modeled as a combination of a translational spring and a rotational spring. The span-wise and chord-wise non-uniform beam vibration is first analyzed by the Rayleigh-Ritz method, to establish the non-uniform beam modeshapes, which act as admissible functions to the Galerkin’s method for plate vibration. Eigenvalue analysis generates the plate natural frequencies and the plate modeshapes. Alternately, uniform beam modeshapes themselves are used as admissible functions into the Galerkin’s method. Frequencies are analyzed for various pivot positions, taper ratios, and NACA sections. For the wet vibration, constant strength source distribution technique is used to generate the added mass of a 2D aerofoil. Also, 3D panel method is used to generate the modal added masses, and hence the wet natural frequencies. The added mass coefficient is generated for various aerofoil fineness ratios, pivot fixities, taper ratios, aspect ratios.

14. Forced wet flexural dynamics of spade rudder in propeller slipstream: semi-analytical approach of pivoted FFFF plate

Abstract

A semi-analytical approach of forced vibration of a trapezoidal, two-way tapered, hollow model spade rudder, is presented. The pivoted rudder is in the propeller slipstream; which causes harmonic hydrodynamic load. The excitation frequency of propeller-induced vibration (PIV) equals the propeller blade passing frequency. The rudder operates at a Reynolds’s number of the order of 108. It has a span of 1 m, root chord of 0.6 m, and tip chord of 0.5 m. It is considered as a hollow Free–Free–Free–Free Kirchhoff’s plate, with the chord section as an NACA0018 profile. The dry vibration is analyzed by the energy-based Galerkin’s method, using closed-form admissible functions in the two perpendicular directions. 3D constant-strength panel method is used to generate the modal added masses, and hence the wet natural frequencies. The drag and the lift coefficient of the rudder, at various angles of attack, are estimated to find the spatial distribution of the total PIV-causing normal force. The mean flow velocity about the rudder is the velocity of advance behind the propeller. The fluid velocity past the aerofoil section depends on the profile thickness and the angle of attack, thereby influencing the local lift. The hydrodynamic pressure varies parabolically along the span. The effect of the wake developed by the propeller, the stern shape, and the rudder angle on the hydrodynamic loading on rudder is included to express the transverse time-varying loading. The wet, forced vibration of the Kirchhoff’s plate is analyzed in MATLAB. The maximum bending stress and twisting sheer at the rudder stock is calculated for various angles of attack.

Abstract

Introduction. We describe ethical/moral issues in patient selection in a new living donor kidney transplant program in Guyana, South America. Case Reports. Over 3 years, we screened 450 patients with chronic kidney disease among which 70 were suitable for kidney transplantation. There were five patients whose evaluations raised possible ethical dilemmas: one had nonadherence to dialysis; two of Guyanese origin living abroad wished to have the transplant performed in Guyana; a minor wished to donate to her mother; and another subject was considering commercialization of the transplant process. Results. Since inception of the renal replacement program in 2008, we have completed 13 living kidney transplantations, 17 peritoneal dialysis placements, and 20 vascular access procedures. In the five patients wherein faced ethical dilemmas, three were rejected for consideration despite having living donors: one was nonadherent, the second excluded due to an attempt to commercialize the process, and the third, a minor who wished to donate to the mother. The other two patients were considered Guyanese ex-patriots acceptable for the program. Discussion. The consequence of kidney failure in Guyana prior to introduction of renal replacement therapy was a virtual death sentence. These cases illustrate ethical dilemmas serving to throw into stark relief the implications of decisions made in a developing country versus those in a developing country.

Abstract

BACKGROUND We investigated the effect of smoking on postkidney transplant outcomes in the United States Renal Data System. METHODS In a retrospective cohort of 41,705 adult Medicare primary renal transplant recipients in the United States Renal Data System database transplanted from January 1, 2000, to June 30, 2006, and followed through October 31, 2006, we assessed Medicare claims for smoking. The association between renal allograft loss and death and smoking as a time-dependent variable was assessed with Cox nonproportional hazards regression. RESULTS Of 41,705 Medicare primary adult renal transplant patients, there were 9.9% patients who had evidence of prior smoking and 4.6% patients with new claims for smoking after transplant. Incident smoking (new onset smokers) occurred at a mean of 1.29±0.88 years after transplant. In the adjusted analysis, factors associated with new smoking included male gender, history of drug or alcohol use, history of chronic obstructive pulmonary disease, and later year of transplant. Conventional cytogenetics has always been a favourite to detect chromosomal aberrations. Carriers of chromosomal translocation are often phenotypically normal but are infertile. Couples are often advised to go for karyotyping, but culture failure or improper metaphase spread with poor banding often makes the analysis difficult. We report here a novel translocation between short arm of chromosome 4 and long arm of chromosome 6 in an infertile man using an advanced molecular cytogenetic technique of Interphase Chromosome Profiling (ICP).

Abstract

The protein solubility, emulsifying, foaming and gelation properties, and viscosity of solutions of chhana whey protein powders, produced by ultrafiltration and reverse osmosis followed by drying, were studied over the pH range 2.5–9.0. Protein solubility varied from 57–100% and was greatest at low pH values. Chhana whey protein powders had similar emulsifying properties to commercial cheese whey protein powders of similar protein content, although the capacity to form gels when heated to 80°C was much lower, particularly at alkaline pH. the viscosity of solutions of the chhana whey powders was sensitive to pH, but was particularly high in the acidic range. These studies demonstrate considerable potential for the utilization of chhana whey, products in the food industry.

Influents were prepared by mixing the synthetic wastewater with CdCl2 ∙ H2 O at concentrations of 5, 10, 25, and 50 mg∕L for each of the four simultaneous experiments during three runs. The performance of the wetland system was evaluated for three hydraulic retention times (HRT)—5, 7, and 10 days. Removal efficiencies of the soluble chemical oxygen demand were found to be in the range of 72–91% during the experimental period. The mean effluent cadmium concentrations varied between 0.17 and 12.73 mg∕L . The overall average cadmium removal efficiency during the three runs ranged between 75 and 97%. The liver performs many key functions, the most prominent of which is serving as the metabolic hub of the body. For this reason, the liver is the focal point of many investigations aimed at understanding an organism’s toxicological response to endogenous and exogenous challenges. Because so many drug failures have involved direct liver toxicity or other organ toxicity from liver generated metabolites, the pharmaceutical industry has constantly sought superior, predictive in-vitro models that can more quickly and efficiently identify problematic drug candidates before they incur major development costs, and certainly before they are released to the public. In this broad review, we present a survey and critical comparison of in-vitro liver technologies along a broad spectrum, but focus on the current renewed push to develop “organs-on-a-chip”. One prominent set of conclusions from this review is that while a large body of recent work has steered the field towards an ever more comprehensive understanding of what is needed, the field remains in great need of several key advances, including establishment of standard characterization methods, enhanced technologies that mimic the in-vivo cellular environment, and better computational approaches to bridge the gap between the in-vitro and in-vivo results.

This study was conducted to investigate the cadmium removal in the free water surface and subsurface flow constructed wetland systems under different environmental conditions. Removal efficiencies of the chemical oxygen demand were found to be in the range of 79–92% for the two wetland systems. The removal efficiencies for total kjeldahl nitrogen, total phosphorus, total suspended solid, and volatile suspended solid were in the range of 64–91, 62–90, 68–91, and 50–84%, respectively. Influents were obtained by mixing a synthetic wastewater with Cd Cl2 . H2 O at four concentrations 1, 5, 10, and 20 mg∕L , respectively, for the four experimental runs. The mean effluent cadmium concentrations varied between 0.02 and 0.16 mg∕L . Cadmium removal efficiencies were 98.6–99.4% in free water surface, and 99.3–99.9% in the subsurface flow wetland systems. The viscosity ofpura (pancakes) batter samples prepared with bovine, buffalo and mixed wheys (80 : 20 whey to water) was measured at different time intervals at a temperature of 25 ± 2°C using a Brookfield viscometer (LVT 98534). The viscosity of sweet pura batter samples increased after 1 h, remained constant for another 2 h and then decreased, whereas the viscosity of salty pura batter samples increased from freshly prepared batter to 1 h and then remained constant. Sensory evaluation of both sweet and salty puras at different time intervals from batter preparation was also carried out. The overall acceptability scores for sweet puras prepared using bovine milk chhana whey, buffalo milk paneer whey and mixed milk chhana whey and stored for up to 4 h at 25 ± 2 °C and for 24 h at 4-5 °C were not significantly different, and the same conclusion applied to salty puras made from bovine milk chhana whey. Puras (pancakes) are widely accepted traditional Indian foods. Studies were conducted on the use of chhana and paneer whey in the preparation of sweet and salty puras. Six samples of chhana and paneer whey were substituted in sweet and salty puras at different whey to water ratios (0 : 100, 20 : 80, 40 : 60, 60 : 40, 80 : 20, 100 : 0). The effects of chhana and paneer whey on sensory evaluation showed that sweet puras containing mixed milk chhana whey and salty puras containing cow's milk chhana whey scored highest with respect to appearance, body and texture, flavour and taste, and overall acceptability. In general, commercial samples scored less, which may be due to poor handling and maintenance of whey by commercial manufacturers. Whey substitutions also improved fat, total solids, protein and ash of both sweet and salty puras. It is interesting to note that none of sweet and salty pura samples was below the limits acceptable to the panellists. It can be thus concluded that chhana and paneer whey can be used successfully in the preparation of puras for value addition.

The flame-excited emission (candoluminescence) of the oxides Ta 2 O 5 , Nb 2 O 5 , TiO 2 , ZnO, and SnO 2 have been reexamined. By measuring the emission spectra as a function of temperature in comparison with the optical band gap, also measured as a function of temperature, it is concluded that a portion of the emission, particularly in Ta 2 O 5 and Nb 2 O 5 , is non-thermal. The results support the hypothesis that the excess emission is due to recombination of electron–hole pairs created by de-excitation of neutral radicals on the phosphor surface.

Faecal samples were collected from seventy-eight diarrhoeic cow and buffalo calves between November 1998 and February 1999 to study the genomic diversity and prevalence of Rotavirus infection by ribonucleic acid polyacrylamide gel electrophoresis (RNA-PAGE) and enzyme-linked immunosorbent assay (ELISA). In the organised dairy farm (where daily production and health records were maintained), the overall prevalence of infection with Rotavirus, recorded by RNA-PAGE and ELISA, was 27.02% (10/37) in both cow and buffalo calves. In unorganised dairy herds (where no production or health records were maintained), RNA-PAGE and ELISA detected infection with Rotavirus in 26.8% (11/41) of cow and 19.5% (8/41) of buffalo calves. Five distinct electropherotypes were found to circulate in cow and buffalo calves. All were short electropherotypes except the single long electropherotype observed in a buffalo calf in an unorganised dairy herd. Some differences in RNA migration pattern were observed when these electropherotypes were compared with the neonatal calf diarrhoea virus strain of Rotavirus. Some electropherotypes were restricted to one farm while others were found in both organised and unorganised dairy herds and in both cow and buffalo calves.

Pilot-plant experiments were conducted for on-site water treatment along a canal in Bangkok to evaluate the performance of a rock-bed filtration system under different operating conditions. Three different rock sizes with approximate equivalent diameters of 10 cm, 15 cm, and 20 cm were used as filter media in two rectangular reactor units (5.04 m long, 0.5 m wide, and 0.75 m deep). A diffuser network of 1.25 cm diameter PVC tubes was installed at the bottom along the length of each reactor to provide aeration. The reactors were fed with raw canal water by a pump through a head tank unit to vary the hydraulic retention time (HRT) from 3 to 13 h. We present here the solutions of a bounded linear harmonic potential and abounded quartic potential using the modified Airy function (MAF) method. Resultsobtained by the MAF method have been compared with the analytical (numerical)results and with those obtained by the JWKB method. The comparison showsthat the MAF method gives very accurate results and is, in general, the moreaccurate of the two methods. The MAF method also gives an accurate descriptionof the eigenfunction. The effect of thermally-induced surface segregation on the electrochemical corrosion behavior of Al-Ni-Y amorphous alloys was investigated by potentiodynamic polarization and electrochemical impedance techniques. Annealing below the crystallization temperature at 150 degrees C for 5 min caused a reduction in passive current density and improved polarization resistance. Synchrotron X-ray diffraction patterns showed no change in structure while nuclear magnetic resonance spectra confirmed no change in chemical ordering or structural relaxation within the glass. The surface segregation of yttrium, as confirmed by X-ray photoelectron spectroscopy, after annealing below the crystallization temperature is proposed to be responsible for the improved passivity. Aim: Investigate the possibility that gastroesophageal reflux disease (GERD) is related to the development of subglottic stenosis (SGS) and is the likely cause in cases previously determined as unknown or idiopathic. Methods: Ten patients presented with subglottic stenosis. None had a previous history of trauma, intubation, granulomatous disease, or other disorder that causes SGS. There were nine females and one male, with an age range of 36 to 66 years. Results: The nine female patients were evaluated by an esophagram in all cases, pH monitoring in seven, and esophagoscopy in three. The one male patient was lost to follow-up and not studied. Six patients had abnormal results on studies indicative of GERD and three patients had normal examination results. As a group, there were 25 procedures for repair of SGS before the start of anti-reflux therapy and 11 afterwards, with follow-up a minimum of 1 year up to 8 years. The interval time between repairs was greatly lengthened after anti-reflux therapy was instituted. Discussion: There are suggestions that GERD contributes to the severity of most cases of acquired SGS. Based on results of the tests and responses to anti-reflux therapy, it is likely that GERD causes SGS in previously categorized idiopathic cases.

Abstract

Introduction In 2008, we initiated the first Guyanese comprehensive kidney replacement program, comprising hemodialysis (HD), peritoneal dialysis (PD), vascular access procedures, and living-donor kidney transplantation. The government of Guyana, US-based philanthropists, US-based physicians, and Guyanese caregivers teamed up to form a public–private partnership. This pilot program was free of cost to the patients. Methods From July 2010 to the time of writing, we placed 17 patients with end-stage kidney disease on PD, which was used as a bridge to living-donor kidney transplantation. During the same period, we placed 12 primary arteriovenous fistulae. Results The 17 patients who received a PD catheter had a mean age of 43.6 years and a mean follow-up of 5.3 months. In that group, 2 deaths occurred (from multi-organ failure) within 2 weeks of catheter placement, and 2 patients were switched to HD because of inadequate clearance. Technical issues were noted in 2 patients, and 3 patients developed peritonitis (treated with intravenous antibiotics). An exit- site abscess in 1 patient was drained under local anesthesia. The peritonitis rate was 0.36 episodes per patient–year. Of the 17 patients who received PD, 4 underwent living-donor kidney transplantation. Conclusions In Guyana, PD is a safe and cost-effective option; it may be equally suitable for similar developing countries. In Guyana, PD was used as a bridge to living-donor kidney transplantation. We have been able to sustain this program since 2008 by making incremental gains and nurturing the ongoing public– private partnership.

Abstract

Results from the first study of isolated-photon+jet correlations in relativistic heavy ion collisions are reported. The analysis uses data from PbPb collisions at a centre-of-mass energy of 2.76TeV per nucleon pair corresponding to an integrated luminosity of 150μb-1 recorded by the CMS experiment at the LHC. For events containing an isolated photon with transverse momentum pTγ>60GeV/c and an associated jet with pTJet>30GeV/c, the photon+jet pT imbalance is studied as a function of collision centrality and compared to pp data and pythia calculations at the same collision energy. Using the pTγ of the isolated photon as an estimate of the momentum of the associated parton at production, this measurement allows an unbiased characterisation of the in-medium parton energy loss. For more central PbPb collisions, a significant decrease in the ratio pTJet/pTγ relative to that in the pythia reference is observed. Furthermore, significantly more pTγ>60GeV/c photons in PbPb are observed not to have an associated pTJet>30GeV/c jet, compared to the reference. However, no significant broadening of the photon+jet azimuthal correlation is observed. © 2012 CERN.
