Funding Acknowledgements Type of funding sources: None. Introduction Cardiolaminopathies are rare genetic alterations, which present diverse mechanical and electrical alterations. OBJECTIVE To present a family carrying an LMNA gene mutation that although not described manifest clinical and electrocardiographic characteristics of the disease. Methods We present the case of two sisters and one brother, aged 65, 57 and 51 years, respectively. At the beginning of their fourth decade, all of them debuted with atrial flutter associated with second and third degree atrioventricular block. Two of them (one or the sisters and the brother) underwent ablation of isthmus-dependent atrial flutter. However, in both, the flutter recurred and no new interventional treatment was further offered. The male subject received a pacemaker implant for treatment of AV block; some years later, he presented a cerebrovascular event and subsequently a recurrence, both while in oral anticoagulation and, actually remains with moderate motor deficit and dysarthria. The youngest woman suffered an embolic cerebrovascular event when she was 50 y/o; she remained asymptomatic in relation to the rhythm disorder and refused a pacemaker. The oldest sister who is asymptomatic, received a pacemaker when she was 45 y/o and now is anticoagulated. By echocardiography, all siblings presented left ejection fraction in between 40 and 50% with decreased strain values and normal left ventricular geometry. Results Recently in our country it is possible to carry out a genetic test, for which the three were subjected to it. In this family, the genetic testing was done using a comercial kit for 172 genes. The result was a Frameshift Indels (non misense) mutation NM_170707.3 c.1526del p.(Pro509LeufsTer39) Exon:9/12, in all three. The caveat is that this LMNA gene mutation has not been previously described. In light of these results, the youngest woman with total AV block and sequelae of cerebrovascular disease and mild left ventricular dysfunction, received an implantable cardiac defibrillator (ICD) implant for primary prevention of sudden death and her siblings are undergoing an upgrade to ICD. Conclusion The mutation found in this family is unique and for the first time reported in the LMNA gene in the world. Genetic diagnosis is limited in our country, which makes it difficult to determine this type of pathology. However, given the clinical presentation of an electrical disorder in all the siblings of a family, in this particular case, it was mandatory to perform this test. These results allow us to better understand the prognosis and intervene appropriately to prevent sudden death, even though there is no treatment to avoid the progression of the disease.

Mixed couples face more marital conflict than endogamous couples. Drawing on intersectional theory and narrative victimology, this study examines women’s accounts of abuse in mixed heterosexual Arab/Palestinian–Israeli Jewish intimate partnerships amid the Israeli–Palestinian conflict. The narratives of 25 women formerly in an abusive relationship are the primary data, which are supplemented by a comprehensive list of calls seeking advice or intervention from a non-governmental organization (NGO) that assists women in mixed relationships, and the NGO’s recorded in-service training sessions during which social workers discuss clients’ plights and abuse exposure. Consistent with research on mixed couples, the women’s narratives connect their abuse to differences, dynamics, and tensions rooted in cultural, religious, and social beliefs and practices. Importantly, the narratives also highlight how the Israeli–Palestinian conflict amplifies and escalates the women’s abuse. Intersections of gender, religion, and nationality as well as life in a conflict zone critically affect the abuse dynamic the women experience. The article concludes with a discussion of the relevance of narrative victimology and political enmity for intersectional approaches to domestic violence.

In pediatrics, there are few standard criteria to classify and consolidate postoperative complications, particularly in appendectomy, where according to the literature, complications range from 5% to 30%.A cross-sectional, observational, retrospective study of patients undergoing surgery as a result of suspected acute appendicitis (AA) from December 2018 to January 2020 was carried out. Complications were grouped and consolidated according to the Clavien-Dindo (CD) classification. Postoperative complications and factors involved were analyzed by conducting a bivariate and multivariate statistical study using SPSS statistical software, version 25.A total of 124 patients were studied. Mean age was 9 years (3-14 years). 62% were boys, and 38% were girls. All patients underwent appendectomy - 80.6% through laparotomy, and 19.4% through laparoscopy. 20% of patients had postoperative complications, which were grouped according to the CD classification (p = 0.002). Most complications were included in the CD I group (64%). Collections treated with antibiotic therapy were included in the CD II group (28%). Intra-abdominal collections requiring re-intervention for drainage purposes were included in the CD IIIb group (8%). The main factors driving complications were complicated AA (81% gangrenous and perforated) (p0.001) and progression time (80%24 h of progression) (p = 0.036), which increased mean hospital stay by 7 ± 4 days (p = 0.016). 137 ± 37 CRP levels were associated with plastron identification (p0.001), whereas 109 ± 19 CRP levels were associated with peritonitis (p0.001).The Clavien-Dindo classification allows post-appendectomy complications in pediatric surgery to be classified using a common language, by associating complication grade with treatment complexity.En pediatría hay pocos criterios estándares para clasificar y unificar las complicaciones postquirúrgicas y en particular en la apendicectomía, donde, según la literatura, las complicaciones varían de un 5-30%.Realizamos un estudio retrospectivo, observacional tipo transversal de los pacientes intervenidos por sospecha de apendicitis aguda (AA) durante diciembre 2018 a enero 2020. Las complicaciones fueron agrupadas y unificadas según la clasificación de Clavien-Dindo (CD). Se analizan las complicaciones postoperatorias y factores implicados, aplicando estudio estadístico bivariante y multivariante según el programa estadístico SPSS versión 25.Un total de 124 pacientes incluidos. Edad media 9 años (3-14 años). 62% niños, 38% niñas. A todos se les realizó apendicectomía, el 80,6% apendicectomía abierta, 19,4% por laparoscopia. El 20% presentó algún tipo de complicación en el postoperatorio y fueron agrupadas según la clasificación de CD (p = 0,002). En CD I (64%) se incluyen la mayoría de las complicaciones, En el grupo CD II (28%) colecciones tratadas con antibioticoterapia. En el grupo CD IIIb (8%), colección intraabdominal que requirió reintervención para su drenaje. Los principales factores implicados en la aparición de complicaciones fueron: AA complicadas (81% gangrenosas y perforadas) (p0,001), tiempo de evolución (80%24 h de evolución) (p = 0,036) que incrementó la estancia media 7 ± 4 días (p = 0,016). El valor de proteína C reactiva (PCR) (137 ± 37) se relacionó con la identificación de plastrón (p0,001), y el valor de PCR (109 ± 19) con peritonitis (p0,001).La clasificación de Clavien-Dindo permite utilizar un lenguaje común para clasificar las complicaciones postapendicectomía en cirugía pediátrica, expresando el grado de estas según complejidad del tratamiento utilizado para su resolución.

Congenital nephrourological abnormalities (CAKUT) are a particularly relevant group of diseases due to their high prevalence and the fact they are the main cause of chronic renal disease (CRD) in the pediatric population. Our objective was to determine the characteristics and prevalence of CAKUT in our setting, while identifying the factors associated with the occurrence of renal damage.A retrospective, descriptive, analytical, cross-sectional study of patients seen in the Pediatric Nephrology Department of a third-level hospital from January 1 to December 31, 2018 was carried out. Epidemiological, clinical, and analytical variables were assessed, and potential risk factors associated with CRD were searched for.The study involved 685 patients with 827 kidney units affected by CAKUT with a mean age of 9.98 ± 5.12 years. 62.2% were male, and the mean follow-up period after diagnosis was 9.95 ± 5.09 years. 58.8% were non-obstructive dilations, followed by renal dysplasia, obstructive dilations, and number and position abnormalities. The most frequent malformation was vesicoureteral reflux (VUR). The most commonly affected side was the left (47.5%). 55% of the diagnoses were prenatal. 172 patients underwent surgery. The initially chosen treatment for VUR was endourological. Overall re-intervention rate was 20%. Of the total number of patients, glomerular filtration rate was analyzed in 383, 95 (24.8%) of whom had CRD (86% in stage 2). Male sex, bilaterality, and proteinuria were risk factors associated with CRD.Knowledge of the epidemiological and clinical characteristics of children with CAKUT and the factors associated with CRD helps to individualize the clinical follow-up of these patients, thus customizing diagnostic tests and healthcare resources.Las anomalías nefrourológicas congénitas (CAKUT), constituyen un grupo de enfermedades de gran relevancia por su alta prevalencia y por ser la principal causa de enfermedad renal crónica (ERC) en la población pediátrica. Nuestro objetivo es conocer las características y la prevalencia de CAKUT en nuestro medio, identificando los factores asociados a la aparición de daño renal.Estudio retrospectivo, descriptivo, analítico y transversal, que incluyó los pacientes atendidos en la consulta de Nefrología Pediátrica de un hospital de tercer nivel desde el 1 de enero al 31 de diciembre de 2018. Se analizaron variables epidemiológicas, clínicas y analíticas, y se buscaron posibles factores de riesgo asociados a ERC.Se incluyeron 685 pacientes con 827 unidades renales con CAKUT con una edad media de 9,98 ± 5,12 años. El 62,2% fueron varones y el seguimiento medio desde el diagnóstico fue de 9,95 ± 5,09 años. El 58,8% fueron dilataciones no obstructivas, seguido por displasia renal, dilataciones obstructivas y anomalías de número y posición. La malformación más frecuente fue el reflujo vesicoureteral (RVU). El lado más afectado fue el izquierdo (47,5%). El 55% de los diagnósticos fueron prenatales. Fueron intervenidos 172 pacientes. El tratamiento inicial de elección en el RVU fue endourológico. La tasa de reintervención global alcanzó el 20%. Del total de pacientes, se pudo analizar el filtrado glomerular en 383 pacientes, de los cuales 95 (24,8%) tenían ERC (86% en estadio 2). El sexo masculino, la bilateralidad y la proteinuria fueron factores de riesgo asociados a la ERC.El conocimiento de las características epidemiológicas y clínicas de los niños con CAKUT y de los factores asociados a la ERC ayuda a individualizar el seguimiento clínico de estos pacientes adecuando las pruebas diagnósticas y los recursos sanitarios.

Previously unrecognized diseases continue to threaten the health of forest ecosystems globally. Understanding processes leading to disease emergence is important for effective disease management and prevention of future epidemics. Utilizing whole genome sequencing, we studied the phylogenetic relationship and within diversity of two populations of the bacterial oak pathogenLonsdalea quercinafrom western North America (Colorado and California) and compared these populations to otherLonsdaleaspecies found worldwide. Phylogenetic analysis separated Colorado and California populations into two well supported clades within the genusLonsdalea, with an average nucleotide identity between them near species boundaries (95.31%) for bacteria, suggesting long isolation. Populations comprise distinct patterns in genetic structure and distribution. Genotypes collected from different host species and habitats were randomly distributed within the California cluster, while most Colorado isolates from introduced planted trees were distinct from isolates collected from a natural stand of CO nativeQ. gambelii, indicating the presence of cryptic population structure. The distribution of clones in California varied, while Colorado clones were always collected from neighboring trees. Despite its recent emergence, the Colorado population had higher nucleotide diversity, possibly due to migrants moving with nursery stock. Overall results suggest independent pathogen emergence in two states likely driven by changes in host-microbe interactions due to ecosystems conditions changes. To our knowledge, this is the first study onL. quercinapopulation structure. Further studies are warranted to understand evolutionary relationships amongL. quercinapopulations from different areas, including the native habitat of red oak in northeastern USA.ImportanceBacterial pathogens from genusLonsdaleaseverely affect oak forest ecosystems worldwide. In Colorado, USA,L. quercinais one of the causal agents of drippy blight disease on introduced red oak trees. Prior to discovery of drippy blight in Colorado,L. quercinawas reported on oak trees in California, causing drippy nut on acorns of native oaks. Due to its recent emergence in Colorado, the origin and movement ofL. quercinaare unknown. In this study we investigated evolutionary relationships within genusLonsdaleaworldwide andL. quercinapopulation structure in western USA. Our results demonstrate thatL. quercinaColorado and California populations comprise distinct patterns of genetic structure and distribution, suggesting that accidental pathogen introduction from California to Colorado is unlikely. Higher nucleotide diversity in a recently emerged Colorado population suggests the bacterial strains might be migrants that initially moved with nursery stock from other areas in the last century. For example, Colorado strains ofL. quercinamay have moved from native stands of red oaks in the northeastern or southern USA. Curiously, however, this disease is not known in native red oak in the northeastern USA. Initial causes of recent disease emergence are likely driven by environmental/ecosystem changes since isolates for this study were collected from established mature trees. Results presented here give a better understanding of population biology of the bacterial oak pathogen and provide a framework for investigation of evolutionary relationships among pathogen populations from different areas.

Background: The aim was to determine the effect of feeding on chiral inversion of Ibuprofen isomers. Method: Six healthy volunteers participated in a two-treatment crossover study where a single dose of Ibuprofen racemate was given. One stage consisted in an 8-hour interval of fasting, while in the other, the drug was administered with a solution of saccharose and a standard food intake regimen was implemented.

New findings What is the central question of this study? In a model of salt-sensitive hypertension in ovariectomized (oVx) adult Wistar rats, we evaluated the expression of proteins related to sodium transport in peripheral blood mononuclear cells (PBMC), and we compared the response of proteins to high sodium intake and changes in blood pressure in intact female rats. What is the main finding and its importance? Sodium transport proteins in PBMC react to high sodium and blood pressure markedly differently in oVx versus intact female rats. Protein expressions indicate sodium and pressure sensitivity. Renal immune cells increase in oVx under high salt. Abstract Hypertension is a worldwide public health problem. High sodium consumption is associated with hypertension, and hypertensive mechanisms involve immunity cells. Peripheral blood mononuclear cells (PBMC) are endowed with proteins related to sodium transport. We studied their abundance in PBMC from intact (IF) or ovariectomized (oVx) adult Wistar rats under normal (NS) or high salt (HS) intake. Ovariectomy was performed at 60 days of life. At 145 days, one group of IF and oVx rats received NS or HS intake for 5 days. Another group of IF HS and oVx HS rats received hydralazine (HDZ) to reduce blood pressure (BP). Sodium balance and BP were recorded. Expression of Na+ , K+ -ATPase (NKA), Na/K/2Cl 1 (NKCC1), serum/glucocorticoid-regulated kinase 1 (SGK1), Dopamine D1 like receptor (D1DR), CD4+ and CD8+ were determined in PBMC, and CD45+ leukocytes in renal tissue. IF HS rats showed increased natriuresis and normal BP. NKA and CD4+ expression diminished in IF HS. Instead, oVx HS rats had sodium retention and high BP and increased the expression of NKA, NKCC1, D1DR, CD4+ and CD8+ in PBMC. Renal CD45+ leukocytes increased in oVx HS. HDZ decreased BP in all rats. Upon HDZ treatment, NKA did not change, NKCC1 decreased in oVx HS rats, while SGK1 increased in both IF HS and oVx HS rats. Hormonal background determines BP response and the expression of proteins related to sodium transport in PBMC and renal immune cells at HS intake. The analysis of NKA, NKCC1, and SGK1 expression in PBMC differentiated salt-sensitivity from BP variations. This article is protected by copyright. All rights reserved.

The cyclopropane ring-opening reaction of riolozatrione, a natural product obtained from Jatropha dioica, afforded a 2,2-disubstituted 1,3-cyclohexandione displaying an alkyl methyl ether group at position 5. The conformational analysis of this product showed a high preference for the trans-diaxial conformation in both solution and solid state. Such conformation was possible from the noncovalent intramolecular nX → π*C═O interactions (X = an element having an unshared electron pair), allowing the determination of the interaction energies. Since the nX → π*C═O interactions can be regarded as additive, the energy values ranged from 4.52 to 6.51 kcal mol-1 for each carbonyl group with a strong dependency on the interatomic distances. The rigorous analysis of the electron density in the topological theory of atoms in molecules framework clearly shows that the origin of O-C═O interactions are through the nO → π*C═O electron transfer mechanism. Such interactions are slightly weaker than a canonical hydrogen bond but seemingly stronger than a van der Waals interaction. This interaction must be considered as a stereoelectronic effect due the electronic transfer between the interacting groups, which are limited by their relative stereochemistry and can be represented by a bond-no bond interaction, causing the pyramidalization of the carbonyl, which is the charge acceptor group.

Management skills have become one of the basic needs of the labor-business sector, identifying them for graduates as areas of opportunity and growth, as well as being able to open spaces in their strengths and identifying those that are considered essential according to the profiles of each of the skills developed during their academic stage. Within the educational sector, through the accreditation of its programs, it is possible to follow up on what the labor sector has modified, and which now considers requirements within students, considering these spaces as administrative areas, research and entrepreneurship.

In light of the COVID-19 pandemic situation, the separation of the primary colors of ink markers and food coloring was performed using a simplified column chromatography setup at home to accomplish the educational outcome of the course. The virtual class instruction, for the students enrolled in the Organic Techniques Laboratory course, allows the understanding of the basic principles of chromatography in a way similar to that in the face-to-face experience. The materials proposed for conducting the experiment are readily available at home or easily found in a drugstore or in a grocery store. A wide variety of results were observed where the separation of at least two primary colors was achieved by 80% of the students.

Endoperoxides have received widespread interest as a research topic due to its ubiquitous structure, which confers interesting biological properties. It has been demonstrated that some endoperoxides possess antimalarial, cytotoxic, anti-inflammatory, neuroprotective, antiparasitic, antitumor, and antimicrobial activities. The present review summarizes the current knowledge about the structure, occurrence, peroxide detection methods, stability, biosynthesis, and biological activities of 206 endoperoxides isolated from 1911 to 2019, found in 115 plant species belonging to 36 different plant families. Thirty-five new endoperoxides of different plant species have been reported since the last published review. In order to simplify our analysis, we classified plant endoperoxides considering the nature of the secondary metabolite.

Female sex hormones participate in the regulation of blood pressure and renal epithelial proliferation, effects not related to their reproductive function. About one-third of the world's population has abnormally high levels of blood pressure, hypertension, which is responsible for almost 50% of deaths from stroke and coronary heart disease. Salt sensitivity is a risk factor for cardiovascular morbidity and mortality and other diseases as well. We reported a model of salt sensitive hypertension in adult ovariectomized (oVx) Wistar rats. oVx rats are normotensive under normal salt intake (NS, 0.24% NaCl), but upon a high salt intake (HS, 1% NaCl) oVx rats developed a blood pressure profile of salt-sensitive hypertension. Our studies on kidney molecules related to sodium balance found that the circuit dopamine D1-like receptor, cytochrome P450 4A and Na+, K+-ATPase is altered by the absence of ovary hormones which is accompanied by a reduced ability to excrete sodium. In oVx rats HS intake also promotes changes in the expression of proteins related to sodium transport in peripheral blood mononuclear cells, mainly peripheral lymphocytes. Therefore, sodium transport is modified at several levels of normal physiology. Lately, we described that estradiol increases the rate of renal epithelial cell proliferation in primary cultures developed from human renal cortex. Thus, salt sensitivity, adaptive immunity, blood pressure and renal cell proliferation are complex biological responses regulated by female sex hormones.

Mood disturbances such as anxiety and depression are common in patients with inflammatory bowel disease (IBD), and impact negatively on their quality of life and disease course. An integrated multidisciplinary IBD team, which includes access to psychology and psychiatry opinion, makes possible the prompt recognition and management of psychological disturbance in patients with IBD. Based on our experience and existing literature, including systematic reviews of the effectiveness of available treatment modalities, a stepwise approach to the maintenance and restoration of psychological well-being is recommended, evolving upwards from lifestyle advice, through behavioural therapies to pharmacotherapy.

This work investigates the role of hydroxyl and epoxy bridges in room-temperature ferromagnetism (FM) of pyrolytic graphene oxide nanoplatelets (GOs). Graphene oxide nanoplatelets were synthesized from bamboo pyroligneous acid (BPA), varying oxide coverage (OC) from 5.3% to 13.0%. The amount of hydroxyl and epoxy functional groups in all the GO samples were estimated from results analysis of X-ray photoelectron spectroscopy (XPS). An FM signal was identified, measured at room temperature, which scaled with oxide coverage. Decreased oxide coverage results in enhanced FM. A combination of results from high-resolution transmission electron microscopy (HR-TEM), XPS, energy-dispersive X-ray spectroscopy (EDX), X-ray diffraction (XRD), electron energy loss spectroscopy (EELS), Fourier transform infrared (FTIR), Raman, and electrical characterization allowed constructing an atomic model for each graphene oxide (GO) structure. First-principles calculations of the atomic model suggest that FM is induced by the adsorption of –OH and –O– atoms on graphene nanoplatelets; therefore, their magnetism is a response to the number of uncompensated spins due to topographic defects. Chemical substitution is one of the most efficient tools to tune and optimize magnetic and magnetocaloric properties of the giant magnetocaloric materials. In particular, Indium substitutions could be useful both for tuning properties of these interesting intermetallic materials and to unveil their local-scale behavior across the magnetostructural transition via hyperfine techniques. Hence, in order to investigate the effect of Indium additions on the crystal structure, micro-structure, magnetic and magnetocaloric properties, a series of In-containing samples derived from the base Gd5Si1.2Ge2.8 stoichiometry were prepared. The major findings are that while In is insoluble in the 5: 4 phase, it will instead promote the emergence of the impurity 5: 3 phase and segregates into this phase. Hence, In leads to major crystallographic changes, which enhance atomic disorder and disrupt the Si to Ge ratio in the 5: 4 phase. Subsequently, a higher 5: 4 unit cell volume and a lower magnetic ordering temperature are found in the In-substituted samples. Mismatches are incongruencies between programmatic policies and working practices that engender counter-productive dynamics or outcomes relative to aspirations. The present article addresses electronic monitoring programs for domestic violence/intimate partner violence, examining how mismatches may emerge during their implementation or later. Electronic monitoring programs aim to surveil and deter defendants facing domestic violence/intimate partner violence–related charges during court-supervised release, with the aim of enhancing victim safety. Analysis of qualitative and survey data from a national (US) study suggests that domestic violence/intimate partner violence–focused strategies reliant on surveillance technology may be prone to generating mismatches rooted in discrepancies between victims’ expectations, defendant risk profiles, and program operations. Policy implications of mismatches in victim-focused initiatives are considered.

Background Ischemia-reperfusion (IR) injury is one of the leading causes of early graft dysfunction in liver transplantation. Techniques such as ischemic preconditioning protect the graft through the activation of the hypoxia-inducible factors (HIF), which are downregulated by the EGLN family of prolyl-4-hydroxylases, a potential biological target for the development of strategies based on pharmacological preconditioning. For that reason, this study aims to evaluate the effect of the EGLN inhibitor sodium (S)-2-hydroxyglutarate [(S)-2HG] on liver IR injury in Wistar rats. Methods Twenty-eight female Wistar rats were divided into the following groups: sham (SH, n = 7), non-toxicity (HGTox, n = 7, 25 mg/kg of (S)-2HG, twice per day for two days), IR (n = 7, total liver ischemia: 20 minutes, reperfusion: 60 minutes), and (S)-2HG+IR (HGIR, n = 7, 25 mg/kg of (S)-2HG, twice per day for two days, total liver ischemia as the IR group). Serum ALT, AST, LDH, ALP, glucose, and total bilirubin were assessed. The concentrations of IL-1β, IL-6, TNF, malondialdehyde, superoxide dismutase, and glutathione peroxidase were measured in liver tissue, as well as the expression of Hmox1, Vegfa, and Pdk1, determined by RT-qPCR. Sections of liver tissue were evaluated histologically, assessing the severity of necrosis, sinusoidal congestion, and cytoplasmatic vacuolization. Results The administration of (S)-2HG did not cause any alteration in the assessed biochemical markers compared to SH. Preconditioning with (S)-2HG significantly ameliorated IR injury in the HGIR group, decreasing the serum activities of ALT, AST, and LDH, and the tissue concentrations of IL-1β and IL-6 compared to the IR group. IR injury decreased serum glucose compared to SH. There were no differences in the other biomarkers assessed. The treatment with (S)-2HG tended to decrease the severity of hepatocyte necrosis and sinusoidal congestion compared to the IR group. The administration of (S)-2HG did not affect the expression of Hmox1 but decreased the expression of both Vegfa and Pdk1 compared to the SH group, suggesting that the HIF-1 pathway is not involved in its mechanism of hepatoprotection. In conclusion, (S)-2HG showed a hepatoprotective effect, decreasing the levels of liver injury and inflammation biomarkers, without evidence of the involvement of the HIF-1 pathway. No hepatotoxic effect was observed at the tested dose.
