Abstract

Introduction:: The consequences of neurogenic bowel dysfunction in patients with neurogenic lower urinary tract disease requiring urinary tract reconstruction with bowel harvest remains unclear. A working group was formed by the International Continence Society (ICS) to generate a consensus statement highlighting the key issues to be addressed and optimised peri-operatively. Methods:: Nominal group technique was used to derive consensus. Principal aspects of assessment and surgery decision-making were agreed and a series of statements was generated by a core focus group of experts, which were subsequently modified and ratified by the wider working group. This was followed by final voting by the full working group. Results:: General considerations included the importance of understanding the neurological condition in terms of degree of disability, prognosis and risk of progression, an assessment of cognition and dexterity and an inter-disciplinary assessment to ensure suitability and feasibility of surgery. Peri-operative recommendations included using an enhanced recovery after surgery (ERAS) protocol when appropriate and taking additional precautions if there is a risk of autonomic dysreflexia or the presence of implants such as ventriculo-peritoneal shunts, baclofen pumps, sacral or spinal cord stimulators. Extra consideration must be taken post-operatively to minimise the risk of venous thrombo-embolism formation, formation/exacerbation of pressure sores and long-term bowel disturbance. Conclusion:: The consensus opinion indicates that urinary tract reconstruction using bowel segments is feasible in carefully selected and optimised patients with neurogenic bowel dysfunction, provided the potential implications for serious adverse events are carefully considered and there is access to appropriate inter-disciplinary expertise.

2. Pattern of menstrual cycle after kidney transplant in reproductive women

Abstract

Background. In reproductive women, transplant disturbs the menstrual cycle pattern. The two major conditions usually encountered are amenorrhea and menorrhagia.The objective of the study was to assess the pattern of menstrual cycle after kidney transplant in reproductive women.Materials and methods. This cross-sectional study was carried out in a public sector hospital of Karachi, Pakistan. A total 69 patients of reproductive age were included who underwent living kidney donor transplant for more than a year ago. Women having genital tract infection, using hormonal treatment, organic cause of genital tract, clotting disorder and severe cardiac and/ or peripheral vascular disease were excluded. Frequency and percentages were calculated for demographic characteristics. Correlation and association analysis was calculated for type of menstruation with menstrual cycle pattern. A P-value less than 0.05 was considered statistically significant.Results. Majority of female included in the study aged between 35–39 years (36, 52.2%). The most frequent menstrual disturbance observed was heavy menstrual bleeding (22, 31.9%) and amenorrhea (21, 30.4%). Only 2.9% cases showed normal menstrual pattern. The cross tabulation indicated that 26.1% patients had amenorrhea, 24.6% had oligomenorrhea and 31.9% had menorrhagia. The Durbin–Watson value of 0.656 indicated a strong positive relationship between menstruation cycle pattern (dependent variable) and type of menstruation, marital status, donor’s age, children and living location of the patients (independent variables).Conclusion. From the result of the present study, it is concluded that the reproductive age women have shown a disturbed pattern of menstrual cycle after kidney transplant. The major observation was that such patients reported amenorrhea, menorrhagia, oligomenorrhea and hypomenorrhea.

3. Assessing the Quality of Concrete Tunnel Lining Exposed to Tunnel Fire through Residual Compressive Strength

Abstract

Compressive strength performance of concrete after exposure to the elevated temperature is important for evaluating and repairing concrete structures. This paper presents an experimental study to determine the residual compressive strength of concrete used in tunnel lining after exposed to tunnel fire. Two types of concrete tunnel lining segments are evaluated in this study. One of it was constructed using a patented fire-resistant concrete (MYC) containing high volume fly ash and nanosilica (HVFANS). Another concrete tunnel lining segment was constructed using concrete containing silica fume normally used in the current construction, coded as SPC concrete. The drilled core results show that, after exposure to tunnel fire temperature up to around 1045°C, the compressive strength of MYC has dropped to 66% of the design strength. In comparison, the SPC concrete showed a decrease in compressive strength to 62% of design strength. The experimental results confirmed that the SPC segments have shown slightly lower residual compressive strength compared to the MYC segments. However, the MYC tunnel segment shows high resistance to the spalling of cover concrete compared to the SPC tunnel segment. Therefore, it can be said that the residual strength alone is not sufficient to compare the damage of concrete exposed to tunnel fire; the spalling damage observation is similarly important as it is one of the important serviceability criteria for designing concrete structures.

Abstract

Introduction:: Potential consequences of inflammatory bowel disease (IBD) need evaluation for patients considering urinary tract reconstruction for benign disease. A working group was formed by the International Continence Society, which considered urinary tract reconstruction in IBD. Methods:: Nominal group technique was used to derive consensus. Principal aspects of IBD assessment and surgery decision-making were agreed. A questionnaire was used to facilitate the generation of statements by a core focus group of experts, which were modified and ratified by the wider working group. This was followed by final voting by the full working group. Results:: General considerations included identifying the importance of the specialist IBD multi-disciplinary team. Peri-operative considerations recommended avoiding pre-operative fasting from midnight, and using an enhanced recovery after surgery (ERAS) protocol. Selection of bowel segment, pre-operative optimisation and post-operative issues were considered for both Ulcerative colitis (UC) and Crohn’s disease. UC is not an absolute contraindication to urinary tract reconstruction using small or large bowel. Elective reconstructive surgery should wait at least three months following resolution of any acute UC flare-up to correct all abnormalities. Crohn’s disease is a high-risk disease for urinary tract reconstruction, even if in remission. In Crohn’s, reconstructive surgical options are limited by the location and extent of gastrointestinal segment(s) affected and the phenotype of disease. Conclusion:: The consensus opinion indicates that urinary tract reconstruction using bowel segments is feasible in carefully selected and optimised patients with IBD lacking alternative management options, provided there is access to appropriate multidisciplinary skills. UC is relatively low risk for surgical procedures, whereas Crohn’s has considerably increased risk of morbidity. The potential risks must be properly discussed with patients considering reconstructive urological procedures. Outcomes should be carefully monitored and published to identify the safety and efficacy of reconstructive surgery in IBD, including full description of the disease status.

14. Physical and Chemical Modifications of Plant Fibres for Reinforcement in Cementitious Composites

Abstract

This paper highlights the physical and chemical surface modifications of plant fibre (PF) for attaining suitable properties as reinforcements in cementitious composites. Untreated PF faces insufficient adhesion between the fibres and matrix due to high levels of moisture absorption and poor wettability. These conditions accelerate degradation of the fibre in the composite. It is also essential to reduce the risk of hydrophilic PF conditions with surface modification, to enhance the mechanical properties of the fibres. Fibres that undergo chemical and physical modifications had been proven to exhibit improved fibre-matrix interfacial adhesion in the composite and contribute to better composite mechanical properties. This paper also gives some recommendations for future research on chemical and physical modifications of PF.

15. Two Cases of Oculofaciocardiodental (OFCD) Syndrome due to X-Linked BCOR Mutations Presenting with Infantile Hemangiomas: Phenotypic Overlap with PHACE Syndrome

Background. Oculofaciocardiodental (OFCD) syndrome is due to mutations in BCOR (BCL-6 corepressor). OFCD has phenotypic overlaps with PHACE syndrome (Posterior fossa anomalies, Hemangioma, Arterial anomalies, Cardiac defects, Eye anomalies). Infantile hemangiomas are a key diagnostic criterion for PHACE, but not for OFCD. A previous study reported two cases of infantile hemangiomas in OFCD, but the authors could not exclude chance association. Case Presentation. We describe two novel cases of female patients (one initially diagnosed with PHACE syndrome), both of whom had infantile hemangiomas. Ophthalmological findings were consistent with oculofaciocardiodental (OFCD) syndrome. Upon genetic testing, these two females were determined to have X-linked BCOR mutations confirming OFCD syndrome diagnoses. Conclusion. These case reports add support to the hypothesis that infantile hemangiomas may be a feature of OFCD. BCOR may potentially be within a pathway of genes involved in PHACE syndrome and/or in infantile hemangioma formation.

Abstract

In recent times, there is an increasing need for the fabrication of mortar and concrete that can be characterised as sustainable and environmentally friendly. Ideally, this concrete should be inexpensive, lightweight, and outstanding in terms of its physical and mechanical specifications. Plastic materials have increasingly been used in the fabrication of different types of concrete admixtures and mortar constituents. These plastic materials take the form of fillers or shredded fibres derived from polypropylene and polyethylene terephthalate. The use of plastic materials presents the following benefits: (i) enhanced mixture quality and (ii) a reduction in the amount of accumulated single-use plastic materials that negatively impact the environment. This work reviews several previous studies on the utilisation and preparations of plastic materials and their effects on the physical and mechanical properties of concrete. Other topics, including hardened concrete, fresh concrete, application, and thermo-physical characteristics, are also elaborated.

Abstract

A study was conducted to examine the effect of mixing additives with concrete to increase the strength and durability of the concrete. The study looked at the how the use of additives affect several properties of the concrete. In this study a polypropylene (PP) fibre was mixed with concrete to study the mixture’s dynamic response under impact load. References related to dynamic impact test for polypropylene fibre reinforced concrete (PPFRC) is very scarce and there is no specific research and information relating to this research. Therefore, this study looked at the dynamic impact of PPFRC through the use of Split Hopkinson Pressure Bar (SHPB). The samples used in this study comprised of concrete mixed with 1.0 kg/m³ PP fibre and 2.0 kg/m³ PP fibre, as well as plain concrete as a control sample. The PP fibre contains twisted bundle nonfibrillating monofilament and fibrillating network fibres. Samples were prepared using a cylindrical mould and three samples of each mixture were prepared with a 28-day curing period and a concrete grade of 35 MPa. These samples were then tested for dynamic impact using the SHPB at 2 MPa pressure under the strain rate of 10 s-1. The results for dynamic compressive strength showed that the values for samples SC1 and SC2, 13.22 % and 76.9 % respectively, increased compared to value of the control sample, and the dynamic compressive strength for sample SC1 and SC2 are74.5 MPa and 116.4 MPa respectively compared to 65.8 MPa for the control sample. The dynamic increase factor (DIF) showed that sample SC2 yielded the highest value (4.15) whereas the values for samples SC1 and SC3 are 2.14 and 1.97 respectively.

18. Global-scale object detection using satellite imagery

Abstract

In recent years, there has been a substantial increase in the availability of high-resolution commercial satellite imagery, enabling a variety of new remote-sensing applications. One of the main challenges for these applications is the accurate and efficient extraction of semantic information from satellite imagery. In this work, we investigate an important instance of this class of challenges which involves automatic detection of multiple objects in satellite images. We present a system for large-scale object training and detection, leveraging recent advances in feature representation and aggregation within the bag-of-words paradigm. Given the scale of the problem, one of the key challenges in learning object detectors is the acquisition and curation of labeled training data. We present a crowd-sourcing based framework that allows efficient acquisition of labeled training data, along with an iterative mechanism to overcome the label noise introduced by the crowd during the labeling process. To show the competence of the presented scheme, we show detection results over several object-classes using training data captured from close to 200 cities and tested over multiple geographic locations.

Abstract

Wood fibre cement (WFC) boards are well established commercially and widely used in many developed countries. The combination of the properties of two important materials, i.e., cement, and previously treated fibrous materials like wood or agricultural residues; which made up the board, contributed in the performance of the board as building material. In this work, the WFC matrix (WFCM) samples are produced to determine the physical properties of WFCM such as the density and water absorption. The wood fibres are incorporated/treated with three different chemical additives; calcium formate (Ca(HCOO)2), sodium silicate (Na2.SiO3) and magnesium chloride (MgCl2) prior to mixing with cement. The mechanical properties of the WFCM, with or without chemicals treatment of fibres, such as the compressive strength and flexural strength are evaluated. Three wood/cement ratios (50:50, 40:60, 30:70) are used and the percentages of water and accelerator were 80% and 3% based on the cement weight, respectively. Three moisture-conditioned samples; accelerated aging, dry and wet conditions are used for flexural test. The results reveal that the wood/cement ratio, chemical additives and moisture content had a marked influence on the physical and mechanical properties of the matrix. Finally, it has been shown that the 40:60 wood/cement ratio samples with prior chemicals treatment of the fibres that undergo accelerated aging conditioning achieve higher strength then dry and wet-conditioned boards.

20. Dynamic Stress-Strain Behaviour of Steel Fiber Reinforced High-Performance Concrete with Fly Ash

Abstract

The addition of steel fibers into concrete mix can significantly improve the engineering properties of concrete. The mechanical behaviors of steel fiber reinforced high-performance concrete with fly ash (SFRHPFAC) are studied in this paper through both static compression test and dynamic impact test. Cylindrical and cube specimens with three volume fractions of end-hooked steel fibers with volume fraction of 0.5%, 1.0%, and 1.5% (39.25, 78.50, and 117.75 kg/m3) and aspect ratio of 64 are used. These specimens are then tested for static compression and for dynamic impact by split Hopkinson pressure bar (SHPB) at strain rate of 30–60 s−1. The results reveal that the failure mode of concrete considerably changes from brittle to ductile with the addition of steel fibers. The plain concrete may fail under low-strain-rate single impact whereas the fibrous concrete can resist impact at high strain rate loading. It is shown that strain rate has great influence on concrete strength. Besides, toughness energy is proportional to the fiber content in both static and dynamic compressions.

21. Pneumothorax and Pneumomediastinum in Pregnancy: A Case Report

Abstract

Case Report. A 37 years old patient at 40 weeks gestation presented with acute severe hypoxia with a seizure followed by fetal bradycardia. Caesarean section was performed under GA and she was intubated and ventilated. History revealed longstanding right pleural endometriosis with multiple pneumothoraces and hydrothoraces. A CT chest showed extensive bilateral pnenumothoraces. Her clinical condition improved with a left-sided chest drain. Discussion. Severe hypoxia and seizures in a patient with previous history of pnenumothorax are highly suggestive of tension pneumothorax. Radiological investigations are vital for diagnosis. The traditional treatment approach to recurrent pneumothorax has been thorocotomy with bleb or bulla resection and pleurodeisis. The advantages of thorocoscopic surgical treatment over thorocotomy are decreased time of exposure to anaesthetic drugs, rapid lung expansion, decreased post operative pain, and a potentially shorter post operative recovery. In any future pregnancy due to the high risk of recurrence of pneumothorax Contemporary obstetric management should determine the method of delivery and continuous lumbar/epidural anesthesia should be used if at all feasible. Preconceptual counseling about this risk is vital, and women must be advised about potential serious adverse outcomes.

Abstract

Importance: Lip, oral, and pharyngeal cancers are important contributors to cancer burden worldwide, and a comprehensive evaluation of their burden globally, regionally, and nationally is crucial for effective policy planning. Objective: To analyze the total and risk-attributable burden of lip and oral cavity cancer (LOC) and other pharyngeal cancer (OPC) for 204 countries and territories and by Socio-demographic Index (SDI) using 2019 Global Burden of Diseases, Injuries, and Risk Factors (GBD) Study estimates. Evidence review: The incidence, mortality, and disability-adjusted life years (DALYs) due to LOC and OPC from 1990 to 2019 were estimated using GBD 2019 methods. The GBD 2019 comparative risk assessment framework was used to estimate the proportion of deaths and DALYs for LOC and OPC attributable to smoking, tobacco, and alcohol consumption in 2019. Findings: In 2019, 370 000 (95% uncertainty interval [UI], 338 000–401 000) cases and 199 000 (95% UI, 181 000–217 000) deaths for LOC and 167 000 (95% UI, 153 000‐180 000) cases and 114 000 (95% UI, 103 000–126 000) deaths for OPC were estimated to occur globally, contributing 5.5 million (95% UI, 5.0‐6.0 million) and 3.2 million (95% UI, 2.9‐3.6 million) DALYs, respectively. From 1990 to 2019, low-middle and low SDI regions consistently showed the highest age-standardized mortality rates due to LOC and OPC, while the high SDI strata exhibited age-standardized incidence rates decreasing for LOC and increasing for OPC. Globally in 2019, smoking had the greatest contribution to risk-attributable OPC deaths for both sexes (55.8% [95% UI, 49.2%‐62.0%] of all OPC deaths in male individuals and 17.4% [95% UI, 13.8%‐21.2%] of all OPC deaths in female individuals). 26. Improving Accessibility of Technical Drilling Applications via Wells on Cloud-based Platform

Abstract

Technical engineering applications have long been an important tool for engineers in the well design process. The nature of being on-prem in individual devices has posed several issues, where lengthy installation processes often lead to a large amount of time consumption and disrupts the engineers’ work efficiency. As the industry moves into the era of digitalization, the Wells on Cloud solution is introduced. The migration of current on-prem applications into a SaaS Cloud-based solution aims to mitigate the issues encountered by users via the elimination of installation processes and automatic push updates. The WoC solution aligns with Wells Digital Roadmap to act as a central hub and a single source of truth while focusing on three key principles. It aspires to cater to solutions from different vendors, capitalize on established Cloud-based infrastructure to host the solution platform and facilitate data transparency. Being a Cloud-based solution, all updates can be implemented and pushed to the user's devices, and users can access their data with a device anytime and anywhere. Direct links to the Open-End Community are incorporated into the solution. Moving on, the WoC's security system consists of active detection of anomalies, protecting against these issues, and comprehensive strategies to respond to any potential breakdown. To measure the value creation resulting from this initiative, the quantitative process cycle efficiency and qualitative feedback survey are conducted. PCE improvements demonstrated the timeline improvement brought by this solution, along with the positive comments shared by heavy users of these applications. The successful deployment of the Wells on Cloud solution has equipped engineers with the tools to unlock the full capability of technical engineering applications right at their fingertips. It also opened the avenues for business agility and scalability to match business requirements, reducing the Total Cost of Ownership and leading to savings. This solution sets the path for Cloud-based working and drives the industry to move towards digitally enabled businesses.

Abstract

The identification of factors impeding normal fetal development and growth is crucial for improving neonatal health. Historical studies are relevant because they show which parameters have influenced neonatal health in the past in order to better understand the present. We studied temporal changes of neonatal health outcomes (birth weight, gestational age, stillbirth rate) and the influence of different cofactors in two time periods. Moreover, we investigated particularly neonatal health in the wake of the 1918/19 influenza pandemic. Data were transcribed from the Bern Maternity Hospital and consists of two time periods: A) The years 1880, 1885, 1890, 1895 and 1900 (N = 1530, births’ coverage 20%); B) The years 1914–1922 (N = 6924, births’ coverage 40–50%). Linear regression models were used to estimate the effect of birth year on birth weight, and logistic regression models to estimate the effect of birth year and of the exposure to the pandemic on premature birth, stillborn and low birth weight (LBW). Mean birth weight increased only minimally between the two datasets; whereas, in the years 1914–1922, the preterm birth and stillbirth rates were markedly reduced compared with the years 1880–1900. Sex, parity, gestational age and maternal age were significantly associated with birth weight in both time periods. The probability of LBW was significantly increased in 1918 (OR 1.49 (95% CI 1.00–2.23)) and in 1919 (OR 1.55 (95% CI 1.02–2.36)) compared to 1914. Mothers who were heavily exposed to the influenza pandemic during pregnancy had a higher risk of stillbirth (OR 2.27 (95% CI 1.32–3.9)). This study demonstrated that factors influencing neonatal health are multifactorial but similar in both time periods. Moreover, the exposure to the 1918/19 pandemic was less associated with LBW and more associated with an increased risk of stillbirth. Abstract

This article presents a hybrid metasurface split beam antenna for fifth-generation (5G) mobile applications at 3.5 GHz. Multi-beam antennas with high directivity are required for a 5G mobile network. It can be achieved by having an array antenna. Using an antenna array at low frequency increases the complexity and size of the whole network. Therefore, a metasurface (MS) is proposed to direct surface current and to have high gain and multibeam properties. To achieve that, a square split ring resonator (SSRR) and U-shaped unit cell metasurface is implemented as a superstrate to a single square patch antenna. The manipulation of this hybrid metasurface configuration can create opposite current flow on the unit cell and thus split the beam. The hybrid metasurface superstrate and antenna are fabricated on FR-4 ( $\varepsilon \text{r}\,\,=4.4$ , tan $\delta =0.02$ ). Results show that the antenna resonates well at 3.5 GHz, with a less than −10 dB reflection coefficient. The arrangement of the unit cells on the superstrate metasurface is able to split the current, and thus the radiation pattern beam is split into two beams in the E-plane at ± 45°. This antenna is a good candidate for future 5G Pico cell base stations in urban or suburban areas with high capacity and interferences.

Abstract

Domestic violence is a worldwide problem, and there are over a billion women who have been exposed to violence from their husband or male partner. In Iraq, since the 2003 invasion, domestic violence has risen dramatically. In this study we aim to estimate the prevalence of domestic violence against women (and some associated factors) in Baghdad city. A sample of 735 women was collected from the attendants of the outpatients-Obstetrics/Gynaecology clinics of four randomly chosen health facilities located on either side of Baghdad city. A questionnaire form was developed to enquire about the occurrence of violence, its frequency, source, and reasons. The results showed that 81.2% of the women were exposed to violence (100% verbal/emotional and 39.18% physical), usually by the husband (41.4%) or brother (20.4%), mostly attributed to a stressful life of unrest (41.9%). Older and currently married women are significantly more exposed to domestic violence (p = 0.0010.004, respectively). Consanguinity was associated with less violence (p = 0.0001) as was higher education (p = 0.001), employment (p = 0.044) and women's independence (p = 0.044). A stressful life of unrest was significantly associated with all types of violence (p 0.001).

CYP3A4*1B is a single nucleotide polymorphism of CYP3A4 and is associated with prostate cancer which exhibits higher nifedipine oxidase activity in liver. This research provides details of the effects of structural variation and medium effects for the recently reported split-oligonucleotide (tandem) probe system for excimers-based fluorescence detection of DNA. In this approach the detection system is split at a molecular level into signal-silent components, which must be assembled correctly into a specific 3-dimensional structure to ensure close proximity of the excimer partners and the consequent excimer fluorescence emission on excitation. The model system consists of two 11-mer oligonucleotides, complementary to adjacent sites of a 22-mer DNA target. Each oligonucleotide probeis equipped with functions able to form an excimer on correct, contiguous hybridization. The extremely rigorous structural demands for excimer formation and emission required careful structural design of partners for excimer formation, which are here described. This study demonstrates that the excimer formed emitted at ~480 nm with alarge Stokes shift (~130 - 140 nm).

Abstract

Potential consequences of inflammatory bowel disease (IBD) need evaluation for patients considering urinary tract reconstruction for benign disease. A working group was formed by the International Continence Society, which considered urinary tract reconstruction in IBD.Methods Nominal group technique was used to derive consensus. Principal aspects of IBD assessment and surgery decision-making were agreed. A questionnaire was used to facilitate the generation of statements by a core focus group of experts, which were modified and ratified by the wider working group. This was followed by final voting by the full working group.Results General considerations included identification importance of the specialist IBD multi-disciplinary team. Peri-operative considerations recommended avoiding pre-operative fasting from midnight and using an enhanced recovery after surgery (ERAS) protocol. Selection of bowel segment, pre-operative optimisation and post-operative issues were considered for both Ulcerative colitis (UC) and Crohn’s disease. UC is not an absolute contraindication to urinary tract reconstruction using small or large bowel. Elective reconstructive surgery should wait at least three months following resolution of any acute UC flare-up to correct all abnormalities. Crohn’s disease is a high-risk disease for urinary tract reconstruction, even if in remission. In Crohn’s, reconstructive surgical options are limited by the location and extent of gastrointestinal segment(s) affected and the phenotype of disease.Conclusion The consensus opinion indicates that urinary tract reconstruction using bowel segments is feasible in carefully selected and optimised patients with IBD lacking alternative management options, provided there is access to appropriate multidisciplinary skills. UC is relatively low risk for surgical procedures, whereas Crohn’s has considerably increased risk of morbidity. The potential risks must be properly discussed with patients considering reconstructive urological procedures. Outcomes should be carefully monitored and published to identify the safety and efficacy of reconstructive surgery in IBD, including full description of the disease status.

Abstract

A compact bandpass filter using U-shaped resonators and Defected Ground Structures is proposed and designed at 5.8 GHz. The U-shaped resonators are placed around an indirectly coupled feed line while the Defected Ground Structures are positioned beneath them. The U-shaped resonator and U-shaped Defected Ground Structure are responsible for the high and low band rejection respectively. The proposed bandpass filter obeys the second order Chebyshev response which has low insertion loss of – 1.87 dB, high rejection level and a sharp roll-off performance. The design is carried out using CST Microwave studio. The design is verified by fabricating and measuring the prototype in the laboratory. A good agreement is observed between the simulated and measured results.

Abstract

Introduction: The Transradial artery access for Primary Percutaneous Coronary Intervention (PCI) is the preferred access for coronary interventions in ST Elevation Myocardial Infarction STEMI due to less complications post intervention and it has been shown to improve quality of life of patients post intervention. This decrease in complications may give the patient sense of ease and underestimation of disease severity, and hence worsen the patient’s compliance with medications. This retrospective observation study aimed to assess the impact of radial versus femoral access coronary interventions on adherence to post-PCI medications. Methods: We conducted a retrospective analysis for all patients who presented with STEMI and underwent primary PCI in a tertiary cardiac hospital in Qatar between January 1, 2016 and December 31, 2018. Adherence was assessed by refilling the first prescription post discharge. Adherence rates were compared between the patients who underwent PCI with radial versus femoral access using the Chi square test. Results: A total of 1339 patients who presented with acute STEMI and underwent primary PCI were included in our retrospective analysis. The majority of patients included were male (96%) with mean age of 51 years. The majority of patients (1082; 81%) underwent radial access, while 257 (19%) underwent femoral access. The two groups were comparable in terms of age, nationality, the presence of full versus partial insurance, and using medications before admission. The patients adherence to medications post-PCI, including aspirin, P2Y12 inhibitor, statin, beta-blocker, angiotensin converting enzyme (ACE) inhibitor/ angiotensin II receptor blocker (ARB) was comparable between the radial versus femoral access ( aspirin: 78% vs. 74%; p-value= 0.16, P2Y12 inhibitor: 81% vs. 79%; p-value=0.45, statin: 80% vs. 76%; p-value= 0.15, beta-blocker: 72% vs. 70; p-value= 0.51, ACE inhibitor/ARB: 79% vs. 74%; p-value= 0.09). Conclusion: In comparison to femoral access in primary coronary intervention in STEMI, the radial access resulted in similar adherence to post-PCI medications assessed by refilling of first prescription post discharge. Therefore, the radial access is a safe approach in terms medications adherence.

Abstract

Background: Accurate and up-to-date assessment of demographic metrics is crucial for understanding a wide range of social, economic, and public health issues that affect populations worldwide. The Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019 produced updated and comprehensive demographic assessments of the key indicators of fertility, mortality, migration, and population for 204 countries and territories and selected subnational locations from 1950 to 2019. Methods: 8078 country-years of vital registration and sample registration data, 938 surveys, 349 censuses, and 238 other sources were identified and used to estimate age-specific fertility. Spatiotemporal Gaussian process regression (ST-GPR) was used to generate age-specific fertility rates for 5-year age groups between ages 15 and 49 years. With extensions to age groups 10–14 and 50–54 years, the total fertility rate (TFR) was then aggregated using the estimated age-specific fertility between ages 10 and 54 years. 7417 sources were used for under-5 mortality estimation and 7355 for adult mortality. ST-GPR was used to synthesise data sources after correction for known biases. Adult mortality was measured as the probability of death between ages 15 and 60 years based on vital registration, sample registration, and sibling histories, and was also estimated using ST-GPR. HIV-free life tables were then estimated using estimates of under-5 and adult mortality rates using a relational model life table system created for GBD, which closely tracks observed age-specific mortality rates from complete vital registration when available. Independent estimates of HIV-specific mortality generated by an epidemiological analysis of HIV prevalence surveys and antenatal clinic serosurveillance and other sources were incorporated into the estimates in countries with large epidemics. Abstract

Developing trustworthy, cost effective, minimally or non-invasive glucose sensing strategies is of great need for diabetic patients. In this study, we used an experimental type I diabetic mouse model to examine whether the skin would provide novel means for identifying biomarkers associated with blood glucose level. We first showed that skin glucose levels are rapidly influenced by blood glucose concentrations. We then conducted a proteomic screen of murine skin using an experimental in vivo model of type I diabetes and wild-type controls. Among the proteins that increased expression in response to high blood glucose, Trisk 95 expression was significantly induced independently of insulin signalling. A luciferase reporter assay demonstrated that the induction of Trisk 95 expression occurs at a transcriptional level and is associated with a marked elevation in the Fluo-4AM signal, suggesting a role for intracellular calcium changes in the signalling cascade. Strikingly, these changes lead concurrently to fragmentation of the mitochondria. Moreover, Trisk 95 knockout abolishes both the calcium flux and the mitochondrial phenotype changes indicating dependency of glucose flux in the skin on Trisk 95 function. The data demonstrate that the skin reacts robustly to systemic blood changes, and that Trisk 95 is a promising biomarker for a glucose monitoring assembly.

Abstract

Introduction Reports comparing clinical outcomes using nickel-titanium (NiTi) reciprocating instruments with other instrumentation modalities are scarce. This study examined initial shaping outcomes after an instrumentation change of root canal instrumentation technique in a doctor of dental surgery educational program. Student characteristics, faculty/student ratio, facility, and overall endodontic treatment guidelines remained unchanged. Methods A total of 200 nonsurgical initial molar root canal treatments completed by third-year dental students were evaluated. The cases were examined regarding the number of treatment appointments, access cavity preparation, canal taper, canal transportation, perforations, missed canals, presence of ledges, fractured instruments, obturation length, obturation quality, and sealer extrusion. Two independent evaluators determined the number of appointments per case; 4 independent and blinded evaluators analyzed radiographs at 4 treatment stages: preoperative situation, working length, cone fit, and obturation. Results The following factors were significantly different between the 2 cohorts: the number of appointments, preparation length, taper, and occurrence of ledges. The WaveOne (Dentsply Sirona, York, PA) cohort had a significantly reduced treatment time compared with hand/GT rotary instrumentation (Dentsply Tulsa Dental, Tulsa, OK) (average of 3.3 vs 4.3 appointments). Appropriate length control and adequate taper were significantly more frequent in the WaveOne group. The frequency of ledges was significantly larger in the hybrid group. Other variables, such as access cavity preparation, canal transportation, perforations, missed canals, fractured instruments, obturation quality, and sealer extrusion, were statistically similar between the 2 groups. Conclusions NiTi reciprocation instrumentation was superior to hybrid hand/NiTi rotary instrumentation in reducing both patient appointments and the incidence of ledging and in improving obturation length and taper in a dental student clinic setting.
