Purpose: Clear cell renal cell carcinoma (ccRCC) has recently been redefined as a highly heterogeneous disease. In addition to genetic heterogeneity, the tumor displays risk variability for developing metastatic disease, therefore underscoring the urgent need for tissue-based prognostic strategies applicable to the clinical setting. We have recently employed the novel PET/magnetic resonance (MR) image modality to enrich our understanding of how tumor heterogeneity can relate to gene expression and tumor biology to assist in defining individualized treatment plans.Experimental Design: ccRCC patients underwent PET/MR imaging, and these images subsequently used to identify areas of varied intensity for sampling. Samples from 8 patients were subjected to histologic, immunohistochemical, and microarray analysis.Results: Tumor subsamples displayed a range of heterogeneity for common features of hypoxia-inducible factor expression and microvessel density, as well as for features closely linked to metabolic processes, such as GLUT1 and FBP1. In addition, gene signatures linked with disease risk (ccA and ccB) also demonstrated variable heterogeneity, with most tumors displaying a dominant panel of features across the sampled regions. Intriguingly, the ccA- and ccB-classified samples corresponded with metabolic features and functional imaging levels. These correlations further linked a variety of metabolic pathways (i.e., the pentose phosphate and mTOR pathways) with the more aggressive, and glucose avid ccB subtype.Conclusions: Higher tumor dependency on exogenous glucose accompanies the development of features associated with the poor risk ccB subgroup. Linking these panels of features may provide the opportunity to create functional maps to enable enhanced visualization of the heterogeneous biologic processes of an individual's disease. The latest Permian mass extinction (LPME) was triggered by magmatism of the Siberian Traps Large Igneous Province (STLIP), which left an extensive record of sedimentary Hg anomalies at Northern Hemisphere and tropical sites. Here, we present Hg records from terrestrial sites in southern Pangea, nearly antipodal to contemporaneous STLIP activity, providing insights into the global distribution of volcanogenic Hg during this event and its environmental processing. These profiles (two from Karoo Basin, South Africa; two from Sydney Basin, Australia) exhibit significant Hg enrichments within the uppermost Permian extinction interval as well as positive Δ199Hg excursions (to ~0.3‰), providing evidence of long-distance atmospheric transfer of volcanogenic Hg. These results demonstrate the far-reaching effects of the Siberian Traps as well as refine stratigraphic placement of the LPME interval in the Karoo Basin at a temporal resolution of ~105 years based on global isochronism of volcanogenic Hg anomalies.

Rapid climate change was a major contributor to the end-Permian extinction (EPE). Although well constrained for the marine realm, relatively few records document the pace, nature, and magnitude of climate change across the EPE in terrestrial environments. We generated proxy records for chemical weathering and land surface temperature from continental margin deposits of the high-latitude southeastern margin of Gondwana. Regional climate simulations provide additional context. Results show that Glossopteris forest-mire ecosystems collapsed during a pulse of intense chemical weathering and peak warmth, which capped ∼1 m.y. of gradual warming and intensification of seasonality. Erosion resulting from loss of vegetation was short lived in the low-relief landscape. Earliest Triassic climate was∼10–14 °C warmer than the late Lopingian and landscapes were no longer persistently wet. Aridification, commonly linked to the EPE, developed gradually, facilitating the persistence of refugia for moisture-loving terrestrial groups. To evaluate the inter-reader reproducibility and prognostic accuracy of the Liver Imaging Reporting and Data System (LI-RADS) treatment response algorithm (LR-TR) at the time of initial post-treatment evaluation following drug-eluting beads transarterial chemoembolization (DEB-TACE) for hepatocellular carcinoma (HCC). This retrospective study included patients with HCC who underwent first-line DEB-TACE between January 2011 and December 2015. Six readers (three fellowship-trained radiologists and three radiology trainees) independently assessed lesion-level response in up to two treated lesions per LR-TR and modified Response Evaluation Criteria in Solid Tumors (mRECIST)-target criteria, as well as patient-level response per mRECIST-overall criteria, on the initial post-treatment CT/MRI. Inter-reader agreement was calculated by Fleiss’ multi-reader κ. We tested whether LR-TR, mRECIST-target, and mRECIST-overall response were associated with overall survival using Kaplan–Meier and Cox proportional hazard model analyses. A total of 82 patients with 113 treated target lesions were included. Inter-reader agreement was moderate for LR-TR and mRECIST-overall (κ range 0.42–0.57), and substantial for mRECIST-target (κ range 0.62–0.66), among all three reader-groups: all readers, experienced readers, and less-experienced readers. LR-TR and mRECIST-target response were not significantly associated with overall survival regardless of reader experience (P > 0.05). In contrast, mRECIST-overall response was significantly associated with overall survival when assessed by all readers (P = 0.02) and experienced readers (P = 0.03), but not by the less-experienced readers (P = 0.35). Although LR-TR algorithm has moderate inter-reader reproducibility, it alone may not predict overall survival on the initial post-treatment CT/MRI after first-line DEB-TACE for HCC.

PURPOSE: To develop and evaluate domain-specific and pretrained bidirectional encoder representations from transformers (BERT) models in a transfer learning task on varying training dataset sizes to annotate a larger overall dataset. MATERIALS AND METHODS: The authors retrospectively reviewed 69 095 anonymized adult chest radiograph reports (reports dated April 2020–March 2021). From the overall cohort, 1004 reports were randomly selected and labeled for the presence or absence of each of the following devices: endotracheal tube (ETT), enterogastric tube (NGT, or Dobhoff tube), central venous catheter (CVC), and Swan-Ganz catheter (SGC). Pretrained transformer models (BERT, PubMedBERT, DistilBERT, RoBERTa, and DeBERTa) were trained, validated, and tested on 60%, 20%, and 20%, respectively, of these reports through fivefold cross-validation. Additional training involved varying dataset sizes with 5%, 10%, 15%, 20%, and 40% of the 1004 reports. The best-performing epochs were used to assess area under the receiver operating characteristic curve (AUC) and determine run time on the overall dataset. RESULTS: The highest average AUCs from fivefold cross-validation were 0.996 for ETT (RoBERTa), 0.994 for NGT (RoBERTa), 0.991 for CVC (PubMedBERT), and 0.98 for SGC (PubMedBERT). DeBERTa demonstrated the highest AUC for each support device trained on 5% of the training set. PubMedBERT showed a higher AUC with a decreasing training set size compared with BERT. Training and validation time was shortest for DistilBERT at 3 minutes 39 seconds on the annotated cohort. CONCLUSION: Pretrained and domain-specific transformer models required small training datasets and short training times to create a highly accurate final model that expedites autonomous annotation of large datasets. Pelvic floor disorders are common and can negatively impact quality of life. Imaging of patients with pelvic floor disorders has been extremely heterogeneous between institutions due in part to variations in clinical expectations, technical considerations, and radiologist experience. In order to assess variations in utilization and technique of pelvic floor imaging across practices, the society of abdominal radiology (SAR) disease-focused panel on pelvic floor dysfunction developed and administered an online survey to radiologists including the SAR membership. Results of the survey were compared with published recommendations for pelvic floor imaging to identify areas in need of further standardization. MRI was the most commonly reported imaging technique for pelvic floor imaging followed by fluoroscopic defecography. Ultrasound was only used by a small minority of responding radiologists. The survey responses demonstrated variability in imaging utilization, patient referral patterns, imaging protocols, patient education, and interpretation and reporting of pelvic floor imaging examinations. This survey highlighted inconsistencies in technique between institutions as well as potential gaps in knowledge that should be addressed to standardize evaluation of patients with pelvic floor dysfunction.

Most sequence stratigraphic models are based on the premise that relative changes in sea level (RSL) control stacking patterns in continental-margin settings. An alternative hypothesis, however, is that upstream factors, notably variations in relative water discharge (RQW) or the ratio of water to sediment discharge can influence or control stratal stacking patterns in fluvial systems. Sequence boundaries of RQW-driven systems differ from those driven by base-level fluctuations in that: 1) the depth of incision increases updip, and 2) rates of erosion are spatially uniform, leading to the formation of widespread, planar sequence boundaries. This paper presents an architectural and stratigraphic analysis of the well-exposed Masuk Formation of the Henry Mountains Syncline in southern Utah, an Upper Cretaceous coastal-plain fluvial succession that is interpreted to have been influenced significantly by RQW. Six lithofacies are recognized, three (Facies 1–3) recording floodbasin, mire, and (in one short interval) estuarine environments, and three (Facies 4–6) record different kinds of channel fills on a coastal alluvial plain. Seven major composite channel bodies (Facies 4–6), separated by intervals of non-channel deposits (Facies 1–3), are recognized in the stratigraphic interval. Composite channel bodies display planar, sheet-like geometry and are laterally continuous to a significantly greater extent (> 10 km) than would be expected from purely autogenic channel-belt construction. Together, these intervals record a series of high-frequency sequences, formed along the western margin of the Western Interior Seaway. In each individual sequence is a repetitive facies succession from a basal chaotic sandstone with admixed mudrock and sandstone transitioning upward to a more organized cross-bedded and stratified sandstone. This is interpreted to record cyclical changes from a peaked (flashy) discharge regime to a more normal runoff regime. Paleoflow data indicate a dominance of transverse (eastward-directed) dispersal early in the accumulation of the Masuk Formation, shifting to a pattern of greater axial (northward) dispersal over time. The RQW signal is strong in the lower part of the formation, decreasing upward. This suggests that the relatively short-headed streams draining from the rising Sevier fold–thrust belt were strongly influenced by climatic cyclicity, whereas more distally sourced systems were not. This study provides new insights into the architecture and stacking patterns of coastal-plain fluvial successions, emphasizing the plausible role that climate can play in shaping alluvial architecture in the rock record.

The global programme for the eradication of Guinea worm disease, caused by the parasitic nematode Dracunculus medinensis, has been successful in driving down human cases, but infections in non-human animals, particularly domestic dogs (Canis familiaris), now present a major obstacle to further progress. Dog infections have mainly been found in Chad and, to a lesser extent, in Mali and Ethiopia. While humans classically acquire infection by drinking water containing infected copepods, it has been hypothesized that dogs might additionally or alternatively acquire infection via a novel pathway, such as consumption of fish or frogs as possible transport or paratenic hosts. We characterized the ecology of free-ranging dogs living in three villages in Gog woreda, Gambella region, Ethiopia, in April-May 2018. We analysed their exposure to potential sources of Guinea worm infection and investigated risk factors associated with infection histories. The home ranges of 125 dogs and their activity around water sources were described using GPS tracking, and the diets of 119 dogs were described using stable isotope analysis. Unlike in Chad, where Guinea worm infection is most frequent, we found no ecological or behavioural correlates of infection history in dogs in Ethiopia. Unlike in Chad, there was no effect of variation among dogs in their consumption of aquatic vertebrates (fish or frogs) on their infection history, and we found no evidence to support hypotheses for this novel transmission pathway in Ethiopia. Dog owners had apparently increased the frequency of clean water provision to dogs in response to previous infections. Variations in dog ranging behaviour, owner behaviour and the characteristics of natural water bodies all influenced the exposure of dogs to potential sources of infection. This initial study suggests that the classical transmission pathway should be a focus of attention for Guinea worm control in non-human animals in Ethiopia.

Discharge event frequency, magnitude and duration all control river channel morphology and sedimentary architecture. Uncertainty persists as to whether alluvial deposits in the rock record are a time-averaged amalgam from all discharge events, or a biased record of larger events. This paper investigates the controls on channel deposit character and subsurface stratigraphic architecture in a river with seasonal discharge and very high inter-annual variability, the Burdekin River of north-east Australia. In such rivers, most sediment movement is restricted to a few days each year and at other times little sediment moves. However, the maximum discharge magnitude does not directly correlate with the amount of morphological change and some big events do not produce large deposits. The Burdekin channel deposits consist of five main depositional elements: (i) unit bars; (ii) vegetation-generated bars; (iii) gravel sheets and lags; (iv) antidune trains; and (v) sand sheets. The proportions of each depositional element preserved in the deposits depend on the history of successive large discharge events, their duration and the rate at which they wane. Events with similar peak magnitude but different rate of decline preserve different event deposits. The high intra-annual and inter-annual discharge variability and rapid rate of stage change make it likely that small to moderate-scale bed morphology will be in disequilibrium with flow conditions most of the time. Consequently, dune and unit bar size and cross-bed set thickness are not good indicators of event or channel size. Antidunes may be more useful as indicators of flow conditions at the time they formed. Rivers with very high coefficient of variance of maximum discharge, such as the Burdekin, form distinctive channel sediment bodies. However, the component parts are such that, if they are examined in isolation, they could lead to misleading interpretation of the nature of the depositional environment if conventional interpretations are used.

We used logistic regression to investigate whether the risk of an Irish cattle herd undergoing a bovine tuberculosis (bTB) breakdown increased with the size of the Ingoing Contact Chain (ICC) of previous herd to herd cattle movements, in a sequence up to eight moves back from the most recent, direct, movement into the herd. We further examined whether taking into account the bTB test history of each herd in the chain would improve model fit. We found that measures of cattle movements directly into the herd were risk factors for subsequent bTB restrictions, and the number of herds that animals were coming from was the most important of these. However, in contrast to a previous study in Great Britain, the ICC herd count at steps more remote than direct movements into the herd did not result in better fitting models than restricting the count to direct movements. Restricting the ICC counts to herds which had previously or would in the future test positive for bTB resulted in improved model fits, but this was not the case if only the previous test status was considered. This suggests that in many cases bTB infected animals are moving out of herds before being identified through testing, and that risk-based trading approaches should not rely solely on the previous test history of source herds as a proxy for future risk. Model fit was also improved by the inclusion of variables measuring bTB history of the herd, bTB in neighbouring herds, herd size, herd type, the movement network measures "in strength" and "betweenness", altitude, modelled badger abundance and county. Rainfall was not a good predictor. The most influential measures of bTB in nearby herds (a proxy for neighbourhood infection) were the proportion of herds with a history of bTB whose centroids were within 6 km, or whose boundaries were within 4 km, of the index herd. As well as informing national control and surveillance measures, our models can be used to identify areas where bTB rates are anomalously high, to prompt further investigation in these areas.

Appetite loss/anorexia of aging is a highly prevalent and burdensome geriatric syndrome that strongly impairs the quality of life of older adults. Loss of appetite is associated with several clinical conditions, including comorbidities and other geriatric syndromes, such as frailty. Despite its importance, appetite loss has been under-evaluated and, consequently, under-diagnosed and under-treated in routine clinical care. The International Conference on Frailty and Sarcopenia Research (ICFSR) Task Force met virtually on September 27th 2021 to debate issues related to appetite loss/anorexia of aging. In particular, topics related to the implementation and management of appetite loss in at-risk older adult populations, energy balance during aging, and the design of future clinical trials on this topic were discussed. Future actions in this field should focus on the systematic assessment of appetite in the care pathway of older people, such as the Integrated Care for Older People (ICOPE) program recommended by the World Health Organization. Moreover, clinical care should move from the assessment to the treatment of appetite loss/anorexia. Researchers continue to pursue their efforts to find out effective pharmacologic and non-pharmacologic interventions with a favorable risk/benefit ratio.

A standardized, holistic artificial intelligence (AI) curriculum is necessary to meet trainee demand and prepare them for effective AI tool use in their future practice.

Uncertainty persists over whether repetitive stratal rhythms in the Pennsylvanian of Euramerica (so-called “cyclothems”) were externally forced, in all likelihood by waxing and waning of glacial ice centers on Gondwana, or were controlled by autogenic processes. A key to resolving this dispute is the lateral extent of the individual cyclothems, with broad regional extent (beyond the plausible breadth and length of individual depositional systems such as deltas) arguing in favor of an external forcing control. This study provides a sedimentological and sequence stratigraphic analysis of the middle Pennsylvanian (Desmoinesian to early Missourian in North American stratigraphic terminology, Moscovian to early Kasimovian in the terms of the global stratigraphic nomenclature) succession of the southern Illinois Basin in Illinois, Indiana, and Kentucky, eastern USA. An array of eleven lithofacies is recognized, recording deposition of clastic, humic organic, and bioclastic carbonate sediments on a broad, low-gradient, low-paleolatitude shelf and coastal plain that were undersupplied by sediment. These facies are arranged into thirteen repetitive vertical cycles (sequences), each of which can be traced across the entire basin west to east (perpendicular to the paleoslope direction) across a distance of 250 km. Sequences are bounded by erosion surfaces that define 1–4 km-wide, deeply incised valley-fills (IVFs) that are mostly elongate towards the south-southwest, the dominant direction of paleoflow. In the west–east direction, valley erosion surfaces pass laterally into well-developed paleosols, incised locally by smaller channels. Each of these surfaces is laterally persistent across the basin. IVFs comprise multi-story bodies of conglomerate–breccia and sandstone, passing upward into heterolithic sandstone–mudrock associations, recording fluvial and later estuarine environments. Coal bodies typically occur at the tops of IVFs and are interbedded with heterolithic facies recording tidal influence, indicative of initial flooding by the sea. They are in turn overlain by estuarine and marine mudrocks and bioclastic carbonates, recording the maximum extent of marine flooding in a cycle. Each sequence is completed by heterolithic to sandstone-dominated facies of deltaic aspect that are typically truncated by the next erosion surface (sequence boundary). Plausible modern analogs suggest that sea-level excursions were of the order of 20–40 m. The great lateral persistence of not only the thirteen sequences, but also many of their component beds, argues strongly for an external control on sediment accumulation. Eccentricity-paced glacial cycles in Gondwana are invoked as the most likely cause of the cyclicity. The low-accommodation context of the Illinois Basin (average accumulation rate 6 cm/ky) contributed to the incomplete, condensed, and strongly top-truncated nature of preserved sequences.

Briny groundwater is present below the extremely cold and dry surface of the McMurdo Dry Valleys and below the seafloor of the adjacent McMurdo Sound in Antarctica. The lack of reliable groundwater samples in the region, however, has long limited understanding of its origin, nature, and spatial distribution. In this regard, intergranular carbonate cements, widespread in subsurface Cenozoic strata and recently recognized as brine precipitates, provide an indirect means of solving these issues. This study examines the petrography and isotope geochemistry of intergranular aragonite cement phases that occur in subsurface Pliocene-Quaternary sedimentary sections that formed in the lower Taylor Valley (cores DVDP-10, -11) and in offshore McMurdo Sound (core AND-1B). Aragonite cement in the coastal Taylor Valley sections is characterized by very low δ18O values (−26.9 to −19.4‰ VPDB) compared to values in the offshore section (−12.5 to −2.7‰ VPDB). These differences are interpreted to reflect two settings for cryogenic brine formation, which produced isotopically distinct brines during Quaternary glaciation. In the coastal region, seawater-meltwater mixtures were isolated and cryogenically concentrated in an ice-dammed lake setting that formed in response to the expansion of the East and West Antarctic Ice Sheets into the lower Taylor Valley. In McMurdo Sound, cryogenic concentration of seawater occurred in a semi-isolated flexural trough that was deepened by lithospheric depression of volcanic edifices and the expanded West Antarctic Ice Sheet. Aragonite cement phases serve as excellent proxies for tracing the extents of subsurface brine bodies along the continental margin of Antarctica. Given the propensity for cryogenic brine formation in glaciomarine settings, the likelihood of brine cements in rock records from other analogous high-latitude, cold settings must not be overlooked.

The collapse of late Permian (Lopingian) Gondwanan floras, characterized by the extinction of glossopterid gymnosperms, heralded the end of one of the most enduring and extensive biomes in Earth’s history. The Sydney Basin, Australia, hosts a near continuous, age-constrained succession of high southern paleolatitude (∼65–75°S) terrestrial strata spanning the end-Permian extinction (EPE) interval. Sedimentological, stable carbon isotopic, palynological, and macrofloral data were collected from two cored coal-exploration wells and correlated. Six palynostratigraphic zones, supported by ordination analyses, were identified within the uppermost Permian to Lower Triassic succession, corresponding to discrete vegetation stages before, during, and after the EPE interval. Collapse of the glossopterid biome marked the onset of the terrestrial EPE and may have significantly predated the marine mass extinctions and conodont-defined Permian–Triassic Boundary. Apart from extinction of the dominant Permian plant taxa, the EPE was characterized by a reduction in primary productivity, and the immediate aftermath was marked by high abundances of opportunistic fungi, algae, and ferns. This transition is coeval with the onset of a gradual global decrease in δ13Corg and the primary extrusive phase of Siberian Traps Large Igneous Province magmatism. The dominant gymnosperm groups of the Gondwanan Mesozoic (peltasperms, conifers, and corystosperms) all appeared soon after the collapse but remained rare throughout the immediate post-EPE succession. Faltering recovery was due to a succession of rapid and severe climatic stressors until at least the late Early Triassic. Immediately prior to the Smithian–Spathian boundary (ca. 249 Ma), indices of increased weathering, thick redbeds, and abundant pleuromeian lycophytes likely signify marked climate change and intensification of the Gondwanan monsoon climate system. This is the first record of the Smithian–Spathian floral overturn event in high southern latitudes. Machine learning (ML) is being used to process and understand data more and more throughout geology and geophysics in oil and gas exploration. However, its uptake has been slower than it has been for other industries and within exploration has been much stronger for some types of data and disciplines than it has for others. For instance, there has been significant ML focus on seismic data, while other data, such as those generated by geochemistry, have received relatively little. This imbalance in ML uptake leaves much opportunity for the growth of ML applications in these undersaturated fields.

In the Big Snowy Mountains of central Montana, USA, late Visean to Bashkirian strata preserve a nearly complete, but poorly documented, paleotropical stratigraphic succession that straddles the range of current estimates of the onset of the Late Paleozoic Ice Age (LPIA). Sedimentologic and stratigraphic investigation of the Otter (late Visean to Serpukhovian) and Heath (Serpukhovian) formations, with secondary focus on the overlying Tyler (late Serpukhovian to Bashkirian) and Alaska Bench (Bashkirian) formations, facilitated an appraisal of paleotropical environmental change preserved in this succession. Three facies associations reminiscent of environments currently forming in Shark Bay, Australia, were identified in the Otter Formation: shallow semi-restricted littoral platform, intertidal platform, and supratidal plain. Five facies associations broadly comparable to modern environments present in the Sunda Shelf and southern coast of the Persian Gulf were identified in the Heath Formation: offshore outer ramp, mid- to outer ramp, inner ramp, coastal plain, and sabkha. Facies associations preserved in the Heath Formation are here explained in the context of a protected, homoclinal carbonate ramp situated in a partially silled epicontinental embayment. A shift from low-magnitude relative sea-level oscillations preserved in the Otter Formation to a cyclothemic stratigraphic pattern entailing ≥ 6 fourth-order, high-frequency and high-magnitude relative sea-level fluctuations in the Heath Formation is here interpreted to record the main eustatic signal of the LPIA in central Montana. Current published biostratigraphic constraints for the observed stratigraphy estimate the main eustatic signal of the LPIA to have occurred approximately between 331 (base Serpukhovian) and 327 Ma in central Montana. A distinct upward transition from coal and paleosol-bearing depositional sequences in the lower Heath to evaporite and limestone-bearing depositional sequences in the upper Heath preserves a broad humid to arid paleoclimate shift during deposition of this unit, which influenced hydrographic circulation patterns and the resultant distribution of anoxic environments in the Big Snowy Trough during this time interval. Improved depositional and sequence stratigraphic models of the Heath Formation proposed in this study permit new insight into the theoretical distribution of, and water depth necessary to preserve, black, organic-rich claystone and shale in partially silled intracratonic basins, in addition to new temporal constraints on LPIA onset in paleotropical western Laurentia.

To develop recommendations for magnetic resonance (MR) defecography technique based on consensus of expert radiologists on the disease-focused panel of the Society of Abdominal Radiology (SAR). An extensive questionnaire was sent to a group of 20 experts from the disease-focused panel of the SAR. The questionnaire encompassed details of technique and MRI protocol used for evaluating pelvic floor disorders. 75% agreement on questionnaire responses was defined as consensus. The expert panel reached consensus for 70% of the items and provided the basis of these recommendations for MR defecography technique. There was unanimous agreement that patients should receive coaching and explanation of commands used during MR defecography, the rectum should be distended with contrast agent, and that sagittal T2-weighted images should include the entire pelvis within the field of view. The panel also agreed unanimously that IV contrast should not be used for MR defecography. Additional areas of consensus ranged in agreement from 75 to 92%. We provide a set of consensus recommendations for MR defecography technique based on a survey of expert radiologists in the SAR pelvic floor dysfunction disease-focused panel. These recommendations can be used to develop a standardized imaging protocol.

Upper Permian to Lower Triassic coastal plain successions of the Sydney Basin in eastern Australia have been investigated in outcrop and continuous drillcores. The purpose of the investigation is to provide an assessment of palaeoenvironmental change at high southern palaeolatitudes in a continental margin context for the late Permian (Lopingian), across the end‐Permian Extinction interval, and into the Early Triassic. These basins were affected by explosive volcanic eruptions during the late Permian and, to a much lesser extent, during the Early Triassic, allowing high‐resolution age determination on the numerous tuff horizons. Palaeobotanical and radiogenic isotope data indicate that the end‐Permian Extinction occurs at the top of the uppermost coal bed, and the Permo‐Triassic boundary either within an immediately overlying mudrock succession or within a succeeding channel sandstone body, depending on locality due to lateral variation. Late Permian depositional environments were initially (during the Wuchiapingian) shallow marine and deltaic, but coastal plain fluvial environments with extensive coal‐forming mires became progressively established during the early late Permian, reflected in numerous preserved coal seams. The fluvial style of coastal plain channel deposits varies geographically. However, apart from the loss of peat‐forming mires, no significant long‐term change in depositional style (grain size, sediment‐body architecture, or sediment dispersal direction) was noted across the end‐Permian Extinction (pinpointed by turnover of the palaeoflora). There is no evidence for immediate aridification across the boundary despite a loss of coal from these successions. Rather, the end‐Permian Extinction marks the base of a long‐term, progressive trend towards better‐drained alluvial conditions into the Early Triassic. Indeed, the floral turnover was immediately followed by a flooding event in basinal depocentres, following which fluvial systems similar to those active prior to the end‐Permian Extinction were re‐established. The age of the floral extinction is constrained to 252.54 ± 0.08 to 252.10 ± 0.06 Ma by a suite of new Chemical Abrasion Isotope Dilution Thermal Ionization Mass Spectrometry U‐Pb ages on zircon grains. Another new age indicates that the return to fluvial sedimentation similar to that before the end‐Permian Extinction occurred in the basal Triassic (prior to 251.51 ± 0.14 Ma). The character of the surface separating coal‐bearing pre‐end‐Permian Extinction from coal‐barren post‐end‐Permian Extinction strata varies across the basins. In basin‐central locations, the contact varies from disconformable, where a fluvial channel body has cut down to the level of the top coal, to conformable where the top coal is overlain by mudrocks and interbedded sandstone–siltstone facies. In basin‐marginal locations, however, the contact is a pronounced erosional disconformity with coarse‐grained alluvial facies overlying older Permian rocks. There is no evidence that the contact is everywhere a disconformity or unconformity.

Highlights • We analysed high resolution proximity networks and GPS locations of dairy cattle. • Contact rates varied markedly in time and space, and in relation to husbandry. • Contacts were more frequent and longer in buildings. • Few cows exhibited evidence of consistent relationships. • When given free access across the farm, cows showed greater variation in contacts., The nature of contacts between hosts can be important in facilitating or impeding the spread of pathogens within a population. Networks constructed from contacts between hosts allow examination of how individual variation might influence the spread of infections. Studying the contact networks of livestock species managed under different conditions can additionally provide insight into their influence on these contact structures. We collected high-resolution proximity and GPS location data from nine groups of domestic cattle (mean group size = 85) in seven dairy herds employing a range of grazing and housing regimes. Networks were constructed from cattle contacts (defined by proximity) aggregated by different temporal windows (2 h, 24 h, and approximately 1 week) and by location within the farm. Networks of contacts aggregated over the whole study were highly saturated but dividing contacts by space and time revealed substantial variation in cattle interactions. Cows showed statistically significant variation in the frequency of their contacts and in the number of cows with which they were in contact. When cows were in buildings, compared to being on pasture, contact durations were longer and cows contacted more other cows. A small number of cows showed evidence of consistent relationships but the majority of cattle did not. In one group where management allowed free access to all farm areas, cows showed asynchronous space use and, while at pasture, contacted fewer other cows and showed substantially greater between-individual variation in contacts than other groups. We highlight the degree to which variations in management (e.g. grazing access, milking routine) substantially alter cattle contact patterns, with potentially major implications for infection transmission and social interactions. In particular, where individual cows have free choice of their environment, the resulting contact networks may have a less-risky structure that could reduce the likelihood of direct transmission of infections.

To evaluate the inter-reader reproducibility and prognostic accuracy of the Liver Imaging Reporting and Data System (LI-RADS) treatment response algorithm (LR-TR) at the time of initial post-treatment evaluation following drug-eluting beads transarterial chemoembolization (DEB-TACE) for hepatocellular carcinoma (HCC).This retrospective study included patients with HCC who underwent first-line DEB-TACE between January 2011 and December 2015. Six readers (three fellowship-trained radiologists and three radiology trainees) independently assessed lesion-level response in up to two treated lesions per LR-TR and modified Response Evaluation Criteria in Solid Tumors (mRECIST)-target criteria, as well as patient-level response per mRECIST-overall criteria, on the initial post-treatment CT/MRI. Inter-reader agreement was calculated by Fleiss' multi-reader κ. We tested whether LR-TR, mRECIST-target, and mRECIST-overall response were associated with overall survival using Kaplan-Meier and Cox proportional hazard model analyses.A total of 82 patients with 113 treated target lesions were included. Inter-reader agreement was moderate for LR-TR and mRECIST-overall (κ range 0.42-0.57), and substantial for mRECIST-target (κ range 0.62-0.66), among all three reader-groups: all readers, experienced readers, and less-experienced readers. LR-TR and mRECIST-target response were not significantly associated with overall survival regardless of reader experience (P 0.05). In contrast, mRECIST-overall response was significantly associated with overall survival when assessed by all readers (P = 0.02) and experienced readers (P = 0.03), but not by the less-experienced readers (P = 0.35).Although LR-TR algorithm has moderate inter-reader reproducibility, it alone may not predict overall survival on the initial post-treatment CT/MRI after first-line DEB-TACE for HCC.

Annual peaks in reproductive activity have been identified in multiple domestic dog populations. However, there is little evidence to describe how these peaks may be associated with environmental factors such as daylength, which plays a well-established role in breeding patterns of seasonally-reproductive species. Data were collected 2016-2020 during 7,743 and 4,681 neuter surgeries on adult female unowned free-roaming dogs in veterinary clinics in Goa and Tamil Nadu respectively. Temperature, precipitation, relative humidity, and daylength data were gathered for time periods preceding the neuter surgery that may have influenced the likelihood of pregnancy (potential influence periods). A multivariable generalised additive model was used to assess the relationship between these factors and pregnancy. The prevalence of pregnancy varied by month in both locations indicating seasonality of reproduction in these groups. The annual pattern was more distinct in Goa with a peak in pregnancies between September and December. In Goa, decreasing daylength was associated with a higher probability of pregnancy (p = 0.040). Decreasing temperature was associated with decreasing probability of pregnancy in the Nilgiris (p = 0.034). Bitches had a median of 6 foetuses, with no evidence of seasonal variation. Environmental factors were associated with patterns of pregnancy in free-roaming dogs, however statistically-significant factors varied by geographical location. Establishing local seasonal patterns of breeding in free-roaming dogs and assessing their relationship with environmental influences is recommended to facilitate effective and efficient population management strategies, which aim to reduce conflict between human and free-roaming dog populations.

Hematuria evaluation remains a common problem, particularly in patients who smoke and are at risk for urothelial tumors. Lifetime surveillance of the urothelium is often required once urothelial cancer is diagnosed. Computed tomography urography (CTU) has exquisite sensitivity and specificity for identification of renal and urothelial lesions. The examination is well accepted by patients and physicians. Possible harms include radiation exposure and contrast-induced nephropathy. MR imaging is also an accurate test, but requires longer exam times, and may not demonstrate stones. We present the technical and interpretation skills required to use MR urography and CTU effectively.

The Drummond Basin of central Queensland preserves a large-volume succession of little studied, predominantly fluviatile, coarse-grained sedimentary rocks of mid-Mississippian age. The stratigraphy of the basin has been subdivided into three sedimentary cycles. The Cycle 1/Cycle 2 boundary records a distinct, but poorly understood change in provenance from a volcanic-dominated succession related to initial basin rifting (Cycle 1) to a quartz-rich, craton-derived succession (Cycle 2). Cycle 3 has been thought to mark a resumption of intrabasinal volcanism and related sedimentation. The purpose of this study was to enhance the understanding of the basin-wide siliciclastic sedimentation of Cycles 2 and 3, and causes for the changes in sediment provenance. This objective was achieved by constraining large-scale spatial and temporal depositional trends and investigating sediment transport pathways into and through the basin. Petrographic, QFL, paleocurrent and conglomerate clast analyses were undertaken. The observations presented here have several implications relevant to understanding the stratigraphy of the Drummond Basin and regional tectonic events at this time. Cycle 3 is revised here primarily to be a continuation of Cycle 2-style basement-derived sedimentation, rather than recording a resumption of volcanism in the area, as per prevailing models. Quartz-rich sedimentation in the Drummond Basin was, therefore, more long-lived than previously envisaged, and once established, was not significantly disrupted by volcanism. Cycle 2 formation thicknesses appear highly variable across the basin. This is unlikely to be a result of pre-existing rift-related topography as suggested in previous models. The thickness variations are more likely related to sediment bypassing and post-depositional deformation in the area. The distinctive coarse-grained, relatively quartz-rich sedimentation of Cycles 2 and 3 is unusual in its volume and extent. The sediment was transported into the basin from its southern/southwestern margin, implying long-distance transport and extrabasinal sediment supply. While the specific source terrain(s) remain unknown, one plausible tectonic driver was far-field influence of the intraplate Alice Springs Orogeny.

INTRODUCTION Liver segmentation and volumetry have traditionally been performed using computed tomography (CT) attenuation to discriminate liver from other tissues. In this project, we evaluated if spectral detector CT (SDCT) can improve liver segmentation over conventional CT on 2 segmentation methods. MATERIALS AND METHODS In this Health Insurance Portability and Accountability Act-compliant institutional review board-approved retrospective study, 30 contrast-enhanced SDCT scans with healthy livers were selected. The first segmentation method is based on Gaussian mixture models of the SDCT data. The second method is a convolutional neural network-based technique called U-Net. Both methods were compared against equivalent algorithms, which used conventional CT attenuation, with hand segmentation as the reference standard. Agreement to the reference standard was assessed using Dice similarity coefficient. RESULTS Dice similarity coefficients to the reference standard are 0.93 ± 0.02 for the Gaussian mixture model method and 0.90 ± 0.04 for the CNN-based method (all 2 methods applied on SDCT). These were significantly higher compared with equivalent algorithms applied on conventional CT, with Dice coefficients of 0.90 ± 0.06 (P = 0.007) and 0.86 ± 0.06 (P < 0.001), respectively. CONCLUSION On both liver segmentation methods tested, we demonstrated higher segmentation performance when the algorithms are applied on SDCT data compared with equivalent algorithms applied on conventional CT data.

Determining the clinical impact of imaging exams at the enterprise level is problematic, as radiology reports historically have been created with the content meant primarily for the referring provider. Structured reporting can establish the foundation for enterprise monitoring of imaging outcomes without manual review providing the framework for assessment of utilization and quality. Ultrasound (US) for deep vein thrombosis evaluation (DVT) is an ideal testbed for assessing this functionality. The system standard template for Doppler US for extremity venous evaluation for DVT was updated with a discrete fixed picklist of impression options and implemented system wide. Template utilization and interpretive outcomes were actively monitored and use reinforced as part of standard clinical practice. From January 1, 2017 to December 31, 2017, 9111 US exams for DVT were performed with 8997 utilizing structured reporting (98.75%). Of those in the structured reporting group, 1074 (11.79%) were positive for any type of DVT with 732 (8.03%) reported as Acute/New above the knee. Positive rates for any type of DVT were 10.29% emergency department, 14.17% inpatient, and 13.20% outpatient. While being the lowest positive rate, the emergency department had the highest overall volume of exams. Structured reporting for DVT US assessment outcomes can be implemented with a very high rate of radiologist adoption and adherence providing accurate determination of positive rates, month by month, in differing patient locations. Structured elements can be used to automatically trigger downstream processes; in our institution, this will alert providers in the EHR if the patient does not receive anticoagulation within 2 h of a positive test. This lays the foundation for effective enterprise assessment of imaging outcomes forming the basis of future quality and safety initiatives on optimizing health system resource utilization.
