1. Direct measurements of low-energy resonance strengths of the 23Na(p,γ)24Mg reaction for astrophysics

Abstract: The NeNa and the MgAl cycles play a fundamental role in the nucleosynthesis of asymptotic giant branch stars undergoing hot bottom burning. The Na23(p,γ)24Mg reaction links these two cycles and a precise determination of its rate is required to correctly estimate the contribution of these stars to the chemical evolution of various isotopes of Na, Mg and Al. At temperatures of 50≲T≲110MK, narrow resonances at Ep=140 and 251keV are the main contributors to the reaction rate, in addition to the direct capture that dominates in the lower part of the temperature range. We present new measurements of the strengths of these resonances at the Laboratory for Underground Nuclear Astrophysics (LUNA). We have used two complementary detection approaches: high efficiency with a 4π BGO detector for the 140keV resonance, and high resolution with a HPGe detector for the 251keV resonance. Thanks to the reduced cosmic ray background of LUNA, we were able to determine the resonance strength of the 251keV resonance as ωγ=482(82)μeV and observed new gamma ray transitions for the decay of the corresponding state in Mg24 at Ex=11931keV. With the highly efficient BGO detector, we observed a signal for the 140keV resonance for the first time in a direct measurement, resulting in a strength of ωγ140=1.46−0.53+0.58neV (68% CL). Our measurement reduces the uncertainty of the Na23(p,γ)24Mg reaction rate in the temperature range from 0.05 to 0.1GK to at most −35%+50% at 0.07GK. Accordingly, our results imply a significant reduction of the uncertainties in the nucleosynthesis calculations. Abstract: The 18O(p,α)15N reaction affects the synthesis of 15N, 18O and 19F isotopes, whose abundances can be used to probe the nucleosynthesis and mixing processes occurring deep inside asymptotic giant branch (AGB) stars. We performed a low-background direct measurement of the 18O(p,α)15N reaction cross-section at the Laboratory for Underground Nuclear Astrophysics (LUNA) from center of mass energy Ec.m.=340 keV down to Ec.m.=55 keV, the lowest energy measured to date corresponding to a cross-section of less than 1 picobarn/sr. The strength of a key resonance at center of mass energy Er=90 keV was found to be a factor of 10 higher than previously reported. A multi-channel R-matrix analysis of our and other data available in the literature was performed. Over a wide temperature range, T=0.01–1.00 GK, our new astrophysical rate is both more accurate and precise than recent evaluations. Stronger constraints can now be placed on the physical processes controlling nucleosynthesis in AGB stars with interesting consequences on the abundance of 18O in these stars and in stardust grains, specifically on the production sites of oxygen-rich Group II grains. Abstract: Light elements offer a unique opportunity for studying several astrophysical scenarios from Big Bang Nucleosynthesis to stellar physics. Understanding the stellar abundances of light elements is key to obtaining information on internal stellar structures and mixing phenomena in different evolutionary phases, such as the pre-main-sequence, main-sequence or red-giant branch. In such a case, light elements, i.e., lithium, beryllium and boron, are usually burnt at temperatures of the order of 2–5 × 106 K. Consequently, the astrophysical S(E)-factor and the reaction rate of the nuclear reactions responsible for the burning of such elements must be measured and evaluated at ultra-low energies (between 0 and 10 keV). The Trojan Horse Method (THM) is an experimental technique that allows us to perform this kind of measurements avoiding uncertainties due to the extrapolation and electron screening effects on direct data. A long Trojan Horse Method research program has been devoted to the measurement of light element burning cross sections at astrophysical energies. In addition, dedicated direct measurements have been performed using both in-beam spectroscopy and the activation technique. In this review we will report the details of these experimental measurements and the results in terms of S(E)-factor, reaction rate and electron screening potential. A comparison between astrophysical reaction rates evaluated here and the literature will also be given.

Abstract: The observation of oxygen isotopes in giant stars sheds light on mixing processes operating in their interiors. Due to the very strong correlation between nuclear burning and mixing processes it is very important to reduce the uncertainty on the cross sections of the nuclear reactions that are involved. In this paper we focus our attention on the reaction O18(p,γ)19F. While the O18(p,α)15N channel is thought to be dominant, the (p,γ) channel can still be an important component in stellar burning in giants, depending on the low energy cross section. So far only extrapolations from higher-energy measurements exist and recent estimates vary by orders of magnitude. These large uncertainties call for an experimental reinvestigation of this reaction. We present a direct measurement of the O18(p,γ)19F cross section using a high-efficiency 4π BGO summing detector at the Laboratory for Underground Nuclear Astrophysics (LUNA). The reaction cross section has been directly determined for the first time from 140 keV down to 85 keV and the different cross section components have been obtained individually. The previously highly uncertain strength of the 90 keV resonance was found to be 0.53 ± 0.07 neV, three orders of magnitude lower than an indirect estimate based on nuclear properties of the resonant state and a factor of 20 lower than a recently established upper limit, excluding the possibility that the 90 keV resonance can contribute significantly to the stellar reaction rate. Abstract: Abstract Background Low-dose spiral computed tomography (LDCT) may not lead to a clear treatment path when small to intermediate-sized lung nodules are identified. We have combined flow cytometry and machine learning to develop a sputum-based test (CyPath Lung) that can assist physicians in decision-making in such cases. Methods Single cell suspensions prepared from induced sputum samples collected over three consecutive days were labeled with a viability dye to exclude dead cells, antibodies to distinguish cell types, and a porphyrin to label cancer-associated cells. The labeled cell suspension was run on a flow cytometer and the data collected. An analysis pipeline combining automated flow cytometry data processing with machine learning was developed to distinguish cancer from non-cancer samples from 150 patients at high risk of whom 28 had lung cancer. Flow data and patient features were evaluated to identify predictors of lung cancer. Random training and test sets were chosen to evaluate predictive variables iteratively until a robust model was identified. The final model was tested on a second, independent group of 32 samples, including six samples from patients diagnosed with lung cancer. Results Automated analysis combined with machine learning resulted in a predictive model that achieved an area under the ROC curve (AUC) of 0.89 (95% CI 0.83–0.89). The sensitivity and specificity were 82% and 88%, respectively, and the negative and positive predictive values 96% and 61%, respectively. Abstract: Studies of charged-particle reactions for low-energy nuclear astrophysics require high sensitivity, which can be achieved by means of detection setups with high efficiency and low backgrounds, to obtain precise measurements in the energy region of interest for stellar scenarios. High-efficiency total absorption spectroscopy is an established and powerful tool for studying radiative capture reactions, particularly if combined with the cosmic background reduction by several orders of magnitude obtained at the Laboratory for Underground Nuclear Astrophysics (LUNA). We present recent improvements in the detection setup with the Bismuth Germanium Oxide (BGO) detector at LUNA, aiming to reduce high-energy backgrounds and increase the summing detection efficiency. The new design results in enhanced sensitivity of the BGO setup, as we demonstrate and discuss in the context of the first direct measurement of the 65 keV resonance (E x = 5672 keV) of the 17O(p,γ)18F reaction. Moreover, we show two applications of the BGO detector, which exploit its segmentation. In the case of complex γ-ray cascades, e.g. the de-excitation of E x = 5672 keV in 18F, the BGO segmentation allows to identify and suppress the beam-induced background signals that mimic the sum peak of interest. We demonstrate another new application for such a detector in form of in situ activation measurements of a reaction with β + unstable product nuclei, e.g. the 14N(p,γ)15O reaction.

Abstract: Low dose computed tomography (LDCT) is the standard of care for lung cancer screening in the United States (US). LDCT has a sensitivity of 93.8% but its specificity of 73.4% leads to potentially harmful follow-up procedures in patients without lung cancer. Thus, there is a need for additional assays with high accuracy that can be used as an adjunct to LDCT to diagnose lung cancer. Sputum is a biological fluid that can be obtained non-invasively and can be dissociated to release its cellular contents, providing a snapshot of the lung environment. We obtained sputum from current and former smokers with a 30+ pack-year smoking history and who were either confirmed to have lung cancer or at high risk of developing the disease. Dissociated sputum cells were counted, viability determined, and labeled with a panel of markers to separate leukocytes from non-leukocytes. After excluding debris and dead cells, including squamous epithelial cells, we identified reproducible population signatures and confirmed the samples’ lung origin. In addition to leukocyte and epithelial-specific fluorescent antibodies, we used the highly fluorescent meso-tetra(4-carboxyphenyl) porphyrin (TCPP), known to preferentially stain cancer (associated) cells. We looked for differences in cell characteristics, population size and fluorescence intensity that could be useful in distinguishing cancer samples from high-risk samples. We present our data demonstrating the feasibility of a flow cytometry platform to analyze sputum in a high-throughput and standardized matter for the diagnosis of lung cancer.

9. Fusion of 12C + 24Mg at extreme sub-barrier energies

Abstract: A recent experiment on 12C + 24Mg sub-barrier fusion was performed. Standard coupled-channels calculations clearly over-estimated the low energy cross sections and there was indication of a maximum of the astrophysical S factor vs energy. In the present work further measurements have been performed on the same system, and we extended the excitation function to lower energies down to σ ≃ 4 μb, confirming the existence of the hindrance phenomenon. Additional energy points near the barrier were added as well. In agreement with the result of previous experiment, the hindrance energy threshold is observed to correspond to a remarkably large cross section. The present new data allow to better define the S-factor maximum, that is well fitted using two parametrizations of either the hindrance or the adiabatic model. We also performed coupled-channels calculations using a Woods–Saxon potential. Far below the barrier we observe that the coupling strengths decrease and tend to vanish, indeed one-dimensional tunnelling calculations well reproduce the excitation function. Further precise cross section measurements at slightly lower energies are needed to discriminate between the adiabatic and hindrance models. This would give a reliable guidance for the extrapolation to the systems of astrophysical interest, like 12C + 12C and 16O + 16O.

Abstract: Direct measurements of reaction cross-sections at astrophysical energies often require the use of solid targets able to withstand high ion beam currents for extended periods of time. Thus, monitoring target thickness, isotopic composition, and target stoichiometry during data taking is critical to account for possible target modifications and to reduce uncertainties in the final cross-section results. A common technique used for these purposes is the Nuclear Resonant Reaction Analysis (NRRA), which however requires that a narrow resonance be available inside the dynamic range of the accelerator used. In cases when this is not possible, as for example the $$^{13}\hbox {C}(\alpha ,\hbox {n})^{16}\hbox {O}$$ reaction recently studied at low energies at the Laboratory for Underground Nuclear Astrophysics (LUNA) in Italy, alternative approaches must be found. Here, we present a new application of the shape analysis of primary $$\gamma $$ rays emitted by the $$^{13}\hbox {C}(\hbox {p},\gamma )^{14}\hbox {N}$$ radiative capture reaction. This approach was used to monitor $$^{13}\hbox {C}$$ target degradation in situ during the $$^{13}\hbox {C}(\alpha ,\hbox {n})^{16}\hbox {O}$$ data taking campaign. The results obtained are in agreement with evaluations subsequently performed at Atomki (Hungary) using the NRRA method. A preliminary application for the extraction of the $$^{13}\hbox {C}(\alpha ,\hbox {n})^{16}\hbox {O}$$ reaction cross-section at one beam energy is also reported.

Abstract: Introduction: Many health systems lack internal smoking cessation programs despite the need to personalize smoking cessation support for patients. Though smoking cessation services are important, systems often lack needed resources to create programs from scratch. Mount Sinai Hospital System implemented a smoking cessation program by cross training CDEs as smoking cessation health coaches since they were embedded across the network and experts in health behavior change. Methods: The entire CDE team was trained via WebEx lectures and through shadowing a tobacco treatment specialist pulmonologist in her practice. Patient and HCP engagement materials were developed as well as EHR enablement and policies/procedures to support the new program. The initial, target population was smokers with upcoming surgery since they were a discrete population and likely motivated to quit smoking to improve surgical outcomes. CDEs created personalized smoking cessation plans with patients and obtained medications through collaborative discussions with the patients’ primary care physician. Results: Eight CDEs participated. The CDEs report increased job satisfaction by operating at the top of their licenses and learning a new therapeutic area. Patient and provider feedback has been strongly positive, and the program is expanding. Conclusion: CDEs may serve as an internal resource to expand smoking cessation services within a health system. Abstract: The ^{22}Ne(p,γ)^{23}Na reaction, part of the neon-sodium cycle of hydrogen burning, may explain the observed anticorrelation between sodium and oxygen abundances in globular cluster stars. Its rate is controlled by a number of low-energy resonances and a slowly varying nonresonant component. Three new resonances at E_{p}=156.2, 189.5, and 259.7 keV have recently been observed and confirmed. However, significant uncertainty on the reaction rate remains due to the nonresonant process and to two suggested resonances at E_{p}=71 and 105 keV. Here, new ^{22}Ne(p,γ)^{23}Na data with high statistics and low background are reported. Stringent upper limits of 6×10^{-11} and 7×10^{-11} eV (90% confidence level), respectively, are placed on the two suggested resonances. In addition, the off-resonant S factor has been measured at unprecedented low energy, constraining the contributions from a subthreshold resonance and the direct capture process. As a result, at a temperature of 0.1 GK the error bar of the ^{22}Ne(p,γ)^{23}Na rate is now reduced by 3 orders of magnitude.

Abstract: Background: The fusion reaction 12C+30Si is a link between heavier cases studied in recent years, and the light heavy-ion systems, e.g., 12C+12C, 16O+16O that have a prominent role in the dynamics of stellar evolution. 12C+30Si fusion itself is not a relevant process for astrophysics, but it is important to establish its behavior below the barrier, where couplings to low-lying collective modes and the hindrance phenomenon may determine the cross sections. The excitation function is presently completely unknown below the barrier for the 12C+30Si reaction, thus no reliable extrapolation into the astrophysical regime for the C+C and O+O cases can be performed. Purpose: Our aim was to carry out a complete measurement of the fusion excitation function of 12C+30Si from well below to above the Coulomb barrier, so as to clear up the consequence of couplings to low-lying states of 30Si, and whether the hindrance effect appears in this relatively light system which has a positive Q value for fusion. This would have consequences for the extrapolated behavior to even lighter systems. Methods: The inverse kinematics was used by sending 30Si beams delivered from the XTU Tandem accelerator of INFN-Laboratori Nazionali di Legnaro onto thin 12C (50μg/cm2) targets enriched to 99.9% in mass 12. The fusion evaporation residues (ER) were detected at very forward angles, following beam separation by means of an electrostatic deflector. Angular distributions of ER were measured at Ebeam=45, 59, and 80 MeV, and they were angle integrated to derive total fusion cross sections. Results: The fusion excitation function of 12C+30Si was measured with high statistical accuracy, covering more than five orders of magnitude down to a lowest cross section ≃3μb. The logarithmic slope and the S factor have been extracted and we have convincing phenomenological evidence of the hindrance effect. These results have been compared with the calculations performed within the model that considers a damping of the coupling strength well inside the Coulomb barrier. Conclusions: The experimental data are consistent with the coupled-channels calculations. A better fit is obtained by using the Yukawa-plus-exponential potential and a damping of the coupling strengths inside the barrier. The degree of hindrance is much smaller than the one in heavier systems. Also a phenomenological estimate reproduces quite closely the hindrance threshold for 12C+30Si, so that an extrapolation to the C+C and O+O cases can be reliably performed.

Abstract: Direct measurements of small nuclear reaction cross sections require a low background in the signal region of interest to achieve the necessary sensitivity. We describe two complementary detector setups that have been used for studies of $({\rm{p}},\gamma )$ reactions with solid targets at the Laboratory for Underground Nuclear Astrophysics (LUNA): a high-purity germanium detector and a bismuth germanate (BGO) detector. We present the effect of a customised lead shielding on the measured background spectra in the two detector setups at LUNA. We developed a model to describe the contributions of environmental and intrinsic backgrounds in the BGO detector measurements. Furthermore we present an upgrade of the data acquisition system for our BGO detector, which allows us to exploit the features of the segmented detector and overcome some of the limitations encountered in previous experiments. We conclude with a discussion on the improved sensitivity of the presented setups, and the benefits for ongoing and possible future measurements.

Abstract: The experimental study of nuclear reactions of astrophysical interest is greatly facilitated by a low-background, high-luminosity setup. The Laboratory for Underground Nuclear Astrophysics (LUNA) 400 kV accelerator offers ultra-low cosmic-ray induced background due to its location deep underground in the Gran Sasso National Laboratory (INFN-LNGS), Italy, and high intensity, 250-500 $\mu$A, proton and $\alpha$ ion beams. In order to fully exploit these features, a high-purity, recirculating gas target system for isotopically enriched gases is coupled to a high-efficiency, six-fold optically segmented bismuth germanate (BGO) $\gamma$-ray detector. The beam intensity is measured with a beam calorimeter with constant temperature gradient. Pressure and temperature measurements have been carried out at several positions along the beam path, and the resultant gas density profile has been determined. Calibrated $\gamma$-intensity standards and the well-known $E_p$ = 278 keV $\mathrm{^{14}N(p,\gamma)^{15}O}$ resonance were used to determine the $\gamma$-ray detection efficiency and to validate the simulation of the target and detector setup. As an example, the recently measured resonance at $E_p$ = 189.5 keV in the $^{22}$Ne(p,$\gamma$)$^{23}$Na reaction has been investigated with high statistics, and the $\gamma$-decay branching ratios of the resonance have been determined., Comment: 11 pages, 11 figures, accepted in Eur. Abstract: We investigate the impact of the new LUNA rate for the nuclear reaction Ne-22(p, gamma) Na-23 on the chemical ejecta of intermediate-mass stars, with particular focus on the thermally pulsing asymptotic giant branch (TP-AGB) stars that experience hot-bottom burning. To this aim, we use the PARSEC and COLIBRI codes to compute the complete evolution, from the premain sequence up to the termination of the TP-AGB phase, of a set of stellar models with initial masses in the range 3.0-6.0M(circle dot) and metallicities Z(i) = 0.0005, 0.006 and 0.014. We find that the new LUNA measures have much reduced the nuclear uncertainties of the Ne-22 and Na-23 AGB ejecta that drop from factors of similar or equal to 10 to only a factor of few for the lowest metallicity models. Relying on the most recent estimations for the destruction rate of Na-23, the uncertainties that still affect the Ne-22 and Na-23 AGB ejecta are mainly dominated by the evolutionary aspects (efficiency of mass-loss, third dredge-up, convection). Finally, we discuss how the LUNA results impact on the hypothesis that invokes massive AGB stars as the main agents of the observed O-Na anticorrelation in Galactic globular clusters. We derive quantitative indications on the efficiencies of key physical processes (mass-loss, third dredgeup, sodium destruction) in order to simultaneously reproduce both the Na-rich, O-poor extreme of the anticorrelation and the observational constraints on the CNO abundance. Results for the corresponding chemical ejecta are made publicly available.

Abstract: The authors perform a direct measurement of radiative proton capture on ${}^{22}$Ne at the LUNA underground laboratory whose low background allows unprecedented sensitivity. They report detailed experimental information for three recently observed new resonances in the ${}^{22}$Ne($p$,$\ensuremath{\gamma}$)${}^{23}$Na reaction which is the most uncertain process in the neon-sodium cycle of stellar hydrogen burning. At temperatures relevant for nucleosynthesis in asymptotic giant branch stars and classical novae, its uncertainty is mainly due to a large number of predicted but hitherto unobserved resonances at low energy. The updated thermonuclear reaction rate is significantly higher than previous evaluations at temperatures below 0.3 GK.

Abstract: The nucleosynthesis of light elements, from helium up to silicon, mainly occurs in Red Giant and Asymptotic Giant Branch stars and Novae. The relative abundances of the synthesized nuclides critically depend on the rates of the nuclear processes involved, often through non-trivial reaction chains, combined with complex mixing mechanisms. In this review, we summarize the contributions made by LUNA experiments in furthering our understanding of nuclear reaction rates necessary for modeling nucleosynthesis in AGB stars and Novae explosions.

Abstract: Introduction: Weight loss amongst women with polycystic ovary syndrome (PCOS) is crucial to reduce the risk of endocrine, reproductive and metabolic complications including hirsutism, menstrual disturbances and cardiovascular disease. With approximately 50% of women with PCOS being overweight or obese, effective dietary management of weight in PCOS is essential. However, there is inconsistent evidence as to whether specifically modified diets (e.g. reduced carbohydrate diets) are more effective at achieving weight loss amongst women with PCOS than are conventional healthy hypocaloric diets. Material and Methods: A systematic review and meta-analysis of randomized controlled trials that had compared weight and BMI between women with PCOS who had undergone either a specifically modified diet or a conventional healthy hypocaloric diet were performed. Six electronic databases were searched, a manual search of the reference lists of the included studies was carried out and authors were contacted for additional information. Nine studies with a total of 395 participants (all with a body mass index [BMI] ≥30) were included in the meta-analysis. The effect size used was the mean difference in post-intervention weight and BMI between participants who had undergone a specifically modified diet and participants who had undergone a conventional healthy hypocaloric diet. Results: There were no differences between groups in post-intervention weight (mean difference 1.26, 95% confidence interval (CI) -0.92 to 3.43, p = .26; heterogeneity I2 = 50%, p = .04) or BMI (mean difference 0.15, 95% CI -0.93 to 1.23, p = 0.79; heterogeneity I2 = 44%, p = 0.10). Subgroup analyses according to the presence of a dietary run-in period (a period at the start of the study during which all participants are placed on an identical diet in order to equalize them on variables influenced by diet), intervention duration and type of diet and a sensitivity analysis according to study quality were not significant. Conclusions: Whilst the results should be interpreted in light of the moderate heterogeneity observed, they suggest that specifically modified diets offer no added benefit for weight loss in women with PCOS over conventional healthy hypocaloric diets. The findings of this meta-analysis may promote the unification of guidelines for the dietary management of PCOS and allow clinicians to be confident in prescribing conventional healthy hypocaloric diets for weight loss amongst their PCOS patients.

28. Towards a study of22Ne(pγ)23Na at LUNA

Abstract: The 22Ne(p,γ)23Na reaction is involved in the NeNa cycle of hydrogen burning. This cycle plays an important role for nucleosynthesis in the Red Giant Branch and Asymptotic Giant Branch phases of stellar evolution, as well as in classical novae and type Ia supernovae explosions.The 22Ne(p,γ)23Na reaction rate is highly uncertain because of a large number of resonances lying in the energy region of the Gamow peak. Several of these resonances have never been studied in either direct or indirect experiments, and only upper limits exist for their strengths. A measurement of the 2Ne(p,γ)23Na cross section is on-going at the Laboratory for Underground Nuclear Astrophysics (LUNA) in Gran Sasso. With the LUNA setup, it will be possible to study the 22Ne+p reaction inside the Gamow window.The results of a feasibility test, as well as the measurement strategy and the setup for the first experimental campaign are discussed

Abstract: WOS: 000300864200005, The Advanced GAmma Tracking Array (AGATA) is a European project to develop and operate the next generation gamma-ray spectrometer. AGATA is based on the technique of gamma-ray energy tracking in electrically segmented high-purity germanium crystals. This technique requires the accurate determination of the energy, time and position of every interaction as a gamma ray deposits its energy within the detector volume. Reconstruction of the full interaction path results in a detector with very high efficiency and excellent spectral response. The realisation of gamma-ray tracking and AGATA is a result of many technical advances. These include the development of encapsulated highly segmented germanium detectors assembled in a triple cluster detector cryostat, an electronics system with fast digital sampling and a data acquisition system to process the data at a high rate. The full characterisation of the crystals was measured and compared with detector-response simulations. This enabled pulse-shape analysis algorithms, to extract energy, time and position, to be employed. In addition, tracking algorithms for event reconstruction were developed. The first phase of AGATA is now complete and operational in its first physics campaign. In the future AGATA will be moved between laboratories in Europe and operated in a series of campaigns to take advantage of the different beams and facilities available to maximise its science output. Abstract: The 22Ne(p,γ)23Na reaction is involved in the hydrogen burning neon-sodium (NeNa) cycle. This cycle is active in asymptotic giant branch stars (AGB) as well as in novae. In the Gamow window temperature range (30MK < T < 420 MK corresponding to proton center of mass energies 40 keV < E < 430 keV), the 22Ne(p,γ)23Na reaction rate is highly uncertain because of the contribution of a large number of resonances never measured directly. A study of this reaction has been carried out at the Laboratory for Underground Nuclear Astrophysics (LUNA), in the Gran Sasso National Laboratory, using a windowless gas target and two high-purity germanium detectors. Several resonances have been observed for the first time in a direct experiment.

Abstract: Background: The clinical value of computed tomographic (CT) scanning of the chest in the initial assessment of sarcoidosis was investigated. Methods: One hundred consecutive patients referred to the sarcoidosis outpatient services of the Mount Sinai Medical Center, New York from 1990 to 1992 with a presumptive diagnosis of sarcoidosis were studied. The diagnosis was subsequently confirmed in all by a positive tissue biopsy sample or the Kveim-Siltzbach test. Clinical and laboratory data of each patient were reviewed. Chest radiographs were classified according to the classical stages of sarcoidosis. Thirty five of the 100 patients had a CT scan of the chest performed before presentation. The CT scans were compared with the presenting clinical data and standard chest radiographs in order to determine if they yielded useful additional information regarding diagnosis or treatment. Results: The chest CT scan revealed no additional clinically relevant information compared with conventional chest radiographs in any of the 35 studies performed. In two patients mediastinal adenopathy was detected by CT scan which was not seen on standard radiographs. Two patients thought to exhibit hilar adenopathy and pulmonary infiltrations by standard radiography had no parenchymal disease on the CT scan. Bilateral parenchymal infiltrates were seen in one patient which were interpreted as unilateral infiltrates by standard radiographs. The variance between conventional radiographs and CT scans in these five patients was not clinically valuable. Conclusions: CT scans of the chest do not add clinically useful information to the standard chest radiographs in the initial assessment of sarcoidosis in patients presenting with the typical standard radiological patterns. CT scanning of the thorax is indicated in patients with proven or suspected sarcoidosis when the standard chest radiographs are normal or not typical of sarcoidosis, when signs or symptoms of upper airway obstruction are present, when the patient has haemoptysis, if there is a suspicion of a complicating second intrathoracic disease, or the patient is a candidate for lung transplantation.

Abstract: We report a patient with tissue-proven sarcoidosis receiving adrenocorticosteroid medication, who developed an enlarging mediastinal mass. Transcutaneous needle biopsy of the mass yielded pus which grew Nocardia asteroides on culture. Pleural effusion, bronchoesophageal fistula and brain nocardia metastases occurred. All evidence of active infection cleared with sulfa therapy. An enlarging mass in a patient with sarcoidosis unresponsive to corticosteroid therapy should provoke studies for other causes of mediastinal disease, including opportunistic infections.

34. Fatal dyspnea in a patient with renal failure

Abstract: A 67-year-old woman with hypertension, diabetes, hypothyroidism and chronic renal failure reported to the hospital for her regularly scheduled hemodialysis, complaining of shortness of breath. Despite fluid removal during her hemodialysis, she remained tachypneic and developed stridor. She was admitted to the hospital for a work-up of a known mediastinal mass, thought to be a goiter. However, she deteriorated over the next several hours and expired. A post-mortem examination confirmed fatal pulmonary emboli. This case illustrates the fact that, while most patients with chronic renal failure are considered to be at low risk for pulmonary emboli, it is often not diagnosed when present. In this paper, we will review the epidemiological data supporting this notion, examine proposed pathophysiological mechanisms, and review the diagnostic approach that should be considered in the setting of chronic renal failure.

Abstract: A case of a 79-year-old female admitted to the hospital with a hip fracture and suffering a near-fatal embolism, is presented. The article then reviews the epidemiology of deep venous thrombosis and pulmonary embolism in the setting of hip fracture and orthopedic surgery and outlines the diagnostic approach to a critically ill patient with pulmonary embolism. The use of thrombolysis as an adjunct to usual heparin therapy is examined, as are the potential benefits versus the absolute risks of thrombolysis. Finally, practical recommendations outlining a reasonable approach to this group of patients, based on available evidence, are proposed.

Abstract: We report the development of a nontransformed line of human airway smooth muscle cells retaining smooth muscle-specific contractile protein expression and physiological responsiveness to agonists implicated in inflammatory airway diseases. Specific responses to histamine, leukotrienes, bradykinin, platelet-activating factor, substance P, and thromboxane analogues are demonstrated as well as functional coupling to beta-adrenergic receptors. The cell line was characterized using indirect immunofluorescence, as well as electrophoretic separation and immunoblot analysis of smooth muscle-specific actin. Functional responses were assessed by measurements of cytosolic calcium and stimulation of adenosine 3',5'-cyclic monophosphate production. The cells retain their responsiveness over many population doublings and should be a useful model to examine specific receptor-effector mechanisms, as well as the effects of neurohumoral agents on the regulation of airway smooth muscle growth and differentiation.

Abstract: Studying interactions of radioactive ions with neutrons is particularly demanding and has been performed only in a few cases. Some of these interactions are crucial in several astrophysical contexts. In the present work, the case of the 7Be destruction induced by the (n, α) reaction is investigated at the energies typical of the primordial nucleosynthesis by means of the Trojan Horse Method applied to the 2H(7Be, αα)p quasi-free reaction. The 7Be(n, α)4He cross-section has been measured in a single experiment from ∼2 MeV down to cosmological energies. Abstract: In stars, the fusion of $^{22}$Ne and $^4$He may produce either $^{25}$Mg, with the emission of a neutron, or $^{26}$Mg and a $\gamma$ ray. At high temperature, the ($\alpha,n$) channel dominates, while at low temperature, it is energetically hampered. The rate of its competitor, the $^{22}$Ne($\alpha$,$\gamma$)$^{26}$Mg reaction, and, hence, the minimum temperature for the ($\alpha,n$) dominance, are controlled by many nuclear resonances. The strengths of these resonances have hitherto been studied only indirectly. The present work aims to directly measure the total strength of the resonance at $E$_{r}$\,=\,$334$\,$keV (corresponding to $E$_{x}$\,=\,$10949$\,$keV in $^{26}$Mg). The data reported here have been obtained using high intensity $^4$He$^+$ beam from the INFN LUNA 400 kV underground accelerator, a windowless, recirculating, 99.9% isotopically enriched $^{22}$Ne gas target, and a 4$\pi$ bismuth germanate summing $\gamma$-ray detector. The ultra-low background rate of less than 0.5 counts/day was determined using 67 days of no-beam data and 7 days of $^4$He$^+$ beam on an inert argon target. The new high-sensitivity setup allowed to determine the first direct upper limit of 4.0$\,\times\,$10$^{-11}$ eV (at 90% confidence level) for the resonance strength. Abstract: Direct measurements of small nuclear reaction cross sections require a low background in the signal region of interest to achieve the necessary sensitivity. We describe two complementary detector setups that have been used for studies of reactions with solid targets at the Laboratory for Underground Nuclear Astrophysics (LUNA): a high-purity germanium detector and a bismuth germanate (BGO) detector. We present the effect of a customised lead shielding on the measured background spectra in the two detector setups at LUNA. We developed a model to describe the contributions of environmental and intrinsic backgrounds in the BGO detector measurements. Furthermore we present an upgrade of the data acquisition system for our BGO detector, which allows us to exploit the features of the segmented detector and overcome some of the limitations encountered in previous experiments. We conclude with a discussion on the improved sensitivity of the presented setups, and the benefits for ongoing and possible future measurements. [ABSTRACT FROM AUTHOR]

Abstract: Accurate knowledge of thermonuclear reaction rates is important in understanding the generation of energy, the luminosity of neutrinos, and the synthesis of elements in stars. Cross-section measurements for quiescent stellar H-burning are mainly hampered by extremely low counting rate and cosmic background. The LUNA Collaboration has shown that, by going underground and by using the typical techniques of low background physics, it is possible to measure nuclear cross-sections down to the energy of the nucleosynthesis inside stars. This paper reports an overview of the experimental techniques adopted in underground nuclear astrophysics through a summary of the main recent results and achievements. Abstract: The direct measurement of reaction cross sections at astrophysical energies often requires the use of solid targets of known thickness, isotopic composition, and stoichiometry that are able to withstand high beam currents for extended periods of time. Here, we report on the production and characterisation of isotopically enriched Ta$_2$O$_5$ targets for the study of proton-induced reactions at the Laboratory for Underground Nuclear Astrophysics facility of the Laboratori Nazionali del Gran Sasso. The targets were prepared by anodisation of tantalum backings in enriched water (up to 66% in $^{17}$O and up to 96% in $^{18}$O). Special care was devoted to minimising the presence of any contaminants that could induce unwanted background reactions with the beam in the energy region of astrophysical interest. Abstract: The ^{12}C/^{13}C ratio is a significant indicator of nucleosynthesis and mixing processes during hydrogen burning in stars. Its value mainly depends on the relative rates of the ^{12}C(p,γ)^{13}N and ^{13}C(p,γ)^{14}N reactions. Both reactions have been studied at the Laboratory for Underground Nuclear Astrophysics (LUNA) in Italy down to the lowest energies to date (E_{c.m.}=60 keV) reaching for the first time the high energy tail of hydrogen burning in the shell of giant stars. Our cross sections, obtained with both prompt γ-ray detection and activation measurements, are the most precise to date with overall systematic uncertainties of 7%-8%. Compared with most of the literature, our results are systematically lower, by 25% for the ^{12}C(p,γ)^{13}N reaction and by 30% for ^{13}C(p,γ)^{14}N. We provide the most precise value up to now of 3.6±0.4 in the 20-140 MK range for the lowest possible ^{12}C/^{13}C ratio that can be produced during H burning in giant stars.

47. Optimizing the Ovarian Tissue Cryopreservation in the 'Oncofertility' Institutional Program at an Italian National Cancer Institute.

Abstract: Background: The majority of female cancer patients undergoing anticancer treatments are at risk of experiencing 'cancer treatment-related infertility', which can result in permanent damage to their reproductive prospects. Among the fertility preservation methods, ovarian tissue cryopreservation (OTC) has emerged as an alternative for these patients. The Cancer Institute of Bari initiated a research program to assess the feasibility of OTC. This study compares the viability of ovarian cortical fragments cryopreserved using slow freezing (SF) and ultra-rapid freezing (URF) methods., Methods: Ovarian cortex biopsies were obtained from 11 fertile women enrolled in our oncofertility service between June 2022 and January 2023. After tissue collection, a histological assessment was performed before cryopreservation. OTC was carried out using both SF and URF methods. Six months later, thawed samples were evaluated for follicle counts and histological integrity., Results: No statistically significant difference was observed in the proportion of intact follicles (means of 31.5% and 73.0% in the SF and URF groups, respectively; p = 0.064). However, there was a significant difference in the number of follicles between the SF group (n = 149) and the URF group (n = 37) ( p = 0.046)., Conclusions: We assessed the viability of ovarian cortex after freezing and thawing, focusing on the structural integrity of follicles. Our findings suggest that there are no significant differences between the SF and URF methods.

Abstract: One of the main neutron sources for the astrophysical s process is the reaction ^{13}C(α,n)^{16}O, taking place in thermally pulsing asymptotic giant branch stars at temperatures around 90 MK. To model the nucleosynthesis during this process the reaction cross section needs to be known in the 150-230 keV energy window (Gamow peak). At these sub-Coulomb energies, cross section direct measurements are severely affected by the low event rate, making us rely on input from indirect methods and extrapolations from higher-energy direct data. This leads to an uncertainty in the cross section at the relevant energies too high to reliably constrain the nuclear physics input to s-process calculations. We present the results from a new deep-underground measurement of ^{13}C(α,n)^{16}O, covering the energy range 230-300 keV, with drastically reduced uncertainties over previous measurements and for the first time providing data directly inside the s-process Gamow peak. Selected stellar models have been computed to estimate the impact of our revised reaction rate. For stars of nearly solar composition, we find sizeable variations of some isotopes, whose production is influenced by the activation of close-by branching points that are sensitive to the neutron density, in particular, the two radioactive nuclei ^{60}Fe and ^{205}Pb, as well as ^{152}Gd.

Abstract: We recently reported that activation of Trop-2 through its cleavage at R87-T88 by ADAM10 underlies Trop-2-driven progression of colon cancer. However, the mechanism of action and pathological impact of Trop-2 in metastatic diffusion remain unexplored. Through searches for molecular determinants of cancer metastasis, we identified TROP2 as unique in its up-regulation across independent colon cancer metastasis models. Overexpression of wild-type Trop-2 in KM12SM human colon cancer cells increased liver metastasis rates in vivo in immunosuppressed mice. Metastatic growth was further enhanced by a tail-less, activated ΔcytoTrop-2 mutant, indicating the Trop-2 tail as a pivotal inhibitory signaling element. In primary tumors and metastases, transcriptome analysis showed no down-regulation of CDH1 by transcription factors for epithelial-to-mesenchymal transition, thus suggesting that the pro-metastatic activity of Trop-2 is through alternative mechanisms. Trop-2 can tightly interact with ADAM10. Here, Trop-2 bound E-cadherin and stimulated ADAM10-mediated proteolytic cleavage of E-cadherin intracellular domain. This induced detachment of E-cadherin from β-actin, and loss of cell-cell adhesion, acquisition of invasive capability, and membrane-driven activation of β-catenin signaling, which were further enhanced by the ΔcytoTrop-2 mutant. This Trop-2/E-cadherin/β-catenin program led to anti-apoptotic signaling, increased cell migration, and enhanced cancer-cell survival. In patients with colon cancer, activation of this Trop-2-centered program led to significantly reduced relapse-free and overall survival, indicating a major impact on progression to metastatic disease. Recently, the anti-Trop-2 mAb Sacituzumab govitecan-hziy was shown to be active against metastatic breast cancer. Abstract: Purpose: Oocytes and embryos can be vitrified with and without dimethyl sulfoxide (DMSO). Objectives were to compare no vitrification (No-Vitr), vitrification with DMSO (Vitr + DMSO), and vitrification without DMSO (Vitr - DMSO) on fresh/warmed oocyte survival, induced parthenogenetic activation, parthenogenetic embryo development, and embryonic maternal imprinted gene expression., Methods: In this prospective controlled laboratory study, mature B6C3F1 female mouse metaphase II oocytes were treated as: i) No-Vitr, ii) Vitr + DMSO/warmed, and iii) Vitr - DMSO/warmed with subsequent parthenogenetic activation and culture to the blastocyst stage. Oocyte cryo-survival, parthenogenetic activation and embryo development, parthenogenetic embryo maternal imprinted gene expression were outcome measures., Results: Oocyte cryo-survival was significantly improved in Vitr + DMSO versus Vitr - DMSO at initial warming and 2 h after warming. Induced parthenogenetic activation was similar between all three intervention groups. While early preimplantation parthenogenetic embryo development was similar between control, Vitr + DMSO, Vitr - DMSO oocytes, the development to blastocysts was significantly inferior in the Vitr - DMSO oocytes group compared to the control and Vitr + DMSO oocyte groups. Finally, maternal imprinted gene expression was similar between intervention groups at both the 2-cell and blastocyst parthenogenetic embryo stage., Conclusion(s): Inclusion of DMSO in oocyte vitrification solutions improved cryo-survival and developmental potential of parthenogenetic embryos to the blastocyst stage without significantly altering maternal imprinted gene expression., (© 2021.
