Thoracoscopic bilateral T3 sympathectomy for primary focal palmar hyperhidrosis in children has excellent short-term outcomes. However, data in the literature, on the long-term outcomes of the operation are scarce.We conducted a retrospective institutional review of all children and adolescents undergoing T3 bilateral thoracoscopic sympathectomy for primary focal palmar hyperhidrosis between June 2013 and October 2020. We composed a quality of life (QoL) questionnaire evaluating the patient's perception of how much the hyperhidrosis affected their daily life in multiple domains. The questionnaire was completed before the operation and at every postoperative follow-up visit.We operated on 58 patients with a median age of 15 (6-25) years. There were no intraoperative or postoperative complications, and all patients had immediate complete postoperative resolution of their palmar hyperhidrosis. Fifty-three patients (91.4%) had long-term follow-up data available with a median of 2.5 (range 0.1-7.5) years. Two patients (3.4%) experienced recurrence of their palmar hyperhidrosis. Nine patients (15.5%) experienced compensatory hyperhidrosis and required occasional medical management with oral anticholinergics. Two patients reported regretting having undergone the operation. Overall, the mean QoL score improved remarkably, from 42/100 before the operation to 92/100 at 1 month, 89/100 at six months to a year, 97/100 between two and four years, and 80/100 ≥ 5 years after the operation.Thoracoscopic bilateral T3 sympathectomy has a high success rate for primary palmar hyperhidrosis in children in the mid- to long-term. Compensatory sweating and recurrence can occur years after the operation, so long-term follow up is mandatory.IV.

OBJECTIVE: To assess the safety of Partial-Amniotic-Insufflation-of-heated-humidified-CO2 (hPACI) during fetoscopic spina bifida repair (fSB-repair). METHOD: A simulated fSB-repair through an exteriorized uterus under hPACI was performed in 100-day fetal lambs (term = 145 days) under a laboratory anesthesia protocol (n = 5; group 1) which is known to induce maternal-fetal acidosis and hypercapnia. Since these may not occur clinically, we applied a clinical anesthesia protocol (n = 5; group 2), keeping maternal parameters within physiological conditions, that is, controlled maternal arterial carbon dioxide (CO2) pressure (pCO2 = 30 mmHg), blood pressure (≥67 mmHg), and temperature (37.1-39.8°C). Introduction: INTELLiVENT–Adaptive Support Ventilation (INTELLiVENT–ASV), an advanced closed–loop ventilation mode for use in intensive care unit (ICU) patients, is equipped with algorithms that automatically adjust settings on the basis of physiologic signals and patient’s activity. Here we describe its effectiveness, safety, and efficacy in various types of ICU patients. Areas covered: A systematic search conducted in MEDLINE, EMBASE, the Cochrane Central register of Controlled Trials (CENTRAL), and in Google Scholar identified 10 randomized clinical trials. Expert opinion: Studies suggest INTELLiVENT–ASV to be an effective automated mode with regard to the titrations of tidal volume, airway pressure, and oxygen. INTELLiVENT–ASV is as safe as conventional modes. However, thus far studies have not shown INTELLiVENT–ASV to be superior to conventional modes with regard to duration of ventilation and other patient–centered outcomes. Future studies are needed to test its efficacy.

Background INTELLiVENT–Adaptive Support Ventilation (ASV) is a fully automated closed-loop mode of ventilation for use in critically ill patients. Evidence for benefit of INTELLiVENT–ASV in comparison to ventilation that is not fully automated with regard to duration of ventilation and quality of breathing is largely lacking. We test the hypothesis that INTELLiVENT–ASV shortens time spent on a ventilator and improves the quality of breathing. Methods The “Effects of Automated Closed–loop VenTilation versus Conventional Ventilation on Duration and Quality of Ventilation” (ACTiVE) study is an international, multicenter, two-group randomized clinical superiority trial. In total, 1200 intensive care unit (ICU) patients with an anticipated duration of ventilation of > 24 h will be randomly assigned to one of the two ventilation strategies. Investigators screen patients aged 18 years or older at start of invasive ventilation in the ICU. Patients either receive automated ventilation by means of INTELLiVENT–ASV, or ventilation that is not automated by means of a conventional ventilation mode. The primary endpoint is the number of days free from ventilation and alive at day 28; secondary endpoints are quality of breathing using granular breath-by-breath analysis of ventilation parameters and variables in a time frame of 24 h early after the start of invasive ventilation, duration of ventilation in survivors, ICU and hospital length of stay (LOS), and mortality rates in the ICU and hospital, and at 28 and 90 days. Discussion ACTiVE is one of the first randomized clinical trials that is adequately powered to compare the effects of automated closed-loop ventilation versus conventional ventilation on duration of ventilation and quality of breathing in invasively ventilated critically ill patients. The results of ACTiVE will support intensivist in their choices regarding the use of automated ventilation. Trial registration ACTiVE is registered in clinicaltrials.gov (study identifier: NCT04593810) on 20 October 2020.

In view of the shortage of medical staff, the quality and continuity of care may be improved by employing advanced practice providers (APPs). This study aims to assess the quality of these APPs in critical care. In a large teaching hospital, rapid response team (RRT) interventions led by APPs were assessed by independent observers and intensivists and compared to those led by medical residents MRs. In addition to mortality, the MAELOR tool (assessment of RRT intervention), time from RRT call until arrival at the scene and time until completion of clinical investigations were assessed. Process outcomes were assessed with the crisis management skills checklist, the Ottawa global rating scale and the Mayo high-performance teamwork scale. The intensivists assessed performance with the handoff CEX recipient scale. Mortality, MAELOR tool, time until arrival and clinical investigation in both groups were the same. Process outcomes and performance observer scores were also equal. The CEX recipient scores, however, showed differences between MRs and APPs that increased with experience. Experienced APPs had significantly better situational awareness, better organization, better evaluations and better judgment than MRs with equal experience (p < 0.05). This study shows that APPs perform well in leading an RRT and may provide added quality over a resident. RRTs should seriously consider the deployment of APPs instead of junior clinicians.

Background and Objectives: Clinical decision support systems are advocated to improve the quality and efficiency in healthcare. However, before implementation, validation of these systems needs to be performed. In this evaluation we tested our hypothesis that a computerized clinical decision support system can calculate the CHA2DS2-VASc score just as well compared to manual calculation, or even better and more efficiently than manual calculation in patients with atrial rhythm disturbances. Materials and Methods: In n = 224 patents, we calculated the total CHA2DS2-VASc score manually and by an automated clinical decision support system. We compared the automated clinical decision support system with manually calculation by physicians. Results: The interclass correlation between the automated clinical decision support system and manual calculation showed was 0.859 (0.611 and 0.931 95%-CI). Bland-Altman plot and linear regression analysis shows us a bias of −0.79 with limit of agreement (95%-CI) between 1.37 and −2.95 of the mean between our 2 measurements. The Cohen’s kappa was 0.42. Retrospective analysis showed more human errors than algorithmic errors. Time it took to calculate the CHA2DS2-VASc score was 11 s per patient in the automated clinical decision support system compared to 48 s per patient with the physician. Conclusions: Our automated clinical decision support system is at least as good as manual calculation, may be more accurate and is more time efficient.

OBJECTIVES Cardiac tamponade is a life-threatening complication after cardiac surgery. Echocardiography, both transthoracic (TTE) and transesophageal (TEE), may help to identify cardiac tamponade after surgery, but its diagnostic value remains unverified after cardiac surgery. METHODS This retrospective single-centre cohort study used the electronic medical record and echocardiography database of the Catharina Hospital Eindhoven, a tertiary referral cardiothoracic centre, to identify patients who received echocardiography because they were clinically suspected of having cardiac tamponade within the 4 weeks after cardiac surgery. Overall diagnostic accuracy of both TTE and TEE was calculated (sensitivity, specificity, positive predictive value, negative predictive value, and receiver operation characteristics curves). Subgroup analyses were performed based on the timing of the echocardiography after primary surgery (72 h). RESULTS The query identified 427 echocardiographs, 373 TTEs and 54 TEEs, being performed in 414 patients (65% males, mean age 67 years). Of them, 116 patients underwent surgical re-exploration in which a cardiac tamponade was determined in 105 patients with a 30-day mortality of 8.6%. The area under the receiver operation characteristics curve for echocardiography in the 4 weeks after cardiac surgery was 0.78 [95% confidence interval (CI): 0.72–0.84, P < 0.001]. In the first 24 h after surgery was the positive predictive value of echocardiography 58.3% (95% CI: 28.6–83.5) with an area under the curve of 0.64 (95% CI: 0.49–0.80, P = 0.06). The diagnostic accuracy improved over time for both TTE and TEE. Since the early 1980's, with the clinical advent of in vitro fertilization resulting in so-called "test tube babies," a wide array of ethical considerations and concerns regarding artificial womb technology (AWT) have been described. Recent breakthroughs in the development of extracorporeal neonatal life support by means of AWT have reinitiated ethical interest about this topic with a sense of urgency. Most of the recent ethical literature on the topic, however, pertains not to the more imminent scenario of a physiologically improved method of neonatal care through AWT, but instead to the remote scenario of "complete ectogenesis" that imagines human gestation occurring entirely outside of the womb. This scoping review of the ethical literature on AWT spans from more abstract concerns about complete ectogenesis to more immediate concerns about the soon-to-be-expected clinical life support of what we term the fetal neonate or fetonate. Despite advances in clinical care and modest improvement in mortality rates for extreme prematurity, morbidity remains a significant challenge. The ideal environment to support prematurity would be fluidic and rely on natural fetal circulation to mimic the natural fetal amniotic environment, yet such an environment has been unsuccessful in long-term support until recently. Our group has succeeded in developing such a support system to foster fetal growth in the premature lamb model that shows promise for clinical translation. Here, we describe the EXTrauterine Environment for Neonatal Development (EXTEND) from its conception onwards, review published literature on fetal development and support of the premature lamb model in EXTEND, and discuss future applications.

Large numbers of asynchronies during pressure support ventilation cause discomfort and higher work of breathing in the patient, and are associated with an increased mortality. There is a need for real-time decision support to detect asynchronies and assist the clinician towards lung-protective ventilation. Machine learning techniques have been proposed to detect asynchronies, but they require large datasets with sufficient data diversity, sample size, and quality for training purposes. In this work, we propose a method for generating a large, realistic and labeled, synthetic dataset for training and validating machine learning algorithms to detect a wide variety of asynchrony types. We take a model-based approach in which we adapt a non-linear lung-airway model for use in a diverse patient group and add a first-order ventilator model to generate labeled pressure, flow, and volume waveforms of pressure support ventilation. The model was able to reproduce basic measured lung mechanics parameters. Experienced clinicians were not able to differentiate between the simulated waveforms and clinical data (P = 0.44 by Fisher’s exact test). The detection performance of the machine learning trained on clinical data gave an overall comparable true positive rate on clinical data and on simulated data (an overall true positive rate of 94.3% and positive predictive value of 93.5% on simulated data and a true positive rate of 98% and positive predictive value of 98% on clinical data). Our findings demonstrate that it is possible to generate labeled pressure and flow waveforms with different types of asynchronies.

Background Timely and consistent recognition of a ‘clinical crisis’, a life threatening condition that demands immediate intervention, is essential to reduce ‘failure to rescue’ rates in general wards. Aim To determine how different clinical caregivers define a ‘clinical crisis’ and how they respond to it. Design An international survey. Methods Clinicians working on general wards, intensive care units or emergency departments in the Netherlands, the United Kingdom and Denmark were asked to review ten scenarios based on common real-life cases. Then they were asked to grade the urgency and severity of the scenario, their degree of concern, their estimate for the risk for death and indicate their preferred action for escalation. The primary outcome was the scenarios with a National Early Warning Score (NEWS) ≥7 considered to be a ‘clinical crisis’. Secondary outcomes included how often a rapid response system (RRS) was activated, and if this was influenced by the participant’s professional role or experience. The data from all participants in all three countries was pooled for analysis. Results A total of 150 clinicians participated in the survey. The highest percentage of clinicians that considered one of the three scenarios with a NEWS ≥7 as a ‘clinical crisis’ was 52%, while a RRS was activated by Conclusion This international survey indicates that clinicians differ on what they consider to be a ‘clinical crisis’ and on how it should be managed. Even in cases with a markedly abnormal physiology (i.e. NEWS ≥7) many clinicians do not consider immediate activation of a RRS is required.

The most recent modes for mechanical ventilation are closed-loop modes, which are able to automatically adjust certain respiratory settings. Although closed-loop modes have been investigated in various clinical trials, it is unclear to what extent these modes are actually used in clinical practice. The aim of this study was to determine closed-loop ventilation practice on intensive care units (ICUs) in the Netherlands, and to explore reasons for not applying closed-loop ventilation. Our hypothesis was that closed-loop ventilation is increasingly used.A short survey was conducted among all non-paediatric ICUs in the Netherlands. Use of closed-loop modes was classified as frequently, occasionally or never, if respondents stated they had used these modes in the last week, in the last month/year, or never, respectively.The response rate of the survey was 82% (72 of 88). Respondents had access to a closed-loop ventilation mode in 58% of the ICUs (42 of 72). Of these ICUs, 43% (18 of 42) frequently applied a closed-loop ventilation mode, while 57% (24 of 42) never or occasionally used it. Reasons for not using these modes were lack of knowledge (40%), insufficient evidence reporting a beneficial effect (35%) and lack of confidence (25%).This study does not support our hypothesis that closed-loop ventilation is increasingly used in the Dutch ICU setting. While industry continues to develop new closed-loop modes, implementation of these modes in clinical practice seems to encounter difficulties. Various barriers could play a role, and these all need attention in future investigations.

Background: exercise interventions in physical therapy (pt) are often not well described in research reports. Omitting details of the intervention hampers the transfer to daily clinical practice.objective: to develop a framework for describing goals and content of exercise interventions, in order to provide structured and detailed information for use in research reports.methods: a framework was developed in three steps using a mixed method approach. First, the scientific literature was systematically searched for available methods to describe pt interventions. An inventory of these methods was made and relevant elements were extracted. Second, based on the results of the literature search, a global framework was developed by a focus group of experts in the field of pt. Third, the framework was field tested by describing four different exercise interventions, leading to a final version.results: in the developed framework intervention goals at the level of body functions and activities/participation were listed using the international classification of functioning, disability and health. To guide the description of the content of the exercise intervention a list of intervention details was developed, which was derived from an existing classification of pt interventions. Goals and content of exercise interventions were linked. Field testing of the framework led to adequate descriptions of exercise interventions.conclusion: the developed framework is a promising first step in offering a feasible guide for a structured and detailed description of goals and content of exercise interventions within research reports.

In a prospective cohort study, AFS was compared with Olerud and Molander ankle score (OMAS), patient-specific complaints (PSC), and global perceived effect. Sensitivity and specificity were calculated to estimate prognostic validity; correlation was calculated for concurrent validity. Effect Size, Standardized Response Mean, and Responsiveness Ratio were calculated to estimate responsiveness. The sensitivity of AFS was 76% and the specificity was 57%. Correlation between AFS and OMAS at baseline, end of treatment and difference was 0.82, 0.70 and 0.79, respectively. Outcomes for responsiveness of AFS were comparable to outcomes for OMAS and PSC. This study found limited evidence for the AFS as a prognostic and evaluative instrument. The AFS is a simple instrument based on basic functional outcomes (pain, stability, weight bearing, swelling and gait) and may be used in addition to subjective clinical judgment as a prognostic and evaluative tool for recovery after acute ankle injury.

Objective Joint protection (JP) is an important part of the treatment concept for patients with rheumatoid arthritis (RA). The Joint Protection Behavior Assessment short form (JPBA-S) assesses the use of hand JP methods by patients with RA while preparing a hot drink. The purpose of this study was to develop a German version of the JPBA-S (D-JPBA-S) and to test its validity and reliability. Methods A manual was developed through consensus with 8 occupational therapist (OT) experts as the reference for assessing patients' JP behavior. Twenty-four patients with RA and 10 healthy individuals were videotaped while performing 10 tasks reflecting the activity of preparing instant coffee. Recordings were repeated after 3 months for test-retest analysis. One rater assessed all available patient recordings (n = 23, recorded twice) for test-retest reliability. The video recordings of 10 randomly selected patients and all healthy individuals were independently assessed for interrater reliability by 6 OTs who were explicitly asked to follow the manual. Rasch analysis was performed to test construct validity and transform ordinal raw data into interval data for reliability calculations. Results Nine of the 10 tasks fit the Rasch model. The D-JPBA-S, consisting of 9 valid tasks, had an intraclass correlation coefficient of 0.77 for interrater reliability and 0.71 for test-retest reliability. Conclusion The D-JPBA-S provides a valid and reliable instrument for assessing JP behavior of patients with RA and can be used in German-speaking countries.

Treatment options for rheumatoid arthritis (RA) include pharmacological interventions, physical therapy treatments and balneotherapy.To evaluate the benefits and harms of balneotherapy in patients with RA.A systematic review.Studies were eligible if they were randomised controlled trials consisting of participants with definitive or classical RA.We searched various databases up to December 2014. Balneotherapy had to be the intervention under study, and had to be compared with another intervention or with no intervention. We considered pain, improvement, disability, tender joints, swollen joints and adverse events among the main outcome measures. We excluded studies when only laboratory variables were reported as outcome measures. Two review authors independently selected trials, performed data extraction and assessed risk of bias.This review includes nine studies involving 579 participants. Most studies showed an unclear risk of bias in most domains. We found no statistically significant differences on pain or improvement between mudpacks versus placebo (1 study; N.=45; hand RA; very low level of evidence). As for the effectiveness of additional radon in carbon dioxide baths, we found no statistically significant differences between groups for all outcomes at three-month follow-up (2 studies; N.=194; low to moderate level of evidence). We noted some benefit of additional radon at six months in pain (moderate level of evidence). One study (N.=148) compared balneotherapy (seated immersion) versus hydrotherapy (exercises in water), land exercises or relaxation therapy. We found no statistically significant differences in pain or in physical disability (very low level of evidence) between groups. We found no statistically significant differences in pain intensity at eight weeks, but some benefit of mineral baths in overall improvement at eight weeks compared to Cyclosporin A (1 study; N.=57; low level of evidence).Overall evidence is insufficient to show that balneotherapy is more effective than no treatment; that one type of bath is more effective than another or that one type of bath is more effective than exercise or relaxation therapy.We were not able to assess any clinical relevant impact of balneotherapy over placebo, no treatment or other treatments.

Background Multi-directional electrodes in STN-DBS give the possibility to steer the high frequency current into the direction for clinical improvement and moreover recording of multi-array local field potential recordings (LFPs) give the possibility to determine the spatiotemporal distribution of oscillatory power across the STN that is related to the symptomatology in PD patients. Ultimately this information can be used as a feedback to optimize the stimulation parameters. Methods Eight PD patients were included in the study. After micro-electrode recording (MER), to determine the boundaries of the STN, temporarily a prototype of a new 32-contact DBS lead was inserted during the DBS procedure. Different modes of current steering were employed to improve motor symptoms and patients were scored on beneficial and adverse effects. Prior to and immediately after stimulation the distribution of spectral power was recorded by LFPs and spatiotemporal changes were evaluated with Fourier analysis. Results Thresholds of the effects of spherical stimulation were comparable to thresholds of stimulation through the conventional electrode at the same location in 89% of the cases. In eight of fourteen side effects, steering stimulation current increased the threshold for side effects by ⩾ 1 mA compared to spherical stimulation. The size of the therapeutic window could be widened in patient 6, 7 and 8 by steering stimulation in the posterior, posterior and anterior direction, respectively. LFP recordings prior to stimulation yielded the boundaries of the STN by showing increased spectral power particularly in the 13–40 Hz range. Recordings across all directions showed distinct spatiotemporal patterns of neuronal activity, which were related to the pattern of stimulation and the PD symptoms of the patient. Conclusions With a new DBS lead it is possible to steer stimulation current in STN-DBS such that it leads to a larger therapeutic window than with conventional spherical stimulation. Simultaneous LFP recordings across the entire STN provide spatial information about the location of the STN and its disease-related electrical activity. This may potentially be of benefit in predicting how to steer the current towards the sensorimotor part, while avoiding adverse effects.

The aim of this exploratory study was to investigate the predictive value of a fatigue test of the lumbar extensor muscles for training progression in a group of 28 healthy but predominantly sedentary male students, in an 8-week resistance exercise program.A three-phased fatigue test of the lumbar extensor muscles was designed, consisting of two consecutive measurements of full-range isometric back strength on a lumbar measurement device, separated by a dynamic back extension set to volitional fatigue. Differences between the strength values of the 1st and 3rd step is thought to reflect individual back muscle fatigue characteristics. The training program was primarily aimed at improving lumbar extensor endurance, by using a relative high number of repetition and low training loads. Linear regression analysis was used to evaluate the relationship between lumbar strength progression and several fatigue test parameters.The main fatigue indicator in our regression models (isometric strength decline between 1st and 3rd step) did not show predictive value in lumbar strength progression in training and testing, respectively. On the other hand, the work capacity that subjects delivered in the dynamic set (2nd step) had some predictive value.Based on the results, isometric strength decline measurement has no additional value to a standardized set of repetitions until failure in predicting future training performance. In practice, this means that a lower back training machine could be used at baseline to assist in tailoring individual lumbar training regimes, without the additional use of an isometric-strength testing module.

Deciding whether or not to perform a health behaviour is an active decision-making process which has an impact on current and future behaviour and can be influenced by the beliefs both of patients and their healthcare professionals. The aim of this study was to explore rheumatoid arthritis (RA) patients' and occupational therapists' (OTs) perceptions of the benefits of and barriers to performing joint protection (JP).A mixed methods design was used. Questionnaires applied a theoretical framework of key themes to assess the relevance of JP benefits and barriers both to people with RA and OTs. Focused interviews with people with RA then enabled data triangulation. Investigator triangulation was used to check the validity of data interpretation.Ten people with RA and nine OTs participated. From the questionnaires, both groups agreed that highly relevant key themes for JP benefits were physical well-being, potential benefit and personal control. By contrast, the three key themes for JP barriers - negative attitude of others, negative impact on others and taking time from other things - were relevant for the majority of the OTs but not patients. The interviews enabled an understanding of the meaning behind RA patients' ratings, particularly their differences from OTs. People with RA explained JP benefits, and disease acceptance had altered some initial barriers into perceived benefits over time.Emphasizing benefits and identifying individually relevant barriers could be an important communication strategy for OTs in understanding patients' rationale for whether or not to adopt JP methods.

The objective of the trial was to evaluate the effectiveness of a short and intensive physiotherapy programme versus usual care, mainly consisting of walking. Fifty-eight male unilateral lower limb amputees were randomised over intensive care (N = 29) and usual care (N = 29); this study was carried out in a clinical environment, within the premises of the International Committee of the Red Cross, in Southern Myanmar. The amputees in the experimental group received a three-day programme built around 7 specific exercises, lasting approximately one hour altogether. Effects were assessed on: 2-minute walk test, physiological cost index (PCI), amount of weight bearing on both legs, 5 questions of the Functional Measure for Amputees questionnaire and the timed up and go test. Trans-tibial amputees were tested the first day they were fitted and then 2 days later; trans-femoral amputees were tested when walking out of the parallel bars and 4 days later. The experimental group showed significant improvement in the 2-minute walk test (p = 0.02), in the PCI (p = 0.02) and in the maximal weight bearing test on the amputated leg (p = 0.04). Despite limited means, research remains feasible in a developing country; this study has demonstrated that physiotherapy is effective in improving functional performance of lower limb amputees and therefore has its place in every rehabilitation centre.

Background Ankle sprains are one of the most commonly treated musculoskeletal injuries. The three main treatment modalities for acute lateral ankle ligament injuries are immobilisation with plaster cast or splint, 'functional treatment' comprising early mobilisation and use of an external support (e.g. ankle brace), and surgical repair or reconstruction. Objectives We aimed to compare surgical versus conservative treatment for acute injuries of the lateral ligament complex of the ankle in adults. Search methods We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (January 2006), the Central Register of Controlled Trials (CENTRAL) (The Cochrane Library 2005, Issue 4), MEDLINE (1966 to December 2005), EMBASE, CINAHL and reference lists of articles, and contacted researchers in the field. This review is considered updated to January 2006. Selection criteria Randomised or quasi-randomised controlled trials comparing surgical with conservative interventions for treating ankle sprains in adults. Data collection and analysis At least two authors independently assessed methodological quality and extracted data. Where appropriate, results of comparable studies were pooled. We performed sensitivity analyses to explore the robustness of the findings. Main results Twenty trials were included. These involved a total of 2562 mostly young active adult males. All trials had methodological weaknesses. Specifically, concealment of allocation was confirmed in only one trial. Data for pooling individual outcomes were only available for a maximum of 12 trials and under 60% of participants. The findings of statistically significant differences in favour of the surgical treatment group for the four primary outcomes (non-return to pre-injury level of sports; ankle sprain recurrence; long-term pain; subjective or functional instability) when using the fixed-effect model were not robust when using the random-effects model, nor on the removal of one low quality (quasi-randomised) trial that had more extreme results. A corresponding drop in the I² statistics showed the remaining trials to be more homogeneous. The functional implications of the statistically significantly higher incidence of objective instability in conservatively treated trial participants are uncertain. There was some limited evidence for longer recovery times, and higher incidences of ankle stiffness, impaired ankle mobility and complications in the surgical treatment group. Authors' conclusions There is insufficient evidence available from randomised controlled trials to determine the relative effectiveness of surgical and conservative treatment for acute injuries of the lateral ligament complex of the ankle. High quality randomised controlled trials of primary surgical repair versus the best available conservative treatment for well-defined injuries are required.
