Abstract

Zusammenfassung Hintergrund und Ziele Schädelbasismetastasen sind eine seltene Manifestation onkologischer Erkrankungen. Wenn Hirnnerven beteiligt sind, können schon kleine Läsionen erhebliche funktionelle Beeinträchtigungen hervorrufen. Spezifische klinische Charakteristika wie neurologische Symptome, assoziierte Primärtumoren, Prognose und optimale Therapie der Erkrankung sind schlecht definiert und sollen in dieser Arbeit systematisch dargestellt werden. Methoden Mit einem monozentrischen retrospektiven Ansatz wurden Schädelbasismetastasen bei Patienten, die im Zeitraum von 2006 bis 2018 behandelt wurden, detailliert hinsichtlich klinischer Charakteristika, der durchgeführten Therapie und des weiteren Erkrankungsverlaufs analysiert. Ergebnisse Insgesamt 45 Patienten mit Schädelbasismetastasen und Hirnnervenausfällen wurden erfasst. Die häufigsten Primärtumoren waren Prostatakarzinom (27 %), Mammakarzinom (22 %) und multiples Myelom (16 %). Die am häufigsten betroffenen Hirnnerven waren Nervus trigeminus (42 %), Nervus oculomotorius (33 %) und Nervus facialis (27 %). 84 % aller Patienten wiesen außerhalb der Schädelbasis liegende weitere Knochenmetastasen auf. Eine durale Infiltration oder eine Meningeosis neoplastica lagen bei je 13 % der Patienten vor. Nach Bestrahlung waren 61 % der Patienten hinsichtlich der auf die Schädelbasismetastase zurückzuführenden Symptome klinisch stabil, bei 22 % hatten sich die Symptome gebessert. Das mediane Gesamtüberleben betrug 8 Monate (Spanne: 0,4–51 Monate). Bei Patienten, die mit einer dosiseskalierten Bestrahlung behandelt wurden, bestand eine längere Überlebenszeit (16,4 Monate vs. 4,7 Monate). Dieser Effekt persistierte auch in der multivariaten Analyse unter Berücksichtigung der Faktoren Karnofsky-Index, Metastasenanzahl, Primärtumor und Bestrahlungsdosis (HR 0,37, p = 0,02). Diskussion Schädelbasismetastasen mit Hirnnervenausfällen haben ein vielgestaltiges Bild und oft eine schlechte Prognose. Um potenziell eine Überlebenszeitverbesserung zu erreichen, sind präzise Diagnostik und Therapie Voraussetzung. Prospektive kontrollierte Untersuchungen sind notwendig.

Abstract

Ein integraler Bestandteil der kurativen Therapie padiatrischer Tumoren ist die Strahlentherapie (RT). Die Entwicklungen der modernen RT erreichen eine bessere Zielvolumenerfassung sowie Schonung von Normalgewebe. Eine Steigerung der Uberlebensraten und eine Senkung des Nebenwirkungsrisikos kann erwartet werten. Aktuelle Fortschritte bei der Implementierung moderner radioonkologischer Behandlungsmethoden in die padiatrische Radioonkologie werden dargestellt. Die Methodenvielfalt und die daraus resultierenden Workflows werden analysiert und erortert. In der padiatrischen Radioonkologie lassen sich die innovativen Bestrahlungstechniken vorteilhaft einsetzten. Die klinische Anwendung der neuen Technologien werden begleitet von prospektiven Qualitatssicherungsmasnahmen innerhalb klinischer Studien und interdisziplinaren Programmen zur Erfassung und Evaluierung von Spatfolgen. Standards fur die Durchfuhrung der Strahlentherapie werden in nationalen und internationalen Arbeitsgruppen entwickelt. Die Vorteile der modernen Technologien erfordern eine individuelle Anpassung an die speziellen padiatrisch-onkologisch-klinischen Konstellationen. Neue radioonkologische Behandlungsmethoden zeigen in kleineren Serien zufriedenstellende Ergebnisse, die jedoch in weiteren prospektiven Untersuchungen bestatigt werden mussen. Die Etablierung von Radiotherapiestandards sowie die Erfassung und Auswertung der Bestrahlungsdaten werden dazu beitragen, den Stellenwert der Strahlentherapie im Gesamtmanagement der Erkrankung zu definieren und die Langzeitergebnisse zu verbessern.

Abstract

Atypical teratoid rhabdoid tumor of the brain is a rare embryonal tumor of infancy which has become more widely recognized due to specific immunostaining now more routinely available. These tumors still carry a poor prognosis and no standard therapy is currently available. However, the recent development of aggressive multimodality strategies seems to be associated with improved survival rates. In this chapter, we provide a review of our current knowledge on the demographics and epidemiology, pathology, and molecular characteristics. We review each individual therapeutic modality used (surgery, conventional chemotherapy, high-dose chemotherapy with stem cell rescue, intrathecal chemotherapy, and radiation) and the contribution of each to outcome when available. We provide an overview of the most recent therapeutic trials published and discuss the future options in development.

Abstract

Central nervous system (CNS) germ cell tumors (GCTs) are separated into germinomas and nongerminomatous germ cell tumors (NGGCTs). The latter group includes teratoma (mature and immature), teratoma with malignant transformation, yolk sac tumor, embryonal carcinoma, choriocarcinoma, and mixed GCTs. CNS GCTs can be classified based on their histologic appearance and immunohistochemical profiles with histologic subtype being the best predictor of outcome. Germinomas respond exceptionally well to chemotherapy and irradiation and have been shown to have survival rates in excess of 90 %. In contrast, NGGCTs are more resistant to therapy and require more intensive chemotherapy, as well as higher doses of irradiation. There is little information on the histogenesis and molecular genetics of CNS GCTs. Fluorescent in situ hybridization (FISH) studies have shown a gain of X chromosome in a majority of GCTs. Other studies have shown that 12p abnormalities (gains and isochromosome 12p) are common, especially in germinomas. Gene expression analysis studies show overexpression of self-renewing pluripotency genes and genes responsible for tumor growth and differentiation in various tumors with no significant differences in gene expression between CNS GCTs of similar histology arising at different sites and different ages within the pediatric age group. Differences in microRNA expression have been demonstrated between histologic subtypes of CNS GCTs, and their potential use as a biomarker in the serum and cerebrospinal fluid is being explored. KIT mutations are the most commonly found abnormality on whole genome sequencing, predominantly in germinomas, followed by genes involved in the MAPK signaling pathway, suggesting a role for KIT inhibitors in the management of these tumors.

Abstract

Die Strahlentherapie gehort unverandert zu den wesentlichen Bestandteilen der multimodalen Behandlung von Tumoren im Kindesalter. Bei vielen Erkrankungen bildet sie eine wichtige Grundlage fur eine kurative Therapie. Sie erfordert jedoch eine einheitliche Erfassung innerhalb prospektiver Protokolle zur Erhebung von Spatfolgen wie durch die RiSK-Studie („risk intervention skills study“). In den letzten 20 Jahren gelang es, die Strahlentherapie als festen Partner in die multidisziplinaren Therapieoptimierungsstudien der GPOH (Gesellschaft fur Padiatrische Onkologie und Hamatologie) einzubinden und so auch deren Risikoadaptierung voranzutreiben. Die Strahlentherapie ist gefordert, weitere Forschung zur Therapieoptimierung innerhalb klinischer Studien entweder in Form einer primaren radioonkologischen Fragestellung oder aber als reproduzierbaren Bestandteil eines prospektiven Behandlungsprotokolls zu betreiben. Zu den wesentlichsten Forschungsinhalten gehoren die Verbesserung der Tumorkontrolle, die Reduktion von Spatfolgen sowie der Stellenwert innovativer Techniken.

Abstract

BACKROUND: Late effects after radiotherapy in childhood and adolescence have mainly been characterized retrospectively with small patient numbers. Therefore the German Group of Pediatric Radiation Oncology (APRO) established the " Reg Ister for the evaluation of late Side effects after radiation in Childhood and adolescence” (RiSK). After a pilot phase starting in 2001 documentation has been performed all over Germany since 2004. This analysis shows the first results of “RiSK”. PATIENTS AND METHODS: Radiation parameters including detailed organ doses as well as toxicity evaluations were collected prospectively from centers all over Germany in the study center. Standardized documentation forms were used. Documentation is planned for all children who receive radiotherapy in one of the German pediatric therapy trials. RESULTS: Until December 31st 2006, 696 documentations of radiotherapy and 526 acute as well as 836 late follow-up documentation forms have been collected. Altogether, 41 patients with late grade 3 and 16 patients with late grade 4-side effects were identified. Side effects mainly concerned joints with functional impairment (after combined radiotherapy and surgery), the bowel, skin and subcutis as well as blood parameters under continued chemotherapy. Patients with late side effects of a higher grade were mainly treated for Ewing's or soft tissue sarcomas (n=235 patients), representing 33.8% of all patients in this study. CONCLUSION: Fortunately, up to now only a few late grade 3 or 4 side effects of radiotherapy are shown for almost 700 documented patients. For further results, especially for the characterization of dose-effect-relationships, this study has to be continued with a higher patient number and a longer follow-up.

Abstract

BACKGROUND: Malignant pediatric brain tumors constitute a heterogeneic group of central nervous system tumors, and general markers of diagnosis and prognosis are not available. Recently, a panel of CD markers (CD15, CD24, CD29) was used to define increasing neural differentiation of embryonic stem cells. Although these CD markers have been associated with worse prognosis due to presence of tumor propagating cells, alterations in adhesion and migration in various cancer types, no studies of multiple CD markers in pediatric brain tumors have been performed. METHODS: We have collected tumors, including medulloblastomas (MB), ependymomas (EP) and juvenile astrocytomas, from >20 pediatric brain tumor patients. Frozen tumor sections and cultured tumor cells were stained for CD15, CD24 and CD29 and analyzed using fluorescence microscopy or flowcytometry. RESULTS: MB contained a mixture of cells with strong CD15 labeling inside vessels and cells with diffuse CD15 staining in the parenchyma of the tumor tissue. Cells strongly labeled with CD15 were also positive for the leukocyte marker CD45. The previously proposed association between CD15 expression and prognosis in MB could instead of reflecting abundance of tumor propagating cells depend on infiltrating immune cells. In low-grade pediatric tumors and EP, larger areas stained weakly for CD15 while few cells displayed strong staining. CD24 was expressed on the vast majority of cells in pediatric brain tumors, despite grade of malignancy. MB, however, displayed an intense and aberrant staining for CD24. Computerized image analysis of frozen tumor sections showed that proliferation of cells and the expression of CD29 correlated in a sub-group of MB. CONCLUSIONS: Our preliminary data show that CD15, CD24 and CD29 are differentially expressed in high- and low-malignant pediatric brain tumors in vivo. By defining the patterns of CD antigen expression in different pediatric tumors their relationship to biological behavior and thus prognosis can be established. (Less)

Abstract

Das Medulloblastom ist der haufigste maligne Hirntumor im Kindes- und Jugendalter. Medulloblastome bei Erwachsenen sind relativ selten und stellen nur einen Anteil von 1% aller Hirntumoren des Erwachsenenalters. Die Inzidenz bei Erwachsenen wird um 0,5/Mio./Jahr angegeben [11, 26], betragt jedoch in der Altersgruppe von 15–19 Jahren um 2,33/Mio./Jahr und nimmt dann mit steigendem Alter kontinuierlich ab [26]. Der Beginn des Erwachsenenalters wird in der Literatur unterschiedlich definiert. Die meisten der klinischen Studien schliesen diesbezuglich Patienten >15 Jahre ein. Die Gruppe der sog. embryonalen Tumoren reprasentieren im Kindesalter mit einem Anteil von 25% unter den Hirntumoren die zweithaufigste Entitat. Sie entstehen wahrscheinlich aus unreifen neuralen Progenitorzellen des ZNS. Am haufigsten ist ein primitiv neuroektodermaler Tumor (PNET), der zu 85% vom Kleinhirn ausgeht und dann Medulloblastom genannt wird [24]. Nach der WHO-Klassifikation fur zentralnervose Tumoren sollte der Begriff PNET ausschlieslich solchen Tumoren vorbehalten sein, die Eigenschaften eines primitiv neuroektodermalen Zelltyps im engeren Sinn auspragen [53].

Abstract

Rhabdoid tumors mainly affect infants and other very young children with a marked vulnerability towards intensive therapy such as invasive surgery, high dose chemotherapy (HDCT) and dose intense radiotherapy. Radiotherapy (RT) is a promising option in rhabdoid tumors but its application in infants remains controversial. Neurocognitive and vascular side effects occur even long after completion of therapy. Therapeutic recommendations suggested by the European Rhabdoid Registry including RT, high dose chemotherapy (HDCT) and methotrexate (MTX) were developed by a consensus committee. Unique to our EU-RHAB database is the ability to analyze data of 64 of 81 registered infants (under one year of age) separate from older children. 20 (age at diagnoses 2–12 months) of these had received radiotherapy. To our knowledge, this is the first report specifically analyzing treatment data of infants suffering from malignant rhabdoid tumors. Our results suggest that radiotherapy significantly increases the mean survival time as well as the 3 year overall survival in infants. We detected a doubling of survival times in infants who received RT. Overall, our results suggest that infants benefit from RT with tolerable acute side effects. Severe long term sequelae likely due to intraventricular MTX and/or RT were reported in 4 patients (leukoencephalopathy). No differences in chemotherapy-related toxicity were observed between infants and children. We suggest that a nihilistic therapeutic approach towards young infants is not warranted and that RT may not be a priori rejected as a therapeutic option in infants.

Abstract

Die Chancen auf Heilung haben sich beim Medulloblastom und bei intrakraniellen Keimzelltumoren im Kindesalter in den letzten Jahrzehnten durchgreifend verbessert. So werden heute langfristige Uberlebensraten von 60–80% bzw. mehr als 90% erreicht. Das seltene Vorkommen und die Notwendigkeit arztlicher Erfahrung in der Steuerung der Therapie und ihrer Nebenwirkungen auf hohem Niveau haben dazu gefuhrt, dass heute uber 90% der Kinder innerhalb von nationalen und internationalen Studien behandelt werden, um eine standige Verbesserung der Resultate zu erreichen. Die Weiterentwicklung operativer Verfahren ermoglicht eine zunehmend bessere und schonendere Resektion von Hirntumoren.

Abstract

Wahrend im Erwachsenenalter vorwiegend Tumoren epithelialen Ursprungs im Vordergrund stehen, werden Tumoren im Kindesalter vorwiegend durch Neoplasien embryonaler Genese gebildet. Diese zeigen eine erhohte Empfindlichkeit gegenuber ionisierender Strahlung und chemotherapeutischen Substanzen. Hierdurch werden die im Vergleich zum Erwachsenenalter deutlich erhohten Heilungsraten erklart. Gleichzeitig reagiert aber der wachsende Organismus sensibler auf zytotoxische Agenzien. Einige chemotherapeutische Substanzen konnen zwar durch einen strahlensensibilisierenden Effekt die Tumorkontrolle verbessern, erhohen jedoch gleichzeitig das Risiko fur akute, maximale Nebenwirkungen und Spatfolgen (Tabelle 2). Hierdurch konnen nicht nur einzelne Organfunktionen beeintrachtigt werden, sondern auch die gesamte Entwicklung des betroffenen Kindes. Die Dosisschwelle fur die Toleranz des kindlichen Gewebes v. a. der sich entwickelnden Organe liegt haufig unter den Dosierungen, die fur die lokale Tumorkontrolle notwendig sind, und andern sich durch eine zusatzliche Chemotherapie. Daher werden spezielle Anforderungen an die Strahlentherapie gestellt, um Normalgewebe und v. a. Risikoorgane zu schonen.

Abstract

Background: The rationale of the HIT-LGG protocol is to delay standard radiotherapy through administration of chemotherapy for children with progressive low grade glioma at an age under 5 years and in older children upon individual decision. Patients and Method: Until October 10th., 1999, 402 patients from 69 hospitals were registered. 130 children with progressive tumors were treated after a median observation time of 5 months: 46 patients received primary radiotherapy and 84 primary chemotherapy. A ten week induction period with weekly Vincristine and pulses of Carboplatin at weeks 1, 4, 7 and 10 is followed by consolidation with simultaneous application of both drugs every 4 weeks until week 53. Results: Of 84 patients in the chemotherapy arm of the study (49 male, 35 female, 23 NF I, median age 2.99 years) 36 received treatment at diagnosis and 43 after a median observation time of 19.7 months. 94,3% achieved a clinical and neuro-radiological response according to protocol criteria (5 CR, 30 PRIOR, 31 SD) after a median of 5.1 months. 4 tumors showed primary progression (9 too early, 5 not known). Only 6 of 84 children received radiation therapy for progressive disease during (2) or after termination (4) of chemotherapy, after a median delay of 25.6 months at a median age of 6.0 years. At a median observation time of 21.0 months, 6 children are in CR, 11 in PR, 48 have SD, 4 tumors are progressive and 3 children died of their tumor. (9 too early, 3 not known). PFS is at 72% after 36 months. 24 of 27 children experiencing allergic reactions to Carboplatin had to interrupt chemotherapy prematurely. Conclusions: Combination therapy with Carboplatin and Vincristine can effectively delay the start of radiotherapy in children with progressive low-grade glioma. The high rate of hypersensitivity reactions has to prompt future modifications of treatment.

Zusammenfassung Die Segmentierung von Schnittbildserien spielt bei der Definition von Zielvolumen und Risikoorganen fur die 3D-Bestrahlungsplanung eine grose Rolle. In dieser Arbeit wird eine Methode zur Quantifizierung der rein technisch bedingten Ungenauigkeit bei der manuellen Bildsegmentierung beschrieben. Zur Validierung dieser Methode wurde ein Graustufenphantom mit verschiedenen geometrischen Figuren unterschiedlichen Kontrastes entwickelt. die von verschiedenen Versuchspersonen mit Hilfe des Segmentierungsprogramms TOMAS des Heidelberger VOXELPLAN-Systems konturiert wurden. Uber die erzielten Ergebnisse wird berichtet.

Abstract

High-grade (HGG) and diffuse intrinsic pontine gliomas (DIPG) with primary metastatic spread are extremely rare and have a dismal prognosis. Analogous to simultaneous radiochemotherapy in non-metastatic HGG and DIPG, concurrent craniospinal irradiation (CSI) and metronomic temozolomide (metroTMZ) may represent a reasonable therapeutic approach. However, the antitumor efficacy and toxicity of this treatment still have to be investigated.Between March 2007 and December 2012, six children with primary metastatic HGG (n = 4) or DIPG (n = 2) received CSI and concurrent metroTMZ based on individual treatment recommendations and, in some cases, within the HIT-HGG 2007 multicenter trial. Outcome and treatment-related toxicities were evaluated.All patients received irradiation to the entire craniospinal axis (35.2 Gy, n = 5; 36 Gy, n = 1:) and 5 received a local boost to macroscopic tumor deposits. Simultaneously, metroTMZ (75 mg/m(2)/day, n = 5; 60 mg/m(2)/day, n = 1) was administered. Additionally, 1 patient received nimotuzumab once per week. Within a median follow-up of 10.0 months (range 6.5-18.7 months), all patients experienced disease progression and 5 patients died. Median progression-free survival was 4.0 ± 0.8 months (range 2.4-10.7 months) and median overall survival was 7.6 ± 3.5 months (range 4.0-17.6 months). Acute myelosuppression most severely limited application of this aggressive treatment strategy. Severe hematotoxicities (≥ grade 3) occurred in all patients and metroTMZ had to be interrupted or discontinued in 4 out of 6 cases.Concurrent CSI and metroTMZ might represent a feasible treatment approach for primary metastatic HGG and DIPG. On the basis of our experience, severe but manageable acute hematotoxicity has to be expected. An international effort is warranted to reassess the efficacy and toxicity of this approach within a prospective study.

Abstract

Purpose: To evaluate whether the standardized helmet technique is adequate to reliably cover the clinical target volume (whole brain including cranial meninges) during treatment planning and treatment delivery. Methods and Materials: In 21 patients undergoing irradiation of the brain in acute lymphoblastic leukemia or primary cerebral lymphoma, the coverage of the clinical target volume was checked with a repeat computed tomography (CT) in the treatment position (head fixation with face mask). The accuracy of field alignment was quantitatively assessed with sequential verification films. For each patient, linear and rotational discrepancies were measured between the simulation and first check film, and between five consecutive verification films. Results: Coverage of clinical target volume . In 11 cases (52%), the CT examinations showed that parts of the subfrontal region and midcranial fossa were not included by the field assigned under simulation. Accuracy of field alignment . For the total group of patients, all deviations were normally distributed with mean values between –1.2 mm and 1.5 mm and standard deviations of 2.9 mm to 3.7 mm for linear discrepancies, and 0.3 degrees ± 3.2 degrees for rotational discrepancies. For all patients, deviations were similar for the transition from simulation to the treatment machine and for subsequent treatment delivery, with 50% and 95% of absolute differences being less than 2.0 mm and 6.5 mm, respectively. Maximum linear deviations were less than 9.5 mm. Conclusions: The currently used helmet technique is inadequate to cover the clinical target volume. Repeat CT examinations are a useful method to delineate the clinical target volume on an individual patient basis. In addition, statistical fluctuations of field displacements up to 1.0 cm have to be considered when prescribing safety margins for reliable coverage of the clinical target volume during treatment planning and delivery.

Abstract

Deviations between simulation and first check films were quantitatively assessed for 95 unselected head and neck cancer patients. All measured deviations — calculated on the basis of a total of 190 simulation and 380 verification films — were normally distributed, with mean values of 0–3 mm and standard deviations of 3–5 mm. Of the absolute deviations, 50% and 95% were within 3 mm and 9 mm, respectively. These results should be considered in clinical practice when prescribing safety margins and adequate cut off doses for sparing critical organs in head and neck cancer.

Abstract

BACKGROUND: Glutathione S-transferase (GST) enzymes are involved in detoxifying chemotherapy agents and clearing reactive oxygen species formed by radiation. In this study, we explored the relationship between the host GSTP1-105 polymorphism (rs1695), tumor GSTpi protein expression, and clinical outcomes in pediatric medulloblastoma. We hypothesized that the GSTP1-105 G-allele and increased tumor GSTpi expression would be associated with lower progression-free survival and fewer adverse events. METHODS: The study included 106 medulloblastoma/primitive neuroectodermal tumor (PNET) patients seen at Texas Children’s Cancer Center. Genotyping was performed using an Illumina HumanOmni1-Quad BeadChip and tumor GSTpi expression was assessed using immunohistochemistry. We used the Kaplan-Meier method for survival analyses and multivariable logistic regression for toxicity comparisons. RESULTS: Patients with a GSTP1-105 AG/GG genotype or who had received a higher dose of craniospinal radiation (median 36 Gy) had a greater risk of requiring hearing aids than their respective counterparts (OR 4.0, 95%CI 1.2 - 13.6, and OR 3.1, 95%CI 1.1 - 8.8, respectively). Additionally, there was a statistically significant interaction between the two variables. Compared with the lowest risk group (GSTP1-105 AA-lower dose radiation) patients with a GSTP1-105 AG/GG genotype who received a higher dose radiation were 8.4 times more likely to require hearing aids (95%CI 1.4 - 49.9, p-trend ¼ 0.005). When adjusted for age, gender, and amifostine use, the association remained. CONCLUSIONS: The GSTP1-105 G-allele is associated with permanent ototoxicity in pediatric medulloblastoma/PNET and strongly interacts with radiation dose. A possible mechanism for this finding is that the GSTP1-105 G-allele leads to reduced GSTpi free radical detoxification in the setting of multimodality therapy including cisplatin and radiation. Patients with this allele should be considered for clinical trials employing radiation dose modifications and more targeted cytoprotectant strategies than are currently being used with amifostine.

Abstract

Purpose : Quantitative assessment of the accuracy of field alignment in a homogeneous group of patients with difficult positioning (postoperative irradiation after total hip replacement). Methods and Materials : In 95 patients linear and rotational discrepancies were measured between the simulation and first check film and between five consecutive verification films. Results : For the total group of patients, all deviations were normally distributed with mean values of approximately zero and standard deviations of 4.0–8.0 mm (linear discrepancies) and 3.5–5 degrees (rotational discrepancies). Deviations were similar for the transition from simulator to the treatment machine and for subsequent treatment delivery, with 50% and 95% of absolute differences being less than 5 mm and 15 mm, respectively. Conclusions : Our analysis indicates that statistical fluctuations are considerably more important than errors introduced at start of treatment. Therefore, a first check film seems to be inadequate to predict the expected inaccuracies for the whole course of treatment. In addition, our results should help to prescribe appropriate safety margins for patients with difficult positioning.

Abstract

Specific tools of decision analysis, a set of mathematical rules for simplifying complex decisions, were applied to evaluate the impact of secondary leukaemia on the selection of initial treatment in Hodgkin's disease (HD). For this purpose, a combined ‘expected utility' considering survival, relapse free survival, and secondary leukaemia was determined for different treatment strategies. Our analysis revealed that considerations of secondary leukaemia for initial therapy should include the a priori estimation of all possible events which may occur after initial treatment, e.g. the probabilities of recurrence and success of salvage therapy. In early and intermediate stage HD, for example, the minimal risk of leukaemia after successful radiotherapy (RT) must be weighed against the increased risk after treatment failure and subsequent salvage therapy. Thus, the difference of expected risk of leukaemia between RT and combined modality treatment (CMT) is within 4% for HD, stage II B and near to 0% in stage III A. In advanced stage HD, the addition of RT to chemotherapy has no adverse effect on the expected utility of initial treatment. These conclusions are only marginally affected by reported differences in rates of recurrence, salvage success, and secondary leukaemia. Subjective quality of life considerations, such as the latency period between treatment and leukaemia and patients' attitudes towards the occurrence of leukaemia, did not significantly affect expected utilities. In summary, our results strongly suggest that presently there is no sound basis for reducing the intensity of initial treatment in HD to avoid secondary leukaemia.

Abstract

A total of 38 patients (18 female/20 male) with childhood meningioma were recruited from the German registry HIT-Endo (1989–2009). In 5 cases meningioma occurred as second malignant neoplasm (SMN). Histologies were confirmed by reference assessment in all cases (SMN: 2 WHO I, 1 WHO II, 2 WHO III). The SMNs were diagnosed at a median age of 12.4 years with a median latency of 10.2 years after primary malignancy (PMN; 4 brain tumors, 1 lymphoblastic leukemia; median age at diagnosis 2.7 years). Meningioma occurred as SMN in the irradiated field of PMN (range 12–54 Gy). The outcome after treatment of SMN meningioma (surgery/irradiation) was favorable in terms of psychosocial status and functional capacity in 4 of 5 patients (1 death). We conclude that survivors of childhood cancer who were exposed to radiation therapy at young age harbor the risk of developing meningioma as a SMN at a particularly short latency period in case of high dose exposure.

Abstract

From 1982 to 1989, 68 patients with Stages IA to IIIB Hodgkin's disease were treated by simultaneous supra- and infradiaphragmatic irradiation (SSI-RT). 46 patients received exclusive radiotherapy for Stages IA and IIA (extended mantle field irradiation (EMF) 31, and total lymphatic irradiation (TLI) 15). Combined modality treatment including pre-irradiation chemotherapy, was given to 22 patients (CH-EMF 12 and CH-TLI 10). The median follow-up was 64 months. 5-year overall survival was 94% in combined Stages IA and IIA, and 100% in Stage IIIA. 5-year freedom from relapse was 87% in combined Stages IA and IIA, and 80% in Stage IIIA. Toxicity was evaluated in 64 patients. Acute and long term toxicity was similar to previously reported data on sequential supra- and infradiaphragmatic irradiation. In SSI-RT prior chemotherapy affected pre-irradiation blood counts and was associated with delayed post-irradiation haematological recovery. The relative mean white blood cell (WBC) decrease ranged from 47% (EMF) to 61% (TLI). The mean platelet decrease ranged from 43% (EMF) to 80% (CH-TLI). Both prior chemotherapy and total lymphoid irradiation increased haematotoxicity. The mean duration of breaks ranged from 3.9 days in EMF to 14.9 days in CH-TLI. The mean treatment time, ranging from 43 days in EMF to 54 days in TLI, was significantly shorter than in sequential schedules (up to 112 days). Our results indicate that SSI-RT is an effective and safe treatment. Its use seems to be limited mainly by prior chemotherapy. As compared to sequential schedules, the risk of match line overlap is avoided, overall treatment time is decreased, and treatment costs are minimized.

Abstract

Objective Sphenoid wing meningiomas represent a difficult to access subtype of intracranial meningiomas involving important neurovascular structures such as the optic nerve, cavernous sinus or carotid artery. They cause neurological compromises by direct compression of adjacent cranial nerves. Insidious and aggressive dural, bony, and orbital involvement produces several difficulties for adequate resection leading to higher rates of recurrence. Methods This retrospective case analysis consists of 73 patients who were surgically treated for meningiomas involving the sphenoid wing, where a pterional approach was performed between April 2001 and February 2006. 51 women and 22 men with a mean age of 59.4 years were operated on. The follow-up period ranged from 3-75 months (mean 29.8 months). Patients were divided into the following groups based on the site of the tumor: group 1: outer part of the sphenoid ridge (lateral, n=16); group 2: middle part of the sphenoid ridge (intermediate, n=5); group 3: inner part of the sphenoid ridge (medial, n=22); and group 4: spheno-orbital meningioma (n=30). Results The majority of patients presented with visual impairment (55%), followed by generalized headaches (36%) and visual field defects (33%). Total microscopic tumor resection was achieved in 35 patients (47.9%). Visual acuity improved in 58% of the patients, with 23% returning to normal vision. Preexisting cranial nerve deficits remained unchanged in the majority of patients (79%) and improved in 18%. Temporary new cranial nerve deficits occurred in 6 cases, and 1 patient (1%) developed permanent third nerve palsy. The mortality rate was 3% (2 patients) and the rate of permanent nonvisual morbidity was 7% (5 patients). 12 patients (16%) received postoperative radiotherapy. In 6 of 7 patients who were observed for at least 1 year after radiotherapy, stable tumor volume was noted at the follow-up review (mean 30.2 months, range 16-50 months), which provides a tumor growth control rate of 86%. The overall recurrence rate was 15% (11 of 73 patients). Conclusion The result of this study affirms the safety of microsurgical treatment strategies, so that sufficient tumor control can be achieved with minimal morbidity and satisfying functional results in most cases.

Abstract

Through a variety of technical enhancements modern radiotherapy offers the possibility to apply a high dose to an exactly defined target volume while the surrounding normal tissue is optimally spared. This article gives a short overview of new methods and techniques for radiotherapy of patients with brain tumors. Developments in the fields of biological imaging, radiation treatment planning, patient positioning and monitoring during radiotherapy will be especially discussed.

48. Anxiety and depression in cancer patients compared with the general population

Abstract

The objective of this study was to compare the prevalence of anxiety and depression in cancer patients with the prevalence found in the general population, using the Hospital Anxiety and Depression Scale (HADS). Participants were 1529 cancer patients treated between 2002 and 2004 in Germany and 2037 persons from the German general population. In the cancer patients, the risk of psychiatric distress was nearly twice that of the general population. While for older age groups (61 years and above) there were only small differences between cancer patients and the general population, the differences in both scales were high for young persons. There were differences between the HADS mean scores of the patients with different tumour localisations, with high values for brain cancer and low scores for prostate cancer. The influence of the tumour stage on anxiety and depression was weak. However, depression scores of patients with a survival time less than 1 year were elevated. The results show that large sample sizes are necessary to evaluate the psychological situation of cancer patients, and that age and gender differences must be taken into account when several samples are compared.

Abstract

Each year approximately 1,800 children are diagnosed with cancer in Germany. The orbit is a very rare manifestation site for malignancies in childhood. The most common primary malignant orbital tumour is the rhabdomyosarcoma. Secondary orbital tumours are generally metastases of neuroblastomas or haematological diseases. The five-year survival rate of children with rhabdomyosarcomas has increased to 90 % in the past 30 years due to innovations in radiation and chemotherapy.A paediatric oncological interdisciplinary approach is taken in the diagnostics and therapy for these rare tumours and is based on therapy optimisation studies that include polychemotherapy, radiation therapy, and surgical treatment.We discuss the current multimodal therapy strategies and results, as well as the role of the ophthalmologist, in the medical care of young patients with malignant orbital tumours, with examples of individual courses of rhabdomyosarcoma, non-Hodgkin's lymphoma, and metastases.Orbital rhabdomyosarcoma is one of the few life-threatening diseases often first seen by an ophthalmologist. The ophthalmologist's prompt diagnosis and initiation of therapy therefore strongly influences the patient's chance of survival. The role of the ophthalmologist in the medical care of children with orbital malignancies includes a timely clinical diagnosis with histological confirmation, close monitoring of visual functions, the management of ocular complications, as well as long-term follow-up care to detect later therapeutic side effects and/or complications. The treatment of children with suspected tumours is best performed in paediatric oncological centres with access to all necessary specialties in order to ensure rapid diagnosis and therapy.
