Background Cancer-related inflammation is a distinctive feature of the development and progression of PC. However, the relationship between the systemic inflammatory response and survival has not been evaluated as a predictive factor of chemotherapy. The aim of this study was to evaluate the prognostic and predictive value of a baseline SIRI based on peripheral neutrophil, monocyte, and lymphocyte counts in metastatic PC. Methods Retrospective review of 178 metastatic pancreatic cancer patients. Associations between overall survival (OS), time to progression (TTP), chemotherapy schedule and SIRI at diagnosis were analyzed. Results Median age 67 years, 52% were male. First line chemotherapy regimens: 41% Gemcitabine, 31% Gemcitabine plus Nab-Paclitaxel and 17% mFOLFIRINOX. Patients with SIRI Conclusions An elevated SIRI (≥2.3x109) is an independent prognostic factor for survival in patients with metastatic pancreatic cancer. Patients with an elevated SIRI (≥2.3x109) show an increased benefit from mFOLFIRINOX in comparison to other first line chemotherapy regimens. Background Immunotherapy is a new and effective form of therapy in several types of cancer, although molecular predictors of response are limited. Recently, we have described that immune-related adverse-events (irAEs) are significantly associated with immunotherapy efficacy, and it has been suggested that body mass index (BMI) might be a favourable factor. We have studied whether excess weight influences the efficacy outcomes of immunotherapy and whether it is associated with immune-related adverse events. We have also evaluated the combined effect of overweight and irAEs. Methods We calculated BMI in patients treated with single-agent anti-PD-1 antibodies for advanced cancer. Efficacy of anti-PD-1 treatment was evaluated with both objective response (OR) rate, and progression-free survival (PFS), and toxicity with irAEs. We established the association between overweight and OR, PFS and irAEs. Results One hundred and thirty-two patients were included in the study. Primary malignancies were lung cancer (n = 93), melanoma (n = 12), head and neck carcinoma (n = 9), renal carcinoma (n = 6), urothelial carcinoma (n = 4), Hodgkin’s lymphoma (n = 3), and other cancers (n = 5). Median BMI was 24.9 kg/m2, and 64 patients (48.4%) were excess weight (BMI≥25 kg/m2). An OR was achieved in 50 patients (38.0%), and median PFS was 6 months. IrAEs occurred in 44 patients (33.3%). OR was significantly higher in excess weight patients that in patients with BMI Conclusions Excess weight patients with advanced cancer that receive single-agent anti-PD-1 antibody therapy exhibit a significantly improved clinical outcome compared with patients with normal BMI. Background Malignant bowel obstruction can occur in 18% of cases. SEMS can be an alternative to surgery. BV has been associated with bowel perforation but data on the safety of SEMS for occlusive colon cancer during BV-containing regimens are lacking. Methods Retrospective analysis of 136 patients with malignant bowel obstruction who underwent placement of SEMS between 2012 and 2017 in two hospitals. Chemotherapy and BV-containing regimens, stent-related complications and outcomes were recorded. Results 58 (42.6%) SEMS were placed as a bridge to surgery and 78 (57.4%) as a palliative intent for stage IV disease. 129 patients (94.9%) had a left-sided obstruction. Technical success was achieved in 133 of 136 (98%). Overall, major stent-related complications after two-weeks of insertion were observed in 36 patients (26.5%): Re-obstruction in 18 patients (50%), 11 perforations (30.5%), 5 bleeding (14%) and 2 migrations (5.5%). Complications in the metastatic group (n = 78) 14 (18%) had re-obstructions, 7 (9%) perforations, 4 (5%) minor bleeding and 2 (2.5%) migrations. Multivariate analysis identified stage IV and chemotherapy as significant risk factors for stent-related complications. Patients who received systemic therapy are more likely to develop complications (OR 1.84; CI95% 1.29-6.22, p = 0.0007). 16 patients received BV, 2 (12.5%) had a perforation. Univariate analysis did not show that BV was more likely to develop perforations, although the incidence was higher in this subset of patients. No differences were observed between chemotherapy alone and BV in overall complications. Kaplan-Meier analysis showed significant longer OS for patients treated with systemic therapy (27 vs. 11 months, p = Conclusions Placement of SEMS is effective and relatively safe but, with an overall complication rate of 26%. The major early risk is perforation that can increase up to 12% during BV treatment. In patients with obstructing advanced colorectal cancer that would benefit from SEMS, we should consider the risks associated with systemic therapies, taking into account the improvement in survival with BV. Presentamos tres pacientes afectos de tumores germinales malignos, primitivos del mediastino, de origen no seminomatoso. Remarcamos los signos radiologicos mas constantes de estos tumores. Si bien los hallazgos clinico-radiologicos no permiten distinguirlos de otras lesiones mediastinicas, debemos sospechar su presencia cuando a un varon joven y asintomatico se le descubre casualmente en una radiografia una gran masa en el mediastino anterior. Hacemos hincapie en la utilidad de la tomografia computadorizada en el diagnostico y seguimiento de estos tumores.
