Abstract: Purpose: To study the treatment and outcomes of children with retinoblastoma (RB) with extraocular tumor extension (RB-EOE) and compare them with RB without extraocular tumor extension (RB-w/o-EOE). Design: Multicenter intercontinental collaborative prospective study from 2017 to 2020. RB-EOE cases included those with overt orbital tumor extension in treatment-naive patients. Cases with microscopic orbital extension detected postenucleation were excluded from the study. Participants: A total of 319 children with RB-EOE and 3116 children with RB-w/o-EOE. Intervention: Chemotherapy, enucleation, exenteration, radiotherapy. Main Outcome Measures: Systemic metastasis and death. Results: Of the 3435 RB patients included in this study, 309 (9%) were from low-income countries (LIC), 1448 (42%) from lower-middle income, 1012 (29%) from upper-middle income, and 666 (19%) patients from high-income countries. There was an inverse relationship between the percentage of RB-EOE and national income level, with 96 (31%) patients from LIC, 197 (6%) lower-middle income, 20 (2%) upper-middle income, and 6 (1%) patients from high-income countries (P = 0.0001). The outcomes were statistically significant for RB-EOE compared with RB-w/o-EOE: systemic metastasis (32% vs. 4% respectively; P = 0.0001) and metastasis-related death (63% vs. 6% respectively; P = 0.0001). Multimodal treatment was the most common form of treatment (n = 177; 54%) for RB-EOE, with most cases undergoing a combination of intravenous chemotherapy and enucleation (n = 97; 30%). Adjuvant external beam radiotherapy (EBRT) after surgery (enucleation/orbital exenteration) was given in only 68 (21%) cases. Kaplan–Meier analysis for systemic metastasis and metastasis-related death in RB-EOE was 28% and 57% at 1 year, 29% and 60% at 2 years, and 29% and 61% at 3 years, respectively. 3. Influence of winter waves on the longitudinal growth of the Punta Banda Estuary sandbar, Todos Santos Bay, Baja California, Mexico

Abstract: Punta Banda Estuary, located within Todos Santos Bay, on the northwest coast of the Baja California Peninsula, is separated from the bay by a single sandbar, approximately 8 km long. From 1972 to 2003 this sandbar showed a longitudinal net growth of 420 m, which decreased the width of the estuary mouth by more than 400 m. Despite the bar's rapid growth it has been suggested that the estuary mouth remains open because in spring, summer and autumn the longshore sand transport is northeastward, but during winter the longshore sand transport should be toward the southwest, away from the mouth, establishing a balance between the amount of sediment transported both ways. In this study we analyze wave data acquired from November 2004 to March 2005 on the bay side of the sandbar, and for the first time provide the magnitude and direction of the longshore sand transport during winter. The results show that during this season the dominant waves approach the bay from the west-northwest and the sandbar from the west, inducing a net longshore sand transport of 817 m3 day–1 to the northeast. This result contradicts the hypothesis of a longshore sand transport balance, and shows that throughout a typical year the net longshore sand transport is to the northeast. The fact that the estuary mouth has not closed up should be attributed to other causes, such as extraordinary amounts of rain during the rainy season or inequalities between the ebb and flood tidal current velocities to the estuary.

Abstract: Summary: Background: We undertook a Grand Challenges in Global Eye Health prioritisation exercise to identify the key issues that must be addressed to improve eye health in the context of an ageing population, to eliminate persistent inequities in health-care access, and to mitigate widespread resource limitations. Methods: Drawing on methods used in previous Grand Challenges studies, we used a multi-step recruitment strategy to assemble a diverse panel of individuals from a range of disciplines relevant to global eye health from all regions globally to participate in a three-round, online, Delphi-like, prioritisation process to nominate and rank challenges in global eye health. Through this process, we developed both global and regional priority lists. Findings: Between Sept 1 and Dec 12, 2019, 470 individuals complete round 1 of the process, of whom 336 completed all three rounds (round 2 between Feb 26 and March 18, 2020, and round 3 between April 2 and April 25, 2020) 156 (46%) of 336 were women, 180 (54%) were men. The proportion of participants who worked in each region ranged from 104 (31%) in sub-Saharan Africa to 21 (6%) in central Europe, eastern Europe, and in central Asia. Of 85 unique challenges identified after round 1, 16 challenges were prioritised at the global level; six focused on detection and treatment of conditions (cataract, refractive error, glaucoma, diabetic retinopathy, services for children and screening for early detection), two focused on addressing shortages in human resource capacity, five on other health service and policy factors (including strengthening policies, integration, health information systems, and budget allocation), and three on improving access to care and promoting equity. Interpretation: This list of Grand Challenges serves as a starting point for immediate action by funders to guide investment in research and innovation in eye health. It challenges researchers, clinicians, and policy makers to build collaborations to address specific challenges. The methodological quality of the studies was assessed with the Jadad scale. Results: We included 13 studies, all of which had a low methodological quality (Jadad scale score ≤2). In these studies, high-intensity, high-frequency auditory cues had a positive impact on gait initiation and turning. More specifically, they (1) improved spatiotemporal and kinematic parameters; (2) decreased freezing, turning duration, and falls; and (3) increased gait initiation speed, muscle activation, and gait speed and cadence in patients with Parkinson's disease. Conclusions: We need studies of better methodological quality to establish the Parkinson's disease stage in which auditory cues are most beneficial, as well as to determine the most effective type and frequency of the auditory cue during gait initiation and turning in patients with Parkinson's disease. Abstract: Resumen: Introducción: La mHealth, definida como la prestación de información o asistencia sanitaria a través del uso de dispositivos móviles o tabletas, se postula como una de las grandes apuestas para pacientes, proveedores e inversores. Una app es un programa, con unas características especiales, que se instala en un dispositivo móvil, ya sea tableta digital o teléfono inteligente, y que suele tener un tamaño reducido, y cuyo objetivo es facilitar la consecución de una tarea determinada o asistir en gestiones diarias, siendo el modo de interacción entre el usuario y la aplicación el tacto. Objetivo: El objetivo del presente trabajo fue realizar una revisión sistemática acerca de la información publicada sobre las apps enfocadas al campo de la neurorrehabilitación, con el fin de clasificarlas y llevar a cabo una descripción de las principales características de las mismas. Material y métodos: Se realiza una revisión sistemática mediante búsqueda bibliográfica en bases de datos biomédicas, así como en otras fuentes de información propias del ámbito de las aplicaciones tipo apps. Se clasificaron las apps en 5 categorías: hábitos saludables, informativas, valoración, tratamiento y específicas. Conclusiones: Existe gran cantidad de apps con potencial uso en el campo de la neurorrehabilitación, por lo que es importante que los desarrolladores y diseñadores apps conozcan cuáles son las necesidades de la población con patología neurológica para que sus productos sean válidos y eficaces en dicho contexto. Del mismo modo, los profesionales, pacientes, familiares y cuidadores deberían disponer de criterios claros e indicadores que pudieran ayudarles a seleccionar las aplicaciones óptimas para sus necesidades concretas. Abstract: Introduction: Mobile health or mHealth, defined as the provision of health information or healthcare by means of mobile devices or tablets, is emerging as a major game-changer for patients, care providers, and investors. An app is a program with special characteristics installed on a small mobile device, either a tablet or smartphone, with which the user interacts via a touch-based interface. The purpose of the app is to facilitate completion of a certain task or assist with daily activities. Objective: The aim of this study was to conduct a systematic review of published information on apps directed at the field of neurorehabilitation, in order to classify them and describe their main characteristics. Material and methods: A systematic review was carried out by means of a literature search in biomedical databases and other information sources related to mobile applications. Apps were classified into five categories: health habits, information, assessment, treatment, and specific uses. Conclusions: There are numerous applications with potential for use in the field of neurorehabilitation, so it is important that developers and designers understand the needs of people with neurological disorders so that their products will be valid and effective in light of those needs. Similarly, professionals, patients, families, and caregivers should have clear criteria and indicators to help them select the best applications for their specific situations. Abstract: Cross-sectionally, older age and obesity are associated with increased coronavirus disease-2019 (COVID-19) risk. We assessed the longitudinal associations of baseline and changes in adiposity parameters with COVID-19 incidence in older adults at high cardiovascular risk.This analysis included 6874 men and women (aged 55-75 years) with overweight/obesity and metabolic syndrome in the PREDIMED-Plus lifestyle intervention trial for cardiovascular risk reduction. Body weight, body-mass-index (BMI), waist circumference, waist-to-height ratio (WHtR), and a body shape index (ABSI) were measured at baseline and annual follow-up visits. COVID-19 was ascertained by an independent Event Committee until 31 December 2021. Cox regression models were fitted to evaluate the risk of COVID-19 incidence based on baseline adiposity parameters measured 5-6 years before the pandemic and their changes at the visit prior to censoring.At the time of censoring, 653 incident COVID-19 cases occurred. Higher baseline body weight, BMI, waist circumference, and WHtR were associated with increased COVID-19 risk. During the follow-up, every unit increase in body weight (HRadj (95%CI): 1.01 (1.00, 1.03)) and BMI (HRadj: 1.04 (1.003, 1.08)) was associated with increased COVID-19 risk.In older adults with overweight/obesity, clinically significant weight loss may protect against COVID-19.This study is registered at the International Standard Randomized Controlled Trial (ISRCT; http://www.isrctn.com/ISRCTN89898870 ).© 2023. Abstract: Anemia causes hypo-oxygenation in the brain, which could lead to cognitive disorders. We examined dietary iron intake as well as anemia markers (i.e., hemoglobin, hematocrit, mean corpuscular volume) and diabetes coexistence in relation to neuropsychological function and quality of life. In this study, 6117 community-dwelling adults aged 55-75 years (men) and 60-75 years (women) with overweight/obesity and metabolic syndrome were involved. We performed the Mini-Mental State Examination (MMSE), the Trail Making Test parts A and B (TMT-A/B), Semantic Verbal Fluency of animals (VFT-a), Phonological Verbal Fluency of letter P (VFT-p), Digit Span Test (DST), the Clock Drawing Test (CDT), and the Short Form-36 Health Survey (SF36-HRQL test). Dietary iron intake did not influence neuropsychological function or quality of life. However, anemia and lower levels of anemia markers were associated with worse scores in all neurophysiological and SF36-HRQL tests overall, but were especially clear in the MMSE, TMT-B (cognitive flexibility), and the physical component of the SF36-HRQL test. The relationships between anemia and diminished performance in the TMT-A/B and VFT tasks were notably pronounced and statistically significant solely among participants with diabetes. In brief, anemia and reduced levels of anemia markers were linked to inferior cognitive function, worse scores in different domains of executive function, as well as a poorer physical, but not mental, component of quality of life. It was also suggested that the coexistence of diabetes in anemic patients may exacerbate this negative impact on cognition. Abstract: The tumor suppressor BRCA1 regulates the DNA damage response (DDR) and other processes that remain incompletely defined. Among these, BRCA1 heterodimerizes with BARD1 to ubiquitylate targets via its N-terminal E3 ligase activity. Here, it is demonstrated that BRCA1 promotes oxidative metabolism by degrading Oct1 (POU2F1), a transcription factor with proglycolytic and tumorigenic effects. BRCA1 E3 ubiquitin ligase mutation skews cells toward a glycolytic metabolic profile while elevating Oct1 protein. CRISPR-mediated Oct1 deletion reverts the glycolytic phenotype. RNA sequencing (RNAseq) confirms deregulation of metabolic genes downstream of Oct1. BRCA1 mediates Oct1 ubiquitylation and degradation, and mutation of two ubiquitylated Oct1 lysines insulates the protein against BRCA1-mediated destabilization. Oct1 deletion in MCF-7 breast cancer cells does not perturb growth in standard culture, but inhibits growth in soft agar and xenograft assays. In primary breast cancer clinical specimens, Oct1 protein levels correlate positively with tumor aggressiveness and inversely with BRCA1. Abstract: Background Apprehensions of undocumented immigrants in the Rio Grande Valley sector of the U.S.-Mexico border have grown to account for nearly half of all apprehensions at the border. The purpose of this study is to report the prevalence, mechanism, and pattern of traumatic injuries sustained by undocumented immigrants who crossed the U.S.-Mexico border at the Rio Grande Valley sector over a span of 5 years and were treated at a local American College of Surgeons verified Level II trauma center. Methods A retrospective chart review was conducted from January 2014 to December 2019. Demographics, comorbidities, injury severity score (ISS), mechanism of injury, anatomical part of the body affected, hospital and ICU length of stay (LOS), and treatment costs were analyzed. Descriptive statistics for demographics, injury location and cause, and temporal trends are reported. The impact of ISS or surgical intervention on hospital LOS was analyzed using an analysis of covariance (ANCOVA). Results Of 178 patients, 65.2% were male with an average age of 31 (range 0–67) years old and few comorbidities (88.8%) or social risk factors (86%). Patients most commonly sustained injuries secondary to a border fence-related incident (33.7%), fleeing (22.5%), or motor vehicle accident (16.9%). There were no clear temporal trends in the total number of patients injured, or in causes of injury, between 2014 and 2019. The majority of patients (60.7%) sustained extremity injuries, followed by spine injuries (20.2%). Border fence-related incidents and fleeing increased risk of extremity injuries (Odds ratio (OR) > 3; p < 0.005), whereas motor vehicle accidents increased risk of head and chest injuries (OR > 4; p < 0.004). Extremity injuries increased the odds (OR: 9.4, p p Conclusion In addition to border fence related injuries, undocumented immigrants also sustained injuries while fleeing and in motor vehicle accidents, among others. Extremity injuries, which were more likely with border fence-related incidents, were the most common type. This type of injury often requires surgical intervention and, therefore, a longer hospital stay for severe injuries.

Abstract: Resumen: Introducción: En las últimas décadas ha existido un especial interés por las teorías que podrían explicar el gobierno del control motor y sus aplicaciones. Estas teorías suelen basarse en modelos de función cerebral, reflejando criterios filosóficamente diferentes sobre la forma en la que el movimiento es controlado por el cerebro, enfatizando cada una de ellas en los distintos componentes neurales del movimiento. Asimismo, en el contexto de las neurociencias, toma relevancia el concepto de aprendizaje motor, considerado como el conjunto de procesos internos asociados a la práctica, y la experiencia, que producen cambios relativamente permanentes en la capacidad de producir actividades motoras, a través de una habilidad específica. Por lo que ambos, control y aprendizaje motor, se posicionan como campos de estudio fundamentales para los profesionales sanitarios en el campo de la neurorrehabilitación. Desarrollo: Se describen las principales teorías de control motor como la teoría de la programación motora, la teoría de sistemas, la teoría de la acción dinámica o la teoría del procesamiento de distribución en paralelo, así como los factores que influyen en el aprendizaje motor y sus aplicaciones en neurorrehabilitación. Conclusiones: En la actualidad no existe un consenso sobre qué teoría o modelo es definitorio en dar explicación al gobierno del control motor. Las teorías sobre el aprendizaje motor deben ser la base para la rehabilitación motora. Las nuevas líneas de investigación deben aplicar los conocimientos generados en los campos del control y aprendizaje motor en neurorrehabilitación. These theories are often based on models of brain function, philosophically reflecting different criteria on how movement is controlled by the brain, each being emphasised in different neural components of the movement. The concept of motor learning, regarded as the set of internal processes associated with practice and experience that produce relatively permanent changes in the ability to produce motor activities through a specific skill, is also relevant in the context of neuroscience. Thus, both motor control and learning are seen as key fields of study for health professionals in the field of neuro-rehabilitation. Development: The major theories of motor control are described, which include, motor programming theory, systems theory, the theory of dynamic action, and the theory of parallel distributed processing, as well as the factors that influence motor learning and its applications in neuro-rehabilitation. Conclusions: At present there is no consensus on which theory or model defines the regulations to explain motor control. Theories of motor learning should be the basis for motor rehabilitation. The new research should apply the knowledge generated in the fields of control and motor learning in neuro-rehabilitation. Abstract: Introduction: The limitations in performing functional activities in children and adolescents with cerebral palsy are important. The use of virtual reality systems is a new treatment approach that reinforces task-oriented motor learning. The purpose of this guide is to study the impact of the use of virtual reality systems in the improvement and acquisition of functional skills, and to evaluate the scientific evidence to determine the strength of recommendation of such interventions. Development: All available full-text articles, regardless of their methodology, were included. The following databases were consulted: PubMed (Medline), PEDro, EMBASE (OVID-Elsevier), Cochrane Library, Medline (OVID), CINAHL, ISI Web Knowledge. An assessment was made of methodological quality, the level of scientific evidence, and the strength of recommendations using the tools: Critical Review Form for Quantitative Studies and the Guidelines for Critical Review Form for Quantitative Studies and U.S. Preventive Services Task Force. Finally, we included 13 articles and 97 participants were recruited. We obtained significant improvements in outcome measures that assessed postural control and balance, upper limb function, the selective joint control, and gait. Conclusions: The guide has some limitations: the limited number of patients enrolled, clinical diversity and age range, as well as the methodological quality of existing trials. Virtual reality is a promising tool in the treatment of children with cerebral palsy. There is strong scientific evidence of an acceptable recommendation for the use of virtual reality systems in the treatment of cerebral palsy. Resumen: Introducción: Las limitaciones para realizar actividades funcionales en niños y adolescentes con parálisis cerebral son importantes. El empleo de sistemas de realidad virtual constituye un nuevo enfoque de tratamiento que refuerza el aprendizaje motor orientado a tareas. El objetivo del presente trabajo consiste en analizar qué repercusión tiene el empleo de sistemas de realidad virtual en la mejora y adquisición de habilidades funcionales; y evaluar la evidencia científica existente para determinar qué fuerza de recomendación tienen dichas intervenciones. Desarrollo: Se incluyeron todos los artículos disponibles a texto completo independientemente de su metodología. Se consultaron las siguientes bases de datos: Pubmed (Medline), PEDro, Embase (OVID-Elsevier), Cochrane Library Plus, Medline (OVID), CINHAL, ISI web Knowledge. Se evaluaron la calidad metodológica, el nivel de evidencia científica y la fuerza de las recomenda-ciones con las herramientas: Critical Review Form-Quantitative Studies and the Guidelines for Critical Review Form-Quantitative Studies y U.S. Preventive Services Task Force. Finalmente, se incluyeron 13 artículos y se reclutó a 97 participantes. Se obtuvieron mejoras significativas en medidas de resultado que evalúan el control postural y el equilibrio, la funcionalidad del miembro superior, el control selectivo articular y la marcha. Conclusiones: La guía posee algunas limitaciones: número de pacientes reclutados, diversidad clínica y rango de edad; así como la calidad metodológica de los ensayos existentes. La realidad virtual es una prometedora herramienta en el tratamiento de nĩnos con parálisis cerebral. Abstract: Introduction: Cerebral palsy (CP) is one of the most common developmental disorders. Technological development has enabled a transformation of the healthcare sector, which can offer more individualised, participatory, and preventive services. Within this context of new technology applied to the healthcare sector, mobile applications, or apps, constitute a very promising tool for the management of children with CP. Objective: The purpose of this article is to perform a systematic review of the information published about various mobile applications either directly related to CP or with potential to be useful in the context of the disease, and to describe, analyse, and classify these applications. Material and methods: A literature search was carried out to gather articles published in English or Spanish between 2011 and 2017 which presented, analysed, or validated applications either specifically designed or potentially useful for CP. Furthermore, a search for mobile applications was conducted in the main mobile application markets. Conclusions: A total of 63 applications were found in biomedical databases and mobile application markets, of which 40 were potentially useful for CP and 23 were specifically designed for the condition (11 for information, 3 for evaluation, and 9 for treatment). There are numerous mobile applications either specifically designed for or with potential to be useful in the field of CP. However, despite the existing scientific evidence, the low methodological quality of scientific articles makes it impossible to generalise the use of these tools. Resumen: Introducción: La parálisis cerebral infantil (PCI) es una de las discapacidades más comunes del desarrollo. Gracias a la tecnología, el sector sanitario tiene la posibilidad de transformarse para ofrecer servicios más individualizados, participativos y preventivos. Dentro de este contexto de nuevas tecnologías aplicadas al sector sanitario, el fenómeno de las aplicaciones móviles o app resulta muy prometedor en la PCI. Objetivo: El objetivo del presente trabajo es realizar una revisión sistemática acerca de la información publicada sobre las diferentes aplicaciones móviles creadas, relacionadas directamente con la PCI o con utilidad potencial en ella, con el fin de describirlas, analizarlas y clasificarlas para su mejor conocimiento. Material y métodos: Se llevó a cabo una búsqueda bibliográfica que incluyó artículos publicados en inglés o castellano, del año 2011 al 2017, y que presentasen, analizasen o validasen un sistema basado en una app con utilidad o diseño específico para la PCI. A su vez, se llevó a cabo una búsqueda de aplicaciones móviles en los principales mercados de aplicaciones móviles. Conclusiones: Se encontraron 63 aplicaciones en bases de datos biomédicas y en mercados específicos de app, de las cuales 40 presentaban potencial utilidad en la PCI y 23 estaban diseñadas específicamente para la PCI: 11 de estas eran de carácter informativo, 3 de valoración y 9 de tratamiento. Existe un gran número de aplicaciones móviles con potencial utilidad, así como con diseño específico en el campo de la PCI. Sin embargo, pese a la cierta evidencia científica existente, la baja calidad metodológica de los artículos científicos hace que no se pueda generalizar su uso.

Abstract: El clima es uno de los factores que influyen en el comportamiento del suelo y tiene gran importancia en el proceso de meteorización. En la presente investigación se analizó un suelo en un clima tropical húmedo, con el fin de establecer su estado de meteorización y el efecto que dicha meteorización tiene en las propiedades mecánicas del suelo; para tal fin se caracterizó el material mediante ensayos de laboratorio específicos para suelos tropicales, cuyos resultados permiten describir un suelo laterítico joven con baja cantidad de sesquióxidos y pocas agregaciones, que le confieren una baja erodabilidad interna y poca capacidad de desagregación

37. Aplicaciones móviles para lesión medular. Una revisión sistemática

Abstract: Resumen Introduccion La lesion medular congenita o adquirida produce deficiencias motoras, sensitivas y autonomicas. Por tanto, la recuperacion de la funcion motora es uno de los factores mas importantes en el proceso de rehabilitacion. En este contexto, las aplicaciones moviles se plantean como herramientas que pueden facilitar el proceso de rehabilitacion. Objetivo Conocer el efecto del uso de aplicaciones moviles sobre la funcion motora en personas con lesion medular. Materiales y metodos Se realizo una busqueda sistematica en la literatura acerca del uso de aplicaciones moviles para personas (en ingles, applications o apps) para personas con lesion medular de cualquier edad y origen, en diferentes bases de datos, cuya elegibilidad se realizo mediante la estrategia CONSORT y la calidad metodologica se evaluo aplicando la escala PEDro. Asimismo, se realizo una busqueda en los principales mercados de aplicaciones moviles para identificar la utilidad de estas en la recuperacion de la funcion motora. Resultados Se incluyeron dos articulos que cumplian con los criterios de inclusion, con un total de participacion de 46 personas, 18 asignados al grupo control y 28 que recibieron intervencion usando aplicaciones moviles. El grupo de aplicaciones moviles mostro mejorias en la funcion motora tras la intervencion. La busqueda en los mercados de aplicaciones moviles arrojo 65 apps, de las cuales 27 se centraron especificamente en la funcion motora de personas con lesion medular y 40 tenian potencial utilidad sobre la misma. Conclusion Debido al numero de estudios y a la calidad metodologica de los mismos, no son concluyentes los efectos del uso de aplicaciones moviles sobre la funcion motora en personas con lesion medular. Aun asi, existe una gran cantidad de aplicaciones moviles no validadas en mercados de aplicaciones moviles que fueron especificamente disenadas o que tienen potencial utilidad para mejorar la funcion motora de personas con lesion medular.

Abstract: Introducción: el adenoma serrado sésil (ASS) es una lesión descrita recientemente y que puede estar relacionada con el desarrollo de hasta un 15% de los cánceres colorrectales (CCR). Objetivo: determinar la eficacia de los criterios morfológicos para el diagnóstico del ASS evaluando el grado de acuerdo entre patólogos. Material y métodos: se estudió la concordancia entre dos patólogos para el diagnóstico de las lesiones serradas de colon en 195 lesiones (187 pólipos hiperplásicos y 7 adenomas serrados). De cada lesión se recogió el tamaño, la localización, la morfología y la forma de obtención de la muestra. Los dos patólogos eran desconocedores del diagnóstico inicial, las características macroscópicas y la localización de las lesiones. Los posibles diagnósticos fueron: ASS, adenoma serrado tradicional (AST), pólipo hiperplásico (PH), pólipo serrado, adenoma tubular o formas mixtas. Las dudas diagnósticas debían describirse. La concordancia entre los dos observadores se evaluó mediante el índice kappa (κ). También se evaluó la influencia de las variables recogidas de las lesiones en el grado de acuerdo en el diagnóstico. Resultados: el acuerdo global para el diagnóstico histológico fue pobre (κ = 0,14). También lo fue el acuerdo para el diagnóstico de ASS (κ = 0,23). La concordancia para el diagnóstico de ASS mejoró con el tamaño > 5 mm (κ = 0,64) y para la localización proximal (κ = 0,43). Conclusión: en un contexto clínico real, los criterios morfológicos existentes para la identificación del ASS pueden ser de difícil aplicación.Introduction: sessile serrated adenoma (SSA) is a recently described lesion that may be related to the development of up to 15% of colorectal cancers (CRCs). Objective: to determine the accuracy of morphological criteria for the diagnosis of SSA by assessing concordance between pathologists. Material and methods: concordance between two pathologists in the diagnosis of serrated lesions of the colon was studied for 195 lesions (187 hyperplastic polyps and 7 serrated adenomas). Size, location, morphology, and sampling method were collected of each lesion. Both pathologists were unaware of the previous diagnosis, macroscopic characteristics, and location of lesions. Possible diagnoses were: SSA, traditional serrated adenoma (TSA), hyperplastic polyp (HP), serrated polyp, tubular adenoma, or mixed lesions. Diagnostic doubts had to be described. Concordance between both observers was assessed using the kappa index (κ). The influence of collected variables on concordance degree was also evaluated. Results: overall agreement on the histological diagnosis was poor (κ = 0.14), and so was agreement on the diagnosis of SSA (κ = 0.23). Concordance in the diagnosis of SSA improved with size > 5 mm (κ = 0.64) and proximal location (κ = 0.43). Conclusion: in a real clinical setting, the existing morphological criteria for SSA identification may be difficult to use.

Abstract: Introduction and objectives: This report presents the findings of the 2018 Spanish Catheter Ablation Registry. Methods: Data collection was retrospective. A standardized questionnaire was completed by each of the participating centers. Results: Data sent by 100 centers were analyzed, with a total number of 16,566 ablation procedures performed (the highest historically reported in this registry) for a mean of 165.5 ± 127.9 and a median of 119 procedures per center. The ablation targets most frequently treated were atrial fibrillation (n = 4234; 25.6%), atrioventricular nodal re-entrant tachycardia (n = 3525; 21.3%) and cavotricuspid isthmus (n = 3425; 20.7%). A new peak was observed in the ablation of atrial fibrillation, increasing the distance from the other substrates. The overall success rate was 91%. The rate of major complications was 2.2%, and the mortality rate was 0.04%. A total of 2.1% of the ablations were performed in pediatric patients. Conclusions: The Spanish Catheter Ablation Registry systematically and continuously enrolls the ablation procedures performed in Spain, showing a progressive increase in the number of ablations over the years, with a high success rate and low percentage of complications.

Abstract: Resumen Introduccion Nuevas alternativas terapeuticas estan emergiendo en el ambito de la tecnologia, mediante el uso de smartphones en la enfermedad de Alzheimer (EA). Objetivo Realizar una revision sistematica de la literatura sobre el uso de app en la EA, asi como revisar los mercados de app, al objeto de conocer las principales aplicaciones moviles con el fin de clasificarlas y describirlas en el contexto de la EA. Materiales y metodo Revision sistematica de articulos desde el ano 2000 hasta el 2018, sobre app relacionadas con la EA. Paralelamente, se llevo a cabo una busqueda de aplicaciones moviles en los principales mercados de aplicaciones moviles. Resultados Se hallaron 294 aplicaciones, de las cuales 178 se clasificaron con potencial utilidad en la EA, y 116 con un diseno especifico para la EA (73 app de informacion, 8 de valoracion, 31 de tratamiento, 3 de informacion y valoracion y una app de valoracion y tratamiento). Conclusiones Existe un gran numero de app con potencial utilidad, asi como con diseno especifico en el campo de la EA. Sin embargo, el escaso numero de articulos cientificos y su baja calidad hacen que no se pueda generalizar su uso. Por todo esto, son necesarias futuras investigaciones de regulacion promovidas por los organismos sanitarios en el contexto de la EA.

Abstract: Background Diagnosing pulmonary embolisms can be challenging, given that its presentation shares clinical manifestations with other conditions. For patients 80 years of age or older, the healthcare challenge increases due to associated comorbidity when compared with younger patients ( Objective The aim of our study was to report the incidence of pulmonary embolism in elderly patients in our setting and identify differences in the clinical manifestations, comorbidity and laboratory parameters between these 2 groups of patients. Method We conducted a hospital-based, case–control study to review the pulmonary embolisms diagnosed in our center using computed tomography pulmonary angiography between 2013 and 2016. Results The study included 413 patients, 124 of whom were 80 years of age or older, with a median age of 72 years (IQR, 58–81). The typical presentation triad was uncommon and showed no differences between subgroups. The main symptoms presented by these groups were dyspnea (73.4% vs. 63.7% for the elderly and younger groups, respectively; p = .055), chest pain (26.6% vs. 39.5%; p = .013) and cough/haemoptysis (31.1% vs. 18.9%; p = .021). The elderly group had more comorbidity (4.88 ± 2.55 vs. 2.89 ± 2.85; p = .0001). Conclusion Thirty percent of the pulmonary embolisms occurred in the elderly group. The typical presentation form was uncommon. We detected significant differences in individual symptoms, comorbidity and laboratory parameters compared with the younger patients.

Abstract: BACKGROUND: Malaria was eliminated in Spain in 1964. Since then, more than 10,000 cases of malaria have been reported, mostly in travellers and migrants, making it the most frequently imported disease into this country. In order to improve knowledge on imported malaria cases characteristics, the two main malaria data sources were assessed: the national surveillance system and the hospital discharge database (CMBD). METHODS: Observational study using prospectively gathered surveillance data and CMBD records between 2002 and 2015. The average number of hospitalizations per year was calculated to assess temporal patterns. Socio-demographic, clinical and travel background information were analysed. Bivariate and multivariable statistical methods were employed to evaluate hospitalization risk, fatal outcome, continent of infection and chemoprophylaxis failure and their association with different factors. RESULTS: A total of 9513 malaria hospital discharges and 7421 reported malaria cases were identified. The number of reported cases was below the number of hospitalizations during the whole study period, with a steady increase trend in both databases since 2008. Males aged 25-44 were the most represented in both data sources. Most frequent related co-diagnoses were anaemia (20.2%) and thrombocytopaenia (15.4%). The risks of fatal outcome increased with age and were associated with the parasite species (Plasmodium falciparum). The main place of infection was Africa (88.9%), particularly Equatorial Guinea (33.2%). Most reported cases were visiting friends and relatives (VFRs) and immigrants (70.2%). A significant increased likelihood of hospitalization was observed for children under 10 years (aOR:2.7; 95% CI 1.9-3.9), those infected by Plasmodium vivax (4.3; 95% CI 2.1-8.7) and travellers VFRs (1.4; 95% CI 1.1-1.7). Only 4% of cases reported a correct regime of chemoprophylaxis. Being male, over 15 years, VFRs, migrant and born in an endemic country were associated to increased risk of failure in preventive chemotherapy. CONCLUSIONS: The joint analysis of two data sources allowed for better characterization of imported malaria profile in Spain. Despite the availability of highly effective preventive measures, the preventable burden from malaria is high in Spain. Pre-travel advice and appropriately delivered preventive messages needs to be improved, particularly in migrants and VFRs. Abstract: Approximately, half of the stroke patients suffered a fall during the first year after the injury, affecting its functionality. Gait evaluation scales could evaluate to predict the risk of falls.To study the ability of the Wisconsin Gait Scale (WGS) to predict the risk of falls in stroke subjects.Retrospective observational study that included stroke patients between May 2010 and October 2016. Data from the WGS and the Berg Balance Scale (BBS) were collected after hospitalization (acute phase), at 3 months (subacute phase) and at 6 and 12 months (chronic phases). BBS was used as the outcome variable to determine the ability of the WGS to predict the risk of falls. The calculation of the predictive validity was carried out using the ROC (Receiver Operating Characteristics) curves and the analysis of the area under the curve (AUC, area under the curve).Data were collected from 61 patients who met the inclusion criteria (after admission: 61; 3 months: 61; 6 months: 58; 12 months: 42). In all measurements, the AUC was greater than 0.9, so the WGS distinguishes stroke patients with moderate risk of falling from those with low risk.The WGS scale is a valid instrument to predict the risk of falls in patients with a criterion in relation to the cut-off points established in the BBS.Validez de la Wisconsin Gait Scale para predecir el riesgo de caídas en pacientes con ictus.Introducción. Aproximadamente, la mitad de las personas que han sufrido un ictus sufre una caída durante el primer año tras la lesión, lo que afecta a su funcionalidad. Las escalas de evaluación de la marcha podrían utilizarse para predecir el riesgo de caídas. Objetivo. Estudiar la capacidad de la Wisconsin Gait Scale (WGS) para predecir el riesgo de caídas en pacientes con ictus. Pacientes y métodos. Estudio observacional retrospectivo que incluyó a pacientes con ictus entre mayo de 2010 y octubre de 2016. Se recopilaron datos de la WGS y de la Berg Balance Scale (BBS) tras la hospitalización (fase aguda), a los tres meses (fase subaguda) y a los seis y 12 meses (fases crónicas). Se empleó la BBS como variable de resultado para determinar la capacidad de la WGS para predecir el riesgo de caídas. El cálculo de la validez predictiva se llevó a cabo mediante las curvas receiver operating characteristics y el análisis del área bajo la curva (AUC). Resultados. Se recopilaron datos de 61 pacientes que cumplieron los criterios de inclusión (tras la admisión: 61; a los tres meses: 61; a los seis meses: 58; a los 12 meses: 42). En todas las mediciones, el AUC fue mayor de 0,9, por lo que la WGS distingue a los pacientes con ictus con riesgo moderado de sufrir caídas de los de riesgo bajo. Conclusiones. La WGS es un instrumento válido para predecir el riesgo de caídas en pacientes con un ictus en relación con los puntos de corte establecidos en la BBS.

Materials and methods: We conducted a literature search of articles published in English or Spanish between 2011 and 2016 analysing or validating apps specifically designed for or useful in PD. In addition, we searched for apps potentially useful for PD patients in the leading app stores. Conclusions: The literature and app searches yielded a total of 125 apps, 56 of which were classified as potentially useful in PD and 69 as specifically designed for PD (23 information apps, 29 assessment apps, 13 treatment apps, and 4 assessment and treatment apps). Numerous mobile apps are potentially useful in or specifically designed for PD management. However, scientific evidence of their usefulness is scarce and of poor quality. Further studies are needed to validate these tools and regulate their use. Abstract: Biomethane potential (BMP) test was performed to access the anaerobic degradation of three macrophytes (Eichhornia crassipes, Pistia stratiotes and Salvinia molesta). Sonication pretreatment was evaluated in order to pre-hydrolyze the plants, aiming to remove the lag time and potentially increase the methane production. Two sonication energies were tested: 40,000 and 80,000 kJ kgTS− 1. Results indicated that not all the aquatic plants presented the same behavior. Raw biomass of Eichhornia and Pistia required a long lag-phase (11 and 8 days, respectively), while Salvinia did not, being the faster and most productive raw biomass (64% of biomethanization factor). Pretreated biomass was solubilized, however the results did not have a direct translation to the digestion performance. In fact, no substantial improvement on the amount of methane production occurred. The main experimental evidence was the full reduction of the lag-phase for Eichhornia and Pistia, while Salvinia did not exhibited a substantial change neither in methane production or in kinetics. The high biodegradability of Salvinia biomass without pretreatment indicated that ultrasound pretreatment is not required. Energetic Sustainability Index evidenced that sonication pretreatment process was not sustainable, due to the required high water content of the bulk.
