Restoration loss based on interfacial adhesive defects and associated caries at the restoration margin are the main causes for invasive replacement of dental restorations. Assessment of the interfacial quality based on clinical inspection and radiographic examination is often difficult and not reliable. In this work, we present spectral domain optical coherence tomography (SDOCT) and polarization sensitive optical coherence tomography (PSOCT) for the evaluation of tooth-composite bond failure. Imaging of two composite restorations at the occlusal surface are presented using intensity-based images obtained by SDOCT and PSOCT based degree of polarization uniformity (DOPU). Both modalities revealed several defects beneath the surface such as inhomogeneous adhesive layers, marginal gaps and bubbles. In addition, DOPU representation showed an inhomogeneous structure within the composite material. OCT based imaging of dental restorations could add a valuable diagnostic tool for the evaluation of structural defects in clinical practice. The representation of polarization characteristics with the DOPU algorithm provides further information on the homogeneity of the restoration.

Medical ultrasound (US) is a commonly used modality for image-guided procedures. Recent research systems providing an in situ visualization of 2D US images via an augmented reality (AR) head-mounted display (HMD) were shown to be advantageous over conventional imaging through reduced task completion times and improved accuracy. In this work, we continue in the direction of recent developments by describing the first AR HMD application visualizing real-time volumetric (3D) US in situ for guiding vascular punctures. We evaluated the application on a technical level as well as in a mixed-methods user study with a qualitative prestudy and a quantitative main study, simulating a vascular puncture. Participants completed the puncture task significantly faster when using 3D US AR mode compared to 2D US AR, with a decrease of 28.4% in time. However, no significant differences were observed regarding the success rate of vascular puncture (2D US AR-50% vs. 3D US AR-72%). On the technical side, the system offers a low latency of 49.90 ± 12.92 ms and a satisfactory frame rate of 60 Hz. Our work shows the feasibility of a system that visualizes real-time 3D US data via an AR HMD, and our experiments show, furthermore, that this may offer additional benefits in US-guided tasks (i.e., reduced task completion time) over 2D US images viewed in AR by offering a vividly volumetric visualization.

Purpose: Augmented reality (AR) and head-mounted displays (HMD) in medical practice are current research topics. A commonly proposed use case of AR-HMDs is to display data in image-guided interventions. Although technical feasibility has been thoroughly shown, effects of AR-HMDs on interventions are not yet well researched, hampering clinical applicability. Therefore, the goal of this study is to better understand the benefits and limitations of this technology in ultrasound-guided interventions., Methods: We used an AR-HMD system (based on the first-generation Microsoft Hololens) which overlays live ultrasound images spatially correctly at the location of the ultrasound transducer. We chose ultrasound-guided needle placements as a representative task for image-guided interventions. To examine the effects of the AR-HMD, we used mixed methods and conducted two studies in a lab setting: (1) In a randomized crossover study, we asked participants to place needles into a training model and evaluated task duration and accuracy with the AR-HMD as compared to the standard procedure without visual overlay and (2) in a qualitative study, we analyzed the user experience with AR-HMD using think-aloud protocols during ultrasound examinations and semi-structured interviews after the task., Results: Participants (n = 20) placed needles more accurately (mean error of 7.4 mm vs. 4.9 mm, p = 0.022) but not significantly faster (mean task duration of 74.4 s vs. 66.4 s, p = 0.211) with the AR-HMD. All participants in the qualitative study (n = 6) reported limitations of and unfamiliarity with the AR-HMD, yet all but one also clearly noted benefits and/or that they would like to test the technology in practice., Conclusion: We present additional, though still preliminary, evidence that AR-HMDs provide benefits in image-guided procedures. Our data also contribute insights into potential causes underlying the benefits, such as improved spatial perception. Still, more comprehensive studies are needed to ascertain benefits for clinical applications and to clarify mechanisms underlying these benefits.

Caries, the world's most common chronic disease, remains a major cause of invasive restorative dental treatment. To take advantage of the diagnostic potential of optical coherence tomography (OCT) in contemporary dental prevention and treatment, an intraorally applicable spectral-domain OCT probe has been developed based on an OCT hand-held scanner equipped with a rigid 90°-optics endoscope. The probe was verified in vitro. In vivo, all tooth surfaces could be imaged with the OCT probe, except the vestibular surfaces of third molars and the proximal surface sections of molars within a "blind spot" at a distance greater than 2.5 mm from the tooth surface. Proximal surfaces of 64 posterior teeth of four volunteers were assessed by intraoral OCT, visual-tactile inspection, bitewing radiography and fiber-optic transillumination. The agreement in detecting healthy and carious surfaces varied greatly between OCT and established methods (18.2-94.7%), whereby the established methods could always be supplemented by OCT. Direct and indirect composite and ceramic restorations with inherent imperfections and failures of the tooth-restoration bond were imaged and qualitatively evaluated. The intraoral OCT probe proved to be a powerful technological approach for the non-invasive imaging of healthy and carious hard tooth tissues and gingiva as well as tooth-colored restorations.

Current developments in the field of extended reality (XR) could prove useful in the optimization of surgical workflows, time effectiveness and postoperative outcome. Although still primarily a subject of research, the state of XR technologies is rapidly improving and approaching feasibility for a broad clinical application. Surgical fields of application of XR technologies are currently primarily training, preoperative planning and intraoperative assistance. For all three areas, products already exist (some clinically approved) and technical feasibility studies have been conducted. In teaching, the use of XR can already be assessed as fundamentally practical and meaningful but still needs to be evaluated in large multicenter studies. In preoperative planning XR can also offer advantages, although technical limitations often impede routine use; however, for cases of intraoperative use informative evaluation studies are mostly lacking, so that an assessment is not yet possible in a meaningful way. Furthermore, there is a lack of assessments regarding cost-effectiveness in all three areas. The XR technologies enable proven advantages in surgical workflows despite the lack of high-quality evaluation with respect to the practical and clinical use of XR. New concepts for effective interaction with XR media also need to be developed. In the future, further research progress and technical developments in the field can be expected.

In the context of waste upgrading of polyethylene terephthalate (PET) by pyrolysis, this study presents three on-line mass spectrometric techniques with soft ionization for monitoring the emitted decomposition products and their thermal dependent evolution profiles. Pyrolysis experiments were performed using a thermogravimetric analyzer (TGA) under nitrogen atmosphere with a heating rate of 5 °C/min from 30 °C to 600 °C. Single-photon ionization (SPI at 118 nm/10.5 eV) and resonance enhanced multiple photon ionization (REMPI at 266 nm) were used with time-of-flight mass spectrometry (TOF-MS) for evolved gas analysis (TGA-SPI/REMPI-TOFMS). Additionally, the chemical signature of the pyrolysis products was investigated by atmospheric pressure chemical ionization (APCI) ultra high resolution Fourier Transform ion cyclotron resonance mass spectrometry (FT-ICR MS) which enables assignment of molecular sum formulas (TGA-APCI FT-ICR MS). Despite the soft ionization by SPI, the fragmentation of some compounds with the loss of the [O-CH = CH2] fragment is observed. The major compounds were acetaldehyde (m/z 44), benzoic acid (m/z 122) and a fragment of m/z 149. Using REMPI, aromatic species were selectively detected. Several series of pyrolysis products were observed in different temperature intervals, showing the presence of polycyclic aromatic hydrocarbons (PAHs), especially at high temperatures. FT-ICR MS data showed, that the CHO 4 class was the most abundant compound class with a relative abundance of 45.5%. The major compounds detected with this technique corresponded to m/z 193.0495 (C 10 H 9 O 4 ) and 149.0233 (C + ) and 149.0233 (C 8 H 5 O 3 + ). The aim of this study was to evaluate the ability of spectral-domain optical coherence tomography (SD-OCT) to display the roof of the pulp chamber and to estimate the residual dentin thickness (RDT) of the pulp complex. The roots of 20 extracted human molars were embedded in epoxy resin, and crowns were longitudinally sectioned in the mesial-distal direction, exposing the pulp chamber. The coronal part of the crown was removed up to an RDT to the pulp chamber roof of 2 mm. Samples were imaged by SD-OCT from coronal view and by light microscopy (LM) in the sagittal plane. Using a microtome, dentin was subsequently removed in four levels from the occlusal aspect in steps of 250 μm. At each level, RDT was documented and measured by both methods. The data were compared (Spearman's rho correlation coefficient, Wilcoxon signed-rank test). Using OCT, the roof of the pulp chamber was first displayed at a maximum RDT of 1.94 mm. The minimal RDT that could be imaged by OCT was 0.06 mm. Values from both methods were strongly correlated (r, 0.83-0.95; p i  ≤ 0.05) and differed significantly for large RDTs (dentin levels 1, 2; p i  < 0.05) but not for small RDTs (levels 3, 4; p i  ≥ 0.226). The roof of the dental pulp chamber could be already visualized by SD-OCT with a RDT of 1.94 mm. Therefore, the method could be a useful diagnostic tool during the preparation of deep dentin cavities and might help to preserve the integrity of the pulp chamber.

Objectives: The purpose of this study was to evaluate the effects of artificial aging on thin glass-ceramic occlusal premolar veneers, adhesively bonded to dentin, by examining the changes caused by artificial aging using spectral domain optical coherence tomography (SD-OCT). In addition, the development of cracks in the ceramic veneers and their possible influence on the behavior of the ceramic restorations were examined., Methods: In total, 48 extracted sound upper premolars were prepared in the dentin for occlusal veneers milled from lithium disilicate ceramic blocks (IPS e.max CAD, Ivoclar Vivadent, Liechtenstein). All restorations were adhesively bonded using resin cement (Variolink Esthetic DC, Ivoclar Vivadent). Specimens were 3-dimensionally and 2-dimensionally imaged by SD-OCT (Telesto II, Thorlabs GmbH, Germany), then subjected to thermal-dynamic loading in a chewing simulator with 1,200,000 cycles at a load of 10 kg. Specimens were 2D and 3D imaged again after the artificial aging. Finally, they were subjected to quasi-static loading using a universal testing machine until failure occurred and later examined microscopically to assess the mode of failure. ANOVA test was performed for statistical analysis of data and Tukey's post-hoc test was used to compare the groups at 5% significance level. Chi-Square Test and Fischer's Exact Test of Independence were conducted to test the association between nominal variables., Results: No changes or irregularities were observed in the cement layer or tooth substrate after the aging process. However, wear of the ceramic was noticed at the surface of contact with the antagonist during the test. The development of cracks was detected in 23% of the specimens. Cracks did not affect the fracture strength (p > 0.05) but influenced the mode of failure (p ≤ 0.001)., Significance: Optical coherence tomography allows an easy and non-invasive method to internally scan teeth and restorations. Development of cracks in the ceramic did not affect the fracture strength of the restorations but might lead to a more catastrophic type of failure., (Copyright © 2018 Elsevier Ltd.
