Concussions are increasingly recognized as a public health concern. This paper evaluates Canadian concussion care guidelines, advocating for the inclusion of a broader range of healthcare professionals (HCPs) in concussion assessment, diagnosis, and management. It emphasizes the role of chiropractors, in addition to medical doctors (MDs) and nurse practitioners (NPs), highlighting their extensive training in musculoskeletal and neurological disorders. Chiropractors are adept at managing symptoms like headache, neck pain, and dizziness, and employing evidence-based, comprehensive interventions including patient education, exercise therapy, manual therapy, cervicovestibular rehabilitation, and return-to-sport (RTS) protocols. The paper also addresses regional variations in chiropractors' roles, focusing on Ontario's "Rowan's Law," and argues that limiting aspects of concussion care (assessment, diagnosis, RTS clearance) to MDs and NPs may result in healthcare inefficiencies and inequities. The findings are significant for policymakers and healthcare leaders, indicating a need for updated concussion care guidelines that integrate and utilize diverse HCPs. Although most cases of Achilles tendinitis are caused by some form of biomechanical stress, other factors may play a role in its etiology. Fluoroquinolone use is a wellknown contributing factor to Achilles tendon pathology. This report describes a case of fluoroquinolone-induced Achilles tendinitis in a 77-year-old woman. Eight weeks after she presented to our office, the patient reported significant improvements after a course of care that included low level laser therapy (LLLT), activity modification and graduated exercises. Management can include many of the treatment tools used for biomechanically induced tendinopathy, with particular consideration given to delaying more advanced weightbearing rehabilitative exercise due to the increased risk of rupture. Introduction: Mal de Debarquement Syndrome (MdDS) is a debilitating neurootological disorder. Patients experience almost continuously a perception of self-motion. This syndrome can be motion-triggered (MT-MdDS), such as on a boat, or occur spontaneously or have other triggers (SO-MdDS) in the absence of such motion. Because the pathophysiological mechanism is unknown, treatment options and symptom management strategies are limited. One available treatment protocol involves a readaptation of the vestibular ocular reflex (VOR). This study assesses the effectiveness of vestibulo-ocular reflex (VOR) readaptation in 131 consecutive patients with a fixed protocol. Methods: We administered 131 treatments involving optokinetic stimulation (OKS) paired with a fixed head roll at 0.167 Hz over two to five consecutive days. Each day, four-minute treatment blocks were scheduled twice in the morning and afternoon. Treatment effectiveness was evaluated through questionnaires and posturography. Results: We observed significant improvements in the visual analog scale (VAS), MdDS symptom questionnaire, and posturography measures from pre- to posttreatment. No significant differences were found in outcome variables between MT- and SO-MdDS onsets. Conclusion: Symptoms improved subjectively and objectively in patients' posttreatment. The overall success rate was 64.1%, with no significant difference between MT (64.2%) and SO (63.3%). This study supports the conclusion that VOR readaptation treatment provides relief for two-thirds of MdDS patients, irrespective of the onset type. An assessment was made of visual contrast sensitivity and tremor micromovements of the eyes under conditions of "dry" immersion (DI), which simulates the physiological effects of weightlessness in the human body. Visual contrast sensitivity and ocular microtremor were evaluated in 10 human subjects (mean age 30.8 ± 4.6 years) during dry immersion (DI) as a model of the physiological effects of microgravity. Contrast sensitivity was assessed by visocontrastmetry upon presentation of Gabor patches with spatial frequencies of 0.4, 1.0, 3.0, 6.0, and 10.0 cycles per degree (cpd). The amplitude and frequency of ocular microtremor were measured using a high-frequency optical tweezer. Measurements were performed a day prior to DI; on DI days 1, 3, 5, and 7; and on the next day after DI completion. Contrast sensitivity was found to be altered within both low and high spatial frequencies; ocular microtremor amplitudes changed within a wide range. At this point in time, the findings are a step forward in searching for a method to objectively assess the function of the central nervous system in a changed environment. [ABSTRACT FROM AUTHOR]

The present study aimed to investigate the phenomenon of reverse culture shock experienced by Thai youths upon their return to Thailand. It adopted an interpretive paradigm utilising a qualitative research method. Semi-structured interviews were used to collect data from 25 Thai youths completing an exchange study abroad programme in a European country. When experiencing their own home culture again, all the returnees encountered the effects of reverse culture shock in terms of changes in their identities, behaviours, cognitions and emotions. These effects could be categorised into five main dimensions, namely: personal growth, cultural distance, social distance, moral distance, and emotional aspects. Although the negative effects of returning home were largely addressed by these students, positive consequences were also noted. The findings of this study have informed the development of a framework that serves as a guide for investigating the experience of reverse culture shock among overseas students. Objective: The purpose of this review was to identify existing prevention strategies for recreational windsurfing-related acute injuries and provide clinicians with a practical overview of current evidence supporting proposed potential prevention strategies. Methods: A literature search was conducted through March 8, 2023, using relevant keywords with Boolean operators, such as "windsurfing" AND "injury prevention" and "windsurfing" AND "exercise interventions," from the PubMed and Google Scholar databases. Only peer-reviewed English-articles were included. Results: Existing prevention strategies, right-of-way rules, a new proposed set of eight potential primary to tertiary prevention strategies for windsurfing-related acute injuries, and proposed definitions of injury prevention levels equivalent to Haddon's matrix were identified and tabled. Conclusions: The proposed potential prevention strategies may facilitate clinicians in preventing recreational windsurfing-related acute injuries. Injury prevention for recreational windsurfing is underresearched. Future studies should focus on large prospective clinical trials evaluating the efficacy of prevention strategies for recreational windsurfingrelated injuries. [ABSTRACT FROM AUTHOR]

The article is dedicated to digital learning tools application as a means of readapting Ukrainian veterans to social life. The aim of the article is to investigate the importance of digital learning tools as a means of readapting Ukrainian veterans to social life and to provide recommendations for their application in the educational process. References on veterans' adaptation, applying digital tools in education have been analysed. The results of a survey conducted by the authors among Ukrainian veterans and military personnel within the framework of the UK-Ukraine twinning grants scheme (Project UUT06 "Digitisation as a means of improving accessibility of Higher Education to Ukrainian veterans") have shown that the veterans and military personnel are ready to get a new education, but have rather low level of digital learning tools awareness. The article examines the existing approaches to the adaptation of personnel under the influence of main trends and challenges of the Ukrainian labor market in the conditions of war. The essence of the personnel adaptation (Onboarding) is clarified and the main types of new employees' adaptation are characterized with the concern to the demands of different generations of newcomers and the new realities in recruiting process. The methodological principles of building effective Onboarding programs are worked out, among which the provision of a systematic approach to the process of personnel adaptation and the need in its providing as a separate company's HR policy is highlighted. The main focus is made on taking into account the peculiarities of the digital generation while developing the meaningful content and choosing the effective forms of Onboarding programs, implementation of psychological adaptation of new employees, individualized approaches to working out different trajectories of adaptation programs taking into account the needs of new employees, diversification of personnel adaptation methods, introduction of innovative technologies and communication channels basing on the modern trends in corporate training. Scientific approaches to the integration of mentoring, shadowing, budding, the immersion method and using, artificial intelligence in Onboarding programs are substantiated. The main aspects of adopting onboarding programs and company's personnel adaptation policy to the requirements of wartime are stressed out. It is proposed to use mixed adaptation formats for the security of employees, more actively develop the different types of mentoring programs, strengthen the psychological support of employees, develop special information materials with the regulation of the procedures in emergency situations, create modern content for effective distance learning of new employees. The article is devoted to the problem of psychological features of readaptation of post-combatants after returning from the combat zone. The aims of the article are the substantiation and experimental study of the psychological features of readaptation of post-combatants to peaceful life. The tasks are defined as elucidating the essence of the concept of “readaptation” in scientific research, determination of psychological features of readaptation of post-combatants to peaceful life, and conducting a study of psychological features of readaptation of post-combatants to peaceful life. The following methods were used, including theoretical (analysis, generalization, comparison, and systematization), and empirical ones (multi-level personality questionnaire “Adaptivnist-200”). It has been established that the “readaptation” concept is applied when it comes to the need of an individual to “fight” against the psych-traumatic consequences of various stressful situations, as a result of which personality’s re-adaptation to ordinary life should take place after experiencing extreme situations. A critical analysis of approaches to defining the essence of the concept of “readaptation” by various researchers has been carried out. It has been emphasized that the process of readaptation of an individual can take place in two ways. In the first case, the person aligns their actions with the real situation on the basis of their own social experience, and in the second case, they transform the social situation to allow for their own life needs as a result of assimilating new norms, stereotypes and principles of social behaviour. It has been found out that the maladaptation of post-combatants can be pathological and non-pathological in nature. The non-pathological maladaptation is formed through individual deviations in the subject’s behaviour and experiences, caused by insufficient resocialization, socially unacceptable individual attitudes after returning from the combat zone, the breakdown of significant interpersonal relationships, the inability to engage in usual work or continue education due to injuries or mutilation etc. It has been shown that the system of rehabilitation of post-combatants has to be based on a complex of methods, principles, norms, laws and procedures which should take into account both non-pathological and possible pathological signs of post-traumatic stress disorders, as well as their depth and significance for the post-combatant’s life. The key goal of readaptation at the stage of the post-combatants’ exit from the combat zone is to satisfy their natural needs (recognition, understanding, and protection). Considering today’s requirements and the growing number of post-combatants who need psychological help, a corresponding study of their adaptability indicators was conducted via the BOO “Adaptivnist-200” methodology. Within the framework of this methodology, the levels of the personal potential of socio-psychological adaptation of post-combatants were calculated, which proved the prevalence of low resistance to combat stress. In addition, the following results were fixed including: mostly the sufficient level of military-professional orientation, the presence of some signs of deviant behaviour as well as the existence of individual rates of suicidal tendency. According to the results of the conducted research, practical psychological recommendations are proposed including more comprehensive approaches to improving the level of post-combatants’ readaptation to a peaceful life.

Rowing, when practiced as a sport, is currently typically conducted on sliding-seat boats which permit efficient lower limb use. Nevertheless, traditional fixed-seat rowing, with its own particular technique, is experiencing a revival and competitions utilizing this technique are held on a regular basis around the world. This paper investigated injuries sustained by competitive fixed-seat rowers when compared to rowers using the more widely studied sliding-seat technique. Two cohorts of male rowers, who train and compete on fixed-seat and sliding seat boats at national levels were asked to fill in a questionnaire looking into musculoskeletal injuries sustained as a result of their particular rowing style. Portuguese sliding-seat rowers rowed standard World Rowing (FISA) approved boats whilst fixed-seat rowers from Malta used traditional wooden boats and oars. Injury probabilities between the two cohorts were compared. Statistically significant diminution, P = 0.022, of knee injuries in the fixed-seat group was observed when compared to standard sliding-seat rowing, something which may be explained from the biomechanics of the stroke. Also, pelvic, other lower limb, neck and rib injuries were absent in the fixed-seat cohort. The incidence of back, shoulder, forearm and wrist injuries was comparable in both groups despite the increased reliance on the upper body, in the fixed-seat stroke. This means that contrary to common perception, fixed-seat rowing is a safe sport to practice. Canada's Medical Assistance in Dying (MAID) legislation changed in 2021; persons without a reasonably foreseeable natural death (RFND) could now be eligible for MAID and would have to wait at least 90 days before their intervention. This legislative change caused a new ethically complex question to arise, which we explore in this commentary, namely: Where should individuals without a RFND wait (for 90 days) in a publicly funded health system? This case series presents four cases of scapholunate interosseous ligament (SLIL) injury. We discuss the process of early recognition and highlight difficulties encountered in current diagnostic and conservative management strategies. The cases profile interdisciplinary management of four scapholunate (SL) injuries that have different clinical presentations and injury mechanisms. Three sport-related SL injuries were managed conservatively by a sport specialist chiropractor and one sport-related case was self-managed. Management strategies included early wrist immobilization, soft tissue manual therapy, low-level laser therapy, proprioceptive and strengthening exercises, and early orthopedic referral. This case series highlights: (1) the spectrum of SL injuries (2) the importance of early specialty referral, (3) the paucity of literature with respect to conservative management of these injuries and (4) the role of manual therapy and rehabilitation professionals as part of the healthcare team in detecting and helping manage these wrist injuries. IntroductionHealth affects work absenteeism and productivity of workers, making it a relevant marker of an individual's professional development.ObjectivesThe aims of this article were to investigate whether changes in the main cause of the sick leaves and the presence of mental health illnesses are associated with return to work with readaptation.Materials and methodsA historical cohort study was carried out with non-work-related illnesses suffered by statutory workers of university campuses in a medium-sized city in the state of São Paulo, Brazil. Two exposures were measured: (a) changes, throughout medical examinations, in the International Classification of Diseases (ICD-10) chapter regarding the main condition for the sick leave; and (b) having at least one episode of sick leave due to mental illness, with or without change in the ICD-10 chapter over the follow-up period. The outcome was defined as return to work with adapted conditions. The causal model was established a priori and tested using a multiple logistic regression (MLR) model considering the effects of several confounding factors, and then compared with the same estimators obtained using Targeted Machine Learning.ResultsAmong workers in adapted conditions, 64% were health professionals, 34% had had changes in the ICD-10 chapter throughout the series of sick leaves, and 62% had diagnoses of mental health issues. In addition, they worked for less time at the university and were absent for longer periods. Having had a change in the illness condition reduced the chance of returning to work in another function by more than 30%, whereas having had at least one absence because of a cause related to mental and behavioral disorders more than doubled the chance of not returning to work in the same activity as before.ConclusionThese results were independent of the analysis technique used, which allows concluding that there were no advantages in the use of targeted maximum likelihood estimation (TMLE), given its difficulties in access, use, and assumptions.

The objective of the research is to study the influence of restorative motor mode at the high altitude on functional condition of the sportsmen body during their stay in the mountains and the subsequent readaptation period. Materials and methods. Ten male sportsmen, whose specialization was medium and long-distance running took part in the research. The research was conducted in Bishkek during the 3-week stay in highlands (2350 m) and after their return to the foothills. During their stay in the mountains the sportsmen followed a specific motional mode in the form of outdoor activities. At the high altitude (2nd, 7th, 12th and 22nd days) and during the readaptation period (5th–6th and 18th–19st days) the condition of the respiratory function, cardio respiratory system and the maximum level of oxygen intake of sportsmen were evaluated. Results. During their stay in the mountains the increase of external expiration reserves and optimization of cardiovascular activity were observed. The level of maximum oxygen consumption after a decrease increased during the period of readaptation by 9,8 percent (P less than 0,05). Conclusions. Outdoor activities at the high altitude conditions allow you to preserve and expand the functional reserves of the body of sportsmen during the transient period of the oneyear training macrocycle.

Objective: We set out to create a Family Medicine EHR (electronic health record) embedded exercise application. This was done to evaluate the utility of the exercise app for providers and to understand the usefulness of the exercise app from the perspective of patients. Methods: This exercise application was developed through an iterative process with repeated pre-testing and feedback from an interprofessional team and embedded into the EHR at an academic family medicine clinic. Anecdotal feedback from patients was used to inform pre-testing adaptations. Results: The application required six iterations prior to clinical utility. It had several features that clinicians and patients felt were beneficial. These features involved a customizable exercise directory with pre-made templated plans which could be further modified. To overcome accessibility barriers, the application was developed to include digital and printable copies with an integrated direct email option for ease of remote sharing with patients. Conclusion: A customizable, open-source exercise application was developed to facilitate provider exercise prescription and support patient self-management. This project may be useful for other providers interested in developing similar programs to address musculoskeletal conditions in their patients. Background: Injuries to the meniscus are particularly prevalent in soccer players, with an incidence of 0.448 injuries per 1000 hours of playing. However, in the adolescent soccer player population, it has been reported that up to 63% of asymptomatic knees may demonstrate horizontal or oblique tears on MRI. These results may negatively influence clinical decision-making and plan of management for adolescent soccer players with knee problems. Case presentation: A case of a 15-year-old soccer player is presented after having been diagnosed by his family physician with a left lateral meniscus tear as per MRI, following a 10-week period of anterior knee pain. He presented to a chiropractor for a second opinion before consulting with the orthopedic surgeon. Management and outcome: Recommendations for progressive rehabilitation owing to the lack of clinical evidence for meniscal abnormality were made. A primary diagnosis of left patellar tendinopathy was determined and after a 6-week comprehensive rehabilitation program, the patient made a complete recovery. Summary: A thorough history, physical examination, and understanding of the patient's injury mechanism are suggested before confirming/refuting suspicions of meniscal abnormalities via MRI. In this article, I discuss two recent films, Frankenstein (Kevin Connor 2004) and Victor Frankenstein (Paul McGuigan 2015), in terms of how they represent disability for mainstream television and cinema spectators. Using a critical framework that blends disability and adaptation studies, I analyse both films from a 'crip' perspective – that is, by interpreting how they propagate or resist the able-bodied assumptions upon which many Frankenstein narratives are based. As screen readaptations, I explore how Connor's Frankenstein and Victor Frankenstein reflexively engage with this historical legacy, whilst providing textually 'deformed' versions of the story for contemporary audiences. More specifically, my analysis illustrates how the visual depiction of the Creature in Connor's Frankenstein serves to maintain a non-disabled gaze. Although the film subverts the typical conflation of a character's atypical body with a malevolent disposition, it does not, in fact, disrupt the longstanding ableist paradigm that views disability as monstrous. In contrast, Victor Frankenstein, although witness to the visual erasure of Igor's physical deformity from the cinema screen, nevertheless provides a critical exploration of the disabled individual's social exclusion and oppression by foregrounding Igor's subjective voice. La sexualité est une fonction prioritaire à récupérer pour les patients souffrant de lésion médullaire, mais cette prise en charge est peu abordée en réadaptation. L'objectif de cette étude transversale multicentrique est de traduire en français et de valider le questionnaire « Knowledge, Comfort, Approach and Attitudes Towards Sexuality Scale » (KCAASS), permettant d'évaluer les connaissances et les compétences des soignants concernant la dysfonction sexuelle du patient atteint de lésion médullaire. Le questionnaire KCAASS a été traduit en français selon les recommandations internationales. À deux reprises et à trois semaines d'intervalle, le questionnaire a été distribué via l'outil REDCap aux médecins, infirmiers, kinésithérapeutes et ergothérapeutes travaillant en réadaptation en Belgique francophone et au Luxembourg. Le questionnaire KCAASS a été complété par 131 participants (88,5 % de Belgique et 11,5 % du Luxembourg). Trente-quatre participants l'ont complété une seconde fois 24 jours (± 5 jours) plus tard. Les analyses psychométriques ont montré une cohérence interne élevée et une fiabilité test-retest excellente pour les sous-échelles « connaissance », « confort » et « approche » (α de Cronbach respectivement de 0,92, 0,97 et 0,86 et coefficient de corrélation intraclasse respectivement de 0,84, 0,85 et 0,85). Pour la sous-échelle « attitude », la cohérence interne et la fiabilité test-retest se sont révélées modérées (α de Cronbach de 0,63 et Coefficient de corrélation intraclasse de 0,64). La version française du questionnaire KCAASS est fiable et peut être utilisée en pratique clinique dans le monde francophone. Sexuality is a critical function to be recovered for spinal cord injury patients, yet the management of sexual dysfunction is poorly addressed in rehabilitation. The main objective of this multicentric cross-sectional study is to translate and validate the " Knowledge, Comfort, Approach and Attitudes Towards Sexuality Scale " (KCAASS) questionnaire into French. This questionnaire assesses caregivers' knowledge and skills regarding sexual dysfunction in spinal cord injury patients. The KCAASS questionnaire was translated into French according to international guidelines. The questionnaire was distributed twice at a three-week interval via "REDCap" to doctors, nurses, physiotherapists, and occupational therapists working in rehabilitation in French-speaking Belgium and Luxembourg. The KCAASS questionnaire was completed the first time by 131 participants (88.5% from Belgium and 11.5% from Luxembourg) and by 34 participants the second time 24 days ± 5 days later. The psychometric analyses showed high internal consistency and excellent test-retest reliability for the "knowledge", "comfort" and "approach" sub-scales (Cronbach's α of 0.92, 0.97 and 0.86 respectively and intraclass correlation coefficient of 0.84, 0.85 and 0.85 respectively). For the "attitude" sub-scale, internal consistency and test-retest reliability were moderate (Cronbach's α of 0.63 and intraclass correlation coefficient of 0.64). The French version of the KCAASS questionnaire proved to be reliable and can be used in clinical practice in the French-speaking world. [ABSTRACT FROM AUTHOR]

The purpose of this commentary is to consider circumstances under which it may be ethical to permit patients to use licit substances in rehabilitation contexts. While the content of this commentary may be transferable to other healthcare spaces, our focus on rehabilitation is based on some important distinctions that exist between rehabilitation and acute care spaces. Introduction. Analysis of biomedical aspects of the development of high-altitude pathology includes the concept of "readaptation" that has recently become a matter of scientific interest. Readaptation is the process of reverse adaptation of the structure and function of the human or animal body to the conditions of the external environment, aimed at maintaining the relative constancy of the internal environment of the body — homeostasis. This article is devoted to the study of the course of myocardial necrosis at different periods of readaptation to low mountain conditions after 3 and 30 days of staying in high mountain conditions. Materials and methods. The experimental part of the study was performed using 40 white outbred laboratory male rats aged 3-4 months weighing 200-300 g. The course of catecholamine necrosis of the myocardium in different periods of readaptation was the subject of the study. Blood serum enzymes and the myocardium of laboratory animals represented the objects of the study. Results. It was found that after simulating cardionecrosis on the 3rd, 7th and 30th days of animal readaptation to low mountain conditions, the content of creatine phosphokinase-MB, troponin T, aspartate aminotransferase, the cardiac form of the fatty acid-binding protein in the blood serum was higher than in animals with experimental cardionecrosis in low mountains. Conclusion. Our study indicates a progressive course of cardionecrosis at different points of readaptation, and, therefore, during the period of readaptation, the body functions undergo significant changes that require further careful studying and evaluation.

The classical repertoire and, especially, its stage rewritings define a thematic obsession for the great theatre creators of the moment. Just as each great director has their own aesthetic, so each of them has their own way of appropriating representative plays. Henrik Ibsen is one of the indispensable playwrights in the artistic portfolio of the most important names whose creations were performed on the Romanian stages. This article aims at a parallel analysis of two recent versions of the Ibsenian play The Lady from the Sea, directed by Radu Afrim (2014) and Andriy Zholdak (2022), respectively, in order to deepen the specific differences of two theatrical aesthetics decisive for understand the evolution of contemporary theatre. We also aim to question the possibilities that the readaptations, the rewritings of the great classical texts involve in the attempt of the creators to “contemporize” a play representative of the sensitivity of a past century.
