Ureaplasma spp. are frequently isolated from the genital tract of women of reproductive age. To date, it remains unclear whether they are commensal or pathogenic. In our study, we assessed the prevalence of Ureaplasma spp. in a group of 1,155 women of childbearing age. In addition, we assessed how often women with positive Ureaplasma spp. develop genital tract co-infections and how the vaginal pH changes. This study showed a relationship between colonization by Ureaplasma spp. and presenting symptoms. In fact, we showed that colonization of the genital tract by Ureaplasma spp. can affect the occurrence of co-infections such as Gardnerella vaginalis. We also observed a relationship between increased pH values and the presence of Ureaplasma spp.

OBJECTIVE:Conventional risk stratification models for mortality of acute myocardial infarction (AMI) have potential limitations. This study aimed to develop and validate deep-learning-based risk stratification for the mortality of patients with AMI (DAMI). METHODS:The data of 22,875 AMI patients from the Korean working group of the myocardial infarction (KorMI) registry were exclusively divided into 12,152 derivation data of 36 hospitals and 10,723 validation data of 23 hospitals. The predictor variables were the initial demographic and laboratory data. The endpoints were in-hospital mortality and 12-months mortality. We compared the DAMI performance with the global registry of acute coronary event (GRACE) score, acute coronary treatment and intervention outcomes network (ACTION) score, and the thrombolysis in myocardial infarction (TIMI) score using the validation data. RESULTS:In-hospital mortality for the study subjects was 4.4% and 6-month mortality after survival upon discharge was 2.2%. The areas under the receiver operating characteristic curves (AUCs) of the DAMI were 0.905 [95% confidence interval 0.902-0.909] and 0.870 [0.865-0.876] for the ST elevation myocardial infarction (STEMI) and non ST elevation myocardial infarction (NSTEMI) patients, respectively; these results significantly outperformed those of the GRACE (0.851 [0.846-0.856], 0.810 [0.803-0.819]), ACTION (0.852 [0.847-0.857], 0.806 [0.799-0.814] and TIMI score (0.781 [0.775-0.787], 0.593[0.585-0.603]). DAMI predicted 30.9% of patients more accurately than the GRACE score. As secondary outcome, during the 6-month follow-up, the high risk group, defined by the DAMI, has a significantly higher mortality rate than the low risk group (17.1% vs. 0.5%, p < 0.001). CONCLUSIONS:The DAMI predicted in-hospital mortality and 12-month mortality of AMI patients more accurately than the existing risk scores and other machine-learning methods.

There are limited data about clinical outcomes of biodegradable polymer biolimus-eluting BioMatrix stents (BP-BES) and durable polymer everolimus-eluting Xience stents (DP-EES) in real world practice. We sought to compare the clinical outcomes of BP-BES and DP-EES in real world cohorts of patients undergoing percutaneous coronary intervention. A prospective multicenter registry enrolled 999 patients treated with BP-BES and 1,000 patients treated with DP-EES. The primary outcome was target lesion failure, defined as a composite of cardiac death, target vessel-related myocardial infarction, or target lesion revascularization. Definite or probable stent thrombosis was also compared in total and propensity score-matched cohorts. The median follow-up duration was 24 months, and mean age was 65 years (interquartile range, 56-72 years). Patients receiving BP-BES had a lower prevalence of acute coronary syndrome, prior myocardial infarction, multi-vessel disease, bifurcation lesions, and left anterior descending artery lesions than those receiving DP-EES. After propensity score matching (692 pairs), target lesion failure occurred in 22 patients receiving BP-BES and in 25 patients receiving DP-EES (3.2% versus 3.6%; adjusted hazard ratio [HR], 0.92; 95% confidence interval [CI], 0.53 to 1.60; p = 0.77). The risk of definite or probable stent thrombosis did not differ between the 2 groups (0.4% versus 0.4%; adjusted HR, 1.03; 95% CI, 0.21 to 4.98; p = 0.97). The results were consistent across various subgroups. In the propensity score-matched analysis of real world cohorts, BP-BES showed similar clinical outcomes compared to DP-EES. We need to investigate about whether differences in clinical outcome emerge during long-term follow-up.

BACKGROUND:Limited data are available regarding the long-term clinical outcomes of second-generation drug-eluting stents (DES) versus first-generation DES in patients with coronary chronic total occlusion (CTO) who undergo percutaneous coronary intervention (PCI). The aim of this study was to compare the clinical outcomes of second-generation DES with those of first-generation DES for the treatment of CTO. METHODS AND RESULTS:Between March 2003 and February 2012, 1,006 consecutive patients with CTO who underwent successful PCI using either first-generation DES (n = 557) or second-generation DES (n = 449) were enrolled in a multicenter, observational registry. Propensity-score matching was also performed. The primary outcome was cardiac death over a 2-year follow-up period. No significant differences were observed between the two groups regarding the incidence of cardiac death (first-generation DES versus second-generation DES; 2.5% vs 2.0%; hazard ratio [HR]: 0.86; 95% confidence interval [CI]: 0.37 to 1.98; p = 0.72) or major adverse cardiac events (MACE, 11.8% vs 11.4%; HR: 1.00; 95% CI: 0.67 to 1.50; p = 0.99). After propensity score matching, the incidences of cardiac death (HR: 0.86; 95% CI: 0.35 to 2.06; p = 0.86) and MACE (HR: 0.93; 95% CI: 0.63 to 1.37; p = 0.71) were still similar in both groups. Furthermore, no significant differences were observed between sirolimus-eluting, paclitaxel-eluting, zotarolimus-eluting, and everolimus-eluting stents regarding the incidence of cardiac death or MACE. CONCLUSION:This study shows that the efficacy of second-generation DES is comparable to that of first-generation DES for treatment of CTO over 2 years of follow-up.

Disorders of the vaginal microbiota can lead to many complications and affect fertility. This study evaluates the role of Lactobacillus in the vagina and its impact on the incidence of colonization by pathogenic microorganisms, analyzing the results of 1,039 women of reproductive age (18–49 years) who underwent a microbiological examination of the reproductive tract in 2020. Samples were examined by microscopy, culture, and NAAT. As the number of Lactobacillus increases, the chance of developing symptoms decreases. In fact, it has been shown that the higher the number of Lactobacillus spp. the less frequently Gardnerella vaginalis and Streptococcus group B are observed. As the concentration of Lactobacillus spp. increases by one category, the risk of G. vaginalis after adjustment to age and pH decreases by 80% (p < 0.001). Similarly, the correlation between Lactobacillus spp. and vaginal pH was shown. After adjustment to age, the odds of prevalence pH > 4.5 for people with Lactobacillus category higher 1 is 76% lower.

Neisseria gonorrhoeae (gonococcus) is a human pathogen, the aetiological agent of gonorrhoea, which is the second most common bacterial sexually transmitted disease (STD) in the world. The structure of N. gonorrhoeae cell wall is typical of Gram-negative bacteria, poses variable antigens porin B (PorB), and opacity-associated proteins (Opa proteins), lipooligosaccharide (LOS) and type IV pili (TFP) playing an essential role in pathogenesis. In addition to adhesins, gonococcus presents other virulence factors such as reducing modifiable protein (Rmp), iron transporters, membrane pumps, and IgA peptidase. The pathogen produces outer membrane vesicles (OMVs), releases peptidoglycan (PG) fragments and is well adapted to develop infection in diverse niches of the female and male reproductive tracts. The characteristic genotypic trait of N. gonorrhoeae is the state of natural competence, which allows DNA uptake from the environment. The antigenic and phase variability is essential to gonococcal defence against the human immune system. Because of the increasing antimicrobial resistance (AMR) of N. gonorrhoeae and the high incidence rate of gonococcal infections, developing an antigonococcal vaccine has become an urgent need. Vaccine development difficulties are mainly due to the gonococcal ability of immune evasion, the lack of an animal model, and the limited understanding of protective immune response mechanisms.

Neisseria gonorrhoeae is an aetiological agent of gonorrhoea, which is a sexually transmitted disease (STD), a public health challenge worldwide. Gonorrhoea is undoubtedly a serious health risk, especially for women, due to its often asymptomatic course and the possibility of upper genital tract complications such as pelvic inflammatory disease (PID), which can result in infertility. The data show that the diagnostic method for N. gonorrhoeae should be specific, simple, sensitive, rapid and inexpensive. Currently, phenotypic identification methods have been dominated by NAAT methods, which allow detection and identification of gonococcus directly in the clinical specimen. However, up today molecular methods do not allow full determination of drug susceptibility.

Purpose Previous studies have reported that presarcopenia negatively affects rectal cancer treatment. However, most studies have analyzed patients including majority of open surgery, and the association between presarcopenia and clinical outcomes after laparoscopic rectal cancer surgery remains unclear. This study aimed to evaluate the impact of presarcopenia on the clinical and oncological outcomes after laparoscopic rectal cancer surgery. Methods Three hundred and one patients undergoing laparoscopic rectal cancer surgery between December 2009 and May 2016 were enrolled. Body composition was assessed using computed tomography by measuring the muscle and fat areas at the third lumbar (L3) vertebra. The L3 skeletal muscle area was used to calculate the skeletal muscle index and evaluate presarcopenia. Many studies show that breastfeeding is associated with numerous direct and indirect health benefits for children in later life, although the relationship between breastfeeding and cardiovascular risk is still equivocal. The aim of this study was to investigate the relationship between duration of breastfeeding and cardiovascular risk in young adults. The cross-sectional study was conducted on a group of 128 volunteers (71 women and 57 men) at the age of 19 years. It consisted of a questionnaire, together with anthropometric and blood pressure measurements. It was demonstrated that 90.3% of the participants had been breastfed and the mean duration of breastfeeding was 8.8±7.6 months. Participants who were breastfed for longer than six months had a significantly lower systolic blood pressure than those breastfed for less than six months. Results of analyses were consistent for the female group, the male group and the combined group. The results suggest that breastfeeding for longer than six months decreases systolic hypertension risk in young adults. The promotion of breastfeeding may decrease the rate of cardiovascular disease and thus improve health status of the population.

Introduction: Inappropriate antiplatelet therapy discontinuation increases the risk of thrombotic complications and bleeding after dental procedures. To determine the platelet reactivity recovery time after aspirin withdrawal in vivo, our study was conducted in patients with low-risk cardiovascular disease who can stop aspirin administration following the guidelines stipulated by the American College of Chest Physicians. The time it takes for platelet activity to normalize and the diagnostic accuracy of testing methods were assessed for a residual antiplatelet activity with multiple electrode aggregometry. Our study included patients with clinically indicated hypertension preparing for a dental extraction procedure. Materials and methods: A total of 212 patients not taking aspirin (control group) and 248 patients with hypertension receiving long-time aspirin treatment at a 100-mg daily dose were prospectively included in the study, which involved stopping aspirin intake before dental extraction. The residual platelet activity and dental bleeding in patients who stopped aspirin intake were analyzed and compared with those of the control group. In addition, platelet reactivity recovery time and bleeding risk in patients who stopped taking aspirin every 24 hours for 0 to 5 days (0–143 hours) before dental extraction was also assessed. Results: Platelet reactivity normalized 96 hours after aspirin withdrawal. The cut-off value of 49 arbitrary units in the arachidonic acid platelet aggregation test excluded the effect of aspirin with 91% sensitivity and 66% specificity. AUC showed 0.86 (P < 0.001) diagnostic accuracy. The immediate bleeding complications in all treatment groups were similar to those seen in the control group and were successfully managed with local hemostatic measures. Conclusions: The antiplatelet effects of aspirin disappeared 96 hours after aspirin withdrawal in our study, and dental extractions may be safely performed in this period when appropriate local hemostatic measures are taken. Based on these results, a shorter aspirin intake cessation period may be allowable in complex dental procedures and surgery for which a longer aspirin intake cessation period (7–10 days) is recommended based on the American College of Chest Physicians guidelines.
