15. Phacoémulsification versus chirurgie de cataracte par mini incision manuelle : résultats anatomiques et fonctionnels

Resume Objectifs Comparer, de facon prospective et randomisee, l’impact de la phacoemulsification (PHACO) et de la technique de small incision cataracte surgery (SICS) sur la structure endotheliale (densite et morphologie cellulaire) et d’evaluer leurs resultats refractifs (acuite visuelle, astigmatisme induit). Materiels et methodes Etude prospective de 57 patients (63 yeux) sur une periode de 20 mois (mai 2012–janvier 2014) operes par 2 chirurgiens experimentes. Nous n’avons inclus que les patients presentant une cataracte senile ou presenile de grade nucleaire 2, 3 et 4 selon la classification LOCS III. Nous avons randomise les patients en 2 groupes en fonction de leur mois de naissance (groupe 1 : PHACO [33] ; groupe 2 : SICS [30]). Toutes les donnees pre-, per- et postoperatoires ont ete collectees prospectivement. Le recul de 6 mois minimum. L’analyse statistique a ete faite par le SPSS 18.0. Les tests statistiques utilises etaient le test- t Student, la variable Anova, le test non parametrique de Mann-Witney et le test Khi 2 . Un seuil de signification est fixe a 0,05. Resultats En preoperatoire, la densite cellulaire endotheliale (DCE) moyenne etait de 2447,5 ± 225 mais sans difference significative entre les deux groupes ( p = 0,207). Le pourcentage d’hexagonalite moyen etait de 55,5 ± 8,2 % dans les groupes 1 et 2. La perte cellulaire la plus importante etait lors de la premiere periode postoperatoire immediate pour les 2 groupes. A j15 postoperatoire, la diminution de la DCE moyenne etait significative ( p 2 ( p −2 ). En postoperatoire, la meilleure acuite visuelle corrigee (MAVC) moyenne etait de 0,057 log MAR pour tous nos patients, aucun patient n’avait une acuite ≤ 1/10. L’astigmatisme moyen au terme du recul etait de 1,08 ± 0,42 D dans le groupe 1 et de 1,51 ± 0,55 D dans le groupe 2 avec une difference significative ( p = 0,001). Conclusion La PHACO et la SICS donnent d’excellents resultats tant sur le plan anatomique que refractif. Neanmoins, la SICS semble avoir plus d’avantage par rapport a la PHACO et ce concernant la rapidite de l’acte, le cout, l’independance par rapport a la machine et semble etre mieux adaptee aux cataractes denses et a la chirurgie de masse.

Prospective randomised study to compare the impact of phacoemusification (PHACO) and small incision cataract surgery (SICS) on endothelial structure (cell density and morphology) and refractive results (visual acuity, induced astigmatism).Prospective study of 57 patients (63 eyes) over 20 months (May 2012-January 2014) undergoing surgery by 2 experienced surgeons. We included patients with senile or presenile cataracts, LOCS III class 2, 3 and 4. Patients were randomized into 2 groups based on their birth month (group 1: PHACO [33]; group 2: SICS [30]). All pre- intra- and postoperative data were collected prospectively. The minimum follow-up was 6 months. We used the SPSS 18.0 for statistical analysis. Statistical tests used included the test-t Student, the Anova test, the Mann-Witney non-parametric test and the KhiThe mean preoperative endothelial cell density was 2447.5±225 c/mmBoth SICS and PHACO give excellent results, both anatomical and refractive. However, SICS appears to be more advantageous than PHACO in terms of speed, cost, and independence from technology, and appears to be better suited to dense cataracts and mass surgery.

22. Aspects cliniques et étiologique des uvéites dans un centre hospitalo-Universitaire du grand Tunis

Introduction L’uveite est la maladie inflammatoire la plus frequente de l’œil et constitue l’une des principales causes de perte de vision grave et de cecite. L’objectif de ce travail est d’etablir les particularites epidemiologiques et cliniques des uveites et etudier les differentes etiologies. Patients et methodes Il s’agit d’une etude retrospective, incluant 233 patients atteints d’uveite, sur une periode de 10 ans. Tous les patients ont beneficie d’un examen ophtalmologique complet au service d’ophtalmologie de l’hopital Charles-Nicolle de Tunis, suivi d’un bilan clinique et paraclinique adapte. Resultats L’analyse a porte sur 375 yeux de 233 patients. L’âge moyen de nos patients au moment du diagnostic etait de 34 ans avec un sex-ratio (H/F) de 0,58. L’atteinte etait unilaterale dans 37,3 %. Les panuveites etaient la forme anatomique la plus frequente (43,7 %), suivies des uveites anterieures (37,7 %), des uveites posterieures (11,6 %) et des uveites intermediaires (6,8 %). Les etiologies les plus frequentes etaient la tuberculose (17,6 %), la maladie de Behcet (6,4 %), la maladie de Vogt-Koyanagi-Harada (VKH) (4,7 %) et les infections virales (4,2 %). Le groupe des infections representait dans notre serie 25,7 % des diagnostics, celui des maladies systemiques representait 18,8 %. Les formes idiopathiques representaient 21 %. Les principales complications retrouvees etaient la cataracte (24,0 %), la seclusion pupillaire (13,7 %) et l’hypertonie oculaire (10,3 %). Conclusion Dans notre serie, les etiologies des uveites etaient dominees par la tuberculose, la maladie de Behcet, la maladie de VKH, les infections virales, la spondylarthrite ankylosante et la toxoplasmose.

23. Manifestations ophtalmologiques au cours des maladies neurométaboliques

Introduction Les maladies neurometaboliques hereditaires sont un ensemble de maladies genetiques dues a des mutations dans des genes codant, le plus souvent pour des enzymes, parfois pour d’autres proteines du metabolisme cellulaire. Le deficit enzymatique est pathogene du fait soit de l’accumulation d’un substrat toxique en amont du bloc metabolique, soit d’un defaut de synthese d’un compose indispensable en aval. Les anomalies ophtalmologiques sont frequemment associees et sont d’une grande valeur diagnostique. Le but de notre travail est de determiner les maladies metaboliques les plus frequemment associees a une atteinte ophtalmologique et de determiner la frequence des anomalies ophtalmologiques dans les maladies neurometaboliques, preciser leur type et leur valeur predictive positive. Patients et methodes Etude retrospective sur 5 ans sur les patients ayant une maladie neurometabolique associee a une atteinte ophtalmologique. Resultats L’etude a collige 50 patients dont l’âge moyen de debut de la maladie etait de 18 mois (± 2,1 mois). L’atteinte oculaire a ete detectee lors de l’examen ophtalmologique initial pratique a un âge moyen de 5,6 ans. Les PEV sont etaient alteres dans 20 % des cas. Les maladies neurometaboliques associees a des manifestations ophtalmologiques etaient les maladies du metabolisme energetique dans 48 % des cas, les maladies des molecules complexes dans 28 % des cas et les maladies d’intoxication dans 18 % des cas. Les atteintes ophtalmologiques les plus frequentes etaient une atrophie optique (60 %) et retinite pigmentaire (12 %) associees principalement aux mitochondriopathies, une paralysie supra-nucleaire (12 %) retrouvee dans la maladie de Gaucher type III et de Niemann Pick type C et une macula rouge cerise (6 %) dans la gangliosidose a GM2. Conclusion L’examen ophtalmologique est crucial pour l’approche diagnostique des maladies neurometaboliques. Le diagnostic precoce de ces maladies neurometaboliques, permet de prevenir la progression de certaines maladies traitables, d’ameliorer la qualite de vie des patients en prenant en charge le probleme ophtalmologique et de proposer un conseil genetique.

Resume Introduction La blepharite chronique est une maladie ophtalmique courante posant un probleme therapeutique. Le diagnostic etiologique est indispensable permettant ainsi, d'adapter le traitement. But de l'etude Le but de cette etude est de determiner le role pathogene du Demodex sp, du Malassezia sp et du Candida sp dans la genese des blepharites chroniques. Patients et methodes Il s'agit d'une etude prospective realisee durant une periode de 14 mois. Elle a interesse un groupe de 69 malades atteints de blepharite chronique et un groupe de 96 temoins n'ayant jamais eu de blepharite. La prevalence du Demodex sp, du Malassezia sp et du Candida sp au niveau des cils a ete determinee dans les deux groupes. Resultats D. folliculorum a ete trouve chez 58 % des malades atteints de blepharites chroniques et 15,6 % des temoins. La difference est significative (p Conclusion Dans notre etude, D. folliculorum et la levure du genre Malassezia sp semblent etre incrimines dans la genese des blepharites chroniques. Leur recherche systematique est necessaire afin d'adapter le traitement.

But De recents travaux ont montre l’incrimination des molecules d’adhesion dans la pathogenie de la retinopathie diabetique. Ces molecules d’adhesion s’expriment sur diverses cellules intervenant dans divers processus inflammatoires et de neovascularisation. Le but dans cette etude est d’analyser l’expression de la molecule d’adhesion intercellulaire de type 1 (ICAM-1) dans la conjonctive des patients diabetiques sans retinopathie et de la comparer a celle des patients non-diabetiques. Methodes Quinze biopsies conjonctivales ont ete prelevees chez des patients diabetiques sans retinopathie ophtalmoscopique et angiographique. La conjonctive humaine « normale » a ete prelevee chez cinq patients lors d’une cure chirurgicale de cataracte senile. L’analyse immunohistochimique a ete effectuee par la methode d’immunoperoxydase indirecte, en utilisant un anticorps monoclonal dirige contre ICAM-1. Resultats La molecule d’adhesion ICAM-1 etait presente au niveau des cellules epitheliales, endotheliales vasculaires et inflammatoires. Un profil different d’expression a ete trouve chez les patients diabetiques pour une meme duree de diabete. Dans la conjonctive humaine normale, l’expression d’ICAM-1 etait tres faible. Conclusion Cette etude preliminaire montre que la molecule ICAM-1 est presente au niveau de la conjonctive des patients diabetiques sans retinopathie clinique. Ce resultat pourrait permettre une nouvelle approche de la pathogenie de la retinopathie diabetique.

26. Traitement des décollements de la rétine par déchirure géante : résultats de l’échange perfluorocarbones liquides-silicone avec indentation circulaire

Introduction Les resultats anatomiques et fonctionnels du traitement des decollements de retine par dechirures geantes ont ete nettement ameliores par le developpement des techniques de vitrectomie et, surtout, l’utilisation des perfluorocarbones liquides. Dans ce travail, nous rapportons les resultats du traitement du decollement de retine par dechirure geante chez 17 patients. Materiels et methodes Cette etude a porte sur 17 yeux de patients, âges de 18 a 61 ans, presentant un decollement de retine par dechirure geante. La taille de la dechirure allait de 90° a 220°. Une proliferation vitreo-retinienne stade C etait presente dans deux yeux. L’acuite visuelle preoperatoire variait de 8/10 e a « une perception lumineuse ». Tous les patients ont eu un cerclage avec une vitrectomie a trois voies, une mise a plat de la retine par perfluorocarbone liquide, un endolaser, une cryoapplication et un tamponnement transitoire par huile de silicone. Resultats Le recul moyen etait de 20 mois ± 16. Une recidive du decollement de retine a ete notee dans 5 cas sous silicone et dans 2 cas apres ablation de silicone. Le succes anatomique au dernier recul a ete obtenu pour 14 yeux (82,3 %). Sur le plan fonctionnel, l’acuite visuelle s’est amelioree dans tous les cas de succes anatomique sauf un. Les complications postoperatoires les plus frequentes etaient la cataracte qui a ete notee dans 10 yeux parmi 12 phaques et l’hypertonie qui a ete retrouvee dans 4 cas. Conclusion La vitrectomie avec tamponnement interne par huile de silicone permet d’ameliorer le pronostic anatomique et fonctionnel du decollement de la retine par dechirure geante. Les recidives sont dues le plus souvent a la reproliferation vitreo-retinienne. Les complications postoperatoires les plus frequentes demeurent l’opacification cristallinienne et l’hypertonie.

Nous rapportons un cas de chorioretinite sereuse centrale (CRSC) chez un patient greffe du rein pour insuffisance renale chronique, en decrivant les particularites cliniques, angiographiques et evolutives de la CRSC dans ce cas. Plusieurs facteurs peuvent etre incrimines dans la survenue d’une CRSC chez ces patients, en particulier les troubles hemodynamiques lies a l’insuffisance renale et a l’hypertension arterielle qui sont souvent associees, le stress lie a la greffe renale et la corticotherapie au long cours prescrite en postoperatoire.

But Le but de cette etude est d’analyser la topographie de la tete du nerf optique par l’ophtalmoscope laser a balayage confocal dans les hypertonies oculaires (HTO) et de la comparer a celle d’une population normale. Materiels et methodes Nous avons examine 25 patients avec HTO isolee et 27 patients normaux. Tous les patients ont beneficie d’un examen ophtalmologique complet. L’analyse de la tete du NO a ete faite par l’ophtalmoscope laser a balayage confocal type Heidelberg retina tomograph (HRT). La surface du disque, la surface et le volume de l’excavation, le rapport C/D, la surface et le volume de l’anneau neuroretinien, la profondeur moyenne et maximale de l’excavation, la mesure de la forme de l’excavation, la hauteur de la variation du contour, l’epaisseur moyenne et la surface de la couche des fibres nerveuses retiniennes ont ete mesurees. Resultats Sur l’ensemble des parametres stereometriques etudies, nous avons constate une reduction significative du volume de l’anneau neuroretinien, de la variation de la hauteur du contour et de l’epaisseur de la couche des fibres nerveuses retiniennes chez les patients presentant une HTO par rapport aux sujets normaux. Conclusion Le HRT semble etre d’un apport potentiel dans le depistage precoce des alterations de la couche des fibres nerveuses retiniennes dans les HTO.

A pterygium is characterized by abnormal fibrovascular corneoconjunctival tissue. A number of investigations have attempted to elucidate this incompletely understood pathology. Since vascular endothelial growth factor (VEGF) and p53 are known to participate in tumor vascularization, our purpose was to study VEGF and p53 expression in active primary and recurrent pterygium from Tunisian patients. To this end, 15 cases of active primary pterygium and five cases of recurrent pterygium from Tunisia were studied by immunohistochemistry. Antibodies raised against VEGF and p53 were used to analyze the distribution and expression of these markers in pterygium and normal human conjunctiva were used as negative control. VEGF and p53 proteins were found in all cases of primary pterygium in epithelial, fibroblast and vascular endothelial cells. Active primary and recurrent pterygium have different patterns of expression. In primary pterygium, an important variability of p53 and VEGF expression was observed. However, in recurrent pterygium, p53 immunoreactivity was weak to moderate, whereas VEGF immunoreactivity was strong. In normal human conjunctiva, VEGF and p53 expression was weak to negative. The overexpression of VEGF in active primary and recurrent pterygium suggests that angiogenesis may play a role in pterygium pathogenesis and the expression of p53 in active primary pterygium, which might be associated with its mutated form, supports the hypothesis that actinic radiation may be involved in the genesis of pterygium. Thus, VEGF and p53 may be useful biomarkers for understanding the physiopathology of pterygium.

PURPOSE. To evaluate the frequency, characteristics, causes, and risk factors of late recurrent retinal detachments (LRRD). METHODS. The authors retrospectively analyzed 445 eyes operated consecutively for rhegmatogenous retinal detachment between 1990 and 2003 by the same surgeon. Only eyes with complete reattachment after a minimum follow-up of 6 months were included. The authors defined LRRD as detachment occurring at least 6 months after a complete retinal reattachment. RESULTS. Nine eyes had a LRRD (2.02%). Mean age was 52.55 years. Six eyes had scleral buckle procedure and three eyes had vitrectomy silicone oil injection then silicone oil removal. LRRD occurred after an average period of 54 months. During follow-up two patients had a removal of extrusion of scleral buckling material. New or reopened breaks were associated with LRRD in all eyes. Endo-ocular surgery was carried out in seven eyes. The retinal reattachment was achieved in the seven eyes. Final visual acuity ranged from 20/400 to 20/60 after an average follow-up of 37.8 months. Relation between aphakic eyes (and more generally nonphakic eyes) and LRRD was close to significant value (p=0.05). LRRD was statistically independent (p>0.05) of myopia, preoperative pseudophakia, vitrectomy for initial detachment, scleral buckle removal, and cataract surgery after reattachment. CONCLUSIONS. LRRD were rare. Most of them were due to a retinal break. This break can be a new break, or reopened break especially after removal of scleral buckle material. Vitreous traction, at the vitreous base, seemed to be the cause of these breaks. (Eur J Ophthalmol 2008; 18: 960-4)

Purpose Diabetic fibrovascular membranes are the main pathological changes of proliferative diabetic retinopathy that can cause serious complications leading to blindness. Since the mechanism of fibrovascular membrane development is still unknown, the aim of our study was to identify potential biomarkers for this pathology. To this end, we analyzed the simultaneous expression of ICAM-1, VCAM-1 and VEGF within tissues of diabetic fibrovascular membranes. Patients and methods Fibrovascular membranes were taken from nine diabetic patients with proliferative diabetic retinopathy. The fibrovascular membrane specimens were analyzed by immunohistochemistry to determine ICAM-1, VCAM-1 and VEGF expression. Controls were collected on nine normal conjunctivas removed during senile cataract surgery. Results Coexpression of ICAM-1, VCAM-1 and VEGF was found in most of the diabetic fibrovascular membranes studied. Thus, ICAM-1 was positive in eight of nine membranes (82%), VCAM-1 in seven of nine membranes (78%) and VEGF in all the membranes. Conclusions The substantial overexpression of adhesion molecules ICAM-1, VCAM-1 and of VEGF suggests that these molecules might contribute to the development of fibrovascular membranes in patients with proliferative diabetic retinopathy, and that they could constitute suitable markers of this pathology.

Familial exudative vitreoretinopathy (FEVR) is a well-defined inherited disorder of retinal vessel development. Association of FEVR with other congenital ocular diseases has already been described. We report the first clinical case of FEVR associated with hyaloid artery persistence. An 18-year-old Tunisian woman and four members of her family (mother, two brothers, and sister) presented signs of FEVR. Examination of this patient also revealed functional hyaloid artery persistence in her right eye. These two disorders have many common aspects in their pathological process, in particular vascular endothelial growth factor expression, which can explain this association.

The chronic blepharitis is a current ophthalmic disease posing a therapeutic problem. The etiologic diagnosis is essential for adapting the treatment.The aim of this study is to describe the role of Demodex sp, Malassezia sp and Candida sp in the pathogenesis of chronic blepharitis.It's a prospective study realized during a period of 14 months. It interested 69 patients with chronic blepharitis and 96 controls. The prevalence of Demodex sp, Malassezia sp and Candida sp on the eyelash of chronic blepharitis patients and controls was investigated.Demodex folliculorum was found in 58% of patients with chronic blepharitis and in 15,6% of controls. The difference between the two groups was statistically significant (P0,00001). The sex and the age intervene neither in the frequency, nor in the abundance of D. folliculorum in the two groups. Malassezia sp was identified in 10,4% of the control group and 31,88% of the group with chronic blepharitis. The difference is significant (P0,00001). There is no correlation between frequency and abundance of Malassezia, sex and age in the two groups.In our study, D. folliculorum and Malassezia sp seem to be accused in the genesis of the chronic blepharitis. Their systematic search is necessary in order to adapt the treatment.

Recent research has incriminated adhesion molecules in the pathogenesis of diabetic retinopathy. These molecules have been found to be expressed in many cells participating in inflammatory processes and neovascularization. The purpose of our investigation was to study the expression of intercellular adhesion molecule type 1 (ICAM-1) in the conjunctiva of diabetic patients without retinopathy in comparison with normal human conjunctiva.Fifteen conjunctival biopsies were obtained from diabetic patients without retinopathy. The ocular fundus examination and retinal fluorescein angiography were normal. The normal human conjunctiva were taken from five patients undergoing senile cataract surgery. Immunohistochemical analysis consisted of indirect immunoperoxidase using the monoclonal antibody ICAM-1.The adhesion molecule ICAM-1 was immunolocalized in epithelial, vascular endothelial, and inflammatory cells. The expression of this molecule was different in diabetic patients for the same duration. In the normal human conjunctiva, the expression of ICAM-1 was very low.This preliminary study shows that ICAM-1 is present in the conjunctiva of diabetic patients without retinopathy and thus may add new insights into the pathogenesis of diabetic retinopathy.

Anatomical and functional results of the treatment of retinal detachment associated with giant retinal tear have been markedly improved with the development of the vitrectomy procedure and especially the use of perfluorocarbon liquids. In this study, we report treatment results of retinal detachment associated with giant tear in 17 patients.Our study included 17 patients (17 eyes), aged between 18 and 61 years, with retinal detachment associated with giant retinal tear. The size of the tear was between 90 degrees and 220 degrees . Proliferative vitreoretinopathy grade C was present in two eyes. Preoperative visual acuity ranged between 40/50 and light perception. All patients underwent scleral buckling, vitrectomy, perfluorocarbon liquid injection to flatten the retina, endophotocoagulation, cryoapplication and temporary silicone oil tamponade.Mean follow-up was 20 months +/- 16. Recurrent retinal detachment occurred in five eyes under silicone oil and in two eyes after its removal. Anatomical success at final follow-up was obtained in 14 eyes (82.3%). Visual acuity improved in all cases with anatomical success in all but one. The most frequent postoperative complications were cataract in 10 cases among 12 phakic eyes and ocular hypertension in four cases.Vitrectomy with internal silicone oil tamponade improves the anatomical and functional prognosis of retinal detachment associated with giant retinal tear. Recurrent retinal detachment is usually due to relapsing proliferative vitreoretinopathy. The most frequent postoperative complications remain lens opacification and ocular hypertension.

We report a case of central serous chorioretinopathy in a patient who received a kidney transplant for chronic renal failure. We describe clinical and angiographic particularities and the progression of central serous chorioretinopathy in this case. Many factors can be involved in the occurrence of central serous chorioretinopathy in patients with kidney transplants, especially the hemodynamic problems related to renal failure and to the arterial hypertension that are often associated, the stress due to transplantation surgery, and the long-term corticotherapy prescribed postoperatively.

To analyze optic nerve head topography using confocal the scanning laser ophthalmoscope (CSLO) in ocular hypertension in comparison to normal eyes.We examined 25 patients with ocular hypertension and 27 normal controls. All of them had a complete ophthalmic examination. Optic nerve head analysis was performed by using a CSLO-type Heidelberg retina tomograph (HRT). The following stereometric parameters were evaluated: disc area, area and volume of cup, cup/disc ratio, area and volume of neuroretinal rim, mean and maximal cup depth, cup shape measure, height variation contour, mean retinal nerve fiber layer thickness, and retinal nerve fiber layer (RNFL) cross-section area.In ocular hypertension eyes, rim volume, height variation contour and RNFL thickness showed a statistically significant reduction compared to normal eyes.HRT appears to be an important tool in detecting early damage of retinal nerve fiber layer in ocular hypertension.

The purpose of this study is to show the result of outpatient cataract surgery and its differences or advantages with inpatient cataract surgery.The retrospective study concern 722 consecutives patients collected from June 1995 till May 1997; 388 underwent outpatient cataract surgery and 384 underwent inpatient cataract surgery.There were non significant difference between the two groups in type of cataract surgery. Immediate postoperative complications and final visual acuity did not show any significant difference in both groups.This analysis shows the great advantages of this simple surgical procedure such an increase in cataract operation leading to a decrease of patients in waiting list.

To evaluate the patient satisfaction as a performance indicator of quality of health care delivery in outpatient cataract surgery.We performed a prospective study using a questionnaire containing 14 items (preoperative work-up, administrative formalities, local anesthesia..) evaluating all the phases of ambulatory surgery for cataract.We collect 75 responding patients and we observed 2/3 of relative satisfaction about outpatient cataract surgery. Negative opinion was in relation with the prolonged time required for preoperative work-up which is done outside the outpatient surgery structure.Advances in techniques of locoregional anesthesia and surgery permit today the frequent use of ambulatory treatment in cataract surgery. This method seems to be well accepted by patients. However the prolonged time used to do the preoperative work-up require in the future a centralisation for this procedure.

But Evaluer l’efficacite des boli de methylprednisolone peri-operatoire (MPS) dans l’uveite anterieure compliquee de cataracte. Materiels et Methodes Etude prospective portant sur 19 yeux de 19 patients porteurs d’uveite anterieure chronique (UAC) non infectieuse compliquee de cataracte obturante ayant au moins 6 mois d’accalmie inflammatoire. D’apres differents criteres, trois groupes de patients ont ete individualises selon le risque d’inflammation post-operatoire. Les patients a faible risque ou groupe A (n = 7) etaient : uveite idiopathique, traitement topique sans corticodependance, absence ou synechies iridocristaliniennes (SIC) non significative. Les patients a risque intermediaire ou groupe B (n = 2) etaient : maladie dysimmunitaire oculaire ou uveite idiopathique avec ou sans corticodependance locale, ne necessitant pas de corticotherapie systemique, SIC non significatives. Les patients a haut risque ou groupe C (n = 11) etaient : uveite liee a une affection systemique, traitement per os (corticoides ou immunosuppressseur), SIC significatives (necessitant des manœuvres de dilatation pupillaire instrumentales). Chaque patient a beneficie d’une phakoemulsification avec implant intraoculaire. Seuls les patients du groupe C (sauf 2 cas contre-indiques) ont beneficie d’un bolus de MPS le jour de l’intervention (15 mg/kg/j en IV lente 3 jours de suite). La surveillance post-op etait clinique, avec fluoroangiographie a 6mois. La recherche de facteurs de risque (âge, sexe, etiologie de l’uveite et traitement, SIC, densite de la cataracte) a ete realisee par une analyse multivariee en regression logistique. Resultats Le recul moyen etait de 26 mois. L’âge moyen etait respectivement dans les groupes A, B et C a 40,7, 16 et 42,8 ans et un sex ratio de 1, 1 et 1,5. L’uveite etait surtout idiopathique dans les gpes A (4/7) et B (2/2) et dominee par la maladie de Behcet dans le groupe C (9/11). 8 reactions inflammatoires post-operatoires avec une bonne reponse therapeutique dans tous les cas ont ete notees : 2 cas dans le groupe A, 2 cas dans le groupe B, 4 cas dans le groupe C, sans effet secondaire deletere apres boli. Apres etude statistique, aucune reaction inflammatoire ni facteur de risque, en particulier dans le groupe C, n’etaient significatifs. Discussion Plusieurs protocoles ont ete proposes dans les cataractes post-uveitiques. Le boli par MPS est disponible, rapide et efficace. Conclusion Le boli peri-operatoire de MPS a permis pour les yeux uveitiques operes de cataracte, une maitrise de l’inflammation. Des etudes plus larges randomisees permettraient de confirmer ces resultats, et mieux definir les groupes « a haut risque » dans la chirurgie des yeux uveitiques.

The purpose of this study was to determine the prevalence of open angle glaucoma in a population aged 40 years and more living in a county in Tunis (Bardo).All subjects were examined according to the same protocol, which included applanation tonometry, examination of the anterior chambre and the optic disc. Goldmann and/or oculokinetic perimetry were performed in subjects with high intraocular pressure (22 mmHg or more) cup to disc ratio of 0.4 or more, optic disc palor, nasal deviation of vessels, presence of hemorhhage of the optic disc. Open angle glaucoma was defined by the presence of any two or all three of the following: high intraocular pressure, glaucomatous visual field defects, and the optic disc changes described above. In cases of denses media opacities the open angle glaucoma was defined when the intraocular pressure was more than 30 mmHg.The prevalence of open angle glaucoma in the 856 subjects was 2.68%. The prevalence increased with age: 0.54% in people 40 to 50 years, 1.71% in people 51 to 65 years and 50.63% in people 65 years or older. Glaucoma was unknown in 91% of the glaucomatous patients screened, and 30.4% had advanced glaucoma.The prevalence of open angle glaucoma in our study is slightly higher or similar to that in other white populations but lower than that reported in black populations.

Leaking filtering blebs must be repaired immediately to avoid major complications such as hypotony and endophtalmitis. We report four cases of leaking filtering blebs occurring between one to six years after trabeculectomy. Revealing signs were hypotony in two cases and athalamia in two cases. Patching was done in one case and conjunctival reinforcement by rotation of conjunctival flap in 2 cases and free conjunctival graft in one case. Leaking disappeared in all the cases. Normal intraocular pressure (10-12 mmHg) was obtained in all cases except one which was achieved by rotation of conjunctival flap and complicated by retraction of conjunctiva. Conjunctival reinforcement without exicion of the bleb is an easy technique and respect the filtration site. Major complication in our patients is retraction of conjunctival flap which can be avoided by: exicion of Tenon, good fixation to the cornea, and free conjunctival graft.

Introduction Le schwannome palpebral, egalement appele neurilemomme, est une tumeur nerveuse benigne se developpant aux depends des cellules de Schwann des nerfs peripheriques. Le schwannome se constitue rarement dans les tissus oculaires. Il a ete decrit parfois dans l’orbite, moins frequemment au niveau des tissus uveaux et la conjonctive, ou encore moins de la paupiere. Objectifs et Methodes Nous rapportons le cas de 3 patients presentant un schwannome palpebral. Observation Observation 1 : patient âge de 47 ans, qui consultait pour un nodule sous-cutane indolore de la paupiere inferieure droite. Observation 2 : patiente âgee de 20 ans, operee de chalazion de la paupiere inferieure gauche a 2 reprises et qui presentait une recidive. Observation 3 : patiente âgee de 17 ans qui presentait un chalazion recidivant de la paupiere superieure droite, opere quelques jours plus tot. Tous les patients ont beneficie d’une resection pentagonale. L’examen anatomopathologique a conclu a un schwannome dans les 3 cas. Discussion Dans la litterature, le schwannome palpebral est exceptionnel, a ce jour il n’en a ete reporte que 8 cas. Il pose des difficultes diagnostiques dont notre serie donne une illustration. C’est l’examen anatomopathologique qui a permis dans tous les cas de poser le diagnostic. Conclusion Le schwannome est une tumeur benigne rare, de diagnostic essentiellement anatomo-pathologique, qu’il faut evoquer devant tout nodule solide palpebral et tout particulierement devant tout chalazion recidivant.

The authors report two cases of macular oedema in two patients suffering from chronic renal failure. One of the patients, a women aged 36, with a renal transplant, presented with intra-retinal macular oedema, caused by a marked impairment of renal function. In the second case, the macular oedema, associated with papilloedema occurred in a patient aged 56, the day after renal dialysis and spontaneously regressed. The mechanism of macular oedema, seems to be related to modifications of the osmotic pressure and is probably associated with renal dialysis as shown in the second case.

But Rapporter l’observation d’une uveite anterieure aigue a hypopion chez un patient porteur d’une spondylarthrite ankylosante, avec evolution apres traitement, et confrontation aux donnees de la litterature. Objectifs et Methodes Examen ophtalmologique complet avec fluoroangiographie retinienne. Observation Un jeune homme de 22 ans, suivi depuis 5 ans pour spondylarthrite ankylosante, avec plusieurs episodes d’uveite anterieure aigue (UAA) dans ses antecedents, consultait pour un œil gauche (OG) rouge aigu depuis 2 jours, sans notion de traumatisme. A l’examen clinique de l’OG, l’acuite visuelle etait a 1/50.il existait une UAA avec hypopion et le fond de l’œil etait normal. L’acuite visuelle a l’œil droit etait a 10/10 et l’examen ophtalmologique etait normal, en particulier absence de sequelle d’UAA. La fluoroangiographie retinienne confirmait l’absence d’inflammatoire du segment posterieur aux deux yeux. Des le 5 e jour de corticotherapie per os a 1 mg/kg/j, l’evolution a ete nettement favorable, avec disparition totale de l’UAA, sans sequelles apres surveillance de plusieurs semaines. Le patient avait un typage HLA B27 positif. Discussion L’UAA avec hypopion, decrite au cours de certaines affections, comme la SPA, est habituellement correlee a un pronostic visuel severe. Dans la SPA, la survenue de cette forme particuliere d’UAA ne semble pas etre liee a un pronostic visuel severe. En effet, certaines etudes ont demontre que c’est l’absence du typage HLA B27 qui a ete le plus souvent associee a une evolution defavorable de l’UAA, avec formation de synechies irido-cristalliniennes et de cataracte. Pour d’autres auteurs, le pronostic visuel final ne depend pas de la presence ou non du typage HLA B27. Conclusion L’UAA a hypopion est l’une des manifestations inflammatoires du segment anterieur de la SPA. Son pronostic peut etre favorable, que l’affection soit associee ou non a la presence du typage HLA B27.

Familial exudative vitreoretinopathy (FEVR) is a well-defined inherited disorder of retinal vessel development. Association of FEVR with other congenital ocular diseases has already been described. We report the first clinical case of FEVR associated with hyaloid artery persistence. An 18-year-old Tunisian woman and four members of her family (mother, two brothers, and sister) presented signs of FEVR. Examination of this patient also revealed functional hyaloid artery persistence in her right eye. These two disorders have many common aspects in their pathological process, in particular vascular endothelial growth factor expression, which can explain this association.
