Background: Surgical programming is a priority for hospital management due to its impact on health. Objective: To analyze the aspects that affect the surgical programming in a secondary care hospital in Mexico. Material and methods: Hospital operating room programming records for 2021 were consulted. Surgical interventions, operating room usage times, causes of deferrals, surgical delay and turnover times were described. Percentages, mean and standard deviation were estimated; the analysis was carried out with chi-squared and Student's t. A panel of experts was integrated to identify the factors that affect the performance of operating rooms. Results: In 2021, the hospital had 2660 records of surgical programming for 2317 patients; 2.7% of surgeries were deferred. Mostly urgent (46.1%) and scheduled (43.5%) surgeries were performed. The average surgical delay time was 65 minutes and the turnover time was 92 minutes; these times were inadequate in emergency surgeries, in night and evening schedules surgeries (p < 0.05). Deficiencies in surgical time management and surgery scheduling with a degree of urgency = 7 days mainly affected the performance of the operating room. Conclusion: Surgical programming of the hospital presents areas of opportunity for improvement and shows factors that affect surgical management, useful for similar hospitals. [ABSTRACT FROM AUTHOR]

Background: Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease that affects both the upper and lower motor neurons, it has a heterogeneous clinical presentation, there are atypical variants that differ from the classic form of the disease. The criteria for diagnosis have evolved over time, with the support of electromyography (EMG), we present a patient series with these variants in which EMG was crucial to make the diagnosis. Clinical cases: Six cases are described with atypical presentation of motor neuron disease, for the isolated bulbar ALS phenotype, three cases are reported: two male patients (68 and 62 years old) and one woman (33 years old), with initial symptoms in the bulbar segment and late progression. to a second segment, corroborating characteristic findings by EMG. For the variant of Vulpian-Bernhardt syndrome (VBS), two male patients aged 82 and 72 years are reported, with initial symptoms in the thoracic segment with electromyographic support for the diagnosis; Finally, a case of amyotrophic diplegia of the legs (APD) is described in a 50-year-old female patient with symptoms isolated to the pelvic limbs, with a slow clinical evolution, corroborated by EMG with involvement of other spinal segments. Conclusions: ALS is part of the spectrum of motor neuron diseases, a neurodegenerative disease of the central nervous system, without curative treatment and with a fatal outcome. The diagnosis of ALS is complex and becomes more complicated for atypical phenotypes, as observed in the cases presented, EMG is an essential part of the approach and is part of the most recent diagnostic criteria. Introduction. The new era of surgery is increasingly dependent on technology, and an example of this is the widespread use of electrocautery as a primary part of surgical practice. Surgical smoke is a byproduct of the dissection and coagulation of tissues produced by energy equipment, which represents multiple potential health risks for the surgical group; however, the dangers caused by cumulative exposure have been minimized. Methods. A critical analysis was carried out from a reflective position of the available information, establishing the possible risks related to exposure to surgical smoke. Discussion. The imperative need to establish national normative guidelines and standardized recommendations to comply with the demands given by the occupational health and work safety management systems, whose main objective is to make effective the use of appropriate surgical masks, implementation of environmental epidemiological surveillance program in the operating room, prioritizing the constant use of vacuum cleaners and evacuation systems, and carrying out educational awareness programs aimed at the personnel involved. Likewise, there is concern about the need for new studies to more precisely define the danger of this aerosol. Conclusion. Objective: To determine the standard surgical times of the four most common general surgery procedures (unilateral inguinal hernioplasty, bilateral inguinal hernioplasty, umbilical hernioplasty and cholecystectomy) in a second-level hospital and to estimate the probability of extending the time of each of the procedures. Efficiency is a widely studied subject in economics. It involves the need for fewer elements in the production of a certain level of goods and services. Therefore, it is extremely important to consider it in the field of surgery. Materials and methods: An observational, descriptive and retrospective study. It used the operating room records from 2017 to 2019 of the General Surgery service in a second-level hospital. Based on this information, the time required for each procedure was standardized using the mean for each one (umbilical hernioplasty, unilateral or bilateral inguinal hernioplasty and cholecystectomy). The probability of extending surgical times was estimated based on the obtained data and confidence interval. Results: The mean for unilateral inguinal hernioplasty was 76 min (95.00 % CI: 72-80 min, SD 23), for bilateral inguinal hernioplasty 104.38 min (95.00 % CI: 91-116 min, SD 41.7), for umbilical hernioplasty 59.31 min (95.00 % CI: 54-63 min, SD 29.99) and for cholecystectomy 85.735 min (95.00 % CI: 83-88 min). The probability of scheduling three surgical interventions and completing all of them on time (upper limit of the CI) is 92.69 %, and the probability of scheduling three surgical interventions and extending the time of all of them is 0.0016 % (lower limit of the CI). Conclusions: Planning scheduled operations using standardized surgical times is feasible. Updated statistics on surgical procedures (average time for each procedure) are required since it is possible to more accurately detect and supervise operating room dynamics by identifying opportunity areas. Introduction: Categorizing surgical emergencies is necessary due to the continued imbalance between the supply and demand of surgical services in most institutions where the service is enabled. There are approaches to the subject, with strategies for prioritizing surgical cases, which consider scales and flowcharts, but their lack of external validity and the particularities of the institutions and insurers have limited the generalization of the results. Methods: A conceptualization of the triage of surgical emergencies is carried out with critical and reflective approaches supported by evidence. Potential research opportunities are also identified. Discussion: The potential benefits of surgical triage in emergent situations are extensive to all health system actors, reducing the possibility of adverse outcomes and economic repercussions for institutions and insurers. Queuing theory offers support for understanding the issue and contributes to solutions. However, its adoption is scarce in an emergency as part of a local surgical prioritization strategy. Conclusion: The creation of strategies that establish triage for the patient with a surgical emergency is influenced by the continuous and effective participation of the actors involved in the process and its impact on clinical outcomes. Objetivo: Determinar los tiempos quirúrgicos estándar de los cuatro procedimientos más comunes en cirugía general (hernioplastia inguinal unilateral, hernioplastia inguinal bilateral, hernioplastia umbilical y colecistectomía) de un hospital de segundo nivel y calcular la probabilidad de extensión de cada uno de los procedimientos. La eficiencia es un fenómeno ampliamente estudiado en el ámbito económico, pues hace referencia a la necesidad de menor cantidad de factores para la producción de un determinado nivel de bienes y servicios, por ello, es de vital importancia incluirlo en el ámbito quirúrgico. Materiales y métodos: Estudio observacional, descriptivo y retrospectivo. Se utilizaron los registros de quirófano de un hospital de segundo nivel del año 2017 al 2019 del servicio de Cirugía General. A partir de esta información, se estandarizó el tiempo necesario para cada procedimiento mediante la media de cada uno (hernioplastia umbilical, hernioplastia inguinal unilateral o bilateral y colecistectomía). Se calculó la probabilidad de extensión de las cirugías tomando en consideración los datos obtenidos y el intervalo de confianza. Resultados: Para el procedimiento de hernioplastia inguinal unilateral se obtuvo una media de 76 min (IC 95,00 %: 72-80 min, DE 23); en hernioplastia inguinal bilateral, una media de 104,38 min (IC 95,00 %: 91-116 min, DE 41,7); en hernioplastia umbilical, una media de 59,31 min (IC 95,00 %: 54-63 min, DE 29,9), y en colecistectomía, una media de 85,735 min (IC 95,00 %: 83-88 min). La probabilidad de que se programen tres cirugías y todas estén a tiempo (límite superior del IC) es de 92,69 %, la probabilidad de que se programen tres cirugías y todas se prolonguen es de 0,0016 % (límite inferior del IC). Conclusiones: Es posible realizar la planeación de las cirugías programadas mediante el uso de tiempos quirúrgicos estandarizados. Se requiere contar con estadística actualizada de los procedimientos quirúrgicos (promedios del tiempo de realización de cada procedimiento), ya que es posible detectar y supervisar de manera más precisa la dinámica de quirófano mediante la detección de las áreas de oportunidad, de esta manera, se eficientizará el tiempo de quirófano para beneficio de los sistemas de salud y los pacientes.

Introducción. La nueva era de la cirugía es cada vez más dependiente de la tecnología, y un ejemplo de ello es el uso generalizado de electrocauterio como parte primordial de la práctica quirúrgica. El humo quirúrgico es un subproducto de la disección y la coagulación de los tejidos producidas por los equipos de energía, que representa múltiples riesgos potenciales para la salud del grupo quirúrgico, sin embargo, se han minimizado los peligros causados por la exposición de manera frecuente y acumulativa a este aerosol. Métodos. Se realizó un análisis crítico, desde una posición reflexiva de la información disponible, estableciendo los posibles riesgos relacionados con la exposición al humo quirúrgico. Discusión. Es visible la necesidad imperativa de establecer directrices nacionales, pautas normativas y recomendaciones estandarizadas para cumplir con las exigencias dadas por los sistemas de gestión en salud ocupacional y seguridad del trabajo, cuyo objetivo principal es hacer efectivo el uso de mascarillas quirúrgicas apropiadas, la implementación de programa de vigilancia epidemiológica ambiental en sala de cirugía, la priorización del uso constante de aspiradores y sistemas de evacuación, y la ejecución de programas educativos de sensibilización dirigidos al personal implicado. De igual manera, se abre la inquietud de la necesidad de nuevos estudios para definir con mayor precisión el peligro de este aerosol. Conclusión. Se recomienda de manera responsable utilizar todas las estrategias preventivas existentes para intervenir en salas de cirugía los riesgos minimizados y olvidados del humo quirúrgico.

Introducción: La categorización de las urgencias quirúrgicas es una necesidad en razón al continuo desbalance entre la oferta y la demanda de servicios quirúrgicos en la mayoría de las instituciones donde se encuentra habilitada la prestación del servicio. Hay abordajes en el tema, con estrategias de priorización de los casos quirúrgicos, que consideran escalas y flujogramas, pero su ausente validez externa y las particularidades de las instituciones y aseguradores, han limitado una generalización de los resultados. Métodos: Se efectúa una conceptualización del triaje de las urgencias quirúrgicas con planteamientos críticos y reflexivos soportados en la evidencia. Se identifican, asimismo, las posibles oportunidades para la investigación. Discusión: Los beneficios potenciales de un triaje quirúrgico en situaciones de urgencia, son extensivos a todos los actores del sistema de salud, disminuyen la posibilidad de desenlaces y repercusiones económicas negativas para las instituciones y los aseguradores. La teoría de las colas ofrece el soporte para un entendimiento del tema y contribuye en las soluciones. Su adopción es escasa como parte de una estrategia local de priorización quirúrgica en un contexto de urgencia. Conclusión: La creación de estrategias que establezcan el triaje para el paciente con una urgencia quirúrgica están influenciadas por la participación continua y efectiva de los actores involucrados en el proceso y en su impacto en los desenlaces clínicos.

As surgeons we must ask ourselves three questions: • Is climate change and its impact on the environment a reality? • What responsibility do we have as surgeons to act and mitigate and reverse this impact? • What strategies can we implement to achieve this?

Introduction: Prevention, identification, analysis and reduction of adverse events (AEs) are all activities designed to increase safety of care in the clinical setting. Closed claims reviews are a strategy that allows to identify patient safety issues. This study analyzes adverse events resulting in malpractice lawsuits against anesthesiologists affiliated to an insurance fund in Colombia between 2013-2019. Objective: To analyze adverse events in closed medicolegal lawsuits against anesthesiologists affiliated to an insurance fund between 2013-2019. Methods: Cross-sectional observational study. Convenience sampling was used, including all closed claims in which anesthesiologists affiliated to an insurance fund in Colombia were sued during the observation period. Variables associated with the occurrence of AEs were analyzed. Results: Overall, 71 claims were analyzed, of which 33.5% were due to anesthesia-related AEs. Adverse events were found more frequently among ASA I-II patients (78.9%), and in surgical procedures (95.8%). The highest number of adverse events occurred in plastic surgery (29.6%); the event with the highest proportion was patient death (43.7%). Flaws in clinical records and failure to comply with the standards were found in a substantial number of cases. Conclusions: When compared with a previously published study in the same population, an increase in ethical, disciplinary and administrative claims was found, driven by events not directly related to anesthesia. Most of the anesthesia-related events occurred in the operating room during surgical procedures in patients and procedures categorized as low risk, and most of them were preventable. Tras el inicio de la pandemia en marzo de 2020, alrededor de 3,5 millones de personas han muerto a causa del COVID-19, afectando la operatividad en todos los sistemas de salud. En consecuencia, se ha intentado llevar a un segundo plano la atención quirúrgica electiva, incluso diferirse, tratándose de un tema crítico, pues equilibrar el beneficio quirúrgico con el riesgo desconocido a contraer COVID-19 y sus complicaciones en pacientes, y personal médico, permanece latente. Sin embargo, las cirugías deben practicarse eventualmente por comprometer pronóstico y funcionalidad de algunos individuos, lo cual ha permitido recuperar el número de cirugías electivas bajo condiciones especiales, siendo un factor clave en la administración sanitaria. La presente revisión resalta la necesidad de establecer prácticas y/o protocolos clínicos seguros para prevenir contagios por COVID-19 en quirófano. Para ello, se realizó la búsqueda en repositorios virtuales, PubMed y Google Scholar utilizando descriptores sobre el tema en artículos científicos y páginas web institucionales, evidenciándose actualizaciones sobre medidas de protección para el manejo perioperatorio del paciente, destacando que la detección de COVID-19 mediante pruebas diagnóstico, es esencial para proteger a este y al personal médico-quirúrgico del riesgo infeccioso, especialmente durante el período intraoperatorio. Asimismo, el aislamiento obligatorio de pacientes con criterio quirúrgico positivos para COVID-19 o aquellos con sintomatología sospechosa para el virus, minimizando la propagación del patógeno durante su hospitalización. After the beginning of the pandemic in March 2020, around 3.5 million people have died from COVID-19, affecting the operation of all health systems. Consequently, an attempt has been made to take elective surgical care to the background, even its deferral, being this a critical issue, since the surgical benefit is balanced with the unknown risk of contracting COVID-19 and its complications in patients, and medical personnel, remains latent. However, surgeries must eventually be performed due to compromising prognosis and functionality of some individuals, which has made it possible to recover the number of elective surgeries under special conditions, being this a key factor in health administration. This review highlights the need to establish safe clinical practices and / or protocols to prevent COVID-19 infections in the operating room. For doing this, a research was carried out in virtual repositories, PubMed and Google Scholar using descriptors on the subject in scientific articles and institutional web pages, evidencing updates on protection measures for perioperative patient management, highlighting that detection of COVID-19 through diagnostic tests are essential to protect the patient and the medical-surgical personnel from infectious risk, especially during the intraoperative period. Likewise, the mandatory isolation of patients with positive surgical criteria for COVID-19 or those with suspicious symptoms for the virus, minimizing the spread of the pathogen during their hospitalization time.

Aim: To determine the frequency, the causes and the associated factors of the surgical cancellation of scheduled surgeries at the Hospital Base Valdivia between the years 2014 and 2018. Materials and Method: A descriptive cross-sectional study was carried out. Were described the frequency of suspension of scheduled surgeries of the establishment, by specialty and their main causes, identifying those that can be avoided. In addition, the factors associated with suspension were identified by logistic regression. Results: The suspension rate in the 5 years of study was 11.2%. Neurosurgery and Traumatology had the highest frequency of surgical cancellation (18.8% y 13.9%, respectively), Obstetrics, and Gynecology the lowest (4.1%). The most frequent causes of suspension were the absence of the patient (16.9%), the prolongation of the previous surgery (16.4%) and the suspension of activities due to Union reasons (7.9%). 80.1% of the causes were avoidable. The age and surgical specialty were the most relevant associated factorsmDiscussion: A high rate of surgical suspensions and most for avoidable reasons were evident. Reducing surgical cancellations can be the most cost effective intervention to help reduce the extensive post-health crisis surgical waiting lists by COVID 19, as it only requires optimizing existing resources. Conclusions: Surgical suspension is a common problem in the surgical process. Objective: to identify the leadership perceived by perioperative nurses and to determine the role of leadership predictors of patient safety in the operating room. Method: descriptive and explanatory study in a sample of 1,001 nurses, using the patient safety questionnaire in the operating room and Quinn’s leadership instrument adapted to health. The IBM SPSS Statistics application, version 25.0, was used in the data processing. Result: all leadership dimensions were higher than the midpoint of the scale (M>4). A minimum value was observed in the role of Innovator (M=4.63) and maximum in the Producer (M=5.04). The regression models allowed pointing out that leadership roles are predictors of patient safety in the operating room, especially the roles Monitor, Producer and Facilitator. Conclusion: perioperative nurses have moderately positive perception of leadership exercise, indicating the need to promote the development of different leadership roles exercised by perioperative management nurses. Dada la incertidumbre que acompaña a la pandemia por SARS-CoV-2 y ante la necesidad de dar respuesta a múltiples problemas de salud crónicos y agudos que afectan a la población general, incluidos aquellos que precisan de una intervención quirúrgica, se presentan las recomendaciones implementadas en clínicas y hospitales de Colombia como guía para lograr una reapertura de los servicios de cirugía electiva de forma segura, escalonada y monitoreada acorde a la dinámica que impone la pandemia, la regulación nacional e internacional y la velocidad en la producción de evidencia científica relacionada con la COVID-19.

El proceso de análisis de la percepción de riesgos biológicos a tener en cuenta en un quirófano es complejo, ya que, a menudo, es insuficiente el conocimiento sobre las normas de bioseguridad en el área quirúrgica de un Hospital General. El objetivo del presente trabajo es desarrollar un análisis de la percepción del personal de salud sobre riesgos biológicos en quirófano a través de un Mapa Cognitivo Neutrosófico que permita recomendar las medidas de bioseguridad a tener en cuenta en quirófano de un Hospital General, la investigación es aplicativa con intervención tecnológica que analiza la percepción del personal de salud sobre riesgo biológico y recomienda las medidas de bioseguridad a tener en cuenta en quirófano de un Hospital General. En el trabajo se presenta un Mapa Cognitivo Neutrosófico útil para analizar la percepción del personal de salud en quirófano y recomendar las medidas de bioseguridad de mayor importancia a tener en cuenta en quirófano de un Hospital General, alcanzándose la identificación de la percepción del personal de salud sobre riesgos biológicos en quirófano

Introduction: the term risk in the field of health services refers immediately to a hazard for the patient in an unexpected way and almost always having an unfavorable result, the responsible practice of nursing is delimited by the strict compliance with moral and legal standards, quality and efficiency are responsibilities of the nursing personnel, as protective factors in the prevention of errors in the care of patients, in recent times there has been a deeper reflection on the presence of adverse events and the resulting error in the care of patients Objective: to determine the existence of risks and their management in the operating room at 1º de Mayo hospital in Cabinda, Angola, from March to May 2017. Methods: the target group included 17 nurses who worked in the operating room, and the Chief Nurse. Out of the surgical interventions performed in the chosen period, those completed on April were observed. Results: it was found that the largest number of nurses who worked in the operating room completed the proper technical rank and have more than 10 years of experience in the surgical and administrative service, the medical and biological risks were identified, mainly the ones related to disinfection methods, the permanence of open doors and collection of residuals. Conclusions: it is recommended to develop an administrative strategy to increase the safety of patients in the surgical room.

Introducción. La cancelación de cirugía programada es un indicador de calidad que puede relacionarse con ineficiencia en los procesos de planificación estratégica para la seguridad del paciente y es un fenómeno frecuente en las instituciones de salud. Objetivo. Identificar las causas de cancelación de cirugía programada durante el segundo trimestre de 2016 en una clínica de alta complejidad de Popayán, Colombia. Materiales y métodos. Estudio descriptivo retrospectivo de corte transversal con 185 cirugías canceladas de 2 787 programadas. La información se obtuvo de los registros magnéticos de la coordinación de cirugía de la institución objeto de estudio y se realizó una distribución del evento según los actores del Sistema General de Seguridad Social en Salud (SGSSS): asegurador, prestador y paciente. Resultados. La proporción de cancelación fue del 2.7%. El 56.7% de las causas fueron atribuidas al paciente, el 40.5% al prestador y el 2.7% al asegurador. Conclusiones. Las causas de cancelación de cirugía programada identificadas son altamente prevenibles y evidencian fallas organizacionales y procedimentales que requieren mejoramiento a nivel administrativo y asistencial para disminuir los costos de no calidad.

Background: Most healthcare incidents occur in the operating room. Thus, it is essential to implement patient safety policies. Objective: Characterize perioperative nurses' perceptions of patient safety in the operating room. Methodology: Descriptive study using sampling by clusters of hospitals. The sample consisted of1,001 nurses from 46 operating rooms. The Patient Safety in the Operating Room Questionnaire was applied and IBM SPSS Statistics software, version 25.0, was used for data processing. Results: The analysis of the percentages of positive answers revealed that most patient safety dimensions have a low level of implementation (<50%), particularly those regarding audits. Only the dimensions concerning good practices in unambiguous patient identification and antimicrobial resistance and infection control and prevention had high levels of implementation (≥75%). Medication errors represent one of the main causes of incidents and adverse events during the perioperative period. Therefore, this study analyzes errors before, during, and after the administration of general anesthesia for abdominal surgery at a high-complexity hospital in Bogota, Colombia. A descriptive cross-sectional study was conducted with 390 patients between January and September 2019. Of the 3,677 medication administrations, some type of error was made in 60% of cases, mostly in emergency surgeries. The pharmacological group with the most errors was general anesthetics, with 32%. All identified errors constituted situations with harm potential, indicating the need to promote the standardization of activities involving the use of medications and a culture of healthcare safety in order to avoid adverse events. La seguridad del paciente es uno de los aspectos de mayor relevancia en la atención en un quirófano. El trabajo en equipo y coordinado, sumado al liderazgo, permite que los errores sean menos. Mantener un equipo estable en el quirófano, que conozca bien los procedimientos, y donde cada participante sabe el rol que juega, es uno de los factores más importantes para lograr un trabajo eficiente, con disminución de las complicaciones y del tiempo quirúrgico. Comparar el trabajo en el quirófano con una orquesta sinfónica, nos ayuda a entender la importancia del trabajo coordinado.

Introduction: The need for surgery in the context of the current COVID-19 pandemic may be a risk for health professionals. For urgent diseases requiring surgical interventions, appropriate decisions should be made according to international recommendations on management of the SARS-CoV2 virus. The study aims to describe and analyze the different recommendations for managing suspected COVID-19 patients in surgical settings. Materials and Methods: An integrative descriptive literature review was conducted on articles published between November 2019 and April 2020 in PubMed, Medline, Science-Direct, Google Scholar, and Scopus databases. Results: 12 studies related to measures and recommendations for the management of suspected COVID-19 patients met the inclusion criteria and were selected. During preoperative care, aspects such as the preparation of the operating room, the use of personal protection equipment, and patient transfer to the operating room were highlighted. During intraoperative care, measures for anesthesia, airway management, cleaning and disinfection of operating rooms, and specific recommendations for postoperative care are highlighted. Discussion: Personal protective equipment and techniques to minimize the risk of cross-infection are reviewed. Conclusions: The surgical team should maintain standard measures for the management of surgical suspected COVID-19 patients to prevent the spread and possible infection with SARS-CoV-2 virus and ensure safe surgical care. Background: Improving surgical safety is one of the objectives of the National Patient Safety Plan. Assessing the perception of nurses will contribute to a greater commitment to its application. Objectives: To construct and validate a questionnaire to evaluate patient safety in the operating room (OR). Methodology: Methodological research study. Development of a questionnaire, submitted to a Delphi panel, and study of its psychometric characteristics in a sample of 1,001 nurses. Construct validity study carried out by exploratory factorial analysis with Varimax rotation and convergent and discriminating validity. Its reliability was assessed using Cronbach's alpha coefficient (α). Results: The validation by the Delphi panel resulted in a version composed of 79 items, which assess nine areas of patient safety (APS). Correlation matrices (p > 0.00) and Kaiser-Meyer-Olkin measures > 0.8 in the APS 9 ensure good adequacy of the factorial model, which groups the items into 19 dimensions with α values between 0.66 and 0.98. Objective: Measuring instruments play, namely, an important role in the safety and quality of care. The aim of this study consists in validating a scale conceived to evaluate the structure of the operating room (OP). Materials and Methods: Validation and psychometric evaluation were carried out of the instrument. The sample was comprised of 1019 professionals working in the OP for at least two years in 71 Portuguese hospitals; the research was conducted in 2017. Results: The results obtained in the reliability and validity tests reveal good internal consistency. The 28 items of the scale, and after the principal components analysis, were grouped into seven dimensions: "Environment and equipment", "Resources for quality and safety", "Circuits in the operating room", "Facilities and operating requirements", "Training and praxis in the operating room", "Continuity in nursing care", and "Specificities of professional groups". Conclusion: The psychometric study allows us to state that the scale of structure indicators in the operating room (SIOR) is a reliable and valid instrument. AbstractOBJECTIVEEvaluate pre- and intraoperative practices adopted by medical and nursing teams for the prevention of surgical infections.METHODA prospective study carried out in the period of April to May 2013, in a surgical center of a university hospital in Belo Horizonte, Minas Gerais.RESULTS18 surgeries were followed and 214 surgical gloves were analyzed, of which 23 (10.7%) had postoperative glove perforation detected, with 52.2% being perceived by users. Hair removal was performed on 27.7% of patients in the operating room, with the use of blades in 80% of the cases. Antibiotic prophylaxis was administered to 81.8% of patients up to 60 minutes prior to surgical incision. An average of nine professionals were present during surgery and the surgery room door remained open in 94.4% of the procedures.CONCLUSIONPartial adhesion to the recommended measures was identified, reaffirming a need for greater attention to these critical steps/actions in order to prevent surgical site infection.

Identificar los conocimientos y el tiempo de exposición a riesgos laborales del personal adscrito a quirófano en un hospital para personas sin seguridad social. Métodos: Estudio de tipo descriptivo, transversal y prolectivo. Criterios de inclusión: Personal con más de dos años trabajando en el quirófano. Muestreo no probabilístico y consecutivo, n= 140 personas. Se diseñó un cuestionario anónimo que constó de dos áreas: la primera de conocimientos con 14 preguntas, con la puntuación de: 0-5 aciertos correctos = desconocen, 6-10 aciertos correctos = conocen poco, 11 o más aciertos correctos = sí conocen. La segunda área constó de 22 preguntas con los días de exposición: De 25 días a más y no expuestos. Se aplicó una prueba piloto y se obtuvo una confiabilidad con alfa de Cronbach de 0.71 La recolección de datos se realizó en un periodo de 3 meses. Resultados: Es poco significativo el desconocimiento que tienen los servidores del área quirúrgica; se encontró que 55.7% están expuestos a riesgos biológicos, a riesgos físicos 66.4%, a riesgos químicos 58.6%, en cuanto a riesgos ergonómicos el trabajar de pie ocupó el 60.7% y sentado durante la jornada 80.0%. En cuanto a riesgos psicológicos, los trabajadores se someten a estrés laboral en un 61.4%. Conclusiones: La falta de conocimientos aunado al tiempo de exposición incrementa el riesgo de padecer alguna enfermedad laboral. Introduction: Intraoperative critical events are rare situations and their management in some occasions may be inaccurate or not done in time. Objective: To translate and update checklists for the management of critical events in the operating room developed by Adriadne Labs, based on secondary clinical evidence. Materials and methods: For the translation and update of checklists, we followed the recommendations of Adriadne Labs for the modification of the checklists according to a systematic methodology divided in steps: 1) translation of the original checklists; 2) systematic research of the literature; 3) evaluation and selection of the evidence; 4) adaptation of the checklist for the management of local anesthetic toxicity; 5) changes, subtractions, and additions to the translated lists, and 6) diagramming of the checklists. Results: We translated to Spanish the 12 original checklists and adapted a new checklist for the management of local anesthetic systemic toxicity. As a result of the systematic research of the literature, we retrieved 1.407 references and selected 7 papers that were used to update the new checklists based on evidence. The new checklists were diagrammed according to the recommendations of design of the original lists. Surgical care is an integral component of healthcare services in all countries and its quality is evaluated through monitoring of national indicators of safe surgical and anaesthesia care. In Peru, monitoring is only partial because of the lack of data regarding some of the indicators. However, there is a need for comprehensive monitoring in order to gain knowledge of the progress towards the healthcare goals proposed by the World Health Organisation and The Lancet Commission Global Surgery. OBJECTIVE: to verify the effectiveness of warmed intravenous infusion for hypothermia prevention in patients during the intraoperative period. METHOD: experimental, comparative, field, prospective and quantitative study undertaken at a federal public hospital. The sample was composed of 60 adults, included based on the criteria of axillary temperature between 36ºC and 37.1ºC and surgical abdominal access, divided into control and experimental groups, using the systematic probability sampling technique. RESULTS: 22 patients (73.4%) from both groups left the operating room with hypothermia, that is, with temperatures below 36ºC (p=1.0000). The operating room temperature when patients arrived and patients' temperature when they arrived at the operating room were statistically significant to affect the occurrence of hypothermia. CONCLUSION: the planning and implementation of nursing interventions carried out by baccalaureate nurses are essential for preventing hypothermia and maintaining perioperative normothermia. OBJETIVO: verificar la eficacia de la intervención de infusión venosa calentada en la prevención de la hipotermia en pacientes en el período intraoperatorio. MÉTODO: estudio experimental, comparativo, de campo, prospectivo y cuantitativo, en un hospital público federal. La muestra abarcó a 60 adultos, que tuvieron como uno de los criterios de inclusión la temperatura axilar entre 36ºC y 37,1ºC y acceso quirúrgico abdominal, divididos en grupos control y experimental, compuestos utilizándose la técnica de muestreo probabilístico sistemático. RESULTADOS: en los 2 grupos, 22 pacientes (73,4%) salieron del quirófano con hipotermia, o sea, temperatura inferior a 36ºC (p=1,0000). La temperatura del quirófano cuando de la entrada del paciente y la temperatura del paciente cuando de la entrada en el quirófano fueron estadísticamente significativas para influir en la ocurrencia de hipotermia. CONCLUSÍON: la planificación e implementación de las intervenciones de enfermería practicadas por el enfermero son esenciales para prevenir la hipotermia y mantener la normotermia perioperatoria. OBJETIVO: verificar a eficácia da intervenção de infusão venosa aquecida, na prevenção da hipotermia em pacientes no período intraoperatório. MÉTODO: estudo experimental, comparativo, de campo, prospectivo e quantitativo, realizado em um hospital público federal. A amostra foi constituída por 60 adultos, que tiveram, como um dos critérios de inclusão, a temperatura axilar entre 36 e 37,1ºC e acesso cirúrgico abdominal, divididos em grupos controle e experimental, compostos utilizando-se a técnica de amostragem probabilística sistemática. RESULTADOS: nos 2 grupos, 22 pacientes (73,4%) saíram da sala de operação com hipotermia, ou seja, temperatura inferior a 36ºC (p=1,0000). A temperatura da sala de operação na entrada do paciente e a temperatura do paciente na entrada da sala de operação foram estatisticamente significativas para influenciar a ocorrência de hipotermia. CONCLUSÃO: o planejamento e implementação das intervenções de enfermagem, realizadas pelo enfermeiro, são essenciais para prevenção da hipotermia e manutenção da normotermia perioperatória.

Introducción. Existe una especial preocupación por las características de la comunicación en las salas de cirugía, dado el impacto que esta tiene en los resultados tanto en el paciente como en las relaciones entre los diferentes grupos de trabajo que interactúan en tales situaciones. Materiales y métodos. Se hicieron la observación y el registro en tiempo real de fallas de comunicación durante los procedimientos quirúrgicos de diferentes especialidades y complejidades. Se hizo un análisis estadístico de las frecuencias de los errores más comunes y un análisis "etnográfico" caracterizando los tipos de errores en la comunicación. Resultados. Se hicieron la observación y el registro en tiempo real de 179 fallas de comunicación en 35 procedimientos quirúrgicos, cuya complejidad era alta en 17 %, baja en 40 % y mediana en 43 %; en 80 % tuvo lugar en horas de la tarde, en 60 % se usó anestesia general, y raquídea y local en 20 % en cada una. En 92 % se encontraba presente, al menos, un residente; en 54 % la falla se debió al receptor, 84,3 % ocurrieron durante la cirugía y se presentaron más de tres fallas en un mismo procedimiento en 71,5 % de las cirugías. Discusión. Existen múltiples variables y factores que afectan el proceso de comunicación y su desarrollo en salas de cirugía es especialmente "sensible" por el tipo de actividad que allí se lleva a cabo. Si se encuentran los diferentes tipos de patrones y fallas en la comunicación, se pueden generar e implementar acciones para mejorarlas de tal forma que podrían disminuirse los efectos en la morbimortalidad que generan estas fallas en la comunicación.Introduction. There exists special concern about the patterns of communication in the operating rooms, given their impact on patient outcome and on the relationships among the working groups interacting in this scenery. Materials and methods. This is an observational study with real time registry of the failure of communication in the course of surgical procedures in the different specialties and different degrees of complexity. Statistical analysis of the frequency of the most common errors and ethnographic analysis characterizing the types of miscommunication were completed. Results. Real time observation and registry of 179 communication failures in 35 surgical procedures, 17% high complexity, low complexity 40% and median complexity 43%; 80% occurred at noon time and 60% in the afternoon; general anesthesia was used in 60% of cases, spinal anesthesia in 20%. In 92% of cases at least, a resident was present; in 54% failure was due to the receptor, 84,3% occurred during surgery, and more than three failures in the same procedure occurred in 71,5% of the surgical procedures. Discussion. There are several variables and factors that affect the communication process and the operating rooms are especially prone to errors considering the type of activity that takes place there. Identifying the different patterns of failure in communication permits generating actions to diminish their impact on patient morbidity and mortality that may result from miscommunication.
