Abstract

Objectives: Cervico-facial haemorrhages are an indication for surgeon and operative neuroradiologist cooperation. Patient with traumatic vascular lesions, stabilised according to Advanced Trauma Life Support procedures, is entrust to the Maxillo-Facial surgeon to manage the bleeding. Lesions of inaccessible vessels, dangerous for haemorrhagic shock and airways obstruction, are treated according to Damage Control principles to ensure to realize neuroradiologic diagnostic and operative procedures. Methods: Angiography is performed by femoral microcatheterism; if vascular lesion is identified (10% of cases) embolization is performer by use of Enducrilate + Lipiodol 1:3. In case of widespread bleeding with poorly defined origin (90% of cases) the material of choice is Polyvinyl Alcohol particles (150–300 mm). Results: All cases treated to Niguarda Trauma Center Hospital in Milan had a good management of haemorrhages with neuroradiologic diagnostic and operative procedures. The minority of them needed a secondary embolization to treat anastomotic bleeding paths. No ischaemical damage occurred. Conclusion: Evolution of neuroradiologic endovascular procedures offers to Maxillo-Facial Surgeons a mini-invasive alternative to manage life-threatening bleeding refractory to any local haemostatic measures. This kind of treatment is simple, repeatable, counter-indicated only in patients with hypersensivity to iodine. Surgical ligature of neck blood vessel must be avoided because of frequent onset of collateralisation of blood supply. This procedure may exclude neuroradiologic diagnostic and operative procedure

Abstract

In patients with ischemic stroke, endovascular treatment results in a higher rate of recanalization of the affected cerebral artery than systemic intravenous thrombolytic therapy. However, comparison of the clinical efficacy of the two approaches is needed.We randomly assigned 362 patients with acute ischemic stroke, within 4.5 hours after onset, to endovascular therapy (intraarterial thrombolysis with recombinant tissue plasminogen activator [t-PA], mechanical clot disruption or retrieval, or a combination of these approaches) or intravenous t-PA. Treatments were to be given as soon as possible after randomization. The primary outcome was survival free of disability (defined as a modified Rankin score of 0 or 1 on a scale of 0 to 6, with 0 indicating no symptoms, 1 no clinically significant disability despite symptoms, and 6 death) at 3 months.A total of 181 patients were assigned to receive endovascular therapy, and 181 intravenous t-PA. The median time from stroke onset to the start of treatment was 3.75 hours for endovascular therapy and 2.75 hours for intravenous t-PA (P0.001). At 3 months, 55 patients in the endovascular-therapy group (30.4%) and 63 in the intravenous t-PA group (34.8%) were alive without disability (odds ratio adjusted for age, sex, stroke severity, and atrial fibrillation status at baseline, 0.71; 95% confidence interval, 0.44 to 1.14; P=0.16). Fatal or nonfatal symptomatic intracranial hemorrhage within 7 days occurred in 6% of the patients in each group, and there were no significant differences between groups in the rates of other serious adverse events or the case fatality rate.The results of this trial in patients with acute ischemic stroke indicate that endovascular therapy is not superior to standard treatment with intravenous t-PA. 41. Transradial Cerebral Angiography: Technique and Indications: A Single Centre Study

Abstract

Transradial angiography is a useful tool for diagnostic and interventional neuroangiographic procedures. It is routinely used for coronary angiography but there are few reports on the technique in neuroradiolocgical literature. Between April 2004 and February 2005 we enrolled 67 patients for transradial selective cerebral catheterization. We performed 56 diagnostic procedures, four interventional and seven failed accesses. Sheaths were used in all cases ranging in size from 4F to 6F. Standard shape angled Berenstein and Simmons (I, II) 4F catheters were used for three- and four-vessel diagnostic arteriography and 40° 5F and 6F guide catheters for interventional procedures (two external carotid, one vertebral embolization and one left cervical ICA stenting). Periprocedural complications included failure to access the radial/brachial artery (five patients), and transient radial artery spasm with pain (two patients). There were no major complications and/or deficits related to cannulation of the radial artery. Compared with transfemoral and transbrachial approaches, transradial access is a less invasive and safer technique for neuroangiography. Anticoagulant or antiplatelet therapy need not be discontinued for this method. All major vessels can be accessed for diagnostic studies and interventions in the vertebral system and in selected conditions of carotid anatomy are facilitated.
