Abstract Background Bone fractures are a global public health issue and a major source of significant illness and financial hardship. However, to date, there is limited research on patella, tibia or fibula, or ankle fractures (FPTFA) disease burden. Methods Using the framework of the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019, we conducted temporal trends of disease burden, effects of sex and socio-demographic index (SDI) on age standardized prevalence rate (ASPR) and age-period-cohort model. Machine learning models were built to predict the ASPR of FPTFA in 2035. Results In 1990 and 2019, the top three of all fractures with the highest ASPR was FPTFA. Globally, in 2019, there were 13529.45 million (95%UI: 11811.02, 15642.25), 9198.67 million (95%UI: 8518.9, 10009.91) and 77.54 million (95%UI: 34.09, 149.9) prevalent cases of fracture attributable to fall, transport injury and mechanical external force, respectively. The number of fracture prevalence and years lived with disability (YLDs), and ASPR were higher in males than in females. From 1990 to 2019, most of GBD regions showed a downward trend in average annual percent change (AAPC) of ASPR attributable to transport injuries, High-income Asia Pacific had the highest decline (AAPC= -1.81%, 95%CI: -1.89% to -1.74%). However, an upward trend in East Asia was observed (AAPC = 1.60%, 95%CI: 1.48-1.73%). With increasing SDI, the ASPR for FPTFA attributed to the fall and mechanical external force slowly increased and then turned downward, especially in high-income and high-SDI countries. The longitudinal age curve suggested that the attributed rate was increased for the three sites of lower limb fractures. The best predictive models for FPTFA caused by falls, transport injuries and mechanical external force were autoregressive integrated moving average model (ARIMA) (0,2,1), ARIMA (2,0,1) and ARIMA (0,2,2), with R2 of 0.96, 0.78 and 0.94, respectively. By 2035, ASPR for the fall-related FPTFA will continue to rise rapidly, whereas the ASPR of the transport injury-related FPTFA will gradually decrease. Conclusion FPTFA attributed to the falls is the dominant type and the burden and trends of the fall-related FPTFA are substantial. Fall prevention and access to treatment are crucial for reducing FPTFA burden.

Background: The incidence of early-onset colorectal cancer (EOCRC), which exhibits differential clinical, pathological, and molecular features compared to late-onset CRC (LOCRC), is rising globally. The potential differential effects of blood glucose on EOCRC compared to LOCRC have not been investigated. Methods: This study analyzed 374,568 participants from the UK Biobank cohort and 172,809 participants from the Kailuan cohort. The linear associations between blood glucose and EOCRC/LOCRC were estimated using Cox regression models. Restricted cubic spline (RCS) analysis and non-linear Mendelian randomization (MR) analysis using a 70-SNPs genetic instrument for fasting glucose were used to explore the potential non-linear associations. Results: Participants in the highest quintile of blood glucose had higher overall CRC risk compared to the lowest quintile (HR = 1.10 in the UK Biobank cohort, 95% CI: 1.01–1.21, P-trend = 0.012; HR = 1.23 in the Kailuan cohort, 95% CI: 1.01–1.51, P-trend = 0.036). Elevated glucose (>7.0 mmol/L) was more strongly associated with increased risk of EOCRC (HR = 1.61, 95% CI: 1.07–2.44) than with LOCRC (HR = 1.14, 95% CI: 1.02–1.27) in the UK Biobank cohort (P-heterogeneity = 0.014). Elevated glucose (>7.0 mmol/L) was associated with increased risk of LOCRC (HR = 1.25, 95% CI: 1.04–1.65) in the Kailuan cohort as well. There was no evidence for non-linear associations between blood glucose and risks of EOCRC/LOCRC. Conclusions: This study showed a positive association between blood glucose and CRC risk in a dose-response manner, particularly for EOCRC, suggesting that tighter glucose control should be a priority for younger age groups.

Abstract Background Toxic anterior segment syndrome (TASS) is a rare, noninfectious inflammation that occurs after anterior segment surgery. We report a case herein that developed presumed atypical late-onset TASS after V4c implantable collamer lens (ICL) implantation surgery. Case presentation A 26-year-old man underwent ICL implantation surgeries of both eyes on two separate days. The 1-day and 7-day postoperative routine follow-up visits revealed no abnormalities. However, one month after surgery, dense white spots attached to the posterior surface and scattered ones to the anterior surface of ICL in the left eye were noted on anterior segment examination. His uncorrected distance visual acuity (UDVA) was 20/16 in both eyes and the fundus examination was normal. Despite the absence of typical clinical manifestations, late-onset TASS was suspect and intense topical steroid was administered. After 6 weeks of tapering topical steroid therapy, the white spots disappeared and the patient had no subjective complains throughout the treatment period. Conclusions This case suggested that the traditionally considered acute and serious TASS could also present as delayed and insidious onset after ICL implantation surgery. Due to its variabilities, the awareness of TASS should be raised to ophthalmologists and regular follow-up visits should be emphasized to patients. Once TASS was suspected, intensive steroid therapy should be implemented in time.

Water inrush disasters in karst areas have caused great losses to underground engineering construction, so it is urgent to control water gushing disasters in karst pipelines. In this paper, solution polymerization is used to prepare a grouting and sealing expanded matrix material to control disasters. Noncovalent weak interactions were used to improve the surface properties of the expanded matrix material, the effects of the natural polymer content on the properties of the matrix material were studied, and a modified expanded matrix material with an optimal response rate was prepared. A cross-linked curing agent (CCA) was developed and synthesized, and a new cross-linked expansive grouting and sealing material (WIS grouting material) with various particle sizes was synthesized with noncovalent interactions such as hydrogen bonding, static electricity, van der Waals forces, etc. The results showed that a 4 % solution polymer content (accounting for the particle mass of the expanded matrix) was the optimal dosage, and the optimal ratio of the modified expansion matrix material to the crosslinking curing agent was 1:1. The early compressive strength exceeded 0.2 MPa, and the water absorption rate reached 170 times. There was a power function relationship between the water absorption rate and time, and the rate was controlled by adjusting the particle sizes. The mechanism through which the WIS grouting material underwent expansion and crosslinking was explained at the microscopic level. The gel formed in response to water resisted dispersion in dynamic water and rapidly sealed karst pipe water gushers. This paper proposes a novel approach to utilizing the expansion characteristics of polymer chemical synthetic materials for crosslinking to seal karst pipe water gushers, effectively addressing the issue of poor resistance to dynamic water dispersion in traditional grouting materials used in karst areas. These results provide a scientific basis for the development and application of new materials to control water inrush in karst pipes.

To reveal the specific reason for the bubble release and expansion of municipal solid waste incineration fly ash (MSWI FA) during polymerisation, the chemical composition and source of the emitted gas were studied at length in this paper. To further explore the advantage of an alkali washing pre-treatment for MSWI FA, the influence and mechanism of the alkali washing pre-treatment on the macroscopic mechanical properties, microscopic pore structure and heavy metal leaching of geopolymers have been investigated. The results show that the emerging bubbles are mainly H₂ and NH₃ released by the reaction of elemental aluminium and ammonium salts from MSWI FA with alkali, respectively. The alkaline washing pre-treatment can ensure the volume stability of the geopolymers by releasing bubbles in advance, which is conducive to improving the strength of hardened paste and hindering the leaching of heavy metals. Although the alkaline washing process cannot significantly remove heavy metal ions directly, it can remove almost all the soluble chlorine salts and has a certain significance for the development of the overall compressive strength.

Thread of pin has been proved beneficial to increasing shear action of tool and material intermixing, which however induces abundant formation of intermetallic compounds (IMCs) in Mg/Al dissimilar friction stir welding (FSW) leading to deteriorating the performance of joints. In this work, Mg alloy and Al alloy were well joined by threaded pin and unthreaded pin under the same welding parameters. For threaded pin joints, cracks occured in the banded structure (BS) region and bottom interface, then the main crack propagated along the Mg/Al interface, resulting in low strength and elongation. For unthreaded pin joints, necking and fracture occurred at the softened Al alloy to the retreating side (SAA-RS), and the fracture mode changed from brittle fracture (Mode I) to ductile fracture (Mode III). The tensile strength and strain of unthreaded pin joints are 201 MPa and 7.14 % respectively, significantly higher than those of threaded pin joints (i.e. 140 MPa and 1.15 %), and is the highest combination of strength and strain reported up until now. An unthreaded pin could reduce the local micro-intermixing between Mg and Al to suppress the formation of IMCs, which reduced the hardness in the BS region, preventing the formation and propagation of cracks.

With the fast-paced growth of underground engineering, the performance demands for construction materials are increasing. It is essential to explore the performance optimization of bentonite-based materials from admixture and material property’s view. In this paper, the rheological and filtration properties were selected as the key indices of the bentonite slurry performance. The synergistic effect of bentonite particle size and content, carboxymethyl starch (CMS), and Na2CO3 content on bentonite slurry performance were investigated by conducting the rheology measurement and filtration test. Rheological behaviour, parameters, and filtration loss of the bentonite slurry with different factors were obtained. Range analysis was employed to evaluate the effect degree of each factor on the slurry performance, and an optimal bentonite-based slurry ratio was presented. Results showed that bentonite slurry exhibited shear-thinning characteristic as the bentonite content and CMS content increased, and bentonite particle size decreased. Excessive addition of Na2CO3 weakened the shear-thinning characteristic. The rheological parameters of the bentonite slurry were positively correlated with the bentonite and CMS contents and negatively correlated with the bentonite particle size. An addition of 0.5 %–1.5 % Na2CO3 resulted in the largest rheological parameter. The bentonite content had the greatest impact on the rheological parameters and filtration loss, followed by the particle size. The influences of the Na2CO3 and CMS contents were relatively weak, but the effect of the CMS content on filtration loss was stronger than that of the Na2CO3 content. An optimal ratio of bentonite-based slurry was presented for engineering reference: the bentonite particle size range of 10–23 μm, bentonite content range of 10 %–15 %, Na2CO3 content range of 0.5 %–0.8 %, and CMS content range of 0.1 %–0.8 %.

BackgroundObesity is as an important risk factor for chronic diseases. Metabolically healthy obesity (MHO) is considered a benign state. The association between metabolic health and obesity categories and cancer risk remains unclear. This study aimed to investigate the relationship between metabolic health status combined with obesity phenotypes and the risk of cancer.MethodsData from 91,834 participants in the Kailuan cohort were analyzed, excluding individuals with a body mass index (BMI) < 18.5 kg/m² and those with a history of cancer. Obesity phenotypes were classified based on BMI and waist circumference (WC) combined with metabolic health status, resulting in six phenotypes. Cox proportional hazard regression models were used to assess the association between metabolic health and obesity phenotypes with cancer risk and all-cause mortality.ResultsThe prevalence of metabolically healthy obesity and metabolically unhealthy obesity defined by BMI was 6.86% and 12.18%, while that defined by WC was 20.79% and 25.76%, respectively. Compared to metabolically healthy participants, individuals with an unhealthy metabolic status had a significantly higher risk of cancer (HR, 1.09; 95% CI, 1.03–1.15; p=0.004). The hazard ratios for cancer were 1.19, 1.23, 1.20, and 1.55 for individuals with one, two, three, and four metabolic disorders, respectively. Among those classified as metabolically unhealthy, both overweight and obesity were associated with a protective effect on cancer risk (HR, 0.88; 95% CI, 0.80–0.96; p=0.006 for overweight; HR, 0.87; 95% CI, 0.78–0.97; p=0.010 for obesity). However, abdominal obesity significantly increased cancer risk in both metabolically healthy and unhealthy participants. In subgroup analysis, simple obesity showed a protective trend against cancer in those with respiratory cancers, while abdominal obesity consistently posed a risk for various cancer types.ConclusionMetabolically unhealthy status and abdominal obesity are risk factors for cancer and all-cause mortality, whereas simple obesity offers protective effects against cancer and all-cause mortality in metabolically unhealthy individuals. These findings suggest that maintaining metabolic health and reducing the metabolic risks associated with abdominal obesity should be key targets for cancer prevention.

Objective To compare the clinical outcomes of a modified arthroscopic triple‐row (TR) repair technique with the suture bridge (SB) repair technique in treating L‐shaped delaminated rotator cuff tears. Various surgical techniques for L‐shaped delaminated rotator cuff tears have been reported, many of which aid in increasing the contact area and pressure of the rotator cuff. However, there is still debate over which technique yields superior results. Methods From January 2017 to March 2020, 61 cases of L‐shaped delaminated rotator cuff tears were included in this study. Of these, 34 cases underwent the modified arthroscopic triple‐row repair technique, while 27 cases were addressed with the suture bridge repair technique. Functional assessment was conducted using the American Shoulder and Elbow Surgeons (ASES) score, the University of California Los Angeles (UCLA) shoulder score, the Constant score (CS), and the visual analogue scale (VAS) score. Magnetic Resonance Imaging (MRI) assessments for rotator cuff healing were performed at the 24‐month postoperative mark. Statistical evaluations were conducted using SPSS for Windows (Version 25.0, IBM, Armonk, NY, USA), employing the Wilcoxon signed‐rank test to compare preoperative and postoperative data and ROM differences, and the Mann–Whitney U test for statistical differences in clinical outcome scores between the two groups. Abstract To investigate the associations between corneal curvature (CC) and other anterior segment biometrics in young myopic adults. In this retrospective multi-center study, 7893 young myopic adults were included. CC and other anterior segment biometrics were measured by Scheimpflug imaging (Pentacam). Assessing carbon emissions in China’s swine industry and understanding its spatial–temporal characteristics under carbon peak and carbon neutrality goals are vital for promoting low-carbon farming. This study measures the swine industry’s carbon emissions across 30 provinces in China from 2006 to 2022 using life cycle assessment and IPCC coefficient methods. Spatial and temporal patterns were analyzed with exploratory spatial data analysis, and an XGBoost model was used to predict emissions from 2023 to 2032. The results show a “W”-shaped oscillating trend in total carbon emissions, with phases of rapid decline, fluctuating rise, fluctuating decline, and rapid resurgence. Emissions were highest in the Central region, followed by the West, East, and Northeast. Manure management and Feed crop cultivation were the primary emission sources, accounting for 59.7% and 29.9% of total emissions, respectively. The spatial pattern of high and low emission regions remained stable, with dynamic changes in moderate emission regions. High emissions were concentrated in the major grain-producing and livestock-raising and the South China coastal farming regions. The XGBoost model projects that China’s green development measures in the swine industry will have a significant future impact. The carbon emission intensity shows a trend of initial decline followed by stabilization, whereas total carbon emissions are projected to remain high and gradually increase, primarily due to continuous growth in production capacity. The conclusions of this study provide a reference basis for the green and low-carbon transformation of the swine industry and the optimization of the regional layout of the livestock industry.

Northeast China, recognized as a global flash drought hotspot and a region of nationally important commercial grains and ecological fragility, is highly susceptible to the profound impacts of droughts on both food security and ecological safety. However, the regional-scale characteristics, possible causes and impacts of flash droughts across Northeast China are rarely investigated. Soil moisture data from 2000 to 2022 were utilized to identify the onset, frequency and duration of flash droughts using the quantile method. The spatial trajectories of flash droughts were determined based on the patch-scale centroid transfers. Further, the possible drivings and ecological impacts of flash droughts were analyzed using datasets of climatic variables and gross primary productivity. We found that flash drought coverage with short onset (1- pentad onset) was generally larger than that with long onset (2-, 3- and 4-pentad onset). The affected area of flash droughts generally decreased, while the onset speed increased over the past two decades. Flash droughts frequently occurred the intersection of western Jilin Province, southern Inner Mongolia Autonomous Region, and northern Liaoning Province but with short duration. Compared with 2001–2011, the start, middle and end centroids of flash droughts in 2012–2021 showed more spatial dynamic changes, mainly shifting towards the northeast-southwest and southeast-northwest directions. The onset of flash drought due to potential evapotranspiration, temperature, precipitation deficit, and vapor pressure anomalies account for an average of 33%, 28%, 22%, and 16%, respectively. However, the contributions of dominant meteorological factors and their combinations varied remarkably in different sub-regions. Long onset flash droughts exhibited larger impact on GPP than short onset flash droughts. This study highlights that due to climate change, the affected area of flash droughts in hotspot regions decreased, but the onset speed and spatial dynamics increased, and followed by a more severe ecological impact of short-onset flash drought. Therefore, it is imperative to incorporate the increasing impacts of flash droughts, which is critical to ensuring regional food security and ecological safety in the Northeast China.

Abstract Background The gene cell division cycle associated 5 (CDCA5), also called sororin, has oncogenic characteristics and is upregulated in various carcinomas. Nevertheless, the involvement of CDCA5 in ovarian cancer (OC), a highly aggressive form of cancer, and the underlying mechanism of metastasis remain inadequately investigated. Results The bioinformatics data revealed a negative correlation between the patient’s survival and CDCA5 expression, which was overexpressed in OC. Functional assays also confirmed high expression levels of CDCA5 in OC tissues and cells. This suggests that CDCA5 may potentially enhance the motility, migration, and proliferation of OC cells invitro. It impedes DNA damage and apoptosis in OC cells, inhibiting xenograft development in nude mice. The RNA sequencing results suggest CDCA5 is majorly associated with biological functions related to the extracellular matrix (ECM) and influences the transforming growth factor (TGF) signaling pathway. Moreover, subsequent functional investigations elucidated that CDCA5 facilitated the migration and invasion of OC cells viathe TGF-β1/Smad2/3 signaling pathway activation. Conclusions CDCA5 may be a strong potential therapeutic target for the treatment and management of OC.

Abstract The interactions between white-to-white corneal diameter (WTW) and other ocular biometrics are important for planning of refractive surgery and understanding of ocular structural changes in myopia, but such interactions are rarely investigated in young myopic adults. This is a retrospective study involving 7893 young myopic adults from five centers. WTW and other ocular biometrics were measured by Pentacam. The ocular biometrics included anterior corneal curvature (AK) and posterior corneal curvature (PK), central corneal thickness (CCT) and corneal volume (CV), anterior and corneal eccentricity and asphericity, anterior corneal astigmatism (ACA) and posterior corneal astigmatism, anterior chamber depth (ACD), and anterior chamber volume (ACV). The ocular biometrics were compared among eyes of different WTW quartiles. Multivariate linear regression was used to assess the linear associations between WTW and other ocular biometrics adjusting for age, gender and spherical equivalent. Abstract Background Inflammatory diseases may occur within the crypt beside the preputial frenulum in men. This study was performed to gain an understanding of the etiology, clinical manifestations, and management methods of cryptitis beside the preputial frenulum in men. Results Thirteen patients treated for cryptitis beside the preputial frenulum served as the observation group, and 40 healthy individuals served as the control group. The patients’ clinical manifestation was the presence of a yellowish oily substance embedded in the crypt. Wiping off the substance revealed a conical blind cavity-like structure with an opening diameter of 1 to 5 mm (2.8 ± 1.3 mm) and depth of 1 to 4 mm (2.5 ± 1.1 mm). No blind cavity-like structures in the crypt were found in the control group. The shortest distance between the opening edges of the bilateral crypts in the observation and control groups was 6 to 14 mm (10.3 ± 2.4 mm) and 2 to 10 mm (3.9 ± 1.9 mm), respectively, with a statistically significant difference. Examination for pathogens in the secretions from skin lesions showed that the three most common pathogens were Candida albicans, Staphylococcus aureus, and Escherichia coli. All patients recovered after antibiotic treatment. Conclusions A blind cavity-like structure in the crypt may be related to excessive width of the preputial frenulum. Cryptitis may be a secondary infection caused by smegma trapped in the blind cavity-like structure. Maintaining cleanliness in the frenulum area may help to prevent the occurrence of cryptitis. Antibiotic treatment is effective.

The exploitation of shear dilatation behavior exhibited by natural rock fractures can serve as an effective strategy for stimulating reservoirs in the enhanced geothermal systems (EGS). However, few studies have considered the shear dilatation characteristics of rock fractures treated by real-time high temperature. Firstly, the self-developed Mechanical(M)-Thermal(T) coupling shear test cell and the MTS816.01 rock shear test system were used to carry out the direct shear tests of granite fractures. The M-T coupling effect on granite's physical properties such as cohesion, the mechanical properties such as shear strength, and strain energy characteristics such as dissipated strain energies were analyzed. On these Base, the shear dilatation mechanism of granite fractures treated by real-time high temperature is further revealed. Subsequently, based on the dynamic changes in the area degradation degree of granite fractures, the shear dilatation model of rock fractures treated by real-time high temperature were improved and verified. The results reveal that the M-T coupling effect can considerably impair the shear dilatation behavior of granite fractures. The temperature undermines the shear dilatation behavior of granite fractures by negatively affecting their physical and mechanical properties, while the normal stress deteriorates their shear dilatation behavior by constraining the normal displacement of the rock fractures. The modified shear dilatation model of rock fractures exhibits high accuracy and can accurately represent the shear dilatation behavior of rock fractures under M-T coupling effects. The novel insights provided in this paper contribute to a more comprehensive understanding of the wellbore stability and permeability improvement mechanism of EGS.

The sustainable use of groundwater resources is highly important for regional groundwater system safety and food security, especially in large-scale agricultural planting areas. In the Sanjiang Plain (SJP) in Northeast China, irrigation water consumption has increased sharply due to the large-area ''conversion of dry land (mainly maize and soybean fields) into rice fields'', threatening the regional groundwater system security. However, little is known about the impact of large-scale agricultural planting structure adjustment (i.e., changes in cropping systems) on regional groundwater sustainability. This study estimated the groundwater storage anomaly (ΔGWS) using the Gravity Recovery and Climate Experiment (GRACE) satellite data and the Global Land Data Assimilation System data. Additionally, the groundwater sustainability index (SI) of this region was evaluated based on GRACE Groundwater Drought Index (GGDI). Collectively, the influence of agricultural planting restructuring on the SJP groundwater sustainability was analyzed. The findings were: (1) The groundwater reserves in SJP and Xingkai Lake Plain decreased with maximum declines of 6.79 and 10.37 cm/year, respectively. (2) The groundwater sustainability deteriorated rapidly after 2009, from extremely high to extremely low levels. The lowest sustainability was observed in the Jiansanjiang area, which underwent the most drastic adjustment of agricultural planting structure. (3) Although precipitation increase can alleviate groundwater overexploitation, the groundwater sustainability continued to deteriorate due to the sharp increase in irrigation water consumption arising from the large-scale land conversion. Curbing the expansion of paddy rice fields or reducing their planting areas can restrain the demand for irrigation water. Moreover, proactively designing diversion projects to increase available surface water supply can also effectively achieve sustainable agricultural development in this region.

Abstract Background Although many studies have investigated the association between body composition, cancer risk and mortality, predicting these risks through a single body composition measurement undoubtedly increases the limitations of the study. Few studies have explored the association between the trajectory of changes in body composition and the risk of cancer and death. We aimed to explore the association of predicted lean mass trajectories with cancer risk, cancer‐specific mortality and all‐cause mortality. Methods The participants in this study were all from the Kailuan cohort, a prospective, periodic, resurvey cohort study initiated in 2006. Latent mixture modelling was used to identify predicted lean mass trajectories for 2006–2010. The hazard ratios (HRs) and 95% confidence intervals (95% CIs) of the Cox proportional hazard models were used to describe the association between predicted lean mass trajectories and cancer risk and cancer‐specific and all‐cause mortality during follow‐up (2010–2021). Results A total of 44 374 participants (average age, 53.01 ± 11.41 years, 78.99% men and 21.01% women) were enrolled in this study. Five distinct trajectories were identified: low‐stable (n = 12 060), low‐increasing (n = 8027), moderately stable‐decreasing (n = 4725), moderately stable‐increasing (n = 8053) and high‐stable (n = 11 509). During the 11‐year follow‐up period, 2183 cancer events were recorded. After adjusting for age, predicted fat mass in 2010, sex, BMI, sedentary, physical activity, smoke, alcohol use, salt consumption, high‐fat diet, high‐sensitivity C‐reactive protein, serum creatinine, family history of tumour, hypertension, diabetes mellitus, compared with the low‐stable group, participants in the low‐increasing group (HR = 0.851, 95% CI, 0.748–0.969), moderately stable‐increasing group (HR = 0.803, 95% CI, 0.697–0.925) and high‐stable group (HR = 0.770, 95% CI, 0.659–0.901) had a lower cancer risk, but not in the moderately stable‐decreasing group (HR = 0.864, 95% CI, 0.735–1.015). Compared with the low‐stable group, the risk of cancer‐specific mortality was reduced by 25.4% (8.8–38.9%), 36.5% (20.3–49.4%) and 35.4% (17.9–49.2%), and the risk of all‐cause mortality was reduced by 24.2% (16.9–30.8%), 37.0% (30.0–43.2%) and 47.4% (41.0–53.1%) in the low‐increasing, moderately stable‐increasing group and high‐stable groups, respectively. Conclusions Predicted lean mass trajectories may be closely associated with cancer risk and cancer‐specific and all‐cause mortality. Regular monitoring of body composition is necessary.

With the implementation of the global two-carbon policy, energy saving and CO2 emission reduction have become important developmental goals of the automobile manufacturing industry. At present, the combination of high temperature for the automobile gear steel and short carburizing time is the most direct carbon reduction countermeasure for gear production enterprises. However, the problem of abnormally coarsened austenite grains often occurs in the high-temperature carburization of gear steel. With the increase in carburizing temperature, the degree of mixing crystals becomes serious. As a requirement of gear manufacturing enterprises, microalloying is carried out on the carburized gear steel. Upon the addition of microalloying elements, the second-phase particles are precipitated during heating, and the pinning effect is generated to prevent the movement of austenite grain boundaries, thus preventing the abnormal growth of austenite grains. Although the second-phase particles precipitate at the usual carburizing temperature, the partial solid solution of particles appears. In this work, the effects of the heating temperature and holding time on the austenite grain size of gear steel are studied to clarify the mechanisms of complex austenite grain growth and second-phase particle precipitation for the realization of fine austenite grain size after high-temperature carburization. The influence of the contents of microalloying elements (Nb, Al) on the pinning effect, precipitation position, and solution temperature of the second-phase particles (Nb (C, N)/AlN) are also discussed. Austenite grain growth models, critical sizes of austenite grain abnormal growth, and pinning force models of second-phase particles are summarized. The austenite grain growth model is based on the Beck equation, and the most common models are the modified Sellars and Arrhenius models. For the study of the inhibition effect on austenite grains, the pinning force model is used to study the critical size of austenite by modifying the dimensionless constant (A) of the Zener equation mainly through the pinning effect (Pz) produced by all particles on the grain boundary. After the experimental data are obtained, the trend of austenite growth can be predicted accurately by fitting the curve using the mathematical method. The precipitated second-phase particles are generally distributed along the grain boundary. Nb (C, N) particles have a higher solution temperature than AlN particles, so they are more stable at high temperatures. When the temperature exceeds the grain coarsening temperature, the precipitated particles become dissolved or coarsened. The mixed crystal structure generally starts to appear at about 1000 ℃, and adding the appropriate amount of microalloying elements can increase the coarsening temperature.

Abstract To assess the effect of sodium fluorescein (NaF) strip on corneal parameters commonly used in Laser-assisted in-situ keratomileusis (LASIK). Eighty-six subjects (172 eyes) scheduled for LASIK were recruited between January and March 2022. The study and statistical analysis test were conducted in April 2022. Topographic measurements of corneal parameters, including central corneal thickness (CCT), anterior keratometric (K) readings (K1, flat keratometry; K2, steep keratometry), horizontal corneal diameter (white to white, WTW), and corneal asphericity (Q value), were obtained using a Scheimpflug device (Pentacam) before and 10 min after NaF strip treatmentThe Pentacam recorded a small significant increase in CCT (mean 538.88 ± 28.78 μm to 547.90 ± 29.94 μm; p .05, for all) as well as WTW(mean 11.58 ± 0.32 mm to 11.58 ± 0.32 mm, P > .05) before and after NaF strip intervention. Furthermore, there was no significant difference was observed in Q value (mean − 0.30 ± 0.13 to − 0.30 ± 0.14, P > .05). These results indicate that clinicians should avoid NaF strip application before obtaining precise topographic measurements of cornea parameters using the Pentacam.

Production process outsourcing not only enhances farmers’ operation capability but also contributes to income growth. Utilizing field survey data from five provinces—Inner Mongolia, Gansu, Ningxia, Henan, and Shaanxi—this study employs an endogenous switching regression model to analyze the impact of production process outsourcing on the enhancement of farmers’ operation capability and the income-enhancing effect. The results reveal the following: (1) Production process outsourcing significantly improves farmers’ operation capability and increases income. (2) A higher degree of adoption of production process outsourcing correlates with greater improvements in farmers’ operation capability. (3) The impact of production process outsourcing on farmers’ operation capability varies with individual endowments; farmers with higher education levels, a larger number of laborers, and smaller planting areas experience more pronounced improvements in management capabilities when participating in outsourcing. (4) Production process outsourcing partially mediates the income-enhancing effect through its influence on farmers’ operation capability. To further promote income growth, it is essential to enhance the agricultural outsourcing market supply system, expand farmers’ access to production service information, and prioritize the development of farmers’ operation capability.

A bentonite slurry mixed with seawater is prone to sedimentation, which will reduce the quality of the filter cake and lower the stability of the excavation surface in undersea tunnels. It is necessary to study the performance and influencing factors of the bentonite slurry mixed with seawater. This article simulates the process of undersea slurry shield tunnel construction, where the pressurized slurry penetrates into the sand layer and forms a filter cake when the shield stops pressurizing. We investigated the effects of bentonite, additives (CMC), fine sand, pressure, and formation permeability on the performance of the seawater slurry and filter cake. The sedimentation of mud caused by seawater interfered with the experiment, which manifested as the increase in bentonite, and delayed the formation of the filter cake. Fine sand with a particle size close to the average value of the formation can improve the speed and quality of filter cake formation. By conducting a sensitivity analysis on experimental data, the degree of influence of various factors on the formation rate, thickness, and porosity of the filter cake was determined. Fine sand and bentonite are the factors that have the greatest impact on the formation rate, thickness, and porosity of the filter cake.

Abstract A smooth shaft production process is important to ensure the safe and efficient development of mineral resources. Due to the influence of ore rock and the mismatch between rock size and ore pass design, an arching phenomenon occurs frequently during actual ore pass release, stagnating production. Therefore, an ore pass release simulator with a freely variable ore pass angle and height is designed and used to conduct a four-factor, five-level orthogonal ore pass release experiment. An analysis is performed to determine the influence of four factors—ore particle size, ore moisture, ore pass inclination and slip height—on the probability of ore pass arching; from there, an ore pass arching probability equation is constructed and the characteristics of the ore pass arching location are clarified, effectively predicting the probability of ore pass arching and providing an effective basis for handling arching events within an applicable range. This analysis effectively predicts the probability of ore pass arch within the applicable range, and provides an effective basis for handling ore pass arch events.

Red-bed mudstone (RBM) has the characteristics of being easily softened and disintegrated under dry and wet cycles. To investigate the weakening and disintegration characteristics of the RBM, typical mudstone from Guangxi highway slope was taken as the research object in this study. Disintegration tests, water chemistry tests, and microscopic tests were performed to investigate the disintegration mechanism of this RBM. The results indicate that the RBM exhibits strong disintegration, releasing soluble salts such as potassium chloride and sulfates during the disintegration process. The hydrolysis of clay minerals is the main cause of changes in ion concentration in the solution. Throughout the dry and wet cycles, the proportion of macro-pores and meso-pores significantly increases, while the percentage of small and micro-pores gradually decreases. Under the influence of dry and wet cycles, the contact area between particles gradually decreases, transitioning from face-to-face contact to face-edge contact and finally to edge-to-edge contact. These research findings provide a basis for the stability analysis and prevention of landslide hazards in the red-bed soft rock areas.

Failure to adequately reconstruct the tendon-to-bone interface constitutes the primary etiology underlying rotator cuff retear after surgery. The purpose of this study is to construct a dynamic chondroitin sulfate and chitosan hydrogel scaffold (CHS) with bone morphogenetic protein 2 (BMP2), then seed tendon stem cells (TSCs) on BMP2-CHS for the rotator cuff reconstruction of tendon-to-bone interface. In this dynamic hydrogel system, the scaffold could not only have good biocompatibility and degradability but also significantly promote the proliferation and differentiation of TSCs. The ability of BMP2-CHS combined with TSCs to promote regeneration of tendon-to-bone interface was further verified in the rabbit rotator cuff tear model. The results showed that BMP2-CHS combined with TSCs could induce considerable collagen, fibrocartilage, and bone arrangement and growth at the tendon-to-bone interface and promote the biomechanical properties. Overall, TSCs seeded on CHS with BMP2 can enhance tendon-to-bone healing and provide a new possibility for improving the poor prognosis of rotator cuff surgery.

BackgroundBaseline sleep duration is associated with cancer risk and cancer-specific mortality; however, the association between longitudinal patterns of sleep duration and these risks remains unknown. ObjectiveThis study aimed to elucidate the association between sleep duration trajectory and cancer risk and cancer-specific mortality. MethodsThe participants recruited in this study were from the Kailuan cohort, with all participants aged between 18 and 98 years and without cancer at baseline. The sleep duration of participants was continuously recorded in 2006, 2008, and 2010. Latent mixture modeling was used to identify shared sleep duration trajectories. Furthermore, the Cox proportional risk model was used to examine the association of sleep duration trajectory with cancer risk and cancer-specific mortality. ResultsA total of 53,273 participants were included in the present study, of whom 40,909 (76.79%) were men and 12,364 (23.21%) were women. The average age of the participants was 49.03 (SD 11.76) years. During a median follow-up of 10.99 (IQR 10.27-11.15) years, 2705 participants developed cancers. Three sleep duration trajectories were identified: normal-stable (44,844/53,273, 84.18%), median-stable (5877/53,273, 11.03%), and decreasing low-stable (2552/53,273, 4.79%). Compared with the normal-stable group, the decreasing low-stable group had increased cancer risk (hazard ratio [HR] 1.39, 95% CI 1.16-1.65) and cancer-specific mortality (HR 1.54, 95% CI 1.18-2.06). Dividing the participants by an age cutoff of 45 years revealed an increase in cancer risk (HR 1.88, 95% CI 1.30-2.71) and cancer-specific mortality (HR 2.52, 95% CI 1.22-5.19) only in participants younger than 45 years, rather than middle-aged or older participants. Joint analysis revealed that compared with participants who had a stable sleep duration within the normal range and did not snore, those with a shortened sleep duration and snoring had the highest cancer risk (HR 2.62, 95% CI 1.46-4.70). ConclusionsSleep duration trajectories and quality are closely associated with cancer risk and cancer-specific mortality. Abstract Mounting evidence has linked changes in human gut microbiota to proton pump inhibitor (PPI) use. Accordingly, multiple studies have analyzed the gut microbiomes of PPI users, but PPI–microbe interactions are still understudied. Here, we performed a meta-analysis of four studies with available 16S rRNA gene amplicon sequencing data to uncover the potential changes in human gut microbes among PPI users. Despite some differences, we found common features of the PPI-specific microbiota, including a decrease in the Shannon diversity index and the depletion of bacteria from the Ruminococcaceae and Lachnospiraceae families, which are crucial short-chain fatty acid-producers. Through training based on multiple studies, using a random forest classification model, we further verified the representativeness of the six screened gut microbial genera and 20 functional genes as PPI-related biomarkers, with AUC values of 0.748 and 0.879, respectively. Functional analysis of the PPI-associated 16S rRNA microbiome revealed enriched carbohydrate- and energy-associated genes, mostly encoding fructose-1,6-bisphosphatase and pyruvate dehydrogenase, among others. In this study, we have demonstrated alterations in bacterial abundance and functional metabolic potential related to PPI use, as a basis for future studies on PPI-induced adverse effects.

Abstract China’s rapid increase in mass excreta and its environmental discharge have attracted substantial attention. However, cropland as a main destination of excreta utilization has not been extensively evaluated. Here, a national survey was used to assess the utilization of manure in croplands across China. The data included the inputs of manure nitrogen (N), phosphorus (P), and potassium (K) for cereals, fruits, vegetables, and other crops, along with the manure proportion of total N, P, and K inputs at the county level. The results showed that the manure N, P, and K inputs were 6.85, 2.14, and 4.65 million tons (Mt), respectively, constituting 19.0%, 25.5%, and 31.1% of the total N, P, and K, respectively. The spatial distribution of the manure proportion of total inputs was lower in Eastern China and higher in Western China. The results provide a detailed description of the utilization of manure nutrients in agricultural areas throughout China, which will serve as basic support for policymakers and researchers involved in future agricultural nutrient management in China.

In the stage of aircraft development and airworthiness verification, it is necessary to master the influence of lithium-ion battery (LIB) thermal runaway (TR) propagation. In this paper, the battery TR propagation behavior under different trigger positions and modes is studied experimentally, and the calculation and comparison are carried out from the parameters of real-time temperature, voltage, propagation speed, total energy released, and solid ejecta. When the two adjacent cells at the top corner, side, and center of the module are overheated, TR occurs at about 1000 s for the triggered cells, while the whole-overheating trigger mode takes a longer time. The latter’s transmission speed is extremely fast, spreading 2.67 cells per second on average. The heat generated by the solid ejecta of the whole-overheating trigger mode is 82,437 J, which is more destructive. The voltage of the triggered cell fluctuates abnormally in a precursor manner when the internal active substances in the cell undergo a self-generated thermal reaction. This work can provide a reference for the safety and economical design of system installations and the correct setting of airworthiness verification Method of Compliance (MoC) experiments to verify whether the aircraft can bear and contain the adverse effects caused by LIB TR.

The phenomenon of flash droughts, marked by their fast onset, limited predictability, and formidable capacity for devastation, has elicited escalating concern. Despite this growing interest, a comprehensive investigation of the spatiotemporal dynamics of flash drought events within zones of ecological sensitivity, alongside their consequential ecological ramifications, remains elusive. The Songnen Plain, distinguished as both an important granary for commodity crops and an ecological keystone within China, emerges as an indispensable locus for the inquiry into the dynamics of flash droughts and their repercussions on terrestrial biomes. Through the application of daily soil moisture raster datasets encompassing the years 2002 to 2022, this investigation delves into the spatiotemporal progression of two distinct categories of flash droughts—those instigated by heatwaves and those precipitated by water deficits—within the Songnen Plain. Moreover, the ecosystem’s response, with a particular focus on gross primary productivity (GPP), to these climatic variables was investigated. Flash drought phenomena have been observed to manifest with a relative frequency of approximately one event every three years within the Songnen Plain, predominantly lasting for periods of 28–30 days. The incidence of both heatwave-induced and water deficit-induced flash droughts was found to be comparable, with a pronounced prevalence during the summer and autumn. Nevertheless, droughts caused by water scarcity demonstrated a more extensive distribution and a heightened frequency of occurrence, whereas those rooted in heatwaves were less frequent but exhibited a propensity for localization in specific sectors. The sensitivity of GPP to these meteorological anomalies was pronounced, with an average response rate surpassing 70%. This spatial distribution of the response rate revealed elevated values in the northwestern segment of the Songnen Plain and diminished values towards the southeastern sector. Intriguingly, GPP’s reaction pace to the onset of heatwave-driven flash droughts was observed to be more rapid in comparison to that during periods of water scarcity. Additionally, the spatial distribution of water use efficiency during both the development and recovery periods of flash droughts largely deviated from that of base water use efficiency. The insights from this study hold profound implications for the advancement of regional drought surveillance and adaptive management.

ABSTRACTCholestasis is a condition characterized by the abnormal production or excretion of bile, and it can be induced by a variety of causes, the factors of which are extremely complex. Although great progress has been made in understanding cholestasis pathogenesis, the specific mechanisms remain unclear. Therefore, it is important to understand and distinguish cholestasis from different etiologies, which will also provide indispensable theoretical support for the development of corresponding therapeutic drugs. At present, the treatment of cholestasis mainly involves several bile acids (BAs) and their derivatives, most of which are in the clinical stage of development. Multiple lines of evidence indicate that ecological disorders of the gut microbiota are strongly related to the occurrence of cholestasis, in which BAs also play a pivotal role. Recent studies indicate that probiotics seem to have certain effects on cholestasis, but further confirmation from clinical trials is required. This paper reviews the etiology of and therapeutic strategies for cholestasis; summarizes the similarities and differences in inducement, symptoms, and mechanisms of related diseases; and provides information about the latest pharmacological therapies currently available and those under research for cholestasis. We also reviewed the highly intertwined relationship between gut microbiota-BA-cholestasis, revealing the potential role and possible mechanism of probiotics in the treatment of cholestasis.

Abstract Serum uric acid (SUA) may play an important role in the occurrence of colorectal cancer (CRC). This study aims to explore the association of SUA with the risk of CRC incidence by drawing data from the Kailuan Study. We prospectively examined the association between SUA and risk of CRC incidence among 93,356 Chinese. Eligible participants were divided into three groups based on their tertiles of SUA. Cox proportional hazards regression was used to calculate the hazard ratios (HRs) and 95% confidence intervals (95% CIs) of CRC. During a median follow-up of 13.02 years, 583 new-onset CRC cases were identified. After adjustments were made for confounders, participants in the highest tertiles of SUA exhibited a 1.55-fold increased risk of CRC compared with patients with the lowest SUA levels (HRT3 vs. T1 = 1.55, 95% CI: 1.09–2.30). The associations of SUA with the risk of CRC were slightly reduced but remained substantial in the competing risk analyses when treating CRC unrelated death as the competing risk event. This study found a positive association of SUA with CRC incidence. Specific prevention efforts could be focused on the population with higher levels of SUA.

Circular RNAs (circRNAs) are a class of single-stranded non-coding RNAs that form circular structures through irregular splicing or post-splicing events. CircRNAs are abnormally expressed in many cancers and regulate the occurrence and development of tumors. Circulating circRNAs are cell-free circRNAs present in peripheral blood, they are considered promising biomarkers due to their high stability. In recent years, more and more studies have revealed that circulating circRNAs participate in various cellular communication and regulate the occurrence and development of tumors, which involve many pathological processes such as tumorigenesis, tumor-related immunity, tumor angiogenesis, and tumor metastasis. Understanding the role of cell communication mediated by circulating circRNAs in tumor will further reveal the value and significance behind their use as biomarkers and potential therapeutic targets. In this review, we summarize the recent findings and provide an overview of the cell-cell communication mediated by circulating circRNAs, aiming to explore the role and application value of circulating circRNAs in tumors.

Abstract Background No previous prospective research has explored the association of the TyG (fasting triglyceride-glucose) index and TG/HDL-C ratio as insulin resistance markers with the risk of colorectal cancer (CRC) incidence in the Northern Chinese population. Methods In this prospective cohort study, we included 93,659 cancer-free participants with the measurements of TyG index and TG/HDL-C ratio. Participants were divided by the quartiles of the TyG index or TG/HDL-C ratio. The associations of TyG index, TG/HDL-C ratio, and their components with CRC risk were assessed using Cox proportional hazards regression models. Results During a median follow-up of 13.02 years, 593 incident CRC cases were identified. Compared with the lowest quartile of the TyG index (Q1), the risk of CRC was higher in persons in the third (Q3) and highest quartiles (Q4) of the TyG index, with corresponding multivariable-adjusted HRs (95% CI) of 1.36 (1.06, 1.76) and 1.50 (1.19, 1.91), respectively. The elevated risks of CRC incidence were observed in people in the second, third, and highest quartiles of the TG/HDL-C ratio groups, with corresponding multivariable-adjusted HRs (95% CI) of 1.33 (1.05, 1.70), 1.36 (1.07, 1.73) and 1.37 (1.07, 1.75), respectively. Conclusions Elevated TyG index and TG/HDL-C ratio were associated with a higher risk of developing CRC among adults in Northern China.

Abstract Objective To evaluate the efficacy of the double‐row and double‐pulley technique in treating anterior shoulder glenoid fracture (Ideberg type Ia) using shoulder arthroscopy. Methods Thirty‐six patients with Ideberg type Ia admitted from March 1, 2017, to March 1, 2020, were retrospectively reviewed. Data of the patients' history included age, sex, side of the affected arm, the mean time from injury to surgery, the surgical duration, the average blood loss, and the average total duration of hospital stay. The double‐row and double‐pulley technique was used to repair the scapular glenoid fracture under arthroscopy. Computed tomography (CT) was used to evaluate fracture healing after surgery. The American Shoulder and Elbow Surgeons (ASES) score, the University of California at Los Angeles (UCLA) shoulder joint scoring system, and the Constant–Murley shoulder function score were used to assess the function of the affected shoulder. Results The surgical duration was 90–150 min, with a mean of 127 min. The average blood loss was 90 mL (range, 60–120 mL), and the average total duration of hospital stay was 9.2 days (range, 3 to 14 days). At 9 months after surgery, the CT results showed that all fractures healed, and all patients returned to their previous levels of activity and regained an excellent range of motion. In the pursuit of establishing a more environmentally sustainable and low-carbon hog farming system, the accurate quantification of emissions of greenhouse gas emanating from these systems, especially within the context of China, becomes imperative. Here, drawing insights from a life cycle approach, exhaustive field surveys, and context-specific analyses, we establish an emission measurement index system tailored to hog farming enterprises in China’s Shaanxi Province. Using this methodology, we probed the emission profiles and characteristics of three emblematic hog farming enterprises in the region. Our key findings are as follows: (1) The carbon dioxide emissions per kilogram of pork, factoring in feed cultivation, processing, and transportation, for Pucheng Xinliu Science and Technology, Baoji Zhengneng Farming, and Baoji Zhenghui Farming were quantified as 0.80298 kg, 1.52438 kg, and 0.81366 kg, respectively. (2) Presently, the methane emission coefficient due to enteric fermentation in large-scale hog farms in Shaanxi surpasses the default value set by the Intergovernmental Panel on Climate Change (IPCC). There appears to be a consistent underestimation of enteric methane emissions from live pigs in the province, as gauged against the IPCC metrics. Notably, the emission factor for fattening pigs averaged 2.61823 kgCH4/head/year, while that for breeding pigs stood at 2.96752 kgCH4/head/year. (3) When examining methane and nitrous oxide outputs from manure across various production stages, we observed that emissions from lactating pigs significantly outweigh those from other stages. Interestingly, nitrous oxide emissions from breeding pigs during fattening, finishing, and gestation remained nearly the same, regardless of the manure treatment method. (4) Under the management protocols followed by Pucheng and Baoji, the total carbon emissions from an individual fattening pig amounted to 328.5283 kg and 539.2060 kg, respectively, whereas for breeding pigs, these values were 539.2060 kg and 551.6733 kg, respectively. Further calculations showed that the average carbon footprint CF of large-scale pig farms in China was 3.6281 kgCO2/kg pork. In conclusion, optimizing feed cultivation and transportation logistics, promoting integrated breeding and rearing practices, refining feed formulation, and advancing manure management practices can collaboratively attenuate greenhouse gas emissions. Such synergistic approaches hold promise for steering the hog industry towards a greener, low-carbon, and sustainable trajectory.

Researchers have previously described the response of crop productivity and greenhouse gas (GHG) emissions to fertilizer nitrogen (N) additions, but they have not determined how to maximize yields while minimizing GHG emissions. We conducted an experiment at 2293 sites with four N levels to simulate both grain yield and yield-scaled GHG emissions in response to the N addition. The yield-scaled GHG emissions decreased by 16% as the N rate increased from treatments without the N addition to the minimum yield-scaled GHG emissions, which was comparable to the values associated with the maximum grain yields. The sites with both high soil productivity and high crop productivity had the highest yield and lowest yield-scaled GHG emissions, with 43% higher yield and 38% lower yield-scaled GHG emissions than sites with low soil and low crop productivity. These findings are expected to enhance evaluations of wheat production and GHG emissions in China, and thereby contribute to addressing disparities in the global food and GHG budget.

People are increasingly aware of the role of vegetarian diets in modulating human gut microbial abundance and intestinal physiology. A plant-based diet is thought to benefit host health by contributing to establish a diverse and stable microbiome. In addition, microbe-derived metabolites of specific nutrients known to be abundant in vegetarian diets (such as indigestible carbohydrates, arginine, and others) are important to promote effective intestinal immune responses, maintain intestinal barrier function, and protect against pathogens. This review explores the characteristics of the gut microbiome formed by vegetarian diets and the effects of diet-associated nutrients on intestinal microbial abundance. The interactions between the microbe-derived metabolites of vegetarian diet-associated nutrients and intestinal physiology are also discussed.

Abstract Objective The study aimed to explore the efficacy of direct anterior approach combined with direct posterior approach in Pipkin IV femoral head fractures. Methods The study enrolled 64 patients with Pipkin IV femoral head fractures who were treated at our hospital between March 2019 and April 2020. They were assigned to the control group and the study group using the random number table method with 32 patients in each group and received treatment by the direct anterior approach and treatment by the direct anterior approach combined with the direct posterior approach. The operative time, intraoperative estimated blood loss, postoperative drainage time, drainage volume, time to partial and full weight-bearing, total length of hospital stay and the levels of hemoglobin (Hb) and hematocrit (Hct) in the two groups were compared, and severity of pain and hip function at different time points postoperatively were observed, and the occurrences of complications were compared. Results There was no statistical difference in the operative time and intraoperative estimated blood loss between the two groups (P > 0.05). Compared with the control group, the study group had shorter postoperative drainage time, lower drainage volume, shorter time to partial and full weight-bearing, and shorter total length of hospital stay, and the difference was statistically different (P 0.05). The levels of Hb and Hct in both groups at postoperative day (POD) 1 were lower than those before surgery, and the levels of Hb and Hct in the study group were significantly higher than those in the control group (P

A sufficiently refined mesh at the stress position is the basis for finite element analysis of gears to obtain accurate tooth surface contact stress and tooth root bending stress. While ensuring the accuracy of finite element analysis results，it is one of the hot research issues to reduce the solution time and the requirements on computer performance. The mesh of tooth surface and tooth root is refined based on Ansys software mesh division method，and the finite element modeling and simulation analysis are carried out by using multipoint constraint （MPC） method to coupling the degree of freedom of mesh nodes with different sizes. The results show that this method can greatly reduce the numbers of mesh and nodes，shorten the calculation time，and ensure the accuracy of calculating the tooth root bending stress and the tooth surface contact stress. Secondly，the results of calculating the tooth root bending stress and the tooth surface contact stress of the herring gear according to the ISO calculation standard are both large than finite element analysis，which provides important reference for engineering practical application of herringbone gear.

The study aimed to understand the incidence, site, skin lesion manifestations, and treatment of gonococcal infection of the glans skin. We enrolled men with gonococcal infection of the glans skin and men with gonococcal urethritis from January 2014 to February 2020. Demographic data, site of onset, and skin lesion manifestations were recorded for all patients. Ceftriaxone (1 g) was injected intramuscularly once daily for 5 days in patients with lesions comprising abscesses or nodules. A single dose of ceftriaxone (1 g) was injected intramuscularly in patients with pustules. Incision and drainage were performed in patients with nonruptured abscesses. Thirteen patients had gonococcal infection of the glans skin (0.65%; 95% confidence interval = [0.30, 1.01]) among 1,989 patients with gonococcal urethritis. Mean age was 35.48 ± 2.37 (range = 26–45) years. Nonmarital sexual behavior patterns were genital–genital in eight patients (61.54%) and genital–oral in five patients (38.46%). All skin lesions occurred on the ventral side of the glans. Eleven patients (84.62%) had a single lesion and two (15.38%) had multiple lesions. The lesions manifested as abscesses in five patients (38.46%), nodules in five patients (38.46%), and pustules in three patients (23.08%). All lesions exhibited tenderness. All 13 patients were cured after treatment. The study shows that gonococcal infection of the glans skin is a rare local complication of gonorrhea. Lesions often occur on the ventral side of the glans, presenting as abscesses, nodules, and pustules. Ceftriaxone treatment was effective for gonococcal infection of the glans skin.

Due to the hidden nature of the grouting project, it has not been possible to get a clear picture of the slurry transport law, and most of the existing permeation grouting theories have ignored the tortuous characteristics of the pore channels of porous media. Based on the fractal theory of porous media and the equation of seepage motion of viscous time-varying Bingham fluid, a theoretical model of permeation grouting with viscous time-varying Bingham fluid is established, and viscosity tests are carried out for C-S slurry to obtain the time-varying equation of viscosity of C-S slurry. A set of permeation grouting simulation experiment device was designed, and the permeation grouting simulation experiment of C-S slurry was carried out to obtain the variation curve of grouting pressure with time under different injection media and different grouting rates. The results show that the grouting pressure considering the diffusion path of the slurry is 0.9–1.1 times of the experimental results, the grouting pressure obtained from the experimental is 0.7–1.2 times of the theoretical value considering the diffusion path, and the calculated value considering the diffusion path agrees well with the experimental value. The theoretical model can provide scientific guidance for field grouting construction.

Abstract Background The study aimed to explore the additional value of repair of Lafosse I subscapularis injury compared with debridement in anterosuperior rotator cuff injury. Methods The prospective study was conducted on a total of 41 patients with supraspinatus tendon tear combined with Lafosse I subscapularis injury. Eighteen patients were divided into the repair group and 23 patients were divided into the non-repair group. The two groups were compared for intraoperative parameters, pain score, range of motion of the shoulder joint, shoulder joint function and quality of life (QoL) at pre-operation, 3 and 6 months postoperatively and the final follow-up visit. Results The width of supraspinatus tendon tear did not exceed 3 cm and did not retract beyond the glenoid in among patients. There was no statistical difference of preoperative data between two groups, including age, course of disease, positive Jobe test, positive Bear-hug test, positive Lift-off test, Patte stage, longitudinal tear and pain severity (P > 0.05). Compared to preoperative levels, the severity of pain, ASES scores and EQ-5D-3L scores were significantly lower at 3 and 6 months postoperatively and the final position (P 0.05). Similarly, compared to preoperative levels, the range of motion of shoulder joint was significantly improved after operation, including internal rotation, external rotation, forward flexion and elevation (P 0.05). Conclusion Operative treatment can effectively lessen severity of pain in the patients, improve shoulder joint function, increase the range of motion of the shoulder joint and enhance the QoL in treating anterosuperior rotator cuff injury. However, repair of subscapularis brings no benefit compared to debridement in treating supraspinatus tendon tear combined with Lafosse I subscapularis injury.

BackgroundThe close association of abdominal obesity rather than general obesity with colorectal cancer (CRC) risk might be mediated by IR and inflammation, which has never been systematically explored in large-scale prospective studies.MethodsWe prospectively examined the mediation effects of the fasting triglyceride-glucose (TyG) index and C-reactive protein (CRP) on the associations of obesity (general and abdominal) with CRC risk among 93,659 participants. We used the Cox proportional hazards regression models and subgroup analyses to evaluate the hazard ratios (HRs) and 95% confidence intervals (95% CIs) of CRC. The CAUSALMED procedure was used to perform the mediation analyses.ResultsDuring 13.02 years of follow-up, a total of 586 CRC cases were verified. Male participants with general obesity and abdominal obesity had a 1.29-fold and a 1.28-fold increased risk of CRC. However, a significant association was only observed among female individuals with abdominal obesity. Both TyG index and CRP were associated with an elevated risk of CRC, and A significant interaction between the TyG index and CRP was found for the risk of CRC (P for interaction

Poor adhesion of nickel surface limits its further application in the aerospace field. In this study, plasma modification was conducted on the surface of the nickel plate pretreated by sandblasting, and then ultrasonic vibration was applied during the adhesively bonding process of the CFRP(Carbon fibre-reinforced polymer)/Ni joints. The bonding strength of the joints was increased by 65%. The adherend surface and the bonding interface were analyzed from microstructure, element distribution and chemical bonding to study the strengthening mechanism. By the sandblasting, irregular pits were formed on the nickel surface, effectively increasing the surface roughness. The plasma modification could introduce active functional groups including hydroxyl, amino and carbonyl on the nickel surface, which improved the surface wettability macroscopically. However, at a microscopic level, the adhesive with high viscosity and poor fluidity did not form a compact interface with the nickel. The ultrasonic application could promote the filling of the adhesive in irregular micro-scale pits on the surface, thereby strengthening the mechanical anchoring effect. Furthermore, the ultrasonic application produced dynamic impingement at the interface, enhancing the contact between the adhesive and the nickel plate. The adhesive molecules could fully collide and react with the active functional groups introduced on the nickel surface to form more chemical bonds, thus effectively improving the bonding strength of the CFRP/Ni joints.

BackgroundHypertension and high-salt intake may act synergistically to increase the risk of primary liver cancer (PLC). We prospectively examined the joint effect of hypertension and salt intake on the risk of PLC incidence.MethodsA total of 92,978 participants were included in the final analyses. The study population was divided into 4 groups according to the presence or absence of hypertension and salt intake. Cox proportional hazards regression models were used to evaluate the association of hypertension and/or high-salt intake with the risk of incident cancers. The CAUSALMED procedure was used to perform the mediation analyses.ResultsDuring a median follow-up of 12.69 years, a total of 418 incident cancer cases were identified. Hypertension was a risk factor for PLC in women but not in men. High salt intake was associated with an elevated risk of PLC in men. A significant interaction between salt intake and hypertension was found for the risk of PLC (P for interaction=0.045). Compared with Group 1 (hypertension-, high salt intake-), participants in Group 2 (hypertension-, high salt intake+) and Group 4 (hypertension+, high salt intake+) were associated with an elevated risk of PLC with the corresponding multivariate HRs (95%CIs) of 1.73(0.96,3.10) and 1.96(1.09,3.53) respectively. No significant mediation effect was found for the association between hypertension, salt intake and PLC risk.ConclusionsThe combination of high salt intake and hypertension could significantly increase the risk of PLC. It may be reasonable to recommend a low-salt intake to prevent and control the prevalence of PLC and hypertension.Trial registrationKailuan study, ChiCTR–TNRC–11001489. Abstract Background Single 6 ml Hylan G-F 20 injection, is indicated for knee osteoarthritis patients who have failed to respond to non-pharmacologic therapy and/or simple analgesics. To obtain more thorough understanding of the clinical efficacy and safety, a randomized clinical trial was conducted comparing intra-articular (IA) administration of single 6 ml Hylan G-F 20 injection versus placebo in knee OA patients of Chinese ethnicity. Methods This was a randomized, multi-center, double-blind, placebo-controlled clinical trial conducted in 21 centers across China. Four hundred forty adults with knee OA received a single 6 ml Hylan G-F 20 or placebo injection and were evaluated for clinical efficacy and safety outcomes over 26 weeks. Western Ontario and McMaster Universities OA (WOMAC) A1 index, treatment-emergent adverse events (TEAEs) and standard safety parameters were measured at pre-injection, and at weeks 1, 4, 8, 12, 16, 20 and 26 post-injection. Results Four hundred forty patients (male: 98 [22.3%]; female: 342 [77.7%]) were randomized. The mean age [standard deviation (SD)] was 61.5 (7.9) years. All patients were of East Asian ethnicity. Mean WOMAC A1 score at baseline was 5.3 (1.2) and 5.2 (1.3) in single 6 ml Hylan G-F 20 injection and placebo groups, respectively. Significant reductions of WOMAC A1 score were observed in both treatment groups when compared to baseline at 26 weeks post-injection, with the mean reduction of [standard error (SE)/percentage] -2.146 (0.108)/− 40.5% and − 2.271 (0.110) /− 43.7% in the single 6 ml Hylan G-F 20 injection and the placebo groups, respectively. Additionally, clinically important reductions in pain at 26 weeks was reported in 67.0 and 68.2% in single 6 ml Hylan G-F 20 injection and placebo groups (p = 0.36). Regarding safety, TEAEs were similar between the two treatment groups (hylan G-F 20 single: 61.5%; placebo: 64.5%). Conclusions While the magnitude of the effect of a single 6 ml Hylan G-F 20 injection in this study is consistent with previously published literature with respect to the efficacy and safety of the drug, the current study shows a strong IA placebo effect and did not established superiority of single 6 ml Hylan G-F 20 injection over IA placebo in Chinese knee OA patients.
