Objective To determine the prognostic value of thrombomodulin (TM) combined with thrombinantithrombin complex (TAT) in patients with sepsis. Methods A retrospective analysis of the clinical data from 80 patients with sepsis who met the inclusion and exclusion criteria admitted to the 908th Hospital of Chinese PLA Logistical Support Force from May 2018 to July 2019. The conventional coagulation tests, thromboelastographic (TEG) parameters, TM, TAT, α2-plasmin inhibitor-plasmin complex (PIC, namely plasmin/alpha 2-antiplasmin complex, PAP) and tissue plasminogen activator-inhibitor-1 complex (t-PAIC) were collected within 2 hours at admission. According to the prognosis of 90 days, the patients were divided into survival group and death group and statistical analysis were performed. Results Compared with TM [11.7(8.8, 15.9) TU/ml] and TAT [11.3(7.0, 20.5) ng/ml] in the survival group, TM [20.2(14.1, 23.8) TU/ml] and TAT [17.7(11.8, 54.6) ng/ml] were significantly increased in the death group (P16.95 TU/ml combined with TAT>10.55 ng/ml. Conclusion TM combined with TAT can effectively judge the prognosis of sepsis patients and early identify sepsis related coagulation disorders. DOI: 10.11855/j.issn.0577-7402.2020.07.12

Naoxintong Capsule (NXT), a Chinese medicine, has been widely used for the treatment of cardiovascular diseases in clinic. The present study was designed to clarify the effects of NXT on haemorheology, blood coagulation, hepatic function, antioxidant capacity and inflammatory responses in a rat model of blood stasis syndrome. Rats were placed in ice-cold water for 5 min during the time interval of two adrenaline injections to induce blood circulation disorders after intragastric administration of 10 consecutive days. Blood was collected from abdominal aorta to determine whole blood viscosity (WBV), plasma viscosity (PV), activated partial thromboplastin time (APTT), prothrombin time (PT), fibrinogen (FIB), total antioxidant capacity (T-AOC), superoxide dismutase (SOD), malonaldehyde (MDA), alanine transaminase (ALT), aspartate transaminase (AST), interleukin-1β (IL-1β), interleukin-6 (IL-6), platelet activating factor (PAF). The results showed that NXT could significantly decrease the WBV, PV, FIB and prolong the APTT and PT. Meanwhile, NXT significantly decreased the serum contents of MDA, IL-1β, IL-6, ALT, AST and PAF, while elevating T-AOC and SOD levels. These results indicate that NXT could improve coagulation function, enhance antioxidant capacity and suppress inflammation, which might partly account for its mechanisms of action in preventing and treating cardiovascular diseases.

Context Naoxintong Capsule (NXT), a Chinese medicine, has been widely used for the treatment of coronary heart disease (CHD) in clinics. Objective This study evaluated the cardioprotective effects of NXT alone and in combination with ticagrelor (TIC) and atorvastatin (ATO). Materials and methods Qi deficiency and blood stasis rats were established by 8 weeks high fat diet feeding and 16 days exhaustive swimming and randomly divided into seven groups, that is, NXT (250, 500 and 1000 mg/kg/d), TIC (20 mg/kg/d), ATO (8 mg/kg/d), NXT (500 mg/kg/d)+TIC (20 mg/kg/d) and NXT (500 mg/kg/d)+ATO (8 mg/kg/d) group, with oral administration for 12 weeks. The contents of TC, TG, LDL-C, HDL-C, IL-1β, IL-6, IL-8, TNF-α, AST, ALT, SOD, MDA, CK-MB, LDH, TXA2, PGI2, IgA, IgG, IgM and C3 in serum were measured. Results NXT + TIC group was significantly superior to the TIC group in decreasing the levels of TC (4.34 vs. 5.54), TG (3.37 vs. 4.66), LDL-C (1.21 vs. 1.35), LDH (4919.71vs. 5367.19) and elevating SOD level (248.54 vs. 192.04). NXT + ATO group was significantly superior to the ATO group in decreasing the levels of AST (195.931 vs. 241.63), ALT (71.26 vs. 83.16), LDH (4690.05 vs. 5285.82), TXA2 (133.73 vs. 158.67), IgG (8.08 vs. 9.80), C3 (2.03 vs. 2.35) and elevating the levels of HDL-C (1.19 vs. 0.91), SOD (241.91vs. 209.49). Conclusions The present findings demonstrate that the combined use of NXT with TIC and ATO had better integrated regulating effects than TIC and ATO, respectively. The mechanism of action requires further research.

Objective To explore the clinical characteristics of coagulation dysfunction at different stages in trauma patients. Methods Retrospective analysis of 279 trauma patients admitted to the Intensive Critical Unit of 908th Hospital of Chinese PLA from September 2015 to July 2019. The patients were divided into non-traumatic coagulopathy group (NTC group, n=187), traumatic coagulopathy group (TC group, n=44) and traumatic disseminated intravascular coagulation group (TDIC group, n=48) according to the coagulation status at admission. Background Sepsis is prevalent among intensive care units and is a frequent cause of death. Several studies have identified individual risk factors or potential predictors of sepsis-associated mortality, without defining an integrated predictive model. The present work aimed to define a nomogram for reliably predicting mortality. Methods We carried out a retrospective, single-center study based on 231 patients with sepsis who were admitted to our intensive care unit between May 2018 and October 2020. Patients were randomly split into training and validation cohorts. In the training cohort, multivariate logistic regression and a stepwise algorithm were performed to identify risk factors, which were then integrated into a predictive nomogram. Nomogram performance was assessed against the training and validation cohorts based on the area under receiver operating characteristic curves (AUC), calibration plots and decision curve analysis. Results Among the 161 patients in the training cohort and 70 patients in the validation cohort, 90-day mortality was 31.6%. Older age and higher values for the international normalized ratio, lactate level, and thrombomodulin level were associated with greater risk of 90-day mortality. The nomogram showed an AUC of 0.810 (95% CI 0.739 to 0.881) in the training cohort and 0.813 (95% CI 0.708 to 0.917) in the validation cohort. The nomogram also performed well based on the calibration curve and decision curve analysis. Conclusion This nomogram may help identify sepsis patients at elevated risk of 90-day mortality, which may help clinicians allocate resources appropriately to improve patient outcomes.

Objective To investigate the clinical value of thrombelastography (TEG) in the diagnosis and prognosis judgement of heatstroke with disseminated intravascular coagulation (DIC). Methods Thirty-two patients with heatstroke were selected from the 94th Hospital of Chinese PLA during Jun. 2016 and Sep. 2017. The general information and laboratory test of the patients within two hours after admission were collected or performed, including prothrombin time, activated partial thromboplastin time (APTT), fibrinogen (FIB), thrombin time (TT), plasma fibrin degradation products (FDP), D-Dimer (D-D), antithrombin Ⅲ (ATⅢ) and the TEG parameters. The TEG-DIC and ISTH-DIC criteria were compared. The patients were divided into survival group (n=24) and death group (n=8) according to prognosis. The results of coagulation test and thromboelastograph were compared. The relationship between TEG score and mortality were investigated. Firstly, this article focous on the requirements for RFID antenna of ecommerce--a fractal design of folded dipole antenna by moment method for simulation analysis of the designed antenna, the return loss performance of the antenna are calculated. In order to have more redundancy for the performance of the antennas in actual use when in all kinds of bad environment to function properly, we try to use methods of additional passive patch, microcosmic fractal structure and mirror compensation structure to improve the performance of the antenna. Then we compare the effects of multiple antenna performance improvement methods, and find the best optimization design project. According to the design of antenna, it’s resonant frequency is 2.4GHz, the minimum return loss value is -46.27dB, the bandwidth is 544MHz. Secondly, in view of the engineered practice, the article also discusses the influence of substrate parameters on the performance of the antenna in detail.

Dissoluble manganese supramolecule containing rhodamine 6G luminescent particles (M2) are synthesized, based on dissoluble manganese supramolecule (M1) doping rhodamine 6G (R.6G), by crystalline method. The particle diameters of M1 and M2 determined by ETM are both of micron degree. M1 and M2 can emit solid substrate room temperature phosphorescence (SS-RTP) on filter paper. The transition probability from the singlet state (S1) to triplet state (T1) of the luminescent molecules was greatly enhanced, based on the increment of luminescent molecules for each spot and the heavy atom effect of certain amount of Pb2+. As a result, the phosphorescence intensity (Ip) of M2 was increased sharply, and the enhancing value of phosphorescence intensity (ΔIp) is directly proportional to the concentration of Pb2+. Thus, a new method of SS-RTP enhancing for the determination of trace lead is established based on manganese supramolecule containing rhodamine 6G luminescent particles. The linear range of this method is 0.0040–0.400 pg spot−1 of Pb2+ (corresponding concentration, 0.01–1.0 ng mL−1; sample volume, 0.4 μL spot−1), with a detection limit (LD) of 0.0011 pg spot−1 (corresponding concentration, 2.8 × 10−12 g mL−1 of Pb2+, n = 11). For the working solutions containing 0.0040 and 0.40 ng mL−1 of Pb2+, they were determined repeatedly for seven times, respectively. The R.S.D.s were 3.2 and 3.8%, respectively. This method has good repeatability, sensitivity and high precision. It has been applied to the determination of trace lead in human hair and tea samples with satisfactory results.
