1. Measuring the linear and nonlinear elastic properties of brain tissue with shear waves and inverse analysis

Abstract

We use supersonic shear wave imaging (SSI) technique to measure not only the linear but also the nonlinear elastic properties of brain matter. Here, we tested six porcine brains ex vivo and measured the velocities of the plane shear waves induced by acoustic radiation force at different states of pre-deformation when the ultrasonic probe is pushed into the soft tissue. We relied on an inverse method based on the theory governing the propagation of small-amplitude acoustic waves in deformed solids to interpret the experimental data. We found that, depending on the subjects, the resulting initial shear modulus mu0 varies from 1.8 to 3.2 kPa, the stiffening parameter b of the hyperelastic Demiray-Fung model from 0.13 to 0.73, and the third- (A) and fourth-order (D) constants of weakly nonlinear elasticity from -1.3 to -20.6 kPa and from 3.1 to 8.7 kPa, respectively. Paired t-test performed on the experimental results of the left and right lobes of the brain shows no significant difference. These values are in line with those reported in the literature on brain tissue, indicating that the SSI method, combined to the inverse analysis, is an efficient and powerful tool for the mechanical characterization of brain tissue, which is of great importance for computer simulation of traumatic brain injury and virtual neurosurgery.

Abstract

Background: Ureteral injury is common during gynaecological laparoscopic surgery. Real-time auto-segmentation can assist gynaecologists in identifying the ureter and reduce intraoperative injury risk. Methods: A deep learning segmentation model was crafted for ureter recognition in surgical videos, utilising 3368 frames from 11 laparoscopic surgeries. Class activation maps enhanced the model's interpretability, showing its areas. The model's clinical relevance was validated through an End-User Turing test and verified by three gynaecological surgeons. Results: The model registered a Dice score of 0.86, a Hausdorff 95 distance of 22.60, and processed images in 0.008 s on average. In complex surgeries, it pinpointed the ureter's position in real-time. Fifty five surgeons across eight institutions found the model's accuracy, specificity, and sensitivity comparable to human performance. Yet, artificial intelligence experience influenced some subjective ratings. Conclusions: The model offers precise real-time ureter segmentation in laparoscopic surgery and can be a significant tool for gynaecologists to mitigate ureteral injuries.

20. Diagnostic value of two dimensional shear wave elastography combined with texture analysis in early liver fibrosis.

Abstract

BACKGROUND: Staging diagnosis of liver fibrosis is a prerequisite for timely diagnosis and therapy in patients with chronic hepatitis B. In recent years, ultrasound elastography has become an important method for clinical noninvasive assessment of liver fibrosis stage, but its diagnostic value for early liver fibrosis still needs to be further improved. In this study, the texture analysis was carried out on the basis of two dimensional shear wave elastography (2D-SWE), and the feasibility of 2D-SWE plus texture analysis in the diagnosis of early liver fibrosis was discussed. AIM: To assess the diagnostic value of 2D-SWE combined with textural analysis in liver fibrosis staging. METHODS: This study recruited 46 patients with chronic hepatitis B. Patients underwent 2D-SWE and texture analysis; Young's modulus values and textural patterns were obtained, respectively. Textural pattern was analyzed with regard to contrast, correlation, angular second moment (ASM), and homogeneity. Pathological results of biopsy specimens were the gold standard; comparison and assessment of the diagnosis efficiency were conducted for 2D-SWE, texture analysis and their combination. RESULTS: 2D-SWE displayed diagnosis efficiency in early fibrosis, significant fibrosis, severe fibrosis, and early cirrhosis (AUC > 0.7, P < 0.05) with respective AUC values of 0.823 (0.678-0.921), 0.808 (0.662-0.911), 0.920 (0.798-0.980), and 0.855 (0.716-0.943). Contrast and homogeneity displayed independent diagnosis efficiency in liver fibrosis stage (AUC > 0.7, P < 0.05), whereas correlation and ASM showed limited values. AUC of contrast and homogeneity were respectively 0.906 (0.779-0.973), 0.835 (0.693-0.930), 0.807 (0.660-0.910) and 0.925 (0.805-0.983), 0.789 (0.639-0.897), 0.736 (0.582-0.858), 0.705 (0.549-0.883) and 0.798 (0.650-0.904) in four liver fibrosis stages, which exhibited equivalence to 2D-SWE in diagnostic efficiency (P > 0.05). Abstract

Purpose Papillary thyroid carcinoma (PTC) is frequently associated with central lymph node metastasis (CLNM). In the present study, we aimed to identify possible risk factors for CLNM in PTC, including suspicious ultrasound (US) features coexisting with thyroid diseases, immunohistochemical markers, and BRAFV600E. These were used to establish a model to predict the risk of CLNM. Methods From January 2016 to March 2018, we identified a cohort of patients with classic PTC who underwent cervical US and were diagnosed by postoperative pathology. Fine-needle aspiration biopsies were analyzed for the presence ofBRAFV600E, and immunohistochemistry was used to detect tumor markers. US imaging was performed in accordance with a standardized protocol. A model to determine the risk of CLNM was established using the outcomes of univariate and multivariate analyses. Results Age of ≥45 years, male sex, mean tumor diameter of ≥1.0 cm, taller-than-wide tumor shape, multiple PTCs, capsule contact, and HBME-1 expression were significant independent risk predictors of CLNM. Hashimoto’s thyroiditis, nodular goiter, andBRAFV600E were not significantly associated with CLNM. The cutoff value (area under the curve = 0.760) for predicting risk was determined from receiver operating characteristic curves (sensitivity, 75.6 %; specificity, 60.7 %). Conclusions Male sex, age of ≥45 years, mean tumor diameter of ≥1.0 cm, taller-than-wide shape, multiple tumors, capsule contact, and HBME-1 expression were independent predictors of the risk of CLNM in patients with PTC. The risk model may be useful for evaluating patients’ prognoses and selecting optimal surgical strategies.

27. Effect of a poloxamer 407-based thermosensitive gel on minimization of thermal injury to diaphragm during microwave ablation of the liver.

Abstract

AIM: To assess the insulating effect of a poloxamer 407 (P407)-based gel during microwave ablation of liver adjacent to the diaphragm. METHODS: We prepared serial dilutions of P407, and 22.5% (w/w) concentration was identified as suitable for ablation procedures. Subsequently, microwave ablations were performed on the livers of 24 rabbits (gel, saline, control groups, n = 8 in each). The P407 solution and 0.9% normal saline were injected into the potential space between the diaphragm and liver in experimental groups. No barriers were applied to the controls. After microwave ablations, the frequency, size and degree of thermal injury were compared histologically among the three groups. Subsequently, another 8 rabbits were injected with the P407 solution and microwave ablation was performed. The levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), blood urea nitrogen (BUN) and creatinine (Cr) in serum were tested at 1 d before microwave ablation and 3 and 7 d after operation. RESULTS: In vivo ablation thermal injury to the adjacent diaphragm was evaluated in the control, saline and 22.5% P407 gel groups (P = 0.001-0.040). However, there was no significant difference in the volume of ablation zone among the three groups (P > 0.05). Moreover, there were no statistical differences among the preoperative and postoperative gel groups according to the levels of ALT, AST, BUN and Cr in serum (all P > 0.05). CONCLUSION: Twenty-two point five percent P407 gel could be a more effective choice during microwave ablation of hepatic tumors adjacent to the diaphragm. Further studies for clinical translation are warranted.

Amac: Buyuk basit hepatik kistlerin tedavisinde geleneksel alkol skleroterapisinin etkinlii ve guvenilirlii tart›flmal›d›r. Bu cal›fl- mada, basit hepatik kistlerin tedavisinde, tekrarlayan aspirasyon ve alkol enjeksiyonu ad› verilen yeni bir skleroterapi proseduru- nun gelifltirilmesi ve deerlendirilmesi amaclanm›flt›r. Gerec ve Yontem: Araflt›rma prospektif, cift kor, randomize olarak duzen- lenmifltir. Buyuk basit hepatik kisti olan 67 hasta tek seferlik aspirasyon-alkol enjeksiyonu (alkol kaviteye enjekte edildikten 20 da- kika sonra tek seferde aspire edilmifltir) ve tekrarlayan aspirasyon ve alkol enjeksiyonu (kist icindeki hesaplanan alkol konsantras- yonunun %80'e ulaflana kadar kaviteye 30 -70 ml alkol enjeksiyonu ve hemen s›v›n›n k›smen aspirasyonunun 3-6 kez tekrarlanma- s›) olarak 2 gruba randomize edilmifl. Kist boyundaki kuculme yontemler aras› etkinliin karfl›laflt›r›lmas› icin kullan›lm›flt›r. Yon- temlerin guvenilirlii yan etkilerin deerlendirilmesi ve kan alkol duzeyinin 0 - 0,5 - 1 - 2 - 3. saatlerde olculmesi ile deerlendiril- mifltir. Background/aims: The efficacy and safety of traditional alcohol sclerotherapy procedures are controversial in the management of large simple hepatic cysts. In this study, we aimed to develop and evaluate a novel alcohol sclerotherapy procedure, termed repea- ted aspiration and alcohol instillation sclerotherapy, for the treatment of simple hepatic cysts. Materials and Methods: A pros- pective, double-blind, randomized study was performed. Sixty-seven patients with large simple hepatic cysts were randomized into two groups to receive either single-session alcohol retention sclerotherapy (alcohol was instilled into the cyst cavity, kept for 20 mi- nutes and aspirated) or repeated aspiration and alcohol instillation sclerotherapy (instillation of 30-70 ml of alcohol and immedia- te aspiration with repetition 3 to 6 times until the estimated alcohol concentration exceeded 80%). The cyst volume reduction was calculated to compare the efficacy of the two procedures. We evaluated the safety of the procedure by monitoring side effects and as- saying blood alcohol concentrations at 0, 0.5, 1, 2 and 3 hours after sclerotherapy. Results: The cyst volume reduction in patients undergoing repeated aspiration and alcohol instillation sclerotherapy was significantly higher than that in those receiving alcohol- retention sclerotherapy. The concentration of alcohol in the last aspirated cyst fluid was correlated with the mean volume reducti- on in patients undergoing repeated aspiration and alcohol instillation sclerotherapy but not in the alcohol-retention group. Only mi- nor side effects occurred in both groups. Although elevated blood alcohol concentration was noted in all patients, it declined to nor- mal levels within 2-3 hours after treatment. There were no significant differences in blood alcohol concentration between the two groups. Conclusions: Repeated aspiration and alcohol instillation sclerotherapy is superior to single-session alcohol-retention scle- rotherapy in the management of large simple hepatic cysts.

35. Value of Shear Wave Elastography Versus Contrast‐Enhanced Sonography for Differentiating Benign and Malignant Superficial Lymphadenopathy Unexplained by Conventional Sonography.

Abstract

Objectives: This study aimed to compare the efficacy of shear wave elastography (SWE) and contrast‐enhanced sonography in the differential diagnosis of superficial lymphadenopathy with abnormal imaging findings, which could not be otherwise confirmed by conventional sonography. Methods: Forty‐two enlarged superficial lymph nodes in 42 patients who met the screening criteria for this study were evaluated by both contrast‐enhanced sonography and SWE. All lymph nodes underwent both methods using biopsy pathologic findings as a reference standard. Results: The maximum elastic modulus, mean elastic modulus, and standard deviation of the elastic modulus were the main distinguishing features on SWE; they were significantly higher in malignant lesions than benign ones. The threshold value for the maximum elastic modulus was set at 37.9 kPa, and the sensitivity, specificity, and accuracy of differential diagnosis of superficial lymph nodes were 81.8%, 80.0%, and 81.0%, respectively. The diagnosis of benignity and malignancy by this index was statistically significant (P < .001). The lymph nodes were divided into benign and malignant groups according to different types based on the degree and range of intensity on contrast‐enhanced sonography: intense or moderate homogeneous enhancement (n = 26) and heterogeneous, low homogeneous, or absent enhancement (n = 16). The sensitivity, specificity, and accuracy of contrast‐enhanced sonography were 27.3%, 50.0%, and 38.1%. There was no statistically significant difference in the values between the benign and malignant groups (χ2 = 2.295; P = .130). Conclusions: Compared with contrast‐enhanced sonography, SWE has better diagnostic value and efficiency in differentiation of superficial lymph nodes unexplained by conventional sonography. When conventional sonography cannot differentiate malignant superficial lymph nodes from benign ones, SWE is a useful adjunctive tool for assessment of lymph nodes. [ABSTRACT FROM AUTHOR]

Abstract

Objective: To explore the possibility of reverting HBV-related hepatic fibrosis and cirrhosis with traditional Chinese medicine (TCM).Methods: A herbal recipe (861), comprising of 10 herbs includingSalvia miltiorrhiza, Astragalus membranaceus andSpatholobus saberectus were used as the antifibrotic agents. Three controlled clinical trials of treating HBV-related fibrosis and early cirrhosis were carried out. A total of 107 patients were assessed clinically and pathologically with double liver biopsies before and after treatment by means of modified Scheuer and Chevallier scoring system. Rat model of hepatic fibrosis induced by CC14 and human albumin immune injury, culture of separated hepatic stellate cells (HSC) were established. Total collagen content of the liver was determined by detection of hydroxyproline, amount of type I, III, IV collagens by histoimmunochemistry, quantitative measurement of mRNA for collagen I, III, IV, transforming growth factor-β (TGF-(3), matrix metalloproteinase (MMP1), MMP2, tissue inhibitor of metalloproteinase (TIMP) in liver tissue by dot blot hybridization. Serum and liver tissue collagenase activity was detected by method of Rajabi M and Emonard H. Histopathological study of liver specimen was done with H. E., Masson and sirius red stain as well as histoimmunochemistry.Results: (1) In HBV-related patients, after six months treatment with 861, the reversion rate was as high as 78% in S2, 82% in S3 (precirrhotic stage) and 75% in S4 (cirrhosis). These results correspond well with that obtained in animal experiment. (2) Total and type I, III, V collagen content in animal model liver were significantly reduced in amount after 861 treatment, whereas quantitation of mRNA for collagen I, III, V and TGF-β were also markedly suppressed either in liver tissue or cultured HSC, suggesting the suppression by 861 on fibrogenesis. At the same time, serum and liver tissue collagenase activity (latent and active) were enhanced significantly by administration of 861, while mRNA for MMP1 (interstitial collagenase) in cultured HSC and collagenase activity in the supernatant of the HSC culture were both increased, and meantime, mRNA for TIMP1 was significantly suppressed in quantity, indicating the enhancement of excessive extracellular matrix (ECM) degradation after 861 treatment.Conclusions: (1) Contrary to the conventional concept, the liver fibrosis and early cirrhosis due to HBV infection in man could be definitely reversed by TCM treatment of 861. This was also verified in CCI4 and immune injury models. (2) The mechanism leading to the reversal of fibrosis was due to suppression of fibrogenesis, and the concurrent enhancement of ECM degradation. In the latter, suppression of TIMP also plays an important role. (3) Both initiation by imflammation and perpetuation of the activation of HSC were suppressed by the 861.

Abstract

Purpose: Human skin consists of several layers including epidermis, dermis, and hypodermis. The determination of the in vivo mechanical properties of an individual skin layer represents a great challenge to date. In this study, the authors explore the use of the supersonic shear imaging (SSI) technique and inverse analysis to determine the in vivo elastic properties of the dermis layer of human skin. Methods: The measurements are conducted on the volar forearms and dorsal forearms of 18 healthy volunteers (nine females and nine males) using the SSI technique that gives the velocities of the shear wave generated by the acoustic force. Finite element analysis is carried out to simulate the propagation of the shear wave in the multilayer soft media and the results are used to interpret the experimental data and deduce the shear modulus of the dermis layer. Results: The shear moduli of the skin dermis layer obtained for the 18 healthy volunteers exhibit significant anisotropy. A standard statistical analysis demonstrates the differences between sexes. Conclusions: This study demonstrates that the SSI technique together with the inverse analysis represents a useful tool to characterize the in vivo elastic properties of human skin. This study is to evaluate the sustained-release effect of the thermosensitive in situ gel for injection of boanmycin hydrochloride (BAM) by bioluminescence imaging in nude mice. The FITC-labeled BAM (FITC-BAM) was purified by dialysis and Sephadex G25 gel column, and then was identified by matrix-assisted laser desorption ionization/time of flight (MALDI-TOF). The model of experimental hepatoma HepG-2 nude mice was established, and the optical imaging system was applied to evaluate the distribution of FITC-BAM in vivo. Results of MALDI-TOF proved that the major molecular ratio of BAM : FITC was 1 : 1 or 1 : 2. Bioluminescence imaging showed that the diffusion of FITC-BAM in situ gel group was significantly delayed compared with the negative control group. This study demonstrated that the thermosensitive in situ gel can effectively delay the release of boanmycin hydrochloride, and extend the retention time in vivo.

Abstract

Objective: To discuss the diagnostic value of an ultrasonic assessing system for detecting the severity of hepatic fibrosis in patients with chronic hepatitis B (CHB)., Methods: Ultrasonographic variables were analyzed in 110 CHB patients. An ultrasonic semi-quantitative scoring system using seven ultrasonic morphologic parameters, a Fisher discriminating function and three quantitative ultrasonic parameters was developed. The performance of these methods was also studied and compared., Results: The areas under the curve of the scoring system for different liver fibrosis stages were >or= S2: 0.946, >or= S3: 0.914, and S4: 0.915. The total score was well correlated with the histological stage of fibrosis (r=0.824, P < 0.001). There was a significant difference between the stages of fibrosis. The accuracy of the Fisher discriminating function for identifying three study endpoints was 76.5%, 78.2% and 67.3%. Combining the ultrasonic scoring system and the discriminating function, the specificity was 85%-90% and the accuracy was 77%-84%., Conclusion: Our ultrasonic semi-quantitative scoring system is a noninvasive method for quantitating liver fibrosis. If it is used together with a discriminating function, the accuracy of diagnosing liver fibrosis can be significantly increased.
