Abstract: The DIII-D tokamak has elucidated crucial physics and developed projectable solutions for ITER and fusion power plants in the key areas of core performance, boundary heat and particle transport, and integrated scenario operation, with closing the core-edge integration knowledge gap being the overarching mission. New experimental validation of high-fidelity, multi-channel, non-linear gyrokinetic turbulent transport models for ITER provides strong confidence it will achieve Q ⩾ 10 operation. Experiments identify options for easing H-mode access in hydrogen, and give new insight into the isotopic dependence of transport and confinement. Analysis of 2,1 islands in unoptimized low-torque IBS demonstration discharges suggests their onset time occurs randomly in the constant β phase, most often triggered by non-linear 3-wave coupling, thus identifying an NTM seeding mechanism to avoid. Pure deuterium SPI for disruption mitigation is shown to provide favorable slow cooling, but poor core assimilation, suggesting paths for improved SPI on ITER. At the boundary, measured neutral density and ionization source fluxes are strongly poloidally asymmetric, implying a 2D treatment is needed to model pedestal fuelling. Detailed measurements of pedestal and SOL quantities and impurity charge state radiation in detached divertors has validated edge fluid modelling and new self-consistent ‘pedestal-to-divertor’ integrated modeling that can be used to optimize reactors. New feedback adaptive ELM control minimizes confinement reduction, and RMP ELM suppression with sustained high core performance was obtained for the first time with the outer strike point in a W-coated, compact and unpumped small-angle slot divertor. Advances have been made in integrated operational scenarios for ITER and power plants. Wide pedestal intrinsically ELM-free QH-modes are produced with more reactor-relevant conditions, Low torque IBS with W-equivalent radiators can exhibit predator-prey oscillations in T _e and radiation which need control. High- β _P scenarios with q _min > 2, q _95 –7.9, β _N > 4, β _T –3.3% and H _98y2 > 1.5 are sustained with high density ( $\bar n$ = 7E19 m ^−3 , f _G –1) for 6 τ _E , improving confidence in steady-state tokamak reactors. Diverted NT plasmas achieve high core performance with a non-ELMing edge, offering a possible highly attractive core-edge integration solution for reactors.

2. Expression profile analysis of MATE gene family in rice

Abstract: Multidrug and toxic compound extrusion (MATE) proteins is a newly characterized transporter family in plants. However, knowledge of this family in systematic classification, molecular evolution, and expression patterns in plants is limited. In this study, MATE gene sequence, structure, and names as well as MATE protein size and subcellular localization in rice were analyzed using bioinformatics tools, chromosome localizations, and gene clusters. The function of MATE proteins was further elucidated on a basis of phylogenetic relationships. Using available transcriptomic data, the expression pattern and function of MATE were different in the selected organs and developments stages of rice. In addition, the relative abundance of OsMATE1 transcripts increased 3 h after copper treatment and so it was identified as a candidate gene for Cu tolerance in rice. This research provided basic data for further studies on MATE genes in rice and theoretical information about the biological function of MATE proteins.

3. Effects of bone morphogenetic protein-2 (BMP-2) and vascular endothelial growth factor (VEGF) release from polylactide-poly (ethylene glycol)-polylactide (PELA) microcapsule-based scaffolds on bone

Abstract: Multiple growth factors can be administered to mimic the natural process of bone healing in bone tissue engineering. We investigated the effects of sequential release of bone morphogenetic protein-2 (BMP-2) and vascular endothelial growth factor (VEGF) from polylactide-poly (ethylene glycol)-polylactide (PELA) microcapsule-based scaffolds on bone regeneration. To improve the double emulsion/solvent evaporation technique, VEGF was encapsulated in PELA microcapsules, to which BMP-2 was attached. The scaffold (BMP-2/PELA/VEGF) was then fused to these microcapsules using the dichloromethane vapor method. The bioactivity of the released BMP-2 and VEGF was then quantified in rat mesenchymal stem cells (rMSCs). Immunoblotting analysis showed that BMP-2/PELA/VEG promoted the differentiation of rMSCs into osteoblasts via the MAPK and Wnt pathways. Osteoblast differentiation was assessed through alkaline phosphatase expression. When compared with simple BMP-2 plus VEGF group and pure PELA group, osteoblast differentiation in BMP-2/PELA/VEGF group significantly increased. An MTT assay indicated that BMP-2-loaded PELA scaffolds had no adverse effects on cell activity. BMP-2/PELA/VEG promoted the differentiation of rMSCs into osteoblast via the ERK1/2 and Wnt pathways. Our findings indicate that the sequential release of BMP-2 and VEGF from PELA microcapsule-based scaffolds is a promising approach for the treatment of bone defects.

4. An effective depression filling algorithm for DEM-based 2-D surface flow modelling

Abstract: The surface runoff process in fluvial/pluvial flood modelling is often simulated employing a two-dimensional (2-D) diffusive wave approximation described by grid based digital elevation models (DEMs). However, this approach may cause potential problems when using the 2-D surface flow model which exchanges flows through adjacent cells, with conventional sink removal algorithms which also allow for flow exchange along diagonal directions, due to the existence of artificial depression in DEMs. In this paper, we propose an effective method for filling artificial depressions in DEM so that the problem can be addressed. We firstly analyse two types of depressions in DEMs and demonstrate the issues caused by the current depression filling algorithms using the surface flow simulations from the MIKE SHE model built for a medium-sized basin in Southeast England. The proposed depression-filling algorithm for 2-D overland flow modelling is applied and evaluated by comparing the simulated flows at the outlet of the catchment represented by DEMs at various resolutions (50 m, 100 m and 200 m). The results suggest that the existence of depressions in DEMs can substantially influence the overland flow estimation and the new depression filling algorithm is shown to be effective in tackling this issue based upon the comparison of simulations for sink-dominated and sink-free DEMs, especially in the areas with relatively flat topography.

6. A Case of Tuberculosis Misdiagnosed as Sarcoidosis and then Confirmed by NGS Testing.

Abstract: Background: Pulmonary tuberculosis (PTB) is an important infectious disease that threatens the health and life of human beings. In the diagnosis of PTB, imaging plays a dominant role, but due to the increasing drug resistance of Mycobacterium tuberculosis, atypical clinical manifestations, "different images with the same disease" or "different diseases with the same image" in chest imaging, and the low positivity rate of routine sputum bacteriology, which leads to a high rate of misdiagnosis of PTB. We report a case of pulmonary tuberculosis that was misdiagnosed on imaging. We report a case of pulmonary tuberculosis that resembled sarcoidosis on imaging and was negative for antacid staining on sputum smear and alveolar lavage fluid, and was later diagnosed by microbial next-generation sequencing (NGS). The case was initially misdiagnosed as sarcoidosis. Methods: Alveolar lavage fluid NGS, chest CT, bronchoscopy. Results: Chest CT showed multiple inflammatory lesions in both lungs, multiple nodular foci in both lungs, and multiple enlarged lymph nodes in the mediastinum and hilar region on both sides. Fiberoptic bronchoscopy was performed in the basal segment of the left lower lobe of the lungs to carry out bronchoalveolar lavage, and the lavage fluid was sent to the NGS test and returned the following results: Mycobacterium tuberculosis complex group detected in the number of sequences of 293. Based on the results of the NGS test, the diagnosis of pulmonary tuberculosis could be confirmed. Conclusions: The diagnosis of pulmonary tuberculosis cannot be easily excluded in patients with "different images with the same disease" or "different diseases with the same image" on chest imaging without the support of sputum positivity. The goal was to improve the alertness of medical personnel to the misdiagnosis of tuberculosis and the application of NGS technology. [ABSTRACT FROM AUTHOR]

Abstract: Background: Patients with tuberculous empyema (TE) can have a serious impact on lung function as their disease progresses, and, if left untreated, can cause damage to other parts of the body such as the thorax and spine, causing pain and inconvenience to the patient. Early diagnosis and the search for appropriate treatment are key to improving the survival rate of the disease. Methods: We report a case of a young patient with an unexpected finding of right pleural effusion on physical examination, who was eventually diagnosed with TE using next-generation sequencing of pleural tissue. We analyzed the literature to improve clinicians' understanding of TE and how to properly diagnose and treat the disease. Results: Laboratory results of the pleural effusion suggested a possible Mycobacterium tuberculosis infection, but pathogen-related tests were negative, and the diagnosis was eventually successfully confirmed by thoracoscopic pleural biopsy. Conclusions: The diagnosis of TE should be considered in young patients with pleural thickening of the empyema. Adenosine deaminase may provide diagnostic direction in patients with unexplained thorax abscess. Pleural biopsy, although an invasive procedure, is an essential diagnostic tool in some cases. Abstract: There has been a long-standing debate about the mechanism of the unusual superconductivity in alkali-intercalated fulleride superconductors. In this work, using high-resolution angle-resolved photoemission spectroscopy, we systematically investigate the electronic structures of superconducting K3C60 thin films. We observe a dispersive energy band crossing the Fermi level with an occupied bandwidth of about 130 meV. The measured band structure shows prominent quasiparticle kinks and a replica band involving high-energy Jahn-Teller active Hg(8) phonon mode, reflecting strong electron-phonon coupling in the system. The electron-phonon coupling constant is estimated to be about 1.2, which dominates the quasiparticle mass renormalization. Moreover, we observe an isotropic nodeless superconducting gap beyond the mean-field estimation. Abstract: Fast transitions between different types of power density spectra (PDS) happening over time-scales of several tens of seconds are rare phenomena in black hole X-ray binaries. In this paper, we report a broad-band spectral-timing analysis of the fast transitions observed in the 2021 outburst of GX 339−4 using NICER and Insight-HXMT observations. We observe transitions between band-limited noise-dominated PDS and type-B quasi-periodic oscillations (QPOs), and their rapid appearance or disappearance. We also make a detailed comparison between the fast transitions in GX 339−4 with those seen in MAXI J1820+070 and MAXI J1348−630. By comparing the spectra of the periods with and without type-B QPOs, we find that the spectral ratios above 10 keV are nearly constant or slightly decreasing, and the values are different between sources. Below 10 keV, the flux change of the Comptonization component is inversely proportional to the flux change of the thermal component, suggesting that the appearance of type-B QPOs is associated with a redistribution of the accretion power between the disc and the Comptonizing emission region. The spectral ratios between the periods with type-B QPO and those with broad-band noise are significantly different from that with type-B QPO and without type-B QPO, where the ratios (type-B QPO/broad-band noise) show a maximum at around 4 keV and then decrease gradually towards high energies. Finally, we discuss the possible change of the geometry of the inner accretion flow and/or jet during the transitions.

Abstract: We studied the 2018 outburst of the black hole transient H 1743 − 322 with a series of Insight-HXMT, NICER, and NuSTAR observations, covering the 1–120 keV band. With our broad-band X-ray spectral modelling, we confirm that the source remained in the low/hard state throughout the month-long outburst, although it became marginally softer at peak flux. We detected Type-C quasi-periodic oscillations (QPOs) and followed the evolution of their properties. The QPO frequency increased from ∼0.1 to ∼0.4 Hz during the rising phase of the outburst and decreased again in the decline. Continuum X-ray flux, power-law photon index, QPO frequency, and QPO root-mean-square amplitude were positively correlated. The QPO amplitude was slightly higher in the soft X-ray band (typical values of 12–16 per cent, compared with 8–10 per cent in the hard band). Our spectral-timing results shed light on the initial rising phase in the low/hard state, which has rarely been monitored with such high cadence, time resolution, and broad-band coverage. Combining spectral and timing properties, we find that ‘failed’ (hard state only) and ‘successful’ outbursts follow the same initial evolutionary track, although the former class of outburst never reaches the threshold for a transition to softer (thermally dominated) accretion regimes.

Abstract: Background: Current studies have reported that it is rare for the coronavirus disease 2019 (COVID-19) to be combined with two fungal infections and that COVID-19 can be combined with multiple cardiovascular complications, both of which can complicate the condition and increase the risk of death. Methods: We report a case of COVID-19 in which Aspergillus fumigatus and Cryptococcus neoformans were detected by sputum targeted next-generation sequencing (tNGS) and cardiac monitoring during treatment revealed cardiovascular complications. Results: We consider that this patient's fungal infection was associated with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection and the application of corticosteroids. In addition, cardiovascular complications were associated with an inflammatory response and increased sympathetic stimulation due to SARS-CoV-2 infection. Conclusions: The presence of COVID-19-associated fungal infections cannot be excluded when multiple risk factors for fungal infections are present in patients with COVID-19 and the condition is rapidly deteriorating. Abstract: We report the evolution of the X-ray pulsations of EXO 2030+375 during its 2021 outburst using the observations from Insight-HXMT. Based on the accretion torque model, we study the correlation between the spin frequency derivatives and the luminosity. Pulsations can be detected in the energy band of 1–160 keV. The pulse profile evolves significantly with luminosity during the outburst, leading to that the whole outburst can be divided into several parts with different characteristics. The evolution of the pulse profile reveals the transition between the supercritical (fan-beam dominated) and the subcritical accretion (pencil-beam dominated) mode. From the accretion torque model and the critical luminosity model, based on a distance of 7.1 kpc, the inferred magnetic fields are (0.41 − 0.74) × 1012 and (3.48 − 3.96) × 1012 G, respectively, or based on a distance of 3.6 kpc, the estimated magnetic fields are (2.4 − 4.3) × 1013 and (0.98 − 1.11) × 1012 G, respectively. Two different sets of magnetic fields both support the presence of multipole magnetic fields of the neutron star.

Abstract: Insulating joint is an essential component for pipeline cathodic protection system. During the safety confirmation before the excavation of the station foundation pit, a pipeline company found that the insulating joint was failed due to leakage. In order to determine the causes of leakage, data verification, tests and failure component analysis were carried out. The results show that: (1) the leakage point cannot be detected by NDT, hydraulic pressure test, and hydraulic pressure + bending test, and the gas leak test in the water container demonstrates two leakage points on the fixing sleeve of the insulating joint; (2) the insulating joint has an unfilled cavity, and the mechanical holes on the fixing sleeve are blocked by bolts, which are inconsistent with the assembly drawing; and (3) the leakage of the insulating joint was caused by the sealing failure during assembly or service, which allowed some gas to gather in the cavity between the flange and the fixing sleeve and then leak through the gap between the mechanical hole and the bolt. Accordingly, it is suggested that (1) manufacturers should strictly follow the process when producing insulating joints, the product structure be consistent with the assembly drawing, the product inspection be done according to requirements, and NDT be done on all welding areas; and (2) the insulating joint be leak-tested in the water container where possible.

Abstract: In this paper, a new robust computational method that applies the geometrical theory of diffraction (GTD) in conjunction with the ray tracing (RT) technique is developed to evaluate the electromagnetic scattering pattern due to a plane wave represented as beam of parallel rays incident on a rough surface of quite arbitrary statistical parameters. The development of the proposed technique is explained in detail taking into consideration the generation of the geometrical model of the rough surface. The Fresnel reflection model is applied under the assumption of arbitrary electrical and optical properties of the rough surface material. Also the polarization of the plane wave primarily incident on the rough surface is taken into consideration. The algorithm developed in the present work accounts for multiple bounces of an incident ray and, hence, it can be considered arbitrary higher-order GTD-RT technique. The accuracy of the obtained results is verified through the comparison with the experimental measurements of the scattering pattern of a light beam incident on rough sheets with specific statistical properties. The numerical results of the present work are concerned with investigating the dependence of the scattering pattern on the surface roughness, refractive index, angle of incidence, and the resolution of the geometric model of the rough surface. Also, it is shown that, for limited resolution of the rough surface model, the accuracy of the calculated scattered field depends on the angle of incidence of the primary beam and the surface roughness.

Abstract: Here we present a detailed study of the broadband noise in the power density spectra of the black hole X-ray binary MAXI J1820+070 during the hard state of its 2018 outburst, using Hard X-ray Modulation Telescope observations. The broadband noise shows two main humps, which might separately correspond to variability from a variable disk and two Comptonization regions. We fitted the two humps with multiple Lorentzian functions and studied the energy-dependent properties of each component up to 90–150 keV and their evolution with spectral changes. The lowest-frequency component is considered as the subharmonic of the quasiperiodic oscillation component and shows a different energy dependence compared with other broadband noise components. We found that although the fractional rms of all the broadband noise components mainly decreases with the energy, their rms spectra are different in shape. Above ∼20–30 keV, the characteristic frequencies of these components increase sharply with the energy, meaning that the high-energy component is more variable on short timescales. Our results suggest that the hot inner flow in MAXI J1820+070 is likely to be inhomogeneous. We propose a geometry with a truncated accretion disk and two Comptonization regions.

Abstract: We study the spectral evolution of the black hole candidate EXO 1846$-$031 during its 2019 outburst, in the 1--150 keV band,with the {\it {Hard X-ray Modulation Telescope}}. The continuum spectrum is well modelled with an absorbed disk-blackbody plus cutoff power-law, in the hard, intermediate and soft states. In addition, we detect an $\approx$6.6 keV Fe emission line in the hard intermediate state. Throughout the soft intermediate and soft states, the fitted inner disk radius remains almost constant; we suggest that it has settled at the innermost stable circular orbit (ISCO). However, in the hard and hard intermediate states, the apparent inner radius was unphysically small (smaller than ISCO), even after accounting for the Compton scattering of some of the disk photons by the corona in the fit. We argue that this is the result of a high hardening factor, $f_{\rm col}\approx2.0-2.7$, in the early phases of outburst evolution, well above the canonical value of 1.7 suitable to a steady disk. We suggest that the inner disk radius was close to ISCO already in the low/hard state. Furthermore, we propose that this high value of hardening factor in the relatively hard state is probably caused by the additional illuminating of the coronal irradiation onto the disk. Additionally, we estimate the spin parameter with the continuum-fitting method, over a range of plausible black hole masses and distances. We compare our results with the spin measured with the reflection-fitting method and find that the inconsistency of the two results is partly caused by the different choices of $f_{\rm col}$., Comment: 21 pages, 6 figures

Abstract: 4-(2-Chlorobenzyl)-1-(5-nitro-2-(pyrrolidin-1-yl)phenyl)-[1,2,4]triazolo[4,3-a]quinazo-lin-5(4H)-one is a derivative of quinazolinones with antitumor, antibacterial, anti-inflammatory, and antimicrobial effects. Using diabetic jujube as a raw material, the title compound is synthesized by substitution and cyclization steps. The structure of the target compound is confirmed by FTIR, 1H and 13C NMR, and MS spectroscopies. The precise structure of the 4-(2-chlorobenzyl)-1-(5-nitro-2-(pyrrolidin-1-yl)phenyl)-[1,2,4]triazolo[4,3-a]quinazolin-5(4H)-one compound is analyzed by single crystal X-ray diffraction (XRD). The molecular structure is further calculated using density functional theory (DFT) and the result is compared with the XRD value. The molecular electrostatic potential and frontier molecular orbitals of the title compound are investigated using DFT. In addition, the obtained atomic coordinates for the single crystal of the compound are then applied in a molecular docking simulation, and the title compound is found to participate in a number of important binding interactions in the SHP2 binding sites.

Abstract: In this letter, a multifunction aperture array is proposed for ultrawideband (UWB) scanning and polarization reconfiguration. The UWB array consisted of linearly polarized elements is capable of operating in four polarization modes (+45° linear polarization (LP), -45° linear polarization, left-hand circular polarization (LHCP) and right-hand circular polarization (RHCP)). This work involves two essential techniques: (a) A new beam-scanning UWB array synthesis approach. An iterative convex optimization strategy is utilized to determine the element locations and obtain the minimum sidelobe level (SLL) for multiple patterns. (b) The polarization reconfigurable technique for beam-scannable arrays. In this part, a sequential rotation and excitation compensation (SR-EC) technique provides polarization reconfiguration for a beam-scannable array consisting of linearly polarized elements. A beam-scanning UWB array is designed by using the proposed UWB array synthesis approach and the SR-EC polarization reconfigurable technique. The Feko numerical result shows 0°-60° beam peak steering, a 4:1 bandwidth (1-4 GHz), and fourpolarization reconfigurability.

Abstract: Quinazolinones and their derivatives are an important class of broad-spectrum pharmacodynamic groups with a wide range of pharmacological activities. We designed and synthesized the new 4-(2-chlorobenzyl)imidazole[1,2-a]quinazolin-5(4H)-one compound and used 1H and 13C NMR, FTIR spectroscopy and MS to determine its structure. In addition, the structure of the single crystal is confirmed using X-ray diffraction. The optimal structure of the compound is calculated by DFT using the B3LYP functional group (based on 6-311G(2d,p)) and compared with the structure determined by X-ray diffraction. The results show that the crystal structure determined by X-ray diffraction is consistent with the molecular structure optimized using DFT.

29. [Clinical controversy and research thoughts on lymphadenectomy of gastric cancer]

Abstract: Lymphadenectomy, as one of the controversial foci in clinic, is an extremely important part of radical surgery for gastric cancer. So far, the preliminary consensus has been reached on the scope and number of lymph node dissection, based on the etiological mechanism, disease progression, diagnosis and treatment prognosis of gastric cancer. At present, some clinical issues of lymphadenectomy in curative gastrectomy are still need to be addressed. Firstly, standardized procedure in lymph node dissection for gastric cancer is a prerequisite to decrease the incidence of postoperative complications and to improve the prognosis of gastric cancer patients. Furthermore, the plausible treatment strategy in perioperative phase is also deemed as the other key method to offer a benefit of survival rate for advanced stage patients after lymphadenectomy. Last but not least, the technologies for enhancement the prediction accuracy of lymph node metastasis preoperatively or intraoperatively should be worthy in-depth study.淋巴结清扫是胃癌根治性手术中极其重要的操作步骤，也是临床富有争议的焦点之一。目前，依据胃癌的病因机制、病程演进和诊治转归，对于淋巴结清扫的范围和数目已经达成初步的共识，但现阶段在胃癌淋巴结清扫中仍然存有不少需要解决的临床问题。规范化的手术操作是降低淋巴结清扫后并发症发生率和改善患者预后的必要前提，围手术期合理治疗策略的实施则是进一步提高进展期患者淋巴结清扫后生存率的重要方式，增强淋巴结转移的术前和术中检测准确性则仍需要继续深入的研究。.

Publisher Copyright: © 2022 IAEA, Vienna. DIII-D physics research addresses critical challenges for the operation of ITER and the next generation of fusion energy devices. This is done through a focus on innovations to provide solutions for high performance long pulse operation, coupled with fundamental plasma physics understanding and model validation, to drive scenario development by integrating high performance core and boundary plasmas. Substantial increases in off-axis current drive efficiency from an innovative top launch system for EC power, and in pressure broadening for Alfven eigenmode control from a co-/counter-I p steerable off-axis neutral beam, all improve the prospects for optimization of future long pulse/steady state high performance tokamak operation. Fundamental studies into the modes that drive the evolution of the pedestal pressure profile and electron vs ion heat flux validate predictive models of pedestal recovery after ELMs. Understanding the physics mechanisms of ELM control and density pumpout by 3D magnetic perturbation fields leads to confident predictions for ITER and future devices. Validated modeling of high-Z shattered pellet injection for disruption mitigation, runaway electron dissipation, and techniques for disruption prediction and avoidance including machine learning, give confidence in handling disruptivity for future devices. For the non-nuclear phase of ITER, two actuators are identified to lower the L-H threshold power in hydrogen plasmas. With this physics understanding and suite of capabilities, a high poloidal beta optimized-core scenario with an internal transport barrier that projects nearly to Q = 10 in ITER at ∼8 MA was coupled to a detached divertor, and a near super H-mode optimized-pedestal scenario with co-I p beam injection was coupled to a radiative divertor. The hybrid core scenario was achieved directly, without the need for anomalous current diffusion, using off-axis current drive actuators. Also, a controller to assess proximity to stability limits and regulate β N in the ITER baseline scenario, based on plasma response to probing 3D fields, was demonstrated. Finally, innovative tokamak operation using a negative triangularity shape showed many attractive features for future pilot plant operation.

Abstract: Pancreatic cancer (PC) is a highly aggressive and refractory disease, with disappointing 5-year survival rates. Regarding the wide application of neoadjuvant treatment in patients with PC, how the post-neoadjuvant Carbohydrate antigen 19-9 (CA19-9) response could translate into a survival benefit is not clearly understood. We aimed to evaluate the correlation of the CA19-9 response with overall survival (OS) in patients with PC receiving neoadjuvant therapy.An extensive electronic search in PubMed, Embase, and the Cochrane Library was performed to identify relevant articles, from which data relevant to independent correlations of the CA19-9 response with overall survival (OS) were extracted for analysis. A random-effects model was used to calculate the pooled hazard ratios (HRs) with their corresponding 95% confidence intervals (CIs).Altogether, 17 eligible studies were identified in the systematic review. Pooled analysis showed that CA19-9 response 50% (HR, 0.43; 95% CI 0.29-0.56; P 0.001) and normalization of CA19-9 (HR, 0.52; 95% CI 0.42-0.63; P 0.001) after neoadjuvant treatment are significantly associated with promising overall survival. The results also showed that optimal CA19-9 response after neoadjuvant treatment was significantly related to a favorable prognosis (HR = 0.49, 95% CI 0.42-0.55, P 0.001; I2 = 45.1%, P = 0.04). Subgroup analysis revealed there were no prognostic difference between CA19-9 50% and normalization of CA19-9 after neoadjuvant treatment (P = 0.338), but the duration of neoadjuvant chemotherapy over 4 months was significantly associated with expanded postoperative survival (P = 0.013).Serum CA19‑9 is valuable in determining the effect of neoadjuvant treatment in patients with PC. Post-neoadjuvant CA19-9 response 50% or CA19-9 normalization was related to a more promising overall survival, suggesting that optimal CA19-9 response may be a suitable prognostic index to guide treatment decisions.

32. Demirjian method and Willems method to study the dental age of adolescents in Shanghai before and after 10 years

Abstract: The aim of this study is to assess the validity of whether the difference in dental age between 2009 to 2011 and 2021 is affected by environmental factors such as environmental pollution, floating population, and dietary habits. Demirjian and Willems dental age estimation methods were conducted for a Han population of children aged 8 to 14 in Shanghai, China. A total of 1,259 digital panorama radiographs of children aged 8.00-14.99 was estimated. All digital panorama radiographs were estimated using the Demirjian and Willems methods.Both 2009 to 2011 and 2021 were statistically analyzed by paired t-tests. The results show that the Demirjian method overestimates 2009 to 2011 and underestimates 2021 than children's chronological age. According to the Willms method, males are overestimated and females are underestimated from 2009 to 2011, and both genders were underestimated in 2021.In conclusion, the difference in dental age between 2009 to 2011 and 2021 was statistically significant. Factors such as environment and dietary habits have been found to be affected by dental development. However, there are disputes among some researchers about the exact factors, so it is suggested to further study the effects of environmental factors on tooth development.

Abstract: We present a spectral study of the black hole candidate MAXI J1348-630 during its 2019 outburst, based on monitoring observations with Insight-HXMT and Swift. Throughout the outburst, the spectra are well fitted with power-law plus disk-blackbody components. In the soft-intermediate and soft states, we observed the canonical relation L ~ T_in^4 between disk luminosity L and peak colour temperature T_in, with a constant inner radius R_in (traditionally identified with the innermost stable circular orbit). At other stages of the outburst cycle, the behaviour is more unusual, inconsistent with the canonical outburst evolution of black hole transients. In particular, during the hard rise, the apparent inner radius is smaller than in the soft state (and increasing), and the peak colour temperature is higher (and decreasing). This anomalous behaviour is found even when we model the spectra with self-consistent Comptonization models, which take into account the up-scattering of photons from the disk component into the power-law component. To explain both those anomalous trends at the same time, we suggest that the hardening factor for the inner disk emission was larger than the canonical value of ~1.7 at the beginning of the outburst. A more physical trend of radii and temperature evolution requires a hardening factor evolving from ~3.5 at the beginning of the hard state to ~1.7 in the hard intermediate state. Abstract: The High Energy X-ray telescope (HE) on-board the Hard X-ray Modulation Telescope (\textit{Insight}-HXMT) can serve as a wide Field of View (FOV) gamma-ray monitor with high time resolution ($\mu$s) and large effective area (up to thousands cm$^2$). We developed a pipeline to search for Gamma-Ray Bursts (GRBs), using the traditional signal-to-noise ratio (SNR) method for blind search and the coherent search method for targeted search. By taking into account the location and spectrum of the burst and the detector response, the targeted coherent search is more powerful to unveil weak and sub-threshold bursts, especially those in temporal coincidence with Gravitational Wave (GW) events. Based on the original method in literature, we further improved the coherent search to filter out false triggers caused by spikes in light curves, which are commonly seen in gamma-ray instruments (e.g. \textit{Fermi}/GBM, \textit{POLAR}). We show that our improved targeted coherent search method could eliminate almost all false triggers caused by spikes. Based on the first two years of \textit{Insight}-HXMT/HE data, our targeted search recovered 40 GRBs, which were detected by either \textit{Swift}/BAT or \textit{Fermi}/GBM but too weak to be found in our blind search. With this coherent search pipeline, the GRB detection sensitivity of \textit{Insight}-HXMT/HE is increased to about 1.5E-08 erg/cm$^2$ (200 keV$-$3 MeV). We also used this targeted coherent method to search \textit{Insight}-HXMT/HE data for electromagnetic (EM) counterparts of LIGO-Virgo GW events (including O2 and O3a runs). 39. Intestinal Dysbacteriosis-Propelled T Helper 17 Cells Activation Mediate the Perioperative Neurocognitive Disorder Induced by Anesthesia/Surgery in Aged Rats

Abstract: Background: Perioperative neurocognitive disorders (PND) occur frequently in elderly patients after surgery, but the mechanism of PND is not very clear at present. It is reported that anesthesia/surgery could cause intestinal flora imbalance and induce neurocognitive impairment. However, the effect of intestinal flora on PND is poorly understood. We previously found that peripheral interleukin-17A (IL17A) destroyed the blood-brain barrier (BBB), leading to central inflammation and neurocognitive impairment. The small intestine is the main place where Th17 cells are produced. Therefore, we hypothesized that Th17 cells and IL-17 may be an important bridge for intestinal microbes to cause neuroinflammation. Methods: Exploratory laparotomy was performed to establish PND model under sevoflurane anesthesia. 16S rRNA high-throughput sequencing was used to detect the changes of intestinal flora. To explore the relationship between intestinal flora and PND, compound antibiotics were used to eliminate intestinal flora before anesthesia/surgery, and behavior tests, such as open field, Y maze, and fear conditioning tests were applied to detect the changes of memory ability and which was compared with the rats that did not receive compound antibiotics. The number of Th17 cells and Foxp3 cells was detected by flow cytometry in the Peyer's patches (PP), mesenteric lymph nodes (MLN), blood and brain. Hippocampus IL17, IL17RA, IL6 and IL10 were detected by Western blot. Hippocampus IL17, IL17R and IBA1 (ionized calcium binding adaptor molecule1) were detected by immunofluorescence. Results: Anesthesia/surgery caused intestinal flora imbalance and induced neurocognitive impairment, increased the number of Th17 cells in the PP, MLN, blood and brain, up-regulated the lever of IL17, IL17R and inflammatory factor production in the hippocampus. The administration of compound antibiotics before anesthesia/surgery evidently inhibited this effect, including decreased the number of Th17 cells, down-regulated the lever of IL17, IL17R and inflammatory factor production, and improved the memory function. In addition, we found that IL17R was co-labeled with IBA1 in a large amount in the hippocampus through immunofluorescence double-staining. Conclusion: Our study suggested that intestinal dysbacteriosis-propelled T helper 17 cells activation might play an important role in the pathogenesis of PND.

40. Imaging-guided percutaneous transthoracic needle biopsy of nodules in the lung base: fluoroscopy CT versus cone-beam CT

Abstract: To compare the diagnostic performance and safety of fluoroscopy computed tomography (FCT)-guided to cone-beam CT (CBCT)-guided percutaneous transthoracic needle biopsy (PTNB) in patients with nodules in the lung base.The clinical data of 379 patients (228 in the FCT group and 151 in the CBCT group) with nodules in the lung base who underwent FCT-guided or CBCT-guided PTNB from September 2009 to March 2020 were analysed retrospectively. Study outcomes included patient demographics, nodule characteristics, technical success, pathological diagnosis, complications, and radiation exposure.PTNB technical success rates were 98.2% in the FCT group and 98.7% in the CBCT group (p=0.548). For larger lesions (2 cm in diameter), PTNBs under FCT had similar diagnostic accuracy (95.5% versus 93.64%), incidence of pneumothorax (31.5% versus 21.3%) and radiation exposure (16.8 ± 6.1 versus 15.2 ± 4 mSv) compared to those under CBCT guidance. For smaller lesions (≤2 cm in diameter), the diagnostic accuracy was similar (92.86% versus 100%), but there was a higher incidence of pneumothorax (66.7% versus 31%, p=0.030) and more radiation exposure (19.9 ± 8.4 versus 15.3 ± 5 mSv, p=0.040) under FCT guidance than CBCT.PTNBs under FCT and CBCT guidance had similar performance for larger lesions in lung base. CBCT guidance performed better for smaller lesions in terms of radiation dose and complications. Radiologists could optimise the technological resources based on equipment availability and lesion characteristics.

Abstract: 1. MicroRNAs are small noncoding RNA molecules that play crucial roles in gene expression. However, the comparative profiling of testicular and ovarian microRNAs in birds are rarely reported, particularly in pigeon.2. In this study, Illumina next-generation sequencing technology was used to sequence miRNA libraries of the gonads from six healthy adult utility pigeons. A total of 344 conserved known miRNAs and 32 novel putative miRNAs candidates were detected. Compared with those of ovaries, 130 differentially expressed (DE) miRNAs were identified in the testes. Among them, 70 miRNAs showed down-regulation in the ovaries, while another 60 miRNAs were up-regulated.3. Combining the results of the expression of target gene measurements and pathway enrichment analyses, it was revealed that some DEmiRNAs from the gonad samples involved in sexual differentiation and development (such as cli-miR-210-3p and cli-miR-214-3p) could down-regulate AR (androgen receptor). Cli-miR-181b-5p, cli-miR-9622-3p and cli-miR-145-5p were highly expressed in both the ovaries and testes, which could co-target HOXC9, and were related to regulation of primary metabolic processes. KEGG enrichment analysis showed that DEmiRNAs may play biological and sex-related roles in pigeon gonads.4. The expression profiles of testicular and ovarian miRNA in adult pigeon gonads are presented for the first time, and the findings may contribute to a better understanding of gonadal expression in poultry.

Abstract: 原发于鼻腔及鼻窦的肠型腺癌（SNS-ITAC）是一种发生于鼻腔、鼻窦内的恶性腺样肿瘤，局部侵袭性较强，常累及筛窦、鼻腔及上颌窦。本文报道2例原发性SNS-ITAC，镜下观察2例患者肿瘤组织学形态均呈乳头状生长，部分呈小管-腺管样生长，肿瘤细胞核呈长杆状，假复层或复层排列，胞质嗜酸性，可见少量杯状细胞，细胞轻度异型性，可见核分裂象，具有肠型腺癌相似的分化和细胞学特征。免疫组织化学表型：CDX-2、CK20、CK7、Villin阳性，SATB2少数细胞阳性，Ki-67阳性指数约30%，均符合SNS-ITAC。原发性SNS-ITAC是一种罕见肿瘤，发病率为百万分之一，依据临床及病理组织学特点，并结合免疫组织化学染色可以明确诊断。.

Abstract: Associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) can speed up the regeneration of future liver remnant (FLR) in short period of time, and offer a chance for surgical resection for patients without sufficient FLR. However, ALPPS still remains controversy due to its high perioperative morbidity and mortality, as well as the uncertain long-term oncological benefits. How to solve these problems is the key to ensure the safety of surgery.This article focus on the indication selection, liver function reserve evaluation and timing to perform the second stage surgery, surgical mode evolution and comparison with portal venous embolization/portal venous ligation+two-stage hepatectomy.联合肝实质分隔和门静脉结扎的二步肝切除术（ALPPS）可在短期内增大剩余肝脏体积，理论上可为剩余肝脏体积不足的肝肿瘤患者提供手术切除机会。然而，ALPPS的临床应用效果目前尚无定论。现有报道提示，ALPPS具有较高的围手术期并发症发生率和病死率，且长期肿瘤学获益尚不明确。解决以上问题是推广应用该术式的关键。本文从ALPPS适应证选择、肝脏储备功能评估、第二步手术时机选择、手术方式演变、与门静脉栓塞或门静脉结扎+二步法肝切除比较等热点和难点问题进行分析和阐述。.
