The article discusses the use of cross leg flaps as a solution for wound coverage in cases of severe leg trauma. While the frequency of cross leg flap procedures has decreased with the advancement of microsurgical techniques, it is still important to have this technique available as a last resort or in areas without microsurgical expertise. The article provides technical tips and an algorithmic approach for deciding on the type of fixation technique for maintaining the position of the two limbs during the procedure. The authors propose the use of external fixators and describe the process of creating a fixator frame specifically for cross leg flaps. The article also emphasizes the importance of patient comfort and preventing pressure sores during the postoperative period. [Extracted from the article]

Peripheral nerve sheath tumors known as neurofibromas are benign and can be a part of syndromes like neurofibromatosis Type-1 (NF1) or type-2. NF1 is linked to plexiform neurofibroma. They are primarily made up of fibroblasts and Schwann cells. It can involve any nerve. In our case, a branch of the superficial peroneal nerve gave rise to neurofibroma, which extrinsically compressed the deep peroneal nerve. The patient's symptoms resembled those of anterior tarsal tunnel syndrome. [ABSTRACT FROM AUTHOR]

Hypospadias is a common congenital anomaly in males and frequently the patient undergoes multiple surgeries and may have complications. The treatment of severe "iatrogenic strictures" presenting in adulthood resulting from childhood hypospadias surgeries continues to be a challenge. The complexity of these cases is increased manifold due to the reasons of delayed presentation of many decades, varying lengths of the stricture, as well as poor healing capacity due to scarred territory from previous surgical attempts and associated complicating factors of renal failure and fistulae. In this paper, a case of reconstruction of a severe urethral stricture in a hypospadias cripple using a pedicled anterolateral thigh flap is reported. [ABSTRACT FROM AUTHOR]

Bosma arhinia microphthalmia (BAM) is a very rare illness, with fewer than 100 cases documented worldwide. Criteria for diagnosis of BAM syndrome include congenital absence of nose (arhinia) or hypoplasia, hypogonadotropic hypogonadism, and eye defects (microphthalmia) in males. Our patient was a male child of 4 years of age who was brought by parents with left nasal arhinia for reconstruction. We devised three‑layered nasal reconstruction for the child. Flip‑over flap from the other half of the nose was used for the nasal lining, conchal cartilage grafts for the nasal framework, and pedicled forehead flap for skin cover. [ABSTRACT FROM AUTHOR]

20. Surgical management of extracapsular condylar fracture of the mandible: Our method.

Introduction: In face trauma, mandibular fractures are frequently seen, and 20%-60% are associated with condylar fracture. Conservative treatment has its limitations, whereas surgical treatment helps to achieve exact fragment positioning and hence mandibular ramus height and better radiologic result concerning resorption and pathologic changes to the condyle. This article describes our approach with modified Risdon incision. Materials and Methods: Patients admitted to our department with condylar fracture over a period of 4 years underwent clinical examination and radiological investigation. Out of the total 25 patients with condylar fractures, there were 20 extracapsular fractures involving the neck and subcondylar regions. The subcondylar fracture patients were treated with extended submandibular approach. Results: Out of twenty patients, two patients had isolated condyle fractures. The bilateral condylar fracture was found in two patients. Thirteen patients had fractures at the lower neck level and the remaining seven patients had a fracture at the subcondylar level. The undisplaced fracture was found in three patients, and a displaced fracture fragment was found in 12 patients, of which 4 patients had a medial override and 8 had a lateral override. The remaining five patients had a deviation of the proximal fracture fragment. Conclusion: Most of the low-neck and subcondylar fractures can be assessed using a posterior extension of submandibular incision and these fractures should be managed with open reduction and internal fixation (ORIF) to avoid complications associated with conservative management. ORIF can be managed with the use of proper retraction techniques such as reverse L retractor and the use of traction wire using simple stainless steel wires and appropriate mini-plates. Median cleft of lower lip and mandible is a rare anomaly. This Cleft has also been described as Cleft No. 30 of Tessier′s classification. In minor forms only lower lip is cleft. Frequently, the cleft extends into the mandibular symphysis and the tongue is attached to the cleft alveolar margin. At times the tongue may be bifid or absent, hyoid absent, thyroid cartilage underdeveloped, strap muscles atrophic, manubrium sterni absent, clavicles widely spaced etc. The earliest report of this anomaly was by Couronne′ in 1819. Since then very few cases have been reported in literature with variations. We describe a male child who presented at the age of 6 months with an ectopic salivary gland on the dorsum of the tongue in addition to median cleft of lower lip, ankyloglossia and notching of the mandible. Excision of mass on dorsum of tongue, release of ankyloglossia and lip from the alveolus followed by repair was done. No bony work was done since the mandible was only notched. On post-operative follow-up at 18 months, dentition was delayed in both maxillary as well as mandibular teeth and there was a gap between the lower central incisors. At the age of 2 years 4 months, the dentition is still not complete and the gap between the lower central incisors is very apparent. There is a supernumerary upper central incisor on right side. There is no mobility between the two segments of mandible. Speech is normal. A regular follow-up will be done to study the eruption of permanent central incisors at the age of 7 years and till eruption of all permanent teeth to assess the occlusion and to decide whether any bony work is needed or not.

It may occur de novo or as a malignant transformation of an eccrine poroma. It is commonly found in older age group and in the lower extremities. Clinically, it may present as a verrucous plaque, polypoid growth or an ulcerative lesion of long duration. Local recurrence and metastasis to skin, lymphnodes, viscera, and bone may occur. Treatment is wide local excision. Metastatic lesions can be treated with chemotherapy. We report a case of eccrine porocarcinoma of the scalp in a 50 years old female who presented to us with a bosselated, firm, painless, non-tender, freely mobile swelling over left fronto-parietal region of 12 years duration. It was excised and histopathological diagnosis was Eccrine Porocarcinoma. In literature, scalp porocarcinoma is a very rare tumor.

Eastern equine encephalitis virus (EEEV) has a high case-fatality rate in horses and humans, and Florida has been hypothesized to be the source of EEEV epidemics for the northeastern United States. To test this hypothesis, we sequenced complete genomes of 433 EEEV strains collected within the United States from 1934 to 2014. Phylogenetic analysis suggested EEEV evolves relatively slowly and that transmission is enzootic in Florida, characterized by higher genetic diversity and long-term local persistence. In contrast, EEEV strains in New York and Massachusetts were characterized by lower genetic diversity, multiple introductions, and shorter local persistence. Our phylogeographic analysis supported a source-sink model in which Florida is the major source of EEEV compared to the other localities sampled. In sum, this study revealed the complex epidemiological dynamics of EEEV in different geographic regions in the United States and provided general insights into the evolution and transmission of other avian mosquito-borne viruses in this region.
