Background: Persons with multiple sclerosis (MS) often experience a decrease in walking performance while simultaneously performing a cognitive task. This decrease in walking performance is termed dual task cost (DTC). Objective: To examine if mobility and cognitive function are correlates of DTC in persons with MS. Methods: Participants were 96 persons with MS who had Expanded Disability Status Scale scores that ranged between 2.0 and 6.5. To determine DTC, participants walked at a self-selected pace with and without a cognitive task while gait velocity was recorded. The effect of the cognitive task was quantified as the percent change in walking velocity between conditions. Participants further completed the timed 25-foot walk (T25FW) and Symbol Digit Modalities Test (SDMT). Centered scores for the T25FW and SDMT, and the product of the center scores, were placed into a linear regression to determine the correlates of DTC. Results: DTC averaged 12.5% (SD = 9.3) and ranged between −14.1 and 42.4%. Performance on the T25FW ranged between 3.1 and 24.5 s with an average of 6.8 s (SD = 3.1 s). SDMT scores ranged between 15 and 79 with an average of 45 items (SD = 12). Regression analysis revealed that age, disability, walking and cognitive performance explained 17% of the variance in DTC. The interaction between walking and cognition did not explain additional variance. Conclusions: Mobility and cognitive impairment were both independent predictors of DTC of walking in persons with MS. This raises the possibility that DTC could be reduced with modifications of either mobility or cognition. [ABSTRACT FROM AUTHOR]

This review focuses on the differential diagnosis and workup of patients presenting with transient vision loss, focusing on several key areas: the relationship to thromboembolic vascular disease, hypercoagulable testing, retinal migraine, and bilateral vision loss. The objective is to provide the ophthalmologist with information on how to best manage these patients. Thromboembolic etiologies for transient vision loss are sometimes managed with medications, but when carotid surgery is indicated, earlier intervention may prevent future stroke. This need for early treatment places the ophthalmologist in the important role of expediting the management process. Hospital admission is recommended in patients presenting with transient symptoms within 72 hours who meet certain high-risk criteria. When the cause is giant cell arteritis, ocular ischemic syndrome, or a cardioembolic source, early management of the underlying condition is equally important. For nonthromboembolic causes of transient vision loss such as retinal migraine or retinal vasospasm, the ophthalmologist can provide reassurance as well as potentially give medications to decrease the frequency of vision loss episodes. The likelihood that the RNFL thickness estimation between physicians, or between physician and OCT, was within 10 µm of each other was 47%–62%. All physicians had disparities in RNFL thickness estimation greater than 30 µm.Conclusion: This study provides information on the ability of an experienced neuro- ophthalmologist to estimate the RNFL thickness based on fundus photos.Keywords: optical coherence tomography, OCT, RNFL, retinal nerve fiber, estimation

Causes of atypical optic neuritis include connective tissue diseases (eg, lupus), vasculitis, sarcoidosis, or neuromyelitis optica. In this manuscript, updated treatment options for both typical and atypical optic neuritis are reviewed. Conventional treatments, such as corticosteroids, therapeutic plasma exchange, and intravenous immunoglobulin therapy are all discussed with commentary regarding evidence-based outcomes. Less commonly used treatments and novel purported therapies for optic neuritis are also reviewed. Special scenarios in the treatment of optic neuritis – pediatric optic neuritis, acute demyelinating encephalomyelitis, and optic neuritis occurring during pregnancy – are specifically examined.Keywords: optic neuritis, optic neuropathy, treatment, neuroophthalmology

Abstract Background Researchers have recently advocated for the 2-minute walk (2MW) as an alternative for the 6-minute walk (6MW) to assess long distance ambulation in persons with multiple sclerosis (MS). This recommendation has not been based on physiological considerations such as the rate of oxygen consumption (V·O2) over the 6MW range. Objective This study examined the pattern of change in V·O2 over the range of the 6MW in a large sample of persons with MS who varied as a function of disability status. Method Ninety-five persons with clinically-definite MS underwent a neurological examination for generating an Expanded Disability Status Scale (EDSS) score, and then completion of the 6MW protocol while wearing a portable metabolic unit and an accelerometer. Results There was a time main effect on V·O2 during the 6MW (p = .0001) such that V·O2 increased significantly every 30 seconds over the first 3 minutes of the 6MW, and then remained stable over the second 3 minutes of the 6MW. This occurred despite no change in cadence across the 6MW (p = .84). Conclusions The pattern of change in V·O2 indicates that there are different metabolic systems providing energy for ambulation during the 6MW in MS subjects and steady state aerobic metabolism is reached during the last 3 minutes of the 6MW. By extension, the first 3 minutes would represent a test of mixed aerobic and anaerobic work, whereas the second 3 minutes would represent a test of aerobic work during walking.

Background: The aim of the study was to determine clinical manifestations and outcome of Listeria monocytogenes meningitis (LM) and to compare with other forms of bacterial meningitis (BM)., Material and Methods: We analyzed records of all adult patients with BM who were hospitalized between January 2010 and December 2017 in the largest neuroinfection center in Poland., Results: Out of 343 analyzed patients with BM 24 were diagnosed to have LM. Patients with LM were older compared to patients with other forms of BM (62 years vs. 57 years, p=0.039), were more likely to have cancer (16.7% vs. 4.7%, p=0.045), receive immunosuppressive treatment (45.8% vs. 10.7%, p<0.001), or be immunocompromised in any way (62.5% vs. 35.5%, p=0.016). Blood tests showed lower WBC (10.7 × 103 cells/µl vs. 15.5 × 103 cells/µl, p=0.004), C-reactive protein (150 mg/L vs. 221 mg/L, p=0,019) and procalcitonin (1.27 ng/mL vs. 3.78 ng/mL, p=0.003) in LM group. Analysis of cerebrospinal fluid showed lower cell count (531.5 cells/µL vs. 1100 cells/µL, p<0.001) and lower chloride (113 mmol/L vs. 117 mmol/L, p=0.036) in patients with LM. In the multiple logistic regression analysis, immunosuppressive therapy was the only variable independently associated with LM (OR:8.72, CI 95%:1.41-64.34, p=0.024)., Conclusions: LM is associated with older age, cancer and immunosuppressive therapy. However, in multivariate analysis only immunosuppressive therapy turned out to be an independent risk factor for LM.

Regardless of their area of specialization, adult psychiatrists are likely to encounter gender-variant patients; however, medical school curricula and psychiatric residency training programs devote little attention to their care. This article aims to assist adult psychiatrists who are not gender specialists in the delivery of respectful, clinically competent, and culturally attuned care to gender-variant patients, including those who identify as transgender or transsexual or meet criteria for the diagnosis of Gender Dysphoria (GD) as defined by The Diagnostic and Statistical Manual of Mental Disorders (5th edition). The article will also be helpful for other mental health professionals. The following areas are addressed: evolution of diagnostic nosology, epidemiology, gender development, and mental health assessment, differential diagnosis, treatment, and referral for gender-affirming somatic treatments of adults with GD., Competing Interests: No author has any conflict of interest to report.

Conversion therapies are any treatments, including individual talk therapy, behavioral (e.g. aversive stimuli), group therapy or milieu (e.g. "retreats or inpatient treatments" relying on all of the above methods) treatments, which attempt to change an individual's sexual orientation from homosexual to heterosexual. However these practices have been repudiated by major mental health organizations because of increasing evidence that they are ineffective and may cause harm to patients and their families who fail to change. At present, California, New Jersey, Oregon, Illinois, Washington, DC, and the Canadian Province of Ontario have passed legislation banning conversion therapy for minors and an increasing number of US States are considering similar bans. In April 2015, the Obama administration also called for a ban on conversion therapies for minors. The growing trend toward banning conversion therapies creates challenges for licensing boards and ethics committees, most of which are unfamiliar with the issues raised by complaints against conversion therapists. This paper reviews the history of conversion therapy practices as well as clinical, ethical and research issues they raise. With this information, state licensing boards, ethics committees and other regulatory bodies will be better able to adjudicate complaints from members of the public who have been exposed to conversion therapies.

Transgender issues and transgender rights have become increasingly a matter of media attention and public policy debates. Reflecting changes in psychiatric perspectives, the diagnosis of "trans-sexualism" first appeared in the International Statistical Classification of Diseases and Related Health Problems in 1975 and shortly thereafter, in 1980, in the Diagnostic and Statistical Manual of Mental Disorders. Since that time, international standards of care have been developed, and today those standards are followed by clinicians across diverse cultures. In many instances, treatment of older adolescents and adults is covered by national health care systems and, in some cases, by private health insurance. Most recently, the Medicare ban on coverage for gender reassignment surgery was lifted in 2014. In contrast to the relative lack of controversy about treating adolescents and adults, there is no expert clinical consensus regarding the treatment of prepubescent children who meet diagnostic criteria for what was referred to in both DSM-IV-TR and ICD-10 as gender identity disorder in children and now in DSM-5 as gender dysphoria. One reason for the differing attitudes has to do with the pervasive nature of gender dysphoria in older adolescents and adults: it rarely desists, and so the treatment of choice is gender or sex reassignment. On the subject of treating children, however, as the World Professional Association for Transgender Health notes in their latest Standards of Care, gender dysphoria in childhood does not inevitably continue into adulthood, and only 6 to 23 percent of boys and 12 to 27 percent of girls treated in gender clinics showed persistence of their gender dysphoria into adulthood. Further, most of the boys' gender dysphoria desisted, and in adulthood, they identified as gay rather than as transgender. Multiple sclerosis (MS) is a disease that results in widespread damage to the nervous system. One consequence of this disease is the emergence of enhanced tremor. This study was designed to (1) compare the tremor responses of persons with MS to that of healthy adults and to (2) examine the impact of whole body position (i.e., seated/standing) on tremor. Bilateral postural tremor was recorded using accelerometers attached to each index finger. Results revealed some similarity of tremor between groups in regard to the principal features (e.g., presence of peaks in similar frequency ranges). However, significant differences were observed with tremor for the MS persons being of greater amplitude, more regular (lower ApEn) and more strongly coupled across limbs compared to the elderly. The effects of body position were consistent across all subjects, with tremor increasing significantly from sitting-to-standing. However, the tremor increase for the MS group was greater than the elderly. Overall, the tremor for MS group was negatively affected by both this disease process and the nature of the task being performed. This latter result indicates that tremor does not simply reflect the feed-forward output of the neuromotor system but that it is influenced by the task constraints., (Copyright © 2012 Elsevier B.V. Purpose of Review: Functional disorders are a major category of vision loss not uncommonly presenting to an ophthalmologist or neuro-ophthalmologist. There are constant advances in the discrimination of causes for functional vision loss, as well as testing and treatment of these patients., Recent Findings: New tests for diagnosing functional vision loss have been reported. These include techniques for evaluating both decreased visual acuity and visual field loss, allowing the physician to objectively state if a patient's vision is better than what he is claiming. Newer imaging modalities such as functional MRI suggest that certain areas of the brain show impaired modulation in persons with conversion disorder. There are few clinical trials for the treatment of persons with functional vision loss, and therefore no conclusive treatment recommendations., Summary: This article discusses the recent advances regarding functional vision loss relevant to the ophthalmologist.

Horizontal conjugate gaze deviation (h-CGD) in acute vestibular syndrome (AVS) may be detected clinically or radiographically. While upright clinical ocular lateropulsion (OL) predicts central lesions, supine radiographic h-CGD does not. We sought to investigate the cause for this discordance by comparing upright to supine OL in AVS. We prospectively recorded clinical and radiographic h-CGD in 17 AVS patients. Horizontal eye position after brief eyelid closure was tested clinically following postural shifts. Radiographic h-CGD was assessed on axial magnetic resonance imaging (MRI) or computerized tomography (CT) images. All maintained central fixation with eyes open in light. OL was present in 8 (7 strokes, one central demyelination) and radiographic h-CGD in 14 (including all 6 with peripheral lesions). OL was unchanged after static postural testing. OL predicts central pathology and does not vary with postural shifts, regardless of lesion location. Endemic pruritus in a family of four proved to be caused by the "bite and run" habits of the Cheyletiella yasguri mites that lived on the family dog. The mystery was solved only after the 6-year-old daughter experienced an explosive attack of pruritic papules after taking a nap in the dog's sleeping box. Virtually invisible ectoparasites remain a common cause of endemic pruritus within families. Since these patients show no acarologic, clinical, or laboratory evidence for the cause fo their itch, a good rule remains, cherchez le chien.
