The mechanisms of these reactions and of the sensitization to GBCAs are still largely unknown. Abstract

Background: Dupilumab, a fully human monoclonal antibody targeting the alpha subunit of IL-4 was recently approved for the treatment of moderate-to-severe atopic dermatitis (AD) in adult patients. Objective: To assess dupilumab effectiveness and safety in adults with moderate-to-severe AD in a real-life Italian multicentre retrospective cohort. Methods: Adult moderate-to-severe AD patients, referring to 39 Italian centers, received dupilumab in the context of a national patient access program. Disease assessment was performed at baseline, after 4 and 16 weeks of treatment using Eczema-Area-and-Severity-Index (EASI) score, itch and sleep numerical-rating-score (itch-NRS, sleep-NRS) and Dermatology-Life-Quality-Index (DLQI). Results: A total of 109 (71 M/38F) patients was studied. There was a significant reduction in EASI score, itch-NRS, sleep-NRS and DLQI from baseline to week 4 and a further significant decline to week 16. EASI 50, EASI75 and EASI90 were achieved by 59.6%, 28.4% and 9.3% of patients at 4 weeks and by 87.2%, 60.6% and 32.4% of them at 16 weeks, respectively. Adverse events were experienced by 19.2% (21/109) of the patients and they were all mild in intensity, being conjunctivitis the most common side effect. Conclusions: Dupilumab significantly improved disease severity, pruritus, sleep loss and quality of life with an acceptable safety profile.

Abstract

We examined effects of exposing female and male mice for 33 weeks to 45% or 60% high fat diet (HFD). Males fed with either diet were more vulnerable than females, displaying higher and faster increase in body weight and more elevated cholesterol and liver enzymes levels. Higher glucose metabolism was revealed by PET in the olfactory bulbs of both sexes. However, males also displayed altered anterior cortex and cerebellum metabolism, accompanied by a more prominent brain inflammation relative to females. Although both sexes displayed reduced transcripts of neuronal and synaptic genes in anterior cortex, only males had decreased protein levels of AMPA and NMDA receptors. Oppositely, to anterior cortex, cerebellum of HFD-exposed mice displayed hypometabolism and transcriptional up-regulation of neuronal and synaptic genes. These results indicate that male brain is more susceptible to metabolic changes induced by HFD and that the anterior cortex versus cerebellum display inverse susceptibility to HFD.

Abstract

At Mt. Etna (Italy), volcano‐tectonic earthquakes produce impressive surface faulting despite their moderate magnitude (M < 5.5), with historically well‐documented ruptures featuring end‐to‐end lengths up to 6–7 km. The 26 December 2018, Mw 5.0 earthquake represents the strongest event of the last 70 years, with ground ruptures extending for 7.5 km along the Fiandaca fault, a partially hidden structure in the volcano's eastern flank. Field data collected by the EMERGEO Working Group (INGV) are here integrated with high‐resolution photogrammetric surveys and geological‐morphological observations to enable a detailed structural analysis and to reconstruct the morphotectonic process of fault growth. The deformation zone develops in a transtensional regime and shows a complex pattern, consisting of brittle structures arranged in en‐échelon scale‐invariant overlapping systems. Offsets and kinematics vary along the strike due to a major bend in the fault trace. We reconstructed a prevailing right‐lateral displacement in the northern section of the fault and a dextral oblique slip in the southern one (max 35 cm); the dip‐slip component increases southward (max 50 cm) and overall resembles the along‐strike pattern of the long‐term morphological throw. The kinematic analysis indicates a quasi‐rigid behavior of the two fault blocks and suggests a geological model of rupture propagation that explains both the location of the seismic asperity in the northern section of the Fiandaca fault and the unclamping in the southern one. These findings are used to propose a conceptual model of the fault, representing insights for local fault‐based seismic hazard assessment. Abstract

Introduction: There is, so far, no universal definition of severe asthma. This definition usually relies on: number of exacerbations, inhaled therapy, need for oral corticosteroids, and respiratory function. The use of such parameters varies in the different definitions used. Thus, according to the parameters chosen, each patient may result in having severe asthma or not. The aim of this study was to evaluate how the choice of a specific definition of severe asthma can change the allocation of patients. Methods: Data collected from the Severe Asthma Network Italy (SANI) registry were analyzed. All the patients included were then reclassified according to the definitions of U-BIOPRED, NICE, WHO, ATS/ERS, GINA, ENFUMOSA, and TENOR. Results: 540 patients, were extracted from the SANI database. We observed that 462 (86%) met the ATS/ERS criteria as well as the GINA criteria, 259 (48%) the U-Biopred, 222 (41%) the NICE, 125 (23%) the WHO, 313 (58%) the Enfumosa, and 251 (46%) the TENOR criteria. The mean eosinophil value were similar in the ATS/ERS, U-Biopred, and Enfumosa (528, 532 and 516 cells/mcl), higher in WHO and Tenor (567 and 570 cells/mcl) and much higher in the NICE classification (624 cells/mcl). Lung function tests resulted similarly in all groups, with WHO (67%) and ATS/ERS-GINA (73%), respectively, showing the lower and upper mean FEV1 values. Conclusions: The present observations clearly evidence the heterogeneity in the distribution of patients when different definitions of severe asthma are used. However, the recent definition of severe asthma, provided by the GINA document, is similar to that indicated in 2014 by ATS/ERS, allowing mirror reclassification of the patients examined. This lack of homogeneity could complicate the access to biological therapies. The definition provided by the GINA document, which reflects what suggested by ATS/ERS, could partially overcome the problem.

Abstract

Background: Dupilumab, a fully human monoclonal antibody targeting the alpha subunit of IL-4 was recently approved for the treatment of moderate-to-severe atopic dermatitis (AD) in adult patients. Objective: To assess dupilumab effectiveness and safety in adults with moderate-to-severe AD in a real-life Italian multicentre retrospective cohort. Methods: Adult moderate-to-severe AD patients, referring to 39 Italian centers, received dupilumab in the context of a national patient access program. Disease assessment was performed at baseline, after 4 and 16 weeks of treatment using Eczema-Area-and-Severity-Index (EASI) score, itch and sleep numerical-rating-score (itch-NRS, sleep-NRS) and Dermatology-Life-Quality-Index (DLQI). Results: A total of 109 (71 M/38F) patients was studied. There was a significant reduction in EASI score, itch-NRS, sleep-NRS and DLQI from baseline to week 4 and a further significant decline to week 16. EASI 50, EASI75 and EASI90 were achieved by 59.6%, 28.4% and 9.3% of patients at 4 weeks and by 87.2%, 60.6% and 32.4% of them at 16 weeks, respectively. Adverse events were experienced by 19.2% (21/109) of the patients and they were all mild in intensity, being conjunctivitis the most common side effect. Conclusions: Dupilumab significantly improved disease severity, pruritus, sleep loss and quality of life with an acceptable safety profile.

Abstract

Gadolinium-based contrast agents (GBCAs) are considered to be safe, although sometimes patients report a hypersensitivity reaction when undergoing magnetic resonance imaging (MRI). The mechanisms of these reactions and of the sensitization to GBCAs are still largely unknown. We describe four cases of patients who experienced immediate adverse reactions to GBCAs with a demonstrated cutaneous hypersensitivity suggesting an IgE-mediated mechanism.

Abstract

Background and aims: Severe asthma is burdened by frequent exacerbations and use of oral corticosteroids (OCS) which worsen patients’ health and increase healthcare spending. Aim of this study was to assess the clinical and economic effect of adding mepolizumab (MEP) for the treatment of these patients. Methods: Patients >18 years old, referred to 8 asthma clinics, starting MEP between May 2017 and December 2018, were enrolled and followed-up for 12 months. Information in the 12 months before mepolizumab were collected retrospectively. The evaluation parameters included: OCS use, number of exacerbations/hospitalizations, concomitant therapies, comorbidity, and annual number of working days lost due to the disease. The primary objective was to compare the annual total cost per patient pre- and post-MEP. Secondary outcomes included rates of exacerbations and number of OCS-dependent patients. Results: 106 patients were enrolled in the study: 46 male, median age 58 years. Mean annual cost pre- and post-MEP (cost of biologic excluded) was €3996 and €1,527, respectively. Total savings due to MEP resulted in €2469 (95%CI 1945–2993), 62% due to exacerbations reduction and 33% due to productivity increase. Such savings could fund about 22% of the total cost of MEP for one year. The introduction of MEP induced a clinical benefit by reducing both OCS-dependent patients (OR = 0.12, 95%CI 0.06–0.23) and exacerbation rate (RR = 0.19, 95%CI 0.15–0.24). Conclusions: Patients with severe eosinophilic asthma experienced a clinical benefit in asthma control adding MEP to standard therapy. Biologic therapy can be, partially, funded by the savings produced by patients’ improvement.

Abstract

The northern Apennines of Italy are characterized by a complex Tertiary tectonic history, where superposed compressional and extensional deformation occurred. In such regions, characterized by low active extensional strain rate, the coseistnic surface ruptures are rare and often a matter of much debate resulting in a difficult connection between 'geological' faults (i.e., faults which can be mapped at the surface) and 'seismological' faults (i.e., faults which actually generate earthquakes). The availability of detailed geological mapping and high-resolution seismological data for the Colfiorito area, struck in 1997 by a sequence of six 5 < [M.sub.w] [less than or equal to] 6 normal faulting earthquakes, allow us to compare and verify the existence of geometric and kinematic correspondence between the mapped geological Quaternary faults and the activated structures. In map view, the earthquakes distribution reflects the fault pattern mapped at the surface, the length of activated and mapped faults is quite similar (7-10 km), the coseismic subsided region imaged by interferometric synthetic aperture radar data, is located in the hanging wall of the mapped normal faults that bound the Quaternary basins. In section view, there is a geotnetric connection between mapped normal faults and the aftershock alignments used to image fault geometry at depth. Comparison of striated fault planes and aftershock focal mechanism solutions show a strong kinematic consistency. This study points out that the Quaternary tectonosedimentary evolution and the present-day geological and geomorphologic setting of the Colfiorito area can be interpreted as the result of repeated, extensional earthquakes, similar to the 1997 events, occurring on NW-SE trending normal faults. Our data also show that the main shocks of the Colfiorito sequence nucleated close to the intersections between the normal faults and the preexisting compressional/transpressional structures, which in 1997 acted as lateral barriers to rupture propagation and consequently constrained the fault size.

Abstract

We provide a database of the coseismic geological surface effects following the Mw 6.5 Norcia earthquake that hit central Italy on 30 October 2016. This was one of the strongest seismic events to occur in Europe in the past thirty years, causing complex surface ruptures over an area of > 400 km 2. The database originated from the collaboration of several European teams (Open EMERGEO Working Group; about 130 researchers) coordinated by the Istituto Nazionale di Geofisica e Vulcanologia. The observations were collected by performing detailed field surveys in the epicentral region in order to describe the geometry and kinematics of surface faulting, and subsequently of landslides and other secondary coseismic effects. The resulting database consists of homogeneous georeferenced records identifying 7323 observation points, each of which contains 18 numeric and string fields of relevant information. This database will impact future earthquake studies focused on modelling of the seismic processes in active extensional settings, updating probabilistic estimates of slip distribution, and assessing the hazard of surface faulting.

Abstract

We present a 1:25,000 scale map of the coseismic surface ruptures following the 30 October 2016 M-w 6.5 Norcia normal-faulting earthquake, central Italy. Detailed rupture mapping is based on almost 11,000 oblique photographs taken from helicopter flights, that has been verified and integrated with field data (>7000 measurements). Thanks to the common efforts of the Open EMERGEO Working Group (130 people, 25 research institutions and universities from Europe), we were able to document a complex surface faulting pattern with a dominant strike of N135 degrees-160 degrees (SW-dipping) and a subordinate strike of N320 degrees-345 degrees (NE-dipping) along about 28km of the active Mt. Vettore-Mt. Bove fault system. Geometric and kinematic characteristics of the rupture were observed and recorded along closely spaced, parallel or subparallel, overlapping or step-like synthetic and antithetic fault splays of the activated fault systems, comprising a total surface rupture length of approximately 46km when all ruptures were considered.

48. 3‐D Deep Electrical Resistivity Tomography of the Major Basin Related to the 2016 Mw 6.5 Central Italy Earthquake Fault.

Abstract

We provide the first 3‐D resistivity image of the Pian Grande di Castelluccio basin, the main Quaternary depocenter in the hangingwall of the Mt.Vettore–Mt. Bove normal fault system (VBFS), responsible for the October 30, 2016 Mw 6.5 Norcia earthquake (central Italy). The subsurface structure of the basin is poorly known, and its relation with the VBFS remains debated. Using the recent Fullwaver technology, we carried out a high‐resolution 2‐D transect crossing the 2016 coseismic ruptures coupled with an extensive 3‐D survey with the aim of: (a) mapping the subsurface of the basin‐bounding splays of the VBFS and the downdip extent of intrabasin faults; (b) imaging the infill and pre‐Quaternary substratum down to ∼1 km depth. The 2‐D resistivity section highlights under the coseismic ruptures a main dip‐slip fault zone with conjugated splays. The 3‐D resistivity model suggests that the basin consists of two depocenters (∼300 and ∼600 m deep, respectively) filled with silty sands and gravels (resistivity <300 Ωm), bounded and cross‐cut by NNE‐, WNW‐, and NNW‐trending faults with throws of ∼200–400 m. We hypothesize that the NNE‐trending system acted during the early basin development, followed by NNW‐trending and currently active splays of the VBFS that overprint pre‐existing structures and locally control the infill architecture. Moreover, beneath the basin we detect a shallow NW‐dipping blind fault. The latter is likely a hangingwall splay of the adjacent regional Mts. Sibillini Thrust, which may have been partly involved in the rupture process of the Norcia mainshock.
