This book presents a critical examination of the development of user involvement within research, and investigates the issues currently preventing a productive integration of Mad knowledges within research and practice. Drawing on social, linguistic and critical theories, it proposes the conditions needed to address the development of Mad epistemologies. The author's unique approach deliberately highlights her own positionality and draws on decades of experience as a service recipient, survivor, activist and researcher to illustrate the structural and symbolic barriers faced. Employing concepts including epistemic injustice, individualization, normalization and structural violence, it suggests a radically new way of articulating ‘what's the matter with us?'In doing so, the book itself goes some way towards enacting the radical challenge to academic and epistemic hierarchies which, it is argued, will be required to further advance mad knowledges and user-led research.Crucially, it demonstrates how this approach can be both methodologically and conceptually rigorous. The main aspects of developmental psychology can be found in Section I; it includes the concept of attachment, which is a central construct of developmental psychology that has gained more and more importance in clinical psychology and psychotherapy over the years. Neuropsychological disorders, which play a central role in many other psychological disorders (e.g.: executive functions, learning difficulties, dyslexia), are addressed in a separate chapter. This is a follow-up to the 2007 review of the literature on online/offline interventions done by Marks, Cavanagh, & Gega (Psychology Press). Consequently, this review includes summaries of research published since 2006 to date. Since publication of that original work, the number of references about online/offline interventions has mushroomed greatly, to the point that many more chapters were added to cover as many references as possible. None are included in the previous literature review. It is a great pleasure for me to write a foreword to thi~ fine work by many dif­ ferent collaborators under the aegis of my friend and one-time colleague in Geneva, Dr. E. James Anthony, because it represents a collective effort toward a goal that today seems very necessary yet difficult to attain. This goal is the synthesis of developmental psychology with all the other aspects of child psychology into a science of ontogenetic development from birth to maturity encompassing three points of view-the biological, the behavioral, and the internalization of the behavioral into mental life. This synthesis is indeed necessary since it is not possible to understand a disorder or a developmental arrest without having a sufficient knowledge of l the ensemble of elements that has brought it about. At each level of development, the personality of the subject attempts to integrate a multiplex system of factors in varying proportion, and without carefully and fully considering this interdigitating whole, it is not easy to disentangle the mechanisms involved in any particular functional disintegration.

This multiauthored textbook is directed to the psychiatric resident and other professionals who are interested in the issues, strategies, and methods of psychiatric research. Although the field of psychiatry has not attained the scientific rigor and clinical sophistication of some of its sister disciplines in the medical arena, considerable progress has been made in the last decade or two, and a full understanding of the types of articles that now appear in such publications as the American Journal of Psychiatry, the Archives of General Psychi­ atry, and the Journal of the American Academy of Child and Adolescent Psychiatry requires a fair amount of knowledge about research design and strategy. Psychopharmacology has certainly had the greatest impact on psy­ chiatric theory and practice since the psychoanalytic discoveries by Freud. Beginning more than 20 years ago, psychopharmacology has become a vigorous interdisciplinary science, and over the last 10 to 15 years has witnessed a tremendous growth both from a scientific and practical point of view. Lithium occupies a unique and central position in modern psycho­ pharmacology for several reasons. The introduction of the lithium ion in 1949 as a psychoactive drug preceded the advent of reserpine or chlorprom­ azine and placed it as the first agent in the modern era of pharmacopsy­ chiatry. In fact, this first report on the use of lithium by an Australian clinician set a pattern to be followed for all other major groups of psychoac­ tive chemical agents. Unlike the now established antipsychotic and antidepressant drugs, however, no reliable predictions based on preclinical pharmacological studies were available about the profile of lithium's clinical activity. This problem of clinical predictability based on current preclinical pharmacological studies is now surfacing with the advent of many newer investigational psychoactive agents. As in the introduction of other therapeutic modalities in medicine, including psychiatry, serendipity was the midwife for lithium. The use of this ion in psychiatry, from its fortuitous introduction by Cade in 1949 to its present-day acceptance as a universal treatment modality in the affective emotional disorders, presents, however, a clear example of a hapless lag between discovery and application.

The five-year process of preparing for the revision of the Diagnostic and Statistical Manual (DSM) has been organized around a series of conferences convened by the American Psychiatric Association, in collaboration with the World Health Organization and the U.S. National Institutes of Health, to address the future of psychiatric diagnosis. Obsessive-Compulsive Spectrum Disorders: Refining the Research Agenda for DSM-V is the fruit of one of those conferences and presents the most academically sound, thought-provoking, and timely papers from the proceedings. As the conference and book demonstrate, recent advances in psychiatric diagnosis suggest a new approach to obsessive-compulsive disorder (OCD) classification: • Research into the pathogenesis of OCD increasingly supports reclassification out of the anxiety disorders and into a separate group of obsessive-compulsive-related disorders (OCRDs). • The relationships among OCRDs may be better defined, delineated, and understood if the current categorical diagnostic approach is supplemented with a dimensional approach which assesses obsessive-compulsive symptom domains.• Obsessive-compulsive disorders are believed to be underdiagnosed in patients who complain of broad symptoms of anxiety, and reclassification of OCD as an OCRD would promote more careful examination of distinct obsessive-compulsive symptoms, yield more accurate diagnosis, and result in more effective treatments.• Reclassification may facilitate future research directions in examining the biological underpinnings of these disorders. Writing as a medium of professional help and healing in the various interventional tiers of self-help, education, promotion, prevention, and psychotherapy, and rehabilitation has expanded exponentially since the introduction of computers and the Internet in the last generation. This volume does three things. Firstly, it brings together research on different types of writing and distance writing that have been, or need to be, used by mental health professionals. Secondly, it critically evaluates the therapeutic effectiveness of these writing practices, such as automatic writing, programmed writing poetry therapy, diaries, expressive writing and more. And thirdly, in addition to evaluating the effectiveness of various writing practices, the volume will examine how research-based writing approaches will influence the delivery of mental health services now and in the future, including the implications of these approaches.

This book first appeared in Germany in 2004. In response to the great amount of interest in the book expressed by colleagues from all over the world, we subsequently decided to produce this English version. We have also taken this opportunity to update the information on the Department of Psychiatry since 1994 to include further developments up to the present day (see Chapter 15). One can look at a hospital from all kinds of different perspectives. For psychiatrists with the daily medical task of dealing with the life histories of their patients, it is understandable that they are interested in the development of their hospital from a historical perspective.
