Abstract

Richter transformation (RT) is defined as developing an aggressive lymphoma in 2–10% of patients suffering from chronic lymphocytic leukemia (CLL). So far, no complex analysis of RT demographics and treatment outcomes has been performed in Poland. Thus, the retrospective analysis of 124 patients with RT from Polish hematology centers was designed. Ninety-nine patients with diffuse large B-cell lymphoma (DLBCL-RT) were identified. The median overall survival (OS) for DLBCL-RT was 17.3 months, while for Hodgkin lymphoma (HL-RT)-21.3 months. In multivariate analysis, the independent factors of worse OS for DLBCL-RT were: prior CLL therapy, ECOG stage ≥2, and elevated serum LDH activity. Patients who proceeded to hematopoietic stem cell transplantation (HSCT) achieved better results. The median OS in allogeneic HSCT recipients was not reached, while in autologous HSCT median OS was 51.3 months. In conclusion, our study represents the largest dataset of patients diagnosed with RT in Poland and confirms its dismal prognosis. [ABSTRACT FROM AUTHOR]

Abstract

Introduction of tyrosine kinase inhibitors (TKI) to the therapy of chronic myeloid leukemia (CML) remains one of the most remarkable achievements in oncology. Five TKI are currently approved to treat patients with CML. While imatinib revolutionized the treatment of BCR/ABL1-positive leukemias and represents first-in-class selective inhibitor of ABL1 kinase, the place of next generation TKI in CML therapy is being a matter of an intensive debate. Dasatinib and nilotinib, both 2nd generations TKI, are approved as first or second line treatment, while bosutinib (2nd generation TKI) and ponatinib (3rdgeneration TKI) are restricted for patients resistant to the former ones. Recent reports, demonstrating unexpected side effects, including serious cardiovascular events, caused by next-generation TKIs, underscored that mechanism of TKI action is not fully understood and require further studies. The aim of the present study was to examine the possible role of different TKI in telomere-mediated chromosomal instability in CML cells. We employed human BCR-ABL1-positive cells (K562), and BCR-ABL1-negative cells (HL60), as well as murine myeloid 32Dcl3 cells with 32Dcl3 BCR-ABL1-positive counterparts. Additionally, CD34+ primary cells isolated from peripheral blood leukocytes of CML patients at various stages of the disease (chronic and blastic phase) were used. Blood samples were taken after informed consent. TKI concentrations were ranging from 1nM to 1uM for each TKI used in this study (imatinib, dasatinib, nilotinib, bosutinib and ponatinib), relevant to concentrations observed in patients in clinics. Upon incubation with different TKI, cells were subjected to micronuclei (MN) generation analysis by fluorescent in-situ hybridization (FISH) with pan-human centromere probe. FISH analysis was applied to centromere visualization in nuclei and micronuclei to reveal the generation of micronuclei (MN) and to determine if centromere fragments are present within MN. Analysis was performed at 24 and 48h time points. In BCR-ABL1-positive cells, highest (3-fold) change in MN frequency was observed after longer (48h) incubation with 100 nM imatinib, as compared to the control growth conditions. Less pronounced increase in MN generation with centromeric signals was observed in cells treated with next-generation TKI (dasatinib, nilotinib, bosutinib and ponatinib), which may indicate that different TKIs may exert different aneugenic effects. Such effects may be BCR-ABL1 kinase-independent, since we have also observed an increase in the frequency of MN in HL60 cells treated with 100 nM imatinib, as compared to control ones. It is widely accepted that genomic instability may be provoked, when the length of telomeres is affected, which, in turn, may induce the generation of micronuclei leading to aneuploidy or chromothripsis. To verify, if observed increase in MN generation after some TKI is caused by dysfunction of telomeres, maintenance of telomeres in BCR-ABL1-positive and negative cells treated with TKI was investigated. Enzymatic activity of telomerase was measured immunoenzymatically, length of telomeres was determined by Southern blotting and expression of TERT and TERC, the subunits of telomerase, were examined by qPCR. No significant changes in the length of telomeres and enzymatic activity of telomerase were observed upon treatment with different TKIs in K562 and HL60 cells as well as in murine leukemic cells. Neither expression of TERT nor TERC was affected by TKI. In conclusion, we postulate that BCR-ABL1- positive cells may activate the alternative lengthening of telomeres pathway and this might be affected by different TKI in a different manner by mechanisms which require additional studies. Abstract

Introduction: Richter syndrome (RS) is defined as development of an aggressive lymphoma with dismal prognosis in 2-10% of patients suffering from chronic lymphocytic leukemia (CLL). The most often diagnosed variant of RS is diffuse large B-cell lymphoma (DLBCL-RS). The treatment options and risk factors of worse outcome for RS still need to be expanded. Aims: To study the characteristic and treatment outcomes of DLBCL-RS patients. Methods: The retrospective analysis of patients with DLBCL-RS treated in hematology centers between 2011 and 2021. Results: Ninety-nine patients with DLBCL-RS were identified. Median time since CLL diagnosis to Richter transformation was 43.0 months (range 0-252.0). Thirty-seven patients transformed during CLL treatment, 56.7% of them were receiving novel agents (Bruton's tyrosine kinase inhibitor or BCL2 inhibitor). Seventy-six patients had RS treated with intensive chemotherapy protocols, mainly R-CHOP-like regimens. Only 42.3% had response to the treatment (25/76 complete responses and 11/76 partial responses). In forty patients RS was primarily resistant to administered treatment, in 47.5% of these patients p53 pathway aberration was noted. Median overall survival (OS) after transformation was 17.3 months (95% CI 11.9-46.8). Significantly lower OS (3.4 months, 95% CI 14.13 -- not reached) was observed in heavily pretreated CLL patients (>3 lines of therapy preceding transformation). Other factors identified as poor OS predictors in univariate analysis were: ECOG ≥2 (p = 0.0014), hemoglobin level <10 g/dL (p = 0.017) and platelet count <100 × 109/L (p = 0.0065). Median progression-free survival (PFS) was 13.5 months (95% CI 9.6-22.7). Better results were achieved for patients who proceeded to autologous hematopoietic stem cell transplantation (auto-HSCT; n = 8) or allogeneic HSCT (allo-HSCT; n = 10). The median OS in auto-HSCT recipients was 46.1 months, in allo-HSCT -- 18.3 months. Conclusions: Our study represents the largest dataset of DLBCL-RS patients in Polish hematology centers and confirms its poor prognosis. Abstract

Clinical resistance to tyrosine kinase inhibitors (TKI) remains a significant problem in the therapy of patients with chronic myeloid leukemia (CML). Although BCR-ABL1-kinase domain mutations are the major cause of resistance to TKI, some patients manifest primary or develop secondary resistance to TKI without detectable BCR-ABL1mutations. We aimed to assess the prevalence of additional gene mutations in a group of 50 patients with primary (n = 26) or secondary resistance (n = 24) to TKI, in most cases to imatinib (n = 49) and in one to dasatinib. We employed ~1000 genes custom target enrichment kit and next-generation sequencing on Illumina platform, as well as Sanger sequencing. In 21 patients, we were able to match and analyze paired samples collected at diagnosis (before treatment) and at the time of resistance and in five other cases we analyzed additional samples collected later during TKI-resistant chronic phase or in blast crisis.

22. The Role of Shelterin Complex and Post-Translational Non-Enzymatic Modificationin Telomere Maintenance in Chronic Myeloid Leukemia

Abstract

The introduction of tyrosine kinase inhibitors(TKI) revolutionized therapy of chronic myeloid leukemia (CML) and gives long-lasting remission in majority of patients. However, a significant proportion of patients develops resistance and are at risk of progression. Genomic instability is one of the key features associated with CML progression and as in majority of human malignancies, reduction in telomere length characterize transition from chronic phase (CML-CP) to the blastic phase (CML-BP). Telomere maintenance depends on telomerase nucleoprotein complex consisting of reverse transcriptase TERT, RNA template TERC and structural proteins, such as dyskerin (DKC1), but also on telomere-associated proteins including but not limited to shelterin complex(TRF1, TRF2, RAP1, TIN2, TPP1, POT1) and other telomeric-associated proteins (such as TEP1 and Tankyrase). The role of shelterin complex and its post-translational non-enzymatic modificationsin BCR/ABL1-mediated genomic instability in CML progression and resistance to TKIs is not fully elucidated, thus we decided to examine the possible role of different components of shelterin complex and its glycation and carbonylation in promoting genomic instability and CML progression. We employed CML CD34+ primary cells isolated from peripheral blood leukocytes or bone marrow of CML patients at different stages of disease: CML-CP, CML-BP and CML-TKIres (TKI-resistant cells). Blood samples were taken after informed consent. The length of telomeres was determined by Southern blotting and PNA-FISH technique. Enzymatic activity of telomerase, as well as protein glycation was measured immunoenzymatically, while expression of subunits of telomerase and shelterin complexes was examined by RT-qPCR and by Western Blot.
