Abstract Recent studies on fibular osteotomy for varus gonarthrosis and possible subsequent biomechanical changes have attracted increasing attention to the topic. Existing studies have focused mainly on proximal fibular osteotomy with short follow-up periods. The aim of this study was to investigate changes in the alignment of the coronal plane of the ankle and knee joints in patients who underwent vascularized fibula graft harvest (VFGH). The evaluation was based on functional outcomes and radiological measurements.In the comparison between the VFGH side and the contralateral side, no significant differences in the knee inclination (KI) or talar inclination (TI) angle, knee medial clear space (K-MCS) or ankle medial clear space (A-MCS) distance were noted. However, a significant difference in the hip knee (HKA) angle was observed between the operated and nonoperated sides (0.3° ± 1.8° and 1.5° ± 1.9°, respectively [p = 0.019]). Statistically significant differences in both the knee society score (KSS) and the AOFAS scores were found between the ipsilateral donor limb and the contralateral healthy limb. Although the contralateral healthy side had better clinical scores than the VFGH side, the outcomes of the VFGH side were still satisfactory or excellent.

Introduction: Knee osteoarthritis commonly occurs in the medial compartment. Arthroscopic debridement eases pain by irrigating debris and inflammatory cytokines. Proximal fibular osteotomy shifts the loading force more laterally from the medial compartment, eliminating pain and delaying degenerative progression. This study aims to evaluate pain relief by combining both procedures in knee osteoarthritis of the medial compartment. Methods: The study was conducted in Nepal Medical College Teaching Hospital from November 2021 to December 2022 after the ethical approval from institutional review committee (IRC Ref. No. 025-078/079). The outcome was assessed by visual analog score and knee society scores, who underwent a combined procedure of arthroscopic debridement and proximal fibular osteotomy during a study period of 12 months. Results: Diagnostic arthroscopy was done in 32 patients, out of which, 27 patients underwent combined procedure with arthroscopic debridement and proximal fibular osteotomy. The mean visual analog score in combined procedure was 6.89 ± 0.93 initially and 3.11 ± 0.69 at 12 months follow up. The mean knee society score in combined procedure was 46.85 ± 6.1 preoperatively and 84.26 ± 8.27 postoperatively at 12 months follow up. Common peroneal nerve neuropraxia was observed in 8 (29.62%) patients, which resolved spontaneously during 6 months of time period. Conclusions: Combined arthroscopic debridement and proximal fibular osteotomy procedure is effective in treating knee osteoarthritis of the medial compartment, as it decreases pain and improves knee society score.

Gonarthrosis is arthrosis of the knee joint, a chronic non-inflammatory disease manifested by progressive destruction of the intra-articular cartilage, accompanied by abnormal formation of the bones form the joint, changes in the synovial membrane and synovial fluid. Gonarthrosis is the most common type of arthrosis. Gonarthrosis can be treated conservatively and operatively. Among well-established surgical options for the treatment of medial gonarthrosis are high tibial osteotomy (HTO), unicompart-mental knee arthroplasty (UKA), and total knee arthroplasty (TKA). Proximal fibular osteotomy (PFO) or superior partial fibulectomy is a relatively recent procedure proposed to reduce knee pain in patients with medial compartment. Our study aims to demonstrate an alternative treatment for gonarthrosis with proximal fibular osteotomy and reduced knee pain in patients with medial compartment osteoarthritis of the knee. At the Department of Orthopedics and Traumatology at J.Z.U "Borka Taleski" Prilep in the period from 2018 to 2021, 14 cases were treated, of which 11 were female and 3 were male. All patients were aged between 62 and 82 years with a mean age of 71.3 years. Patients had a severe degree of gonarthrosis (III/IV) according to Kellgren-Lawrence classification. Arthroscopy was performed in 2 patients. The fibula osteotomy was 7 cm away from the fibular head, with 1 cm resected bone fragment from the fibula. The average duration of the surgery was 30 minutes. Patients were followed up on the 7th day, first month, 3 months and 6 months after surgery. The final evaluation of function was done after 6 months by examining the active and passive movements of the knee joint. In all 14 patients we have excellent results with pain reduction, improvement of movement and quality of life. Proximal fibular osteotomy is an option for medial compartment osteoarthritis of the knee. Current literature is limited to small case series which report good outcomes in pain reduction, including the correction of varus deformity in medial gonarthrosis. Further studies are needed to determine the place of the PFO in the medial gonarthrosis management algorithm before it can be recommended for routine clinical use. [ABSTRACT FROM AUTHOR]

Purpose: This study aimed to investigate postoperative patient satisfaction at mid- to long-term follow-up after proximal fibular osteotomy and to identify risk factors for patient dissatisfaction. Methods: This was a retrospective cross-sectional study that included 252 knees from 160 osteoarthritis (OA) patients who underwent proximal fibular osteotomy with a follow-up of four to eight years. Patients were categorized into a satisfied group (satisfaction score ≥ 20) or a dissatisfied group (satisfaction score < 20) based on the New Knee Society Score (New KSS). Patient demographics, preoperative pain visual analogue scale (VAS) score, preoperative Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score, Kellgren–Lawrence (K-L) grade, preoperative hip-knee-ankle (HKA) angle, and preoperative medial proximal tibial angle (MPTA) were compared between the two groups. Multiple logistic regression analysis was used to identify risk factors for patient dissatisfaction. Results: Of the 203 knees, 130 (64.0%) were satisfied with their results. Multiple logistic regression analysis demonstrated that severe medial OA (K-L grade = IV) was an independent risk factor for patient dissatisfaction after proximal fibular osteotomy (OR 8.334, 95% CI 3.815–18.206, P < 0.001). Conclusion: Our study confirmed that proximal fibular osteotomy was a simple and effective treatment for medial OA patients, and majority of our patients obtained a higher satisfaction rate within mid- to long-term follow-up after surgery. Background: Osteoarthritis (OA) is a polyarticular chronic degenerative disease of multifactorial aetiology, and the most common joint to be afflicted is the knee. Objectives: To evaluate the clinical and functional outcomes of medial compartment OA of the knee treated with PFO with respect to pain, disability, and range of movements. Materials and Methods: The present prospective cohort study consists of 60 patients with medial compartment knee OA treated by PFO of both genders who attended OPD and emergency in the orthopaedic department. The institutional ethical committee approved the study, and written informed consent was obtained from all the participants in the study. Results: There were 36 (60%) females and 24 (40%) males with an M: F ratio of 1:1.5. The mean age of patients was found to be 58.72±8.29 years (P value = 0.26). Patients were followed up for range of motion at 3 months and 6 months. At the time of 3-month follow-up, 9 (15%) patients had a range of motion <80, whereas 21 (35%) patients had a range of motion between 80 and 90. 19 (31.67%) patients had ROM between 90 and 100. 11 (18.33%) patients were found to have ROM greater than 100. At the time of final follow-up at 6 months, 05 (8.33%) patients had a range of motion <80, whereas 21 (35%) patients had a range of motion between 80 and 90. 23 (38.33%) patients had ROM between 90 and 100. 11 (18.33%) patients were found to have ROM greater than 100. Conclusion: PFO is a relatively easy, safe, fast, and effective procedure for the management of medial joint OA of the knee. Introduction Meniscus tear is one of the most common injuries of the knee joint. Medial meniscus tear accounts for 23–31 % of knee joint injuries. The main method of treatment is partial resection. As reported, 13.5 % of patients undergo arthroplasty within 15 years after meniscus resection, what is regarded as an unsatisfactory result. Purpose To evaluate and compare the long-term clinical, functional and radiological results of surgical treatment in patients with medial meniscus tear combined with varus deformity of the tibia and without it. Materials and methods From 2013 to 2020, medial meniscus tear was treated in 245 patients. The patients were divided into 4 groups. Group 1 included patients with a mechanical femorotibial angle (MFTА) from 0º to 3º who had arthroscopic meniscus resection (AMR). In the 2nd group, all patients also underwent AMR, but their MFTА was more than 3º. In group 3, patients with MFTА from 3º to 5º underwent simultaneous AMR and proximal fibular osteotomy (PFO) with the formation of a defect. Group 4 included patients with MFTА more than 5º, who underwent high tibial osteotomy (HTO) and AMR. Changes in the main lines and angles were assessed with telemetry radiography of the lower extremities. Functional results were evaluated before and after surgery using the Lysholm Knee Scoring Scale, 2000 IKDC, KOOS. Results Isolated meniscus resection in patients with MFTА more than 3º without axial correction leads to a gradual increase in the deformity by 1,070 ± 0.50º during the first year after surgery, and by 2.20 ± 0.70º to the fifth year. In groups 1, 3 and 4, there was no statistically significant increase in MFTА during the entire observation period. The clinical treatment results of patients of the four groups in the first three years after surgery did not differ statistically. However, within the period from 3 to 5 years, patients with MFTА greater than 3º who underwent only AMR had poorer clinical results than in the other groups (p < 0.001). Conclusions A personalized approach to the method of surgical treatment for medial meniscus tear based on the MFTА enables to correct the mechanical axis of the lower extremities and MFTА, and thus improve the long-term clinical, functional and radiological results of surgical treatment.

Results: Out of 30 studied cases, there were 18 (60%) females with a M: F ratio of 1:1.5. Fourteen (46.7%) of the participants had right side affected whereas 16 (53.3%) of the participants had left knee OA. The mean age of affected cases was found to be 71.27±9.92 years. The median (IQR) of age (years) was 72.00 (64-80) and the age ranged from 52 to 90 years. Nine patients (30%) were obese (BMI =30) and 13 (43.33%) patients were overweight (BMI =25 but <30). There was a significant reduction in pain at the time of final followup (P<0.0001). Analysis of functional assessment showed that mean JOA scores at the time of final follow-up had significantly improved as compared to JOA scores at the time of presentation. Complications were seen in 4 (13.33%) patients. Conclusion: Proximal femoral osteotomy is effective in relieving pain and improving joint function in patients with medial compartment OA. Introduction: Valgisation high tibial osteotomy (HTO) is a widely performed procedure for patients with medial compartment unicompartmental osteoarthrosis knee with varus malalignment of lower limbs. Over the last two decades, medial open-wedge HTO has been more popular than lateral closed-wedge techniques due to multiple advantages. Recently, isolated proximal fibular osteotomy/fibulectomy (PFO) has been advocated as a treatment for medial tibiofemoral osteoarthrosis in young patients. The role of fibulectomy as an adjunct to medial open-wedge HTO has been scarcely reviewed in the literature. Materials and Methods: This study is a retrospective comparative evaluation of clinical outcomes between patients undergoing PFO + HTO versus HTO alone. Functional outcome of ten patients in either group was assessed by Oxford Knee Score and Western Ontario and McMaster Universities score. Results: Patients undergoing PFO + HTO had a larger degree of angular correction. There were lesser complications related to HTO (no delayed union and nonunion). Furthermore, there was no loss of limb alignment. However, peroneal nerve (transient) palsy was reported in this group. Overall, patients had better pain relief and improved scores in the PFO + HTO group. Conclusion: PFO seems to be beneficial (albeit statistically insignificant in our cohort) in medial open-wedge osteotomy by allowing compression and promoting union at osteotomy site, maintaining limb alignment for longer period, and providing bone graft for osteotomy site. We recommend a larger prospective study to study the benefits of PFO + HTO for correction greater than 10°, osteoporosis, iatrogenic fracture of lateral cortex of tibia, and slope correction osteotomies. Care should be exercised while handling the common peroneal nerve during PFO.

Background: This study aimed to investigate the clinical and radiological progression of the patients who underwent proximal fibular partial excision (PFPE) for medial compartment knee osteoarthritis. Methods: Patients with medial compartment knee osteoarthritis who did not benefit from conservative treatment and accepted the PFPE surgery were involved in this study. Patients with traumatic arthritis, inflammatory arthritis, previous lower extremity fractures, valgus malalignment of the knee, and infection at the planned surgical site, however, were kept apart. Patients were assessed both clinically by Visual Analog Scale (VAS) and American Knee Society Score (AKSS), radiologically by Kellgren Lawrence classification, tibiofemoral angle, and joint-line convergence angle (JLCA). The patients were systematically assessed especially in the sixth month and first, second, and third years of the post-operative period. Results: A total of 35 patients (mean age: 56.5, min: 45, max: 79) were involved in this study. The mean follow-up period was 22.4 months (min: 6, max: 36). It has been observed that: Statistically significant decrease in VAS and increase in AKSS after PFPE (p < 0.001) No statistically significant improvement in preoperative and postoperative tibiofemoral angle and JLCA measurements Conclusions: The PFPE is a safe and effective surgical treatment for pain relief and functional improvement in medial compartment knee osteoarthritis. Background: Osteoarthritis of the knee is a prevalent form of joint disease that can result in debilitating pain and limited movement. Objectives: This study aimed to know the better management of medial compartment osteoarthritis with varus deformity. Patients and Methods: This clinical trial was conducted at Orthopedic Surgery Department, Zagazig University through the period from January 2019 to June 2022. Twenty-four cases of medial compartment knee osteoarthritis were included in this study, all of which had proximal fibular osteotomy (PFO). The mean age of the included cases was 56.6 years. Results: In this study, American Knee Society (AKS) scores showed a significant increase (p < 0.001) from 41.6 ± 5.4 preoperatively up to 80.6 ± 2.6 at 12th month postoperatively. There was significant decrease in visual analogue scale from 8.4 ± 1.6 pre-operation to 1.5 ± 0.8 at 12th month post-operation. Conclusion: Patients with medial compartment knee osteoarthritis often find relief from their symptoms and an increase in their quality of life following a proximal fibular osteotomy. Osteoarthritis causes joint pain and functional disorder, of which knee osteoarthritis is the most common. Nowadays, clinically effective treatments mainly include conservative treatment, arthroplasty, and osteotomy. However, conservative treatment only offers symptomatic relief and arthroplasty is limited to the patients with a moderate to severe degree of osteoarthritis. For relatively young patients who require greater knee preservation, a surgical treatment with low operation trauma and revision rate is needed. Osteotomy around the knee, based on the notion of “knee preservation,” has been chosen as an alternative surgical treatment. Cutting and realigning the bones corrects the mechanical line of lower limb force bearing. As such, osteotomy around the knee retains normal anatomical structure and obtains good functional recovery of the knee joint. The techniques of osteotomy around the knee includes anti‐varus deformity and anti‐valgus deformity osteotomy, aiming to reallocate the force bearing in the compartment of the knee joint. By choosing the surgical section of the lower limbs, the osteotomy around the knee can achieve the correction of mechanical axis, such as the high tibial osteotomy (HTO), proximal fibular osteotomy (PFO), and distal femur osteotomy (DFO). Numerous modified techniques have been developed to meet the demands of patients based on traditional methods. These modified osteotomy have their own advantages and indications. This paper aims to guide clinical treatment by reviewing different types of osteotomies, and their effects, that have been studied and applied widely in clinical practices.

Background: Osteoarthritis of knee is one of the common conditions seen in older individuals and known to affect quality of life considerably. It makes it difficult for individuals with knee osteoarthritis to climb stairs, walk long distances, and stand for long time and squatting. In intractable cases not responding to conservative management surgical management such as proximal fibular Osteotomy (PFO) or high tibial Osteotomy (HTO) is routinely done. We conducted this comparative study to analyse the functional outcome of patients who had undergone HTO and PFO for medial compartment osteoarthritis of knee joint. Aims and Objectives: 1) To analyse the functional outcome of patients who had undergone HTO and PFO for medial compartment osteoarthritis of knee joint. 2) To compare complication rates in both the groups. Materials and Methods: A total of 60 patients with medial compartment knee osteoarthritis were included in this study on the basis of a predefined inclusion and exclusion criteria. Patients were randomized to 2 groups. Thirty patients undergoing proximal fibular osteotomy (Group A) and 30 patients undergoing high tibial osteotomy (Group B). The patients were followed up for improvements in functional outcome for 12 months. The statistical analysis was done using SSPS 21.0 software and p value less than 0.05 were taken as statistically significant. Results: Mean time for surgery was less for PFO surgery as compared to HTO and the difference was statistically significant. Objective To evaluate and compare the clinical and radiographic outcomes of proximal fibular osteotomy (PFO) in treating medial knee osteoarthritis (KOA) patients with upper fibular curvature and non‐curvature. Methods A retrospective cohort study was performed. From January 2016 to January 2017, a total of 51 patients (nine males and 42 females) at a mean age of 63.7 years (range 48–79 years) with medial KOA who underwent PFO procedure at the Third Hospital of Hebei Medical University were included in the study. The patients were divided into the two groups, namely curvature group (28 patients, six males and 22 females, aged 62.6 ± 7.7 years) and non‐curvature group (23 patients, three males and 20 females, aged 64.5 ± 7.6 years). Perioperative parameters and Kellgren‐Lawrence classification were recorded and analyzed in the two groups, respectively. All patients were followed up at 1, 3, 6, and 12 months at the first year of post‐operation, and then every 6 months from the second year of post‐operation. A telephone survey with standard questionnaire survey, including Visual Analog Scale (VAS) score and Hospital for Special Surgery (HSS) scoring system, was used to evaluate postoperative clinical outcomes. Radiological results were assessed using the femorotibial angle (FTA), hip‐knee‐ankle angle (HKA), and settlement value of medial tibial platform (MTP) in the two groups. Results The average follow‐up periods of the curvature group and the non‐curvature group were 34.8 ± 6.1 and 33.9 ± 5.4 months, respectively. There were no significant differences between the two groups of demographic data in terms of number of patients, age, body mass index (BMI), gender, KOA side, and Kellgren‐Lawrence classification (P > 0.05). The VAS scores of the curvature group and non‐curvature group were (3.53 ± 1.62 vs 3.68 ± 1.43 at 1 month, 3.46 ± 0.79 vs 3.57 ± 0.66 at 3 months, and 2.43 ± 0.88 vs 2.83 ± 0.94 at 6 months, both P > 0.05), while significant differences were found from 12 months post‐operation (1.54 ± 0.72 vs 2.03 ± 0.85 at 12 months, and 1.04 ± 0.69 vs 1.74 ± 0.75 at 24 months, both P 0.05), while significant differences were found from 12 months post‐operation (90.64 ± 4.32 vs 87.71 ± 5.63 at 12 months, and 92.93 ± 2.07 vs 90.06 ± 2.08 at 24 months, both P

Aims: Medial compartment arthritis is very common in the Asian population because of a preponderance of varus knees. High tibial osteotomy and total knee replacement are technically demanding procedures, while proximal fibular osteotomy (PFO) significantly relieves pain and improves joint function in knee osteoarthritis (OA). This retrospective study was conducted at Patna Medical College and Hospital. The objective of this study was to assess functional outcomes after PFO and its correlation with bone mineral density. Settings and Design: The study design was a retrospective study. Subjects and Methods: Twenty patients with unicompartmental OA with varus deformity were included in the study. Inclusion criteria included patients with moderate-to-severe symptomatic medial unicompartmental OA of the knee, who had an indication for a surgical procedure, and who were able to give informed consent for the surgery. Exclusion criteria included patients with posttraumatic knee OA or inflammatory joint disease and patients with a history of previous operations or fractures the primary outcome was measured by calculating the ratio of knee joint space. The secondary outcome measure was pain measured by the visual analog scale (VAS). Statistical Analysis Used: Statistical analysis was performed by measurement of study variable. Results: Immediate postoperative pain relieved was noticed in all twenty patients. While at 1-, 3-, and 6-month follow-up, pain relieved was noticed maximum in osteoporotic group patients (n = 12). At 9-month follow-up, pain relieved was noticed equally in all patients. Radiographs of the weight-bearing lower extremity showed an average increase in the medial knee joint space postoperatively compared with preoperatively. The ratio of the knee joint space (medial/lateral compartment) improved significantly from 0.50 ± 0.30 preoperatively to 0.58 ± 0.28 postoperatively. Conclusions: PFO is a simple, safe, fast, and affordable surgery to relieve pain and improve joint function and the medial joint space in knee osteoarthritis, particularly in osteoporotic individuals.

Background: Osteoarthritis (OA) is the most common form of arthritis in Indian population. Proximal fibular osteotomy (PFO) is an alternative treatment to high tibial osteotomy and unicondylar arthroplasty or total knee arthroplasty. It is a surgical procedure for medial compartment knee osteoarthritis (KOA). The PFO helps in the correction of a varus deformity in KOA, which shift the loading force from the medial compartment more laterally. It, therefore, helps in decreasing the pain and satisfactory functional recovery. Objective: The objective of the study is to evaluate the clinical and radiological outcomes in medial compartment OA knee treated with minimally invasive PFO as a new and alternative modality of treatment. Materials and Methods: A total of 30 patients were selected (18 men and 12 women, age range between 50 and 72 years) from May 2019 to May 2020 who had undergone PFO for medial compartment OA knee were followed in a prospective manner. Preoperative and postoperative weight-bearing whole lower extremity scanogram was obtained to analyze the alignment of lower limb (femorotibial angle) and ratio of joint space (medial/lateral joint space). Functional outcome was assessed with American Knee Society Score (KSS), and knee pain was assessed with visual analog scale (VAS). Results: In our study, we had 30 patients who were managed by PFO and were followed up for a minimum period of 1 year. Following the surgery, all patients reported dramatic relief in pain with the VAS dropping significantly from 6.39 in the preoperative period to 2.1 postoperatively (P < 0.005). Weight-bearing lower extremities radiographs showed significant increase in medial knee joint space in 20% of patients. We also noted an increase in the medial joint space from 1.1 ± 0.29 mm preoperatively to 4.21 ± 0.7 min postoperatively. The femorotibial angle improved by around 7°, while the hip knee ankle angle improved by around 6°. Conclusion: PFO is a new alternative method in the management of medial compartment arthritis of the knee, which is minimally invasive, safe, and effective, relieves pain, and improves joint function. It helps in the correction of varus knee.

Proximal fibular osteotomy (PFO) is relatively newer surgery in field of osteoarthritis (OA) knee. Effect of PFO over gait in OA knee patients have not been studied in any known set up yet. Aim of this study was to assess the effect of PFO by instrumented gait analysis in medial compartment OA knee patients. From 2019 to 2020, 22 patients with grade 2 and 3 (Kellgren Lawrence grade) medial compartment OA knee, without having any symptomatic relief by conservative measure, were enrolled for PFO. Pre-operative fitness was obtained before surgery and patients were studied prospectively. After PFO, patients were advised to walk on 2nd post op day onwards and follow up was done at end of 2nd week and 4th week. Patients were assessed by instrumented gait analysis at pre-operative and 4 week post-operative follow up. Most patients had significant changes in observed gait parameters (reduced varum angle peak in stance, reduced step width, increased cadence, reduced first peak appearance time of vertical Ground Reaction Force) after PFO due to biomechanical changes around knee. PFO is an emerging, low cost, safer surgery with lesser hospital stay. Background: Knee osteoarthritis is a significant cause of functional restriction among the Indian population. In the initial stages when patients present with isolated medial compartment changes, the surgical options in the inventory are few. The scarcity of a simple yet cost-effective procedure has contributed to the growing popularity of isolated proximal fibular osteotomy (PFO) since its inception. This study aims to evaluate the outcome among patients who underwent PFO for symptomatic knee osteoarthritis. Materials and Methods: Fifty patients with medial unicompartmental osteoarthritis of the knee were treated with PFO from December 2017 to December 2020. Patient particulars and intra-operative parameters were recorded. The postintervention functional outcome was evaluated by comparing the preoperative Oxford Knee Score (OKS) with that reported postoperatively. Results: The mean age of the patients included in the study was 48.20 years, with a male: Female ratio of 1.8:1. Sixty-eight percent cases presented with Kellgren–Lawrence Stage III. The patients were followed for a mean of 18.18 months after the procedure. The preoperative, 1-month, 2-month, and 6-month OKS were 24.76 ± 2.63, 33.34 ± 3.84, 39.36 ± 4.65, and 43.02 ± 4.66, respectively. At 6 months postsurgery, 82% of patients were in the satisfactory functional score range (40–48). The most commonly observed complication was superficial peroneal nerve palsy. Conclusion: Despite the simple technique, PFO has been associated with good outcomes and lesser complications. Selection of patients and placement of the osteotomy site are crucial for the success of the procedure. The present study revealed that patients irrespective of their age, gender, or stage of the disease would benefit from the procedure.

Abstract Background Many previous studies lack sufficient quantitative evidences about changes in biomechanical properties of the knee in response to proximal fibular osteotomy (PFO). Therefore, the aim of this study was to compare the preoperative and postoperative effects of PFO on mechanical stresses in the knee joint and provide with a biomechanical basis for PFO in the treatment of mild knee osteoarthritis (KOA) with varus deformity. Methods A total of 10 patients suffering mild KOA with varus deformity were enrolled in this study. Their image data from computerized tomography (CT) and magnetic resonance imaging (MRI) were used for finite element models, and PFO models were established. Static structural analysis was carried out using ABAQUS to compare the von Mises stress distribution and values of the maximal von Mises stress of femoral cartilage, meniscuses, tibial cartilages, and tibial plateau before and after surgery. Results The stress distribution in the cortical bone of the tibial plateau showed that stresses were transferred from the anterior medial area to the posterior medial area after PFO. Values of the maximal von Mises stress in femoral cartilage, medial meniscus, medial tibial cartilage, and tibial plateau after surgery were significantly lower than the preoperative values, with statistically significant differences (P < 0.05). Postoperative values of the maximal von Mises stress of lateral meniscus and lateral tibial cartilage were significantly higher than the preoperative ones, with statistically significant differences (P < 0.05). Conclusion PFO could reduce the stresses in the medial compartment of the knee joint with stress pathways transferring from the anterior medial area to the posterior medial area of the tibial plateau. Therefore, PFO is recommended for the treatment of mild KOA with varus deformity featuring favorably pain-relieving effects. Graphical abstract

Knee osteoarthritis (KOA) is a progressive degenerative disease of chronic nature. The mainstay of surgical management for KOA would be total knee arthroplasty. Joint preserving options like High Tibial Osteotomy (HTO) and Proximal Fibular Osteotomy (PFO) have been offered as an inexpensive option by knee preservation surgeons. Current literature on PFO outcomes lack of clarity for specific indications for offering PFO based on degree of severity of KOA. Therefore, this systematic review aims to critically evaluate clinical and radiological outcomes of PFO stratified by severity of KOA. PubMed, Scopus, CINAHL and Google Scholar databases were searched. Eligible studies included those published up till August 2023, with 271 studies obtained. After duplicate removal, title-abstract screening, and a full text screen based on inclusion and exclusion criteria, 11 papers were included. 46 papers were further identified from snowballing of 7 existing systematic reviews, with 2 additional papers subsequently included. 13 included articles analysed 788 knees. Our study found that indications based on KL grading of KOA do not seem to differ in terms of post-operative clinical outcomes (VAS score) and radiological measures also found that hip knee alignment was improved regardless of KL grading of KOA. Additionally, the most common post-operative complication reported was deep peroneal nerve palsy. PFO is a viable knee joint preserving surgery for medial compartment KOA, however given the high risk for complications reported in the literature, surgeons should pay close attention to the neuroanatomical landmarks and techniques to avoid neurovascular injury. The functional outcome was measured by pre- and postoperative VAS, Tegner Lysholm Knee score; X rays were used to measure pre- and postoperative “Medial joint space” and “Femoro-tibial angle (FTA).” Statistical Analysis: All the data were tabulated, and then analyzed with appropriate statistical tools “MedCalc. Chi-square test, test of significance and student paired and unpaired T-test were used. Results: There was a significant improvement in VAS score, Tegner Lysholm score postoperatively as compared to the pre-perative scores. FTA and medial joint space were also increased postoperatively. Conclusions: PFO is a good surgical alternative for medial compartmental osteoarthritis of the knee with an excellent functional outcome. A simple surgical procedure, non-requirement of specialized instruments, and low cost of surgery are some of the advantages for patients who do not want to undergo HTO or total knee replacement (TKR).

Abstract Proximal fibular osteotomy has been proposed as a simple and inexpensive alternative to high-tibial osteotomy and unicondylar knee arthroplasty and may be useful for low-income populations that cannot afford expensive treatment methods. However, there is no consensus existing regarding the mechanism by which it acts nor the outcome of this procedure. This study was performed to analyze the available evidence on the benefits of proximal fibular osteotomy and to understand the possible mechanisms in play. There are various mechanisms that are proposed to individually or collectively contribute to the outcomes of this procedure, and include the theory of non-uniform settlement, the too-many cortices theory, slippage phenomenon, the concept of competition of muscles, dynamic fibular distalization theory and ground reaction vector readjustment theory. The mechanisms have been discussed and future directions in research have been proposed. The current literature, which mostly consists of case series, suggests the usefulness of the procedure in decreasing varus deformity as well as improving symptoms in medial osteoarthritis. However, large randomised controlled trials with long-term follow-up are required to establish the benefits of this procedure over other established treatment methods.

Osteoarthritis causes joint pain and functional disorder, of which knee osteoarthritis is the most common. Nowadays, clinically effective treatments mainly include conservative treatment, arthroplasty, and osteotomy. However, conservative treatment only offers symptomatic relief and arthroplasty is limited to the patients with a moderate to severe degree of osteoarthritis. For relatively young patients who require greater knee preservation, a surgical treatment with low operation trauma and revision rate is needed. Osteotomy around the knee, based on the notion of "knee preservation," has been chosen as an alternative surgical treatment. Cutting and realigning the bones corrects the mechanical line of lower limb force bearing. As such, osteotomy around the knee retains normal anatomical structure and obtains good functional recovery of the knee joint. The techniques of osteotomy around the knee includes anti‐varus deformity and anti‐valgus deformity osteotomy, aiming to reallocate the force bearing in the compartment of the knee joint. By choosing the surgical section of the lower limbs, the osteotomy around the knee can achieve the correction of mechanical axis, such as the high tibial osteotomy (HTO), proximal fibular osteotomy (PFO), and distal femur osteotomy (DFO). Numerous modified techniques have been developed to meet the demands of patients based on traditional methods. These modified osteotomy have their own advantages and indications. Objective: To evaluate and compare the clinical and radiographic outcomes of proximal fibular osteotomy (PFO) in treating medial knee osteoarthritis (KOA) patients with upper fibular curvature and non‐curvature. Methods: A retrospective cohort study was performed. From January 2016 to January 2017, a total of 51 patients (nine males and 42 females) at a mean age of 63.7 years (range 48–79 years) with medial KOA who underwent PFO procedure at the Third Hospital of Hebei Medical University were included in the study. The patients were divided into the two groups, namely curvature group (28 patients, six males and 22 females, aged 62.6 ± 7.7 years) and non‐curvature group (23 patients, three males and 20 females, aged 64.5 ± 7.6 years). Perioperative parameters and Kellgren‐Lawrence classification were recorded and analyzed in the two groups, respectively. All patients were followed up at 1, 3, 6, and 12 months at the first year of post‐operation, and then every 6 months from the second year of post‐operation. A telephone survey with standard questionnaire survey, including Visual Analog Scale (VAS) score and Hospital for Special Surgery (HSS) scoring system, was used to evaluate postoperative clinical outcomes. Radiological results were assessed using the femorotibial angle (FTA), hip‐knee‐ankle angle (HKA), and settlement value of medial tibial platform (MTP) in the two groups. Results: The average follow‐up periods of the curvature group and the non‐curvature group were 34.8 ± 6.1 and 33.9 ± 5.4 months, respectively. There were no significant differences between the two groups of demographic data in terms of number of patients, age, body mass index (BMI), gender, KOA side, and Kellgren‐Lawrence classification (P > 0.05). The VAS scores of the curvature group and non‐curvature group were (3.53 ± 1.62 vs 3.68 ± 1.43 at 1 month, 3.46 ± 0.79 vs 3.57 ± 0.66 at 3 months, and 2.43 ± 0.88 vs 2.83 ± 0.94 at 6 months, both P > 0.05), while significant differences were found from 12 months post‐operation (1.54 ± 0.72 vs 2.03 ± 0.85 at 12 months, and 1.04 ± 0.69 vs 1.74 ± 0.75 at 24 months, both P < 0.05). The HSS scores of the curvature group and non‐curvature group were (79.67 ± 5.14 vs 78.25 ± 6.37 at 1 month, 84.65 ± 3.76 vs 83.18 ± 3.64 at 3 months, and 86.27 ± 3.13 vs 85.49 ± 3.25 at 6 months, both P > 0.05), while significant differences were found from 12 months post‐operation (90.64 ± 4.32 vs 87.71 ± 5.63 at 12 months, and 92.93 ± 2.07 vs 90.06 ± 2.08 at 24 months, both P < 0.05). In addition, the FTA and settlement value of the curvature group were lower than the non‐curvature group (177.18 ± 1.52 vs 178.35 ± 1.86, and 5.29 ± 1.74 vs 6.49 ± 2.09, both P < 0.05) while the HKA were higher than the non‐curvature group (175.32 ± 2.34 vs 173.83 ± 2.64, P < 0.05) at the final follow‐up. Conclusions: Medial KOA patients with upper fibular curvature is an optimal surgical indication for PFO surgery, with the advantages of pain relief, better functional recovery, and alignment correction. Background: Osteoarthritis of knee is one of the common conditions seen in older individuals and known to affect quality of life considerably. It makes it difficult for individuals with knee osteoarthritis to climb stairs, walk long distances, and stand for long time and squatting. In intractable cases not responding to conservative management surgical management such as proximal fibular Osteotomy (PFO) or high tibial Osteotomy (HTO) is routinely done. We conducted this comparative study to analyse the functional outcome of patients who had undergone HTO and PFO for medial compartment osteoarthritis of knee joint. Aims and Objectives: 1) To analyse the functional outcome of patients who had undergone HTO and PFO for medial compartment osteoarthritis of knee joint. 2) To compare complication rates in both the groups. Materials and Methods: A total of 60 patients with medial compartment knee osteoarthritis were included in this study on the basis of a predefined inclusion and exclusion criteria. Patients were randomized to 2 groups. Thirty patients undergoing proximal fibular osteotomy (Group A) and 30 patients undergoing high tibial osteotomy (Group B). The patients were followed up for improvements in functional outcome for 12 months. The statistical analysis was done using SSPS 21.0 software and p value less than 0.05 were taken as statistically significant. Results: Mean time for surgery was less for PFO surgery as compared to HTO and the difference was statistically significant. Moreover mean bleeding amount, drainage volume and time required for full weight bearing was low in PFO group as compared to HTO group and the difference was found to be statistically significant (P<0.05). The mean VAS score at the time of presentation in PFO and HTO groups was found to be 4.9 +/- 1.12 and 4.7 +/- 1.02 respectively. A statistically significant reduction in pain was documented at the time of follow up of 4 weeks in both the groups. The functional outcome and complication rates were comparable in both the groups. Conclusion: Proximal fibular osteotomy is a better option as compared to high tibial osteotomy in terms of mean surgery time, mean bleeding amount, drainage volume and time required for full weight bearing. Introduction: The medial compartment is commonly involved in age-related osteoarthritis knee because weight-bearing axis passes close to the medial condyle and its large surface area. Various treatments have been proposed ranging from conservative to total knee arthroplasty which is an expensive and complex treatment. Recently, proximal fibular osteotomy (PFO) for medial compartment knee osteoarthritis became popular as a treatment modality. Although, the mechanism of PFO is not clear. So, we conducted a prospective study to assess the clinico-radiological outcome of proximal fibular osteotomy in medial compartment osteoarthritis knee. Materials and methods: We conducted a prospective study from November 2017 to November 2019 on 32 (47 knees) patients with degenerative medial compartmental osteoarthritis knee, varus deformity < 15 and Kellgren and Lawrence grade II and grade III (K–L Grade). We recorded the visual analogue score (VAS) and knee society score (KSS) pre-operatively and post-operatively of all patients. We got weight bearing anterio-posterior and lateral radiograph of knee done. We assessed joint space ratio and lateral tibio-femoral angle (FTA) pre-operatively, post-operatively and at each follow-up. Results: Mean age was 48.4 (35–65) years and the mean follow-up was 18 months. Mean duration for unilateral PFO was 37 min and for bilateral, 55 min. The VAS was improved from 7.33 ± 0.72 to 7.13 ± 1.64 at 3 months and remained the same at final follow-up (p > 0.05). The mean pre-operative clinical and functional KSS was statistical insignificantly improved at 3 months and final follow-up (p > 0.05). The mean pre-operative lateral and mean pre-operative joint space ratio was also improved statistically insignificant (p > 0.05). Extensor hallucis longus (EHL) weakness was documented in five knees (10.6%) and paraesthesia was documented in seven (14.8%) knees. Conclusion: Although, PFO is a simple, less invasive and affordable procedure, we could not reproduce the favourable results in medial compartmental osteoarthritis knee. This procedure is also associated with reversible but noticeable complications. So, we would not consider PFO as an alternative option in the treatment of medial compartmental osteoarthritis knee. We present a case of a middle-aged man with KOA, who was managed by combined procedures of the proximal fibular osteotomy, microfracture of the cartilage lesion, and bone marrow aspirate concentrate.

Background: Osteoarthritis of knee is one of the important causes of knee pain in elderly patients and is a debilitating disease. It often leads to varus deformity of knee. Many treatment options are available for this progressive knee joint disorder. Proximal fibular osteotomy (PFO) is a novel yet simple procedure used to alleviate the symptoms of medial compartment knee osteoarthritis. The present study was undertaken to evaluate whether this procedure improves the symptoms, functions and limb alignment in patients with medial compartment knee osteoarthritis. Methods: Following approval by the Institutional Review Board, this prospective study included 42 cases (56 knees) with Kellgren–Lawrence grade II and III medial compartment knee osteoarthritis and underwent proximal fibular osteotomy. Clinical assessment was done by visual analogue scale (VAS) score and The Western Ontario and McMaster universities osteoarthritis Index (WOMAC) score pre-operatively and at 3, 6 and 12 months follow-up for pain and functional improvement. Radiological assessment was done by measuring femoro-tibial angles (FTA) pre-operatively and at 1 year follow-up. Results: The mean age was 58.30 years. There were 30 females and 12 males. The preoperative mean WOMAC score was 87.3, at 3 months follow-up it was 29.4 this was significantly better (p < 0.05) but the improvement was not significant at subsequent follow-up visits. Similarly the VAS scores also showed significant improvement at 3 months, but not at 6 and 12 months follow-up. There was no significant improvement in the femoro-tibial angle (FTA) at 1 year follow-up. Conclusion: The study highlights that both the VAS scores and WOMAC scores were significantly better at 3 months after surgery. However these did not show any significant improvement at 6 and 12 months. It does not change the alignment of knee at one year follow-up. Purpose: The purpose of this study was to establish a mouse model of proximal fibular osteotomy (PFO), and to determine if PFO could delay degeneration of the medial compartment of the knee joint in a mouse model.Methods: An animal model of destabilization of the medial meniscus (DMM) was used to induce post-traumatic knee osteoarthritis (OA). PFO was performed to examine the effectiveness of PFO on protection against medial compartment knee OA. Micro-CT was used to observe osteosclerosis development in the subchondral bone, and Safranin O-fast green staining was used to evaluate the progression of articular cartilage destruction. The condylar-plateau angle (CPA) and anatomical femorotibial angle (aFTA) were measured to determine whether knee alignment was changed after PFO.Results: PFO treatment could decrease osteophyte formation and osteosclerosis development in the subchondral bone, as observed by micro-CT. The value of the ratio of trabecular bone volume to total volume (BV/TV) of DMM+PFO group was lower than that of DMM group. PFO also inhibited the progression of articular cartilage destruction. DMM + PFO group displayed decreased maximal and summed OA scores, as compared with DMM group. Moreover, the change of knee alignment was reduced by PFO, which might be the mechanism of PFO alleviating medial compartment knee OA.Conclusion: Our results indicated that PFO could alleviate medial compartment knee OA in a mouse model. This systematic review aims to assess the management of knee osteoarthritis through proximal fibular osteotomy (PFO) in the Indian population by synthesizing data from various prospective cohort and interventional studies. We seek to provide an overview of the effectiveness and safety of PFO as a treatment modality and offer insights into its potential implications for clinical practice in India. A systematic search strategy was employed, targeting multiple medical databases to identify relevant studies published from 2018 to 2023. Inclusion criteria encompassed studies involving Indian patients with medial compartment knee osteoarthritis and varus deformity who underwent PFO. Data were extracted and evaluated according to the Newcastle-Ottawa Scale for observational studies. Eight studies were included in this review, each displaying varying designs, patient populations, and follow-up duration. The findings consistently indicated that PFO improved pain, knee function, and radiological outcomes, such as knee joint space and tibio-femoral angles. These improvements were generally sustained over several months to a year. The available evidence underscores the potential of PFO as a promising intervention for managing knee osteoarthritis in the Indian population, particularly in patients with medial compartment involvement and varus deformity. While these results are promising, the limitations inherent in the current literature, including study design variations and small sample sizes, necessitate further research with more extensive and diverse patient populations. Meniscal extrusion treatment is a key focus area for research and clinical study of degenerative knee pathology. The contact forces between the meniscus and the bones cause external displacements of the meniscus that are resisted by the circumferential fibres. The main risk factors for the excessive deformation of the meniscus are meniscal root tears, disruption of the circumferential fibres, knee malalignment and high body mass index. Prior research has shown meniscal extrusion to be a crucial determinant of successful procedures in knee osteoarthritis treatment. The characteristics of medial knee osteoarthritis are an increase in varus malalignment and the load transferred through the medial compartment. This is associated with greatest reduction of the joint space width over the medial meniscus body and an increase in contact forces acting on the meniscus, which results in increased extrusion of the medial meniscal body. Relocating the meniscus in its anatomic position restores the correct distribution of contact force across the knee joint, and prevents destruction of the cartilage. Thus, clinical and biomechanical research focus on improving surgical techniques that allow for diminished meniscal extrusion. Proximal fibula osteotomy is a relatively new technique which involves the removal of a 10 mm piece of fibula, 4–10 cm from the fibular head. This technique has been shown to reduce the knee-lever arm, load transferred through the medial compartment and increase the medial joint space width. Background: knee O.A was treated by total knee arthroplasty but it is new procedure still under trial for treatment of medial compartment knee O.A. Objective: To assess the early results of proximal fibular osteotomy as a new surgery for pain relief and improvement of functions of the knee in patients with medial compartment OA. Patients and methods: From July 2016 to September 2018, 10 patients who underwent proximal fibular osteotomy for medial compartment osteoarthritis. Preoperative and postoperative knee society score (KSS) and visual analogue scale (VAS) were obtained to assess knee functions and pain Results: median knee society score (KSS) show improvement postoperatively from 83 to 105 and visual analogue scale (VAS) decreased postoperatively from 8.5 to 4. Conclusions: This study shows that proximal fibular osteotomy (PFO), a new surgery, can effectively relieves pain and improves knee function in patients with medial compartment osteoarthritis. Background and objective Osteoarthritis (OA) is a polyarticular disease that most commonly afflicts the knee joint. Established operative treatment options for medial joint OA of the knee include high tibial osteotomy, unicompartmental knee arthroplasty, and total knee arthroplasty. Proximal fibular osteotomy (PFO) is a relatively new procedure for treating medial joint OA of the knee. The objective of this study was to describe the functional and radiological outcomes at one year in patients undergoing PFO for medial joint OA of the knee. Materials and methods The study included 21 patients with medial joint OA of the knee who underwent PFO. Visual analog scale (VAS) score, medial to lateral knee joint space ratio (ML ratio), Kellgren-Lawrence (KL) grade, and the American Knee Society Score (AKSS) (clinical and functional) were recorded preoperatively. VAS score, ML ratio, and AKSS (clinical and functional) were documented again at the three-month and one-year follow-ups. Results The mean age of the patients was 58.85 ±6.94 years; 12 (57.1%) were female and nine (42.9%) were males. The mean VAS score for pain decreased from 7.86 ±0.66 at baseline to 5.14 ±1.15 at three months (p<0.001) and 3.78 ±1.26 at one year (p<0.001). The mean clinical AKSS was 56.49 ±6.95 at baseline, which increased to 63.41 ±6.20 at three months (p<0.001) and 72.71 ±9.87 at one year (p<0.001). The mean functional AKSS at baseline was 48.24 ±14.31, which increased to 60.10 ±14.81 at three months (p<0.001) and 71.46 ±15.18 at one year (p<0.001). The mean ML ratio at baseline was 0.33 ±0.19, which increased to 0.41 ±0.20 at three months (p<0.01) and 0.51 ±0.22 at one year (p<0.001).
