The aim of this study was to assess nationwide trends in the exposure of dental students to periodontal and implant surgical procedures and to evaluate the potential factors that influence these clinical experiences. A 19-item questionnaire was sent to all 64 directors of predoctoral periodontics in U.S. dental schools. In addition to gathering information on the percentage of students who perform surgical periodontics or implant placement procedures in each program, data on student research groups, periodontics residency programs, and periodontics faculty practices were collected. A total of 33 responses were received, for a response rate of 51.5%. Among the responding institutions, 97% and 45.5% allowed dental students to perform periodontal and dental implant placement surgeries, respectively, although only 26.4% and 15.2% of the dental class ended up performing periodontal and dental implant placement surgeries, respectively. Crown lengthening was the most commonly reported (84.8%) periodontal surgical procedure performed by dental students. A negative correlation was found between the presence of a periodontics residency program and dental students' placing dental implants, while the size of the residency program positively correlated with dental students' placing dental implants. Overall, a wide variation in the exposure of dental students to periodontal and dental implant placement surgical experiences was found. Future surveys should assess clinical procedures performed in other special-ties to gain a broader picture of the experience students are gaining in these areas.

The aim of this study was to analyze information about advanced education programs in prosthodontics (AEPPs) collected in American Dental Association (ADA) surveys on advanced dental education from 2006-07 to 2016-17. Data recorded included number of AEPPs and information on program directors, applicants, and enrollees in AEPPs. The results showed that, from 2006 to 2016, there was an increase in the number of AEPPs and enrollment of prosthodontic residents, and the number of applicants per program more than doubled. Despite these increases, steps are needed to increase the number of underrepresented minority residents in prosthodontics. Also, a periodic survey of prosthodontic residents to identify their goals, experiences during training, and career plans could benefit the specialty by providing more insight into the future prosthodontic workforce.

Purpose: The purpose of this questionnaire was to ask general dentists in private practice in the state of Iowa about the extent and scope of their prosthodontic practice., Materials and Methods: A 22-item questionnaire was developed and tested on 5 general dentists. The Iowa Dental Association agreed to electronically distribute the survey to all general practice dentists in their database. After three rounds, a total of 289 responses were received from the 996 general dentists in the database., Results: The average age of the respondents was 50.3 ± 13.2 years (range 28 to 78 years), and 56.3% were in solo practice. The respondents stated that 68.1% had made at least one set of complete dentures and 88.9% had made a removable partial denture (RPD) in the last 3 months, while 76.4% had restored a least one implant in the last 3 months and 11.6% had surgically placed one. Nearly 20% of the respondents had a digital impression scanner in their office. There was a statistically significant difference between the dentists who had made complete dentures in the past 3 months and those who had not with regard to age, gender, and years in practice (p < 0.05 for all instances). No significant difference was found between the general dentists who had made an RPD in the past 3 months and those who had not. Compared to their counterparts, general dentists who had an AEGD or GPR training (p = 0.0312), whose primary practices were in cities of 50,000+ (p = 0.0065), or had a digital scanner (p = 0.0062) and a CAD/CAM milling machine (p = 0.0504) in their office were more likely to have restored an implant in the last 3 months. Furthermore, the general dentists who had surgically placed an implant in the last 3 months were more likely to be male (p = 0.0301) or have a digital impression scanner (p < 0.0001) and/or a CAD/CAM milling machine (p = 0.0007) in their office., Conclusions: The majority of general dentists in this survey are still making complete and partial removable dentures, and a majority is using implants, while only a minority is surgically placing them. PRACTICAL  IMPLICATIONS: Although Iowa general dentists are carrying out sophisticated procedures such as implant placement and restoring implants, as well as using digital technology, there is still a need for removable prosthodontic services in their practices. The aims of this study were to measure the research productivity of directors of U.S. advanced education in prosthodontics programs (AEPP) and to identify associations between the directors' publication metrics and professional characteristics. A list of AEPP directors was obtained from the American College of Prosthodontists website in December 2015. Information on gender, academic rank, and highest degree was collected from the institutional website for each individual. Citation databases (Scopus and Google Scholar) were searched for each director's h-index and i10-index (both indexes are based on numbers of citations of the author's articles) and total numbers of publications and citations. The search identified 50 AEPP directors. The majority were male and had a primary appointment at a university. Most held the rank of associate professor or professor and held both DDS and MS degrees. The mean h-index and i10-index of all directors were 6.32±6.97 and 6.84±10.77, respectively. Their mean numbers of publications and citations were 24.60±31.21 and 288.40±625.97, respectively. The analysis showed that the program directors affiliated with a university had significantly higher mean values for all indexes than those of non-university-affiliated program directors. Professors had productivity metrics significantly higher than those of other ranks in all measures of research productivity.

Objectives: A systematic review of randomised and non-randomised controlled trials was conducted to evaluate studies of the effectiveness of different tooth replacement strategies in adult patients with shortened dental arches. The objectives of the review were to determine the survival rates of different prosthodontic interventions, the risk of tooth loss with and without prosthodontic interventions, and the impact of different tooth replacement strategies on oral-health related quality of life (OHRQoL)., Methods: The protocol was registered with the International Prospective Register of Systematic Reviews (PROSPERO CRD42017064851), and the review was conducted in accordance with the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA)., Results: The search strategy identified 112 potentially relevant publications; 22 from Medline (OVID), 54 from EMBASE (OVID), 35 from CENTRAL, one from the authors' knowledge of the subject area, and none from OpenSIGLE. Ten articles were included in this systematic review. Of these, four were analyses of different outcomes from a multicentre randomized controlled trial in Germany, whilst one study was the pilot phase for this trial. Two further randomized controlled trials were included from the United Kingdom and Republic of Ireland. The remaining articles were reports of prospective cohort studies from Denmark and the Netherlands., Conclusions: there is currently insufficient evidence to recommend one tooth replacement strategy over another in adult patients with reduced dentitions., Clinical Significance: There is a need for further research as there are insufficient numbers of good quality randomised controlled trials currently available. Authors should be encouraged to adhere to CONSORT guidelines for randomized controlled trials, and report findings in such a way that facilitates future meta-analysis., (Copyright © 2018 Elsevier Ltd. Objective: To estimate responsibility loadings for a comprehensive list of dental services, providing a standardized unit of clinical work effort., Methods: Dentists (n = 2500) randomly sampled from the Australian Dental Association membership (2011) were randomly assigned to one of 25 panels. Panels were surveyed by questionnaires eliciting responsibility loadings for eight common dental services (core items) and approximately 12 other items unique to that questionnaire. In total, loadings were elicited for 299 items listed in the Australian Dental Schedule 9th Edition. Data were weighted to reflect the age and sex distribution of the workforce. To assess reliability, regression models assessed differences in core item loadings by panel assignment. Estimated loadings were described by reporting the median and mean., Results: Response rate was 37%. Panel composition did not vary by practitioner characteristics. Core item loadings did not vary by panel assignment. Oral surgery and endodontic service areas had the highest proportion (91%) of services with median loadings ≥1.5, followed by prosthodontics (78%), periodontics (76%), orthodontics (63%), restorative (62%) and diagnostic services (31%). The aim of this retrospective study was to quantify differences between general dentists and prosthodontists regarding appointments, treatment time, impressions, and preoperative diagnostic data in teaching predoctoral clinical fixed prosthodontics. Electronic dental records (n=356) of patients treated at one dental school in academic year 2012 were randomly selected for review to obtain the following data: faculty and student demographics, number of appointments and treatment time from preparation to cementation, number of impressions made, completion of oral disease control treatment (ODCT), and presence of preoperative periapical radiographs and diagnostic casts. The results showed that ODCT was completed in 78%, preoperative radiographs were present in 76%, and diagnostic casts made in 53% of the cases reviewed. There was no statistically significant difference in number of appointments, treatment time, or number of final impressions when students were staffed by general dentists or prosthodontists. When students were supervised by multiple faculty members, there was generally an increase in treatment time and number of appointments and final impressions. Although this study found no statistically significant differences between general dentists and prosthodontists regarding the criteria evaluated, the results suggest that faculty development and calibration are needed to ensure ODCT is completed and preoperative radiographs are present prior to initiating fixed prosthodontic procedures.

How dental patients are affected by oral conditions can be described with the concept of oral health-related quality of life (OHRQoL). This concept intends to make the patient experience measurable. OHRQoL is multidimensional, and Oral Function, Oro-facial Pain, Oro-facial Appearance and Psychosocial Impact were suggested as its four dimensions and consequently four scores are needed for comprehensive OHRQoL assessment. When only the presence of dimensional impact is measured, a pattern of affected OHRQoL dimensions would describe in a simple way how oral conditions influence the individual. By determining which patterns of impact on OHRQoL dimensions exist in prosthodontic patients and general population subjects, we aimed to identify in which combinations oral conditions' functional, painful, aesthetical and psychosocial impact occurs. Data came from the Dimensions of OHRQoL Project with Oral Health Impact Profile (OHIP)-49 data from 6349 general population subjects and 2999 prosthodontic patients in the Learning Sample (N = 5173) and the Validation Sample (N = 5022). We hypothesised that all 16 patterns of OHRQoL dimensions should occur in these individuals who suffered mainly from tooth loss, its causes and consequences. A dimension was considered impaired when at least one item in the dimension was affected frequently. The 16 possible patterns of impaired OHRQoL dimensions were found in patients and general population subjects in both Learning and Validation Samples. Purpose: The aim of this study was to investigate missing teeth and prosthodontic replacements in an institutionalized elderly population in China, and to assess dental functionality before and after the prosthodontic replacements based on a hierarchical dental functional classification (HDFC) system., Materials and Methods: A sample of 512 participants (9% aged 60-69 years; 29% aged 70-79 years; 62% aged ≥ 80 years) living in eight nursing homes in Qingdao were categorized by the HDFC with and without tooth replacements. A functional dentition in the HDFC meets all the following criteria: ≥ 10 natural teeth in each jaw; a complete anterior region; 3 or 4 posterior occluding pairs (POPs) in the premolar regions; and ≥ 1 POP bilaterally in the molar region. Participants with tooth replacements were reclassified. The score for effectiveness of replacements were as follows: 4 for ≥ 10 teeth in each jaw; 3 for a complete anterior region; 2 for 3 or 4 POPs in premolar regions; and 1 for ≥ 1 molar POP bilaterally., Results: Twenty percent of the participants had functional dentitions, but 42% met none of the functional criteria. In the branch '≥ 10 teeth in each jaw' the mean number of teeth present was 26.3 ± 2.9, and the mean number of POPs was 6.2 ± 2.0. In the branch '< 10 teeth in each jaw,' there were 13.4 ± 5.5 teeth and 1.1 ± 1.5 POPs. Of the 384 participants with natural teeth in each jaw, 44% had no replacements and 56% had fixed dental prostheses (FDPs), partial removable dental prostheses (PRDPs), or both. FDPs usually replaced 1 or 2 teeth, and PRDPs 3 or more teeth. On the basis of natural teeth plus artificial teeth (214 participants with replacements), 46% had functional dentitions while 18% met none of the functional criteria. In the promoted participants, the mean number of teeth added by FDPs was 3.6 ± 2.5, and for PRDPs it was 11.9 ± 6.5. FDPs had a significantly higher mean promotion value per tooth added than PRDPs., Conclusion: Approximately half the tooth replacements effectively met all criteria for a functional dentition.

Purpose: The objective of this study was to assess the risk of bias of randomized controlled trials (RCTs) published in prosthodontic and implant dentistry journals., Materials and Methods: The last 30 issues of 9 journals in the field of prosthodontic and implant dentistry (Clinical Implant Dentistry and Related Research, Clinical Oral Implants Research, Implant Dentistry, International Journal of Oral & Maxillofacial Implants, International Journal of Periodontics and Restorative Dentistry, International Journal of Prosthodontics, Journal of Dentistry, Journal of Oral Rehabilitation, and Journal of Prosthetic Dentistry) were hand-searched for RCTs. Risk of bias was assessed using the Cochrane Collaboration's risk of bias tool and analyzed descriptively., Results: From the 3,667 articles screened, a total of 147 RCTs were identified and included. The number of published RCTs increased with time. The overall distribution of a high risk of bias assessment varied across the domains of the Cochrane risk of bias tool: 8% for random sequence generation, 18% for allocation concealment, 41% for masking, 47% for blinding of outcome assessment, 7% for incomplete outcome data, 12% for selective reporting, and 41% for other biases., Conclusion: The distribution of high risk of bias for RCTs published in the selected prosthodontic and implant dentistry journals varied among journals and ranged from 8% to 47%, which can be considered as substantial.

A search of the medical and dental records at the Matthew J. Ryan Veterinary Hospital of the University of Pennsylvania was conducted to identify dogs that received full or partial prosthodontic crowns over a 13-year period (2000-2012). Forty-one dogs with a total of 68 prosthodontic crowns were identified. Further criteria for acceptance into the study included presence of complete medical records containing adequate information pertaining to the procedures performed, and current follow-up either by telephone, electronic mail, or via in-person examination. Treatment was considered to be successful when the prosthodontic crown was in place and there was no further structural injury to the tooth upon making contact with the client or at the time of death of the dog confirmed by the client. The mean number of days the prosthodontic crowns remained in place without further injury to the tooth was 1,598 (range, 161-4, 464 days [median, 1,414 days], standard deviation 1,093 days). Bond failure between the cement and the tooth or the cement and the prosthodontic crown occurred in 3 cases (4.4%). Fracture of the prosthodontically treated tooth occurred in an additional 7 cases (10.3%). Treatment was classified as successful in 58 cases (85.3%). The results suggest that prosthodontic crown therapy is a successful, practical, and durable treatment option for protection of previously injured teeth in pet and working dogs.

Purpose: This survey aimed to review how scientific articles were reported and to describe the types of statistical tests that had been recently and commonly used in The International Journal of Prosthodontics., Materials and Methods: All 174 articles published in 2012 and 2013 were hand-searched to identify scientific articles (n = 151) and those using at least one statistical test to explore results (n = 111). Editorials, letters, comments, erratum, and award proceedings were excluded. The number and type of statistical tests used within articles were collated, and the 10 commonly used methods were identified and described., Results: Of the 151 scientific articles, 76% (n = 111) used at least one statistical test and 24% (n = 40) used qualitative methods. Up to 10 tests were used per article, with 237 in total, of which 36 were unique. The 10 most commonly used tests were analysis of variance (ANOVA; n = 34), survival analyses (n = 29), Student t test (n = 19), chi-square (n = 19), Mann-Whitney U (n = 14), logistic regression (n = 13), Wilcoxon signed rank (n = 12), Fisher exact (n = 11), log-rank (n = 10), and Cox proportional hazards (n = 8), and they accounted for 71% (n = 169) of all tests used., Conclusions: The vast majority of articles published in recent years in The International Journal of Prosthodontics employed statistical analyses. Across 2 years, nearly 250 tests were completed, including 36 unique tests. Statistical test use was common but diverse.

Purpose: Confidence intervals (CIs) are integral to the interpretation of the precision and clinical relevance of research findings. The aim of this study was to ascertain the frequency of reporting of CIs in leading prosthodontic and dental implantology journals and to explore possible factors associated with improved reporting., Materials and Methods: Thirty issues of nine journals in prosthodontics and implant dentistry were accessed, covering the years 2005 to 2012: The Journal of Prosthetic Dentistry, Journal of Oral Rehabilitation, The International Journal of Prosthodontics, The International Journal of Periodontics & Restorative Dentistry, Clinical Oral Implants Research, Clinical Implant Dentistry and Related Research, The International Journal of Oral & Maxillofacial Implants, Implant Dentistry, and Journal of Dentistry. Articles were screened and the reporting of CIs and P values recorded. Other information including study design, region of authorship, involvement of methodologists, and ethical approval was also obtained. Univariable and multivariable logistic regression was used to identify characteristics associated with reporting of CIs., Results: Interrater agreement for the data extraction performed was excellent (kappa = 0.88; 95% CI: 0.87 to 0.89). CI reporting was limited, with mean reporting across journals of 14%. CI reporting was associated with journal type, study design, and involvement of a methodologist or statistician., Conclusions: Reporting of CI in implant dentistry and prosthodontic journals requires improvement. Improved reporting will aid appraisal of the clinical relevance of research findings by providing a range of values within which the effect size lies, thus giving the end user the opportunity to interpret the results in relation to clinical practice.

A board certification system for prosthodontic specialists was established in 2005 by the Japan Prosthodontic Society (JPS), significantly later than the system established by the American Board of Prosthodontics in 1947. The purpose of this study is to outline the certification system for prosthodontic specialists in Japan and discuss and evaluate its current status. In 2012, the number of board certified prosthodontic specialists was 1150 and that of mentorial specialists was 693. The number of board certified institutions was 78 and that of certified adjunct institutions was 23. Although the history of the certification system is not very long, we conclude that a well-organized system has been developed. The annual turnover of dental school faculty creates a varying number of vacant budgeted positions from year to year. The American Dental Education Association (ADEA) conducts an annual survey to determine the status and characteristics of these vacant faculty positions. The number of vacant budgeted faculty positions in U.S. dental schools increased throughout the 1990s, with a peak of 417 positions in 2005-06. Since that time, there has been a decrease in the number of estimated vacancies, falling to 227 in 2010-11. The 2008-09 to 2010-11 faculty vacancy surveys explored these decreases, along with information relevant to the number and characteristics of dental faculty vacancies, including data on the distribution of full-time, part-time, and volunteer faculty, reasons for faculty separations, and sources of new faculty.

Purpose: The purpose of this article is to review data and results from past surveys of prosthodontists sponsored and conducted by the American College of Prosthodontists. Surveys were conducted in 2002, 2005, 2008, and 2011. Selected survey results are examined for prosthodontists in private practice., Materials and Methods: Results from past surveys of prosthodontists were statistically examined and used to estimate several characteristics of the current population of practicing prosthodontists. The selected characteristics included age, gender, number of patient visits, hours in the practice, employment of staff, referral sources, and financial conditions (e.g., gross receipts, expenses in the practice, and net income of prosthodontists). While the most recent survey was conducted in 2011, the results reported by respondents are for the previous year, 2010., Results: The average age of a private practicing prosthodontist in 2010 reached 53 years; 26 years since graduation from dental school and 20 years since completion of residency; an average of 13 years in their current practice. Sixty percent were in solo practice. The mean number of hours per week in the practice was 35 hours, and practicing prosthodontists treated an average of 35 patient visits per week. The patient was the single largest source of referrals, while general practitioners were a close second. The largest percentage of time spent treating patients was for fixed prosthodontics (21%), which declined from a mean of 24.1% in 2007. The mean amount of gross billings in 2010 was $721,970, which was a decline from 2007. Average total practice expenses were $538,230, and the mean net earnings of prosthodontists in private practice were $238,010., Conclusion: Changes have occurred since the last survey of prosthodontists in 2008 (with results for the year 2007). The aim of this study was to investigate the production of mandibular implant-supported fixed and removable prostheses among prosthodontic specialists in Sweden and to compare the results with findings in a similar study made in 2001 (17). Questionnaires regarding treatment with mandibular implant prostheses during 20011 were mailed to all specialists in prosthodontics in Sweden (n = 156, according to available data). Of the 156 questionnaires, 129 (83%) were returned and of those 114 were completed. The reported number of treatments with mandibular implant-supported prostheses varied much among the specialists. Fixed implant prostheses were more common than overdentures (means 11 and 3, median values 8 and 2, respectively). However, the range was large for both alternatives. Ten (9%) of the specialists reported no treatment with fixed implant prostheses while 29 (25%) had not made any implant overdenture during 2011. The most common anchorage system for overdentures in 2011 (as well as in 2001) was two un-splinted implants with ball attachments or Locator abutments. The most common reasons for choosing overdenture treatment instead of a fixed implant prosthesis in 2011 were the reduced cost and the patient's main wish to improve denture retention. A majority of the prosthodontists (58%) reported that patients with implant overdentures were as satisfied as those with fixed implant-supported prostheses, whereas 40% claimed they were less satisfied. Two respondents (2%) considered that overdenture patients were more satisfied than those with a fixed prosthesis. It can be concluded that the general attitude among Swedish prosthodontists towards implant overdentures has not changed much during the 10-year period between the present and the previous investigation.An overdenture is still a seldom-used option in implant treatment of patients with edentulous mandibles in Sweden. Instead, a fixed implant-supported prosthesis continues to be the preferred option,

Cleft palate patients are not usually seen in general dental clinics, but this congenital anomaly is one of the most frequent of cases. General dental practitioners are usually unwilling or/and not sufficiently trained to treat such patients for whom rehabilitation and interdisciplinary cooperation is often needed. The aim of this study was to determine the incidence of prosthetic modalities most frequently used by licensed prosthodontists for prosthetic rehabilitation of cleft palate patients. Participants in this study were 56 cleft palate patients (aged 23-66 years) who received prosthetic treatment between 2000 and 2010. Patients' dental status and prosthetic modalities used were noted from patient records archived at Department of Prosthodontics, School of Dental Medicine, University of Zagreb. Data analyses revealed that combined prosthetic constructions (fixed + removable, p < 0.05) were the most frequently used. In a group of molar teeth, the most frequent fixed prosthetic modalities were crowns with rests; in a group of premolar teeth, telescopic crowns; on canines, metal ceramic crowns and telescopic crowns; and in a group of incisors, metal ceramic pontics (p < 0.05). Understanding the distribution of prosthetic modalities for cleft palate patients could serve to guide dental practitioners towards planning adequate prosthetic treatment for their patients since only a well-planned prosthetic therapy will result in satisfactory function and alleviation of the deformities.

Purpose: To develop and validate an instrument for the assessment of patient-based measures of process-related quality of care in prosthodontic patients., Materials and Methods: In this nonrandomized study, the new Burdens in Prosthetic Dentistry Questionnaire (BiPD-Q) was developed in two steps using a total of 128 prosthodontic patients in a mixed-method approach, combining quantitative-qualitative methodologies. First, the item pool for the instrument was created using semistructured interviews and a group of experts in prosthodontics. This resulted in a preliminary version of the questionnaire. Second, an assessment of redundancy, completion rates, face validity, difficulty, and distribution of the core set of the items was performed. The final version of the BiPD-Q had psychometric core properties (reliability and validity) evaluated., Results: The BiPD-Q consisted of 25 items. Reliability was satisfactory (Cronbach's alpha = .87). The mean score of all items of the BiPD-Q was significantly correlated with mean perceived burdens during treatment as rated by the clinician (r = 0.26; P < .01) and with overall satisfaction with the treatment procedures as rated by patients (r = .31; P < .01), indicating sufficient convergent validity., Conclusion: A reliable and valid instrument for the assessment of patient-based process-related quality of care in prosthodontics has been developed. The BiPD-Q allows comparisons of different dental procedures within a treatment course and of different treatment providers. The use of this type of questionnaire appears to be a valuable tool for dental health care research. The outcomes of research using the BiPD-Q may result in a more pleasant treatment experience for future patients.

Objective: Musculoskeletal disorders (MSDs) are commonly experienced in dentistry. The objective of this study is to determine the prevalence of ergonomics and MSDs among dental professionals., Materials and Methods: A cross-sectional survey was conducted among 170 dentists of different specialties. The questionnaire gathered information regarding demographic details, MSDs, work duration, working status, awareness of ergonomics, etc. Data was analyzed using SPSS version 15.0. Student's t-test and analysis of variance (ANOVA) test was used for comparison in mean scores. Stepwise multiple linear regression analysis was used to assess the independent variables that significantly influenced the variance in the dependent variable (pain)., Results: It was found that 73.9% of the participants reported musculoskeletal pain and most common painful sites were neck and back. More than half of the participants, i.e. 232 (59.3%) were aware of correct ergonomic posture regarding dental. Almost percentage of pain increased significantly with increase in age and working time. Among all specialties, prosthodontics were found to have more prevalence of MSDs., Clinical Significance: The appearance of musculoskeletal symptoms among dental professionals was quite common. It suggested that ergonomics should be covered in the educational system to reduce risks to dental practitioners.

Purpose: The aims of this study were to: (1) investigate the perceptions and experiences of predoctoral dental students and advanced standing students on mentorship, exposure to prosthodontics, and future need for the specialty, and (2) establish a baseline of students' perceptions of the impact of prosthodontics on salary, personal and patient quality of life, and the profession of dentistry., Materials and Methods: A survey was distributed to 494 predoctoral and advanced standing students at the University of Pennsylvania School of Dental Medicine. Questions focused on the perceptions and experiences with the specialty of prosthodontics. A total of 410 surveys were analyzed using Chi Square tests and univariate and multivariate analysis with statistical software., Results: Response rate was 83%. A positive initial introduction to prosthodontics was reported by 57% of students. Most students had positive experiences with prosthodontic faculty and enjoyed laboratory work and challenging/complex dentistry. A greater need for prosthodontists in the future was perceived by 82% of respondents, with 63% reporting that the future of prosthodontics had been emphasized. Students reported (1) a preclinical course directed by prosthodontists and (2) working in the clinic with prosthodontic faculty (p < 0.006) as having the biggest impact on their introduction to prosthodontics. A desire to pursue training or a career in prosthodontics was reported by 3.4% of the respondents, with 1.7% of them pursuing prosthodontics. Enjoyment of providing care in prosthodontics was the most important factor for those who decided to pursue prosthodontic postgraduate training. When compared to other specialties, prosthodontics ranked low with regards to its impact on salary (7(th) ), personal quality of life (5(th) ), patient quality of life (4(th) ), and strengthening of the dental field (7(th) )., Conclusion: Reasons few students are interested in prosthodontics as a career, despite a positive first introduction and high perceived future need for prosthodontists may be attributed to a number of factors. Purpose: The purposes of this report were to (1) assess the current occlusion curriculum in the predoctoral prosthodontic education of US dental institutions and (2) to examine the opinions of faculty, course directors, and program directors on the contents of occlusion curriculum., Materials and Methods: The Task Force on Occlusion Education from the American College of Prosthodontists (ACP) conducted two surveys using a web-based survey engine: one to assess the current status of occlusion education in predoctoral dental education and another to examine the opinions of faculty and course directors on the content of occlusion curriculum. The sections in the surveys included demographic information, general curriculum information, occlusion curriculum for dentate patients, occlusion curriculum for removable prosthodontics, occlusion curriculum for implant prosthodontics, temporomandibular disorder (TMD) curriculum, teaching philosophy, concepts taught, and methods of assessment. The results from the surveys were compiled and analyzed using descriptive statistics. The results from the two surveys on general concepts taught in occlusion curriculum were sorted and compared for discrepancies., Results: According to the predoctoral occlusion curriculum surveys, canine guidance was preferred for dentate patients, fixed prosthodontics, and fixed implant prosthodontics. Bilateral balanced occlusion was preferred for removable prosthodontics and removable implant prosthodontics. There were minor differences between the two surveys regarding the occlusion concepts being taught and the opinions of faculty members teaching occlusion., Conclusion: Two surveys were conducted regarding the current concepts being taught in occlusion curriculum and the opinions of educators on what should be taught in occlusion curriculum. Unlabelled: It is generally accepted that dental implant-treatment is "restorative" driven: the virtual blueprint of a prosthesis determines the position and number of implants to be placed. Competent interdisciplinary team-work is essential for the successful completion of implant-retained restorations. The purpose of this survey was to determine the experiences, practices and opinions in terms of the surgical placement of implants among South African prosthodontists., Methods: A questionnaire was e-mailed to a sample of South African prosthodontists. Data were collated and analysed using Epilnfo. Statistical significance was set at 0.05 and strength of association was determined by means of measurement of relative risk (RR) and chi-squared test or Fisher's exact test., Results: The response rate was 49%. The majority of respondents were male. Mean age was 50 years and all treated patients with implant-supported prostheses. Most implants were placed by non-prosthodontists (surgeons/ periodontists) and the majority of prosthodontists reported that they were generally satisfied with implant placement by other specialities. Six prosthodontists reported that they surgically place implants themselves. Of those not placing implants, five reported that they would like to do so. Younger prosthodontists, those who had attended short courses and those who considered their "surgical training" to be adequate, were more likely to place implants themselves or to want to do so., Conclusion: This survey identified areas for further research into the dynamics that may cause changing habits in the management of implants in prosthodontic practices, and carries the mplication that the scope of practice of the different specialities should be continuously evaluated and adapted, to the ultimate benefit of the patient.

Objectives: To investigate, by postal questionnaire, various aspects of primary dental care provision in the United Kingdom. Additionally, to compare the results of this survey with a similar survey completed four years previously., Methods: A questionnaire containing 89 questions was sent to 1,000 general dental practitioners selected at random from databases of practitioners throughout the United Kingdom. Non-responders were sent another questionnaire after a period of four weeks had elapsed., Results: Overall a response rate of 66.2% was achieved. Amalgam was the preferred material for the core build up of vital teeth for 65% of the respondents. Dentine pins were still being used by practitioners, with stainless steel pins being preferred by 34% of respondents. 39.7% of dentists that had graduated between 0-10 years previously reported using no pins. Indirect posts were still used most widely, by 55% of the practitioners in the study, with increasing use of fibre posts noted (34%). Addition cured silicone impression materials were still the most frequently used impression material (71%) with reduced use (10%) of condensation cured silicone impression materials recorded. Glass ionomer luting cements (48%) and zinc phosphate (28%) were the most commonly used luting cements., Conclusions: Within the limitations of this study, the following conclusions can be drawn: amalgam is still the preferred material for the core build-up of vital teeth; dentine pins are still used widely, but not by younger practitioners; fibre posts are being increasingly used by practitioners; addition cured silicone is still the most widely used impression material; traditional glass ionomer cements are still the most widely used luting cements; veneers are still the preferred indirect restoration for restoring anterior teeth; and CAD/CAM systems are increasingly being used by practitioners.

Purpose: This study evaluated the quantity of prosthodontic literature produced globally by continent in three prosthodontic journals over a 10-year period, 1998-2008. Prosthodontic research productivity relative to economic status of countries and collaboration among countries grouped by economic status was assessed., Materials and Methods: Three peer-reviewed prosthodontic journals were used for the analysis of articles published in 1998, 2003, and 2008: The Journal of Prosthetic Dentistry, International Journal of Prosthodontics, and Journal of Prosthodontics. The country of every author listed for each included article was recorded. The number of articles published by each continent and each country was reported. Countries were grouped according to the World Bank economic classification system, and the number of articles published by each economic class was found., Results: The majority of publications over the 10-year period were produced in Asia (Japan), Europe (Germany), and North America (USA). Productivity declined by 14.4% in high-income countries while it increased in upper middle-, lower middle-, and low-income countries. The majority of publications written by upper and lower middle- and low-income countries were independent works. Articles resulting from collaboration increased over time for all economic classes of countries., Conclusions: The origins of prosthodontic literature are becoming more geographically and economically diverse, with increased contributions from Africa, Asia, and South America, and middle- and low-income countries between 1998 and 2008. Introduction: The quality of life is closely related with condition of oral health, influence of subjective factors on the whole human body and patients perception of their oral health. In spite of the fact that the most part of Latvian retirement age residents use low-quality dentures and assess them satisfactory, these patients often have problems associated with quality of dentures. The aim of the present study was to evaluate oral health status, prosthodontics indicators and patients satisfaction with oral health among Latvian retirement age residents., Patients and Methods: There were examined 465 retirement age volunteers (170 men and 295 women), 116 of them in dental clinic Sandent (Riga, 24.9% - control group), 137 (29.5%) in nursing homes in Zemgale and 212 (45.6%) in nursing homes in Kurzeme (Zemgale and Kurzeme are districts of Latvia). There were assessed quantity of prosthodontics among patients with partial defects of dental arches, there were estimated quality of dentures and patients satisfaction with existing prosthodontics., Results: Oral health indicators among Latvian retirement age population are better than the same indicators among elderly residents of nursing homes. Among nursing homes residents with partial edentulousness in most of cases in oral cavities remained only few residual teeth which needed treatment. Removable and fixed dental prostheses used by nursing homes residents do not meet denture's quality criteria. Self-evaluation of dentures quality among retirement age residents of nursing homes is better and not correspond with existing dentures quality., Conclusions: In Latvia the quantity of prosthodontics in respect of partial removable dentures among the retired who regularly visit a dentist does not significantly differ from European retirement age population's dental prosthodontics quantity. Dental prosthodontics indicators of residents of nursing homes in Latvia are unsatisfactory (p<0.001). In retirement age population self-evaluation with quality of dentures is higher than actually existing.

Purpose: Part 2 of this survey reports on the 2009 survey findings distributed to the deans of US dental schools. A national, electronic survey of 58 dental school deans was distributed by e-mail to evaluate an interest in specialty training, an interest in specialty training in prosthodontics, faculty shortage issues, predoctoral curriculum in prosthodontics, ideology regarding dental specialties, and the administrative position of prosthodontics within the schools., Materials and Methods: The survey data were transferred to an online spreadsheet program for statistical analysis (Key Survey, Inc. http://www.keysurvey.com, Braintree, MA). The opinions of dental school deans were viewed as legitimate indicators of change within predoctoral and postdoctoral prosthodontic education. Statistical analysis was carried out using Statistica Version 9.1 (Statsoft, Tulsa, OK)., Results: Of the 58 deans, 42 deans responded, for a 72.4% response rate. Twenty-three deans reported an increase in the number of students seeking specialty training after dental school. Only three deans reported a decrease in those seeking specialty training. In the 2009 survey, 45% the deans responded that there was an increased interest in prosthodontics. One or more open faculty positions in prosthodontics existed at 24 (59%) of the dental schools, and 30 (71%) offered at least one incentive or a variety of incentives to recruit faculty. The 2009 respondents to the deans' survey revealed predoctoral student exposure to prosthodontists was high, and exposure to advanced education in prosthodontics students was low. A survey of internal school programs that might have an impact on an increased interest in prosthodontics revealed the presence of a predoctoral mentoring program for prosthodontics in 36 (88%) of the institutions. The clinical curriculum included treatment of a variety of cases including complex cases as defined by a diagnostic classification system. The 2009 survey respondents reported an increase in the number of schools where prosthodontics is a separate entity or department., Conclusion: Deans reported an increased interest in prosthodontics in the 2009 survey. Open faculty positions in prosthodontics existed in the majority of dental schools, and most schools offered incentives to recruit faculty. The survey of deans found a very high level of exposure of dental students to full-time prosthodontists and a very low exposure level to students enrolled in advanced education in prosthodontics. The establishment of mentoring programs in prosthodontics was reported by most deans, and the predoctoral curriculum included treating complex cases. Most deans stated that dual-specialty training in prosthodontics and periodontics would be beneficial. Objective: To assess the degree and methods of involvement of United Kingdom-based specialists in restorative dentistry in cleft lip and palate services., Design: Postal questionnaire.Setting The questionnaire was sent to specialists in restorative dentistry, periodontics, prosthodontics and endodontics as held by the General Dental Council (n = 709). The study was conducted in January 2008., Subjects (materials) and Methods: Dictated by specialists' entry on the GDC Specialist lists in distinctive branches of dentistry document, published 2006.Main outcome measure Identification of specialists involved in the care of cleft lip and palate patients., Results: The response rate was 54% (382/709). Of those replies 20% (77/382) were involved in the care of cleft lip and palate patients. Of these 77 practitioners only 17% (13/77) were part of a multidisciplinary team (MDT)., Conclusions: This study would suggest that there were few restorative specialists with a coordinated involvement in the care of cleft lip and palate patients through a recognised MDT. The majority of specialists that were involved in such care were not doing so as a result of their position as specialists. This is perhaps at odds with best practice as described by the Clinical Standards Advisory Group report of 1998.

Purpose: This study investigated the perspectives of foreign-trained dentists (FTDs) in comparison with US-trained Dentists (USTDs) in Advanced Education in Prosthodontics (AEP) programs on their current clinical training and future goals., Materials and Methods: This study was conducted by analyzing data from previously published literature. When appropriate, Chi-square statistical analysis was conducted to determine the influence of where the AEP residents earned their DMD/DDS degree (FTDs/USTDs) on all variables. Only results that yielded significant differences were discussed., Results: A majority of both FTDs and USTDs were male. Most USTDs were married, while most FTDs were single. Most FTDs were not US citizens and most originated from Asia, followed by the Middle East, South America, and Europe. Significantly more FTDs had higher ranks in their dental schools, had more advanced degrees, and spent more time practicing before entering the AEP programs. In selecting AEP programs, FTDs placed significantly higher values on a program's reputation and research opportunities. During their AEP training, FTDs paid significantly higher tuition and received lower stipends, but obtained more financial support from families. On the other hand, USTDs received significantly more financial aid and earned income from part-time work, but had significantly higher total educational debts. USTDs showed a significantly higher interest in becoming a student member of the American College of Prosthodontists and participated actively in prosthodontics organizations. USTDs were more interested in becoming maxillofacial prosthodontists, while FTDs were more interested in pursuing academic careers., Conclusion: FTDs differed from USTDs in several ways. Because of their interests in academics and research, FTDs may potentially have a positive impact on the development of the prosthodontics discipline. Purpose: The purpose of this study was to examine gender disparities in prosthodontics by reviewing the trend of female authorship in prosthodontic journals and exploring the role of female leadership in prosthodontic organizations and Advanced Education in Prosthodontic (AEP) programs., Materials and Methods: Three journals representing the prosthodontic specialty were selected to analyze the percentage of female dentist first and last (senior) authors for the years 1995, 2000, 2005, and 2008. Article inclusion criteria were restricted to the first or last authors who held at least a DMD/DDS/BDS degree and were from U.S. institutions. Data on female leadership in prosthodontic organizations and advanced education programs were collected, and the trends were studied. Descriptive statistics were used to analyze the data. A linear regression analysis was performed to investigate the proportion of female authorship compared to male in the dental literature. A Fisher's Exact Test was performed to contrast differences of female first and last authorship in the selected journals between years 1995 and 2008., Results: Overall, there was no statistically significant linear increase in the proportion of either first or last female authorship compared to male authorship over time. With respect to each journal, the linear regression analysis showed that the increase of first female authorship was statistically significant (p= 0.016) compared to male authorship only in the Journal of Prosthetic Dentistry. The percentage of female presidents of prosthodontic organizations has been very limited. A similar trend was also observed in AEP program director positions., Conclusions: Over the past 13 years, female dentists' participation in prosthodontics literature authorship has not increased significantly in the United States. Purpose: The purpose of this article is to analyze data from the results of the 2008 Survey of Pro Bono Services Provided by Practicing Prosthodontists. Survey results are used to examine characteristics and to compare the charitable care rendered by practicing prosthodontists to the dental field at large., Materials and Methods: The character and incidence of pro bono services (PBS) provided by prosthodontists are based on a 2008 survey, made possible through an American College of Prosthodontists Board of Directors' sponsored initiative. Survey results are used to assess the distribution of respondents practicing the specialty of prosthodontics in the United States, percentage of prosthodontists who render pro bono dental services for the community, percentage of total patient care devoted to pro bono treatment at no charge, number of patients treated annually with PBS, monetary value of pro bono care annually, types of pro bono procedures, percentage of practitioners using Prosthodontic Diagnostic Index (PDI), PBS by PDI category to assess complexity of donated work, and percentage of practicing prosthodontists using informatics to track services by the PDI., Results: Thirty-nine states were represented in the survey data. The highest responses were in the most populous states. The percentage of practicing prosthodontists providing PBS was 71.7%. For this cohort, the annual percentage of total care provided for treatment at no fee was greater than 1% in more than 54.8% of the practices. Almost 50% of these prosthodontists reported treating more than five patients per year at no charge. The average annual value of donated services was $25,078.00. The types of services rendered were most frequently diagnostic (83.5%) and radiographic (76.6%), followed by operative dentistry (61.5%) and fixed prosthodontics (49.4%). The percentage of practicing prosthodontists using the PDI to establish the complexity of PBS was 17.9%. For those using the PDI, there was almost an even distribution in categories I-IV. Informatics was used to track PBS in only 3% of the respondents., Conclusion: Based on this survey, practicing prosthodontists compare favorably to dental generalists and other specialists in terms of the annual dollar value donated in pro bono care. Purpose: The purpose of this study was to assess residents' perspectives on their implant surgical training in Advanced Education in Prosthodontic programs in the United States., Materials and Methods: Questionnaires were distributed to all prosthodontic residents (N = 442). The 27 questions assessed the subjective and objective aspects of implant surgical training from the view of prosthodontic residents. The data were compiled and reported as frequencies. Descriptive statistics were used to analyze the data., Results: One hundred and ninety-eight responses (44.8%) were received and analyzed. Forty-seven percent (94) of the respondents felt that the philosophy of their programs regarding implant placement in prosthodontics was "optional but encouraged," whereas 30% (60) felt that it was "mandatory." The majority of the respondents (73%, 144) stated that their programs allowed them to place implants for their own patients. For those respondents who placed their own implants, 40% (58) of them indicated that the level of their clinical training was "competent." Almost half of the respondents expressed that they would like to have a proficient level of clinical training in implant surgery by the completion of their residency programs. Forty-four percent (87) of the respondents felt their residency training adequately prepared them for implant surgery, whereas the other 37% (73) did not. For those who did not, 74% (55) felt their residency programs should have prepared them for implant surgical training., Conclusion: The current generation of prosthodontic residents has an opportunity to place implants in their programs and would like to be trained in surgical aspects of implant dentistry at the level of competency or higher., (© 2010 by The American College of Prosthodontists.)

Purpose: The purpose of this article is to examine data and results from the 2008 Survey of Prosthodontists. Survey results are used to examine current trends and characteristics of prosthodontists in private practice., Materials and Methods: Characteristics of prosthodontists and conditions of private practice are based on surveys conducted in 2002, 2005, and 2008 sponsored by the American College of Prosthodontists. Survey results are used to estimate several characteristics including age, gender, number of patient visits, hours in the practice, employment of staff, referral sources, and financial conditions (gross receipts, expenses of the practice, and net income of prosthodontists)., Results: The average age of a private-practicing prosthodontist reached 51 years in 2007; 12.3 is the number of years in the current practice; and most prosthodontists (71%) are solo private practitioners. The average amount of time per week by prosthodontists in the practice averaged 36.1 hours, and prosthodontists treated an average of 44.1 patient visits per week. The largest source of patient referrals is the patient themselves. The largest percentage of a prosthodontist's treatment time is spent rendering procedures in fixed prosthodontics, but this percentage has declined since 2001. In 2007, the average gross billings of a practicing prosthodontist reached $805,675; average total practice expenses were $518,255; the mean net earnings of practitioners were $268,930., Conclusion: In 2007, prosthodontists in private practice paid out about $1.4 billion in practice expenses to provide $2.2 billion dollars in prosthodontic care. Based on survey results from 2007 and the previous 6 years, specialization in prosthodontic care continues to be an economically attractive and productive healthcare profession in the United States.

Objectives: The goal of the study was to determine the current status of the teaching of color in dental education at both the predoctoral (Pre-D) and postdoctoral (Post-D) levels., Methods: A cross-sectional web-based survey, containing 27 multiple choice, multiple best and single best answers was created. Upon receiving the administrative approval, dental faculty involved in the teaching of color to Pre-D or Post-D dental students from around the world (N=205), were administered a survey. Statistical analysis of differences between Pre-D and Post-D was performed using Chi-square test (α=0.05)., Results: A total of 130 responses were received (response rate 63.4%); there were 70 responses from North America, 40 from Europe, 10 from South America, nine from Asia and one from Africa. A course on "color" or "color in dentistry" was included in the dental curriculum of 80% of Pre-D programs and 82% of Post-D programs. The number of hours dedicated to color-related topics was 4.0±2.4 for Pre-D and 5.5±2.9 for Post-D, respectively (p<0.01). Topics associated with tooth color, shade matching method, tooth whitening, and teaching of appearance parameters other than color, were frequently taught., Conclusions: Significant differences were recorded between the number of hours dedicated to teaching of color at predoctoral and postdoctoral level. The same is true for the prosthodontics and restorative courses, teaching on negative after images; color rendering index, Bleachedguide 3D-Master shade guide, digital camera and lens selection, composite resins, and maxillofacial prosthetic materials. Except for the restorative courses and composite resins, significantly higher results were recorded for Post-D programs. Aims: Before embarking upon changes to teaching in prosthodontics it is important to identify what techniques are being used in general practice. The aim of this study was to assess the current range and quantity of fixed and removable prosthodontics reported in general dental practice in the South East of England., Method: Structured questionnaires were sent to 191 randomly selected dentists from the GDC list in the South East of England, with 71% of the dentists completing the questionnaire. The questionnaire included demographic data, NHS/private and the range and quantity of fixed and removable prosthodontics., Results: The reported number of removable prostheses undertaken was low; most metal dentures were made in private specialist practices, whereas most acrylic partial and complete dentures were made in NHS general practices. The number of bridges, in particular minimal preparation bridges, was low as opposed to the number of crowns, which was high in all types of practices. Implants were provided mainly by the specialists., Conclusions: Dentists reported prescribing low numbers of removable and fixed prostheses with the exception of crowns. Metal dentures and implants were mainly provided by specialists. The implications of this study emphasise the importance in teaching crowns to undergraduates.

Number of the dentate subjects among the elderly is dramatically increasing, as is the elderly proportion in overall population. Such trends indicate more need for prosthodontic appliances in elderly. The aim of this investigation was to determine the distribution of various prosthodontic appliances in older age cohorts, subventioned by the Croatian Institute for Health Insurance. Data on various prosthodontic appliances subventioned by the CIHI and delivered in all Croatian districts in the period 2002-2006 was obtained and statistically analyzed relative to 4 age cohorts of 50 and more years. The oldest age cohort (80+) showed the least appliance deliveries. The most frequently delivered appliance in all age groups were partial removable dentures (382 456 deliveries). Significantly less deliveries in the oldest age cohort compared to other groups (p < 0.05) suggest that the oldest elderly patients tend to maintain prosthodontic appliances for a longer period than functionally acceptable.

Purpose: This paper describes the effects of implant-related dentistry on patient profiles and the types of fixed dental prostheses provided in clinical practice., Materials and Methods: All implant- and tooth-supported prostheses provided in a prosthodontic practice between 1984 and 2007 were tabulated. Incidence was analyzed in relation to gender, age at time of prostheses insertion, and prostheses type., Results: Tooth-supported single crowns (TSCs) and tooth-supported fixed dental prostheses (TFDPs) were involved in 97% of treatments requiring fixed dental prostheses from 1984 to 1991. From 1991 to 2007, however, a marked increase in the number of implants restored directly corresponded with a decrease in the number of TFDPs so that by 2007, implant-supported fixed dental prostheses (IFDPs) accounted for 81% of all tooth replacements. Between 1984 and 2007 the incidence of TFDPs was 61% in females and 39% in males, whereas the incidence of IFDPs was 55% in females and 45% in males. IFDPs were also involved in 35% of restorations in patients under 31 years of age and TFDPs in 19%. In the under-21 years age group, IFDPs were more common in females (9%) than males (4%), but in the 21 to 30 years age group they were more common in males (21%) than females (13%). There was a decrease in three-unit TFDPs, in TFDPs with four or more pontics and those not satisfying Ante's Law, and in teeth used that had been subjectively assessed to have an unfavorable 10-year prognosis at the time of prosthesis insertion., Conclusions: The incorporation of osseointegrated implant dentistry into a clinical practice has resulted in changes in the patient profile and type of fixed dental prostheses provided, including a decrease in the use of TFDPs; an increase in the referral of patients under 31 years of age; a decrease in three-unit, long-span, and complex TFDPs; and a decrease in tooth abutments assessed to be structurally or biologically compromised.

Purpose: The shortage of full-time dental school faculty along with a slow steady increase in student enrollment is not a new phenomenon. The purpose of this study was to determine who is teaching undergraduate prosthodontics in US dental schools-what percent are full-time faculty; what percent are prosthodontists; and what percent are board-certified prosthodontists-to allow schools to make a real-time comparison of the composition of instructors they have teaching prosthodontics to the apparent status quo., Materials and Methods: A short, ten-question survey was sent via e-mail to representatives at the 55 undergraduate US dental schools. A cover letter explained the nature of the study and requested participation and electronic return of the survey. Three weeks after the initial request, an aggressive follow-up was conducted to schools that had not returned the survey. Answers were converted to percentages to compensate for differences between schools with many or few instructors. Schools were also grouped by location to see if regional differences exist., Results: Thirty-eight (69%) of the surveys were completed and returned. In general, the division between full-time and part-time instructors is about 50%. More removable prosthodontic instructors (60%) than fixed instructors (44%) are prosthodontists, and only a small percentage of the total (18% removable and 15% fixed) have completed board certification. The South and Midwest had the highest percentage of prosthodontists involved in teaching; the South had the highest percentage with specialty board certification., Conclusion: This study indicated that only a few schools are fortunate enough to have sufficient full-time, specialty-trained faculty available for complete coverage of all undergraduate prosthodontic courses and clinics. The information presented serves as a baseline for future comparison to see if the percentage of full-time and/or specialty-certified prosthodontic faculty changes.

Number of patients assigned to dental residents, course of treatment and number of various technical tasks performed as described in the Reports on Clinical Training of Dentists published by the Department of General Dentistry, Tokyo Dental College Chiba Hospital since its establishment in 2002 were compared between before (2003-2005) and after (2006-2007) clinical training was made mandatory, and the state of clinical residency evaluated. Number of patients assigned generally increased, and mean number of patients treated by each resident in 2007 (15.08) was approximately 2 times higher than that in 2003 (8.47). Total number of treatments was 1.38 times higher in 2007 than in 2003, and periodontal treatment accounted for approximately 40% of all treatment both before and after clinical training became mandatory. Number of technical tasks in 2004-2007 was less than half the peak in 2003. To further improve clinical training, evaluation and implementation of effective approaches is necessary to secure an adequate number of patients, widen the variety of cases and increase the number of technical tasks.

Purpose: The purpose of this study was to measure dentists' before and after restorative treatment photographs to identify four metrics to describe the esthetic improvement: (a) central incisor width-to-height ratio; (b) central-to-lateral incisor width ratio; (c) lateral incisor-to-canine width ratio; and (d) lateral incisor percentage offset., Materials and Methods: Internet searches for "before after" and "veneers" and "prosthodontic" were used to obtain 198 before and after photographs of patient treatments showing the restoration of the six maxillary anterior teeth with porcelain veneers, crowns or a combination. The four metrics were measured using Adobe Photoshop. Groups were compared with repeated measures ANOVA followed by a post hoc Tukey-Kramer test with the variables of "Before or After Treatment"; "Treatment Type" (veneers, crowns or a combination) and "General Dentist or Prosthodontist"., Results: Mean central incisor width-to-height ratio was 91.7% before treatment, 80.8% after; mean central-to-lateral incisor width ratios were 69.9% and 64.7%; mean lateral incisor-to-canine width ratios were 85.3% and 81.4% and lateral incisor percentage offsets were 9.6% and 8.7%. There were significant (p<0.05) differences for before and after treatment for all variables except central-to-lateral incisor width ratio. Differences between specialist and general dentist were not statistically significant., Conclusions: On average, esthetic prosthetic treatment resulted in reduced central incisor width-to-height ratio, increased proportional width of the mesially positioned tooth of adjacent anterior teeth and reduced lateral incisor offset. The mean values of treatments by GPs and prosthodontists were not statistically significantly different., Clinical Implications: Knowledge of dentists' optimal restorative treatments provides insight on the esthetic outcome of extensive prosthodontic therapy.

Purpose: To assess and compare prosthodontic treatment needs and the related manpower requirements using the normative and sociodental needs approaches for adult Koreans aged 30 to 64 years., Materials and Methods: Data were obtained from 1,029 30- to 64-year-old adults who were a subsample of the 2003 Korean National Oral Health Survey. Subjects were clinically examined for normative needs and interviewed using an oral health-related quality of life (OHRQoL) measure, the Oral Impacts on Daily Performance. Their oral health behaviors were also assessed. The sociodental approach includes impact-related needs, in which normative needs are integrated with OHRQoL, and propensity-related needs, in which oral health behaviors are used to determine appropriate treatments. Prosthodontic treatment needs and number of clinicians needed to treat those needs were estimated using normative needs, impact-related needs, and propensity-related needs., Results: Compared to normative needs, significant decreases of 74.1% to 78.5% were found for prosthodontic treatment needs using impact-related needs and propensity-related needs, respectively. Differences in manpower requirements to deal with prosthodontic treatment were large; per 100,000 people, 86.5 dental clinicians would be needed using normative needs, compared to 22.4 clinicians using impact-related needs and 18.6 clinicians using propensity-related needs., Conclusions: The sociodental approach for assessing dental needs for prosthodontic treatment indicated much lower levels of treatment needs than the normative approach. The sociodental approach should be applied to dental workforce planning.

Virginia dentists were surveyed to reveal how changes in practice activities affected rural and urban practices in terms of prosthodontics and to assess the impact these changes may have on dentists, continuing education courses, and school curricula. Six hundred licensed dentists were selected at random and were mailed a survey containing questions about their demographics and the types of prosthodontic treatment they performed. Of the 333 respondents, 195 were considered eligible to represent general dentists. Based on this study, rural dentists devote slightly more practice time to providing removable prosthodontic treatment and oral surgery than urban dentists and slightly less time to providing implant restorative treatment. Rural dentists provide significantly more patients with removable partial dentures, complete dentures with no implants, and complete dentures of any type. Most prosthodontic fees were significantly lower for rural dentists.

Purpose: This study consisted of two parts. Part 1, a survey of program directors, was conducted to examine current trends in advanced education in prosthodontics in the United States. Part 2 will report on the survey results distributed to the deans of US dental schools to evaluate their observations of trends in prosthodontics., Materials and Methods: A national e-mail survey of 45 program directors was used to collect enrollment data for years 1 to 3 of prosthodontics training for US and international dental school graduates, the total number of applicants and applications considered, and the trends over time of applicants to prosthodontic programs for US dental school graduates and for international graduates. In addition, the program directors were asked to rank 13 key factors that may have contributed to any changes in the prosthodontic applicant pool. Comments were accepted on why more or less US- or internationally trained applicants have applied. Program directors were also asked for information on student financial incentives, whether their programs were state or federally funded, and whether their sponsoring institution was a dental school., Results: Of the 45 program directors, 39 responded, for an 86.7% response rate. Respondents reported that 64% of their enrollments were graduates of US dental schools. Between 2000 and 2004 the applicant pool in prosthodontics increased by 23%, with 41% of program directors reporting an increase in US-trained applicants, 46.2% reporting no change, and only 12.8% reporting a decrease. Using the Spearman correlation, there was a moderate, positive statistically significant correlation that the following factors contributed to an increase in the number of US dental graduates applying to prosthodontic programs: (1) mentoring by prosthodontists at the predoctoral level, (2) interest in prosthodontics among US dental students, and (3) society's demand for a higher level of training and credentialing, (4) data depicting current and projected income for dental specialists, and (5) number of trained prosthodontists full- or part-time faculty at the predoctoral level. Only five programs offered no financial packages to offset tuition. The remaining 34 respondents reported some financial package. Among the respondents, there were 25 state-sponsored programs, 9 sponsored by private universities, and 5 sponsored by hospitals or federal agencies., Conclusion: An increased applicant pool and more US-trained applicants to prosthodontics programs create a more competitive applicant pool for our specialty. Program directors reported that factors such as mentoring, society's demand for a higher level of training and credentialing, data depicting current and projected income for prosthodontists, exposure to prosthodontic faculty at the predoctoral level, the dollar value of prosthodontic training, and advances in implant, aesthetic, and reconstructive dentistry have all had some impact on increasing the applicant pool to prosthodontic training in the United States.
