2. Patrones electroencefalográficos y su asociación con el pronóstico funcional en encefalitis autoinmune anti-NMDA.

Background: Autoimmune encephalitis is a rare disease with low incidence and prevalence but carries high rates of disability and potential fatality. Objective: To determine whether the presence of abnormal electroencephalographic patterns in patients with anti-NMDAR autoimmune encephalitis affects functional prognosis at 3 months. Material and methods: A retrospective, observational, analytical, longitudinal-cross-sectional study was conducted with a retrospective cohort. Results: Out of a total of 44 patients with anti-NMDAR autoimmune encephalitis, concerning electroencephalographic findings, 19 patients had a normal EEG result (43.2%), 18 had cortical dysfunction (40.9%), 7 had epileptiform reports (15.9%), and only 1 had a Delta Brush report (2.3%). The functional outcome of patients at 3 months measured by the modified Rankin Scale (mRS) was as follows: 3 patients without disability (6.8%), 6 patients with mild disability (13.6%), 17 patients with mild to moderate disability (38.6%), 9 patients with moderate to severe disability (20.5%), 1 patient bedridden (2.3%), and 2 deceased (5.6%). Conclusions: In our study, no association was found between an abnormal electroencephalographic pattern and a worse functional prognosis at 3 months in patients with autoimmune encephalitis. Although the electroencephalogram analyzed is the initial one, it would be necessary to assess later whether the persistence of abnormalities in the electroencephalogram is related to a greater number of sequelae and disabilities. Prognostic studies may have a descriptive exploratory objective on an outcome or a comparative objective in the search for factors associated with it. A second objective is explanatory to determine the effect of a particular prognostic factor adjusted for its confounders, with or without the intention of establishing causality. The third objective is the construction of a predictive prognostic scale. For each of these objectives, there are recommended statistical methods for clarification and validity. In this article, the methods and application examples are presented. The proper selection of analytical methods allows for clear and valid communication of the results of a prognostic study. 7. Aneurismas intracraneales no rotos tratados con microcirugía: análisis de resultados clínicos y radiológicos.

La prevalencia de aneurismas intracraneales no rotos es de un 1-3%. La tasa de rotura anual aumenta en pacientes con aneurismas múltiples que han sufrido una hemorragia de uno de ellos. Los objetivos son evaluar los resultados quirúrgicos del clipaje de aneurismas no rotos, comparando pacientes en los que se clipa un único o varios y describir las complicaciones de las intervenciones de aneurismas no rotos estableciendo los factores de riesgo de un pronóstico desfavorable. Estudio retrospectivo que incluye pacientes intervenidos de aneurismas no rotos mediante microcirugía entre 2020-2023 en nuestro centro. Se analiza la tasa de cierre, las complicaciones y el resultado funcional de los pacientes. Se identifican los factores de riesgo de mal pronóstico con un modelo univariante. Se intervinieron 82 pacientes con 114 aneurismas. En 22 pacientes se clipó más de un aneurisma durante la intervención. El 86,5% de las intervenciones se realizaron mediante abordaje miniopen. El 78,6% de los aneurismas seguidos por arteriografía 3 D presentan cierre completo. La tasa de complicaciones fue del 12,2%, incluyendo asintomáticas. La mortalidad fue del 0%. La probabilidad de empeorar el mRS 1 punto fue del 7,3% y 2 o más del 1,2%, con buen resultado funcional en el 98,9%. El clipaje de varios aneurismas en una misma intervención, empleo de minicraneotomías o la cirugía en pacientes con hemorragia subaracnoidea previa no incrementó el riesgo de complicaciones. La cirugía de aneurismas de circulación posterior aumentó el riesgo de isquemia. La indicación de cirugía de aneurismas intracraneales no rotos debe de ser consensuada multidisciplinarmente, conociendo resultados clínicos y radiológicos. La microcirugía es una técnica válida y segura, con una mortalidad 0% y una morbilidad severa del 1,2% en nuestra serie. The prevalence of unruptured intracranial aneurysms is 1-3%. The annual rupture rate increases in patients with multiple aneurysms that presented a previous hemorrhage from another aneurysm. Objectives were evaluate outcomes of clipping unruptured aneurysms, comparing patients with single or multiple aneurysms clipped, describe the complications related to surgery and to identify risk factors predicting an unfavorable outcome. Retrospective study including patients who underwent clipping of unruptured aneurysms between 2020-2023 at our center. Occlusion rate, complications, and functional outcome were analyzed. Risk factors for poor prognosis are identified using univariate model. 82 patients with 114 aneurysms were treated wit microsurgery. Multiple aneurysms were clipped in 22 patients. A mini-open approach was used in 86,5% of cases. Complete occlusion in angio3D was found in 78.6% of clipped aneurysms. Complication rate was 12.2%, including asymptomatic cases. Mortality was 0%. The probability of 1 point mRS worsening was 7.3% and 2 or more points was 1.2%, with a good functional outcome in 98.9%. Clipping multiple aneurysms, miniopen approaches, or surgery in patients with previous subarachnoid hemorrhage did not increase the risk of complications. Posterior circulation aneurysms surgery increased the risk of ischemia. The management for unruptured intracranial aneurysms should be multimodal and based on clinical and radiological outcomes. Microsurgery is a valid and safe technique, with 0% mortality and bleeding rates and 1,2% rate of severe morbidity in our serie. [ABSTRACT FROM AUTHOR]

RESUMEN Objetivo: Valorar si en nuestra población los factores de riesgo cardiovascular (FRCV) se correlacionan con la severidad de la COVID-19 en pacientes hospitalizados. Material y Métodos: Estudio de cohortes retrospectivo de pacientes con COVID-19 ingresados entre los meses de abril y diciembre del 2020 en un centro terciario en el sur de España. Se valoró la asociación entre los FRCV y la severidad del SARS-COV2. Se consideró como COVID severo aquel que ingresó en la Unidad de Cuidados Intensivos (UCI) o que falleció. Resultados: Se analizaron 608 pacientes ingresados con diagnóstico de neumonía por COVID-19, la mediana de edad fue de 73 años (RIC: 64-81) años. Los pacientes fueron predominantemente varones (55,8%). La hipertensión arterial fue el FRCV más común (83,7%), seguido de la dislipemia (50,2%), diabetes (43,1%), obesidad (32,9%) y tabaquismo (15,6%). Durante la estancia un total del 25,8% de los pacientes tuvieron un pronóstico desfavorable: un 7,9% precisó ingreso en UCI y un 22,9% fallecieron. En el análisis multivariante resultaron variables independientes para padecer un COVID severo el género masculino (OR: 1,697; IC 95%: 1,138-2,532), la edad avanzada (OR: 1,697; IC 95%: 1,138-2,532), y la obesidad (OR: 1,634; IC95%: 1,370-1,947). Conclusión. Existe una gran prevalencia de FRCV en los pacientes ingresados por COVID 19 de nuestra población. La obesidad, junto con la edad avanzada y el género masculino se asocian con una mayor gravedad y mortalidad hospitalaria en los pacientes ingresados con SARS-CoV 2. ABSTRACT Objective: To assess whether cardiovascular risk factors (CVRF) are correlated with the severity of COVID-19 in hospitalized patients in our population. Material and Methods: Retrospective cohort study of patients with COVID-19 admitted between the months of April and December 2020 in a tertiary center in southern Spain. The association between CVRF and the severity of SARS-COV2 was assessed. Severe COVID was considered to be one who was admitted to the Intensive Care Unit (ICU) or who died. Results: 608 patients admitted with a diagnosis of COVID-19 pneumonia were analyzed, the median age was 73 years (IQR: 64-81) years. The patients were predominantly male (55.8%). Hypertension was the most common CVRF (83.7%), followed by dyslipidemia (50.2%), diabetes mellitus (43.1%), obesity (32.9%), and smoking (15.6%). During the stay, a total of 25.8% of the patients had an unfavorable prognosis: 22.9% died and 7.9% required admission in the ICU. In the multivariate analysis, significant independent variables were found for suffering from severe COVID: male sex (OR: 1,697; IC 95%: 1,138-2,532), age (OR: 1.697; CI 95%: 1,138-2,532), and obesity (OR: 1,634; 95% CI: 1,370-1,947). Conclusions: There is a high prevalence of CVRF in patients hospitalized for COVID 19 in our population. Obesity, along with older age and male gender, are associated with greater severity and hospital mortality in patients admitted with SARS-CoV-2

The assessment of methodological quality (MQ) in biomedical research is an area that has undergone dynamic development in the last 20 years on a global scale. MQ can be defined as a multidimensional and complex construct composed of items and domains. Different research groups have proposed tools for its assessment, among which our team is included with the development of the MInCir scales. Over the years, we have developed and validated scales to assess MQ in treatment or therapeutic procedure studies (MInCir-T), in diagnostic studies (MInCir-Dg), and prognosis studies (MInCir-Pr in its two versions, MInCir-Pr1 and MInCir-Pr2). However, as is the case with any measurement instrument, it is crucial to specify in a detailed and precise manner how it is applied, aiming to standardize measurements and minimize information biases. A detailed description is provided for the 4 domains and 8 items comprising the MInCir-Pr2 scale, specifying for each of them the characteristics to be assessed and scored in primary prognosis articles. The scale total score ranges from 7 to 60 points, with a cutoff point of 33 points (dichotomous) to define the MQ construct. This cutoff point allows classification into either adequate or high MQ (when the evaluated article receives a score of 33 points or higher), and inadequate or low MQ if the score is 32 points or less. The aim of this article was to elucidate and clarify the application of the MInCir-Pr2 scale for MQ in prognosis studies, through a guide intended to standardize MQ measurements using the MInCir-Pr2 scale, with the goal of enhance both intra- and interobserver reliability. Encephalocele is a rare congenital malformation of the central nervous system. The prevalence is estimated to be about 1 in 5,000-40,000 live births. It can affect various anatomical locations such as the occipital, frontal, temporal, and parietal regions. The fourth and fifth weeks of embryonic development are critical for the development of the head and neck. When there is a failure in the separation of the superficial ectoderm from the neuroectoderm, neural tube defects occur, from which encephaloceles may arise. Genetically it can be explained by the dysembryological theory, which involves certain mutated genes that interfere with important cellular mechanisms in early neuronal development. Currently, thanks to prenatal screening tools such as ultrasound, it is possible to identify them from intrauterine life. It is extremely important to make an early diagnosis to establish protocols and provide individualized treatment, involving a multidisciplinary team will be involved. Even though some cases are usually compatible with life, the expected prognosis of the anatomical site involved as well as timely surgical management. Climate change is one of the most important drivers of ecosystem change, the global‐scale impacts of which will intensify over the next 2 decades. Estimating the timing of unprecedented changes is not only challenging but is of great importance for the development of ecosystem conservation guidelines. Time of emergence (ToE) (point at which climate change can be differentiated from a previous climate), a widely applied concept in climatology studies, provides a robust but unexplored approach for assessing the risk of ecosystem collapse, as described by the C criterion of the International Union for Conservation of Nature's Red List of Ecosystems (RLE). We identified 3 main theoretical considerations of ToE for RLE assessment (degree of stability, multifactorial instead of one‐dimensional analyses, and hallmarks of ecosystem collapse) and 4 sources of uncertainty when applying ToE methodology (intermodel spread, historical reference period, consensus among variables, and consideration of different scenarios), which aims to avoid misuse and errors while promoting a proper application of the framework by scientists and practitioners. The incorporation of ToE for the RLE assessments adds important information for conservation priority setting that allows prediction of changes within and beyond the time frames proposed by the RLE. Amyloidosis is a heterogeneous, hereditary or acquired disease characterized by the abnormal deposition of fibrillar proteins in various tissues. The disease can be either localized or systemic, leading to significant variability in its clinical presentation and often causing diagnostic delays. For this reason, early identification of signs and symptoms is essential to improve prognosis. The first Mexican position paper on cardiac amyloidosis aims to summarize the main features of the disease and its subtypes, identifying warning signs that increase clinical suspicion. Additionally, it provides a diagnostic algorithm incorporating the imaging and laboratory studies available in our country, to streamline decision-making and facilitate a fast, practical, and up-to-date diagnosis based on evidence-based recommendations. Once the diagnosis is confirmed, the various therapeutic challenges of this disease, both pharmacological and non-pharmacological, are discussed, as patients do not respond to conventional heart failure treatments. Therefore, treatment must be individualized for each patient, considering the presence of other comorbidities. Finally, the key prognostic factors are outlined to guide appropriate treatment, including results from genetic counseling. [ABSTRACT FROM AUTHOR]

The generation and application of probabilistic models are required to make assertive decisions based on data associated with user access and service times in various organizations. The objective was to analyze the waiting line system of the central library of a university, using parameters designed to measure its performance. The methodology included quantitative techniques focused on a cross-sectional design. The information was collected with an observation grid and counting the number of students who entered the library in 10 minuts periods to carry out consultation activities. Probability models were adjusted with the data to describe the operation of the system. The results showed that it operated stably, with short waiting times and relevant service times. It was concluded that the analyzed library system achieved a high percentage (89.06%) of use by the students during the observed hours. [ABSTRACT FROM AUTHOR]

Introduction and objectives: To evaluate in sudden sensorineural hearing loss (SSHL) patients with vertigo (V) the vestibular function and identify the clinical characteristics. The present study aimed to investigate the prevalence of vestibulocochlear lesions in SSHL+V and the correlation between vestibular function test results and SSHL disease severity and prognosis. Material and methods: 42 patients diagnosed with SSHL+V were investigated retrospective and prospectively. The patients underwent pure tone audiometry and vestibular function tests consisting of video-nystagmography, video head impulse test and cervical vestibular evoked myogenic potentials. We took into account epidemiological and clinical data, complementing them with a magnetic resonance imaging examination and an extensive analytical study. Finally, we statistically evaluated the data obtained. Results: Vestibular cochlear damage varies according to the time elapsed since the event. There is a close relationship between the degree of hearing loss and the degree of vestibular dysfunction. The alteration of all vestibular receptors studied and acute vertigo are more frequent in case of profound hearing loss. The most frequently damaged vestibular receptors were the saccule and the posterior semicircular canal and the least affected was the anterior semicircular canal. The function of the horizontal semicircular canal is critical in the prognosis of hearing loss. Discussion: Predicting the prognosis of a patient with SSHL+V is a challenge that is being overcome as we recognize the factors that determine its evolution. Conclusions: In SSHL+V syndrome, the severity of hearing loss correlates with the extent of vestibular damage, but the final prognosis depends on other accompanying factors. [ABSTRACT FROM AUTHOR]

Introduction: There are few studies investigating the clinical profile of older patients with interstitial lung disease (ILD), so this study investigated the characteristics of the older population diagnosed with ILD. Material and methods: Retrospective study in a population of new referrals at an ILD clinic from January 2013 to September 2017. Patients over 64 years were selected. Data collection included diseases variables, diagnostic procedures and comorbidities. Gender-age-physiology (GAP) stage, composite physiologic index (CPI) and Charlson index was calculated. Statistical analysis was performed to investigate risk factors associated with survival. Results: A total of 232 patients were included in this study. Mean age was 76.3 years (SD 6.5). As per protocol, 69.3% completed the initial assessment but this was lower in the elderly group (61.5%). The most frequent diagnosis was unclassifiable ILD (24.1%), followed by ILD associated with connective tissue disease (21.6%), IPF (12.1%) and hypersensitivity pneumonitis (10.3%). During follow-up (36.7 months (SD 28.6)) a significant proportion of patients died (55 cases, 23.7% of the cohort), especially in the late older group (30.4%). Introduction: Guillain-Barré syndrome (GBS) is an autoimmune disease that results in demyelination and axonal damage. Although the recovery is good in most patients, 20% remain significantly disabled.Neurofilament light chain (NfL) has been established as a biomarker of axonal damage in many diseases. Methods: We measured NfL, S100B, and glial fibrillary acidic protein (GFAP) concentrations from blood and cerebrospinal fluid (CSF) taken upon admission from 19 patients with a history of GBS between January 2009 and December 2019 and investigated a correlation between them and clinical outcomes. Results: All patients fulfilled levels 1 or 2 of the Brighton diagnostic.Preceding infection was reported in 11 cases (58%).We classified 15 patients as acute inflammatory demyelinating polyneuropathy, 2 as AMAN, 1 as AMSAN, and 2 cases as Miller–Fisher syndrome.Five patients were transferred to an ICU, with a mean stay of 13 days. This article aims to elucidate viral myocarditis in neonates, focusing on a case involving Coxsackie virus, in order to enhance early detection and treatment, and, therefore, mitigating the substantial morbidity and mortality associated with this condition. Our study details the clinical course of a female newborn with Coxsackievirus-induced myocarditis, who underwent diagnostic modalities such as echocardiogram, cardiac magnetic resonance, and serial biomarker analysis, all of which allowed us to outline the disease’s natural progression and its response to intervention over time. Based on a literature review we identified 6 similar cases, spanning from 1962 to 2022. In this analysis we assessed clinical presentation, diagnostic methodologies, treatment strategies, and prognostic outcomes, drawing comparisons between our case and reported instances. The objective is to highlight optimal clinical practices for managing this pathology effectively. Furthermore, we approach the pivotal role of early diagnosis and its impact on disease prognosis. We emphasize that, though rare, viral myocarditis in neonates can lead to a high rate of mortality, which is why a prompt diagnosis and an appropriate intervention are imperative to improve the outcome. Introduction: shoulder plain is a common cause of complain, however a precise diagnosis is hard to achieve. This is why finding factors associated to a good prognosis could help to improve our clinical practice. Objective: to identify demographic and clinical characteristics from the initial assessment associated with substantial clinical benefit (SCB) in patients with shoulder pain one month after treatment or at patient's discharge. Material and methods: this was a secondary analysis of a prospective cohort of patients with shoulder pain. Demographic and clinical (self-reported scales) factors associated with SCB at discharge or four weeks after the initial assessment, were analyzed. SCB was defined as a score ≥ +5 on a Global Rating of Change. A logistic regression model was made in order to identify predictors of SCB. The area under the curve ROC was used to assess the performance of the model with its independent variables. Results: 101 patients of 138 were analyzed. The median age was 55 (RIQ 37-61) years old, there were 55 (54.5%) women in the sample. The variables independently associated to SCB were fracture as a reason for admission [adjusted OR 11.8 (95% CI 1.4-101.8); p = 0.024], and durations of shoulder symptoms shorter than seven months [adjusted OR 4.63 (95% CI 1.9-11.1); p = 0.001]. Conclusion: the diagnosis of fracture and durations of shoulder symptoms shorter than seven months were independently associated with a SCB after one month of treatment or at the patient's discharge. OBJECTIVE: To validate an artificial intelligence model that can predict the mortality prognosis of hospitalized patients with sepsis. MATERIALS AND METHODS: An ambispective observational cohort study, which included electronic records of adult patients from the Central Hospital of the State of Chihuahua, Mexico, from July 2018 to March 2020 and January 2021 to January 2022. Three models were analyzed: neural networks, support vector machine and random forests. For model validation, the sample was divided into 80% for training and 20% for testing. For the last group (20%), a 10-fold cross-validation was implemented to calculate sensitivity, specificity, positive predictive value, and negative predictive value. RESULTS: A total of 353 files were analyzed, of which only 218 were chosen. The best model was the neural networks; however, its area under the curve (AUC) score barely reached 0.80, the random forests algorithm (AUC 0.667) and the support vec- tor machine algorithm (AUC 0.641) were below this value. Of the 3 models, only the cross-validation with the neural networks was done, of 20% of the test data, 10 validations were implemented. The AUC scores obtained in each fold ranged from 0.771 to 0.830. CONCLUSIONS: The model is good, even working with few data. It is intended to collect a larger sample to retrain and validate the model with more data and improve learning and performance and finally be applicable to patients. [ABSTRACT FROM AUTHOR]

One of the complications during an acute coronary syndrome event is the presence of arrhythmias. Among them, those of the supraventricular type, especially atrial fibrillation, carry a poor prognosis both in the short and long term, being the cause of situations such as cerebrovascular event, ventricular arrhythmias, and increased mortality. The arrhythmia tends to appear in a certain population group with particular risk factors during the index event in approximately 10% of cases. Appropriate treatment at the time of its onset, thanks to the use of drugs that modulate heart rate, rhythm, and anticoagulant management in the most vulnerable groups, will lead to a less bleak outcome for these patients. La generación y aplicación de modelos probabilísticos son requeridas para tomar decisiones asertivas basadas en datos asociados con los tiempos de acceso y servicio de usuarios en diversas organizaciones. El objetivo consistió en analizar el sistema de líneas de espera de la biblioteca central de una universidad, mediante parámetros destinados a medir su desempeño. La metodología incluyó técnicas cuantitativas centradas en un diseño de tipo transversal. La información fue recolectada con una rejilla de observación y el conteo del número de estudiantes que entraban a la biblioteca en lapsos de 10 minutos para realizar actividades de consulta. Con los datos se ajustaron modelos de probabilidad para describir la operación del sistema. Los resultados evidenciaron que éste operaba establemente, con tiempos de espera cortos y tiempos de servicio pertinentes. Se pudo concluir que el sistema de biblioteca analizado logra un porcentaje alto (89.06%) de uso por parte del estudiantado en el horario observado.

La investigación tiene como objetivo la elaboración de un modelo agrometeorológico de pronóstico del rendimiento agrícola azucarero. Se tomaron los rendimientos por cepas y totales, área de cada cepa y total, y edad de la plantación de 13 Unidades Empresariales de Base de las provincias Cienfuegos, Sancti Spíritus y Ciego de Ávila en el período 1980–2015; así como, registros mensuales de precipitación, temperatura media y evapotranspiración de siete estaciones meteorológicas. Se calculó el Índice Potencial Productivo según la fase fenológica en la que se encuentre para comprobar su grado de efectividad. Para el procesamiento estadístico se utilizó el tabulador Excel, STATISTICA versión 8.0.5 y el módulo matemático SciPy. La efectividad de las estimaciones de rendimiento agrícola obtenidas con el modelo presentado para las zafras 2011 – 2015, se comparó usando el coeficiente de determinación R2, analizando la relación establecida entre las variables regresoras y el predictando. El modelo de regresión lineal múltiple permite realizar pronósticos de rendimiento agrícola con precisión y ajuste, en dependencia de los escenarios comerciales concretos de la producción cañera del país. La provincia Ciego de Ávila revela, de las tres en estudio, el mayor grado de efectividad en la estimación del rendimiento agrícola de la caña de azúcar.

Background: There is a continuing increase in the prevalence of people affected with progressive chronic diseases. Palliative care is another form of care and organization of health problems that improve quality. Objective: To determine the incidence of patients with advanced chronic illness and need of palliative care (NPC) and intrahospital mortality in a reference hospital. Methods: Prospective observational study in hospitalized patients over 18 years of age in the internal medicine department who met at least one criterion of the NECPAL CCOMS-ICO tool. They were followed until death or discharge to determine intrahospital mortality. We used descriptive and inferential statistics. Results: We studied 370 admissions from April to August 2021, including 110 patients, 59 women (55.3%) and 51 men (44.7%), age 65.5 ± 15.85 years. The incidence of NPC was 29.7%. The most frequent comorbidity was oncologic in 39 patients (35.5%), followed by chronic heart disease in 17 patients (15.5%); 38 patients were classified as NECPAL grade I (34.5%), 44 NECPAL II (40%) and 28 NECPAL III (25.5%). Twenty-five patients died during their in-hospital stay, with a mortality of 22.7%. Conclusions: The incidence of hospitalized patients in need of palliative care is considerable, more than half of them for non-oncologic pathology. The calcium-activated chloride channel (CLCA2) performs a vital function in the intricate process of tumorigenesis. Using a bioinformatics analysis system, we conducted a pan-cancer investigation on CLCA2 to explore its association with tumor prognosis and its involvement in immunology. In order to achieve this objective, we examined the prognostic significance and expression level of CLCA2 in multiple cancer types using the TIMER and Sangerbox databases. The analysis of protein interaction networks revealed proteins linked to CLCA2. To investigate the potential biological functions and enrichment pathways of CLCA2 in cancer, the SangerBox and GSCA databases were utilized. Furthermore, the expression of CLCA2 in different cancer subtypes was evaluated during the analysis. Various functional conditions of cancer cells were then compared with CLCA2 in the CancerSEA database. Using online tools like TISIDB and Assistant for Clinical Bioinformatics, the investigation explored the link between CLCA2 and immune subtypes. Additionally, it assessed immune cell infiltration as part of the analysis. In addition, the application of GDSA was employed to investigate the predictive significance of CLCA2 in relation to drug sensitivity. The research outcomes uncovered abnormal expression patterns of CLCA2 in diverse tumor categories, with its expression level demonstrating a correlation with distinct subtypes of tumors. Strong associations have been observed between enhanced patient survival rates and CLCA2 in specific tumor types. There is a noteworthy connection observed among diverse tumor types, immune cell infiltration, immune subtypes, and CLCA2. The enrichment analysis of KEGG indicates that there may exist a connection between the expression of CLCA2 and renin secretion, pancreatic secretion, as well as other pathways in pan-cancer. CLCA2 appears to primarily activate pathways such as EMT (epithelial-mesenchymal transition), RAS/MAPK, RTK, apoptosis, TSC/mTOR, and PI3K/ AKT in pan-cancer. On the other hand, it seems to inhibit pathways like cell cycle, DNA damage, hormone AR, and hormone ER. Through single-cell functional analysis, it has been confirmed that CLCA2 is associated with diverse cellular functional states, encompassing DNA repair, EMT, hypoxia, invasion, metastasis, and quiescence. Furthermore, a substantial correlation has been observed between the expression of CLCA2 and drug sensitivity towards bosutinib, tipifarnib-P1, as well as other therapeutic agents. This research affirms that various cancer types express CLCA2 and its involvement in tumor advancement and immune penetration. Introduction: the COVID-19 pandemic is being regarded as a worldwide public health issue. The virus has disseminated to 228 nations, resulting in a staggering 772 million global infections and a significant death toll of 6,9 million. Since its initial occurrence in late 2019, many approaches have been employed to anticipate and project the future spread of COVID-19. This study provides a concentrated examination and concise evaluation of the forecasting methods utilised for predicting COVID-19. Method: to begin with, A comprehensive scientometric analysis has been conducted using COVID-19 data obtained from the Scopus and Web of Science databases, utilising bibliometric research. Subsequently, a thorough examination and classification of the existing literature and utilised approaches has been conducted. First of its kind, this review paper analyses all kinds of methodologies used for COVID-19 forecasting including Mathematical, Statistical, Artificial Intelligence - Machine Learning, Ensembles, Transfer Learning and hybrid methods. Results: data has been collected regarding different COVID-19 characteristics that are being taken into account for prediction purposes, as well as the methodology used to develop the model. Additional statistical analysis has been conducted using existing literature to determine the patterns of COVID-19 forecasting in relation to the prevalence of methodologies, programming languages, and data sources. Conclusions: this review study may be valuable for researchers, specialists, and decision-makers concerned in administration of the Corona Virus pandemic. 40. Cociente neutrófilos/linfocitos preoperatorio como factor pronóstico en el cáncer epitelial de ovario.

- Abstract
Objectives: To determine if there is an association between the neutrophil to lymphocyte ratio (NLR) and prognosis in patients with epithelial ovarian cancer (EOC) diagnosed and treated in a Spanish population. Material and methods: Retrospective cohort of patients with epithelial ovarian cancer who had neutrophil and lymphocyte values in complete blood count before the histopathological diagnosis and survival of at least three months, in an intermediate complexity hospital. Convenience sampling. Measured variables included age, menopausal stage, parity, International Federation of Gynecology and Obstetrics (FIGO) stage, treatment type, residual tumor, lymph node involvement, presence of ascites, cytology, histologic type, differentiation grade, and CA-125 values. Additionally, outcomes, overall survival, disease/progression-free survival were also measured. Bivariate inferential and Cox regression analyses were performed. Results: Out of 78 candidates, 60 women with EOC were included. Of them, 24 (40%) had a low NLR (≤2,9) while 36 (60%) had a high NLR (>2,9). An association was found between high NLR levels and suboptimal cytoreductive surgery. High NLR ratios were associated with lower overall survival (Hazard ratio (HR): 4.1; 95% CI: 1.4-11.8) and lower 5-year disease-free survival (HR: 2.6; 95% CI: 1.2-5.7). Conclusions: A plasma neutrophil to lymphocyte ratio of more than 2.9 was associated with poor prognosis in patients with epithelial ovarian cancer in our setting. There is a need to establish the optimal cut-off point and conduct prospective studies with larger patient numbers in order to support this information. [ABSTRACT FROM AUTHOR]

OBJECTIVE: To determine the prevalence of these symptoms in the Mexican population treated at the General Hospital no. 46 and to examine whether anosmia is related to a mild form of the disease. MATERIALS AND METHODS: A descriptive cross-sectional study was carried out, extracting information from patients treated at the General Hospital no. 46 with a positive test for SARS-CoV-2, from June 2020 to September 2021. RESULTS: The sample included 8694 patients with a mean age of 49 years; 52.8% were men; 28.4% were classified as severe patients, with a predominance of males (p = 0.008). A prevalence of 10.6% of anosmia and 10.8% of dysgeusia was found. The relationship between the absence of anosmia and death as the cause of discharge was analyzed, considering sex a covariate. The results indicated that, in the case of men, the absence of anosmia was associated with a higher probability of death as a cause of hospital discharge. CONCLUSIONS: This inverse association between anosmia and mortality could have important implications in the classification and management of patients. BACKGROUND: Primary cutaneous CD8-positive acral T-cell lymphoma is a rare primary cutaneous T-cell lymphoma, with less than 50 published cases. This entity can be confused with more frequent diseases, such as pseudolymphomas, B lymphomas and aggressive epidermotropic CD8 lymphomas. Unlike the latter, acral CD8+ lymphoma has an excellent prognosis. Making the correct diagnosis is essential in order not to overtreat the patient, avoid side effects of the established treatments and avoid patient anxiety. CLINICAL CASE: A 62-year-old male patient with a tumor lesion on the skin of the left ear, with clinical, histological and immunohistochemical characteristics of CD8-positive acral T-cell lymphoma, with a good response to radiotherapy and interferon but with 4 relapses over 12 years of treatment. CONCLUSIONS: CD8-positive primary cutaneous T-cell lymphoma is a rare entity with a good prognosis, which can be confused with numerous inflammatory and granulomatous illnesses. The few published cases generally have an excellent response to treatment; however, follow-up is short. We report a case with a typical presentation, with a long follow-up time and a special behavior due to multiple recurrences. [ABSTRACT FROM AUTHOR]

- Abstract
Purpose: To identify patterns of chest computed tomography (CT) imaging that associate with coronavirus disease 2019 (COVID-19) lethality in palliative care patients. Methods: Data were collected from 44 patients with COVID-19 confirmed by reverse transcription-polymerase chain reaction. The total CT score comprised the sum of lung involvement (5 lobes, scores 1-4 for each lobe). Twenty-two patients who recovered from COVID-19 and 22 patients who died were enrolled. The clinical and imaging characteristics between the deceased patients and survivors were compared. Results: Diffuse lesions were present in 20 (91%) patients who succumbed but only in 12 (55%) patients who recovered (p < 0.006). The presence of pleural effusion on the initial CT chest was also more frequently found in deceased than in surviving patients (55 vs. 9%, p = 0.001). In the death group, the total CT score was higher than that of the recovery group (12.4 ± 2.54 vs. 5.08 ± 3.37, p < 0.001). The optimal cut-off points to predict mortality was 9.5, with sensitivity of 90%, specificity of 91.7%, and AUC of 0.921. Conclusion: A high CT score, the diffuse distribution of lung lesions and the pleural effusion are indicative of short-term mortality in oncology patients in palliative care suffering COVID-19. Background. Food production as the world population increases is an issue of global importance, resulting in uncertainty around agri-food security (AS). It is essential to apply new technology that monitors and makes crop management more efficient to achieve sustainable development (SD). One technology responsible for improving conventional farmers' practices is precision agriculture (PA) using tools such as big data analytics, artificial intelligence and geospatial analysis. In this sense, the monitoring of markers through satellite images requires knowledge for adequate interpretations. Some satellites have multispectral images, monitoring with these has demonstrated high precision in predicting crop yields. There are several satellites, Landsat 8 (L8) and Sentinel-2 (S2) are the most advanced and open access. Objective. To present the spectrophotometric fundamentals and current state of remote sensing with S2 multispectral images in relation to problems facing agriculture. It is concluded that remote sensing is a tool that contributes to the proper management of agricultural production, showing high reliability. Methodology. This review was carried out in accordance with the PRISMA statement (Preferred Reporting Items for Systematic reviews and Meta-Analyses), a systematic analysis of literature in the databases was carried out. MDPI, Springer Link, Science Direct, Taylor and Francis and Google Scholar were consulted. The key words were “Sentinel-2, R2, Salinity, Pest, Nitrogen, Drought”. Zotero (www.zotero.org) was used as a free access bibliography manager; articles that were not related to the research objectives were excluded. Results. Articles related to the objective of this review were identified in the databases; the study was structured based on the needs of precision agriculture (PA). The importance of adequately managing the water demand of crops, salinity stress, pest detection and the movement of nitrogen (N) in the soil-plant-atmosphere interaction. Implications. Remote perception of phenomena at the field level represents an area for improvement in crop management. Increasing resource efficiency is crucial to achieving agri-food security (AS) and thus part of the sustainable development goals (SDGs). Conclusion. The S2 has multispectral images that, when processed, allow obtaining information about the crop, i.e. monitoring stress, humidity in plants and optimization of plant nutrition. With PA, decisions are made for crop management, costs are reduced by reducing losses due to crop stress, and resource efficiency increases.

En los tumores sólidos la hipereosinofilia es un fenómeno raro y se asocia principalmente con carcinomas secretores de mucina. Los tumores tiroideos asociados a neutrofilia y/o eosinofilia se han descrito exclusivamente en pacientes con cáncer anaplásico de tiroides. La eosinofilia asociada con cáncer papilar de tiroides es extremadamente rara y se encuentran muy pocos casos descriptos actualmente. Se ha sugerido que tres citocinas, a saber, la interleucina-3 (IL-3), la interleucina-5 (IL-5) y el factor estimulante de colonias de granulocitos y macrófagos (GM-CSF), pueden actuar como un péptido eosinofílico potencial. Hasta el momento solo se han reportado tres pacientes con cáncer diferenciado de tiroides asociados a eosinofilia, dos de tipo papilar y uno de tipo medular. Paciente de 48 años consultó en el año 2022 por adenopatías cervicales bilaterales de 3 años de evolución asociado a síndrome consuntivo e hipereosinofilia. Se solicitó PET CT que evidenció foco hipermetabólico en lóbulo tiroideo derecho y metástasis ganglionares, pulmonares, óseas y hepáticas; ecografía tiroidea que evidencia en lóbulo derecho nódulo de alta sospecha de malignidad y conglomerado de adenopatías con lavado de aguja positivo para tiroglobulina. Evaluada la hipereosinofilia con valores iniciales de leucocitosis de GB 30310/mm3 (10608/mm3 de eosinófilos) hasta valores máximos de GB 77090/mm3 (eosinófilos 20814/mm3) se interpretó como síndrome paraneoplásico y se inició corticoterapia en dosis inmunosupresoras sin respuesta. Nuestras observaciones presentadas en este artículo están en línea con la mayoría de los estudios que reflejan que la hipereosinofilia paraneoplásica se caracteriza por una enfermedad más avanzada y un mal pronóstico.

Introducción. En los pacientes con falla cardíaca, el sodio urinario se ha propuesto como marcador de gravedad y resistencia a los diuréticos, pero los resultados de los estudios reportados son heterogéneos. Objetivo. Evaluar el sodio en orina ocasional como factor pronóstico de mortalidad en pacientes con falla cardiaca descompensada. Materiales y métodos. Se realizó un análisis anidado de casos y controles de una cohorte prospectiva de falla cardíaca descompensada. El desenlace primario fue mortalidad a los 180 días. Se hizo un análisis bivariado para evaluar las variables que se asocian con la mortalidad. Se analizaron las diferencias de las variables clínicas entre los grupos con sodio urinario mayor o menor de 70 mEq/L. Resultados. Se incluyeron 79 pacientes de los cuales 15 fallecieron a los 180 días. La edad promedio fue de 68,9 años (DE: ±13,8), 30 eran mujeres (38 %). Quince pacientes (18,9 %) tuvieron un sodio en orina inferior a 70 mEq/L. En el análisis bivariado se encontró una asociación significativa de la mortalidad con las hospitalizaciones, la presión arterial sistólica inferior a 90 mm Hg, el uso de inotrópicos y el sodio urinario inferior a 70 mEq/L. Los pacientes con sodio urinario bajo habían estado hospitalizados con mayor frecuencia en el último año, tenían menores valores de sodio sérico y presión arterial al ingreso. Conclusión. Los pacientes con sodio urinario inferior a 70 mEq/L tienen características de mayor gravedad. En el análisis bivariado, el sodio urinario se asoció con mortalidad a los 180 días.

Abstract Aims In heart failure patients renal dysfunction represents impaired tissue perfusion. We investigated the association of customarily used renal function parameters with short-term prognosis in patients admitted with acute decompensated heart failure in class III or IV of New York Heart Association. Material and Methods Univariate Cox proportional hazard model was used to assess the relationship between variables and outcomes. Survival curves were designed using the Kaplan-Meier method. Results We followed 65 patients for a median of 13.7 (Q1-Q3 6.7-18.9) months. Variables associated with an increased risk for short-term rehospitalization were baseline urea (HR: 1.098, 95% CI: 1.022-1.179, P-value=0.01), admission urea (HR: 1.048, 95% CI: 1.013-1.084, P-value=0.006), baseline creatinine (HR: 1.111, 95% CI: 1.004-1.229, P-value=0.041), admission creatinine (HR: 1.047, 95% CI: 1.005-1.092, P-value=0.027) and admission glomerular filtration rate
