8. Concordance Analysis of Lower Third Molar Surgery Classifications: A Comparative Study.

Abstract

The high frequency and complexity of mandibular third molar (M3M) surgery have led several authors to the development of classification systems for better evaluation and management in oral surgery. This study compared the classifications of Juodzabalys and Daugela et al. (JD), Sammartino et al., Chang et al., Jhamb et al., Maglione et al., and Nemsi et al. to understand the concordance between the scores of M3M surgery. Two types of analysis were conducted: the relationship between the M3M and the inferior alveolar nerve (IAN), and the overall difficulty score based on the tooth's angulation and its spatial position with the adjacent structure. The analysis of the classifications on the relationship between M3M and IAN resulted in a concordance of 26.1%. In the pairwise comparisons, the classifications of Nemsi et al. and Jhamb et al. showed the highest concordance of 59.5%. Analyzing the total scores difficulty, the JD et al., Chang et al., and Sammartino et al. classifications demonstrated a concordance level of 25.5%. A pairwise assessment revealed a higher concordance degree between the classifications of Sammartino et al. and Chang et al. (57.4%). The results highlight the limits in establishing a comprehensive and objective classification for the surgical difficulty of M3M, possibly attributed to variations in the methodology for computing total scores. An objective, automated, and non-operator-dependent classification method for assessing the surgical difficulty of M3M is still needed. [ABSTRACT FROM AUTHOR]

Abstract

A sedentary lifestyle associated with unregulated diets rich in high-calorie foods have contributed to the great prevalence of metabolic dysfunction-associated steatotic liver disease (MASLD) latterly, with up to 60% in the high-risk population and 25% in the general population. The absence of specific pharmacological strategies for this syndrome represents one of the major problems in the management of MASLD patients. Lifestyle interventions and adherence to a healthy diet are the main cornerstones of current therapies. The identification of nutraceuticals useful in the treatment of MASLD appears to be one of the most promising strategies for the development of new effective and safe treatments for this disease. The onion, one of the most widely studied foods in the field of nutraceuticals, serves as an inexhaustible reservoir of potent compounds with various beneficial effects. The following preliminary study analyzes, mediating in silico studies, the iteration of a library of typical onion compounds with 3-hydroxy-3-methylglutaryl-coenzyme A reductase, liver receptors X α and β, as well as peroxisome proliferator-activated receptors α and γ. Abstract

The increasing drug resistance of Acinetobacter (A.) baumannii is a growing concern globally. This bacterium, prevalent in hospitals, is responsible for multi-resistant infections and has been identified by the WHO as a priority pathogen for new antibiotic development. While there is extensive information on human isolates, data on animal isolates are scarce and fragmented based on limited clinical cases and genomic analyses. The impact of resistant strains on public and veterinary health is challenging to assess due to a lack of data. However, the potential impact on public health cannot be underestimated. Therefore, it is imperative to strengthen surveillance of A. baumannii in domestic animals, livestock, and wildlife. The growing presence of this pathogen in veterinary clinical samples is alarming, especially given the increasing interaction between animals and humans. Recognizing animals as potential reservoirs is crucial for understanding pathogen transmission dynamics, given their capacity for zoonotic transmission. With the rise in the number of animals testing positive for multi-resistant bacteria, monitoring A. baumannii in the animal population is vital to prevent the pathogen's spread and protect public health. A review of studies conducted in Italy on the topic underscores the role of pets, livestock, and foods of animal origin in the transmission of A. baumannii and Acinetobacter spp. Further animal research is urgently needed to gain a better understanding of the mechanisms of spread and resistance. Abstract

The traditional Mediterranean diet, described for the first time by Ancel Keys in the 1960s, is inspired by the eating habits and traditional foodtypical of southern italy, southern spain, and crete, and currently represents one of the most important dietary health models worldwide. thehigh content of fibers and polyunsaturated fats in cereals, vegetables, legumes and olive oil, which are part of the Mediterranean diet, show anti-inflammatory and antioxidant properties, aim at reducing cellular oxidative stress, responsible for several chronic diseases. Moreover, this dietis characterized by low alcohol intake (mainly red wine) and low amounts of red meat and dairy products. From a nutritional intake perspective,the Mediterranean diet guarantees a correct assumption of proteins (10-15%), carbohydrates (55-60%) and fats (25%). importantly, a strongadherence to this dietary regimen modulates the gut microbiota composition, promote eubiosis and prevents several gut-liver axis diseases. Thepresent narrative review highlights the role of proteins in the Mediterranean diet, showing how plant-derived proteins have a protective effectagainst the development of cardiovascular, intestinal, liver, and metabolic diseases. Abstract

Topological inference of biological interaction networks from experimental data is a fundamental research topic in the broad area of Systems Biology. Several algorithms presented in the literature have been devised in order to infer the topology of a network from time‐course data. The present work introduces a novel method for reverse‐engineering gene regulatory networks from time‐course experiments, which combines the instrumental variables technique for the identification of dynamical systems with a regularization strategy for dealing with over‐parametrized systems. Differently from least squares methods, the proposed approach can explicitly address the bias and nonconsistency issues that arise when dealing with time‐course measurements, thus yielding improved performance with respect to methods designed for steady‐state data. Moreover, the devised approach, which has been named RIVA (Reverse‐engineering of biological networks via Instrumental VAriables), can simultaneously exploit multiple time‐series, thus enabling one to get improved results by collecting and exploiting data from multiple experiments, and is computationally efficient, thus it can be also applied to large‐scale (in the order of thousands of nodes) networks. To analyze the applicability and effectiveness of RIVA, we performed several tests, both with simulated data and with experimental data, and compared the results against other state‐of‐the‐art inference methods designed for time‐series data. Abstract

Nowadays, the fast development of hardware for IoT-based systems creates appropriate conditions for the development of services for different application areas. As we know, the large number of multifunctional devices, which are connected to the Internet is constantly increasing. Today, most of the IoT devices just only collect and transmit data. The huge amount of data produced by these devices requires efficient and fast approaches to its analysis. This task can be solved by combining Artificial Intelligence and IoT tools. Essentially, AI accelerators can be used as a universal sensor in IoT systems, that is, we can create Artificial Intelligence of Things (AIoT). AIoT can be considered like a movement from data collection to knowledge aggregation. AIoT-based systems are being widely implemented in many high-tech industrial and infrastructure systems. Such systems are capable of providing not only the ability to collect but also analyse various aspects of data for identification, planning, diagnostics, evaluation, monitoring, optimization, etc., at the lower level in the entire system's hierarchy. That is, they are able to work more efficiently and effectively by generating the knowledge that is needed for real-time analytics and decision-making in some application areas.

Abstract

Primary biliary cholangitis (PBC) is a chronic autoimmune liver disease characterized by the progressive destruction of the intrahepatic bile ducts. Currently, the first line drug for PBC is ursodeoxycholic acid (UDCA) characterized by anti-apoptotic, anti-inflammatory and protective actions on cholangiocytes. Despite its recognized therapeutic action, 30-40% of PBC patients only partially benefit from UDCA therapy. This has led to the identification of the role of the farnesoid x receptor (FXR) in cholestatic liver diseases and, consequently, to the development of obeticholic acid (OCA), a steroid FXR agonist that has been recently approved for the treatment of PBC. OCA though is not effective in all patients and can cause itch, which eventually induces treatment drop out. Therefore, the search for new therapeutic strategies for PBC has begun. This review, in addition to summarizing the current treatments for PBC, provides overview of the chemical characteristics of new steroid FXR agonist candidates that could represent a future perspective for the treatment of PBC.

Abstract

Cardiovascular diseases represent, to date, the major cause of mortality worldwide. Diagnosis of the most frequent of such disease, acute myocardial infarction (AMI), requires the evaluation of time-series measurement of specific cardiac biomarkers concentration. The aim of this paper is to provide the clinicians with a quantitative tool to analyze such time-series, with the final goal of enhancing the diagnostic and prognostic procedures. The proposed approach is based on a novel dynamical model, which synthetically describes the basic mechanisms underlying cardiac troponin (cTnT) release into the plasma after the onset of AMI. Leveraging tools of system identification and a data set of AMI patients treated at our University Hospital, the model has been assessed as an effective tool to quantify the characteristic release curves observed under different conditions. Furthermore, it has been shown how the devised approach is also suitable in those cases where only partial measurements are available to the clinician to recover important clinical information. Finally, an optimal experimental design analysis has been performed in order to gain insights on how to optimize the experimental data collection phase with potentially relevant implications on both the quality and cost of the diagnosis procedure
