ZusammenfassungDer Radiuskopf nimmt für die funktionelle Anatomie von Ellenbogen und Unterarm eine zentrale Rolle ein. Er stellt neben den Seitenbändern und dem Processus coronoideus einen wichtigen Stabilisator des Ellenbogengelenks dar. Entsprechend der Bedeutung des Radiuskopfs für die Funktion des Ellenbogengelenks ist eine differenzierte Therapie der Radiuskopffraktur unabdingbar. In diesem Artikel sollen sowohl Diagnostik als auch Klassifikation der Radiuskopffraktur, Behandlungsalgorithmen und Techniken der Rekonstruktion in Abhängigkeit der Frakturmorphologie dargestellt werden.

Fragestellung: Die suffiziente Stabilisierung des Processus coronoideus Fragmentes ist die Voraussetzung für eine effektive frühfunktionelle Nachbehandlung bei Ellbogenluxationsfraktur ohne Risiko der Re-(Sub-)Luxation. Background:The aim of our study was to develop a test setup combining realistic force transmission with physiological movement patterns at a frequency that mimicked daily use of the elbow, to assess implants in orthopedic joint reconstruction and trauma surgery.Methods:In a multidisciplinary approach, anin vitrobiomechanical testing machine was developed and manufactured that could simulate the repetitive forceful movement of the human elbow joint. The construction involved pneumatic actuators. An aluminum forearm module enabled movements in 3 degrees of freedom, while motions and forces were replicatedviaforce and angular sensors that were similar toin vivomeasurements.Results:In the initial testing, 16 human elbow joint specimens were tested at 35 Nm in up to 5000 cycles at a range of 10° extension to 110° flexion. The transmitted forces led to failure in 9 out of the 16 tested specimens, significantly more often in females and small specimens.Conclusions:It is possible to construct a testing machine to simulate nearly physiological repetitive elbow motions. The prototype has a number of technical deficiencies that could be modified. When testing implants for the human elbow with cadaver specimens, the specimen has to be chosen according to the intended use of the implant under investigation.

Zusammenfassung Durch angiographische Untersuchungen am Praparat und intra vitam sowie durch experimentelle Untersuchungen sollte geklart werden, ob beim Pferd als atiologischer Faktor bei der Osteochondrosis dissecans im Talokruralgelenk an der kranialen Kante der Tibia (Processus coronoideus) umschriebene Durchblutungsstorungen in Frage kommen. In der distalen Halfte der Tibia besteht ein fein verzweigtes System intraossearer Gefase. Insgesamt 4 nutritive Gefase versorgen die distale Tibiaepiphyse. Diese Gefase des epiphysaren Systems anastomosieren untereinander und mit dem distal gerichteten Zweigen der A. nutricia tibiae des medullaren Systems. Nach dem Ergebnis der Untersuchungen werden primare Storungen der arteriellen Gefasversorgung fur die Entstehung der O. d. beim Pferd im Talokruralgelenk nicht angenommen. Summary Arteriographic studies on the distal tibial end in relation to the pathogenesis of Osteochondrosis dissecans in the horse Angiographic studies on prepared tissues and intra vitam, as well as experimental studies, were carried out to determine whether local vascular damage is concerned in the aetiology of Osteochondrosis dissecans in the tibio-tarsal joint on the cranial border of the tibia (Processus coronoideus). In the distal half of the tibia is a finely branched system of intraosseous vessels. A total of 4 nutritive vessels supply the distal tibial epiphysis. These vessels of the epiphyseal system anastomose with one another and with the distally directed branches of the nutrient tibial artery of the medullary system. The results do not support the view that primary damage to the arterial supply is responsible for the development of Osteochondrosis dissecans in the tibio-tarsal joint of the horse. Resumen Estudios arteriograficos del extremo distal de la tibia con respecto a la patogenia de la osteocondrosis disecante en el caballo Mediante estudios angiograficos en la preparacion e intra vitam asi como por medio de pesquisas experimentales queriamos esclarecer si intervienen en el caballo defectos circunscritos en la irrigacion sanguinea como factor etiologico en la osteocondrosis disecante en la articulacion talocrural en la arista craneal de la tibia (proceso coronoides). Existe en la mitad distal de la tibia un sistema finamente ramificado de vasos intraoseos. Estos vasos del sistema epifisario se anastomosan entre si y con las ramas de direccion distal de la a. nutricia de la tibia del sistema medular. Con arreglo al resultado de las investigaciones realizadas, no admitimos el que las lesiones primarias en el abastecimiento vascular arterial sean decisivas en la patogenia de la o. d. en el caballo en la articulacion talocrural.

The aim of the study was to find the difference between actual and anticipated frequencies of individual types of FCP (fragmented coronoid process) in relation to the extent of radioulnar incongruity. We evaluated the radiographs of elbow joints (n = 135) of dogs (n = 77) with arthroscopically (n = 109) or arthrotomically (n = 26) proven fragmented coronoideus medialis ulnae. Radioulnar incongruity was classified as a congruent joint (0-0.5 mm), moderate incongruity (0.6-2 mm) and marked incongruity (> 2.1 mm). In elbow joints without radiologically identifiable radioulnar incongruity (0-0.5 mm) significantly higher occurrence of fissured PCM (processus coronoideus medialis) was found (p < 0.01). In elbow joints with pronounced radioulnar incongruity (> 2.1 mm) we found significantly higher occurrence of FCP with a dislocated fragment (p < 0.001). The results of this study suggest the possibility of using the assessment of radioulnar incongruity from radiographs of elbow joints in mediolateral projection for specifying the X-ray diagnosis of FCP with regard to the type of FCP lesion.

The aim of the study was to find the frequency of radiologically detectable “kissing” lesion in individual variants of fragmented processus coronoideus medialis (FCP) and its occurrence in relation to sex. The radiographs of elbow joints (n = 135) of dogs (n = 77) in which a fragmented processus coronoideus medialis ulnae was detected arthroscopically (n = 109) or arthrotomically (n = 26) were evaluated. Significantly higher occurrence of radiologically detectable “kissing” lesion was found in elbows with non-dislocated and dislocated fragment (p< 0.01). Significantly lower occurrence of radiologically detectable “kissing” lesion was found in elbows with chondromalacia of processus coronoideus medialis (PCM) (p< 0.01). We found no significant difference (p > 0.05) between the occurrence of a “kissing” lesion in males and females with FCP. The occurrence of the “kissing” lesion extending to the subchondral bone was not affected by sex. The results of our study suggest the possibility of identifying elbow joints with completely fragmented FCP on the basis of occurrence of a radiologically detectable “kissing” lesion.

Als ungewohnliches und seltenes Krankheitsbild wird die Hyperplasie des Processus coronoideus haufig nicht oder zu spat diagnostiziert. Ziel dieser Studie war es, die Morphologie, Atiologie und das klinische Bild der Coronoidhyperplasie zu beschreiben sowie Diagnose und Therapie zu diskutieren. Es erfolgte die Analyse aller Falle mit histologisch gesicherter Hyperplasie des Processus coronoideus, die in unserem Zentrum zwischen 1995 und 2004 behandelt worden waren. Die Auswertung der Patientendaten erfolgte hinsichtlich Alter, Geschlecht, klinischer Symptome, Diagnostik und Therapie. Die so gewonnenen Daten wurden mit den Literaturangaben verglichen. Es wurden 14 neue Falle und 101 publizierte Falle rekrutiert, davon 96 mit bilateraler und 19 mit unilateraler Hyperplasie. Das mittlere Alter der Betroffenen bei Diagnosestellung betrug 23,7 Jahre. In Bonn wurden alle Patienten durch Coronoidektomie und entsprechende Physiotherapie behandelt. Eine Verbesserung der Mundoffnung konnte bei 86% unserer Patienten erzielt werden. Verglichen mit den zum Teil enttauschenden Ergebnissen in der Vergangenheit publizierter Studien im Hinblick auf Mundoffnung und Unterkiefermobilitat scheint durch unser Therapiekonzept eine Verbesserung moglich. Unsere Studie unterstreicht die Bedeutung von dreidimensionalen CT-Techniken bei der Diagnose und Operationsplanung, die Uberlegenheit der Coronoidektomie im Vergleich zur Coronoidotomie und die Bedeutung dynamischer Physiotherapie zur Pravention postoperativer Vernarbungen.

Bei Luxationen des Ellenbogengelenkes finden sich nicht selten ossare Begleitverletzungen. Wahrend sich reine Luxationen mit guten klinischen Ergebnissen meist konservativ behandeln lassen, fuhrt die durch eine erhebliche initiale Instabilitat gekennzeichnete Luxationsfraktur trotz operativer Therapie haufig zu residualen Bewegungseinschrankungen. Verhindert werden kann diese Einsteifung nur durch eine konsequente fruhfunktionelle Behandlung. Eine derartige Behandlung ist jedoch nur moglich, wenn das instabile Gelenk in der fruhen posttraumatischen Phase sicher gefuhrt wird. Der Wunsch, anatomische Verhaltnisse wiederherzustellen, fuhrt zur Vorstellung, den meist medial zerrissenen Bandapparat zu nahen, oder einen abgerissenen Processus coronoideus zu refixieren. Das Ziel, Stabilitat herzustellen, wird dabei jedoch haufig verfehlt. Aus dem Gedanken heraus, dass durch naturliche Heilungsvorgange (Vernarbung) aus der instabilen in fast allen Fallen eine stabile Situation erzeugt wird, erscheint es sinnvoller, den Bandapparat der naturlichen Heilung zu uberlassen und statt dessen mithilfe eines Bewegungsfixateur externe, wahrend der Heilungszeit eine sichere uniaxiale Bewegung im Humeroulnargelenk zu gewahrleisten. Elbow dislocations are frequently accompanied by osseous lesions. Whereas most simple dislocations may be treated conservatively with satisfactory results, in fracture dislocations with a high degree of initial instability a reasonable functional deficit due to stiffness can be expected. This can only be resolved by a consequent early functional treatment of the joint. This however is only possible if a guided movement can be safely applied in the early posttraumatic period. The wish to reconstruct anatomical structures to regain stability leads to the idea to suture the ruptured ligaments (mostly on the medial side), or to reattach the coronoid process as a source of instability. The goal to re-establish joint stability however is frequently not reached. From the observation that unstable injured joints in nearly all cases regain stability without any kind of surgery due to the natural healing mechanisms (scarification) it seems more reasonable to leave the ruptured ligament apparatus to natural healing and with the aid of an early motion hinged fixator to allow for a guided movement of the humeroulnar joint.

Zusammenfassung In Streckstellung des Ellbogens kommt dem Processus coronoideus ulnae und dem Radiuskopfchen eine grose Bedeutung fur die Kraftubertragung zu. Frakturen des Processus coronoideus ulnae, die zur Verkleinerung der Incisura semilunaris

Geschlossene Ellenbogenluxationen ohne neurovascularen oder knochernen Begleitschaden werden in unserer Klinik nach erfolgter Reposition bei fehlender Reluxationstendenz konservativ behandelt. Irreponible oder nicht retinierbare Ellenbogenverrenkungen sowie solche mit begleitendem offenen Weichteilschaden oder knochernen bzw. neurovascularen Begleitverletzungen werden operativ angegangen. Innerhalb von 10 Jahren wurden 113 Patienten entsprechend diesem Therapieregime versorgt. Dabei handelte es sich in 46 Fallen um Luxationen ohne knocherne Begleitverletzung, in 67 Fallen um Luxationsfrakturen. 95 Patienten konnten nach durchschnittlich 55 Monaten nachuntersucht werden. Wir fanden in uber 3/4 der Falle funktionell exzellente und gute Ergebnisse; die schlechten Ergebnisse traten nur bei Patienten mit Luxationsfrakturen auf. Bis auf eine Ausnahme lag am Ende der Behandlung ein stabiles Ellenbogengelenk vor.
