Abstract

Aims: Patients admitted for acute heart failure (HF) are at high risk of readmission and death, especially in the 90 days following discharge. We aimed to assess the safety and efficacy of early optimization of oral HF therapy with beta-blockers (BB), angiotensin-converting enzyme inhibitors (ACEi) or angiotensin receptor blockers (ARB) or angiotensin receptor–neprilysin inhibitors (ARNi), and mineralocorticoid receptor antagonists (MRA) on 90-day clinical outcomes in patients admitted for acute HF. Methods: In a multicentre, randomized, open-label, parallel-group study, a total of 900 patients will be randomized in a 1:1 ratio to either ‘usual care’ or ‘high-intensity care’. Patients enrolled in the usual care arm will be discharged and managed according to usual clinical practice at the site. In the high-intensity care arm, doses of oral HF medications – including a BB, ACEi or ARB, and MRA – will be up-titrated to 50% of recommended doses before discharge and to 100% of recommended doses within 2 weeks of discharge. Up-titration will be delayed if the patients develop worsening. symptoms and signs of congestion, hyperkalaemia, hypotension, bradycardia, worsening of renal function or significant increase in N-terminal pro-B-type natriuretic peptide between visits. The primary endpoint is 90-day all-cause mortality or HF readmission. Conclusions: STRONG-HF is the first study to assess whether rapid up-titration of evidence-based guideline-recommended therapies with close follow-up in a large cohort of patients discharged from an acute HF admission is safe and can affect adverse outcomes during the first 90 days after discharge. Abstract

Aim. To assess safety, clinical and cost-effectiveness of a new method for invasive examination in coronary heart disease (CHD) patients: ambulatory coronary angiography (CAG) in hospitalised or ambulatory patients of a city clinical hospital (CCH) not having its own angiography laboratory.Material and methods. The study included 407 patients of the cardiology emergency department, Moscow CCH №51 (2001-2005), who underwent an ambulatory CAG at the angiography laboratory, Cardiology Research Institute, Russian Cardiology Scientific and Clinical Complex. Mean age of the participants was 56+10 years, with 281 (92 %) men, 236 (58 %) arterial hypertension (AH) patients, and 44 (11 %) diabetes mellitus (DM) patients. In total, 101 (25 %) and 17 (23 %) individuals were hospitalised to the CCH with myocardial infarction (MI) and unstable angina (UA) diagnosis, respectively.Results. CAG was successful in all 407 participants, including 311 with radial access, and 94 with femoral access. Mean total CAG duration, from anaesthesia start to bandaging puncture area, was 21,7±11 minutes; mean time of X-ray radiation - 4,2±2,4 minutes. On average, 135±29 ml of contrast and 2,6±0,7 catheters were used per patient. Mean time from procedure end to discharge was 3,9±1,2 hours. No major complications were observed, minor complication prevalence was 5 %. In cost-effectiveness analysis, ambulatory CAG reduced procedure costs by 19 %, comparing to the control group.Conclusion. Ambulatory CAG in CHD patients from a CCH without its own angiography laboratory, was safe, with low complication risk. Ambulatory CAG reduced mean procedure costs by 19 %, due to decreased hospitalisation expenses.

Abstract

The efficacy and safety of prolonging prophylaxis for venous thromboembolism in medically ill patients beyond hospital discharge remain uncertain. We hypothesized that extended prophylaxis with apixaban would be safe and more effective than short-term prophylaxis with enoxaparin. METHODS: In this double-blind, double-dummy, placebo-controlled trial, we randomly assigned acutely ill patients who had congestive heart failure or respiratory failure or other medical disorders and at least one additional risk factor for venous thromboembolism and who were hospitalized with an expected stay of at least 3 days to receive apixaban, administered orally at a dose of 2.5 mg twice daily for 30 days, or enoxaparin, administered subcutaneously at a dose of 40 mg once daily for 6 to 14 days. The primary efficacy outcome was the 30-day composite of death related to venous thromboembolism, pulmonary embolism, symptomatic deep-vein thrombosis, or asymptomatic proximal-leg deep-vein thrombosis, as detected with the use of systematic bilateral compression ultrasonography on day 30. The primary safety outcome was bleeding. All efficacy and safety outcomes were independently adjudicated. RESULTS: A total of 6528 subjects underwent randomization, 4495 of whom could be evaluated for the primary efficacy outcome - 2211 in the apixaban group and 2284 in the enoxaparin group. Among the patients who could be evaluated, 2.71% in the apixaban group (60 patients) and 3.06% in the enoxaparin group (70 patients) met the criteria for the primary efficacy outcome (relative risk with apixaban, 0.87; 95% confidence interval [CI], 0.62 to 1.23; P = 0.44). By day 30, major bleeding had occurred in 0.47% of the patients in the apixaban group (15 of 3184 patients) and in 0.19% of the patients in the enoxaparin group (6 of 3217 patients) (relative risk, 2.58; 95% CI, 1.02 to 7.24; P = 0.04). CONCLUSIONS: In medically ill patients, an extended course of thromboprophylaxis with apixaban was not superior to a shorter course with enoxaparin. Abstract

With the aim of assessing parameters of heart rate variability (HRV) and heart rhythm turbulence (HRT) in patients with chronic obstructive pulmonary disease (COPD) in dependence on severity of the course of this disease and presence of pulmonary hypertension (PH) we examined 73 patients (28 with COPD and 45 healthy subjects). Invasive measurement of central hemodynamics was conducted. Compared with the control group in patients with COPD we revealed lowering of temporal as well as frequency HRV parameters. No significant changes of HRV parameters depended on severity of COPD course. However a tendency to maximal lowering of HRV parameters was noted in the group of patients with COPD with first sec forced expiratory volume <50%. Comparison of patients with and without PH with controls revealed tendency to maximal lowering of HRV parameters in the PH group. Thus measurement of HRV can be used for supplementary assessment of severity of the disease and detection of PH.

Abstract

We compared Dopplerographic methods and invasive assessment of mean pulmonary artery pressure (MPAP) in 20 men with chronic obstructive pulmonary disease (COPD). We demonstrated possibility of evaluation of mean pulmonary artery pressure by Doppler methods of calculation using diastolic gradient of pressure of pulmonary regurgitation and ratio of flow acceleration time in right ventricular outflow tract and ejection time. In patients with COPD overestimation of MPAP is possible especially in patients with pronounced elevation of MPAP. Methods of evaluation of MPAP based on measurements of intervals of pulmonary blood flow do not allow to adequately evaluate MPAP in patients with COPD.

Abstract

Transesophageal echocardiography (TEE) is routinely used for visualization of left auricular thrombi in patients with atrial fibrillation (AF). Multispiral computed tomography (MSCT) with the use of contrast preparations is a novel method of visualization of intracardiac structures. Forty three patients (27 men, 16 women aged 46-81 years) with duration of atrial fibrillation > 48 hours and scheduled for sinus rhythm restoration were included in this study. TEE and MSCT were carried out in all patients during first 3 days of hospitalization. If according to both methods there were no thrombi in the left auricle cardioversion was performed. If data of one of the methods were suspicious of left auricular thrombus cardioversion was not performed. In these patients both investigations were repeated after 8 weeks of therapy with warfarin. If initially detected mass decreased or disappeared at the background of indirect anticoagulants, it was considered to be a thrombus. In 1/3 of patients repeat examination allowed to reject initial diagnosis of left auricular thrombosis. Simultaneous application of TEE and MSCT detected more thrombi than the use of either of these methods. Rate of detection of thrombi in left atrial auricle with the use of both methods was 32%.

Abstract

Levels of fibrinogen, von Willebrand factor, d-dimer, antithrombin III, protein C, plasminogen, and plasminogen activator inhibitor were measured in 62 men and 37 women with ischemic heart disease before and after 20-min venous occlusion. Women compared with men had higher baseline levels of fibrinogen (4.046-/+0.1785 and 3.584-/+0.1591 g/l, respectively, p=0.021), von Willebrand factor (122.1-/+9.31 and 99.5-/+6.16%, respectively, p=0.035), plasminogen activator inhibitor (4.8-/+0.31 and 2.9-/+0.27 IU/l, respectively, p=0.009). Levels of antithrombin III, protein C, and plasminogen in women were higher than in men both at baseline (108.5-/+1.65 and 100.7-/+1.60 %, p=0.001; 129.1-/+2.91 and 107.2-/+3.79%, p=0.001; 113.6-/+2.13 and 104.1-/+1.89%; p=0.001, respectively) and after venous occlusion. There were no gender differences in dynamics of parameters of hemostasis during venous occlusion. Multifactorial regression analysis showed that gender was independently (of age, duration of hypertension, smoking, body mass index, and total cholesterol level) related to only antithrombin III and protein C levels.

Abstract

Using a mathematical methodology based upon the theory of pattern recognition, the adequacy of this methodology being confirmed by the corroborative results obtained with neuro-net non-parametric methods of analysis, it proved possible to discriminate reliably between generalized organism's statuses, as described with a big number of laboratory indices, of the inhabitants of different areas of a big industrial city which differ in levels of radioactive and non-radioactive environmental pollution.
