The FVIII binding site on von Willebrand factor (VWF) is located in the D’ (766-864) and D3 (1054-1060) regions of the VWF gene. The cysteine residues in the D’ domain form disulfide bridges within the D’ trypsin-inhibitor-like (TIL’) and E’ regions,and these are of critically importance for the binding between TIL’E’ and FVIII. We analyzed the molecular etiology and laboratory phenotype of von Willebrand disease (VWD) 2N patients reported in the literature and added personal experiences from three European VWF VWD Research Centers.

Проблематика. Одним із недоліків використання високих технологій є збільшення рівня несприятливого електромагнітного смогу. Тому актуальним є вивчення впливу зовнішнього електромагнітного поля (ЕМП), у межах широкого ряду частот, на органи та мозок людини. Мета. Моделювання впливу на мозок електричної індукції, яка виникає при проникненні ЕМП різної частоти в замкнену сферу. Методика реалізації. Математична модель впливу ЕМП на мозок людини розглянута в наближенні до моделі голови людини у вигляді декількох замкнутих у коло шарів, що мають різні електричну провідність і діелектричну константу. Ця замкнута сфера оточена непровідним нескінченним простором (σ0 = 0) з діелектричною константою ε0. Шкіра і кістки голови мають магнітну проникність вакууму μ0. Впливи на мозок подаються як індукція електричного поля, що виникає при проникненні змінного електромагнітного поля в замкнену сферу. Результати. Отримані результати цифрових обчислень для тришарової моделі голови показали, що індукція електричного поля в шарі мозку зростала на частотах 107–108 Гц зовнішнього ЕМП. Поширення у сфері електромагнітних полів з f ≤ 106 Гц залишає їх амплітуди незміненими незалежно від глибини проникнення. Затухання амплітуд виявляється тільки для частот 107 і 108 Гц. Висновки. Виявлені в моделі зміни електромагнітних хвиль на частотах 107–108 Гц можуть активувати параметри центральної нервової системи та мозку, що суттєво вплине на активність людини. Подальше вивчення впливу електромагнітних полів різної частоти дасть можливість ідентифікувати ступінь активації мозку, а також стресові, позитивні та негативні впливи зовнішнього ЕМП. Background. One of lacks of the use of high technologies is the megascopic level of unfavorable electromagnetic smog. Therefore, the study of influencing of the external electromagnetic field (EMF) is actual, within the limits of wide row of frequencies, on man's organs and brain. Objective. Modeling of influences on the brain of electric induction arising up at penetration of the EMF of a different frequency in the reserved sphere. Methods. A mathematical model of the EMF influence on human brain is considered. Namely, the model of a multi stratified sphere as an approximation of a human head is proposed. This sphere embedded into the unlimited nonconductive space (σ0 = 0) of the dielectric constant ε0. Skin and bones of head have a magnetic permeability of vacuum μ0. Influences on a brain are given as induction of the electric field, arising up at penetration of the variable electromagnetic field in the reserved sphere. Results. The results of numerical calculations for the three-layered model of head showed that induced of the electric field in the layer of brain has increased on the frequencies 107–108 Hz of external EMF. Distributions on the sphere of electromagnetic fields with f ≤ 106 Hz leave their amplitudes by unchanged regardless of depth of penetration. Fading of amplitudes shows up only for frequencies 107 and 108 Hz. Conclusions. Exposed in a model changes of electromagnetic waves on the frequencies 107–108 Hz can activate the parameters central nervous system and brain, that substantially will affect of man's activity. Will allow the further study of influencing of the electromagnetic field of a different frequency to identify to extent of brain activity, and also stress, positive and negative influencing of external EMF. Проблематика. Одним из недостатков использования высоких технологий является увеличение уровня неблагоприятного электромагнитного смога. Поэтому актуально изучение влияния внешнего электромагнитного поля (ЭМП), в пределах широкого ряда частот, на органы и мозг человека. Цель. Моделирование влияния на мозг электрической индукции, возникающей при проникновении ЭМП разной частоты в замкнутую сферу. Методика реализации. Математическая модель влияния ЭМП на мозг человека рассмотрена в приближении к модели головы человека в виде нескольких замкнутых в круг слоев, обладающих разными электрической проводимостью и диэлектрической константой. Эта замкнутая сфера окружена непроводящим бесконечным пространством (σ0 = 0) с диэлектрической константой ε0. Кожа и кости головы имеют магнитную проницаемость вакуума μ0. Влияния на мозг представляются как индукция электрического поля, возникающая при проникновении переменного электромагнитного поля в замкнутую сферу. Результаты. Полученые результаты цифровых вычислений для трехслойной модели головы показали, что индуцирование электрического поля в слое мозга возрастало на частотах 107–108 Гц внешнего ЭМП. Распространение в сфере электромагнитных полей с f ≤ 106 Гц оставляет их амплитуды неизмененными независимо от глубины проникновения. Затухание амплитуд проявляется только для частот 107 и 108 Гц. Выводы. Выявленные в модели изменения электромагнитных волн на частотах 107–108 Гц могут активировать параметры центральной нервной системы и мозга, что существенно повлияет на активность человека. Дальнейшее изучение влияния электромагнитного поля разной частоты позволит идентифицировать степeнь активности мозга, а также срессовые, положительные и негативные влияния внешнего ЭМП.

Background. As widely known, the geomagnetic field intensity does change abruptly during global magnetic disturbances specified as magnetic storms. Those are due to the changing solar activity influencing solar wind parameters and the state of the magnetosphere. This chain of physical processes with their origin in the Sun is described as the space weather. To understand the space weather effects in the environment is most important since those are related to the biosphere as well. Namely, the low-frequency geomagnetic field dy­namics, linked with space weather conditions, seems to be a quite probable factor influencing both the pathological and functional state of a human being and biological objects, in general, during magnetic storms.Objective. In this study, the dynamics of an oxygen saturation parameter in human arterial blood is analyzed with regard to extreme space weather when magnetic storms occur.Methods. Experimental setup for measuring changes of the relative concentration of oxyhemoglobin in arterial blood (saturation of arterial blood with oxygen), is a pulse oximeter-spectrophotometer. Measurements were carried out using pulse-oximetry sensor covering the first phalanx of a finger, and laser radiation was directed to the side of the second phalanx. The He-Ne laser with a wavelength of 632 nm was used as a source of electromagnetic radiation. Observational values of the geomagnetic field with a high-time resolution (1-sec) are obtained from the midlatitude Hurbanovo Geomagnetic Observatory (Slovak Republic). Deviations/oscillation ranges regarding the average value were calculated at all frequencies, and low pass filter was applied from the "Seewave-1.7.6" package (F-filter) for frequencies of 0.1, 0.5, 0.01, 0.05, 0.001, and 0.0001 Hz. The data on magnetic storms for 2015 were derived from the file of Kyoto Geomagnetic Center which indicated the dates and the amplitudes of magnetic storms on the Earth.Results. The following variables were analyzed: the average arterial oxygen saturation at the patient’s rest, peak-to-peak value of saturation, changes in the average oxygen saturation under laser radiation. A group of 33 persons was daily monitored from March 30, 2015, to May 25, 2015. In the presence of Earth's magnetic field disturbances, the average value of arterial oxygen saturation at the state of rest in 29 persons out of 33 was 93.9 ± 0.9%, peak-to-peak value of oxygen was 1.25 ± 0.24%, and the change in the average arterial oxygen saturation under laser radiation made up 2.34 ± 0.41%. In 4 persons under study the peak-to-peak value of saturation at rest did not differ from the case of the undisturbed geomagnetic field.Conclusions. The results of this study of some blood parameters, namely the oxygen saturation (and oxygenetion) value of the arterial blood and its dynamics due to exposed laser radiation in case of both the undisturbed and disturbed GMF, can be summarized as follows: the decrease of the blood oxygenation in case of the disturbed GMF (magnetic storm conditions) is quite apparent although more detailed statistical investigations along with biophysical explanations are needed; the presence of some experimentees in the observed group who have constant sweep of arterial blood oxygen saturation is, in our opinion, an aspect to explain the different frequency of tick attacks on various people although strict medical and epidemiological investigations are needed; the dynamics of the decrease of oxygen saturation in the arterial blood of human beings exposed to laser radiation, in persons with sensitivity to radiation, may be an indicator of the degree of influence of perturbations of the geomagnetic field., Проблематика. Известно, что напряженность геомагнитного поля резко изменяется во время глобальных магнитных возмущений, определяемых как магнитные бури. Это связано с изменяющейся солнечной активностью, влияющей на параметры солнечного ветра и состояние магнитосферы. Эта цепочка физических процессов с их происхождением на Солнце называется космической погодой. Важным является понимание влияния космической погоды на окружающую среду и биосферу. В частности, низкочастотная динамика геомагнитного поля, связанная с условиями космической погоды, представляется вполне вероятным фактором, влияющим на состояние человека и биологических объектов во время магнитных бурь.Цель. В данной работе исследуется динамика величины сатурации артериальной крови кислородом с учетом экстремальной космической погоды при возникновении магнитных бурь.Методика реализации. Экспериментальная установка для измерения величины относительной концентрации оксигемоглобина в артериальной крови (сатурации артериальной крови кислородом) представляет собой специализированный пульсоксиметр-спектрофотометр. Измерения проводились с помощью пульсоксиметрического датчика, покрывающего первую фалангу пальца, а лазерное излучение направлялось на вторую фалангу. В качестве источника электромагнитного излучения использовался He-Ne-лазер с длиной волны 632 нм. Значения геомагнитного поля с высоким временным разрешением (1 с) получены в Гурбановской геомагнитной обсерватории (Словацкая Республика). Сверхнизкочастотные составляющие геомагнитного поля (0,1, 0,5, 0,01, 0,05, 0,001 и 0,0001 Гц) получены с помощью программного пакета “Seewave-1.7.6”. Данные о магнитных бурях за 2015 год были получены из файла Киотского геомагнитного центра, в котором указаны даты и амплитуды магнитных бурь на Земле.Результаты. Были проанализированы следующие величины: средняя величина сатурации артериальной крови кислородом в состоянии покоя, размах колебаний величины сатурации, изменения средней величины сатурации под действием лазерного излучения. Мониторинг проводился ежедневно с 30 марта по 25 мая 2015 г. для группы из 33 человек. При наличии возмущений магнитного поля Земли среднее значение величины сатурации артериальной крови кислородом в состоянии покоя у 29 человек из 33 составляло 93,9 ± 0,9 %, размах колебания величины сатурации составил 1,25 ± 0,24 %, а изменение средней величины сатурации артериальной крови кислородом под действием лазерного излучения составило 2,34 ± 0,41 %. У 4 исследуемых размах значений сатурации артериальной крови в покое не отличался от случая невозмущенного геомагнитного поля.Выводы. Результаты исследования параметров крови, а именно величины сатурации (и значит, оксигенации) артериальной крови и ее динамики, обусловленной воздействием лазерного излучения в случае как невозмущенного, так и возмущенного геомагнитного поля, можно обобщить следующим образом: снижение оксигенации крови в случае нарушения геомагнитного поля (условия магнитной бури) вполне очевидно, хотя необходимы более подробные статистические исследования наряду с биофизическими объяснениями; присутствие нескольких людей в наблюдаемой группе, которые имеют постоянный размах величины сатурации артериальной крови кислородом, является, по нашему мнению, одним из факторов, объясняющих различную частоту нападения клещей на разных людей, хотя необходимы строгие медицинские и эпидемиологические исследования; динамика уменьшения величины сатурации артериальной крови кислородом под действием лазерного излучения у лиц, которые имеют чувствительность к излучению, может быть показателем степени влияния возмущений геомагнитного поля., Проблематика. Відомо, що напруженість геомагнітного поля різко змінюється під час глобальних магнітних збурень, які визначаються як магнітні бурі. Це пов’язано зі зміною сонячної активності, яка впливає на параметри сонячного вітру і стан магніто­сфери. Цей ланцюжок фізичних процесів з їх походженням на Сонці має назву космічна погода. Важливим є розуміння впливу космічної погоди на навколишнє середовище і біосферу. У 4 досліджуваних розмах значень сатурації артеріальної крові в спокої не відрізнявся від випадку незбуреного геомагнітного поля.Висновки. Результати дослідження параметрів крові, а саме величини сатурації (і отже, оксигенації) артеріальної крові та її динаміки, обумовленої впливом лазерного випромінювання у випадку як незбуреного, так і збуреного геомагнітного поля, можна узагальнити таким чином: зниження оксигенації крові в разі порушення геомагнітного поля (умови магнітної бурі) цілком очевидне, хоча необхідні більш детальні статистичні дослідження поряд з біофізичними поясненнями; присутність кількох людей у спостережуваній групі, які мають постійний розмах величини сатурації артеріальної крові киснем, є, на нашу думку, одним із факторів, що пояснює різну частоту нападу кліщів на різних людей, хоча необхідні суворі медичні та епідеміологічні дослідження; динаміка зменшення величини сатурації артеріальної крові киснем під дією лазерного випромінювання у осіб, які мають чутливість до випромінювання, може буди показником ступеня впливу збурень геомагнітного поля.

Development of factor IX (FIX) inhibitor is a rare but challenging complication in hemophilia B. In addition to inefficacy of specific replacement therapy, FIX inhibitors increase morbidity due to serious allergic reactions/anaphylaxis upon treatment with FIX. Limited experience with immune tolerance induction (ITI) shows a high risk of nephrotic syndrome development and poor ITI outcomes. Recently, immunomodulation therapy has been used in ITI regimens in hemophilia B; however, relevant guidelines for ITI in hemophilia B are still lacking. We describe a 7-year-old hemophilia B patient with "null" mutation Arg29 stop who underwent surgery and massive transfusion therapy in the neonatal period and developed an FIX inhibitor after consecutive 20 exposures to FIX concentrate. At the age of 6 years, a high-dose ITI was commenced combined with immunomodulation therapy including rituximab, dexamethasone, and intravenous immunoglobulin. Allergic reactions that occurred in the third week of ITI were resolved by premedication with antihistamines and continued immunomodulation protocol without any need for ITI interruption. Inhibitor was negative from week 10; however, doses of FIX continued unchanged until pharmacokinetic criteria for success were met at month 9 of ITI. One year after the start of ITI, the patient started regular prophylaxis with FIX 41 IU/kg three times a week. No further allergic reactions or any signs of nephrotic syndrome have occurred.

We present 25-year experience with inhibitors in previously untreated patients (PUPs) with severe hemophilia A in Slovakia, where safe factor VIII (FVIII) concentrates have been used since 1990. A prospective study focused on inhibitor incidence in PUPs was established in 1997. Out of a total 61 PUPs born between January 1997 and October 2015, 59 were eligible for evaluation; 50 and 9 were treated with > 20 exposure days (ED) of plasma-derived FVIII (pdFVIII) and recombinant FVIII (rFVIII) products, respectively. In the entire group 13/59 (22%) PUPs developed inhibitors; i.e. 7/50 (14%) and 6/9 (67%) treated with pdFVIII and rFVIII, respectively. Univariate analysis of inhibitor risk factors in patient groups with and without inhibitors showed the rFVIII and serious/recurrent infections within the first 50 EDs to be associated with inhibitor development (OR of 12.3 [95% CI 2.48–60.83; p = 0.002] and 5.0; [95% CI 1.16–21.9; p = 0.03), respectively]). Also, in multivariate Cox regression analysis, peak treatment ≥ 5 EDs reached statistical significance. The hazard ratio (HR) was 7.15 (95% CI 1.65–31.36) p = 0.0086 for rFVIII and 4.38 (95% CI 1.02–18.67) p = 0.046 for intensive treatment. Between 1993 and 2015, 21 immune tolerance inductions (ITIs) in 19 inhibitor patients were performed in the two largest hemophilia centers in Slovakia. In all but one ITI courses pdFVIII containing von Willebrand factor (FVIII/VWF) was used with preferred use of high-dose ITI (HD ITI) in high responders (HRs). Complete or partial success was achieved in 17/19 (89.5%) patients. Evaluating only the patients who already completed ITI, the success rate was even higher (15/16; 94%), including 7/7 low responders and 8/9 HR. Conclusion: Our national prospective study comprising entire group of PUPs with severe hemophilia A showed higher incidence of inhibitors in patients treated with rFVIII and those with intensive therapy within first 50 EDs. However, our experience is limited to small numbers of patients; thus, our results must be interpreted cautiously. High success rate of the ITI in our inhibitor patients has been achieved with FVIII/VWF concentrates and preferred use of HD ITI in HR patients.

The effects of slight atmospheric pressure fluctuations (APFs) within two ranges of periods attributed mostly to far infrasound (3 s–120 s) and internal gravity waves (120 s–1200 s) on human behaviour related to injury occurrences, within the one-year interval, are examined. Special interest is paid to the analysis of combined effects of APFs and geomagnetic activity (GMA) attributed to low and moderate levels. The relations between the daily number of emergency transport events due to sport injuries (EEI) and daily mean of APF integral amplitudes within the two ranges (DHAI and DHAG, resp.) along with the planetary geomagnetic index Ap are analysed using the regression models based on categorization. As shown, the high DHAI is a rather strong meteorotropic factor, being of relevance to increase in the incidence of sport injuries. The high DHAG appears to be of opposite sense on the background of low DHAI, promoting the decreased EEI number. The consideration of combined effects of the APF and GMA reveals that the negative effects of high DHAI are more pronounced in combination with low Ap levels. The results are discussed from the viewpoint of the necessity of further medico-meteorological studies using databases of most disturbed geophysical conditions.

Changing insights in the diagnosis and classification of autosomal recessive and dominant von Willebrand diseases 1980-2015

The mutual location of high-latitude electrojets, typical regions of the auroral luminosity and regions of auroral energy particle participations into the upper atmosphere under substorm conditions are considered. Three electrojets exist at high latitudes during substorm intervals: WE - westward electrojet, EE - eastward electrojet and PE – polar electrojet. Geomagnetic latitudes of the WE/EE and PE location vary depend on local time and magnetic activity level, respectively. It is shown that the WE is located within the limits of the auroral oval precipitation (AOP), the EE in the evening sector is located within the diffuse auroral zone (DAZ) and the PE near noon is located at the poleward AOP boundary shifting poleward with decreasing the magnetic activity level. The relationship of electrojets with different plasma domains in the magnetosphere is discussed. QC 20120322

There are shortly described results of the analysis of variations in the location and intensity of the auroral electrojets during magnetic storms and substorms using a numerical method for estimating the equivalent ionospheric currents based on data from meridian chains of magnetic observatories. It is shown that the westward electrojet adjoins to the polar electrojet located at cusp latitudes in the dayside sector. The association of electrojets with the field-aligned currents (FACs), namely Region 1 FAC and Region 2 FAC is considered. During intense disturbances a Region 3 FAC (accompanied with diffuse electron precipitation from the plasma sheet boundary layer) with the downward current was identified. The analysis of observational data is summarized in terms of 2D time-latitude distribution of electrojets at ionospheric altitudes. The magnetic field sawtooth variations generated during the storm main and early recovery phases are also discussed. To follow 3D currents in the magnetosphereionosphere system a clarified view of interrelated 3D currents and magnetospheric plasma domains is presented. QC 20120321
