Background/Aim: To analyse, from the perspective of adults with a history of sexual abuse, the impact of the experience on their general health and well-being. Material and Methods: The participants comprised 12 strategically selected informants (10 women), aged 19-56; all sexually abused during child- and/or adulthood. They were interviewed in-depth and encouraged to describe the impact of the sexual abuse on their daily lives. The consequences of sexual abuse on oral health have previously been reported. The interviews were recorded digitally, transcribed verbatim and analysed according to qualitative content analysis with an inductive approach. Results: The overall theme illustrating the latent content was Sexual abuse experience-limiting long-term consequences, always present in body and mind. The first category covering the manifest content was 'A lost foothold', with two subcategories: (i) emotional repercussions and (ii) physical health repercussions. The second category was 'The significance of distance to trauma', with two subcategories (i) keeping a distance and tending to escape and (ii) processing the trauma experience-a struggle towards balance. Conclusions: A history of sexual abuse cannot be understood in isolation: the long-term repercussions pervade daily life. However, after disclosure of the abuse and processing the trauma, it is possible to ameliorate some of the negative effects. In this context, the dental setting emerges as a potentially important venue for disclosure as almost every person, sooner or later, visits the dentist.

Barn och unga upp till 18 år som lever i familjer där barnets förälder eller någon annan vuxen som barnet varaktigt bor tillsammans med har en psykisk störning eller funktionsnedsättning, har en allvarlig fysisk sjukdom, har missbruksproblem, utsätter barnet eller en närstående för våld eller oväntat avlider omfattas av begreppet barn som anhöriga. Barnen har en förhöjd risk att utveckla egna psykosociala eller psykiska problem. Därför är det viktigt att hälso- och sjukvården beaktar deras behov av information, råd och stöd. Sedan drygt tio år sedan finns en bestämmelse i Hälso- och sjukvårdslagen om detta (HSL 2017:30, 5 kap. 7§). Primärvården har unika förutsättningar för att identifiera och stödja dessa barn, men forskningen inom området är hittills mycket begränsad, både nationellt och internationellt. Studiens övergripande syfte var att följa upp en tidigare kartläggning av kliniskt verksamma medarbetares kunskapsläge och arbete med barn som anhöriga samt att ta fram ett forskningsbaserat kunskapsunderlag för Region Värmlands fortsatta arbete med barn som anhöriga i primärvården där medarbetares erfarenheter tas tillvara. 133 kliniskt verksamma medarbetare inom primärvården i Region Värmland deltog i en digital enkät. Endast två tredjedelar av deltagarna angav att de känner till bestämmelsen och ännu färre kände till Region Värmlands rutin VIDA RUT-26224 som reglerar arbetet med bestämmelsen på regional nivå. Efter att ha tagit del av bestämmelsens innehåll ansåg en majoritet av deltagarna att alla patientgrupper förekommer på vårdcentralen. Men endast hälften uppgav att de själva har träffat någon av dessa patienter under den senaste månaden. Av dessa hade en majoritet även frågat om patienten hade barn i hemmet, och cirka en tredjedel av dem som hade frågat uppgav att insatser för att möta barnets behov hade planerats eller genomförts. Aim To investigate the effectiveness of preventive interventions for 8-17-year-old children of patients diagnosed with depression, anxiety, or bipolar disorder. Methods Sixty-two families including 89 children received either the more extensive Family Talk Intervention (FTI; n = 35), the brief Let's Talk about Children (LTC; n = 16), or Interventions as Usual (IAU; n = 38) in routine care in adult psychiatry. Parent-rated questionnaire data were collected at baseline, after 6 and 12 months. We used growth curve models to investigate the effect of intervention on child mental health problems (SDQ-P Total Difficulties) and perceived parental control of child behaviour (PLOC-PPC). Results Parents in the FTI and LTC groups, versus the IAU group, reported more favourable development in terms of preventing increase in child mental health problems with standardised intervention effects of d = -0.86 and -0.88 respectively, by study end, and reported improved perceived parental control, d = 1.08 and 0.71, respectively, by study end. No significant differences in effect were found when FTI and LTC were compared. Conclusions The results support continued use of FTI and LTC in adult psychiatry, and since LTC is a brief intervention, it might be useful as a minimum-level preventive intervention.

The aim of this study was to examine the association between watching pornography and poor mental health in three repeated cross-sectional surveys in Sweden (2004, 2009, 2014) among high school seniors (13,277 students) with an average age of 18 years. The same index questions concerning ever having watched pornography and the frequency of watching pornography during the last year were combined with three different measures of psychological health and background control variables in multiple logistic regression and forward stepwise logistic regression models. The repeated cross-sectional surveys did not find any consistent associations across years between poor mental health and ever having watched pornography or the frequency of watching pornography. Having watched deviant pornography (containing violence, children and/or animals) was associated with poor mental health among boys in two surveys but only in one survey among girls. Other characteristics, such as mother's unemployment (especially boys), parenting style (especially high controlling parents among boys) and experiences of sexual abuse (especially penetrating abuse among girls), were more consistently and strongly associated to poor mental health across the three surveys. This study stresses the importance of controlling for multiple background variables when studying the association between watching pornography and mental health, since the association might primarily be explained by underlying confounding variables.

BACKGROUND: One in ten children have a parent diagnosed with a mental illness by specialised psychiatric services. Severe parental mental illness is a well-established risk factor for children's mental health problems, making the identification and support of these children a public health concern. This study investigated the mental health and family context of children of parents diagnosed with depression, anxiety, or bipolar disorder in this clinical setting. METHODS: Parental reports on 87 children aged 8-17 years were analysed. The children's mental health was compared with that of a Swedish population-based sample. Multiple linear regression was used to investigate associations between child mental health and child gender, child age, parent symptoms and social status, family functioning, and perceived parental control. Furthermore, a cumulative risk index explored the effect of multiple risk factors on child mental health. RESULTS: The children reportedly had significantly more mental health problems than did the population-based sample and about one-third had scores above the clinical cut-off. A significant multiple linear regression explained 49% of the variance in child mental health, with lower perceived parental control and younger child age being associated with more child mental health problems. With more reported risk factors, children reportedly had more mental health problems. CONCLUSIONS: The results underline the importance of identifying a patient’s children and assessing multiple relevant risk factors in the child’s life. Furthermore, the results indicate that the needs of younger children and of patients in their parenting role are important to address.

Eleven participants (10 women), 19 to 56 y of age, who had experienced sexual abuse as children were purposively selected and interviewed in-depth. The participants were encouraged to describe how they perceived the effect of the sexual abuse on their oral health as adults. The interviews were recorded digitally and transcribed verbatim. The collected material was analyzed according to qualitative content analysis. The theme "challenging conditions for maintaining oral health" was identified, comprising 2 categories: first, "the emotional significance," with the subcategories 1) emotional barriers and 2) powerful relief, and second, "the obstacles to oral health," with the subcategories 1) daily self-care with complications and 2) dental appointments with difficulties. The findings indicate that the experience of sexual abuse during childhood can have a negative impact on oral care in adulthood. The informants stated that oral health was of utmost importance but also associated with strong emotions. There were obstacles to maintenance of oral health that were difficult to surmount.Knowledge Transfer Statement:The study provides access to the attitudes of survivors of child sexual abuse regarding oral health and the needs and obstacles that they experience. This is important knowledge for dental professionals to optimize dental care.

The objective was to investigate the validity and reliability of a new instrument assessing sexual harassment at a public university in Sweden. In-depth interviews and focus group discussions resulted in a 10-item instrument, the ‘Lund University Sexual Harassment Inventory’ (LUSHI). A survey was sent to all staff, including PhD students, and students, with a response rate of 33% (n = 2736) and 32% (n = 9667), respectively. Exploratory factor analysis and Cronbach’s alpha statistics were applied. Having experienced one or more of 10 specific behaviors was defined as sexual harassment exposure and was reported by 17.1% of staff/PhD students and 21.1% of students. Exploratory factor analysis yielded two factors with Eigenvalues above 1, labeled ‘unwanted sexual attention of soliciting type’ and ‘unwanted sexual attention of non-soliciting type’. Rape/attempted rape fell outside of the two factors. The Cronbach’s alpha values of the original 10-item scale and of the two newly formed scales were 0.80, 0.80, and 0.66, respectively. The mentioned statistics were markedly similar among men, women, and non-binary individuals and between staff/PhD students and students. We conclude that the 10-item instrument could be used for assessing sexual harassment in university settings or any type of workplace.

Background: Sexual harassment (SH) in the workplace is prevalent and associated with poor health. Universities are large workplaces with complex formal and informal power relations, which may influence the prevalence of SH. Although employees and students share the university context, few studies on SH have included both groups. The overall aim of the study was to investigate SH among employees and students at a large Swedish public university regarding types of harassment, prevalence in different groups, characteristics of the perpetrators, and the circumstances in which it occurs. Methods: A cross-sectional analysis was performed, based on a web-based survey with 120 items that was sent out to all staff, including PhD students (N = 8,238) and students (N = 30,244) in November 2019. The response rate was 33% for staff and 32% for students. Exposure to SH was defined as having experienced at least one of ten defined SH behaviors during their work or studies. Results: Among women, 24.5% of staff and 26.8% of students reported having been exposed to SH. The corresponding figures were 7.0% and 11.3% for male staff and students and 33.3% and 29.4% for non-binary individuals among staff and students. Unwelcome comments, suggestive looks or gestures, and ‘inadvertent’ brushing or touching were the three most common forms of reported harassment, both among staff and students. Attempted or completed rape had been experienced by 2.1% of female and 0.6% of male students. Male and female perpetrators were reported by about 80% and 15%, respectively, of exposed participants. Among staff most reported events occurred during the everyday operation of the university, while among students the majority of the events took place during social events linked to student life. When exposed to a perpetrator from the same group (staff or students), women reported more often being in a subordinate power position in relation to the perpetrator. All had experienced sexual abuse as children or adults and memories of this abuse had been triggered and expressed during a dental appointment. They were encouraged to relate in their own words their experiences of the dental appointment. The interviews were recorded digitally, transcribed verbatim, and analysed according to Qualitative Content Analysis. The overall theme illustrating the latent content was The dental appointment - a volatile base requiring predictability and a secure working alliance. The first category covering the manifest content was The dental care provider "assumes responsibility," with two subcategories: (i) contradictory disclosure, and (ii) alliance formation - a levelling of power. The second category was The patient is "in focus," with two subcategories: (i) alertness to signs of discomfort, and (ii) attention to obvious but subtle expressions of needs. On an understanding that the patient has been sexually abused, an individually tailored, patient-centered approach to treatment is suggested. Dental care providers may also need to be aware of and reflect on their position of power, in relation to the patient and its possible chairside implications.

The participantscomprised 13 sexually abused individuals (11 women), who recalled and expressedthese experiences during a dental appointment. They were encouraged to describe,in detail, aspects of the appointment which triggered memories of the sexual abuse.The interviews were recorded, transcribed verbatim, and analyzed using QualitativeContent Analysis. The identified overall theme illustrating the latent content was‘An echo of sexual abuse transformed into (dys) functional reactions’. The first categorycovering the manifest content was ‘The inner invisible struggle’, with two subcategories:(i) mental inscriptions of the abuse experience; and (ii) consequences ofthe dental encounter. The second category was ‘The discoverable manifestations’,with two subcategories: (i) enigmatic communication; and (ii) expressions of bodilymemories. The dental appointment arouses similar psychological stressful reactionsas the episodes of abuse; both implicit and explicit expressions are recognizable.Dental staff can contribute to disclosure by improved understanding of the strain adental appointment can cause in patients who have been subjected to sexual abuseand familiarity with the associated bodily expressions.

Background The current study aimed at exploring adolescents’ experiences of online sexual contacts leading to online sexual abuse by a perpetrator whom the victim had first met online. Associations with socio demographic background, experience of abuse, relation to parents, health and risk behaviors were studied. Methods The participants were a representative national sample of 5175 students in the third year of the Swedish high school Swedish (M age = 17.97). Analyses included bivariate statistics and stepwise multiple logistic regression models. Results In total 330 (5.8%) adolescents had gotten to know someone during the preceding 12 months for the purpose of engaging in some kind of sexual activity online. Thirty-two (9.7%) of those, the index group, had felt that they had been persuaded, pressed or coerced on at least one occasion. Sexual interaction under pressure was seen as constituting sexual abuse. These adolescent victims of online sexual abuse, the index group, did not differ with respect to socio-demographic background from the adolescents without this experience, the reference group. The index group had significantly more prior experiences of different kind of abuse, indicating that they belong to a polyvictimized group. More frequent risk behavior, poorer psychological health, poorer relationships with parents and lower self-esteem also characterized the index group. Online sexual abuse, without experiences of offline abuse, was associated with a poorer psychological health, at least at the same level as offline sexual abuse only. Conclusions The study made clear the importance of viewing online sexual abuse as a serious form of sexual abuse. Purpose: To compare experiences for adverse events, especially sexual abuse, and mental health in a group of high school students in out-of-home care with a representative sample of peers of the same age and similar educational attainment living with their parents. Materials and methods: A sample of 5839 students in the third year of Swedish high school, corresponding to a response rate of 59.7%, answered a study specific questionnaire. Data from 41 students living in out-of-home care were compared with data from peers not in out-of-home care in a cross-sectional analyze. Results: Students in out-of-home care had more often an immigrant background and a non-heterosexual orientation, had more often experienced physical and penetrative sexual abuse, and more often sought healthcare for mental problems. Disclosure of sexual abuse was less common, and acts of persuasion or adults' use of their social position was more common among students in out-of-home care. Conclusions: Even where the protective factor 'senior educational attainment' is present, risks for abuse and poor mental health are evident for adolescents in out-of-home care. Disclosure of adversity, when it has occurred, ought to be higher among these adolescents with regular contact with social services, but our findings indicate tendencies for the opposite. We therefore suggest routines to be established to screen for adverse life events and mental health actively, along with general and systematic assessments of adversity and mental health during care.

Differences and similarities were studied in the functions of two different self-injurious behaviors (SIB): nonsuicidal self-injury (NSSI) and sex as self-injury (SASI). Based on type of SIB reported, adolescents were classified in one of three groups: NSSI only (n=910), SASI only (n=41), and both NSSI and SASI (n=76). There was support for functional equivalence in the two forms of SIB, with automatic functions being most commonly endorsed in all three groups. There were also functional differences, with adolescents in the SASI only group reporting more social influence functions than those with NSSI only. Adolescents reporting both NSSI and SASI endorsed the highest number of functions for both behaviors. Clinical implications are discussed, emphasizing the need for emotion regulation skills.

So far, little is known about children in Sweden who are at risk of becoming young carers. The aim of this article is therefore to explore the extent and impact of children's caring activities as reported in a pilot study by a sample of children in Sweden. A number of international questionnaires measuring the amount of caring activities, impact of caring, quality of life, and psychological well-being were translated and combined into a survey. The pilot survey was completed by 30 children 10-18 years of age. Also, when completing the survey, the children were interviewed concerning their experiences of caregiving. The participants report on a group level emotional symptoms such as fear and nervousness above the clinical cut-off value. They also rate a lower level of caring compared with findings from the United Kingdom, but they report a higher degree of negative impact of caring than young carers in the United Kingdom.
