A Teoria Crítica contemporânea apresenta um curioso movimento. Ainda que, por um lado, solicita as categorias de Marx para dar contra do fenômeno recente da crise do capitalismo neoliberal, ao mesmo tempo, reconhece que essa teoria não pode dizer tudo, exigindo uma abordagem de sua perspectiva em relação a objetos e problemas outros, talvez obstruída pelos limites dentro dos quais seu pensamento teria se movimentado. Uma das fontes de inspiração mais poderosa deste movimento de ampliação, cujo saldo não é senão uma verdadeira heteroglossia filosófica, se encontra na teoria do espírito do idealismo alemão, mais precisamente em suas modulações hegelianas. O retorno a Hegel seria motivado pela necessidade de investigar, como se observa na obra de Axel Honneth, os dilemas dos vínculos humanos em um mundo social marcado pelo fenômeno da fragmentação. Neste artigo dos deteremos em analisar alguns elementos de um recente debate no interior da Teoria Crítica contemporânea sobre o problema da articulação entre reconhecimento e liberdade, reconstruindo as diferentes estratégias de leitura de Hegel que aqui são postas em movimento

Objective: To assess the effectiveness of Platelet Concentrates (PCs) in the contest of Hemorrhagic, Actinic, and Radiation Cystitis, plus Urethral Obstruction or Stenosis. Eligibility criteria: Open article in English or Italian regarding in situ applications of PCs for the selected pathologies. Information sources: MEDLINE, Cochrane Library, and ELSEVIER. Risk of bias: High (and discussed). Methods for synthesis of results: Selection of relevant contents, resumed by digital tools, checked by authors and used throughout the manuscript. Included studies: 13 screened articles + 7 personal sources + 37 “extra” articles. Synthesis of results: Pre-clinical and clinical studies demonstrated substantial symptom relief, mucosal restoration, and improved growth factor levels, reducing recurrence rates and complications. However, preparation protocols and results varied among studies. Limitations of evidence: Frequent low-quality studies with mall sample size, plus heterogeneous experimental setups and nomenclature/preparations. Interpretation: PCs demonstrate promise due to their bioactive components, enhancing tissue repair and reducing inflammation with no significant adverse events. Despite positive outcomes in pre-clinical and clinical studies, variability in preparation protocols and small sample sizes, together with inconsistent results, highlight the need for high-quality research to validate PCs’ clinical efficacy and cost-effectiveness.

Introduction Depressive symptoms are a common occurrence in people suffering from schizophrenia spectrum disorders (SSDs), representing a separate domain that interacts in peculiar ways with positive and negative symptoms. Nonetheless, available evidence on the relationship between depression and key clinical dimensions of SSDs is limited. Objectives To increase the knowledge regarding depression in SSDs, we performed a cross-sectional study aimed to investigate the association of depressive symptoms with overall, general psychopathology, positive, and negative symptoms in individuals with SSDs. Methods Adult people with SSDs were recruited from two psychiatric inpatient units in the northern area of the Metropolitan City of Milan from May 2020 to March 2023. Study participants with a Calgary Depression Scale for Schizophrenia score >6 were rated as depressed. Symptom severity was assessed by using the Positive and Negative Syndrome Scale (PANSS). Variables associated with depression at the univariate level were included into two multiple logistic regression models to analyse the association between depression and PANSS overall score as well as General Psychopathology, Positive, and Negative sub-scores. Objective: Evidence on the relationship between depression and clinical dimensions of schizophrenia remains limited. This cross-sectional study investigated the association between depression and Positive and Negative Syndrome Scale (PANSS) dimensions in people with schizophrenia spectrum disorders. Methods: Trained assessors administered the PANSS to measure symptoms of schizophrenia and the Calgary Depression Scale for Schizophrenia to measure depression. The association of depression with overall PANSS score and related dimensions was investigated in multiple logistic regression analyses. Results: We included 231 inpatients with schizophrenia spectrum disorders (mean age: 42.4 (SD: 12.9) years; men: 58.9%; mean overall PANSS score: 82.5 (SD: 20.1); drug-free or naïve: 39.3%), including 78 (33.8%) with clinically significant depressive symptoms. Depression was associated with higher overall (regression coefficient, SE: 0.029, 0.008; p < 0.001) and general psychopathology (regression coefficient, SE: 0.118, 0.023; p < 0.001) PANSS scores. We found an inverse relationship between depression and positive symptoms (regression coefficient, SE: -0.088, 0.028; p = 0.002). No association between depression and negative symptoms was found. Conclusion: Despite some limitations, our study shows that people affected by schizophrenia spectrum disorders with depression are likely to show more overall and general psychopathology symptoms but lower positive symptoms. Additional studies are needed to explore the generalizability of our findings.

Video cameras provide vital information on volcanic plumes from explosive eruptions, such as plume height, for monitoring and research. These images must be calibrated to get accurate quantitative data. However, the presence of wind complicates any calibration as the plume may no longer lie in the image plane, i.e. a plane perpendicular to the camera’s line-of-sight. Here, we present a simple new tool to correct for the effect of wind on the position and height of a volcanic plume as determined from imagery by rotating the image plane to be in the direction of the wind. We show the importance of accounting for the effect of wind on the maximum plume height determined from videos for two case-studies; a Vulcanian explosion from Sabancaya volcano, Peru, and a sustained plume from Mount Etna, Italy. This tool can improve the accuracy of quantitative information extracted from images of volcanic plumes, and should prove useful for both research and monitoring purposes.

Abstract Volcano-seismic signals can help for volcanic hazard estimation and eruption forecasting. However, the underlying mechanism for their low frequency components is still a matter of debate. Here, we show signatures of dynamic strain records from Distributed Acoustic Sensing in the low frequencies of volcanic signals at Vulcano Island, Italy. Signs of unrest have been observed since September 2021, with CO2 degassing and occurrence of long period and very long period events. We interrogated a fiber-optic telecommunication cable on-shore and off-shore linking Vulcano Island to Sicily. We explore various approaches to automatically detect seismo-volcanic events both adapting conventional algorithms and using machine learning techniques. During one month of acquisition, we found 1488 events with a great variety of waveforms composed of two main frequency bands (from 0.1 to 0.2 Hz and from 3 to 5 Hz) with various relative amplitudes. On the basis of spectral signature and family classification, we propose a model in which gas accumulates in the hydrothermal system and is released through a series of resonating fractures until the surface. Our findings demonstrate that fiber optic telecom cables in association with cutting-edge machine learning algorithms contribute to a better understanding and monitoring of volcanic hydrothermal systems.

In this article I attempt to systematically reconstruct Theodor Adorno’s account of the relationship between the processes of authoritarian subject formation and the processes of political formation of the democratic common will. Undertaking a reading that brings Adorno into dialogue with contemporary philosophical perspectives, the paper asks the question of whether it is possible to think of a “democratic We” in nihilistic times. In order to achieve this aim, I will analyze in reverse the modifications that the concept of narcissism has undergone, from Adorno’s use of it to account for the symbolic obstacles to the formation of democratic subjectivities after the Holocaust, to the initial formulations of Freudian psychoanalysis. Finally, I will attempt to outline an affirmative answer to the initial question, formulating the potentials and merits of what I will call a politics of negativity.

Brain tumors are a diverse collection of neoplasms affecting the brain with a high prevalence rate in people of all ages around the globe. In this pathological context, glioblastoma, a form of glioma that belongs to the IV-grade astrocytoma group, is the most common and most aggressive form of the primary brain tumors. Indeed, despite the best treatments available including surgery, radiotherapy or a pharmacological approach with Temozolomide, glioblastoma patients’ mortality is still high, within a few months of diagnosis. Therefore, to increase the chances of these patients surviving, it is critical to keep finding novel treatment opportunities. In the past, efforts to treat glioblastoma have mostly concentrated on customized treatment plans that target specific mutations such as epidermal growth factor receptor (EGFR) mutations, Neurotrophic Tyrosine Receptor Kinase (NTRK) fusions, or multiple receptors using multi-kinase inhibitors like Sunitinib and Regorafenib, with varying degrees of success. Here, we focused on the receptor tyrosine kinase AXL that has been identified as a mediator for tumor progression and therapy resistance in various cancer types, including squamous cell tumors, small cell lung cancer, and breast cancer. Activated AXL leads to a significant increase in tumor proliferation, tumor cell migration, and angiogenesis in different in vitro and in vivo models of cancer since this receptor regulates interplay with apoptotic, angiogenic and inflammatory pathways. Based on these premises, in this review we mainly focused on the role of AXL in the course of glioblastoma, considering its primary biological mechanisms and as a possible target for the application of the most recent treatments.

Introduction The existence of a clinical entity on the spectrum of mood disorders characterized by the occurrence of manic episodes without major depressive episodes (Unipolar Mania, UM) is largely debated. Although not classified nosologically, the studies exploring this topic have suggested that UM might differ from bipolar disorder with a manic-depressive course (md-BD), in terms of several clinical characteristics. Individuals with UM might represent a subpopulation with specific clinical profiles and unmet care needs, requiring personalized treatments, as compared with those suffering from md-BD. Objectives To identify clinical factors associated with UM, as compared with md-BD. Methods We performed a systematic review and meta-analysis of observational studies according to the MOOSE guidelines. We searched for articles indexed up to July 2022 in the main electronic databases. We conducted random-effects meta-analyses of the association between UM and relevant correlates, using odds ratio for categorical variables and standardized mean difference for continuous variables. Results Based on data from 21 studies meeting the eligibility criteria, we found that individuals with UM, as compared with md-BD, were more likely to be males (p = 0.007) and to have an earlier age at onset (p = 0.020). Moreover, UM was significantly associated with a higher number of hospitalizations (p < 0.001), the occurrence of psychotic features (p < 0.001), as well as hyperthymic temperament (p = 0.012). Finally, subjects with UM were less likely to report a family history of depression (p = 0.006) and a personal history of suicide attempts (p < 0.001). Conclusions Our work supports the hypothesis that UM might represent a distinctive diagnostic construct, with peculiar clinical correlates. BACKGROUND: Fibromyalgia is a debilitating condition marked by persistent pain and reduced functionality. Various therapeutic methods have been suggested to alleviate symptoms in individuals with fibromyalgia, yet the impact of diverse rehabilitation strategies remains unclear. OBJECTIVE: This systematic review and meta-analysis aimed at assessing the efficacy of rehabilitation interventions in improving functioning in fibromyalgia patients. METHODS: We conducted a comprehensive literature search of multiple international databases (PubMed, Scopus, and Web of Science) from their inception until November 22nd, 2023. We identified 23 randomized controlled trials (RCTs) assessing multiple rehabilitation strategies. The primary outcome was the Fibromyalgia Impact Questionnaire (FIQ). Study quality was assessed using the Cochrane Risk-of-Bias Tool for Randomized Trials (RoB 2). The study protocol was registered in PROSPERO (CRD42020197666). RESULTS: Our meta-analysis rehabilitation interventions significantly reduce FIQ scores (MD = - 11.74, 95% CI: - 16.88 to - 6.59, p < 0.0001). Notably, the subgroup analysis showed that different rehabilitation modalities seem to induce different therapeutic responses. CONCLUSIONS: Rehabilitation strategies hold promise in addressing the functional impairments and improving the overall well-being of individuals with fibromyalgia. The study underscores the need for further research to determine the optimal rehabilitation approach and its potential impact on the multilevel disability characterizing patients with fibromyalgia. Background/Aim: To evaluate the safety and efficacy of lattice radiotherapy (LRT) for large, inoperable breast cancers. Patients and Methods: In this prospective study, patients who underwent LRT for breast tumors that were ulcerating/fungating/extensively eroding the chest wall, and were ineligible/unwilling for surgery, were enrolled from May 2021 to Nov 2023. Baseline Eastern Cooperative Oncology Group (ECOG) status, pre- and post-LRT numerical rating scale (NRS), and post-LRT changes in quality of life (QoL) were recorded. Survival outcomes were reported at 6 months and 1-year. Median rates of survival and dosimetric parameters were calculated. Kaplan–Meier curves for overall survival (OS), cancer-specific survival (CSS), and failure of local control (LC) were constructed. Results: Ten patients (8 females) underwent LRT. The median age was 76 years (range=57-99 years) and the median ECOG performance status was 2.5 (range=1-4). The planned schedule was completed by 9/10 patients, accounting for a 90% compliance rate. Among patients with pain (n=7), NRS rapidly reduced from 7 (range=5-10) to 3 (range=1-6). The median equivalent uniform dose was 0.71 Gy (0.09-1.59 Gy). The actuarial rates of 6-month LC, CSS, and OS were 75%, 89%, and 61%, respectively, with only LC rate changing to 50% at 1 year. Two patients had local relapse at the six-month and 1-year follow-up, respectively, after having achieved a complete response at three months, and two others died of COVID-19 infection and ischemic stroke. Conclusion: LRT was found to be effective and safe in palliating symptoms among patients with large inoperable breast tumors. [ABSTRACT FROM AUTHOR]

This comprehensive review examines the unique attributes, distinctions, and clinical implications of ceftazidime–avibactam (CAZ-AVI) and meropenem–vaborbactam (MEM-VAB) against difficult-to-treat Enterobacterales infections. Our manuscript explores these antibiotics’ pharmacokinetic and pharmacodynamic properties, antimicrobial activities, in vitro susceptibility testing, and clinical data. Moreover, it includes a meticulous examination of comparative clinical and microbiological studies, assessed and presented to provide clarity in making informed treatment choices for clinicians. Finally, we propose an expert opinion from a microbiological and a clinical point of view about their use in appropriate clinical settings. This is the first review aiming to provide healthcare professionals with valuable insights for making informed treatment decisions when combating carbapenem-resistant pathogens.

Aims: To evaluate neurocognitive performance, daily activity and quality of life (QoL), other than usual oncologic outcomes, among patients with brain metastasis ≥5 (MBM) from solid tumors treated with Stereotactic Brain Irradiation (SBI) or Whole Brain Irradiation (WBI). Methods: This multicentric randomized controlled trial will involve the enrollment of 100 patients (50 for each arm) with MBM ≥ 5, age ≥ 18 years, Karnofsky Performance Status (KPS) ≥ 70, life expectancy > 3 months, known primary tumor, with controlled or controllable extracranial disease, baseline Montreal Cognitive Assessment (MoCA) score ≥ 20/30, Barthel Activities of Daily Living score ≥ 90/100, to be submitted to SBI by LINAC with monoisocentric technique and non-coplanar arcs (experimental arm) or to WBI (control arm). The primary endpoints are neurocognitive performance, QoL and autonomy in daily-life activities variations, the first one assessed by MoCa Score and Hopkins Verbal Learning Test-Revised, the second one through the EORTC QLQ-C15-PAL and QLQ-BN-20 questionnaires, the third one through the Barthel Index, respectively. The secondary endpoints are time to intracranial failure, overall survival, retreatment rate, acute and late toxicities, changing of KPS. It will be considered significant a statistical difference of at least 30% between the two arms (statistical power of 80% with a significance level of 95%). Discussion: Several studies debate what is the decisive factor accountable for the development of neurocognitive decay among patients undergoing brain irradiation for MBM: radiation effect on clinically healthy brain tissue or intracranial tumor burden? The answer to this question may come from the recent technological advancement that allows, in a context of a significant time saving, improved patient comfort and minimizing radiation dose to off-target brain, a selective treatment of MBM simultaneously, otherwise attackable only by WBI. The achievement of a local control rate comparable to that obtained with WBI remains the fundamental prerequisite. Trial registration: NCT number: NCT04891471.

Abstract Background The intensive care unit is increasingly recognized as a stressful environment for healthcare professionals. This context has an impact on the health of these professionals but also on the quality of their personal and professional life. However, there is currently no validated scale to measure specific stressors perceived by healthcare professionals in intensive care. The aim of this study was to construct and validate in three languages a perceived stressors scale more specific to intensive care units (ICU). Results We conducted a three-phase study between 2016 and 2019: (1) identification of stressors based on the verbatim of 165 nurses and physicians from 4 countries (Canada, France, Italy, and Spain). We identified 99 stressors, including those common to most healthcare professions (called generic), as well as stressors more specific to ICU professionals (called specific); (2) item elaboration and selection by a panel of interdisciplinary experts to build a provisional 99-item version of the scale. This version was pre-tested with 70 professionals in the 4 countries and enabled us to select 50 relevant items; (3) test of the validity of the scale in 497 ICU healthcare professionals. Factor analyses identified six dimensions: lack of fit with families and organizational functioning; patient- and family-related emotional load; complex/at risk situations and skill-related issues; workload and human resource management issues; difficulties related to team working; and suboptimal care situations. Correlations of the PS-ICU scale with a generic stressors measure (i.e., the Job Content Questionnaire) tested its convergent validity, while its correlations with the Maslach Burnout Inventory-HSS examined its concurrent validity. We also assessed the test–retest reliability of PS-ICU with intraclass correlation coefficients. Conclusions The perceived stressors in intensive care units (PS-ICU) scale have good psychometric properties in all countries. It includes six broad dimensions covering generic or specific stressors to ICU, and thus, enables the identification of work situations that are likely to generate high levels of stress at the individual and unit levels. For future studies, this tool will enable the implementation of targeted corrective actions on which intervention research can be based. It also enables national and international comparisons of stressors’ impact.

Between 2020 and 2022, more than sixty lava fountains occurred at Mt. Etna (Italy), which formed high eruption columns rising up to 15 km above sea level (a.s.l.). During those events, several ballistics fell around the summit craters, sometimes reaching touristic areas. The rather frequent activity poses questions on how the impact associated with the fallout of those particles, can be estimated. In this work, we present field data collected soon after the lava fountain on 21 February 2022. This event produced a volcanic plume of about 10 km a.s.l. which was directed toward the southeast. Several ballistics fell in the area of the Barbagallo Craters (just southeast of the summit area at around 2900 m a.s.l.), which is one of the most popular touristic areas on Etna. Hence, we collected several samples and performed laboratory analyses in order to retrieve their size, shape and density. Those values together with a quantitative analysis of the lava fountain were compared with results obtained by a free-available calculator of ballistic trajectories named the ‘Eject!’. A similar approach was hence applied to other lava fountains of the 2020–2022 sequence for which the fallout of large clasts was reported. This work is a first step to identifying in near real-time the area affected by the fallout of ballistics during Etna lava fountains and quantifying their hazard.

Volcanic plume height is one the most important features of explosive activity; thus, it is a parameter of interest for volcanic monitoring that can be retrieved using different remote sensing techniques. Among them, calibrated visible cameras have demonstrated to be a promising alternative during daylight hours, mainly due to their low cost and low uncertainty in the results. However, currently these measurements are generally not fully automatic. In this paper, we present a new, interactive, open-source MATLAB tool, named ‘Plume Height Analyzer’ (PHA), which is able to analyze images and videos of explosive eruptions derived from visible cameras, with the objective of automatically identifying the temporal evolution of eruption columns. PHA is a self-customizing tool, i.e., before operational use, the user must perform an iterative calibration procedure based on the analysis of images of previous eruptions of the volcanic system of interest, under different eruptive, atmospheric and illumination conditions. The images used for the calibration step allow the computation of ad hoc expressions to set the model parameters used to recognize the volcanic plume in new images, which are controlled by their individual characteristics. Thereby, the number of frames used in the calibration procedure will control the goodness of the model to analyze new videos/images and the range of eruption, atmospheric, and illumination conditions for which the program will return reliable results. This also allows improvement of the performance of the program as new data become available for the calibration, for which PHA includes ad hoc routines. PHA has been tested on a wide set of videos from recent explosive activity at Mt. Etna, in Italy, and may represent a first approximation toward a real-time analysis of column height using visible cameras on erupting volcanoes.

Background: Despite the impact of depression in terms of personal suffering and socioeconomic burden, most currently available treatment options are often ineffective. A particularly difficult-to-treat depressive disorder characteristic of the elderly is vascular depression, a late-life depressive syndrome related to a variety of potential vascular mechanisms. Transcranial Direct Current Stimulation (tDCS), a non-invasive and effective somatic approach to depression, also showed positive effects on cognitive deficits. Aim: We performed a double-blind randomized study to investigate the efficacy of tDCS as augmentation strategy to sertraline in the treatment of vascular depression, hypothesizing a positive effect in both depressive symptoms and cognitive functions. Methods: We enrolled 93 inpatients over 60 years of age with a diagnosis of vascular depression. Depressive symptoms were weekly assessed (T0, T1, T2) with the 21-items Hamilton depression rating scale (HDRS). Cognitive functioning was evaluated with the Milan Overall Dementia Assessment (MODA) at baseline and after the treatment protocol. All patients were randomly assigned into three groups, Group I: one tDCS stimulation per day, Group II: two tDCS stimulations per day, Sham group: one sham tDCS stimulation per day. En un diálogo que tuvo sitio recientemente en la ciudad de Lyon, Nancy Fraser y Luc Boltanski (2016) se formularon la siguiente pregunta: ¿cuál es el objeto de la actual Teoría Crítica de la Sociedad? Si de lo que se trata es de ofrecer medios conceptuales a las luchas sociales de nuestro tiempo, esto es: si queremos comprender el rostro que asume el sistema de explotación y dominación social contemporánea, inevitablemente nos topamos con la realidad del neoliberalismo. Pero al identificar nuestro presente como un tiempo signado por la forma social neoliberal, ¿no se pierde de vista su inscripción esencial en el patrón de acumulación capitalista? Los textos que componen este dossier presentan lecturas que se mueven alrededor de la inevitabilidad de este debate, toda vez que el pensamiento radical del siglo XXI no desiste de ofrecer una correcta caracterización de nuestro mundo social.

Volleyball players are often subject to micro-traumatisms of the heel fat pad and ankle injuries. Recently, mat-based proprioceptive training has assumed a key role in recovery from these disorders. Therefore, this proof-of-principle study aimed to assess the efficacy of proprioceptive mat training on plantar pressures and athletic performance in volleyball players. The participants included adult semi-professional volleyball players allocated into two groups: an experimental group, with mat-based proprioceptive and balance training, and a control group, with a sham protocol. For the outcome, we evaluated the barefoot plantar pressure, performing an analysis on a baropodometric resistive platform. The countermovement jump and squat jump were measured using an inertial measurement unit. Nineteen subjects were included in the two groups: the active proprioceptive group (n = 10) or the control group (n = 9). The results show a more uniform redistribution of loads with pressure hindfoot relief in the experimental group compared to the control group (p = 0.021, RBC = 0.67). Moreover, we observed a significant increase in peak landing force and high concentric power development in the experimental group compared to the controls. Focused proprioceptive management provided hindfoot load attenuation by stimulating higher peaks of concentric force in the experimental group compared to the sham group. Even though the study included a small sample, the results obtained in this proof-of-principle study suggest a positive role of proprioceptive stimulation in the inter-seasonal scenario for volleyball players to improve their jump performance and reduce the micro-traumatisms of the heel fat pad and the ankle injury rate. However, further studies performed on larger samples are needed to confirm these preliminary results.

Targeting cell cycle has become the gold standard for metastatic breast cancer (MBC), being cyclin-dependent kinase inhibitors (CDKIs) cornerstones of its treatment, alongside radiotherapy (RT). To date, no definite evidence regarding safety and efficacy of the combination of CDKIs plus radiotherapy (RT) is currently available. Purpose of this review is to collect data in favor or against the feasibility of the association of CDKIs + RT, describing its potential adverse events. Our review shows how CDKI + RT allows an overall satisfying disease control, proving to be effective and causing a grade of toxicity mainly influenced by the site of irradiation, leaning to favourable outcomes for sites as liver, spine or brain and to poorer outcomes for thoracic lesions or sites close to viscera; controversial evidence is instead for bone treatment. Toxicity also varies from patient to patient. To sum up, our contribution enriches and enlightens a still indefinite field regarding the feasibility of CDKIs + RT, giving cues for innovative clinical management of hormone-responsive MBC.
