Since the passage of the Wild Free‐Roaming Horses and Burros Act of 1971, federal agencies have been responsible for managing free‐roaming equids in the United States. Over the last 20 years, management has been hampered by direct opposition from advocacy groups, budget limitations, and a decline in the public's willingness to adopt free‐roaming horses (Equus caballus). As a result, free‐roaming equid numbers have increased to >3 times the targeted goal of 26,785 (horses and burros [E. asinus] combined), the cumulative sum of the appropriate management levels (AML) for all 177 designated herd management areas (HMA) managed by the Bureau of Land Management. This increase is one of the causes of greater sage‐grouse (Centrocercus urophasianus) population declines, owing to habitat alteration from free‐roaming equids exacerbated by ongoing drought. To evaluate potential demographic mechanisms influencing these declines, we compiled survival data from 4 studies in central Wyoming, USA, including 995 adult female (first‐year breeders or older) sage‐grouse during the breeding season, 1,075 nests, 372 broods, and 136 juveniles (i.e., overwinter survival for fledged young), from 2008–2022. During this period, we also obtained population information for free‐roaming horses from 9 HMAs used by individual grouse in our sample. Population estimates of free‐roaming horses for these HMAs ranged from 59% to 7 times of the maximum appropriate management level (AMLmax). Sage‐grouse monitored outside of HMAs represented control populations and, because we assumed they were not exposed to populations of free‐roaming horses, we set values of AMLmax to zero for all grouse located outside of HMAs. To evaluate whether free‐roaming horses were negatively affecting sage‐grouse, we modeled daily survival of breeding age females, nest, broods, and juveniles. There was strong or moderate evidence that overabundant free‐roaming horses negatively affected nest, brood, and juvenile survival. When horse abundance increased from AMLmax to 3 times AMLmax, survival was reduced 8.1%, 18.3%, 18.2%, and 18.2% for nests, early broods (≤20 days after hatch), late broods (>20 days to 35 days after hatch), and juveniles, respectively. These results indicate increasing free‐roaming horse numbers affected vital rates for important life stages of sage‐grouse, and that maintaining free‐roaming horse numbers below AMLmax would reduce negative effects to sage‐grouse populations. Besides the direct impact on the cardiovascular system, hypertension is closely associated with organ damage in the kidneys, liver, and pancreas. Chronic liver and pancreatic damage in hypertensive patients may be detectable via imaging.To explore the correlation between hypertension-related indicators and extracellular volume fraction (ECV) of liver and pancreas measured by iodine maps, and to evaluate corresponding clinical value in chronic damage of liver and pancreas in hypertensive patients.A prospective study from June to September 2023 included abdominal patients who underwent contrast-enhanced spectral CT. Normal and various grades of hypertensive blood pressure groups were compared. Upper abdominal iodine maps were constructed, and liver and pancreatic ECVs calculated. Kruskal-Wallis and Spearman analyses evaluated ECV differences and correlations with hypertension indicators.In 300 patients, hypertensive groups showed significantly higher liver and pancreatic ECV than the normotensive group, with ECV rising alongside hypertension severity. ECVliver displayed a stronger correlation with hypertension stages compared to ECVpancreas. Regression analysis identified hypertension severity as an independent predictor for increased ECV.ECVliver and ECVpancreas positively correlates with hypertension indicators and serves as a potential clinical marker for chronic organ damage due to hypertension, with ECVliver being more strongly associated than ECVpancreas. Objectives Methods Results Conclusion Point‐of‐care ultrasound (POCUS) use is widespread in EDs and throughout those practising medicine. Between institutions and specialities, there is widespread variety and training. With this comes the risk of patient harm and backlash to a clinically useful modality. Our objective is to form a statement that encompasses current published and unpublished guidance for creating and maintaining robust POCUS programs in EDs.Emergency Medicine Ultrasound Group (EMUG) identified this gap and volunteers from the group undertook a literature search of current best practice and institution guidelines relating to POCUS programs. They contacted colleagues from other specialities to find and get access to other countries and colleges' POCUS guidelines. EMUGs regularly run discussion forums (Collab‐labs) and points from these were considered. Recommendations were then formed from these and recurrent unpublished obstacles the group had encountered. The result was reviewed by clinical leaders in ultrasound and POCUS users in Australasia.The recommendations were organised under five pillars: Infrastructure, Governance, Administration, Education and Quality.These recommendations complement existing guidelines and are not intended to replace them; however, we hope to promote discussion and provide reference support for those developing POCUS programs. Implementing a comprehensive and robust ED POCUS program will ensure safe, effective and standardised high‐quality POCUS use, with the aim of improving patient care across Australia and New Zealand. Patient safety will be enhanced through effective risk management and quality assurance and there will be consistency in POCUS education, training and credentialing across institutions. Advocacy in breast cancer has a history going back eight decades, influencing funding for research, giving women a voice in their treatment, and helping destigmatise the disease and its management. We aim to outline this trajectory. This is a retrospective historical narrative. We have identified a number of prominent women diagnosed with breast cancer who have had significant political influence and describe the grassroots movement of patient advocacy groups who now have a seat at the policy table and are integral to the research agenda. We also report on how as surgeons we can advocate for better treatments and care pathways, and importantly help build the evidence‐base to inform better practice and policy. Purpose: This study aimed to evaluate changes in intraocular pressure following intravitreal dexamethasone implant injection, specifically in patients undergoing glaucoma filtration surgery. Methods: The degree of increase in intraocular pressure was compared retrospectively among three groups. Group 1 comprised patients who underwent prior glaucoma filtration surgery (54 eyes). Group 2 included patients with or suspected glaucoma without such surgical history (20 eyes). Group 3 included patients without glaucoma (33 eyes). Pressure measurements were taken before the injection and at 1, 2, 3, and 6 months post-injection. A subgroup analysis was performed for pressure > 35 mmHg, > 30 mmHg, > 25 mmHg, and a difference > 10 mmHg between the peak and baseline pressure. Results: Group 1 consistently displayed lower pressures compared with Group 2, with significant difference at both 1- and 6-month post-injections (15.09 mmHg vs. 18.10 mmHg, P = 0.042 and 13.91 mg vs. 17.25 mmHg, P = 0.040). The proportion of patients in Group 1 and Group 3 with pressures > 25 mmHg, > 30 mmHg, and a difference > 10 mmHg did not significantly differ (15.6% vs. 9.5%, P = 0.231; 3.1% vs. 2.3%, P = 0.867; and 17.1% vs. 7.1%, P = 0.231). Notably, Group 2 exhibited a significantly higher proportion within each category (> 25 mmHg, 24.0%; > 30 mmHg, 20.0%; > 10 mmHg difference, 28.0%). Conclusion: Intravitreal dexamethasone implant did not increase the risk of elevated intraocular pressure in patients with a history of glaucoma filtration surgery compared with patients with suspected glaucoma; the risk was similar to those without glaucoma. The European Parliament (EP) constitutes a prime site of contemporary party politics in Europe. Its seven political groups represent political interests and ideologies beyond specific national interests. Within the groups, national delegations exert considerable power. The research objective of this article is to generate new knowledge about the role of national delegations within political groups. The research material consists of 140 elite interviews with MEPs and staff. The data allow the article to analyse the construction of the role and the importance of national delegations and to focus not just on formal institutions, but also on informal ones which are often outside the reach of quantitative methods and data. The findings suggest that political groups can be divided into different categories. The role and influence of national delegations are strongly governed by informal institutions, in particular when it comes to distribution of leadership positions and policy making. The paper focuses on how illiberal politics in post-2010 Hungary have affected the political opportunities of advocacy groups in terms of influencing policymaking and how the latter have adapted their strategies to this challenge. Based on a series of interviews conducted with the representatives of advocacy groups as well as members of the central administration, the paper investigates the changing patterns of policy advocacy in three arenas: the venues of social dialogue, the arena of direct lobbying, and that of activism. The expectation of the research was that the undermining of social dialogue by the government would spur advocacy groups to relocate their activities into direct lobbying and activism. However, the general picture is of a decline in advocacy activity. A decade of illiberal politics has changed not only the political and institutional environment of advocacy groups but also the general social context. Faced with these challenges, Hungarian advocacy groups have not been able to come up with strategic innovations which would have redefined their objectives and means. As a theoretical contribution, the paper suggests that the concept of political opportunities should be broadened in order to account for the deep social transformations that are brought about by illiberal politics. In this paper we present the GDL Vulnerability Index (GVI), a new composite index to monitor and analyse the human components of vulnerability to climate change, natural disasters, and other kinds of shocks, for societies and geographic areas across the globe. The GVI is a simple and flexible index designed for use by experts as well as non-experts in the climate field, including researchers, (local) politicians, NGO's, journalists, advocacy groups and grassroot movements. The GVI is based on an additive formula that summarizes the essence of seven socioeconomic dimensions of vulnerability into a single number. This formula approach sets this index apart from other existing indices. Any person who knows the values of the underlying indicators can compute the vulnerability score of an area by filling in these values in the GVI formula. Validity tests show that the data-driven GVI measures the vulnerability dimensions coping capacity, adaptive capacity and susceptibility as well as major expert-based indices. This offers great prospects for use in situations where no other vulnerability information is available. The objective was to study a large, international, ethnically diverse population of patients with atopic dermatitis (AD) to support the creation of patient-centric recommendations for AD management. Qualitative data were generated from 45-min, 1:1 telephone interviews conducted across 15 countries in each patient's native language. Interviews explored the impact of AD on patients' lives, patients' most important symptoms, treatment expectations, and treatment decision-making. Participants were also questioned on their current knowledge of AD scoring systems and what was most important to include in these tools. In total, 88 adult patients (≥ 18 years old) receiving treatment for AD were recruited through a market research database, clinician referrals, and local advertising. All patients were screened to ensure a balanced and diverse sample in terms of age, gender, educational level, employment status, geographic location, and AD severity. Patients involved in market research or activities supporting advocacy groups within the previous 6 months or affiliated with or employed by pharmaceutical companies were excluded. AD had a substantial impact on patients' lives. Itch, skin redness, and dry/flaky skin were the most frequently reported symptoms, with > 75% of patients experiencing these symptoms every 1–3 days. Mental health issues were common and resulted in the greatest negative impact on patients' daily lives. Patients perceived clinicians to underestimate the burden of their AD. Patients had little awareness of AD scoring systems and indicated a preference for these to be more clearly incorporated in clinical practice. For an ideal scoring system, patients favored using a combination of patient-reported and clinician-reported outcomes to reflect disease burden and ensure consistency across all settings. This global study generated diverse patient perspectives on the disease burden of AD, their expectations of treatment, and their views on AD scoring methods. Although accumulated evidence has shown that green human resource management (GHRM) has crucial effects on employees' environmental behavior, there is limited research on the underlying mechanisms of the link. Drawing on social information processing theory, we build a multilevel model to test that how and when GHRM has benefits for employees' environmental behavior. For a sample of 866 frontline employees organized into 53 hotels collected from three data sources, we find that GHRM is positively associated with work group green advocacy, which in turn is positively associated with employees' environmental behavior. Moreover, managerial interpretations of environmental issues strengthen the positive effect of work group green advocacy on employees' environmental behavior. By referring to the smart city industry, this research studies how commercial firms gain legitimacy when their products aim to address grand challenges. Despite the reputation of giant technology companies and the overall legitimacy they enjoy in technology markets, exploiting opportunities in social contexts connected to grand challenges requires a societal‐oriented approach. Firms that engaged in smart cities initially approached cities with a business‐as‐usual approach, to be met with sharp criticism from local communities and pressure groups. In response, firms had to redraw their strategies to include communities in the process to stay close to local reality. This paper theorizes the process and highlights an Integrative Smart Technology Model (ISTM) to narrate how firms strategically include communities in the planning process and gain legitimacy for the technologies. Background: The European Resuscitation Council 2021 guidelines for haemodynamic monitoring and management during post-resuscitation care from cardiac arrest call for an individualised approach to therapeutic interventions. Combining the cardiac function and venous return curves with the inclusion of the mean systemic filling pressure enables a physiological illustration of intravascular volume, vasoconstriction and inotropy. An analogue mean systemic filling pressure (Pmsa) may be calculated once cardiac output, mean arterial and central venous pressure are known. The NEUROPROTECT trial compared targeting a mean arterial pressure of 65 mmHg (standard) versus an early goal directed haemodynamic optimisation targeting 85 mmHg (high) in ICU for 36 h after cardiac arrest. The trial data were used in this study to calculate post hoc Pmsa and its expanded variables to comprehensively describe venous return physiology during post-cardiac arrest management. A general estimating equation model was used to analyse continuous variables split by standard and high mean arterial pressure groups. Results: Data from 52 patients in each group were analysed. The driving pressure for venous return, and thus cardiac output, was higher in the high MAP group (p < 0.001) along with a numerically increased estimated stressed intravascular volume (mean difference 0.27 [− 0.014–0.55] L, p = 0.06). The heart efficiency was comparable (p = 0.43) in both the standard and high MAP target groups, suggesting that inotropy was similar despite increased arterial load in the high MAP group (p = 0.01). The efficiency of fluid boluses to increase cardiac output was increased in the higher MAP compared to standard MAP group (mean difference 0.26 [0.08–0.43] fraction units, p = 0.01). Conclusions: Calculation of the analogue mean systemic filling pressure and expanded variables using haemodynamic data from the NEUROPROTECT trial demonstrated an increased venous return, and thus cardiac output, as well as increased volume responsiveness associated with targeting a higher MAP. Further studies of the analogue mean systemic filling pressure and its derived variables are warranted to individualise post-resuscitation care and evaluate any clinical benefit associated with this monitoring approach. Political parties and interest groups play important roles in incorporating societal interests into democratic decision-making. Recent studies have analysed the causes of interactions between individual political parties and interest groups and of interest groups’ choices to seek access to one or more parties. Meanwhile, we know less about why some parties grant access to a range of interest groups while others focus on a few. This article theorises about party strategies and suggests that party salience – that is parties’ emphasis on different policy areas – affects the range of interest groups to which they grant access. I combine interest group survey data from six European democracies with data from party manifestos and use regression analyses to test this hypothesis. I find that parties provide access to multiple interest groups representing various interests in legislative politics and, above all, in policy areas that are salient to them. Thus, parties are more likely to be aware of diverse interests’ perspectives on policies in areas they prioritise. Introduction: Engaging families in behavioral health services is a high priority for juvenile justice (JJ) systems and family advocacy groups. Family-driven care (FDC) enhances family engagement and decision-making power in youth behavioral health services, ultimately, improving youth and family mental health and substance abuse outcomes. Despite the benefits, there is limited guidance on how to integrate FDC into behavioral health care within the JJ system. Therefore, the goal of this study is to understand factors that promoted adoption of FDC the JJ context. Methods: JJ staff and leadership across the state of Georgia participated in surveys and interviews to understand contextual implementation determinants related to the adoption of FDC. Between November 2021- July 2022, 140 JJ staff participated in the survey from 61 unique JJ organizations. In addition, 16 staff participated in follow-up key informant interviews to explain quantitative findings. Results: Based on a mixed methods analysis, JJ agencies were more likely to implement FDC if they had the following characteristics: (1) presence of site leaders that were strongly committed to family engagement, (2) a shared understanding that family engagement was a top priority, (3) staff training related to family engagement, (4) external partnerships with organizations that serve families, (5) a workplace culture that was supportive of innovation, and (6) presence of family engagement programs that were easier (or more feasible) for staff to implement. Discussion: This mixed methods study underscores the importance of strengthening these 6 inner and outer setting implementation determinants when preparing to integrate FDC into JJ systems. Critical systems heuristics (CSH) has been influential in the development of critical systems thinking. However, it is a relatively underutilised method compared with soft systems approaches such as soft systems methodology (SSM) and cognitive mapping. This may be in part due to the complexity of ideas underpinning CSH. Core ideas with which users must feel confident include boundary critique, coercion, emancipation, and "is" vs "ought to be" framings. These ideas were the subject of the early discourse surrounding CSH, which considered the role of boundary critique in systems research, the extent to which CSH could meaningfully address coercion, and the claims of CSH as an emancipatory approach. The purpose of this review is to provide clarity on these key concepts by reflecting on how they have been addressed in the CSH literature to date. We find that CSH has been applied in a range of problem contexts and is most frequently applied to address coercion or power asymmetries. CSH research is frequently associated with advocacy for marginalised groups, and we believe this is a natural extension of the methodological emancipation to which CSH aspires. In providing an overview of the key ideas underpinning CSH, we hope to lower the barrier to application for systems researchers and practitioners. [ABSTRACT FROM AUTHOR]

Advocacy groups employ narrative strategies and framing techniques to enhance societal support. However, to date, it remains unclear how the dynamics of narrative strategy evolve over time and what factors contribute to these shifts. To fill this research gap, we use the Narrative Policy Framework to analyze the competing advocacy groups' Facebook pages in Taiwan's same‐sex marriage debate from 2016 to 2019. The findings reveal that policy positions offer greater predictive power for the devil–angel shift, yet the utilization of conflict expansion and containment strategies is tied to policy issues, political events, and their intended target audience. Our findings also demonstrate that the phenomenon of policy narrative learning occurs when convergence of potential policy outcome is achieved. Furthermore, this study highlights the role of institutional mechanisms in advocacy through venue shopping, underscoring the contextual nature of advocacy groups' narrative strategies. These findings contribute to a more comprehensive understanding of the utilization of narrative strategy in addressing a contentious social issue. [ABSTRACT FROM AUTHOR]

Background and Aims: Six-monthly ultrasound surveillance for hepatocellular carcinoma (HCC) is recommended in patients with cirrhosis. Surveillance enhances early detection and improves survival. Yet, despite clear benefits, uptake remains low. We aim to identify and explore ways to overcome patient-related barriers to HCC surveillance with the aim of producing invitations for surveillance. Methods: Using the COM-B model of behaviour and a co-design process, we collaborated with patients, liver health charities and advocacy groups, to identify patient-related barriers to attending HCC surveillance. We performed qualitative thematic analysis of co-production workshops on HCC surveillance to develop information leaflets and surveillance invitations. Results: Twenty-eight participants attended five workshops. Fear of a serious diagnosis and stigma from healthcare professionals were highlighted as main patient-related barriers to attending surveillance appointments. Co-design was used to develop informative, user-friendly, non-judgemental invitations and information regarding HCC surveillance relevant to populations with cirrhosis. Conclusion: We identified potential patient barriers to surveillance uptake and developed patient facing material that directly addressed these barriers to be trialled in the clinic. Targeting patient-specific barriers may increase uptake of surveillance and therefore enhance early diagnosis. Plain Language Summary: Ultrasound surveillance for hepatocellular carcinoma (HCC) is recommended for those with chronic liver disease, but surveillance uptake remains low. Fear and concern about stigma from healthcare professionals are widespread and act as barriers to surveillance attendance. Through a co-design process, we designed invitations to surveillance alongside informational material, aiming to address the self-reported barriers and motivators to surveillance attendance. Pancasila as the basis of the Indonesian state has an important role in national and state life. However, along the way, Pancasila experienced various challenges and dynamics, including the emergence of the Pancasila Ideology Policy Bill (RUU HIP) which sparked pros and cons in society. This research aims to analyze the existence of Pancasila values in the Draft Law on Pancasila Ideology and formulate the public's response to the enactment of the resulting law. The method in the research uses a qualitative approach by collecting data through literature studies. After the data is collected, it then involves analysis techniques through three steps, namely simplifying information, presenting data findings, and drawing conclusions. The findings of the research show that supporters of this bill argue that aligning the law with Pancasila values will strengthen the state's ideological foundation and ensure that the nation's noble values become the basis for all activities. However, on the other hand, critics highlight several controversial aspects of this bill that the interpretation of the trisila in the HIP Bill is not in line with the version of Pancasila that is widely accepted by Indonesian society. In addition, the absence of an MPRS regulation regulating the dissolution of the PKI in the bill raises concerns that the removal of this reference could give the impression that the government does not recognize the history of the dissolution of the PKI, which is considered an important moment in Indonesian history. The ability to mobilise public opinion is central to interest group politics. Yet, whether and how groups succeed in swaying the public remains inconclusive. The article assesses this by conducting a field experiment in which a consumer group sent different versions of campaign material to a representative sample of over 5000 citizens. Relying on a two-wave panel survey, it shows that while the campaign affected intended consumer behaviour, it did not influence attitudes. Surprisingly, material by the organisation alone was more effective than material sent with a partner. Moreover, campaign references to personal experiences and facts were not more effective than material referring to public opinion. The findings challenge existing evidence on how sender and message characteristics affect the likelihood of influencing citizens. At the same time, they underline that public opinion is hard to change and have important implications for understanding political representation and interest groups in democratic politics. The commonality of problems surrounding a broad subject of audit shares similarities in the domain of public sector in developing countries. This paper presents the issues faced by public sector auditors in developing countries by analyzing the data collected through quantitative and qualitative means. The prime focus is to analyze the issues by identifying the prevailing causes of these problems through research conducted and literature reviewed. A total of 300 auditors, who work in the public sector under the direct and indirect supervision of Auditor General of Pakistan (AGP), are sampled for this research. The auditors are divided into two categories, which are Internal and External Auditors, that equally contribute to the function of accountability for the Supreme Audit Institution (SAI) of developing country like Pakistan. Pakistan is taken as a sample country for this research due to its similarities of audit procedures among SAIs of all developing countries. The auditors are categorized based on their age group, qualification and nature of work. Furthermore, a number of issues are identified, such as Staff Shortages, lack of Adequate Time, External Pressure Groups, Record Production Issues, lack of Digitalization, Concern of Personal Security and Lengthy Reporting Procedures. These issues are derived from the data collected in this research and are further analyzed to investigate any potential causes of bureaucratic and cultural nature that are also common among other developing countries. Furthermore, a comparison is also drawn and deliberated upon in this paper that focuses on the Issues faced by Internal and External Auditors of a Public Sector in a developing economy.

The foundation of the Child Poverty Action Group (CPAG) in 1965 is seen as a pivotal moment in the history of British welfare policy heralding a new wave of pressure group activism. Many histories of the organisation focus on the expertise of two of their founder members, Brian Abel-Smith and Peter Towsend. But in foregrounding the role of these academic experts in the early history of CPAG the wider ecosystem of professionals, academics, politicians and activists involved has been overshadowed. This insight this article will argue that contributions to welfare policy and practice can be subsumed in organisational histories, particularly work that is local, smaller scale and practical. Often though not exclusively, women have undertaken such grassroots research and activism. Such work can, and does, contribute to the success of voluntary groups as well as feed into wider intellectual and policy currents. This paper will explore one such case study of this: the crucial but under-documented work of three women campaigners Harriett Wilson, Audrey Harvey and Margaret Wynn. It will illustrate how by combining both intellectually rigorous social research and practical social action they were important both in the formation of CPAG and sociological thinking and practice more broadly. The terms “offshore” and “open ocean” have been used to describe aquaculture sites that are further from the coast or in higher energy environments. Neither term has been clearly defined in the scientific literature nor in a legal context, and the terms are often used interchangeably. These and other related terms (for example “exposed”, “high-energy”) variously refer to aspects of a site such as the geographic distance from shore or infrastructure, the level of exposure to large waves and strong currents, the geographic fetch, the water depth, or some combination of these parameters. The ICES Working Group (ICES, 2024) on Open Ocean Aquaculture (WGOOA) therefore identified a need to define the terminology to reduce ambiguity for these types of aquaculture sites or more precisely, to: (1) promote a common understanding and avoid misuse for different classifications; (2) enable regulators to identify the characteristics of a marine site; (3) allow farmers to be able to assess or quantitatively compare sites for development; (4) equip developers and producers to identify operational parameters in which the equipment and vessels will need to operate; (5) provide insurers and investors with the terminology to consistently assess risk and premiums; and (6) circumvent the emergence of narratives that root in different cognitive interpretations of the terminology in public discourse. This paper describes the evolution of the use of the term “offshore aquaculture” and define the most relevant parameters to shift to a more definitive and robust term “exposed aquaculture” that can inherently relay clearer information. Adoption of this more definitive definition of “exposed” will allow the user to define a site with more than just distance from shore. Key differences and the importance of these terms are discussed that affect various interest groups. Follow-up articles in this compilation from scientific members of the WGOOA as well as other scientists outside ICES are incorporated that develop a set of definitions and a rigorous exposure index. Petitioning provided a flexible repertoire for women’s political activity in the decades after 1945. Petitioning was a popular, widespread practice: aside from voting, signing a petition was one of the few political activities that engaged a majority of women. Building on a long tradition of British women as active petitioners, petitioning was used by a variety of different groups—local action groups, voluntary associations, political parties, pressure groups, and radical social movements, including those associated with the Women’s Liberation Movement. Connecting formal and informal politics, petitioning was a relatively cheap, accessible form of political activity that enabled activism. In particular, the practices associated with name-signing created informal, temporary political spaces for women. Petitioning also provided a mechanism for representation and making representative claims to authority on behalf of women. The media-friendly spectacle of presenting petitions to authority was useful for attracting publicity as a range of activists, including conservative moral campaigners like Mary Whitehouse realised. Religion is increasingly acknowledged as a driving force behind development processes; however, it has received little attention as a motivation for donors to give aid. The article analyses the stated motivations and hidden agendas behind the Hungarian government's 'Hungary Helps' aid programme, aimed at supporting Christian communities in the Global South and reducing migration to Europe. The programme fits with the government's rhetoric on religion and migration, however, the article reveals inconsistencies that suggest further motivations. While there is little evidence that there was domestic demand for such a programme, it has been used instrumentally to increase Hungary's influence among Christian interest groups and their political allies in the USA. To compare the differences in postoperative complications and prognosis between patients treated with neuroendoscopy versus conventional craniotomy surgery for hypertensive intracerebral hemorrhage (HICH). In this retrospective study, a total of 107 patients with HICH were included. Among them, 58 underwent neuroendoscopy (Group A), while 49 underwent conventional craniotomy under microscopic guidance (Group B). Intracranial pressure monitoring was applied in both groups. The clinical data, incidence of postoperative complications, preoperative and postoperative intracranial pressure values, and rate of favorable prognosis were compared between the 2 groups. No significant difference in baseline clinical data upon admission was observed between the 2 groups (P > 0.05). The preoperative intracranial pressure did not differ between the 2 groups (P > 0.05), but the postoperative intracranial pressure in Group A was significantly lower than that in Group B (P < 0.05). After intervention with the different surgical approaches, Group A showed a significantly lower incidence of postoperative cerebral infarction and a significantly higher rate of favorable prognosis compared with Group B (P < 0.05). Neuroendoscopy combined with Intracranial pressure monitoring is a safe and reliable approach for the treatment of HICH that reduces the incidence of postoperative cerebral infarction and improves the recovery of neurological function after surgery. Background: The number of cancer survivors has increased in recent decades, and the majority of them suffer from sequelae of their disease and treatment. This study, which is part of the larger research project OPTILATER, aims to explore different aspects of care services for long-term survivors (≥ 5 years after initial cancer diagnosis) in Germany. The study places an emphasis on the situation of people from different age groups, with different socio-demographic and cultural backgrounds, and sexually and gender diverse individuals. Methods: To investigate experiences related to follow-up care, focus groups (n = 2) will be conducted with members of patient advisory councils and advocacy groups, representatives of communities, healthcare workers and networks, as well as members of Associations of Statutory Health Insurance Physicians. Guided interviews will be carried out with patients and relatives (n = 40) to investigate needs, barriers and obstacles in terms of follow-up care. On this basis, additional focus groups (n = 2) will be carried out to derive possible scenarios for improving the consideration of needs. Focus groups and interviews will follow a semi-structured format and will be analysed content-analytically. Focus groups and interviews will be conducted online, recorded, transcribed, and analysed independently by two persons. Discussion: The qualitative approach is considered suitable because of the exploratory research aims. Diversity, equity, inclusion, and accessibility (DEIA) are foundational principles for clinical trials and medical research. In rare diseases clinical research, where numbers of participants are already challenged by rarity itself, maximizing inclusion is of particular importance to clinical trial success, as well as ensuring the generalizability and relevance of the trial results to the people affected by these diseases. In this article, we review the medical and gray literature and cite case examples to provide insights into how DEIA can be proactively integrated into rare diseases clinical research. Here, we particularly focus on genetic diversity. While the rare diseases DEIA literature is nascent, it is accelerating as many patient advocacy groups, professional societies, training and educational organizations, researcher groups, and funders are setting intentional strategies to attain DEIA goals moving forward, and to establish metrics to ensure continued improvement. Successful examples in underserved and underrepresented populations are available that can serve as case studies upon which rare diseases clinical research programs can be built. Rare diseases have historically been innovation drivers in basic, translational, and clinical research, and ultimately, all populations benefit from data diversity in rare diseases populations that deliver novel insights and approaches to how clinical research can be performed. Anti-feminist, anti-Queer politics, and Christianity have long been allies in South Korea fervently against any progressive movement involving women and sexual minorities. Since the 2010s, the societal context has shifted to include the long recession and neoliberal structural reforms after the 1997 Asian Financial Crisis. As a result, far-right religious groups never cease attempts to divide society based on gender, sexuality, and the Anti-Discrimination Law" To prevent "sexual orientation" from being protected under the Anti-Discrimination Act, these groups accused sexual minorities and members of advocacy groups of being pro-North Korea and pro-communist. The anti- LGBTQ groups furthered their discourse in the name of "protecting national security;" simultaneously, sexual minorities and family members of the shipwreck victims, migrant workers, and even disabled persons were treated as "non-nationals" and "pro-North Korea." This article explores the research question: How does the nature of interest group influence on school superintendents vary across school district locale/? Utilizing a survey of Wisconsin superintendents, the author tests several hypotheses related to the relationship between school district locale and superintendent perceptions of interest group influence. The results show that Rural/Town superintendents experience more overall interest group pressure, that less formal parent organizations are generally the most dominant interest groups in school districts, and that there are links between interest group pressures and performance. In liberal democracies, interest groups and political parties constitute the primary organizational carriers of citizens' preferences into the decision-making of the executive and legislative institutions. While political science research has put extensive efforts into understanding both channels of representation, their combined effect for citizen representation has only recently come into focus. In particular, we lack knowledge of the ideological alignment of citizens, parties and interest groups on overarching economic Left-Right and socio-cultural dimensions. We address this gap via an original cross-national survey of interest groups, which includes the self-placement of groups on the Left-Right and Gal-Tan dimensions. The configuration of groups on these dimensions are compared with Chapel Hill Expert Survey data on parties, and information on the preferences of citizens from the European Election Studies. Female Sudanese asylum seekers and refugees in Belfast were instrumental in establishing a women's collective, Anaka. The original goal was provision of a safe space where the founders could help women confronting the harshness of the asylum system alone and afraid, as they had been. The endeavour to mitigate the hard realities of a challenging system in a hostile environment led to the collective becoming an advocacy and campaigning group. The ethos was one of participation and empowerment, deploying skills, creative capacities and political insights forged in the fight for Sudanese democracy. The key issues confronting female refugees -- housing, health and education -- were equally those of the working-class communities in which they were placed. While politicians and mainstream media scapegoated new arrivals for societal deficiencies, through sharing and supporting the struggles of their neighbours, Anaka women established alliances with the potential to transcend racism and promote integration. Purpose: This paper analyses the economic and social impacts of implementing the European Accessibility Act (EAA). The study explores the trade-off between equality and efficiency as conceptualised by Arthur Okun. It evaluates how the EAA aims to enhance accessibility for persons with disabilities within the EU member states. Design/methodology/approach: This paper employs a theoretical framework based on Arthur Okun's concepts of equality and efficiency. The study reviews existing literature, including policy documents and academic research, to outline the EAA's assumptions and provisions. It also analyses the anticipated economic and social effects of the directive's implementation by examining relevant data and case studies. Findings: The study finds that while the EAA promotes greater accessibility and social inclusion for persons with disabilities, significant challenges and risks may limit its effectiveness. These include the economic burden on businesses and national authorities' need for comprehensive and timely implementation. However, the directive has the potential to drive innovation, enhance market competitiveness, and improve the overall quality of life for individuals with disabilities by providing equal opportunities and reducing social disparities. Originality/value: This paper contributes to the discourse on the intersection of economic efficiency and social equality by applying Okun's theoretical framework to the context of the EAA. The valve, armature, and armature pin are critical factors influencing the hydraulic pressure differences in diesel injectors, and are essential for injection and backflow quantity control. These components play crucial roles in enhancing energy efficiency and reducing engine emissions. This experimental study investigated the effects of clearance between the valve, armature, and armature pin guidance. Forty-nine 2000 bar common-rail injectors (Bosch) were tested in calibrated stations. Injection quantities were assessed at both minimum and maximum operational pressures. Backflow rates were specifically examined at maximum pressure. A correlation matrix was created using Python to analyze the relationship between inputs and outputs, identifying dominant characteristics that define injector behavior. Increased injector precision correlated with reduced fuel consumption and enhanced energy efficiency. The study found that the effect of clearance between the armature and armature pins was more significant than that between the valve and armature. Injection quantities were observed to increase with pressure, and no critical difference in injection quantities was noted among different diameter groups at the minimum pressure point. The article discusses the concept of the Responsibility to Protect (R2P) and its impact on international political discourse. While R2P has been hailed as revolutionary by some and criticized as a tool of Western imperialism by others, the author argues that it has had little effect on state behavior. Despite this, R2P has gained traction in international debates and has been supported by various organizations. However, the author contends that the support for R2P is largely symbolic and lacks consensus among governments. The article also examines specific cases, such as the violence against the Rohingya in Myanmar, to highlight the gap between affirmations of R2P and actual action taken. Background: People living with cancer, or carers who are from lesbian, gay, bisexual, transgender, queer, intersex or asexual (LGBTQIA+) communities experience unique information and support needs. Accessible technology-based resources providing tailored support are required to promote wellbeing, however this is a growing area of research requiring further investigation. The purpose of this study was to explore the experiences of healthcare services among people living with cancer, and their carers, who belong to sexual or gender diverse communities (LGBTQIA+), and identify how smartphone applications (apps) could support people from LGBTQIA + communities. Methods: This was a qualitative descriptive study where people living with cancer or carers from LGBTQIA + communities participated in phone interviews. Participants were recruited across Australia via social media advertisements, LGBTQIA + medical practices, and cancer advocacy groups. Participants were asked questions about their experiences, and were provided with screenshots of an existing app and asked to provide feedback on content and inclusiveness. Transcripts were coded and codes grouped together to form similar and concepts. Inductive and deductive analyses were used to create themes. Results: 13 patients (mean age 56 (SD:13)), and three carers (mean age 64 (SD:19)) completed phone interviews. The majority of participants identified their gender as female (patients n = 9, carers 3), and their sexuality as gay or lesbian (patients n = 10, carers n = 3). Four themes were created: (1) navigating disclosure in healthcare, described emotional challenges surrounding disclosure; (2) the power of positive experiences with clinicians, described positive interactions and gaps in care from clinicians; (3) impact of gender and sexuality on informal support, outlined support received from informal network and gaps in support, and; (4) opportunities to increase inclusivity in smartphone apps, generated ideas on how apps can be tailored to meet needs identified. Conclusion: Disclosure of gender or sexuality, and interactions with clinicians had the potential to impact participants' experience of cancer care. Gaps in informal networks pointed at how to better support LGBTQIA + communities, and identified opportunities for inclusion in an app that will be tailored and trialled for this community. Background: The purpose of this study was to compare the efficacy and safety of Ultra-mini-percutaneous nephrolithotomy (UMP) and Retrograde intrarenal surgery (RIRS) for renal/upper ureteral calculi in 1.0–2.0 cm diameter. Methods: From October 2017 to October 2022, the surgical treatment of patients with renal/upper ureteral calculi in 1.0–2.0 cm diameter who were admitted to our hospital was retrospectively analyzed. They were divided into two groups, the UMP group (sixty-two cases) and the RIRS group (one hundred and nine cases), according to the different surgical methods. Baseline data includes general information, stone size, location, CT value, hydronephrosis, creatinine level, etc. Results: Intraoperative blood loss was 33.6 ± 8.5 ml in the UMP group was significantly more than 4.3 ± 0.7 ml in the RIRS group (P < 0.05). Intraoperative renal pelvis pressure of UMP group 10.5 ± 1.3 mmHg was significantly lower than RIRS group 23.6 ± 5.6 mmHg (P < 0.05). The incidence of postoperative infection was higher in the RIRS group (thirteen cases [11.93%]), and one case ([1.61%]) in the UMP group (P < 0.05). Routine CT scans on the second day and two months after surgery showed that the stone clearance rates in the UMP group were 87.1% and 93.5%, respectively, higher than those in the RIRS group (69.7% and 79.8%, respectively; P < 0.05). Conclusion: UMP has the advantage of a higher stone-free rate but a higher risk of injury and bleeding. The advantages of RIRS include less trauma, less bleeding, and faster recovery, but lower stone-free rates and a higher risk of infection. [ABSTRACT FROM AUTHOR]

The patriation of the Constitution and the enshrinement of the Canadian Charter of Rights and Freedoms in 1982 opened a new "window of opportunity" for equity-seeking groups to better access legal arenas in Canada. They have been frequent interveners in Canadian courts within the post-Charter era, where they are perceived to have a heightened potential to influence public policy. However, whether this potential to enact change through the courts has been realized, or whether it has been realized to the same extent for all equityseeking groups, is disputed in the literature. Francophone minority communities in particular have been among the most active equity-seeking interveners, but their legal activism has been significantly understudied. To better understand the legal mobilization of Francophone minority communities, we conducted a mixed-methods analysis of the 35 language-rights cases heard by the Supreme Court of Canada between 1975 and 2020. From an empirical analysis of court transcripts and interest group documentation, as well as semi-directed interviews with key stakeholders, we find these groups to be distinct in a number of ways. Most prominently, the findings reveal that Francophone minority community organizations are strategically collaborative actors, and legal mobilization as a result is more sophisticated than what is typically assumed about equity-seeking groups. Over the past decade the problem of wage theft has burst onto the Australian policymaking scene, seemingly out of nowhere. Its meteoric rise, from a largely unrecognised issue to the broad public agenda, then onto the narrower governmental and decision agendas of state and federal governments, was a major feat in policy advocacy. It stands as a particularly outstanding example of the power of framing to change the salience and esteem of an issue. By tracing the journey of the wage theft issue through the different spheres of public and governmental attention, this research illuminates under-studied aspects of the re-framing and agenda-setting artforms and advances the study of advocacy group interventions in policy conflicts. Cyclin‐dependent kinase‐like 5 (CDKL5) deficiency disorder (CDD) is a developmental and epileptic encephalopathy caused by variants in the CDKL5 gene. The disorder is characterized by intractable early‐onset seizures, severe neurodevelopmental delay, hypotonia, motor disabilities, cerebral (cortical) visual impairment and microcephaly. With no disease‐modifying therapies available for CDD, treatment is symptomatic with an initial focus on seizure control. Another unmet need in the management of people with CDD is the lack of evidence to aid standardized care and guideline development. To address this gap, experts in CDD and representatives from patient advocacy groups from Denmark, Finland, France, Germany, Italy, Poland, Spain, and the United Kingdom convened to form an Expert Working Group. The aim was to provide an expert opinion consensus on how to ensure quality care in routine clinical practice within the European setting, including in settings with limited experience or resources for multidisciplinary care of CDD and other developmental and epileptic encephalopathies. By means of one‐to‐one interviews around the current treatment landscape in CDD, insights from the Expert Working Group were collated and developed into a Europe‐specific patient journey for individuals with CDD, which was later validated by the group. Further discussions followed to gain consensus of opinions on challenges and potential solutions for achieving quality care in this setting. The panel recognized the benefit of early genetic testing, a holistic personalized approach to seizure control (taking into consideration various factors such as concomitant medications and comorbidities), and age‐ and comorbidity‐dependent multidisciplinary care for optimizing patient outcomes and quality of life. However, their insights and experiences also highlighted much disparity in management approaches and resources across different European countries. Development of standardized European recommendations is required to align realistic diagnostic criteria, treatment goals, and management approaches that can be adapted for different settings. Plain language summary: Cyclin‐dependent kinase‐like 5 (CDKL5) deficiency disorder (CDD) is a rare condition caused by a genetic mutation with a broad range of symptoms apparent from early childhood, including epileptic seizures that do not respond to medication and severe delays in development. Due to the lack of guidance on managing CDD, international experts and patient advocates discussed best practices in the care of people with CDD in Europe. The panel agreed that early testing, a personalized approach to managing seizures, and access to care from different disciplines are beneficial. Objective: Recent reports have demonstrated that a wider pulse pressure upon admission is correlated with heightened in-hospital mortality following spontaneous supratentorial intracerebral hemorrhage (ssICH). However, the underlying mechanism remains ambiguous. We investigated whether a wider pulse pressure was associated with hematoma expansion (HE). Methods: Demographic information, clinical features, and functional outcomes of patients diagnosed with ssICH were retrospectively collected and analyzed. Multivariate logistic regression was conducted to identify independent predictors of HE. Weighted logistic regression, restricted cubic spline models, and propensity score matching (PSM) were employed to estimate the association between pulse pressure and HE. Results: We included 234 eligible adult ssICH patients aged 60 (51–71) years, and 55.56% were male. The mean pulse pressure was 80.94 ± 23.32 mmHg. Twenty-seven patients (11.54%) developed early HE events, and 116 (49.57%) experienced a poor outcome (modified Rankin scale 3–6). A wider mean pulse pressure as a continuous variable was a predictor of HE [odds ratios (OR) 1.026, 95% confidence interval (CI) 1.007–1.046, p = 0.008] in multivariate analysis. We transformed pulse pressure into a dichotomous variable based on its cutoff value. After adjusting for confounding of HE variables, the occurrence of HE in patients with ssICH with wider pulse pressure levels (≥98 mmHg) had 3.78 times (OR 95% CI 1.47–9.68, p = 0.006) compared to those with narrower pulse pressure levels (<98 mmHg). A linear association was observed between pulse pressure and increased HE risk (P for overall = 0.036, P for nonlinear = 0.759). After 1:1 PSM (pulse pressure ≥98 mmHg vs. pulse pressure <98 mmHg), the rates of HE events and poor outcome still had statistically significant in wider-pulse pressure group [HE, 12/51 (23.53%) vs. 4/51 [7.84%], p = 0.029; poor outcome, 34/51 (66.67%) vs. 19/51 (37.25%), p = 0.003]. Conclusion: Widened acute pulse pressure (≥98 mmHg) levels at admission are associated with increased risks of early HE and unfavorable outcomes in patients with ssICH. [ABSTRACT FROM AUTHOR]

The changes that occur over time are related to a change in the role and its members. The other significant reference groups are the friend group, the interest groups, the team at work. In each of them, we fulfill a given role, which we reconcile with our other roles. Along with them, there is another role that is very different in nature, essence and purpose from the others. This is the role of the consumer. The main purpose of the consumer role in human life is to satisfy our needs and wants by participating in a process of exchange. The goal of this article is to make a brief review of the theoretical concepts of the consumer, refracted through the prism of different scientific fields, which will serve to build a more multi-layered appearance of this concept. The evolution in the understanding of the concept of consumer has been traced, presented through a historical review of theoretical concepts and research. Modern interpretations of the role of the consumer in the conditions of a market economy are discussed. The model of a consumer in modern society is about an individual who has needs, seeking to satisfy them in a rational way, so as to improve his quality of life. The goal of organizations is to know the needs of consumers by providing them with the most suitable product. An overview of the theoretical statements about the concept of consumer from the positions of an interdisciplinary approach is made. The study of the consumer is subject of various scientific fields. This also allows for its deeper understanding. For one of the scientific fields, it is the focus of all the activities and that is marketing. According to marketing philosophy, people have different needs. This is also the starting point for marketing - the existence of needs that are subject to satisfaction. Placing the consumer and his needs at the heart of marketing requires that they must be studied. This can be achieved through marketing research. With the wide distribution of the Internet, prerequisites were created for the implementation of online marketing research. From the point of view of marketing, the consumer exhibits a certain behavior that is expressed in the decisions and actions he takes in the selection, purchase and way of using a given product. When do interest groups target legislators to approach or lobby under strong presidentialism? This article explores two hypotheses. First, interest groups and lobbyists seek to access committee chairs with gatekeeping power, even in those jurisdictions where the executive holds (most) exclusivity to initiate legislation (Gatekeeping Hypothesis). Second, interest groups and lobbyists target legislators who introduce more bills on issues that concern them but only on policy areas outside the executive's exclusive sphere (Legislative Activity Hypothesis). Background: Patients with complex rare disorders often require the care of multiple specialists. Effective coordination between primary and specialty care is needed to ensure patients receive high‑quality care. Previous research has documented the importance of primary care clinicians providing referrals to specialty care and the importance of specialists in helping patients reach a diagnosis. However, little is known about primary care clinicians’ roles in the ongoing care of patients with rare disorders. In the current study, we explored the role of primary care clinicians in the care of rare and complex vascular anomalies. Materials and Methods: Data were collected using semi‑structured qualitative interviews with 34 parents and 25 adult patients recruited from advocacy groups for patients with complex vascular anomalies participated. We asked participants about their diagnosis, care experiences, and communication with clinicians. We used thematic analysis to identify themes illustrating the roles of primary care clinicians. Results: PCC roles were characterized by four behaviors. Supporting behaviors included learning more about vascular anomalies and asking participants about the care they received from specialists. Facilitating included providing referrals, ordering tests, and engaging in problem‑solving. Interfering included failing to provide referrals or help participants coordinate care, ordering incorrect tests, or making inappropriate recommendations. Disregarding included focusing narrowly on primary care needs and not showing concern about the vascular anomaly. Conclusions: The results reveal opportunities to improve primary care for patients with vascular anomalies. Disregarding and interfering behaviors furthered the division between primary and specialty care for patients with vascular anomalies and prevented patients from receiving comprehensive primary care. Supporting and facilitating behaviors convey genuine interest in the care of the vascular anomaly and a commitment to helping the patient and parent. [ABSTRACT FROM AUTHOR]
