20. Invasive pulmonary aspergillosis: effects of early resection in a neutropenic rat model

Abstract

Objective: Invasive pulmonary aspergillosis is frequent in neutropenic patients. Usually localized in the beginning, the disease spreads and mortality is high despite antifungal treatment. The role of early adjuvant surgery is not clear. Surgery may help to confirm fungal disease, may control fungal disease locally and may prevent systemic spreading. This study examines effects of early resection on survival and dissemination in a rat model of localized invasive pulmonary aspergillosis. Methods: Forty persistently neutropenic male albino rats were challenged with standardized conidial aspergillus inoculum injected into peripheral lung tissue of the right upper lobe under direct vision. Animals were divided into four groups. Twenty animals were treated with amphotericin B at 1 mg/kg per day beginning 48 h after inoculation, 20 animals were left untreated. In each group half the animals underwent early resection of localized invasive aspergillosis by lobectomy. Animals were checked daily and mortality was recorded up to 28 days after which surviving animals were sacrificed. Results: Significantly higher survival was observed in resected animals in the non-Am B groups (survival: 10±19% without early resection and 50±32% with early resection; P=0.044). However, early resection did not lead to improved survival in animals treated with amphotericin B (survival 70±29% without early resection and 50±32% with early resection; P=0.316). Conclusions: In this rat model of localized invasive pulmonary aspergillosis effects of early resection on survival could be demonstrated only in animals not receiving amphotericin B treatment

Abstract

The purpose of this study is to introduce and bring into the clinical practice new screening tool Coin in the Hand Test for detection of simulating disability or weak results in psychological as sessment. Aim: The goal of this study was to define the maximal performance in a general and clinical population and to determine the extent to which participants in experimental group are capable of faking weakened performance so that this fraud would not be recognized. Material and methods: The test Coin in the Hand Test was studied on a non-psychiatric group of participants (N = 153) to determine the ability to adapt to three different scenarios: 1 = the best pos sible performance; 2 = acquiring disability pension; 3 = prolonging sick leave. In the second study the test was administered individually to a group of hospitalized psychiatric patients. Results: The performance score for each scenario significantly differed suggesting to a good ability of experimental group to adapt to the instruction. The mean performance score for the maximal performance was close to the maximum of 10 points (M = 9,98; SD = 0,13; 9-10), while for the serious faking it was the lowest (M = 4,87; SD = 2,37; 0-10). Score for moderate faking was found in between the previous two values (M = 6,85; SD = 1,72; 1-10). The results of the individual as ses sment done on a group of psychiatric patients cor responded with the results of the group with a scenario for the best possible performance. Conclusion: The presented very good ability to differentiate between faking and honest individuals is an encouragement for inclusion of this test into the clinical practice. [ABSTRACT FROM AUTHOR]

Abstract

The 1993 Ground-based Cloud Experiment on Great Dun Fell used a wide range of measurements of trace gases, aerosol particles and cloud droplets at five sites to study their sources and sinks especially those in cloud. These measurements have been interpreted using a variety of models. The conclusions add to our knowledge of air pollution, acidification of the atmosphere and the ground, eutrophication and climate change. The experiment is designed to use the hill cap cloud as a flow-through reactor, and was conducted in varying levels of pollution typical of much of the rural temperate continental norhtern hemisphere in spring-time.
