Aim: To determine the incidence, percentage, possibilities of examination and treatment of peripheral nerve injuries in supracondylar humerus fractures in children. Methods: A retrospective study of patients with peripheral nerve injury in supracondylar fractures treated at the Department of Paediatric Surgery and Traumatology, University Hospital Hradec Králové in 2008-2015. Results: During this period, among the total of 786 patients with supracondylar humerus fractures requiring surgical treatment, concurrent peripheral nerve injury was identifi ed in 59 patients, most frequently n. medianus (57.6%) and n. ulnaris (23.7%). The overall incidence of the isolated n. ulnaris injury (4.6%) corresponded with the published data. Acute treatment of peripheral nerve injury was mostly conservative, more than 37% of nerve lesions recovered within 1 month from the accident, after 6 months the percentage of recovered cases was almost 80%. In two cases (3.4 %), it was necessary to perform microsurgical deliberation of the fi xed nerve from the fracture callus 4 months after the injury. Three patients had minor permanent paresthesias. Conclusion: Peripheral nerve injuries are a common complication of supracondylar humerus fractures in children. The majority of injuries were neuropraxia or axonotmesis receding after adequate conservative treatment. No immediate operative revision or suture was performed in any patient. Surgical deliberation of the peripheral nerve was performed in two cases 4 months after the primary injury. We did not notice a higher incidence of the ulnar nerve injury in the cross-pinning type of osteosynthesis compared to commonly published data on other methods of fi xation. Článek hodnotí migrační trendy ve venkovských oblastech v České republice po roce 1990. K hodnocení byla použita databáze přistěhovalých a vystěhovalých Českého statistického úřadu za období 1990-2010. Zatímco v minulosti převažovala migrace z venkovských oblastí do měst, po roce 1990 se směr migrace změnil: počet obyvatel ve venkovských oblastech s dobrým přírodním a příznivým socio-kulturním prostředím roste díky migraci. Malé obce mají kladné migrační saldo. Lze předpokládat, že tyto trendy by mohly být ovlivněny především sociálními a ekologickými problémy ve městech, zvýšením motorizace a rozšířením komunikačních technologií, stěhováním důchodců do objektů druhého bydlení a změnami preferencí bydlení.

The purpose of this pilot study was to examine the effects of the undergraduate course, Cultural Diversity in Communication, on attitudes and behaviors of undergraduate students in communication sciences and disorders. Pre/post survey data were collected during spring semesters 2005/2006, from 24 white participants (23 female, 1 male) through the Adapted Spiritual Involvement Scale. The surveys assessed attitudes and behaviors related to cultural issues including discrimination, white privilege, and interaction with and knowledge about differing cultures. By the end of the course, it was found that student attitudes improved significantly across 8 of the 14 areas assessed, with the most notable gains found for the recognition of white privilege. The use of polymeric monoliths as stationary phases for liquid chromatography has been limited, despite their ability to enhance the convection flow of the mobile phase with respect to particulate-based columns. This is due to a poor balance between the volume of flow through pores and the number of active sites within polymeric monoliths. In this paper, we present the obtainment of poly(glycidyl methacrylate- co -ethylene glycol dimethacrylate) (P(GMA- co -EDMA)) monoliths with dual pore size distributions (with pore sizes of 60 and 550 nm). Hierarchical pore size distributions were achieved by performing the monolith synthesis by reversible addition-fragmentation chain transfer (RAFT) polymerization as well as using ternary porogen mixtures (containing PEG, dodecanol, and dioxane). While the controlled polymerization mechanism promoted mesopores in the monolith, ternary porogen mixtures allowed the formation of macropores. The monoliths obtained were used as stationary phases for size exclusion chromatography (SEC) for the separation of poly(methyl methacrylate) standards with molar masses between 2.50 × 10 3 and 3.06 × 10 6 g/mol, allowing selectivities that were comparable with commercially available SEC columns packed with porous particles. We believe the approach presented in this work could be the first step toward the obtainment of stationary phases for SEC with enhanced accessibility of exclusion pores. Monolithic columns with accessible porous structures can be beneficial for size-based separations of ultrahigh molar mass analytes with low diffusion coefficients.

Introduction: Midshaft clavicle fractures are common in childhood and their conservative treatment predominates. The indication for surgical treatment of adolescents with significantly displaced fractures remains controversial. The aim of this study was to compare the results of conservative or surgical treatment (intramedullary nailing) of these fractures., Methods: Retrospective clinical study of patients aged 1117 years with significantly displaced clavicle fractures treated at the Department of Pediatric Surgery and Traumatology, University Hospital Hradec Králové in 20112020., Results: In this period, 696 patients with a clavicle fracture (excluding neonatal fractures) were treated. Of these, only 90 (13%) adolescent patients had significantly displaced diaphyseal fractures. Conservative approach was adopted in 52 (58%) of them; the remaining patients underwent a surgical procedure, while an intramedullary inserted and fixed titanium nail (TEN) was used in 20 (22%) cases. Supportive fixation was necessary in 4 of the operated patients. The duration of supportive fixation (basic therapeutic procedure for conservative treatment) was significantly shorter in operated patients (18 vs. 29 days, p=0.0001). No statistically significant difference was found in terms of the time interval of complete healing and return to full activity (74 vs. 85 days, p=0.051). There was one complication in the healing of the surgical wound; all patients healed without sequelae., Conclusion: According to the study, intramedullary osteosynthesis using a fixed titanium nail reduced the need for supportive fixation and allowed to shorten its time. Overall cosmetic and functional results were comparable to the conservative procedure. Therefore, this method can be recommended, especially for active adolescents.

Purpose: The appropriate treatment of pulseless pink supracondylar humerus fractures (SCHF) remains controversial. In this study, the outcomes of two treatment approaches (with and without vascular surgery) were compared., Material and Methods: This was a retrospective multicenter study of patients with pulseless pink SCHFs treated in ten pediatric surgery, trauma, or orthopedics departments in the Czech and Slovak Republic between 2014 and 2018., Results: Of the total 3608 cases of displaced SCHF, 125 had the pulseless pink SCHF. Of those, 91% (114/125) did not undergo vascular surgery and 9% (11/125) underwent vascular surgery. The patients who did undergo vascular surgery had radial artery pulsation restored more frequently in the operating room (73% vs. 36%; p = 0.02), within 6 h (91% vs. 45%; p = 0.004), and within 24 h of surgery (91% vs. 57%; p = 0.05). However, 72 h after surgery, there was no significant difference in palpable radial artery pulsation between the vascular surgery and the non-vascular surgery groups (91% vs. 74%; p = 0.24). Additionally, no significant differences in long-term neurological (9% vs. 22%; p = 0.46) or circulatory (9% vs. 7%; p = 0.57) deficits were found between the two groups., Conclusion: While vascular surgery in patients with pulseless pink SCHFs is associated with a more prompt restoration of radial artery pulsation, no statistical significant differences in terms of the restoration of neurological deficits or the risks of long-term neurological or circulatory deficits were found between patients with and without vascular surgery., (© 2021. Melanomas frequently metastasize to the brain. Despite recent progress in the treatment of melanoma brain metastasis, therapy resistance and relapse of disease remain unsolved challenges. CCT196969 is a SRC family kinase (SFK) and Raf proto-oncogene, serine/threonine kinase (RAF) inhibitor with documented effects in primary melanoma cell lines in vitro and in vivo. Using in vitro cell line assays, we studied the effects of CCT196969 in multiple melanoma brain metastasis cell lines. The drug effectively inhibited proliferation, migration, and survival in all examined cell lines, with viability IC50 doses in the range of 0.18-2.6 μM. Western blot analysis showed decreased expression of p-ERK, p-MEK, p-STAT3 and STAT3 upon CCT196969 treatment. Furthermore, CCT196969 inhibited viability in two B-Raf Proto-Oncogene (BRAF) inhibitor resistant metastatic melanoma cell lines. Further in vivo studies should be performed to determine the treatment potential of CCT196969 in patients with treatment-naïve and resistant melanoma brain metastasis., Competing Interests: The authors declare that no competing interests exist.

Synthetic and natural macromolecules are commonly used in a variety of fields such as plastics, nanomedicine, biotherapeutics, drug delivery and tissue engineering. Characterising macromolecules in terms of their structural parameters (size, molar mass and distribution, architecture) is key to have a better understanding of their structure-property relationships. Size exclusion chromatography (SEC) is a commonly used technique for polymer characterization since it offers access to the determination of the size of a macromolecule, its molar mass and the molar mass distribution. Moreover, detectors that allow the determination of true molar masses, macromolecule's architecture and the composition of copolymers can be coupled to the chromatographic system. Like other chromatographic techniques, the stationary phase is of paramount importance for efficient SEC separations. This review presents the basic principles for the design of stationary phases for SEC as well as synthetic methods currently used in the field. Current status of fully-porous polymeric stationary phases used in SEC is reviewed and their advantages and limitations are also discussed. Finally, the potential of polymer monoliths in SEC is also covered, highlighting the limitations this column technology could address. Despite health literacy being an important assumption for maintaining a healthy population, there are not many studies focusing on health literacy concerning HIV infection among adolescents in the Czech Republic and among Czech authors. In contrast, there are plenty of scientific papers and researches focused on awareness and susceptibility of adolescents in most affected regions of the world, Sub-Saharan Africa and India particularly. Adolescents belong to a high-risk group of the population. Education is one of the most important factors that influence the level of risk of HIV infection. However, increasing health literacy is only implemented in some types of secondary schools in the Czech Republic. The goal of this study is to verify health literacy concerning HIV infection among adolescents in two types of Czech secondary schools (grammar school and vocational school) in two Czech regional cities (Plzeň and České Budějovice). To determine the level of health literacy of HIV infection, a questionnaire (13 open and 9 closed questions) was compiled. A descriptive cross-sectional study was implemented among students (N = 1 942) of 16 chosen schools in two cities of the Czech Republic in 2015. The questionnaire was pre-tested and it took about 15 minutes to complete. The association was tested by a chi-square test, the averages were then compared with an independent t-test, the p-value (0,05) was used in all tests. Most of the students (67,3%) had good health literacy about HIV/AIDS, statistically significant differences were found between students of grammar schools and vocational schools (p = 0,012). Differences were also found between men and women (p = 0,037). The respondents showed the best knowledge about transmission (94,3%), the poorest about epidemiological questions (18,3%). Gender and the type of school had a statistically significant influence on the level of knowledge (gender x2; p = 0,042, types of a school x2; p = 0,031). In this cross-sectional study, women studying at grammar schools had the best knowledge of HIV/AIDS. In contrast, the lowest level of knowledge was found in men studying in vocational schools. The school types and gender had a statistically significant influence on the level of knowledge. However - knowledge is not enough. To reduce infection risk potential will mean to help adolescents to transform knowledge into diminishing of their risky behaviour. The implementation of prevention in all types of schools is crucial for the reduction of exposure from HIV infection. We consider educating adolescents about sexually transmitted diseases important. Despite that our research showed differences among schools and gender. Those differences can make a particular group be more at risk of being infected. Based on the findings, it would be suitable to unify and improve the approach to the education of this topic in all types of schools and to target preventive and educational techniques to boys more, since men are the biggest group of the HIV positive people in the Czech Republic.

Introduction: Evaluation of results of the group of paediatric patients treated for swallowed foreign bodies in the digestive tract., Methods: Retrospective analysis of patients treated for foreign bodies in the digestive tract, managed at the University Hospital of Hradec Králové between 2011-2018., Results: 365 patients (182 boys, 183 girls) with a foreign body in the digestive tract have been evaluated. The mean age was 2.4 years. Most of the children were treated as outpatients (280 children, 76.7%) and only 85 children (23.3%) were hospitalized. The most commonly swallowed frequent bodies included coins (26%) and batteries (14.5%); sharp objects occurred in 5.2%. Unusual foreign bodies were noticed, too. We can mention for example a pen or a plant nutrition product. In 302 cases (82.7%) the foreign bodies passed easily per vias naturales. Endoscopy was necessary in 63 cases (17.2%) - in 51 children the object had to be removed from the oesophagus and in 12 children the object had to be removed from the stomach. None of the swallowed bodies caused complications such as digestive tract perforation or bowel obstruction. No patient in the group was indicated for laparotomy to remove the foreign body., Conclusion: Foreign bodies entrapped in the oesophagus have to be removed endoscopically. In most cases, foreign bodies in the stomach require only observation in the outpatient setting because they leave the stomach easily, without complications, per vias naturales. If a battery remains at the same place of the digestive tract for more than 24 hours, endoscopic removal is indicated at our department. In addition, endoscopy is done in cases of swallowed multiple magnets or bodies that are too large and remain in stomach for several weeks or months. Surgical removal of foreign bodies should be reserved only for complicated cases.

Sleep is ancient and fulfills higher brain functions as well as basic vital processes. Little is known about how sleep emerged in evolution and what essential functions it was selected for. Here, we investigated sleep in Caenorhabditis elegans across developmental stages and physiological conditions to find out when and how sleep in a simple animal becomes essential for survival. We found that sleep in worms occurs during most stages and physiological conditions and is typically induced by the sleep-active RIS neuron. Food quality and availability determine sleep amount. Extended starvation, which induces developmental arrest in larvae, presents a major sleep trigger. Conserved nutrient-sensing regulators of longevity and developmental arrest, AMP-activated kinase and FoxO, act in parallel to induce sleep during extended food deprivation. These metabolic factors can act in multiple tissues to signal starvation to RIS. Although sleep does not appear to be essential for a normal adult lifespan, it is crucial for survival of starvation-induced developmental arrest in larvae. Rather than merely saving energy for later use, sleep counteracts the progression of aging phenotypes, perhaps by allocating resources. Thus, sleep presents a protective anti-aging program that is induced by nutrient-sensing longevity pathways to survive starvation-induced developmental arrest. All organisms are threatened with the possibility of experienced famine in their life, which suggests that the molecular coupling of starvation, development, aging, and sleep was selected for early in the evolution of nervous systems and may be conserved in other species, including humans., (Copyright © 2018 The Authors. An antrolith is a calcified mass found in the nasal cavity or sinus, usually maxillary, described in literature as a rare phenomenon. Its presenting symptoms are variable and include symptoms associated with chronic sinusitis. We describe a 66 year old man with chronic sinusitis who presented with facial pain and epistaxis and upon further evaluation was found to have an antrolith of the left maxillary sinus.

PURPOSE OF THE STUDY Olecranon fractures in skeletally immature patients are rather rare and represent up to 7% of elbow skeletal injuries. Although the majority of olecranon fractures is constituted by undisplaced fractures treated conservatively with good outcomes, a few of them require surgery. The aim of the study was to compare two different approaches of surgical treatment - the open reduction with tension band wiring - cerclage (ORCe) and the closed reduction and percutaneous pinning (CRPP). MATERIAL AND METHODS 37 patients (28 boys, 9 girls) were included in the retrospective multicentric study. The patients were treated at two different institutions (the Clinic of Paediatric Surgery, Orthopaedics and Traumatology, the University Hospital Brno and the Department of Paediatric Surgery and Traumatology, the University Hospital Hradec Králové, Czech Republic). 17 patients underwent the ORCe procedure, while 20 patients were treated using the CRPP method. Different parameters were statistically compared in the groups (demographic data, data concerning the course of the therapy, outcome of the therapy in terms of movements' restrictions and complications such as osteosynthesis failure and infection). RESULTS Both the groups were comparable in terms of demographic data because no statistically significant difference was observed in terms of the age (p = 0.082), the affected site (p = 1.000) and the gender (p = 0.462). Statistically significant difference between these two groups was found in the interval between the implementation and the removal of the osteosynthetic material (p < 0.001) and in the length of cast immobilisation (p = 0.047). The number of patients with movement restriction up to 10° was statistically significantly higher in patients who underwent the CRPP procedure (p = 0.040), but no statistically significant difference was seen between these two groups in terms of movement restriction more than 10° (p = 0.609). One revision surgery was performed in the CRPP group, however with no statistical significance (p = 0.350). DISCUSSION The multicentric study included 37 children, who underwent surgical treatment of a displaced olecranon fracture. Compared to the studies dealing with this topic the number of patients included in this study is relatively high. It advocates the possibility of using the CRPP method as a good alternative to the gold-standard ORCe technique because no difference in terms of the number of revision surgeries and the clinically important movement restriction of more than 10° were seen. It also brings along advantages such as a simple surgical technique, good functional and cosmetic effects, reducing the risk of ischemic insult of growth plate and the possibility of osteosynthetic material removal at an outpatient department with no need for general anaesthesia. The risk of a higher radiation exposure of both the patient and the surgical team should be considered as a disadvantage of the closed method. CONCLUSIONS The mini-invasive CRPP appears to be a good alternative option to the ORCe method for the treatment of isolated olecranon fractures in children offering the advantages such as avoiding extensive open procedure and simple implants removal. A new approach for the calibration of SEC-light scattering (SEC-LS) setups is proposed, which requires solely the molar mass of a reference polymer. Neither the specific refractive index increment of the calibrant nor of the analyte is required. Comparison of the molar masses derived in different solvents for a large number of chemically different polymers shows that the new approach yields the same molar masses as if molar masses were derived using dn/dc to calibrate the light scattering setup. Despite major advances on miRNA profiling and target predictions, functional readouts for endogenous miRNAs are limited and frequently lead to contradicting conclusions. Numerous approaches including functional high-throughput and miRISC complex evaluations suggest that the functional miRNAome differs from the predictions based on quantitative sRNA profiling. To resolve the apparent contradiction of expression versus function, we generated and applied a fluorescence reporter gene assay enabling single cell analysis. This approach integrates and adapts a mathematical model for miRNA-driven gene repression. This model predicts three distinct miRNA-groups with unique repression activities (low, mid and high) governed not just by expression levels but also by miRNA/target-binding capability. Here, we demonstrate the feasibility of the system by applying controlled concentrations of synthetic siRNAs and in parallel, altering target-binding capability on corresponding reporter-constructs. Furthermore, we compared miRNA-profiles with the modeled predictions of 29 individual candidates. We demonstrate that expression levels only partially reflect the miRNA function, fitting to the model-projected groups of different activities. Furthermore, we demonstrate that subcellular localization of miRNAs impacts functionality. Our results imply that miRNA profiling alone cannot define their repression activity. The gene regulatory function is a dynamic and complex process beyond a minimalistic conception of "highly expressed equals high repression".

Two graft copolymer samples of identical average composition were synthesized by grafting living polystyrene anions onto a broadly distributed PMMA backbone. Size exclusion chromatography (SEC) with only RI-detection, SEC with viscometry and light scattering detection, SEC with UV and RI dual detection, gradient chromatography and 2-dimensional chromatography were applied to compare the information that can be obtained by the different techniques. While only limited information was retrieved by conventional SEC or SEC with molar mass sensitive detection, SEC with UV and RI revealed different chemical heterogeneity of the samples. Using gradient chromatography and 2-dimensional chromatography it was possible to identify non-grafted side chains and unreacted parent PMMA besides the actual graft copolymer molecules. While in one sample a heavily grafted product was formed besides non-grafted PMMA, the second sample did not contain ungrafted PMMA but a graft product of lower grafting density. Of all HIV-positive people worldwide, more than two thirds live in Sub-Saharan Africa. Using updated data from UNAIDS, this article analyses whether there is still a signi-ficant increase in HIV in this region or whether national and international programmes and campaigns have successfully contributed to slowing down the epidemic or even reversing its trend. The historical context of the global pandemic of HIV/AIDS is discussed and regional specificities of the HIV spread in the UNAIDS African macro-regions are outlined. The issue of the key factors in the battle against HIV across the Sub-Saharan countries in the second decade of the 21st century is also considered.Key words: HIV/AIDS - Sub-Saharan Africa - regions - ART - history of the spread of HIV/AIDS - pandemics.

Introduction: Herpes virus family's association with visceral lesions is well established. Herpes simplex virus presentations vary based on immune status. Intractable hiccups due to herpes simplex esophagitis, to the best of our knowledge have been described twice in the literature. We present a 68 year-old immunocompromised male with intractable hiccups for 10 months., Case: 68 year-old male with end-stage renal disease and multiple myeloma presented with coffee ground emesis and hiccups of ten months duration. A year earlier, he received cycles of bortezomib and dexamethasone, remaining on lenalidomide. During chemotherapy, he developed pneumococcal meningitis and subsequently intractable hiccups. Preceding admission, endoscopy showed duodenitis and esophagitis. Proton-pump inhibitor therapy was initiated; however, biopsy was not performed. During admission, hiccups often occurred every few seconds while off anti-emetics, persisting despite therapy. Exam showed cachexia/temporal wasting, aphthous stomatitis and abdominal tenderness. MRI of brain/spine, CT of neck, chest, abdomen and neurological evaluation were unremarkable. Endoscopy revealed gastritis and esophagitis with mucosal tears. Biopsy revealed intra-nuclear inclusions with multi-nucleated cells, consistent with herpes virus, later confirmed as herpes simplex by immunostaining. Hiccups and emesis resolved after of 2 days of intravenous acyclovir. 21 days of treatment were completed with oral valacyclovir. He remained free of hiccups during the remaining hospital stay and follow up. This case illustrates an exceptionally rare presentation of herpetic esophagitis in an immunocompromised host. As novel immunotherapeutic/suppressive agents continue to emerge, the evolving role of herpes virus prophylaxis and diagnosis of atypical presentations in new host populations is a topic of growing importance.

The title compound, C25H24N4O, was prepared from 2-chloro-phenanthroline and hexyl-oxyphenyl-tetra-zole. The main difference between the two independent mol-ecules (A and B) in the asymmetric unit is the orientation of the all-anti-configured hex-yloxy chain: in A the C-O-C-C torsion angle is 175.9 (2)° whereas it is -88.3 (3)° in B. The benzene substitution in the bay of the triazolophenanthroline results in a helical distorsion of the heterocyclic core, the dihedral angles between the mean planes formed by quinoline and benzotriazole ring systems are 13.73 (9) for mol-ecule A and 14.87 (8)° for B. The dihedral angles between the triazole ring and the attached benzene ring are 45.87 (15) in A and 53.93 (14)° in B. The angular annulation of four rings and the benzene substituent results in a helical distortion of the aromatic framework. The crystal is formed from layers composed of centrosymmetric pairs of A 2, B 2 mol-ecules with inter-digitating alkyl chains.
